Abstract:
Background: We aimed to identify the long-term risk of recurrence and mortality in patients who experienced acute ischemic stroke (AIS), acute myocardial infarction (AMI), or acute hemorrhagic stroke (AHS) using a population-level database. Methods: This retrospective cohort study included adults aged ≥55 years diagnosed with AIS, AMI, and AHS in the National Health Insurance Service Database between 2004 and 2007. The target outcomes were secondary AIS, AMI, AHS, and all-cause mortality. Predetermined covariates, such as age, sex, socioeconomic status, hypertension, diabetes, and dyslipidemia, were adjusted. Results: We included 151,181, 49,077, and 41,636 patients in the AIS, AHS, and AMI groups, respectively. The AMI (adjusted hazard ratio [aHR], 0.318; 95% confidence interval [CI], 0.306–0.330; p < 0.001) and AHS (aHR, 0.489; 95% CI, 0.472–0.506; p < 0.001) groups had a significantly lower risk of developing secondary AIS than the AIS group. The risk of developing secondary AMI was significantly lower in the AMI (aHR, 0.388; 95% CI, 0.348–0.433; p < 0.001) and AHS (aHR, 0.711; 95% CI, 0.640–0.790; p < 0.001) groups than in the AIS group. Initial AHS was a decisive risk factor for secondary AHS (aHR, 8.546; 95% CI, 8.218–8.887; p < 0.001). The AMI (aHR, 1.436; 95% CI, 1.412–1.461; p < 0.001) and AHS (aHR, 1.328; 95% CI, 1.309–1.348; p < 0.001) groups were associated with a significantly higher risk of long-term mortality than the AIS group. Conclusion: Our results elucidated that initial AIS was a significant risk factor for recurrent AIS and AMI; initial AHS was a decisive risk factor for developing secondary AHS. Further, AMI and AHS were more closely related to long-term mortality than AIS.

Abstract:
The immune system has pattern recognition capabilities based on reinforced learning, memory, and affinity maturation interacting between antigens (Ags) and antibodies (Abs). This article deals with an adaptation of artificial immune system (AIS) into genetic-algorithm (GA)-based multi-objective optimization. The present study utilizes the pattern recognition from an AIS and the evolution from a GA. Using affinity measures between Ags and Abs, GA-based immune simulation discovers a generalist Ab that represents the common pattern among Ags. Non-dominated Pareto-optimal solutions are obtained via GA-based immune simulation in which dominated designs are considered as Ags, whereas non-dominated designs are assigned to Abs. This article discusses the procedure of identifying Pareto-optimal solutions through the immune system-based pattern recognition. A number of mathematical function problems that are described by discontinuity or disconnection in the shape of Pareto surface are first examined as test examples. Subsequently, engineering optimization problems such as rotating flywheel disc and ten-bar planar truss are explored to support the present study.

AbstractAdrenal incidentalomas (AIs) have been associated with an increased risk of metabolic syndrome and dyslipidemia, though evidence regarding the latter is limited. Lipid abnormalities in patients with AIs have been associated with subclinical hypercortisolism. The current study aims to test whether lipid profile in patients with AIs predicts “autonomous cortisol secretion” (ACS). Patients with AIs found on either computerized tomography (CT) or magnetic resonance imaging (MRI), were included in a prospective cohort study. All patients were followed up for at least three years. Alterations in their hormonal and lipid profiles were recorded. Ninety-four patients (69 women) harboring 111 AIs were included. There were no differences between patients with ACS and those without, with respect to their baseline lipid profile [total cholesterol, low-density-lipoprotein cholesterol (LDL-C), triglycerides, high-density lipoprotein cholesterol (HDL-C) and non-HDL-C] and blood pressure (systolic and diastolic). Non-HDL-C concentrations decreased over time (Repeated Measures ANOVA, p=0.013), despite patients’ body mass index (BMI) remaining unchanged. Logistic regression analysis revealed that the only predictor of ACS was the size of AIs, as calculated by CT or MRI. The current study demonstrated that lipid profile at baseline or during follow-up cannot predict ACS in patients with AIs. However, larger AIs may have a greater probability of ACS.

Abstract:
Purpose This paper aims to present the development of prediction models for aircraft actuator failure impact on flight envelope within the artificial immune system (AIS) paradigm. Design/methodology/approach Simplified algorithms are developed for estimating ranges of flight envelope-relevant variables using an AIS in conjunction with the hierarchical multi-self strategy. The AIS is a new computational paradigm mimicking mechanisms of its biological counterpart for health management of complex systems. The hierarchical multi-self strategy consists of building the AIS as a collection of low-dimensional projections replacing the hyperspace of the self to avoid numerical and conceptual issues related to the high dimensionality of the problem. Findings The proposed methodology demonstrates the capability of the AIS to not only detect and identify abnormal conditions (ACs) of the aircraft subsystem but also evaluate their impact and consequences. Research limitations/implications The prediction of altered ranges of relevant variables at post-failure conditions requires failure-specific algorithms to correlate with the characteristics and dimensionality of self-projections. Future investigations are expected to expand the types of subsystems that are affected and the nature of the ACs targeted. Practical implications It is expected that the proposed methodology will facilitate the design of on-board augmentation systems to increase aircraft survivability and improve operation safety. Originality/value The AIS paradigm is extended to AC evaluation as part of an integrated and comprehensive health management process system, also including AC detection, identification and accommodation.

A novel artificial immune system algorithm with social learning mechanisms (AIS-SL) is proposed in this paper. In AIS-SL, candidate antibodies are marked with an elitist swarm (ES) or a common swarm (CS). Correspondingly, these antibodies are named ES antibodies or CS antibodies. In the mutation operator, ES antibodies experience self-learning, while CS antibodies execute two different social learning mechanisms, that is, stochastic social learning (SSL) and heuristic social learning (HSL), to accelerate the convergence process. Moreover, a dynamic searching radius update strategy is designed to improve the solution accuracy. In the numerical simulations, five benchmark functions and a practical industrial application of proportional-integral-differential (PID) controller tuning is selected to evaluate the performance of the proposed AIS-SL. The simulation results indicate that AIS-SL has better solution accuracy and convergence speed than the canonical opt-aiNet, IA-AIS, and AAIS-2S.

Abstract:
During coagulation, the soluble fibrinogen is converted into insoluble fibrin. Fibrinogen is a multifunctional plasma protein, which is essential for hemostasis. Various oxidative posttranslational modifications influence fibrinogen structure as well as interactions between various partners in the coagulation process. The aim was to examine the effects of oxidative stress conditions on fibrin clot formation in arterial atherothrombotic disorders. We studied the changes in in vitro fibrin network formation in three groups of patients—with acute coronary syndrome (ACS), with significant carotid artery stenosis (SCAS), and with acute ischemic stroke (AIS), as well as a control group. The level of oxidative stress marker malondialdehyde measured by LC-MS/MS was higher in SCAS and AIS patients compared with controls. Turbidic methods revealed a higher final optical density and a prolonged lysis time in the clots of these patients. Electron microscopy was used to visualize changes in the in vitro-formed fibrin network. Fibers from patients with AIS were significantly thicker in comparison with control and ACS fibers. The number of fibrin fibers in patients with AIS was significantly lower in comparison with ACS and control groups. Thus, oxidative stress-mediated changes in fibrin clot formation, structure and dissolution may affect the effectiveness of thrombolytic therapy.

The storage of encryption and decryption keys are stored in locations and devices different from those in which the database is stored in the cloud for ensuring more safety.

Aromatase inhibitors (AIs) have radically changed the prognosis of hormone receptor positive breast cancer (BC) in post-menopausal women, and are a mainstay of the adjuvant therapy for BC after surgery in place of, or following, Tamoxifen. However, AIs aren’t side effect-free; frequent adverse events involve the musculoskeletal system, in the form of bone loss, AI-associated arthralgia (AIA) syndrome and autoimmune rheumatic diseases. In this narrative review, we reported the main clinical features of these three detrimental conditions, their influence on therapy adherence, the possible underlying molecular mechanisms and the available pharmacological and non-pharmacological treatments. The best-known form is the AIs-induced osteoporosis, whose molecular pathway and therapeutic possibilities were extensively investigated in the last decade. AIA syndrome is a high prevalent joint pain disorder which often determines a premature discontinuation of the therapy. Several points still need to be clarified, as a universally accepted diagnostic definition, the pathogenetic mechanisms and satisfactory management strategies. The association of AIs therapy with autoimmune diseases is of the utmost interest. The related literature has been recently expanded, but many issues remain to be explored, the first being the molecular mechanisms.

Abstract:
Objective: The cardiovascular outcomes of insulin detemir in patients with type 2 diabetes mellitus (T2DM) after acute coronary syndrome (ACS) or acute ischemic stroke (AIS) are unclear. The aim of our real-life cohort study was to evaluate the cardiovascular outcomes of insulin detemir (IDet) versus insulin glargine (IGlar) in T2DM patients after ACS or AIS. Methods: A retrospective cohort study was conducted between June 1, 2005, and December 31, 2013, utilizing the Taiwan National Health Insurance Research Database. A total of 3,129 ACS or AIS patients were eligible for the analysis. Clinical outcomes were evaluated by comparing 1,043 subjects receiving IDet with 2,086 propensity score-matched subjects who received IGlar. The primary composite outcome included cardiovascular (CV) death, nonfatal myocardial infarction (MI) and nonfatal stroke. Results: The primary composite outcome occurred in 322 patients (30.9%) in the IDet group and 604 patients (29.0%) in the IGlar group (hazard ratio [HR], 1.12; 95% confidence interval [CI], 0.95 to 1.32) with a mean follow-up of 2.4 years. No significant differences were observed for CV death (HR, 1.09; 95% CI, 0.86 to 1.38), nonfatal MI (HR, 0.88; 95% CI, 0.66 to 1.19), and nonfatal stroke (HR, 1.15; 95% CI, 0.97 to 1.35). There were similar risks of all-cause mortality, hospitalization for heart failure and revascularization between the IDet group and the IGlar group ( P = .647, .115, and .390 respectively). Conclusion: Compared with IGlar, in T2DM patients after ACS or AIS, IDet was not associated with increased risks of CV death, nonfatal MI, or nonfatal stroke. The main priority aspect of information and communication technologies is given to control mechanisms, both internal and external in enterprises. It ensures that the reports and the decisions received and generated by the management will support their decision-making. The decisions have honesty and high integrity based on the results of the audit conducted on systems of information and communication technology. The objective of this research is to deliver audit reports of information systems for management and make recommendations on the audit findings in PT Erajaya Swasembada Tbk. Business processes studied included sales, purchasing, finance, and the warehouse. The system used was "Erajaya Live Application Server" and ERP (Enterprise Resource Planning)-based. This research used thedomain of COBIT 4.1: Acquire and Implement. The domain included several sub-domains, which were:identify automated solutions (AI1), acquire and maintain application software (AI2), acquire and maintain technology infrastructure (AI3), enable operation and use (AI4), procure IT resources (AI5), manage changes (AI6), and install and accredit solutions and changes (AI7). Data were collected from interviewing IT Department, distributing questionnaires to respondents, and observing the business processes of this enterprise. Research obtained 57 audit findings on IT implementation. The results of process reference model formulation are 3 findings on AI1subdomain, 5 findings on AI2subdomain, 9 findings on AI3subdomain, 6 findings on AI4 subdomain, 11 findings on AI5subdomain, 13 findings pada AI6subdomain, and 10 findings on AI7subdomain. The level of maturity model of this domain, Acquire and Implement (AI), was found on level 3.

Abstract:
Air intake system (AIS) plays an important role on affecting the performance of the engine and the vehicle. The design and optimization of the structures and materials of AIS contributes to producing a quality reliable system, reducing noise, cost and weight at the same time, which are significant to improve the performance of engine and vehicle. Fresh air hoses/pipes are intended to transport, in air cleaner purified, air from the air cleaner to the throttle fitted on the inlet manifold or to the compressor fitted to the exhaust manifold. Air cleaner box is responsible for filtering out the dust and impurities in the air. The charge air pipes are intended to transport purified compressed air from the compressor to the charge air cooler and then to the throttle fitted on the inlet pipe. For the air cleaner box optimization, through the benchmarking and the innovation ideas from the daily life, two alternative optimized designs were introduced to reduce the cost and weight. The first is four clips connectors and the other is spring clamps. For the hose clamps, another innovation design was introduced to replace the previous hose clamps, which is called friction connector on the inner side of the bellow hose, the outer side of the air cleaner box lid and the clean side duct. For the material of the charge air ducts, TPEE was selected to replace the previous EACM rubber hoses. Further tests and prototypes should be conducted and produced to verify the effect of the optimization.

Arbetet är en fallstudie om aktiva nautikers inställning till användandet av AIS och radar ur en navigatorisk synpunkt. Syftet var att få en uppfattning om hur ett av de nyaste elektroniska hjälpmedlen står sig mot ett av de mer inarbetade och beprövade instrumenten som finns ombord på dagens kommersiella fartyg.För att kunna få en uppfattning om vad nautikerna ansåg utfördes intervjuer ombord på ett svenskt fartyg. Frågorna som ställdes var samma för både AIS och radar för att datan skulle bli jämförbara. De slutsatser som drogs var att AIS som fristående system inte ännu är något som dagens nautiker litar på. Då radarn togs upp till diskussion var mönstret tydligt att systemet ansågs ha hög tillförlitlighet samt att nautikerna näst efter sina egna ögon litar mest på radarn bland de navigatoriska hjälpmedlen. Då en AIS integreras med en radar ansåg samtliga nautiker att tillförlitligheten på båda systemen ökar.


The essay was based upon the active nautical officer’s attitude towards the use of AIS and radar from a navigational point of view. The aim was to get a view of how one of the newest electronic aids keeps well against one of the most and the well-tried instruments that are found onboard on today's commercial vessels. In order to receive an insight the nautical officer’s opinions interviews were conducted onboard a Swedish merchant vessel. The questions that were asked were the same about both AIS and radar in order to get comparable data. The conclusions that were drawn were that AIS as an independent system is not yet something that today's nautical officers trust. When the radar was taken up to discussion all the interviewees stated that the system had high reliability and that it is the second most reliable after their own eyes among the navigational aids. The reliability of both systems increases when an AIS is integrated with a radar according to the result.

An ethnographic study loosely informed by the theoretical framework of distributed cognition was carried out in order to describe how mariners have adopted the Automatic Identification System (AIS) in their work practice, or"made the technology their own". AIS is a transponder-based identification and communication system that allows ships to automatically identify and track each other. In addition to facilitating the identification and tracking of ships, objectives behind the introduction of AIS are to"simplify informational exchange", and"provide additional information to assist situation awareness". Participant observation and interviews were made at four different ships, as well as at two shore stations. A focus group was also held at a maritime conference. The study gave some interesting results. For example, a Problem of Public Information Loss was identified. It is tentatively suggested that this problem has been overlooked partly because of a widespread but impoverished model of communication which does not account for the role of side-participants in a conversation. It is concluded that more research needs to be done on maritime work and the use of new bridge technology.

This paper's main objective is to consolidate the knowledge on context in the realm of intelligent systems, systems that are aware of their context and can adapt their behavior accordingly. We provide an overview and analysis of 36 context models that are heterogeneous and scattered throughout multiple fields of research. In our analysis, we identify five shared context categories: social context, location, time, physical context, and user context. In addition, we compare the context models with the context elements considered in the discourse on intelligent systems and find that the models do not properly represent the identified set of 3,741 unique context elements. As a result, we propose a consolidation of the findings from the 36 context models and the 3,741 unique context elements. The analysis reveals that there is a long tail of context categories that are considered only sporadically in context models. However, particularly these context elements in the long tail may be necessary for improving intelligent systems' context awareness.

Le système d'identification automatique (ou système AIS pour automatic identification system) est un système qui permet aux navires et aux stations côtières de s'échanger certaines informations par radio VHF. Ces informations comprennent l'identifiant, le statut, la position, la direction et la vitesse de l'émetteur. L'objectif de cette thèse est de permettre la réception des messages AIS par un satellite en orbite basse sans modifier le matériel existant équipant les navires. Par l'intermédiaire du système AIS, il devient possible de connaitre la position de tous les navires à travers le monde. Plusieurs nouveaux services sont possibles, comme le contrôle maritime global ou, pour les armateurs, la connaissance constante de la position de leurs bateaux. La réception par satellite des signaux AIS est sujette à un niveau de bruit bien plus élevé que lors de la réception de ces signaux au niveau du sol. Ce niveau de bruit rend les méthodes classiques de réception de ces signaux difficilement utilisables. Une première contribution de cette thèse est le développement de nouveaux démodulateurs utilisant des méthodes de correction d'erreurs. Ceux-ci tirent parti de la présence d'un bloc de contrôle de redondance cyclique (CRC) dans les messages ainsi que de certaines informations connues sur la structure des messages et des données. Des adaptations du récepteur proposé ont également été étudiées afin d'intégrer la poursuite de la phase des signaux reçus et de prendre en compte les collisions des messages envoyés simultanément par plusieurs navires. La dernière partie de cette thèse est consacrée à l'étude des méthodes de localisation des navires ne diffusant pas leur position dans leurs messages AIS. Cette localisation tire parti des paramètres des messages reçus tels que le délai de propagation et le décalage en fréquence de la porteuse dû à l'effet Doppler, et d'un modèle de déplacement des navires
The automatic identification system (AIS) is a system allowing ships and coast stations to exchange some information by VHF radio. This information includes the identifier, status, location, direction and speed of the emitter. The aim of this thesis is to allow the reception of AIS messages by low Earth orbit satellites without modifying the existing ship equipments. With this system, it becomes possible to know the position of all ships over the Earth. As a consequence, several new services become available, such as global traffic monitoring or determining boat location (for ship-owners). Satellite reception of AIS signals is subjected to a higher noise level when compared to ground level reception. This noise makes classical demodulation and decoding methods unusable. A first contribution of this thesis is to develop new demodulators using error correction methods. These demodulators take advantage of the presence of a cyclic redundancy check (CRC) block in the messages as well as known information about the structure of messages and data. Generalizations of the proposed receiver have also been studied in order to take into account the phase noise of the received signals and the possible collision of messages sent simultaneously by several vessels. The last part of this thesis is devoted to the study of localization methods for ships that do not transmit their location in AIS messages. This localization takes advantage of information contained in the received messages such as the propagation delay and the carrier frequency shift due to the Doppler effect, and a ship movement model

The current centralization process on the health area has ocurred by means of a strong deconcentration mechanism. The analysis of the three pro posais for the reorganization of the health area, within the period 1983-1990, aims at a change of strategy in the deconcentration mechanism. Previously, the central institutions kept deconcentrated structures acting over intermediate and local leveis. As from 1983, the trend is to observe intermediate and local leveis operating as deconcentrated structures. Therefore, deconcentration is in fact ocurring in the health area. Deconcentration may have two different aims, i.e., it can be a strengthening and updating strategy for centralization or it can be a strengthening strategy at intermediate and local leveis, and, therefore, a tool to facilitate and promote decentralization. Taking into consideration the historical development of the health area in Brazil, it would be necessary to investigate the present purpose of deconcentration.
O processo de centralização vigente no setor saúde desenvolveu-se utilizando-se fortemente o mecanismo de desconcentração. A análise das três propostas de reorganização do setor saúde, no período de 1983 a 1990, aponta para uma mudança de estratégia no mecanismo de desconcentração. Anteriormente, os órgãos centrais dispunham de estruturas desconcentradas atuando nos níveis intermediários e locais. A partir de 1983 observa-se a tendência da atuação dos níveis intermediários e locais como instâncias desconcentradas do nível central. Portanto, a desconcentração é um processo em curso no setor. A desconcentração pode assumir sentidos distintos, isto é, pode ser uma estratégia de fortalecimento e atualização da centralização ou pode ser uma estratégia de fortalecimento dos níveis intermediários e locais, e, portanto, facilitadora e promotora da descentralização. Considerando o contexto delineado para o setor saúde no Brasil, caberia investigar qual o atual sentido da desconcentração.

Abstract:

Intervjuer med kunniga på AIS-området har utförts parallellt med en enkät riktad till aktiva befäl ute till sjöss. Detta i syfte att ta reda på dels hur detta hjälpmedel bör användas vad avser antikollision, dels hur det faktiskt används i praktiken.

AIS, Automatic Identification System, är ett informationsdelningssystem som hjälper fartyg att navigera säkert genom att sända ut egen navigationsdata och samtidigt ta emot motsvarande information från andra fartyg. Systemet nyttjas även som informationssystem för landbaserade stationer såsom VTS-centraler och kustbevakning.

Undersökningen har engagerat och visat att expertisen anser att AIS, i många avseenden, är ett bra och användbart hjälpmedel som bör komplettera övrig utrustning vid bedömning om kollisionsrisk. Vidare visar enkäten att ARPA-systemet, trots vissa inneboende brister, tillmäts större förtroende än AIS och AIS-vektorer används därför sparsamt. Enkäten visar även att en stor del av de aktiva inte är främmande för att göra avsteg från sjövägsreglerna emedan expertisen i huvudsak avråder från sådant.


Interviews with experts, in the field of AIS, have been conducted alongside a quantitative survey aimed at certified seagoing officers. The purpose of the survey was to investigate how this navigational aid preferably should be used in collision avoidance, in contrast to how it is used in reality.

AIS, Automatic Identification System, is an information sharing system to assist in safe navigation by transmitting the own vessels navigational data while, at the same time, receiving corresponding information from other ships. The system is also utilized by land based stations such as VTS-centers and coast guards.

The investigation shows that experts conclude AIS to be, in many aspects, a good and useful aid which should be used in collaboration with other equipment when assessing risk of collision. Furthermore, the survey show that the ARPA-system, in spite of some internal shortcomings, is awarded more trust than AIS and AIS vectors are therefore rarely used. The survey also shows that a large number of officers are open to deviating from the COLREGs while the experts, in general, advise against it.

This Coastal and Hydraulics Engineering Technical Note (CHETN) describes a method for evaluating the received coverage from Automatic Identification System (AIS) shore sites and the availability of historic vessel position reports along the Ohio River. The network of AIS shoreside sites installed and operated by the US Army Corps of Engineers (USACE) and the US Coast Guard (USCG) receive information transmitted from vessels; however, reception of these transmissions is generally line-of-sight between the vessel and the AIS site antenna. Reception may also be affected by factors such as the quality of the transceiver installation aboard the vessel as well as the state of the equipment at the receiving site. Understanding how to define and quantify coverage gaps along the inland river system can inform research utilizing AIS data, provide information on the performance of the AIS network, and provide guidance for efforts to address identified coverage gaps

The purpose is to identify AIS coverage gaps in the current system. Reception of AIS transmissions between shore-based transceivers and vessels is generally line-of-sight between the vessel and the AIS site antenna. However, signal reception may be affected by factors such as the distance and terrain between the vessel and the transceiver site, quality of the transceiver installation, state of the equipment either aboard the vessel or at the shore transceiver station, and atmospheric phenomena. Quantifying coverage gaps along the inland waterways system can inform research that uses AIS data, provide information on the performance of the AIS network, and provide guidance for efforts to address coverage gaps to improve navigation safety. In autumn 2020, severe shoaling was occurring on the Missouri River. As the shoals were identified, the Kansas City District requested the LOMA system transmit AIS Aid to Navigation (AtoN) to mark the shoals in several critical areas. However, vessel pilots sometimes reported that they were not receiving the AIS AtoN being transmitted. At the request of the Kansas City District, the US Army Engineer Research and Development Center, Coastal and Hydraulics Laboratory (ERDC-CHL), conducted a coverage analysis using data collected from the LOMA AIS transceivers in the area to determine if there were coverage issues and their extent and to aid in determining the best means of addressing any coverage gaps.
