Full textThis thesis focuses on the prescription of painkillers by general practitioners. It is grounded in the study of the “trajectory” of two iconic drugs. The first ones are dextropropoxyphene-containing painkillers whose brandname was Di-Antalvic (among others) and the seconds are tramadol-containing drugs. This research adopts a twofold perspective. On the one hand, the prescription of painkillers is considered as the result of a process, in this case the choice of a molecule available on a market, and it then analyzes how this market emerged and how it has functioned throughout the 20th century. This perspective aims at uncovering the very macro-sociological determinants which play a role in prescribing practices, such as the pharmaceutical industry of course, but also the (national or European) health authorities. All these stakeholders have a decisive influence on the different ways in which these analgesics circulate on the market - as they are manufactured, promoted, reimbursed, but also evaluated, re-evaluated and sometimes withdrawn from the market.On the other hand, this research analyzes the prescription of painkillers as work carried out during the consultation. It shows how general practitioners sort out different types of pain, handle hybrid cases – which often correspond to "complex patients" or "problem patients" – and therefore prescribe with the view to calm pain, whatever its origin. As a result, to prescribe, physicians rely on resources and knowledge from their training, past experiences with drugs, etc., as such work requires very specific skills in the management of side effects, for example. Full textThe ability to feel pain is crucial for life, since it serves as an early warning system forpotential harm to the body. The majority of pain evaluations rely on patient reports. Patients who are unable to express their own pain must instead rely on third-party reportsof their suffering. Due to potential observer bias, pain reports may contain inaccuracies. In addition, it would be impossible for people to keep watch around the clock. Inorder to better manage pain, especially in noncommunicative patients, automatic paindetection technologies might be implemented to aid human caregivers and complementtheir service. Facial expressions are used by all observer-based pain assessment systemsbecause they are a reliable indicator of pain and can be interpreted from a distance.Taking into consideration that pain generally generates spontaneous facial behavior,these facial expressions could be used to detect the presence of pain. In this thesis, weanalyze facial expressions of pain in order to address pain estimation. First, we presenta thorough analysis of the problem by comparing numerous common CNN (Convolutional Neural Network) architectures, such as MobileNet, GoogleNet, ResNeXt-50, ResNet18, and DenseNet-161. We employ these networks in two unique modes: standalone and feature extraction. In standalone mode, models (i.e., networks) are utilized to directly estimate pain. In feature extractor mode, "values" from the middle layer are extracted and fed into classifiers like Support Vector Regression (SVR) and Random Forest Regression (RFR).CNNs have achieved significant results in image classification and have achievedgreat success. The effectiveness of Transformers in computer vision has been demonstrated through recent studies. Transformer-based architectures were proposed in the second section of this thesis. Two distinct Transformer-based frameworks were presented to address two distinct pain issues: pain detection (pain vs no pain) and thedistinction between genuine and posed pain. The innovative architecture for binaryidentification of facial pain is based on data-efficient image transformers (Deit). Twodatasets, UNBC-McMaster shoulder pain and BioVid heat pain, were used to fine-tuneand assess the trained model. The suggested architecture is built on Vision Transformers for the detection of genuine and simulated pain from facial expressions (ViT). Todistinguish between Genuine and Posed Pain, the model must pay particular attentionto the subtle changes in facial expressions over time. The employed approach takes intoaccount the sequential aspect and captures the variations in facial expressions. Experiments on the publicly accessible BioVid Heat Pain Database demonstrate the efficacy of our strategy

In South Africa, inland aquaculture is on the increase, especially among the rural communities. Thus aquaculture is able to provide a source of employment and improve rural incomes. This study assessed algae species and their cyanotoxins in small holder production in Vhembe district, Limpopo, South Africa. Thirteen study sites were selected to assess the algae species and their cyanotoxins. The water samples were collected in four quarters and analysed for water temperature, pH, total dissolved solids, Electrical conductivity, phosphates, nitrates, chlorophyll, cyanobacteria, cyanotoxins, metal species and quality of the water in the fish ponds. In the 1st quarter of the year there was variation in Water Temperature (23.7oC-31.4oC), pH (5.5-9.6), EC (3.82-46.8μS/cm), TDS (2.4-45 mg/L), Phosphate (0.0-1.09mgL-1), Nitrates (0.0-1.00mgL-1), Chlorophyll-a (5.8-11.5mg/m-3). In the 2nd quarter there was variation inwater temperature (22.4-25.0oC), pH (6.6-8.8.9), EC (19.23-21.47μS/cm), TDS (12.5-17.9 mg/L), Phosphate (1.64-1.84mgL-1), Nitrates (1.02-1.88mgL-1), Chlorophyll-a (4.6-15.6mg/m-3). In the 3rd quarter there was variation for water temperature (22.9-25.0oC), pH (7.5-9.1), EC (7.91-293.3μS/cm), TDS (11.7-180.9 mg/L), Phosphate (1.10-1.80mgL-1), Nitrates (1.28-1.84mgL-1),Chlorophyll-a (6.8-15.6mg/m-3). In the 4th quarter there was variation forwater temperature (23.5-30.3), pH (7.1-9.3), EC (18.24-623μS/cm), TDS (23.7-136.4 mg/L), Phosphate (1.45-1.99mgL-1), Nitrates (1.43-1.68mgL-1), Chlorophyll-a (6.6-25.9mg/m-3). The metal content of the fish pond water was variable throughout the year but with moderate levels of Al, Cd, Cr, Mn, Fe, Cu, Zn and Ba were found. The metal Fe, exceeded the DWAF guideline values during this first quarter. The presence of Cd in the fish pond water could be attributed to rainfall eroding the earthen embankments of the fish pond. The results of physico-chemical parameters promotes the growth of cyanobacteria in the fish ponds. Flow cam and SEM were used to identify the cyanobacteria species and most cyanobacteria identified are hazard to human health, fish and other aquatic organisms. Molecular technologies were used to identify cyanotoxins and there was no cyanotoxins detected which was concluded that during collection of water samples no cyanobacteria produced toxins.
