A well-designed implementation of medication concepts, records, and lists in an electronic medical record (EMR) system allows it to successfully perform many functions vital for the provision of quality health care. A controlled medication terminology provides the foundation for decision support services, such as duplication checking, allergy checking, and drug-drug interaction alerts. Clever modeling of medication records makes it easy to provide a history of any medication the patient is on and to generate the patient's medication list for any arbitrary point in time. Medication lists that distinguish between description and prescription and that are exportable in a standard format can play an essential role in medication reconciliation and contribute to the reduction of medication errors. At present, there is no general agreement on how to best implement medication concepts, records, and lists. The underlying implementation in an EMR often reflects the needs, culture, and history of both the developers and the local users. survey of a sample of medication terminologies (COSTAR Directory, the MDD, NDDF Plus, and RxNorm) and EMR implementations of medication records (OnCall, LMR, and the Benedum EMR) reveals the advantages and disadvantages of each. There is no medication system that would fit perfectly in every single context, but some features should strongly be considered in the development of any new system.

This thesis explores how doctors and nurses use documents to share their knowledge within and across healthcare settings. In addressing this question I draw on a 15-month, multi-sited ethnographic study in several pediatric health care settings, following patients from primary care clinics, to emergency rooms, and in-patient units. The analysis focuses on the practices that go into documenting patients' histories and care, which include recordings on various on-line systems, preprinted forms, and whiteboards. By combining the previously distinct lenses of 1) knowing in practice, 2) time-space analysis of social interaction, and 3) communicative genre and genre systems, I suggest that doctors and nurses employ various types of document genres to manage, not only their distributed knowing about patients' care, but also their own movements across time-space. I outline a perspective on documents and knowing which attempts to highlight the role of human practice in how people use documents to coordinate their activities, share their capabilities, and get things done in complex distributed organizational work. The data suggest that doctors and nurses use medical documents as maps and itineraries to organize their distributed work practices. Doctors and nurses record patients' histories many times in different documents, with each document serving as a map and itinerary for a different constituency of people. Each of these documents is rarely used in isolation from other documents. Doctors and nurses constantly recombine the documents they use, which allows them to both appropriate documents from other settings into their local organization of work and build unique local combinations of documents.
(cont.) I introduce the concept of "re-localizing" to describe how doctors and nurses use documents to share their knowing within and across healthcare settings. Re-localization involves many healthcare professionals' parallel rewriting of a patient's history based on a recombination of each other's maps and itineraries and the patient's own accounts. By integrating the concrete case and the maps and itineraries based on those cases the notion of relocalization overcomes the dichotomy between the abstract and the situated, the local and global. Documents are not seen as mere vessels for abstract representations, but integral parts of distributed knowing within and across settings.

Detta dokument är en teknisk rapport för ett projekt vars syfte är att utveckla en Lambda-funktion som letar efter oanvänd Domain Name System (DNS)-information i AWS’ Route 53. Denna information har potential att orsaka säkerhetsbrister genom att missleda användare till en destination annan än den som de förväntar sig nå. Någon med skadliga avsikter skulle därför kunna använda sig av denna brist för att rikta data till sig själv som denne inte egentligen bör ha tillgång till. Problemet förvärras hos molntjänster som erbjuder uthyrning av IP-adresser då dessa ofta kommer från en adressrymd som tjänsten underhåller. När en användare sedan slutar hyra adressen hamnar den återigen i adressrymden. Om det råkar finnas DNS-information kopplad till den adressen så kommer de nu orsaka den tidigare nämnda säkerhetsbristen då de hänvisar till en IP-adress som vem som helst kan ha hyrt. För att åtgärda detta skrevs en funktion i Python 3.9.4 som sedan integreras i AWS Lambda. Funktionen körs från ett baskonto i AWS och hämtar data både från detta konto och andra konton genom att anta en specifik IAM-roll inom dem. Där hämtar den både kontots DNS-information samt IP-adresser som den sedan jämför för att se om de stämmer överens eller ej. Om de gör det så är informationen inte oanvänd och därför ointressant för funktionen. All DNS-information som inte har en matchande IP-adress läggs då till i en lista som sedan skickas ut till behöriga genom SNS i AWS.
This document is a technical report on the development of a Lambda function, the purpose of which is to look for orphaned (dangling) Domain Name System (DNS) records in AWS’ Route 53. These records can open up serious vulnerabilities as they have the potential to misdirect clients to end-points other than the one they think they are accessing; something which can be the cause of malicious intent.  This is particularly an issue in cloud platforms that offer the ability to lease IPs from their own pool of addresses, as said addresses go back into the pool once they are released by the lessee. These addresses are then able to be leased by anyone with access to AWS and could, as such, lease addresses until they happen upon one that has a corresponding orphaned record somewhere and redirect sensitive data to themselves. The resulting function is written in Python 3.9.4 and integrated into AWS via Lambda. It functions by assuming roles in specified accounts in order to fetch DNS records and Elastic IPs from these accounts, which it then tries to match in order to determine whether or not they are orphaned. Any orphaned records found are then compiled into a list containing the account ID and URL of each record which is then sent out to the appropriate parties through AWS SNS.

Administrative programs that govern the day-to-day-management of agencies are founded in law and the federal records management program is no exception. This dissertation explores whether the existing legal framework accommodates the changes occurring in the federal records management program as a result of electronic records and whether the solutions proposed by the University of British Columbia and University of Pittsburgh are viable for the federal records officer. These two universities pioneered the research being done on electronic recordkeeping. The record requirement established under the Federal Records Act is very difficult for agencies to maintain when it comes to electronic records. At the same time, agencies are being pressured to do more work electronically. Most federal records officers are not equipped to meet the dual challenge of maintaining accessibility while also ensuring the records remain authentic. The two approaches for dealing with electronic records, UBC's Diplomatics as implemented with the Department of Defense standard and Pittsburgh's literary warrants, are too complex and difficult. Even if the records managers implemented either of these approaches, they require too much effort on behalf of the record creator for the implementations to be successful. The National Archives can provide advice and guidance to agencies on electronic recordkeeping requirements. However, the success of the records management program depends on the federal agencies because it is a decentralized administrative process. The ability of federal agencies to cope with electronic records is problematic. Therefore, the viability of the federal records management program is problematic.

Dans un premier temps, nous avons etudie le probleme des records provenant d'observations de base se produisant aux instants d'un processus de renouvellement. Uplift rates associated with 12 episodic tremor and slip events on the Cascadia Subduction Zone occurring between 1997 and 2010 have been determined from hourly water level records from 4 NOAA tide gauges (Neah Bay, Port Angeles, Port Townsend, and Seattle). Displacements inferred from water levels generally agree with displacements inferred from modeling GPS data. Examination of uplift between events shows an inter-event deformation rate approximately equal in magnitude, with ETS events, on average, releasing strain accumulated between events, suggesting that ETS is consistent with the elastic rebound theory. Additionally, while the GPS record only extends to the late 1990s and the tremor record includes only recent decades for Cascadia, tidal records in the Pacific Northwest and around the world span many decades. Thus, by showing that ETS can be resolved in tidal records we open up the possibility that tidal records could be used to study ETS where other tools are not available. This thesis contains unpublished coauthored material.

The study sought to investigate the management of medical records for healthcare service at the Victoria Public Hospital in the Eastern Cape Province. The objectives of the study were to describe the present records management practices in Victoria Hospital; find out the existing infrastructure for the management of patient medical records at the Victoria Hospital; determine the compliance of patient medical records management in Victoria Hospital with relevant national legislative and regulatory framework; find out the security of patient medical records at the Victoria Hospital. Quantitative and qualitative approaches were employed. The sample was drawn from the service providers and from the healthcare service users. Questionnaires, interviews and observation were used to collect data. The findings showed that Victoria Hospital uses manual records management system in the creation, maintenance and usage of records. In the findings, there were challenges related to misfiling and missing patient folders which sometimes lead to the creation of new patient folders. Also, the study discovered that the time spent in the retrieval of patient folders could negatively affect the timely delivery of healthcare services. The study recommended the adoption of electronic records management system as most public healthcare institutions in the country are rapidly shifting to electronic records management system. The use of electronic records management system is believed to be efficiently and effectively promoting easy accessibility, retrieval of patient medical records and allows easy communication amongst the healthcare service institutions and healthcare practitioners.

One main requirement for this issue is to correct the stage-discharge relations which are sometimes not available. This paper reconsiders the use of historical hydrologic data in urban areas which have fundamentally changed even concerning the river beds, cross sections and floodplain areas. Hurricane Katrina showed that the current methods for handling medical records are minimally resilient to large scale disasters. This research presents a preliminary model for measuring the resilience of medical records systems against public policy goals and uses the model to illuminate the current state of medical record resilience. From this analysis, three recommendations for how to make medical records more resilient are presented. The recommendations are: 1) Federal and state governments should use the preliminary resilience model introduced here as the basis for compliance requirements for electronic medical record technical architectures. 2) Regional Health Information Organizations (RHIOs) should consider offering services in disaster management to healthcare organizations. This will help RHIOs create sustainable business models. 3) Storage companies should consider developing distributed storage solutions based on Distributed Hash Table (DHT) technology for medical record storage. Distributed storage would alleviate public concerns over privacy with centralized storage of medical records. Empirical evidence is presented demonstrating the performance of DHT technology using a prototype medical record system.

The purpose of this thesis is to verify the SAP RM configuration's possibility to fulfill the requirements of MoReq2 standards. The theoretical part revealed the characteristics of Records Management, MoReq2 standard and SAP Records Management software. Records Management is introduced in the connection with Enterprise Content Management and Document Management. There are other standards mentioned, but Moreq2 due to its importance for the practical part of the thesis is being described closely. SAP Records Management component is explored from its conceptual and technological aspects. The practical part is composed of analysis of MoReq2 requirements in SAP RM environment and of the description of the system configuration, which enables MoReq2 requirements' fulfillment. The configuration is described in details only if SAP RM fulfills MoReq2 requirements.

The research focuses on the factors of perceived efficiency and on the costs that exist in a company. More specifically, this Master thesis is a qualitative exploratory case study which's aim is to examine and present the experiences and the attitudes of 4 individuals who are working in companies that possess an EDRMS. In order to acquire this deeper understanding, the data collection methods that were used were the in person semi structured interviews and the observation. During the research was examined how the perceived efficiency and the costs in a company are affected by a proper RM/ERM program. The analysis of the collected data shown that the specific individuals in the specific firms are benefited by the EDRMS and that their work is improved.

Records management and archival theory recommends that records classification, as a means to identify and organize the records made or received in the course of business, should be based on an analysis of the records creators’ functions and activities and reflect them. However, the purpose of classification, the meaning of the term function, and the methodology for conducting a business analysis are not clearly explained in the relevant literature. Additionally, no studies of actual applications of the functional approach to records classification in real organizational settings exist. This dissertation addresses the question of how the concept of function and the functional approach to records classification are understood by those who are responsible for the development and implementation of records classification systems as well as by the users of such systems. In order to contribute insights that can enrich the theory and methodology of records classification, an empirical, interpretivist research design, based on an initial survey of potential study subjects and a multiple-case study research, was conducted in four selected central banks in Europe and North America. One of the selection criteria was that the organizational cultures of the case study sites had to be as heterogeneous as possible. Findings showed that the meanings of function, functional approach, and even classification are subject to various interpretations, that classification developers find functional methodologies confusing, and that users do not usually appreciate the outcomes of their efforts. Furthermore, because the approach to classification was not always consistent with the nature of the records, some of the classification systems examined did not adequately serve either records management or business-related purposes. The research also provided an explanation of the relationship between organizational culture and the understanding of both records management and business processes.

This study examined the cultural political economic significance of the physical "brick and mortar" independent record store in the digital era. The research was built upon two critical frameworks -- the political economy of communication and critical cultural studies. The political economy of communication situated the independent record store within the dominant music industry, and was concerned with the corporate structuring and standardization of music culture. The study analyzed the extreme market concentration involving the "big four" major record labels (in terms of vertical and horizontal integration, diversification and product synergy), their interconnectedness to the major corporate music retailers, and the implications for the manufacturing of popular musical artists. The independent record store (to varying degrees) counters these tendencies by offering greater cultural diversity in terms of "independent," local, used and obscure music. However, the independent record store is influenced by the policies and practices of the major label system, distribution channels, big box chain retailers, and on-line commerce. The study examined the dominant industry policies and practices, including buying (centralized versus localized), music as a loss leader, co-op advertising, retail "payola," retail censorship, and the overall range of musical diversity, vis-à-vis the practices and semi-autonomous nature of the independent record store. This project was equally invested in the cultural aspects of the independent record store in terms of communities and scenes. Various subcultures gather and connect with each other at independent record stores, where anti-corporate and counter-hegemonic narratives circulate, and individuals learn about the depths of musical history and culture. This project explored the physical atmosphere and vernacular culture associated with the stores, as well as the cultural significance of vinyl record collecting. However, these cultural attributes are framed in terms of the harvesting of commodification, where the perception of "independence" is rendered problematic in terms of the economic realities associated with the logic of capital. The primary ethnographic field sites for this study included the House of Records in Eugene, Oregon; Music Millennium in Portland, Oregon; and Amoeba Music in San Francisco, California. Numerous specialty independent record stores within these geographic areas were also included in the study.

The management of records is an essential part of any organization. Today this implies the management of mostly digital records in electronic record management systems. There are many standards for record management. Three influential standards was compared and used to evaluate such an electronic record management system in this thesis. The purposes of the thesis was to find out how the core features support and interfere with each other, and which changes could be done to a system evaluated according to the standards, as well as what the major challenges were when using document standards to evaluate an electronic record management system. The standards were MoReq2010, ISO 15489 and DoD 5012.02. To conform to the standards, some changes could be done to the studied system. For instance, changing document keys and to include more metadata about disposal of records. The conclusions were that standards are a good complementary source when developing an existing record management system, even though their size and complexity level are issues to deal with.

World records in athletics capture the attention of both the general public and the academic community. Athletics records provide a wealth of quantifiable data, which represent the pinnacle of human ability at the time of the record. This research discusses modelling the progression of athletics world records using a compound distribution model which is found in actuarial statistics. Compound distributions allow for the simultaneous modelling of the frequency of record breaking and of the amount by which the record is improved at each record-breaking performance. A number of assumptions accompany the use of the compound distribution model. This research evaluates the athletics data for adherence to these assumptions, before carrying out distribution-fitting and goodnessof-fit testing. Forecasts of future performance are obtained by means of a simulation method. The forecasts obtained from the compound model are compared to forecasts obtained from non-linear models which have been proposed in literature. This research focuses on short-term forecasts, however, a modification to the compound model which allows for longer-term forecasts is also illustrated.

The importance of the hospital in Canadian society is undisputed. Despite this, the hospital has traditionally received relatively little attention from the archival community. It is only now becoming apparent to both hospital administrators and archivists that this situation must change if the valuable records contained in hospitals are to be preserved. This thesis examines the archival preservation of hospital records, concluding that their retention is of great benefit to those operating the institution as well as the rest of society and that this preservation can be effectively carried out if certain basic considerations are borne in mind. Before archival operations are established in a hospital, it is necessary to investigate various aspects of the institution and its record keeping practices. It is only when the nature and uses of these records are understood that effective programmes can be implemented. A hospital archives will not achieve its full potential unless it is established on a sound foundation with adequate policy and resources. Similarly, the records contained therein must be properly appraised to ensure all the valuable material is retained and protected from improper access. Through an investigation of these and related issues, the viability of hospital archives will be confirmed.

Health information technology (HIT) aims to improve healthcare services by means of technological tools. Patient centered technologies such as personal health records are relatively new HIT tools that enable individuals to get involved in their health management activities. These tools enable the transformation of health consumer behavior from one of passive health information consumers to that of active managers of their health information. This new role is more interactive and engaged, and with such tools, patients can better navigate their lives, and exercise more control over their treatments, hence potentially also leading to improvement in the quality of health services. Despite the benefits of using personal health record systems for health consumers, the adoption rate of these systems remains low. Many free and paid services have not received the uptake that had been anticipated when these services were first introduced. This study investigates some factors that affect the adoption of these systems, and may shed light on some potential reasons for low adoption rates. In developing the theoretical model of this study, social cognitive theory (SCT) and technology acceptance model (TAM) were utilized. The theoretical model was validated through a quantitative survey-based methodology, and the results were derived using structural equation modeling techniques. The key findings of this study highlight the role of individual and environmental factors as determinants of end-user behavior in the adoption of personal health records. The results show that in addition to perceptions of usefulness and ease of use, factors such as social norms and technology awareness are also significantly associated with various factors that directly and indirectly affect intention to use PHRs Based on the results obtained in this study, recommendations are offered for technology providers, and possible directions are proposed for academic researchers.

The eastern Mediterranean area is a region of high archaeological importance, it is also a region where climate has been a force interacting with humans in shaping the landscape and vegetation history. Variations in pollen content and composition in various climate archives (e.g. lake sediments and peat sections) are widely used to reconstruct vegetation changes and human impact in the Quaternary environments. Pollen sampling has been conducted throughout the Peloponnese peninsula but there is a lack of regional synthesis of these locally based studies. The aims of the thesis are partly to show how pollen data may be used in a regional analysis on Late Pleistocene and Holocene vegetation changes, partly to assemble all published pollen data from Peloponnese peninsula in a database. The question formulations are; i) how may a database with pollen dataserve as a basis for interpretations of regional vegetation changes on Peloponnese?, ii) what are the possibilities of using classification of pollen and distinguish between driving factors behind the historic vegetation changes? The constructed database facilitates further research regarding pollen records at Peloponnese. Pollen recordsmay show important patterns in landscape changes during Late Pleistocene and Holocene but using pollen records at a regional scale need comparisons between coring sites which may be troublesome due to different approaches, different species investigated and varied calculation of pollen sum. In order to distinguish between driving forces and actors affecting the vegetation, pollen data may be used both in detail but also in using groups and classifications of the pollen included.

Esta pesquisa partiu das leituras da obra de Elisa Lispector e da seguinte indagação do pesquisador: por que Elisa, publicada, considerada pela crítica e premiada na segunda metade do século XX no Brasil, caiu no esquecimento? A proposta é resgatar os motivos de interesse do trabalho da escritora. Dentre tais motivos, destacam-se -- em construção autônoma, mas inter-relacionada -- a busca da memória de uma família judaica expatriada, que fugiu da Ucrânia para o Brasil, e o questionamento constante da identidade e das relações interpessoais no contexto da dimensão temporal. As personagens, quer evidenciem ou não traços (auto)biográficos, delineiam-se, ao longo dos contos e romances, sempre empenhadas na construção de sua subjetividade. A tarefa projetada desenvolveu-se com o apoio das entrevistas feitas pelo autor da dissertação e de sua coleção de críticas referentes aos livros de Elisa, desenhos e pinturas do seu acervo, bem como fotos da família Lispector. O enfoque da situação da escritora e de sua obra teve, como principal referência teórica, os princípios da crítica biográfica, orientando não apenas a leitura de registros da vida de Elisa em contraponto à análise de amostras escolhidas de seus escritos, como também a inserção das experiências do pesquisador para a construção da figura da escritora. O objetivo maior é evidenciar a importância do resgate da obra de Elisa Lispector para a literatura brasileira.
This research started from readings of Elisa Lispector and the question: why has her work, published, well regarded by critics and awarded prizes in the second half of the twentieth century in Brazil, fallen into neglect? The idea is to highlight the points of interest in Lispector s work. Foremost among these are the following (each a good reason in its own right, but all interconnected): the memories of an expatriate Jewish family who fled Ukraine for Brazil, and the constant questioning of identity and interpersonal relations over time. The characters—whether or not they contain (auto) biographical features—in her short stories and novels are always involved with the construction of their own subjectivity. This study relied on the interviews made by the author and on his collection of criticism related to Elisa Lispector s books, drawings and paintings belonging to her, as well as photographs of the Lispector family. The basic theoretical underpinnings of this study of a writer s life and work are the principles of biographical criticism. They provide guidelines not only for the reading of the records of a writer s life in the light of analyses of samples chosen from her writings, but also for an understanding of the researcher s own experiences in his construction of the figure of Elisa Lispector. The major goal here is to underscore the importance of rediscovering her work for Brazilian literature.

This thesis investigates requirements engineering methods based on process modelling for Electronic Healthcare Record (EHR) systems. The relation between software requirements and user workflows is essential in healthcare settings: EHRs are expected to improve clinical and administrative workflows. In turn, the new workflows are expected to satisfy a number of business goals. If a new software system does not support the desired clinical workflows or patient journeys, then its value and benefits are often disputed by stakeholders. Our hypothesis is that requirements engineering methods based on process models will contribute to the overall success of EHR projects in the industry. By success, we mean software systems that are in use and meet the business benefits expected of them. The experiments presented in this thesis are aimed to develop and evaluate a method that allows business analysts to make use of process models during requirements engineering for EHRs. The goal of the method is to ensure the software specification is aligned to and supports the user workflows. Each of the four experiments addresses a specific research objective, and thus the findings from each experiment constitute the basis for one of our four contributions to science. / Experiment 1: Relating Goal Oriented Requirements Engineering and Process Modelling: This experiment investigates the design of a common framework for describing process models and software requirements. It relates the KAOS framework for goal oriented requirements engineering and the Business Process Modelling Notation (BPMN). Our goal is to facilitate requirements elicitation. Specifically, business analysts using our framework should be able to reason about the alignment of the software specification to the business processes, and identify specific changes that improve this alignment (either changes in the design of the system, or changes in the business processes). This first experiment was conducted as part of the WellbeingUCL project, supported by Boots. / Experiment 2: Inferring Goal Models from Process Models: The second experiment investigates a method for business analysts to derive software requirements from process models. The purpose for defining such a method is to provide sufficient guidance to business analysts, during requirements elicitation. Our aim is to help business analysts elicit meaningful goal models and shape the design of the system-to-be, in light of these goals. A number of heuristics to facilitate requirements elicitation are proposed and evaluated, considering the trade-offs between a fully automated and a human driven process. / Experiment 3: Electronic Healthcare Record for Bupa: The third experiment evaluates the requirements engineering method during an EHR implementation for a chronic condition management service delivered by Bupa nurses in South West England. Action research is used to assess the impact and fit of the requirements elicitation process, in relation to the current work practices of business analysts in the industry. The extended KAOS framework and goal inference heuristics have been used to inform the final software specification, guide the workflow redesign and clarify the business benefits. From a project management perspective, this experiment evaluates how the KAOS method aligns with the Agile and Lean methodologies used in Bupa. The project has delivered an EHR system actively used to support the care of 2,600 patients. / Experiment 4: Personal Health Record for Nuffield Health: The fourth experiment evaluates the extended KAOS framework when developing a new digital customer proposition with an underlying EHR system. It investigates how consumer journeys can be modelled as KAOS process models. Of specific interest is the ability of the framework to clarify the responsibility assignments among the different agents (i.e. system components) that need to collaborate to deliver the end to end customer journey. The experiment was run as an action research project, in partnership with Nuffield Health. The results have informed the architecture of an open source personal health record for lifestyle data. / Contributions to science: This thesis advances the field of requirements engineering by introducing and evaluating a requirements elicitation method based on business process models. It also presents new evidence into the use of goal oriented requirements engineering for the design and implementation of EHR systems in the industry. Our four contributions to science directly follow from the results of the four experiments conducted as part of this research. Our first two contributions cover the conceptual framework and our proposed method for requirement elicitation based on process models. Our last two contributions present evidence for the practical use and benefits of our goal oriented requirements engineering method in industry based projects. First, we present an extension of the KAOS requirements engineering framework which includes a business process view with clearly defined syntax and execution semantic. This approach ensures process models and goal models have a shared semantic. A new concept, that of Intentional Fragment, captures the explicit relation between fragments of a process model and a specific goal. We also define additional consistency rules, to clarify how the process view relates with other KAOS models: object, agent and operation model. Secondly, we present a set of goal inference techniques to help analysts build goal models starting from process models. In effect, analysts can start from the artefacts that are most familiar to them (i.e. the workflow models) and gradually derive a goal model for the system-to-be. A set of 12 heuristics have been fully defined and integrated into a semi-structured method for goal elicitation. Our third contribution is an evaluation of how the goal oriented requirements engineering method (incorporating workflow analysis) supports the design and deployment of a EHR system in a clinical setting. The project was representative for the challenges faced by healthcare organisations wishing to deploy EHRs: quality of care standards that impose constraints on process redesign; legacy systems that have shaped the workflow; organisational complexity and competing stakeholder interests. We show that by methodically applying our goal inference techniques we were able to produce a valid goal model starting from models of the nurses workflows. The resulting goal model was used to reason about alternative design options in the system-to-be, and to clarify the benefit case in deploying the EHR system. Fourth, we examine the requirements engineering process for an EHR system meant to support a new customer proposition. This project was representative for the challenges faced in the digital health industry: a target consumer journey driven by user experience research; many different systems required to collaborate; focus on the architectural design of the system. We show that we can apply our goal inference techniques to customer journey maps and produce a meaningful goal model. This has been used to shape the architecture of the EHR system and reason about integration requirements. We also argue that our goal inference techniques complement agile development practices used within the organisation.

Patient medical records are a valuable resource that can be used for many purposes including managing and planning for future health needs as well as clinical research. Health databases such as the clinical practice research datalink (CPRD) and many other similar initiatives can provide researchers with a useful data source on which they can test their medical hypotheses. However, this can only be the case when researchers have a good set of hypotheses to test on the data. Conversely, the data may have other equally important areas that remain unexplored. There is a chance that some important signals in the data could be missed. Therefore, further analysis is required to make such hidden areas become more obvious and attainable for future exploration and investigation. Data mining techniques can be effective tools in discovering patterns and signals in large-scale patient data sets. These techniques have been widely applied to different areas in medical domain. Therefore, analysing patient data using such techniques has the potential to explore the data and to provide a better understanding of the information in patient records. However, the heterogeneity and complexity of medical data can be an obstacle in applying data mining techniques. Much of the potential value of this data therefore goes untapped. This thesis describes a novel methodology that reduces the dimensionality of primary care data, to make it more amenable to visualisation, mining and clustering. The methodology involves employing a combination of ontology-based semantic similarity and principal component analysis (PCA) to map the data into an appropriate and informative low dimensional space. The aim of this thesis is to develop a novel methodology that provides a visualisation of patient records. This visualisation provides a systematic method that allows the formulation of new and testable hypotheses which can be fed to researchers to carry out the subsequent phases of research. In a small-scale study based on Salford Integrated Record (SIR) data, I have demonstrated that this mapping provides informative views of patient phenotypes across a population and allows the construction of clusters of patients sharing common diagnosis and treatments. The next phase of the research was to develop this methodology and explore its application using larger patient cohorts. This data contains more precise relationships between features than small-scale data. It also leads to the understanding of distinct population patterns and extracting common features. For such reasons, I applied the mapping methodology to patient records from the CPRD database. The study data set consisted of anonymised patient records for a population of 2.7 million patients. The work done in this analysis shows that methodology scales as O(n) in ways that did not require large computing resources. The low dimensional visualisation of high dimensional patient data allowed the identification of different subpopulations of patients across the study data set, where each subpopulation consisted of patients sharing similar characteristics such as age, gender and certain types of diseases. A key finding of this research is the wealth of data that can be produced. In the first use case of looking at the stratification of patients with falls, the methodology gave important hypotheses; however, this work has barely scratched the surface of how this mapping could be used. It opens up the possibility of applying a wide range of data mining strategies that have not yet been explored. What the thesis has shown is one strategy that works, but there could be many more. Furthermore, there is no aspect of the implementation of this methodology that restricts it to medical data. The same methodology could equally be applied to the analysis and visualisation of many other sources of data that are described using terms from taxonomies or ontologies.

The creation of systems for assembling and analyzing medical data is currently one of the major factors in advancing the speed of medical research. To ensure patient privacy, legal limitations have been placed on these systems. The Health Insurance Portability and Accountability Act requires that certain types potential identifiers be removed from the data before it can be shared freely. The process of removing the identifiers is called de-identification. The purpose of this project is to create a de-identification filter for the MIMIC database, a system that retrieves and organizes data from the intensive care unit at the Beth Israel Deaconess Medical Center.

In order to automate data extraction from electronic medical documents, it is important to identify the correct context of the extracted information. Context in medical documents is provided by the layout of documents, which are partitioned into sections by virtue of a medical culture instilled through common practice and the training of physicians. Unfortunately, formatting and labeling is inconsistently adhered to in practice and human experts are usually required to identify sections in medical documents. A series of experiments tested the hypothesis that section identification independent of the label on sections could be achieved by using a neural network to elucidate relationships between features of sections (like size, position from start of the document) and the content characteristic of certain sections (subject-specific strings). Results showed that certain sections can be reliably identified using two different methods, and described the costs involved. The stratification of documents by document type (such as History and Physical Examination Documents or Discharge Summaries), patient diagnoses and department influenced the accuracy of identification. Future improvements suggested by the results in order to fully outline the approach were described.

Apresentamos um modelo estocástico para evolução de espécies utilizando processos de Poisson. Eventos de surgimento de novas espécies e eventos de extinção são dados por dois processos de Poisson independentes. A cada evento de surgimento, uma nova espécie é adicionada ao sistema e uma aptidão aleatória é associada a ela. À cada evento de extinção, é associado um limiar também aleatório e todas as espécies com aptidão inferior ao limiar são retiradas do sistema. Apresentamos critérios necessários e suficientes para recorrência/transitoriedade da configuração vazia. Mostramos a existência da distribuição limite e apresentamos critérios necessários e suficientes para um número in/finito de espécies em tal distribuição.
We present a stochastic model for species evolution based on Poisson Processes. Birth of new species and extinction events are given by two independent Poisson processes. At each birth, a new species enters the system and is given a random fitness mark. To each extinction event is associated a random threshold mark and all species with fitness lower than the threshold are removed from the system. We present necessary and suficient criteria for the recurrence/transience of the empty configuration. We show the existence of the limit distribution and present necessary and suficient criteria for an in/finite number os species in such distribution.

Abstract:
This thesis examines the situation for archivists within the context of e-government in three Swedish authorities. The discussions that are brought up concern the archivists role, professional development and the perspective of information in the context of e-government. Since the purpose with this thesis was mainly concerning the situation for the archivists, the method chosen was a qualitative research method, and it was used in combination with interviews with the archivists at the chosen authorities. The results show that the archivists within the examined authorities are seen as experts on information management and legislation surrounding it. Problems connected to e-government are that they face prejudice about their capability concerning new technology and digital information. This problem has been handled differently within the authorities, change of official titles has been one tactic. Their tasks and professional development have been slightly changed as they are working in a higher capacity ahead of the information being created. The change that can be seen in the context of e-government is that archivists are being more visible within their work places, which can be correlated to the e-government that encourage team work and collaborations to create an efficient and accessible authority. What this means for the archivists is that they can use these collaborations to further their own projects and tasks within the organisation in a better way. Contrary to the belief that archivists can only manage information on paper the participants in this research prove they have adopted well to the way of e-government partly through the Record Continuum Model. They are aware about the different perspectives necessary for handling the new ways of structuring and seeing information, which became clear with the development and structure of e-archives within two of the authorities and the preparation for being connected for the third. Finally, this thesis shows that archivists are beginning to be able to compete with other departments and professions on the same terms and that they are starting to find their own place within the area of IT and information management. This is a two years master's thesis in archival science.

Abstract:
Patients typically enter the healthcare systems at the primary care level from where they are further referred to specialists or hospitals as necessary. In the private healthcare system, primary care is provided by a general practitioner (GP). A GP will refer a patient to a specialist for treatment when necessary, while the GP remains the main healthcare provider. The provision of care is, thus, fragmented which results in continuity of care becoming a challenge. Furthermore, the majority of healthcare providers continue to use paper-based systems to capture and store patient medical data. However, capturing and storing patient medical data via electronic methods, such as Electronic Medical Records (EMRs), has been found to improve continuity of care. Despite this benefit, research reveals that smaller practices are slow to adopt electronic methods of record keeping. Hence this explorative research attempts to identify factors that affect the lack of adoption and meaningful use of EMRs in general practices. Four general practices are surveyed through patient and staff questionnaires, as well as GP interviews. Socio-Technical Systems (STS) theory is used as a theoretical lens to formulate the resulting factors. The findings of the research indicate specific factors that relate to either the social, environmental or technical sub-systems of the socio-technical system, or an overlap between these sub-systems. It is significant to note that within these sub-systems, the social sub-system plays a key role. This is due to various reasons revealed by this research. Furthermore, multiple perceptions emerged from the GP and patient participants during the analysis of the findings. These perceptions may have an influence on the adoption and potential meaningful use of an EMR in a general practice. Additionally, the socio-technical factors identified from this research highlight the challenges related to encouraging the adoption and meaningful use of EMRs. These challenges are introduced by the complexities represented by these factors. Nevertheless, addressing the factors will contribute towards improving the rate of adoption and meaningful use of EMRs in small practices.
