Abstract:

Objective: To synthesize evidence from the recent interventions to improve organ donation rates and attitude towards organ donation. The effective intervention is defined as significant increases in the enrollment rate of organ donation and positive changes of attitude. And the second one is to give recommendations on feasible intervention designs based on the specific situation of Mainland China. Methods: A systematic review was conducted through PubMed, Cochrane Library, Google Scholar and reference lists. The quality of the studies was evaluated by CONSORT guideline. Results: Nine randomized control studies were included in this systematic review, and the quality of all these studies was average. The main targets of the studies were religious beliefs, distrust medical system, lack of knowledge and fear of premature death. Using lay health advisors, implementing lecture, presentation and discussion, and exposure to mass media with donation information frequently were all identified as effective components to change the attitude and improve the enrollment rate. Conclusion: Based on this systematic review, discrepant interventions could change the public attitude towards organ donation and improve the enrollment donation rate in the United States. However, given the cultural differences in China, more research is needed to see whether implementation of these interventions could improve the situation of organ donation in China.

Abstract:

Background. Organ transplantation as one of the most effective treatment options for patients with organ failure is challenged by organ shortage around the world. This systematic review aims to summary relevant factors influencing people’s intention and behavior for organ donation. Method. Three databases, namely PUB med, Medline and China knowledge resources integrated database, were applied for literature searching. Fourteen studies, which meet the inclusion criteria and exclusion criteria, are used in this review. Quality assessment was proceeding upon STROBE checklist. Results. Seven factors influencing individuals’ intentions and behavior for organ donation are summarized, including: demographic factors, cultural factors, attitudes towards death and funeral, family influence, information accessibility for organ donation, distrust medical community and legislation for organ donation. In addition, two more factors, acceptance of brain death and effectiveness in receiving donation request, are associated with family consent to organ donation. Conclusion. This review highlights factors could influence individual’s intention and behavior for organ donation. Potential implications could be implemented base on the effect of these factors. Possible interventions, upon demographic characteristics, dealing with Chinese traditional concept and enhancing knowledge and exposure of organ donation, to Chinese population are discussed. Limitations of this systematic review are also mentioned.

Abstract:

A method for preserving transplant organs for extended periods of time has been developed in the transplant organ preservation cooler. The preservation cooler enhances organ viability by maintaining a temperature controlled organ bath and pumping perfusate through the transplant organ. The emphasis on the transplant organ preservation cooler is to provide a simple and portable system which will be powered by boiled off oxygen from a liquid oxygen source. The design of the preservation cooler pump and temperature control system are presented. Results of tests proving the successful operation of the preservation cooler prototype are also presented.

Abstract:

The developmental potential of mammalian oocytes cryopreserved with procedures similar to those used for embryos has been limited, inasmuch as oocytes differ from embryos in advanced stages of development, both physiologically and morphologically. The objective of this work was to elucidate the precise nature of freeze-thaw damage with the expectation that identification of specific targets will enable devising optimal procedures for cryopreservation of bovine oocytes to prevent specific damage and minimize the loss of developmental capacity.
In the first series of experiments, bovine oocytes were vitrified (V-oocytes) or frozen slowly (S-oocytes) at the germinal vesicle (GV) stage or after maturation in vitro (IVM). Survival was assessed morphologically and also by in vitro fertilization (IVF) and culture (IVC). Morphological integrity and developmental capacity were greater in S-oocytes than in V-oocytes (P $<$ 0.05). Transfer of four embryos (2 morulae and 2 blastocysts) derived from post-IVM S-oocytes into a recipient heifer resulted in the birth of twin calves.
In the second series of experiments, oocytes (GV and IVM) were exposed to a cryoprotectant solution (DAP213: 2M DMSO, 1M acetamide, 3M propanediol) for 1.5 or 5 min and viability assessed by IVM-IVF-IVC. Oocytes were also examined by transmission electron microscopy (TEM) before (control) or after exposure to the cryoprotectant. DAP213 induced profound premature cortical granule (CG) release and vesiculation. These changes were less pronounced in oocytes exposed to DAP213 only after IVM. The results suggest that: (1) the extrusion of CG is one of the important cytological events affected by the treatment of oocytes with DAP213; (2) GV oocytes are more sensitive to the cryoprotectant than IVM oocytes.
In the third series of experiments, GV and IVM oocytes were vitrified with DAP213. On rewarming, DAP213 was removed by a one- or three-step dilution procedure and survival assessed by development after IVM-IVF-IVC. Morphology was assessed by TEM study immediately following DAP213 removal. Both assessments indicated that: (1) IVM oocytes are more tolerant to vitrification than are GV oocytes; (2) the three-step dilution is less damaging than the one-step procedure; (3) changes in the zona pellucida (loss of plasticity) of IVM oocytes following vitrification may result from the premature release of cortical granules.

Abstract:

Stem cell-based therapies are promising in regenerative medicine. However, the success of cell therapy is greatly limited by the low engraftment rate to the target tissues. The present study demonstrated that human mesenchymal stem cells (hMSCs) were subjected to a self selection process via microencapsulation in collagen barrier when they were induced to migrate out from this barrier. While retaining the immuophenotype and self renewal capacity, the selected hMSCs showed a significantly better in vitro migratory response of than those cultured in traditional monolayer. The migratory response could be controlled by varying the fabrication parameters of the collagen barrier, including initial collagen concentration and cells seeding density. Affinity to adhere on endothelial cells layer is another engraftment related property. Significant difference was observed between these selected hMSCs and hMSCs in monolayer culture. In order to investigate the engraftment potential of the selected hMSCs, an animal model was performed. The selected hMSCs were transplanted intravenously into NOD/SCID mice under partial hepatectomy. Presence of human cells in the residual liver was determined by the presence of human HLA-ABC using flow cytometry after 48 hours, 1 week and 1 month. Engraftment of the selected hMSCs was significantly higher than that of monolayer cultured hMSCs in time point of 1 month. It demonstrated that the selected hMSCs favor the engraftment to the injured liver. Further investigation is required to determine the fate of the engrafted hMSCs in order to truly confirm their therapeutic potential. The current work demonstrated that collagen-hMSCs microsphere could act as a barrier to select hMSCs with enhanced in vitro migratory response and in vivo engraftment properties. These findings may contribute towards the development of better stem cell therapies.

Abstract:

Background: Organ transplantation is getting more important and the demand increased exponentially in the past decade. However, due to the shortage of organ supply, some people died while waiting for an organ and the other may rely on all forms of organ support to live. Hong Kong has the lowest organ donation rate among the developed world despite large-scale organ donation enhancement activities implemented for decade. Since consent from family members is mandatory, it is important to understand the reasons relatives given for consenting or refusing to donation. Objectives: (1) To investigate the change of attitudes towards organ donation in Hong Kong during 2004 to 2012. (2) To understand the difficulties of transplant coordinators encountered during counseling Methods: This is a cross-sectional survey looking at the recent change of reasons of the families of the potential donors to make decision about donation. Transplant coordinators of all 7 clusters completed questionnaires after counseling each potential donor referred during the 7 months study period. Demographic data and reasons of refusal or consent of donation were recorded and analyzed and compared with previous results. Difficulties of transplant coordinators encountered during counseling were recorded. Results: Totally 52 questionnaires were completed within 7 months. The conversion rate were similar when compared to that in 2004 (44.3% in 2004 vs 40% in 2012). The proportion of older potential donor in the refusal group is higher than that of consent group (> 60 years old 51.6% vs 38%,). The demographic data was neither statistical significantly different between consent and refusal group nor correlated with refusal of donation. From 2004 to this survey, the most common reason to refusal remained to be the traditional belief of buried intact. Both were 33%. The “expressed wish of the deceased to object donation while alive” increased from 13% in 2004 to 20% in 2012 (p=0.378), and “no consensus from family” dropped from 26% in 2004 to 11% in 2012 (p=0.083). The reason “no expression of any wish” remained static when compared to that in 2004 (15% vs 16%, p=0.982). The reason for consenting to donation changed a lot. Although the most common reason remained the “wish to help others” (94% vs 41%, p<0.0001), the second and third reason changed to “feeling good by being charitable” (21% vs 0%, p=0.0071) and “not to waste useful organs” (18% vs 3%, p=0.0455) while the fourth reason was “respect deceased’s choice”(14% vs 0%, p=0.0278). The reasons of refusal and consenting donation are explained in the context of the decision-making process. The main obstacles the transplant coordinators have encountered were knowledge gap, emotion, time pressure and family or social disharmony. Conclusion: The major reasons for consenting and refusing donation did not only change in position but also change in proportion in last decade. The importance of some reasons increased significantly and strategies to improve conversion rate should be adjusted according to the changing reasons. Education to change the traditional belief, encourage expression of donation wish, create position image of donation and the benefit of transplant are all important.

Abstract:

The aim of this study was to characterise and cryopreserve semen of the indigenous Namaqua Afrikaner breed, and to compare it to that of Dorper and Dohne Merino sheep, whose semen is commercially frozen on a large scale. The study was conducted between January and August 2015. September 2013-born Namaqua Afrikaner (12), Dohne Merino (12) and Dorper (9) rams were used in the study. The rams were kept under kraal conditions with adequate shade, and they received a high protein, high energy diet. Originally it was envisaged to collect semen samples using the artificial vagina (AV) method, which proved to be problematic with the Namaqua Afrikaner rams. Semen samples were subsequently collected twice a week by either AV (Dohne Merino and Dorper) or electro-ejaculation (EE; all three breeds). Macroscopic sperm traits were assessed and sperm concentration determined immediately after collection. Each semen sample was diluted with Triladyl® (1:3) and subsequently frozen in liquid nitrogen vapour in straws. Frozen straws were thawed and evaluated at 7, 30 and 90 days after cryopreservation. A droplet (0.5 ml) from each thawed sample was assessed microscopically for post-thaw motility and percentage live sperm..

O presente trabalho tem como objetivo principal demonstrar a possibilidade do ordenamento jurídico brasileiro abarcar a hipótese de escolha pelo doador do destinatário dos seus órgãos na doação de órgãos post mortem. Inicialmente, busca-se enquadrar o direito ao próprio corpo como direito da personalidade, relativizando o caráter de indisponibilidade desses direitos de forma a reconhecer a incidência da autonomia privada em seu campo. Ainda, será analisado o princípio da autonomia na sua concepção bioética e sua influência na determinação da possibilidade da escolha pelo doador de órgãos post mortem. Outrossim, ficará demonstrado que o sistema atual possui falhas e que a compatibilização do modelo atual com a possibilidade de escolha pelo doador é possível,utilizando-se como parâmetro o tratamento dado à doação em vida no direito pátrio, bem como na legislação estrangeira.

Abstract:

The use of organs from donors after circulatory death (DCD) has been recommended as one strategy to enlarge the donor pool and raise the transplant rate. However, DCD allografts had higher incidence of early post-transplant dysfunction. The general aim of this research project was to develop clinical and experimental strategies to reduce the incidence of early post-transplant dysfunction of kidney and liver allografts from DCD. First the ability of a clinical scoring system based on donor data for identifying DCD kidneys with high-risk of post-transplant dysfunction was evaluated using the Oxford and the UK National DCD kidney transplant cohorts. This works suggest that stratification of DCD kidneys before transplantation might allow early identification of kidneys in which lower graft function and survival could be expected if any additional therapeutic intervention is implemented. Second, as it has been suggested that hypothermic machine perfusion (HMP) may protect DCD kidneys from additional preservation injury and improve their outcome after transplantation, this work explored the benefit of HMP as preservation technique fo DCD kidneys in Oxford and discusses the potential of this technique for reducing the incidence of post-transplant dysfunction in DCD kidneys. The Oxford. Liver Group has provided evidence of the benefit of preservation with normothermic machine perfusion (NMP) on post-transplant function and survival of DCD liver allografts. In this work, the molecular mechanisms associated with this benefit were characterized using micro array technology. This analysis suggests that the beneficial effect ofNMP may be associated with the induction of the ischaemic preconditioning phenomenon and highlights a group of genes with potential for gene therapy. Finally, this works provides the "proof-of-concept" that the use of a non-mammalian viral vector for gene transfer of kidneys and livers during conventional cold preservation is feasible and is not associated with additional tissue injury.
