Full textThe United States (U.S.) is currently facing a shortage of primary care physicians, an issue particularly salient in Arizona. The purpose of this project is to investigate predictors of students entering primary care specialties that may be apparent from their American Medical College Application Service (AMCAS) applications, in order to best serve the needs of the physician workforce. We hypothesized that factors such as female gender, older age at application (“non‐traditional” students), and being raised in a rural/underserved community background may be predictors of students who choose primary care fields. AMCAS applications are completed by aspiring medical students and contain demographic information including gender, age, race, languages spoken, and family/community characteristics. Data provided also include academic factors such as college major, grade point average (GPA), and Medical College Admissions Test (MCAT) scores. Other subjective data reported by applicants include descriptions of extracurricular activities and a personal statement. The National Resident Matching Program (NRMP) is the system used by graduating students during the last semester of medical school to match students with their choice of specialty and the residency program wherein they will spend an additional three years, minimum, in training. Based on the Association of American Medical Colleges (AAMC) designations, we are considering primary care to be Family Medicine, Pediatrics, Internal Medicine, and Medicine‐Pediatrics. We examined data from AMCAS applications of all 149 students who graduated from the University of Arizona College of Medicine‐Phoenix between 2011‐2014, and compared to their NRMP match outcomes. Comparisons were made between non‐primary care versus primary care‐overall, as well as Family Medicine alone versus all other matches given the increasing rate of specialization within Internal Medicine and Pediatrics. Multiple logistic regression revealed two predictors of primary care career choice compared to non‐primary care: having more siblings (P=.003) and non‐physician parents (P=.017). Specific to Family Medicine, several predictors were identified compared to the non‐Family Medicine cohort: a slightly greater percentage of earned community college credits (P=.03), lower MCAT physical science (P=.009), higher MCAT verbal scores (P=.02), and lower paternal education (P=.003). Our analyses suggest having a greater number of siblings and non‐physician parents may predict primary care career choice. Specific to Family Medicine, academic factors including community college enrollment and MCAT scores may be of predictive value. Though the exact implications behind these findings are unclear, it is important to continually examine such data as medical schools can shape admissions selection criteria targeted at increasing the number of graduates seeking careers in primary care.

Full textThe work in this thesis deals with metamaterials, its components and applications. A historical overview about these materials, features and researches in the domain are presented. Chiral media, binaisotropic materials and photonic crystals are also studied in order to visualize physics behind metamaterials.Electromagnetic properties in complex media are widely investigated. Starting from Maxwell’s equations, bi-anisotropic materials and their effect are deeply analyzed whereas two types of Split Ring Resonator (SRR) are treated to determine constitutive parameters of Left Handed Materials (LHM). The metamaterials are also studied as photonic crystals since the effective medium approach is not applicable when the dimensions of the inclusions tend to the operating wavelength. Moreover, SRRs and Multi SRRs are synthesized analytically and electromagnetically in order to extract their resonant frequencies. Consequently, these components could be introduced in any design; Artificial Magnetic Conductor (AMC) surface is an efficient case. Thus, a low profile antenna positioned over such surface is examined as an application of metamaterials. The results of directivity, gain and reflection coefficient are of great importance and affirm the employing of metamaterials in such applications. A dual band PCB antenna, as another application of metamaterials, is designed and simulated. The SRR element studied in the previous chapters is used as the trap which inserted in the arm of the antenna in order to create another resonance, and consequently another band is created

The prevalence of asthma increases recently. The more develops, the more prevails. Most of the injuries for the asthma are irreversible. Now, the common therapy on asthma is bronchodilator agents. In the indicator of asthma, the acid mammalian chitinase (AMCase) was identified in patient. The syndrome can be improved when the AMCase blocked. The inhibitors of AMCase are chemicals or small interfering RNA (siRNA). SiRNA is a short nucleic acid fragment to interference or silence target mRNA by pairing to form double-stranded RNA molecules. The double-stranded RNA molecule recruits enzyme or protein complex with ribozyme activity to cleave the target mRNA. Then, more short nucleic acid fragments are produced and the target mRNA is degraded more efficiently. Finally, the proteins translated from the target mRNA are ceased. The physical function of the proteins from target mRNA is lost. However, the real sequence of target mRNA processed by the siRNA is still unclear. The expression of AMCase is inhibited by siRNA 1336 has been reported. The cells treated with siRNA 1336 were collected. After RNA extraction, the technique of rapid amplified RNA end (RARE) was applied. The truncation site on 3’ end of AMcase mRNA is identified at 1,365.

Several recent studies showed that the acidic mammalian chitinase ( mAMCase ) were highly expressed in the lung epithelial cells of asthmatic mice. Also, the expression of hAMCase in the lungs of asthma patients was significantly higher as compared with that of healthy individuals, indicating a role of chitinase in asthma. The current study therefore aimed to identify and construct the promoters which drive mAMCase and hAMCase expression. Then the reporter system using such promoter candidates to express luciferase was utilized for the study of the allergic mediators on mAMCase and hAMCase expression, leading to the understanding of mAMCase and/or hAMCase contributing to asthma. First, the specific mouse mAMCase promoter and four candidates of hAMCase promoter, namely hAMCase-H, SH, SSH and SB, were cloned according to the results of bioinformatic analysis. The reporter vectors were constructed containing the above promoter sequences and encoding luciferase. The mAMcase and/or hAMCase expression in the cells were therefore evaluated by measuring the luciferase activity in the cells, which were transfected with the reporter vector. A negative control, 2H, was also cloned. Results showed that the significant higher level of luciferase activity in the mouse lung epithelium MLE-12 cells receiving mAMCase reporter. In the human lung cells A549 being transfected with hAMCase reporters, it showed that SB initiated the highest level of luciferase activity. Moreover, there seemed to be no effects on luciferase signals, while MLE-12 treating with different doses of inflammatory ( IL-13 and TNF-α ) and anti-inflammatory ( TGF-β ) factors and A549 treating with IL-10, IL-13, TNF-α and anti-IL-13. In contrast, the luciferase activity was significantly reduced while MLE-12 treating with anti-inflammatory mediators including anti-IL-13 and IL-10 as well as in A549 treating with TGF-β. In addition, not only chitinase-EIRE but also glucocorticoid receptor appeared to be included in the sequences of mAMCase and hAMCase promoters. The effects of adding Dexamethason and chitin derivatives were also determined. It showed that Dexamethason was only able to enhance the luciferase activity in MLE-12 cells receiving mAMCase promoter, but not in A549 cells containing hAMCase promoter. However, most importantly, the exposure of chitin elicited the luciferase activity in both MLE-12 and A549 cells, indicating the correct candidates of chitinase promoters. Although more studies are required to understand the role of chitinase contributing to the development of asthma, the current study provides a practical tool and basis of utilizing chitinase promoters for further investigation.

Recombinant adeno-associated virus (rAAV) is one of the most promising vectors for human gene therapy. However, the production systems that are currently available have a limited capacity and cannot provide sufficient quantities of rAAV for preclinical or clinical trials. Many novel methods for improving rAAV production have been developed, but few researchers have focused on the culture process. In this study, we use a fed-batch culture system to enhance rAAV yield in the baculovirus/insect cell system. When the insect cells were co-infected with MOI=5 of Bac-GFP at a ratio of 1:9:9 (Bac-GFP: Bac-Rep: Bac-VP), the fed-batch culture achieved optimal rAAV yields. In batch culture, the optimal cell density for producing rAAV was found to be 1´106 cells/ ml, and the highest rAAV yield (1.22´108 IVP/ ml, 122 IVP/ cell) occurred at day 5 post-infection. In the fed-batch culture, and the highest rAAV yield was 2.40´108 IVP/ ml (240 IVP/ cell). The batch and fed-batch cultures have very similar costs; however, the rAAV yield was 2.6-fold higher in the fed-batch culture system compared with the batch culture system. Therefore, this method offers an economical, rapid procedure for rAAV production. Moreover, in this study, rAAV was utilized to deliver RNAi for the development of targeting therapy against allergic asthma. Growing evidence suggests that an increased expression of acidic mammalian chitinase (AMCase) may play a role in the pathogenesis of asthma. In the present study, we sought to develop a RNA-interference approach to suppress allergic asthma in mice through silencing of AMCase expression. Mice sensitized with ovalbumin (OVA) were given intratracheal administration of a recombinant adeno-associated virus expressing short hairpin RNA (rAAV-shRNA) against AMCase. In OVA-sensitized mice, the development of allergic symptoms was significantly associated with elevated AMCase expression. After administration of rAAV-shRNA, there was a significant reduction of AMCase expression in the lung and in BALF cells of sensitized mice. Sensitized mice receiving rAAV-shRNA showed a significant improvement in allergic symptoms, including airway hyperresponsiveness (AHR), eosinophil infiltration, eotaxin, IL-13 secretion in BALF and serum OVA-specific IgE level. Our data suggest the hyperexpression of AMCase in asthma can be suppressed by rAAV-mediated shRNA. Silencing AMCase expression by shRNA may be a promising therapeutic strategy in asthma.

IEEE 802.15.4 is a suitable standard to be adopted for wireless sensor networks. It defines the communication physical layer (PHY) and media access control layer (MAC) for wireless personal area network (WPAN). Recently, with the multidisciplinary development of Internet of Things (IoT) the WSN becomes more powerful for its functions, e.g. the wireless sensor and actuator network (WSAN). IEEE 802.15.4e standard is designed to support the complicated development of WSN/WSAN. It officially defines multi-channel communication protocols as part of the standard. In the standard, multi-channel communication can be categorized into beacon-enabled as example of Deterministic & Synchronous Multi-Channel Extension (DSME) and non-beacon enabled as example of Asynchronous Multi-Channel Adaption (AMCA). Traditionally it has flexibility on variable unit data length for MAC layer on non-beacon enabled WSN but less performance balanced due to unpredictable waiting time for network access to every node in the network. With the introduction of the AMCA, it can run with different MAC unit data lengths in different channels in parallel so it’s getting more use cases. We propose enhancements based on native AMCA of the IEEE 802.15.4e and build up an energy analysis model for comparing of native AMCA and enhanced AMCA modes. There are main four approaches to our proposition and the algorithms are easy to be implemented on a micro controller system as a practical method for real deployment of WSN. We finally use OPNET network simulator software to form networks with different node sizes under mesh and cluster-tree hybrid topologies as simulation scenarios. The results present our E-AMCA approaches have better performance than N-AMCA especially in the networks with high volume nodes.

Taiwan plays an important role in the whole world on the fan and forced draught blower market. Taiwan must set the standard for its flow of fan to make the dependable basis. AMCA wind-tunnel generally used in such fan function tests of heat dissipation as CPU of IT industry, etc. the correcting of the flow meter usually send to Inspection Bureau of the national standard but there are some disadvantages in high cost. This thesis studies the feasible method to correct instrument and equipment. In the small flow, we use the device with the compressibility factor Theory (PV =ZmRT) to correct the amount of air flow. In order to calculate the total amount of air through wind tunnel, we precisely,we design a tank. After a while, the air passing through wind tunnel is selected into the tank. We observe the difference of height in water level in U monometer and calculate the pressure in the tank; In higher flow, we find other methods ,for example, wind velocity meter. In base of ideal gas equation, we calculate error between the flow of predicting and the flow measured by the wind-tunnel. Finally, we discuss the pressure difference in different point.
