Abstract:
The importance of biopsychosocial factors in the genesis and maintenance of disease is increasingly being recognized. Most illnesses should be studied from a multifactorial perspective to facilitate understanding and treating them. Many psychopathological processes involve factors such as loneliness, hopelessness, and lack of social cohesion. As early as the nineteenth century, J.M. Charcot defined those illnesses in which no organic lesion was visible as functional disorders. Today, the anthropological view of illness known as the Heidelberg School provides us with a more global and comprehensible assessment of illness. The anthropological approach is complemented by a bioethical one, a bioethics of daily life which, as a practical science, studies and evaluates the living conditions of individuals, seeking practical solutions and contributing its reflections with deliberation and care. In this paper, we aim to highlight the most important factors that have an impact on illness by providing an anthropological view of illness and bringing bioethics closer to everyday life.

Covid 19 infection can affect immune system functions. Although age is seen as a major factor for the development of Herpes Zoster (HZ), immunosuppression or disease-related immune system dysfunction can progress with the reactivation of Varicella Zoster Virus (VZV). A 57-year-old male patient presented with painful lesions on his body after Covid 19 infection and was diagnosed with HZ. The patient was admitted to the algology outpatient clinic due to Postherpetic Neuralgia (PHN) 4 months after the onset of the rash. Our aim is to draw attention to the possibility of HZ and PHN development after Covid 19 infection.

The aim of the study. To investigate the organisational forms of medical care for patients with chronic pain and the methodology of training specialists in algology. Materials and methods. Bibliosemantic, comparative, systematic analysis. Results and discussion. Pain is the main reason why people seek medical care. Pain is not only a medical problem, but also a social and economic one. Chronic pain is a complex biopsychosocial phenomenon with far-reaching consequences not only in terms of clinical care, but also in terms of social and economic impact. This presents a huge challenge for those involved in pain management. Hence the need for a bio-psycho-social model of pain, both through the patient-society and patient-medicine feedback lines, is emerging. Any country loses a lot of material resources due to pain problems. There needs to be a call to action for managers at all levels and policy makers to recognise the importance of chronic pain and invest in comprehensive strategies that include structural change, prevention, education and access to effective treatments. There is a need for government awareness of pain issues, which can lead to new approaches to healthcare delivery, especially in light of the need to reform Ukrainian healthcare. Given the magnitude of the health problem and the associated economic and social burden on livelihoods, chronic pain should be considered a priority in the healthcare system. Only specialised medical care for patients with chronic pain is able to provide sufferers with highly effective help in terms of treatment outcomes, cost-effectiveness and social relevance. Conclusions. Only a combination of structural changes in healthcare institutions and the system of training of algology specialists is the key to improving the efficiency of medical care for patients with chronic pain, reducing the economic burden on both society and individual families, and improving the social atmosphere in society.

In this study, posture and relaxing exercises accompanied by live Turkish music were performed every week for three weeks on 43 head-ache patients who are treated at the algology department. The study has been planned as active and passive therapy. Active therapy includes posture and relaxing exercises with Turkish music, while passive therapy includes listening to Turkish music in a relaxed steady position. After the third therapy session, patients were asked questions regarding the therapy and exercises. Almost all of the patients stated that the exercises and the Turkish music had positive effects on relieving their pain, helped them think positively, helped them relax and feel tranquil and that the therapy with Turkish music should be more common

Abstract This paper reports a 2015-2019 study of freshwater algae found in seven typical sphagnum bogs in Khanty-Mansi Autonomous Okrug – Yugra, Russia. The algocenoses were studied using conventional methods of algology. Microscopes Nikon ECLIPSE E200 and OLYMPUS SX4 were used. The identified species are named considering the recent changes in the global algal database AlgaeBase. The chemical composition of the swamp mass is presented. In total, 459 algae and cyanobacteria were identified in conditions of low salinity (40–70 mg/L), acidified waters (pH 2.4–5.4) and a wide temperature range (2–31 °C). The analysis of the algae composition was performed.Species-rich classes, families and genera, and species-poor taxa were found. The eco-geographical and saprobiological analyses were performed. A very low similarity of algal floras in individual swamps was revealed during assessment of the algal floras using the Chekanovsky-Sorensen similarity index.

The aim of this study was to examine the outcomes of patients who underwent Spinal Cord Stimulation (SCS) in our clinic. In this study, records of 16 patients who underwent SCS in the Algology and Neurosurgery Clinic of the Ministry of Health, Health Sciences University, Samsun Training and Research Hospital between 2015 and 2018 were retrospectively analysed. The pain levels of the patients before and after the procedure were evaluated with the Visual Analogue Scale (VAS). The mean age of the patients was 54.25 ± 11.45 years. After the SCS procedure, it was found that pain levels determined by the VAS decreased significantly (p < 0.001). Eleven (68.7%) patients reported that they were quite satisfied or partially satisfied after SCS, and seven patients (43.8%) underwent revision due to hardware malfunctions and post-surgical complications. Based on our study results, we believe that SCS is an effective treatment method that can be used to reduce pain.

Aim: Chronic migraine affects quality of life negatively, causing high rates of disabilities. There is a requirement for specific prophylactic treatment options in chronic migraine patients. We aimed to evaluate effects of repetitive transnasal sphenopalatine ganglion blockade (SPG) on headache days, severity and duration of attacks, drug abuse, conversion to episodic migraine, and responses to chronic migraine treatment. Materials and Methods: Diagnosed with chronic migraine for at least six months, patients referred to the algology clinic and unresponsive to prophylactic treatments or not tolerating prophylactic treatments were evaluated in the study with a retrospective design. Patients undergoing four-session bilateral transnasal SPG blockade per week were included. SPG was performed with 0.5 cc 0.5% bupivacaine-impregnated swab sticks for 30 minutes. Interventional algology has evolved significantly in recent decades, like the introduction of new antineoplastic therapies, which have revolutionized the results of cancer patients and, with it, the presentation of pain syndromes. For the treatment of sympathetic maintenance visceral pain, there are inhibitions in practically all of this system. In the case of bone pain, the bone augmentation technique has become the first - line treatment in selected patients because of the stability it provides. Ultrasound and some regional anesthesia techniques are currently beneficial for treating chronic pain syndromes. Implantable therapy represents a permanent modality for the control of cancer pain through an intrathecal pump, that is to say, the patient is offered a lower dose of opioid in combination or not with local anesthetic and/or other adjuvants at a determined speed, which provides adequate pain relief and a decrease in opioid - induced adverse events. More recent techniques are beginning to be studied and generated evidence in patients with pain of cancer origin, for example, stimulation.

Abstract The work was carried out on the Khanty-Mansiysk Autonomous Okrug nine typical intra-bog lakes freshwater algae material in the period 2008-2018. The studies were carried out according to the classical methods adopted in algology. Species identification was carried out using light microscopes Nikon ECLIPSE E200 and OLYMPUS SX4, taking into account the nomenclature changes published in the international electronic database Algae Base. Algae, mainly phytoplankton, were studied in nine lakes with a 50 to 1162 hectares area. Each of the analysed limnological systems is characterized by both general and specific features, which, on the one hand, reflects belonging to high latitudes water bodies, and on the other hand, indicates the differences between each of the systems, due to their type and anthropogenic load. A high significance was revealed in the families Desmidiaceae and Eunatiae all lakes, a generic ranks structure significant diversity, in which the genera Dinobryon, Eunotia, Staurastrum are of the greatest importance. The work analyses the identified algae taxonomic composition calculates the algocenoses saturation, specificity, and similarity coefficients, and the dominants’ composition.

e22002 Background: Pain is one of the most important problems that affects more than half of cancer patients at any stage of the disease and disrupts the quality of life of patients. The aim should be to relieve pain as soon as possible and prevent its recurrence. In this study, we aimed to evaluate the clinical characteristics of our patients who were followed up with a diagnosis of cancer in our clinic and whose pain management was planned in a multidisciplinary manner with the department of Algology. The clinic where the study was conducted is the reference center of a middle income developing country. Therefore, the data obtained will represent the characteristics of developing countries. Methods: In this retrospective descriptive study, patients followed up with diagnosis of cancer at Ankara University Faculty of Medicine, Department of Pediatric Oncology between 2018 and 2023 and consulted to Department of Algology for pain management were included. Patients whose file information could not be accessed from the hospital information system were excluded from the study. Demographic and clinical parameters of the patients were recorded. The data were evaluated with descriptive statistical methods appropriate to distribution. Results: A total of 33 patients, 19 girls and 14 boys, aged between 2.2 and 23.5 (median 14.4) were evaluated in the study. For each case, a median of 2 (1-16) consultations were made. The most frequent cancer diagnosis was bone tumors with 42% (n:14). Nociceptive pain was present in 25 of the patients, nociceptive and neuropathic pain were present together in 8 patients. It was determined that the most commonly used treatment for pain management was paracetamol, which was used in all patients. Other agents used, in order of frequency: Fentanyl in 19 (57.6%) of the patients, NSAID in 13 (39.4%), gabapentin in 9 (27.3%), oxycodone in 12 (36.4%), 7 (3%) 21.2) were tramadol, 4 (12.5%) were morphine, and 1 (3%) was codeine-paracetamol. Intravenous patient-controlled analgesia (PCA) was used in one patient. Side effects were observed in 3 patients. Fentanyl-related constipation was observed in 1 patient, fentanyl-related globe vesicale was observed in 1 patient, and thrombocytopenia was observed in 1 patient due to gabapentin. Conclusions: Pain is an important factor affecting the quality of life and treatment compliance of children with cancer. Therefore, it is important to ensure effective treatment and to raise the awareness of physicians and care providers by keeping it constantly on the agenda especially in developing countries. Prospective studies will be most useful guide for clinicians.

The purpose of this revision is to characterize the pain sensation, after a critical analysis of the different philosophical streams related to the human knowledge and its expression in the historical evolution of the algology. On the other hand, to emphasize the importance of electroacupuncture-induced neuro-modulation in the field of experimental pain researches. In this content will be analyzed the concept of Khun paradigm and his ideas about the structure of scientific revolution in the theory of gates control and the explosion of pain researches in the last decades. It will related the introduction to acupuncture and its techniques in pain clinics, with scientific context of the historical moment. In addition, a space will be dedicated to the topic of complementary and alternative medicine on the century of evidence based medicine, given its scientific needs of validation in ours times.

On 14 of March 2022 it was 120 years since the day of birth of Tatiana Fiodorovna Stschapova, outstanding algologist, doctor of biological sciences. On 7 of May 2023 there was 75 years from her including in the staff of Oceanology Institute as senior scientist. She was one of the brilliant scientific pleiad which has created world-wide reputation of our Institute. Tatiana Fiodorovna studied bottom flora and vegetation in Black, Azov, Caspian, White and Barents Seas. In 1948 she organized and headed the first expedition of IO on Far East seas. Those expeditions have continued their activity in 1950th and raised marine algology of Japan Sea and Sakhalin Island to new level of knowledge. Stschapov’s publications on bottom flora and structure of benthic communities are up to now considered to be standard of hydrobotanical investigations. Her doctor thesis has laid the foundation of marine phytogeography. The scientific direction she had created has been continued by her follower Vera Vozzhinskaya and exists nowadays in the Laboratory of coastal ecology IO. Returning to Moscow from Sakhalin expedition Tatiana Fiodorovna perished in the airplane crash – being just in the prime of her talent. The investigation of archive documents resulted in very interesting data on her family and ancestors – on the historical background of our motherland.

Background. Epiduroscopy, or spinal endoscopy, is the visualisation of the epidural space using a percutaneous and minimally invasive imaging fiberoptic device. Recently, as a result of some studies, it has been reported that laser therapy with epiduroscopic laser neural discectomy (ELND) was applied during multiple lesions. Methods. In this study, ELND performed between January 2012 and July 2016 at the Algology Clinic of the Department of Anesthesiology and Reanimation, Sakarya University Training and Research Hospital, was examined retrospectively. The Oswestry Disability Index (ODI) and Visual Analogue Scale (VAS) scores were recorded preoperatively, as well as after 2 weeks and 2, 6, and 12 months after the ELND. Results. According to the preoperative VAS and ODI scores, the decrease in postoperative 2nd week, 2nd, 6th, and 12th month VAS and ODI scores was significant p = 0.001 . Similarly, according to the postoperative 2nd week VAS and ODI scores, decrease in postoperative 6th and 12th VAS and ODI scores was significant p = 0.001 . Conclusions. As a result, ELND with Holmium: YAG laser, which is a new technique in patients with lumbar disc herniated low back and/or leg pain, can reduce VAS and ODI scores from 2 weeks without any complications that open surgery can bring with it. We believe that it is a useful and advanced technique in treatment of lumbar disc herniation and has low complication rates that provides maximum efficacy from the first year.

This study aimed to investigate the effect of increased epidural pressure on intraocular pressure and other ocular findings due to the amount of fluid delivered to the epidural area and the rate of delivery of the fluid. Material and Methods After obtaining approval from the Ethics Committee of Sakarya University Faculty of Medicine, patients who underwent ELNP in the Department of Anesthesiology and Reanimation Department, Algology Clinic, between January 2017 and May 2017 were included in this retrospective study. To evaluate the ocular findings after the operation, measurements obtained using an optical coherence tomography device were retrieved from the patient files and evaluated. Results Data from the medical files of 52 patients from the hospital system were evaluated. There was no significant difference between preoperative and postoperative retinal nerve fiber layer (RNFL) thickness, mean central macular thickness, optic disk area, and vertical cup-to-disk ratio (P &gt; 0.05). Conclusions Epiduroscopy procedures include intermittent or continuous infusion of saline into the epidural area. Currently, the volume of fluid that should be given to the epidural area in epiduroscopy procedures is very controversial. As a result of this study, we concluded that the amount of fluid used during ELNP, at 107.25 mL and 8.33 mL/min, had no effect on the intraocular pressure, optic disk diameter, macular thickness, or peripapillary RNFL thickness; thus, it was safe for ELNP.

Aim: Our aim in this article is to present the mid-term results of the best preoperative and postoperative medical treatment and analgesia treatment in patients with advanced Buerger's disease who underwent saphenous patchplasty and saphenous distal bypass after endarterectomy to the popliteal artery or trifurcation area. Material and Methods: Between 2015 and 2023, we operated on 83 patients, diagnosed with Buerger's disease, who were in the chronic stage of the disease, and whose advanced imaging was performed. Preoperative, postoperative, and 1st year control evaluations were determined according to Rutherford, Fontaine classification, and Ankle-Brachial Indexes. Medical treatments of the patients were organized by the vascular surgery team and algology department. After determining the surgical approach strategy according to the imaging of the patients, saphenous patchplasty and anatomical bypass were performed. Results: The mean preoperative ankle-brachial index was 0.305, the mean postoperative ankle-brachial index was 0.644, and the mean 1-year ankle-brachial index was 0.629 in 83 patients included in the study. Statistically significant improvements were observed in the Rutherford and Fontaine classifications. (p-value < 0,001) Amputation was performed in 9.6% of patients within 1 year. Conclusion: Good clinical results and limb salvage can be achieved with a combined medical and surgical treatment approach in advancedstage patients who do not respond despite optimal medical treatment. Provided that patients quit smoking, good determination of the target surgical site, Saphenous patchplasty, and Saphenous distal bypass applied after endarterectomy in suitable patients are supported with good medical and analgesia treatment, the results will improve further.

Подход к лечению боли, о котором говорится в статье, называется «нарративная алгология» (narrative – повествование, algology – направление в медицине, изучающее управление болью). Сочетание медицинской помощи, техник психотерапии и гипноза дополняется также работой с уникальной личной историей пациента. Философское осмысление практического опыта авторов статьи показывает, насколько действенным может оказаться внимательное слушание человека, который страдает не только непосредственно от самой боли, но и от ее социальных последствий. Образ человека, захваченного болью, – это не просто тот, у кого нарушены те или иные телесные функции, это еще и тот, кто чувствует себя обособленным от своего привычного окружения, а если боль длится долго и начинает разрушать социальные связи, человек и вовсе становится (или чувствует себя) изгоем. Маргинализации страдающего от боли человека способствуют также все более усиливающаяся технологизация медицины и медикализация общества в целом. Тотальной объективации человека и его тела мы можем противопоставить гуманистические техники повествования, благодаря которым человек страдающий получает шанс быть услышанным и понятым. Именно нарративный подход предлагает путь возможного исцеления с помощью принятия, возвращения человеку самоуважения и социальной реинтеграции. Однако сложность нарративного подхода при всей его кажущейся простоте состоит в том, что боль не только трудно выразить и объяснить другому, она плохо поддается описанию и для самого страдающего от боли человека. Обычно мы стараемся описывать боль, визуализируя телесные ощущения через образы и метафоры, пытаясь ее как-то «ухватить» и рассказать о ней, либо, когда она невыносима, мы уже вынуждены выражать ее через крики, стоны – звуки, напоминающие о нашей животной природе. Такой языковой регресс, присущий тяжелобольным пациентам, вынуждает терапевтов придумывать способы, как их понимать, минуя выученный человеческий язык, они стремятся расшифровать язык телесного и душевного страдания, переводя его на более понятный язык симптомов, которые можно измерить с помощью приборов, и в процессе этого «перевода» зачастую выхолащиваются и утрачиваются глубинная суть и экзистенциальный смысл боли. Таким образом, безусловная субъективная реальность боли оказывается трудноуловимой для языка и анализа. Между тем без этого описания и анализа сложно понять боль и лечить ее. Как же тогда мы можем оценивать, понимать и лечить человека, испытывающего боль? Обращаясь непосредственно к нему и к тому, что он выражает, обращая внимание не только на произносимые им слова, но и на все феномены человеческого самовыражения, которые переживаются взаимно, когда люди встречаются и рассказывают друг другу о себе, о других, о своих общих заботах. Когда одно Я делится своей болью и ждет помощи от другого Я – это подлинная антропологическая ситуация, в которой раскрывается наша человечность и проявляется общность между людьми, а рассказывание другим о своем опыте позволяет человеку искать и находить смысл того, что он переживает. Потому повествование о боли – это прорыв через непонимание от одного отдельного бытия к другому. Таким образом, работа с болью представляет собой встречу двух субъектов через повествование и заботу о другом, чтобы человек, страдающий от боли, мог утешиться, исцелиться и вновь найти свое место в жизни. L'approche du traitement de la douleur décrite dans cet article est appelée «algologie narrative» (la narration est une mise en récit du vécu ou de l'histoire, l'algologie est la discipline de la médecine qui étudie la gestion de la douleur). La combinaison de soins médicaux, de psychothérapie et de techniques d'hypnose est complétée par l’accueil et un travail sur l'histoire personnelle unique du patient. La réflexion philosophique des auteurs sur l'approche de l'expérience pratique montre avec quelle efficacité il est possible d'écouter attentivement une personne qui souffre non pas de la douleur, mais de ses conséquences sociales. Si la douleur dure longtemps et reste inexpliquée par la biotechnologie médicale, celle-ci devient suspecte et conflictuelle au sein d’une société humaine qui marginalise la personne douloureuse. Dans cette situation, la narration, essentielle à la condition humaine, propose un chemin de possibles apaisements avec une réinsertion sociale. Cependant, la complexité de cette méthode narrative, malgré son apparente simplicité, réside dans le fait que la douleur est non seulement difficile à exprimer et à décrire à une autre personne. Nous sommes obligés soit de décrire la douleur par métaphore, en essayant de la «capturer» d'une façon ou d'une autre. Parfois, lorsqu'elle est insupportable, la douleur s’exprimera par des cris, des gémissements - des sons qui rappellent notre nature animale. Cette régression linguistique inhérente aux patients gravement malades oblige les thérapeutes à imaginer des moyens de les comprendre. Ainsi, la réalité subjective inconditionnelle de la douleur, inobjectivable sinon de façon très réductrice, nécessite d’être racontée pour mieux la comprendre et la soigner. Comment alors évaluer, comprendre et soigner une personne douloureuse ? En se tournant vers elle et ce qu’elle exprime. Non pas seulement sa parole articulée mais tous les phénomènes d’expression de l’homme, lesquels s’éprouvent réciproquement lorsque les hommes se rencontrent et racontent leur souci d’eux-mêmes et d’autrui ainsi que leurs préoccupations communes. C’est une situation anthropologique normale alors qu’une personne se plaint de douleur à une autre personne, où un « Je » dénonce une douleur et revendique une aide auprès d’un autre «Je». La narration tient une place primordiale car elle est constitutive de la nature humaine, elle est importante parce que raconter son vécu à autrui permet d’en rechercher le sens. Le récit de la douleur est donc une percée à travers l'incompréhension, d'un être individuel à un autre. Ainsi, prendre en charge la douleur représente l’alliance de la bio-technologie à une préalable rencontre intersubjective guidée par la narration et le souci d’autrui afin que la personne douloureuse s’apaise et retrouve sa place dans la cité.

The practice of soil algology shows that algae from the order Chlamydomonadales are among the most poorly studied and difficult to identify due to the high heterogeneity of their morphology and ultrastructure. Only the involvement of molecular genetic methods usually makes it possible to determine their taxonomic status with high accuracy. At the same time, in the algae flora of Ukraine there are more than 250 species from the order Chlamydomonadales, the status of which in most cases is established exclusively on the basis of light microscopy. This work is devoted to the study of the biotechnologically promising green alga Chlorococcum oleofaciens, taking into account the modern understanding of its taxonomic status. Two new strains of this species, separated from samples of forest litter and oak forest soil (the Samara Forest, Dnipropetrovsk region), are described. The strains were studied at the morphological level by using light microscopy methods, as well as using molecular genetic methods based on the studies of the nucleotide genes sequences of the 18S ribosomal DNA (rDNA) and chloroplast rbcL genes, the topology of secondary structures of internal transcribed spacer 2 (ITS2). The obtained results helped to confirm the presence of C. oleofaciens in the algae flora of Ukraine. Also, the authors of the article discuss the differences in the secondary structure of ITS2 in different strains of C. oleofaciens associated with the presence of compensatory base change (CBC), hemi-CBC in helices I and II, as well as deletions in helix IV and providing a basis for the hypothesis of the existence of cryptic species within C. oleofaciens. The obtained data can be used at the stage of preliminary selection of biochemical research objects.

Neuromodulation has a significant place, among the interventional methods, in the treatment of chronic pain. Spinal Cord Stimulation (SCS) is the most applied treatment method among neuromodulation methods. SCS procedure is a neuromodulation method used in the failed back surgery syndrome, neuropathic pain, and cancer pain. The 102 patients who applied SCS were included in the retrospective study from 2017-2022 in the University Algology Department. We asked about pain scores in line with visual analog scale degrees (VAS) before and after the SCS procedure, opioid use and compared the patient satisfaction with the procedure. We divided the patients into two groups as those with a diagnosis of post-laminectomy syndrome (Group 1) and those with a diagnosis of neuropathic pain (Group 2). VAS before SCS was 9.11±0.75 in Group 1 and 8.45±0.77 in Group two. After SCS, the decrease in pain intensity was much more distinct in post-laminectomy patients (Group 1), compared to the neuropathic pain patients (Group 2). The results were as follows: Group 1- 1.98±1.40, Group 2-4.08±0.71, p<0.001. The decrease in using analgesics was observed to be much more significant in Group 2. (P<0.05) Patient satisfaction was higher in Group 1 as well (p<0.05). We hold the opinion that post-laminectomy patients benefit more from the SCS procedure than neuropathic pain patients. We observed that the decrease both in pain intensity and the use of opioid analgesics was statistically and overwhelmingly distinct in post-laminectomy patients. According to the results of our study, we think that SCS is an effective and safe treatment method both for post-laminectomy and neuropathic pain patients.

Abstract:
Aim: The causative agent of herpes zoster (HZ) is the reactivated varicella-zoster virus. HZ leads to severe and painful rashes that can be accompanied by long-term pain, i.e., postherpetic neuralgia (PHN). According to the International Association for the Study of Pain (IASP), post-thoracotomy pain syndrome (PTPS) is defined as “recurrent or persistent pain along the thoracotomy incision at least two months after surgery”⁸. Methods: In this study, the medical records of all the patients were reviewed for age, gender, size, thoracic level, cause of pain, and visual analog scale (VAS) and DN4 scores from the patient files maintained in the archive of the Department of Algology. Two cycles of pulse radiofrequency (PRF) were administered for 2 min each. Then, a total of 5 ml of dexamethasone, lidocaine, bupivacaine, and isotonic solutions of 4 mg, 20 mg, 5 mg, and 5 mg, respectively, were added through the RF cannula at the DRG level in each application. Results: In total, 40 patients, including 25 men (62.5%) and 15 women (37.5%), were analyzed in this study. The mean age of the patients was 60.5 ± 12.4 years, and the median duration of pain was 2 years (0.2–15 years). When PHN and PTPS groups were compared on the basis of the pain etiology, the VAS values before treatment were not statistically different (p = 0.129), whereas the VAS values after treatment were significantly lower in the PTPS group than in the PHN group (p = 0.001). Conclusions: This study aimed to investigate the effectiveness of DRG PRF therapy on the causes of chronic thoracic pain and in different etiologies. The results revealed that PRF therapy is more effective in treating patients with PTPS than those with PHN. Methods: The study was conducted with an online questionnaire directed to the participants online, following the approval of Uludağ University Ethics Committee labeled 2020-16/1. The questionnaire was prepared on SurveyMonkey and distributed via a specially acquired link. Participants were prevented from completing the questionnaire more than once. 38 questions were asked to the participants. At the end of the questionnaire, the participants were asked to fill the DASS-21 scale. Questionnaire link has been distributed through social media and mobile communication applications. Results: 198 volunteers participated in the study. 122 of the participants were female (61.62%) and 76 (38.38%) were males. 75 of the participants (37.88%) were trainees, 113 (57.07%) were anesthesiology and reanimation specialists, 2 (1.01%) were specialist physicians undergoing intensive care training, 4 (%) 2,02) were intensive care specialists, 1 was a specialist physician receiving algology training, and 3 (1.52%) were algologists. 42 participants were (21.21%) in a training-research hospital, 87 participants (43.94%) in a university hospital, 35 participants (17.68%) in a state hospital, 13 participants (6.57%) in city hospitals, and 21 participants (10.61%) were working in private hospitals. %81.6 of all physicians participating in the survey show depression symptoms, %62 of participants were showing symptoms of anxiety and 71.1% of participants described stress symptoms. All scores were higher in female physicians (p <0,05). Depression scores of residents were higher than specialists (p<0,05). All scores of physicians working in training-research hospitals are higher than physicians working in other institutions (p <0,05). Conclusion: Almost all physicians have experienced changes in workload, income and workplaces during the pandemic. High DASS-21 scores in all physicians were thought-provoking and highlighted the psychological pressure that anesthesiology and reanimation physicians had been under.

La patiente se déplaçait en fauteuil roulant et avait un périmètre de marche avec une canne de quelques mètres seulement.Traitement initial par sulfate de morphine plus antidépresseurs (duloxétine seule, puis association avec mirtazapine) avec amélioration très partielle sur la dépression et sans amélioration au niveau fonctionnel.La patiente a bénéficié dans le même temps d’une prise en charge en algologie et en rééducation fonctionnelle avec là encore peu ou pas d’amélioration.La patiente débute un traitement antalgique par kétamine IV (40 mg/j) sous forme de cure de 5 jours toutes les 6 semaines en hôpital de jour, toujours associé à un traitement antidépresseur par duloxétine 120 mg/j avec une amélioration rapide et spectaculaire aussi bien au niveau des symptômes dépressifs qu’au niveau des algies lombosciatiques. Amélioration également au niveau fonctionnel avec disparition de l’aide par fauteuil roulant et reprise d’une marche sans canne avec un périmètre de presque 1 kilomètre.Poursuite actuel du suivi conjoint psychiatre, algologue avec rémission de la symptomatologie dépressive et poursuite de l’évolution favorable au niveau de la symptomatologie fonctionnelle.

Aims: Lumbago is one of the common conditions in our present community. Definite periods of human’s life people encountered in the form of usually regressing attacks that %10-%15 of this important condition requires surgical therapy. Cause of wrong diagnosis and treatment Spinal surgery may not always give favourable result. Patients undergone one or more back surgery exist back and/or leg pain which does not improve, is called failed back surgery syndrome. Epidural steroid administration is one of the most invasive method which is used in back pain. Effects and application results are controversial. In this study we aimed to compare the effects of steroid and local anesthetic solutions which administered by epidural way on pain controlling between the patients that have chronic back pain because of lomber radiculopathy, undergone surgery and not surgery. Methods: It was evaluated retrospectively pain controlling effect of administered steroid and local anesthetics by interlaminal epidural way to ASA 1-2-3, between 18-80 aged 40 patient which undergone piror one or plural back surgery and have no surgery between May 2009-December 2009 apply with back pain complaint to İstanbul Education and Research Hospital Algology Department. Patient’s visuel analog scala (VAS) and severity of back pains, before procedure, second week after procedure, fourth week after procedure, sixth week after procedure in either two groups out of ten points was estimated. Results: Totally of 7 ml solution which contains 1 ml triamsinolon asetonid 40 mg/ml (Sinakort-A ampul, İ.E. Ulugay), 3 ml levobupivacaine 5mg/ml ( Chirocaine ampul, Abbott) and 3 ml serum phsiyologic ( % 0,9 NaCl ) administered by epidural way into either two group patients. There were no significant difference between two groups by mean of age, weight, height, BMI and back pain durations. Evaluating the groups into themselves, opioid requirements and VAS scores before injection was markedly higher than 2,4 and 6. week scores in either two groups (p<0.05). There was no statistically significant difference when we compare into groups of 2,4 and 6. week opioid requirements and VAS scores in either two groups. All patient manifested their pleasure and there was no complication occured. Conclusion: Consequently; it was thought that epidural administration of steroid (triamsinolon) ,local anesthetics (levobupivacaine) and serum physiologic solution was effective especially on early back pain treatment, also safe method and beneficial to patient satisfaction in the patients who has failed back surgery and chronic back pain.

Abstract:
Mud volcanoes are one of unique natural phenomena widely spread around the world. They can be found in Crimea, including the Bulganak sopochnoe field – the largest cluster of active mud volcanoes on the peninsula (45°25′29.04″N, 36°27′51.64″E). Study of mud volcano microalgae in Crimea, as well as in other regions of Russia, has not been conducted so far. Therefore, scientific interest is caused by need and urgency of the study of these volcanoes. First data on microalgae species composition of active mud volcanoes are presented in this article. Samples collected by O. Yu. Eremin (03.08.2012 and 13.04.2013) in the upper 2–3-cm layer of suspension and in surface water were investigated. The ranges of salinity and water temperature were 27–32 g per L and +28…+31 °C, respectively. Microalgae species composition was determined in water preparations using Axioskop 40 (Carl Zeiss) light microscope at magnification of 10×40 with software AxioVision Rel. 4.6. Totally 16 taxa were found: Cyanobacteria (1), Dinophyta (2), Bacillariophyta (6), and Euglenophyta (7). Of these, cyanobacteria Chamaecalyx swirenkoi (Schirshov) Komárek et Anagnostidis, 1986 was found by us in the mud volcano in August 2012. Pennate species of diatoms were also identified – single living (of genera Cylindrotheca (Ehrenberg) Reimann & J. C. Lewin, Lyrella Karajeva, and Nitzschia Hassall) and colonial species (of genera Berkeleya Greville and Pseudo-nitzschia H. Peragallo). The brackish-water, benthic, boreal-tropical species Nitzschia thermaloides Hustedt was recorded for the algal flora of Crimea, the Black Sea, and the Sea of Azov for the first time. Euglenophytes were also found in the samples – 5 species of the genus Trachelomonas Ehrenberg and 2 species of the genus Strombomonas Deflandre. Of all the species found in the mud volcano ecotope, 7 species are common for the Black Sea, and 9 species, including 3 euglenophytes, are common for the Sea of Azov. It is shown that by characteristics of halobility, species found in the mud volcano belong to freshwater complex (53 %), with a significant share of marine (27 %) and brackish-water (20 %) species. Of the phytogeographic flora elements, boreal species make up 33 %, boreal-tropical – 47 %, and cosmopolites – 20 %. Three species of potentially toxic algae are recorded: diatom Pseudo-nitzschia prolongatoides (Hasle) Hasle, 1993, as well as dinophytes Prorocentrum lima (Ehrenberg) Dodge, 1975 and Alexandrium tamiyavanichii Balech, 1994. The last species is marine, boreal-tropical, and new to the algology of Crimea, the Black Sea, and the Sea of Azov. In the article, own and literary data on morphology, ecology, and phytogeography of species, as well as on their general distribution in different waterbodies of the world, are also presented. Some microalgae species are indicators of saprobity; they are able to participate in purification of water from organic substances. Photos of mud volcanoes and micrographs of some species are presented.
