Abstract:
One of the key features of an electronic record is its authenticity. Ensuring the authenticity of managerial electronic records at all stages of its life cycle from the moment of its creation to long-term storage allows us to remove obstacles in the development of electronic records management. The authenticity of electronic records has been the subject of research within the frameworks of an international project InterPARES. A record is considered authentic if its author, place, and time of creation are definitely established, as well as its provenance from the said author. To ensure this property of an electronic record, not only authentication and electronic signature at the moment of the record creation are required, but also creation of the so-called “trusted environment” designed to ensure preservation of records metadata by means of controlled managerial procedures. The focus should be on the information system as a whole. This understanding of electronic record authenticity has been entrenched in international standards on information and documentation. In Russian practice, the concept of “authenticity” is used very rarely; when applied to management electronic records, it is used mainly in the sense of identicalness to some source record. Such understanding involves procedures for comparison with reference record, which should remain unchanged. At the same time, within electronic environment in which the functioning of electronic records takes place, it is practically impossible to ensure that file(s) remain unchanged. Given the record’s social nature, we must understand that immutability of an electronic record is a relative concept that concerns preservation of its social function. That is why we must allow only a relative recognition of the identity of electronic records, allowing for possibility of some changes within which the electronic record retains its social functions. The difference between the copies of electronic records within this difference allows us to talk about authentic records.

Abstract:
Taking as its starting point the discussion of “what is meant by the word Recorde” in a 1581 work by William Lambard, this article considers the development of conceptual ideas about records from the Middle Ages to the early years of the 20th century. After examining the medieval understanding of record as oral testimony of legal judgments, it discusses how concepts of record in England gradually expanded to embrace written texts, and it traces the shift from an exclusive association with courts of law to a perception that records could be made, kept, and used across a much wider range of contexts. The article also examines the changing terminological relationships between records and archives and the dissemination of English understandings of records to other Anglophone countries. It concludes with a brief exploration of points of contact between older debates about the scope of the term record and those of present- day professional discourse.

The records of children and young people growing up in care have multiple purposes and audiences. Cathy Humphreys and Margaret Kertesz discuss the ways in which the characteristics of the documentation determine the record's usefulness to care leavers as a resource for identity at some point in later life. The Who Am I? action research project, based in Victoria, Australia, explores the extent to which records and current record-keeping practices facilitate this. Two approaches were found to be especially useful: the Knowledge Diamond framework, which harnesses the different knowledge brought by diverse groups to the task of developing principles for record-keeping; and the records continuum (constructing, storing and accessing the record), which provided a concept through which to understand the significance of the archive as a dimension of good practice. It was found that workers' accountability to the children now and in the future risks being overshadowed by the requirements of other stakeholders unless the principles that underpin effective record-keeping are articulated and implemented.

The core components of an electronic health record include administrative functions, computerized physician order entry, lab systems, radiology systems, pharmacy systems, and clinical documentation. HL7 is the standard communication protocol technology that an electronic health record utilizes. Implementation of software, hardware, and IT networks are important for a successful electronic health record project. The benefits of an electronic health record include a gain in healthcare efficiencies, large gains in quality and safety, and lower healthcare costs for consumers. Electronic health record challenges include costly software packages, system security, patient confidentiality, and unknown future government regulations. Future technologies for electronic health records include bar coding, radio-frequency identification, and speech recognition.

It is conventional wisdom that record keeping falls apart in disasters. Yet that is not documented in the research record. Drabek's (1986) exhaustive review of empirical studies has no reference to records. Quarantelli's (1983) work focused on a narrow subset of records: medical records in mass casualty situations. There are, in fact, problems with records and disasters. Some records wrich would be useful have never been made. Others are lost or damaged, inadequate or inaccurate. Even backups may not be available. There is also a problem with toxicity. The increasing links between humans and toxic events raise issues about the safety of records after an incident. This paper explores the types of records that would be useful to have about disasters and looks at how they should, or could, be created. Attention is also given to the question of records research strategies, as a means for—among other things—tapping the knowledge of disaster research pioneers before their information is lost. Knowledge will grow scientifically only after the most faithful examination of many catastrophes. Record linkage deals with finding records that identify the same real world entity, such as an individual or a business, from a given file or set of files. Record linkage problem is also referred to as the entity resolution or record recognition problem. To locate those records identifying the same real world entity, in principle, pairwise record analyses have to be performed among all records. Analytical operations between two records vary from comparing corresponding fields to enhancing records through large knowledge bases and querying large databases. Hence, these operations are complex and take time. To reduce the number of pairwise record comparisons, blocking techniques are introduced to partition the records into blocks. After that records in each block are analyzed against one and another. One of the effective blocking methods is the closure approach, where a “related” equivalence relation is used to partition the records into equivalence classes. This paper introduces the closure problem and describes the design and implementation of a parallel and distributed closure prototype system running in an enterprise grid.

Purpose The purpose of this paper is to investigate the recognition of record management profession in the Namibian Public Service. The objectives of the study are to determine appreciation of the importance of records record management; establish policies that support records management; find out the promotion of the records management function in the public service; establish training and job opportunities available for the record management professionals; and come up with recommendations on how the recognition of the record management profession could be enhanced in the public service of Namibia. Design/methodology/approach This case study used qualitative data collection methods, namely, focus group discussions and interviews. The two ministries and records keeping staff who took part in the focus group discussions were conveniently selected, whereas the two government offices where one key informant each was interviewed were purposively selected. Findings This study revealed that the records management profession was not well recognised in the public service of Namibia. The hiring of staff with low educational qualifications and the hiring of records keeping staff with some Diploma qualifications into the same positions as those without qualifications were all evidence of this. In addition to these was the failure to re-grade the records keeping staff through the establishment of a records management cadre. Practical implications The study recommends the creation of record management units in all offices, ministries and agencies headed by qualified record managers supported by qualified records management staff, finalisation and implementation of the records management policy, as well as the creation of a records management cadre. Originality/value The findings of the study on which this paper is based could inform policy for decision makers, and for the records management keeping staff, a strategy for advocating for recognition of their profession was planned.

Purpose This paper examines existing research on the topic of personal health records (PHRs). Areas covered include PHR/patient portal, recordkeeping, preservation planning, access and provider needs for future reuse of health information. Patient and physician PHR use and functionality, as well as adoption facilitators and barriers, are also reviewed. Design/methodology/approach The paper engages in a review of relevant literature from a variety of subject domains, including personal information management, medical informatics, medical literature and archives and records management literature. Findings The review finds that PHRs are extensions of electronic records. In addition, it finds a lack of literature within archives and records management that may lead to a less preservation-centric examination of the new PHR technologies that are desirable for controlling the lifecycle of these important new records-type. Originality/value Although the issues presented by PHRs are issues that can best be solved with the use of techniques from records management, there is no current literature related to PHRs in the records management literature, and that offered in the medical informatics literature treats the stewardship aspects of PHRs as insurmountable. This paper offers an introduction to the aspects of PHRs that could fruitfully be examined in archives and records management.

Despite the importance of financial information on access to credit, growth and sustainability, Micro and Small Businesses (MSBs) in Tanzania hardly keep records. This descriptive study investigated financial record keeping practices in MSBs in Iringa and Kinondoni Municipalities employing a sample of 245 owner managers. Data were collected using a questionnaire. Findings indicated that financial record keeping in MSBs remains a big challenge. Some MSBs occasionally kept records while others did not keep records at all. The most kept records were inventories and purchases rather than sales. Reasons for not keeping records included limited knowledge on the value of record keeping, limited skills, shortage of time, shortage of funds to hire skilled personnel and shortage of facilities such as tables/desks and cabinets to store records. The study established a significant association between record keeping and the performance of MSBs. Yet there were mixed perceptions of MSB owner managers regarding the value of record keeping. The study recommends policy action to enforce financial record keeping and strengthen Business Development Services (BDS) provision. Further studies may consider an experimental design with MSBs in the treatment arm supported to keep financial records, then measure impact over a certain period.

In returning the outpatient medical record file to thesection of the Medical Record Unit, Assembling there was a delay. This is because the return of medical record files has not been carried out according to Standard Operating Procedures, as a result, causing delays in the reporting system. The method used is qualitative using a descriptive approach. Data collection techniques by observation, interviews and literature study. This study aims to determine the return of former medical records of outpatients at Dayeuhkolot Health Center. The results of the study prove that the delay in returning outpatient medical record files at the Dayeuhkolot Health Center with presentations during the 1 week study amounted to 63 or 22% of 285 medical record files. Efforts have been made to disseminate information to all officers related to the efforts made by the person in charge of COVID-19 patients in returning medical record files on time.

OBJECTIVE: To determine the completeness of prescription records, and the extent to which they agreed with medical record drug entries for antihypertensive medications. SETTING: Three clinics affiliated with two staff model health maintenance organizations (HMOs). PARTICIPANTS: Randomly selected HMO enrollees (n=982) with diagnosed hypertension. METHODS: Computer-based prescription records for antihypertensive medications were reviewed at each location using an algorithm to convert the directions-for-use codes into an amount to be consumed per day (prescribed daily dosage). The medical record was analyzed similarly for the presence of drug notations and directions for use. RESULTS: There was a high level of agreement between the medical record and prescription file with respect to identifying the drug prescribed by drug name. Between 5 and 14 percent of medical record drug entries did not have corresponding prescription records, probably reflecting patient decisions not to have prescriptions filled at HMO-affiliated pharmacies or at all. Further, 5–8 percent of dispensed prescription records did not have corresponding medical record drug entry notations, probably reflecting incomplete recording of drug information on the medical record. The percentage of agreement of medical records on dosage ranged from 68 to 70 percent across two sites. Approximately 14 percent of drug records at one location and 21 percent of records at the other had nonmatching dosage information, probably reflecting dosage changes noted on the medical record but not reflected on pharmacy records. CONCLUSIONS: In the sites studied, dispensed prescription records reasonably reflect chart drug entries for drug name, but not necessarily dosage.

We define a simple collection of operations for creating and manipulating record structures, where records are intended as finite associations of values to labels. A second-order type system over these operations supports both subtyping and polymorphism. We provide typechecking algorithms and limited semantic models.Our approach unifies and extends previous notions of records, bounded quantification, record extension, and parametrization by row-variables. The general aim is to provide foundations for concepts found in object-oriented languages, within a framework based on typed lambda-calculus.

Electronic Medical Record (RME) is a computerized health information system that contains social data and patient medical data, and can be equipped with a decision support system. Electronic Medical Records can help better management of patient health services (Andriani et al., 2017). Santa Elisabeth Hospital Medan in inputting its Electronic Medical Record data already uses a web-based application. Where this web-based application is called Sphaira Mobile Electronic Medical Record. This application is still newly implemented and used in the electronic medical record system at the Santa Elisabeth Hospital Medan. With the transfer from manual medical records to electronic medical records, this has resulted in several problems in filling out patient electronic medical records. The purpose of this study was to determine the cause of the incomplete diagnosis data in electronic medical records related to the reporting of the top 10 inpatients (Rl5.3) at the Santa Elisabeth Hospital, Medan. The type of research used is descriptive and qualitative with observation and interview data collection techniques. Based on the results of interviews with the head of the medical record room at Santa Elisabeth Hospital Medan, he stated that the transition from manual medical records to electronic medical records made hospital employees, especially officers in the medical record room, have to adapt, recognize and learn how to operate the Sphaira Mobile Electronic Medical Record application. AbstractSeismic hazard and risk analyses are increasingly tapping into the previously underused resource of local weak-motion records. This is facilitating the development of local- or even application-specific models for the characterisation of earthquake ground motion. In turn, this offers the opportunity to derive non- or partially non-ergodic models and significantly reduce bias and uncertainty. However, weak-motion data, while carrying important information about local earthquake source, path and site effects, are susceptible to noise. We show that high-frequency noise has a record-, or region-specific, impact on pseudo-spectral acceleration (PSA). This impact depends on the shape of the records’ Fourier amplitude spectrum (FAS): PSA from moderately to highly damped ‘soil’ records (e.g. Groningen, the Netherlands) is much less susceptible to high-frequency noise than PSA from weakly damped ‘rock’ records (e.g. Eastern North America). We make use of simulated ground motion records to develop a parametric model for the lower usable period of PSA (Tmin). The model accounts for the impact of high-frequency noise on PSA, conditional on easily measured parameters characterising the shape of a record’s FAS. We then present a workflow, describing processing undertaken for records of induced seismicity from the Groningen gas field. The workflow includes the definition of maximum and minimum usable frequencies and periods of FAS and PSA, respectively. As part of the workflow, we present an approach that considers multiple estimates of Tmin. These include the parametric model and, additionally, record-specific hybrid simulations that artificially extend or modify time series’ FAS beyond the noise floor to assess subsequent impacts on PSA.

We consider the maximum likelihood and Bayesian estimation of parameters and prediction of future records of the Weibull distribution from δ -record data, which consists of records and near-records. We discuss existence, consistency and numerical computation of estimators and predictors. The performance of the proposed methodology is assessed by Montecarlo simulations and the analysis of monthly rainfall series. Our conclusion is that inferences for the Weibull model, based on δ -record data, clearly improve inferences based solely on records. This methodology can be recommended, more so as near-records can be collected along with records, keeping essentially the same experimental design.

About four million photographic records of Brazilian birds are deposited in the WikiAves database. The objective of this study was to examine the spatiotemporal distribution of photographic records available in this database. Searches were performed in the database in 2022. Record numbers produced by citizens were obtained by selecting states and municipalities in different periods. The annual record production in Brazil has increased substantially since 2009, reaching about 400,000 records per year in 2020–2021. Most records were obtained in the Sudeste and Sul geopolitical regions. Seasonal variations in record production in the Cerrado and the Atlantic Forest are comparable, with more records obtained in July–October. In Amazonia, a more substantial decrease in record production occurred between November and April. The monthly record production was more uniform in the Caatinga. When municipalities were compared, similar seasonal variation in record production was observed for four ecosystems (Amazonia, Atlantic Forest, Cerrado and Pampa). However, substantial differences were observed for municipalities in the Pantanal and the Caatinga. The results indicate that high human density, high individual income and the breeding season of birds are potential factors leading to high record production. On the other hand, heavy rainfall, flooding, remoteness and environmental harshness are pointed out as factors potentially leading to relatively fewer records. Further, this article discusses the implications of record availability for ornithological studies that use photographic records deposited in the WikiAves database.

Purpose The purpose of this paper is to examine the significance of company record for journalist when investigating projects relating to conflict of interest that occurs in Hong Kong and in and connected with China. Design/methodology/approach The paper describes what company records are available, and how they are accessible for the public in Hong Kong. It then compares with the company record accessibility in China. The paper uses investigative projects done by the author and other journalists to illustrate how the records are significant. Some of the investigative projects that are related to China are used to illuminate the importance for the company record in Hong Kong for investigating issues in China. Findings Hong Kong maintains an efficient access to the company record that benefits the journalists for probing into the issues of conflict of interest. This efficient system has faced threats when the government proposed to withdraw some of the important records from the general public access. Originality/value This paper will be of interest to journalists and journalism students and scholars who are interested to know the practical uses of company records. Policymakers will also learn from this paper that a restriction in the public access to the company record will make a huge impact to the justice-seeking journalist work.
