Abstract:
Volunteers are sought who will commit themselves to report the incidence of adverse effects in their acupuncture practice over a period of two years. The Department of Complementary Medicine of Exeter University is co-ordinating a survey of 30,000 acupuncture treatments. From the resulting data a complication rate of serious and minor side effects will be compiled. It is hoped that meticulous recording could demonstrate circumstances leading to adverse effects and identify unrecorded complications of acupuncture. Previous reports suggest that the major complications of acupuncture have an incidence of between 1:10,000 and 1:100,000, which is classified as very low. This survey aims to give an accurate assessment of the risk of major complication and to determine the range and incidence of any other unintended and non-therapeutic ill-effects following acupuncture, no matter how minor. It constitutes the first such investigation into the safety of acupuncture.

Acupuncture is increasingly used, so it is important to establish whether its benefits outweigh its risks. Numerous case reports of adverse events show that acupuncture is not free of risk, but accurate data from prospective investigations is scarce. A prospective survey was undertaken using intensive event monitoring. Forms were developed for reporting minor events each month and significant events as they occurred. The sample size was calculated to identify any adverse events that occurred more frequently than once in 10,000 consultations. Acupuncturists were recruited from two professional organisations in the UK. Seventy-eight acupuncturists, all doctors or physiotherapists, reported a total of 2178 events occurring in 31,822 consultations, an incidence of 684 per 10,000 consultations. The most common minor adverse events were bleeding, needling pain, and aggravation of symptoms; aggravation was followed by resolution of symptoms in 70% of cases. There were 43 significant minor adverse events reported, a rate of 14 per 10,000, of which 13 (30%) interfered with daily activities. One patient suffered a seizure (probably reflex anoxic) during acupuncture, but no adverse event was classified as serious. Avoidable events included forgotten patients, needles left in patients, cellulitis and moxa burns. In conclusion, the incidence of adverse events following acupuncture performed by doctors and physiotherapists can be classified as minimal; some avoidable events do occur. Acupuncture seems, in skilled hands, one of the safer forms of medical intervention.

This study focuses on the perceived quality of participatory event experiences by addressing the following question: What are the important aspects of the event experience? The aim of this research is to develop and refine a scale to measure the quality of the event experience for runners at a participatory event. The objective is to combine, apply, test, and refine the existing scales to increase our understanding of the perceived quality of events among amateur running athletes. Both affective and cognitive dimensions are included in the scale. Based on seven dimensions and 36 items, a formal scale development process is adopted. The data consist of 1,923 observations collected during a participatory event with approximately 60,000 registered participants. The seven-factor model, including immersion, surprise, participation, fun, social aspects, hedonic aspects, and service quality, was gradually revised in favor of a four-factor solution: service quality, hedonic aspects, fun, and immersion. As a result, 73.1% of the variance is extracted. This study contributes to a refined scale measuring the perceived event quality of participatory events. Service quality accounts for more than half of the variance extracted. Researchers should continue to develop research on the critical experiential dimensions in an event context. Furthermore, the links between the constructs need attention. The results suggest that event organizers should evaluate their events and event portfolios based on the scale and take actions to increase the perceived quality of these events.

Numerous case reports of adverse events of acupuncture have been published, mostly in journals which are not related to acupuncture. The authors usually have no training in acupuncture. In principle, case reports on adverse events are of value as an aid for learning. They can only achieve this effect if useful information is provided. In the case of acupuncture related adverse events and complications, basic information about the patient and the therapist as well as the treated acupuncture points and topographical regions and the needling technique are desirable. This article offers suggestions on what details should be included in a case report on adverse events related to acupuncture.

Objective To summarise the range and frequency of significant adverse events associated with acupuncture in order to provide evidence on which to base continuing efforts to improve the safety of acupuncture practice. Methods Searches were conducted of computerised databases, previous reviews of case reports, population surveys, prospective surveys of acupuncture practice and relevant sections of textbooks for primary and secondary reports to indicate the range of significant adverse events associated with acupuncture. Data from prospective surveys of acupuncture were combined to estimate the incidence of serious adverse events. Results A total of 715 adverse events was included. There were 90 primary reports of trauma, and 186 secondary reports; the most common were pneumothorax and injury to the central nervous system. Infection accounted for 204 primary reports and 91 secondary reports. Over 60% of these cases were hepatitis B. The next most common infection was of the external ear, as a complication of auricular acupuncture. The 144 miscellaneous events mainly comprised seizures and drowsiness judged severe enough to cause a traffic hazard. There were 12 primary reports of deaths. According to the evidence from 12 prospective studies which surveyed more than a million treatments, the risk of a serious adverse event with acupuncture is estimated to be 0.05 per 10 000 treatments, and 0.55 per 10 000 individual patients. Conclusions The risk of serious events occurring in association with acupuncture is very low, below that of many common medical treatments. The range of adverse events reported is wide and some events, specifically trauma and some episodes of infection, are likely to be avoidable.

Abstract. Double-layer structures in polar mesospheric clouds (PMCs) are observed by using Solar Occultation for Ice Experiment (SOFIE) data between 2007 and 2014. We find 816 and 301 events of double-layer structure with percentages of 10.32 and 7.25 % compared to total PMC events, and the mean distances between two peaks are 3.06 and 2.73 km for the Northern Hemisphere (NH) and Southern Hemisphere (SH) respectively. Double-layer PMCs almost always have less mean ice water content (IWC) than daily IWC during the core of the season, but they are close to each other at the beginning and the end. The result by averaging over all events shows that the particle concentration has obvious double peaks, while the particle radius exhibits an unexpected monotonic increase with decreasing altitude. By further analysis of the background temperature and water vapour residual profiles, we conclude that the lower layer is a reproduced one formed at the bottom of the upper layer. 56.00 and 47.51 % of all double-layer events for the NH and SH respectively have temperature enhancements larger than 2 K locating between their double peaks. The longitudinal anti-correlation between the gravity waves' (GWs') potential energies and occurrence frequencies of double-layer PMCs suggests that the double-layer PMCs tend to form in an environment where the GWs have weaker intensities.

Abstract:
The paper describes the type and frequency of adverse events and transient reactions following consultations with professional acupuncturists. In a postal survey, involving 1848 professional acupuncturists, all of whom were members of the British Acupuncture Council and practising in the UK, details of adverse events and transient reactions following treatment were recorded on standardised self-report forms. A sample size of 30,000 treatments was sought, and piloting indicated that a four-week period was required. Practitioners also provided information on themselves, including age, sex, length of training and years of practice. A total of 574 practitioners responded, 31% of the total population. These practitioners reported on adverse events and transient reactions associated with 34,407 treatments. No serious adverse events were reported, where these were defined as requiring hospital admission, prolonging hospital stays, permanently disabling, or resulting in death (95% CI: 0 to 1.1 per 10,000 treatments). A total of 43 significant minor adverse events were reported, a rate of 1.3 per 1,000 treatments (95% CI: 0.9 to 1.7). These included severe nausea and actual fainting (12), unexpected, severe and prolonged aggravation of symptoms (7), prolonged and unacceptable pain and bruising (5) and psychological and emotional reactions (4). There were three avoidable events: two patients had needles left in by mistake, and one patient had moxa burns to the skin, also caused by practitioner error. The acupuncturists also recorded 10,920 mild transient reactions occurring in 5136 treatments, 15% (95% CI: 14.6 to 15.3) of the 34,407 total. In terms of local reactions, there were reports of mild bruising (1.7%), pain (1.2%) and bleeding (0.4%). Practitioners reported that patients experienced an aggravation of existing symptoms after 2.8% of treatments. The most common mild transient reactions to treatment were feeling relaxed (11.9%) and feeling energised (6.6%). In this prospective survey of 34,407 treatments, practitioners reported no serious adverse events. This conclusion was based on data collected from one in three members of the British Acupuncture Council. Given that the whole membership delivers between one and a half and two million treatments a year, this is important evidence on public health and safety. When compared with medication routinely prescribed in primary care, the results suggest that acupuncture is a relatively safe treatment modality.

Background Most patients with obstructive sleep apnoea (OSA) do not tolerate treatment with nasal continuous positive airway pressure, the ‘gold standard’ treatment for this condition. It was shown in a pilot study that acupuncture was more effective than placebo treatment (sham acupuncture) in producing significant changes in the respiratory events assessed by polysomnography (PSG). Objectives To investigate the immediate effect of manual acupuncture (MA) and electroacupuncture (EA) on the sleep pattern of patients presenting with moderate OSA. Methods 40 patients with an Apnoea–Hypopnoea Index (AHI) of 15–30/h were randomly allocated to MA treatment (n=10), EA 10 Hz treatment (n=10), EA 2 Hz treatment (n=10) and a no-treatment control group (n=10). The patients received MA or EA (2 or 10 Hz) just before the PSG study at 20:00. Results The AHI (p=0.005; p=0.005), the Apnoea Index (p=0.038; p=0.009) and the respiratory events (p=0.039; p=0.014) decreased significantly in the MA and EA 10 Hz groups, respectively (AHI (21.9, 11.2), Apnoea Index (5.15, 0.7), respiratory events (120.5, 61.0) in the MA group before and after. AHI (20.6, 9.9), Apnoea Index (8.2, 0.3), respiratory events (117.0, 56.0) in the EA 10 Hz group before and after). The micro-arousals decreased only in the MA group (146.0 vs 88.5, p=0.0002). There were no significant changes in the EA 2 Hz group or in the control group. Conclusion A single session of either MA or EA 10 Hz had an acute effect in reducing the AHI as well as the number of nocturnal respiratory events of patients presenting with moderate OSA.

Considering the urban transformations and the development of the hotel sector in the City of Rio de Janeiro driven by the 2016 Olympic Games, this study aims to understand how the hotel industry behaves in terms of organic growth and investments during periods of mega international events. To this end, a qualitative case study research was carried out that included 7 different hotel brands whose results were coded and categorized resulting in a theoretical-conceptual framework responsible for identifying some competitive pressures on traditional hospitality, including 6 different challenges: (1) political institutional insecurity, (2) perceptions of the media, (3) public security, (4) other national and foreign locations, (5) new videoconference technologies, and (6) Online Travel Agencies. It was found that the exponential growth in the number of beds in the city in the years leading up to the 2016 Olympics generated several complications for the industry at that time. The logic of this expansion is discussed throughout the paper during different phases and the aim is to help fill a gap left by empirical research in the area, which has little explored the logic behind the option of investing during Olympic Cycles.

Abstract Research on news organizations’ handling of ‘what-a-story’s proposes that journalists find routines for handling these events based on their previous experiences of similar situations. Still, conceptual discussions on how to define extraordinary events or ’what-a-story’s have thus far attracted limited interest. In response, the present article proposes a definition of ‘what-a-story’s in order to provide an understanding of what events become a part of news organizations’ historical case banks. Accordingly, the aim of the article is to present a definition of crisis news events from an organizational perspective, which can help distinguish critical news events of importance to news organizations’ learning and preparedness. The article argues that crisis news are to be understood as surprise events that challenge key organizational values and demand a swift response. Based on interviews with Swedish broadcasting media managers, the article illustrates how the September 11th terror attacks can be defined as a crisis event.

Aim The aim of this article is to investigate the vascular safety profile of triptans through an analysis of the United States Food and Drug Administration Adverse Event Reporting System (FDA_AERS) database with a special focus on serious and unexpected adverse events. Methods A case/non-case analysis was performed on the reports entered in the FDA_AERS from 2004 to 2010: Cases were reports with at least one event included in the MedDRA system organ classes ‘Cardiac disorder’ or ‘Vascular disorders’, whereas non-cases were all the remaining reports. Co-reported cardiovascular drugs were used as a proxy of cardiovascular risk and the adjusted reporting odds ratio (adj.ROR) with 95% confidence intervals (95% CI) was calculated. Disproportionality signals were defined as adj.ROR value >1. Adverse events were considered unexpected if not mentioned on the relevant label. Results Among 2,131,688 reports, 7808 concerned triptans. Cases were 2593 among triptans and 665,940 for all other drugs. Unexpected disproportionality signals were found in the following high-level terms of the MedDRA hierarchy: ‘Cerebrovascular and spinal necrosis and vascular insufficiency’ (103 triptan cases), ‘Aneurysms and dissections non-site specific’ (15), ‘Pregnancy-associated hypertension’ (10), ‘Reproductive system necrosis and vascular insufficiency’ (3). Discussion Our analysis revealed three main groups of unexpected associations between triptans and serious vascular events: ischaemic cerebrovascular events, aneurysms and artery dissections, and pregnancy-related vascular events. A case-by-case assessment is needed to confirm or disprove their plausibility and large-scale analytical studies should be planned for risk rate estimation. In the meantime, clinicians should pay special attention to migraine diagnosis and vascular risk assessment before prescribing a triptan, also promptly reporting any unexpected event to pharmacovigilance systems.

Everyone wants safe medicine. The traditional approach to adverse events has developed within a culture of blaming the individual practitioner. Such an approach is likely to be damaging to individuals and possibly counterproductive by creating an atmosphere of defensiveness and denial. Industries such as airlines have developed an alternative culture using a systems approach. This approach concentrates on assessing and improving the systems of working rather than blaming an individual's performance. Frameworks have been developed for applying this approach to investigating and avoiding medical accidents. These form the basis of a check-list for acupuncture practice that is presented here, and may be useful for individuals and organisations who are concerned to reduce the risk of adverse events.

The aim was to identify adverse events arising from the continued use of atypical antipsychotics through cohort and case control studies, carried out on a worldwide level, in the period from 2013 and 2014. Method: A literature review was performed from an exploratory study seeking cohort and case control studies about atypical antipsychotics with bibliometrics application according to Bradford's Law. Results: The study resulted in 291 surveyed articles, of which 18 were selected and analyzed. Conclusion: the main findings were cardiovascular risk, QT interval prolongation, diabetes, increased body mass index, extrapyramidal effects.

The phenomenon of events can hardly be described as a new one. The aim of this paper is to determine the economic benefits of three major cultural events in South Africa to the host community. Measuring the economic impact normally entails some estimation of the cash injection into a region by visitors and applying the relevant multiplier to arrive at a monetary estimate of the economic impact. But few regions or municipal areas have detailed economic data to construct a type of input-output model and derive a multiplier. The purpose of the methods used in this research were firstly to determine the estimated cash injection, secondly to estimate the size of leakages in the local economy and thirdly to derive an appropriate multiplier to estimate the economic impact of the event.

An association between migraine and ischaemic vascular events, particularly ischaemic stroke, has been debated for many years. The pathophysiology of migraine has been explored in detail, and it is known that a dysfunction of brain cells and arteries is a major component of this disorder. The involvement of cerebral arteries during the migraine attack as well as the high prevalence of migraine among young individuals with ischaemic stroke has led to the hypothesis that migraine may be a risk factor for ischaemic stroke. Furthermore, there is evidence that the vascular nature of migraine is not limited to meningeal blood vessels and that migraine and overall cardiovascular disease may share aetiological pathways. The aim of this review is to summarize the epidemiological evidence that links migraine with ischaemic stroke and ischaemic heart disease and to discuss potential biological mechanisms.

Results In both cases described, unknown atrial communications existed resulting in foam emboli. One case involving the vertebral system resolved without treatment. The other involving the cerebral system was treated with hyperbaric oxygen. Conclusions Foam sclerotherapy can cause serious neurologic phenomenon even though the incidence is rarely described. Immediate treatment with 100% O2 and possible hyperbaric O2 therapy should be considered.

BackgroundThe aetiology of miscarriage is poorly understood. The aim of this study is to determine whether women admitted to hospital with miscarriage have experienced more psychosocial stress in early pregnancy than women whose pregnancies progress to labour.MethodInterviews of 48 case-control pairs, matched for known predictors of miscarriage risk, using the Life Events and Difficulties Schedule.ResultsThe miscarriage group were more likely to have experienced a “severe life event” in the three months preceding miscarriage (35% v. 15%; P < 005); more likely to have been experiencing a “major social difficulty” (31% v. 4%; P < 0.002); and more likely to have experienced “life events of severe short-term threat” in the fortnight immediately beforehand (31% v. 4%; P < 0.002). Fifty-four per cent of the miscarriage group had experienced at least one of the above indicators of psychosocial stress before miscarrying, compared with 15% of controls (P = 0.0001). Other factors significantly associated with miscarriage included childhood maternal separation; non-UK European origin; poorer reported relationships with partners, and fewer social contacts.ConclusionPsychosocial stress may be a factor in the aetiology of miscarriage. Alternatively, there may be a common determinant (e.g. personality, lifestyle or environmental) predisposing to both stress and miscarriage.

The aim of this study is to understand the relationship between the thematic choices of event management in an effort to create consumer experience as a form of event differentiation strategy. The variety of event themes at custom cultural events in Indonesia, represented by Indonesian Custom Show (ICS), Kustomfest and House of Customland activities, as research sites from August to November 2022, demonstrates the differentiation of event design in creating unique customer satisfaction. The thematic differences of the event are the main concern in this study as the implementation of a differentiation strategy. A descriptive qualitative research method was chosen to describe the thematic relations of the event with the implementation of a differentiation strategy through interviews, observation and document review. The results of the study show that the thematic variety of events greatly determines the differentiation strategy in event design. An important contribution from this research can be used as a reference for event organizers' managerial policies to design events, sponsorship decisions to carry out partnership strategies, and consumer decisions to process purchasing decisions.

The effects of different forms of predication have been insightfully (and almost exclusively) studied for 'simple' cases of predication, of which the 'presentational sentence' is maybe the paradigm instantiation. It is the aim of this paper to show that thc same kind of effects as well as in fact the same kind of structures are present at embedded levels in thematically and otherwise more complex structures. Beyond presentational sentences, 'unaccusative' experiencing constructions involving a dative subject, 'double object constructions' and - to a lesser extent - spraylload constructions are discussed. For all of these, it is argued that they comprise a predication encoding the ascription of a transient temporal property to a location. On this basis, a proposal is made as to how the scope asymmetry between the two arguments involved in the colistructions can be explained. Furthermore, a proposal is made as to how what has been called 'argument shift' is motivated.

Case reports are particularly valuable in specialist clinical areas such as acupuncture to report new adverse events and to suggest possible new hypotheses. They can also be used to report events that have been reported previously but are rare or serious, in order to illustrate their frequency. They may illuminate the wider side of clinical practice by describing personal experiences of one practitioner. Constraints to writing case reports include finding time, working in isolation, and not having enough experience at the task. This article reproduces and develops a set of guidelines that were previously published, in an attempt to help authors to write thorough but succinct reports in a structured manner. The format for case reports includes an abstract, description of the case, literature search, discussion and summary or conclusions. Recommendations are made for the material to be included in each section. The policy of Acupuncture in Medicine is to require patient consent before accepting a report for publication.

Background: Systemic therapies commonly used in adult psoriasis are mostly used only off-label in children and little is known about the efficacy and tolerability of these drugs in this population. In this study, we aimed to evaluate the efficacy and safety of systemic treatments in pediatric patients with psoriasis. Methods: Data were obtained retrospectively from the Department of Dermatology, Ondokuz Mayis University, School of Medicine between 2010–2019. Our study consisted of 742 pediatric patients (age ≤18 years) with psoriasis. Demographic data, adverse events of systemic treatments and healing periods were considered. Results: A total of 195 patients received systemic treatment. The mean age of onset of disease and the initiation of systemic therapy were 9.68±4.62 and 11.33±4.38 years, respectively. Patients received methotrexate (n=52, 26.67%), cyclosporine (n=18, 9.24%), acitretin (n=106, 54.35%) and others (biologics and/or one of conventional treatments) (n=19, 9.74%) as systemic therapy. Adverse events occurred in 12 patients (incidence of 6.15%, and its related 95% confidence interval of 2.75%, 9.56%) and nine of them had to discontinue the medication due to those adverse events. Healing periods calculated in the remaining 186 patients were 13.25±5.87, 10.85±5.67, 11.05±7.00, and 9.41±4.16 (mean±SD) weeks for acitretin, methotrexate, cyclosporine, and others, respectively. No statistically significant differences were noted between the treatments regarding the healing periods. Conclusion: All treatments were effective and none of them was superior in terms of the healing period. Systemic treatments used in adults can also be used in pediatric patients with psoriasis with similar efficacy and safety rates as long as routine monitoring is provided.

Spatiotemporal datasets based on player tracking are widely used in sports analytics research. Common research tasks often require the analysis of game events, such as passes, fouls, tackles, and shots on goal. However, spatiotemporal datasets usually do not include event information, which means it has to be reconstructed automatically. We propose a rule-based algorithm for identifying several basic types of events in soccer, including ball possession, successful and unsuccessful passes, and shots on goal. Our aim is to provide a simple procedure that can be used for practical soccer data analysis tasks, and also serve as a baseline model for algorithms based on more advanced approaches. The resulting algorithm is fast, easy to implement, achieves high accuracy on the datasets available to us, and can be used in similar scenarios without modification.

Proportional rates models are frequently used for the analysis of recurrent event data with multiple event categories. When some of the event categories are missing, a conventional approach is to either exclude the missing data for a complete-case analysis or employ a parametric model for the missing event type. It is well known that the complete-case analysis is inconsistent when the missingness depends on covariates, and the parametric approach may incur bias when the model is misspecified. In this paper, we aim to provide a more robust approach using a rate proportion method for the imputation of missing event types. We show that the log-odds of the event type can be written as a semiparametric generalized linear model, facilitating a theoretically justified estimation framework. Comprehensive simulation studies were conducted demonstrating the improved performance of the semiparametric method over parametric procedures. Multiple types of Pseudomonas aeruginosa infections of young cystic fibrosis patients were analyzed to demonstrate the feasibility of our proposed approach.

In order to assess how many reported adverse reactions to acupuncture are truly associated with the physiological mechanisms of acupuncture, we performed a literature review of published RCTs of acupuncture for osteoarthritis of the knee. We searched for reports of RCTs using two data sources: PubMed and Japana Centra Revuo Medicina (Igaku Chuo Zasshi). Of the twelve RCTs located, seven included information on adverse events. No serious adverse events were reported. Joint swelling, local inflammation, haematoma and back pain occurred more frequently in the dummy electroacupuncture or minimal acupuncture group. We confirmed the possibility that many adverse reactions to acupuncture treatment reported in RCTs, at least for the knee OA, are non-specific, and that not all reported events should be attributed to the mechanism of action of acupuncture. It is likely this is also true for RCTs of acupuncture in other conditions, and for prospective surveys on adverse events of acupuncture.

Using an event study analysis, we aim to investigate the impact of political, economic, social and terrorism events, on the Tunisian financial sector, over the period of the Tunisian Revolution; from (12)2010 to (04)2014. Based on a daily data analysis using three selected variables ; Sectoral index of performance of Tunisian banks ,Index of Tunisian stock market and the exchange rate Euro/ Dinar, the EGARCH model results have highlighted that general events decrease the return of our variables, and increase their volatility. More, results have shown that stock market is very sensitive to political and terrorism events, bad economic events increase the volatility of the exchange rate, and decrease the performance of banking sector. Political events remain the more important component, they affect negatively all the endogenous variables; coefficients in the mean equation show an important decline in term of the return of banking sector ,the stock market and the exchange rate.

Background Patients are attracted to acupuncture partly by its reputation for having low risks. The safety of acupuncture should be established by positive evidence. Methods Two prospective surveys were conducted among different groups of professionals in the UK, including doctors, physiotherapists and practitioners primarily trained in acupuncture. Participants monitored adverse events over a defined period of time, and reported minor and significant events on purpose designed forms. Results A total of 652 acupuncturists reported 6733 adverse reactions including tiredness in 66 229 patients, an adverse event rate of 10.2%. The most common events were tiredness (3%) bleeding or bruising (3%), aggravation of symptoms (2%) and pain at the needling site (1%). There were no serious adverse events. A total of 86 (0.1%) of the treatments was associated with an event that the practitioner judged to be significant though without persistent consequences for the patient's health. Conclusion The risks associated with acupuncture can be classified as negligible, and acupuncture is a very safe treatment in the hands of competent practitioners.

The overall aim of this paper is to discuss how Talmy’s (1985, 2000) typological framework and Slobin’s (1996) thinking for speaking hypothesis can be fruitful for the investigation of how adult language learners come to express motion events in an L2. We report an empirical study which compares the expression of the semantic components of Path and Manner of motion by three groups of informants: (a) learners whose L1 and L2 belong to different typological patterns (Danish learners of Spanish; (b) learners whose L1 and L2 share the same typological pattern (Italian learners of Spanish); and (c) Spanish native speakers. Based on previous research on L1 acquisition, it was hypothesized that the Danish learner group would exhibit a higher degree of elaboration of the two semantic components than the other informant groups. The results of the study, however, show a limited role for the L1 thinking for speaking patterns in advanced second language acquisition.

<span lang="EN-US">As people, we have no way of knowing whether our heart rate is considered normal or not. The strength and quality of our pulses will deteriorate as we get older. As a result, this may indicate a heart attack or another illness that requires immediate attention. The main goal of this paper is to define the heart rate stage using fuzzy logic systems (FLSs). In practice, however, designing or developing fuzzy logic systems is extremely difficult. To achieve this aim, we suggested a solution that involves: i) classifying the medical expert's criterion for signs of heart rate; ii) developing an explainable fuzzy logic system for heart rate measurement; and iii) evaluating the proposed system with human experts. In addition, the aim of this research was to provide an explainable fuzzy system that people could use to self-monitor heart rate levels and determine their health status. As a result, it is hoped that this research would provide insight into how to improve the development of fuzzy logic systems, especially in the field of medical applications.</span>

Objectives The aims of this study were to develop a sham cupping device and to validate its use as a placebo control for healthy volunteers. Methods A sham cupping device was developed by establishing a small hole to reduce the negative pressure after suction such that inner pressure could not be maintained in the cup. We enrolled 34 healthy participants to evaluate the validity of the sham cupping device as a placebo control. The participants were informed that they would receive either real or sham cupping and were asked which treatment they thought they had received. Other sensations and adverse events related to cupping therapy were investigated. Results 17 patients received real cupping therapy and 17 received sham cupping. The two groups felt similar sensations. There was a tendency for subjects to feel that real cupping created a stronger sensation than sham cupping (48.9±21.4 vs 33.3±20.3 on a 100mm visual analogue scale). There were only mild to moderate adverse events observed in both groups. Conclusion We developed a new sham cupping device that seems to provide a credible control for real cupping therapy by producing little or no negative pressure. This conclusion was supported by a pilot study, but more rigorous research is warranted regarding the use of this device.

Specifically, the aim of the study was to investigate to what extent experiences at work, both positive and negative, were shared with the family, depending on (a) characteristics of the event and (b) general characteristics of work and family life (e.g., chronic stressors, chronic resources in each domain). Results of multilevel analysis showed that the longer lasting (for positive events only) and less familiar (for positive and negative events) a work event was, the more intense the event was shared with family members. Furthermore, besides work events also general social support in the work and the family context predicted sharing such events with family members. That is, the more supportive family members were and the less supportive the work environment was in general, the stronger was the effect of sharing work events with the family. Combining qualitative and quantitative data was successful in uncovering the situational and general determinants of work–family processes.

In the renewable energy source (RES) market, which is highly atomized, business events play an important role in supporting inter-organizational cooperation. The aim of this paper was to examine the impact of three characteristics of the events, namely, the location of the event, type of event (brokerage event versus company mission), and type of leading partner (public versus private), on the effectiveness of the business events (expressed by the number of meetings during an event and the number of partnerships initiated as a result of these meetings). We analyzed 304 promotional events organized in the European RES market. We employed basic statistical tests to verify our hypotheses. Our findings suggest that (1) the numbers of meetings and partnerships differ depending on the event’s location (country and region of Europe), (2) the type of the event (brokerage event versus company mission) affects the numbers of meetings and partnerships (in particular, during brokerage events (BEs), significantly more meetings take place than during company meetings (CMs)), and (3) the type of an event’s leading partner (public versus private) has no impact on the numbers of meetings and partnerships. With its findings, this paper contributes to RES market development and the theory of inter-organizational relations. It presents meaningful implications and recommendations for enterprises, business networks, and event managers operating in RES markets.

According to this review, it is concluded that in sporting events it is necessary to have the interest groups from the moment of planning the event, generate initiatives to avoid environmental damage in the construction of stages, seek economic sustainability, and give conscious use to the sports infrastructure built and leave a long-term positive legacy. El impacto de los eventos deportivos está relacionado con aspectos sociales, económicos, ambientales y políticos que inciden en el desarrollo de las comunidades por medio de la integración social. El objetivo del estudio fue examinar el estado actual de las investigaciones sobre los eventos deportivos y la responsabilidad social corporativa. El diseño de investigación es una revisión sistemática de la literatura científica (SLR) de 268 documentos en las bases de datos Scopus, SportDiscus, Proquest y Dialnet, posterior al proceso de filtrado se seleccionaron 26 artículos que cumplieron con los criterios de inclusión. La principal conclusión de este estudio es que en los eventos deportivos se debe contar con los grupos de interés desde el momento de planificación del evento, generar iniciativas para evitar daños ambientales en la construcción de escenarios, buscar sostenibilidad económica, dar uso consiente a la infraestructura deportiva construida y dejar un legado positivo a largo plazo.

Background: - Physicians are confronted on having ECG in patients with acute stroke as it can mimic that of myocardial infarction or ischaemia. Repolarization and ischemic-like electrocardiographic (ECG) changes observed during acute phase of stroke may cause diagnostic and management dilemmas for the physicians. Aim:- The aim and objective of this study to identify the prevalence of ECG changes in acute stroke patients admitted to medicine units of Dhaka Medical College Hospital. Materials and Method: - This 6-month period observational study was carried out among 100 of acute stroke patients admitted in different medicine wards of Dhaka Medical College Hospital (DMCH). ECG was done in all patients after their admission to hospital within 48 hours of developing their symptoms. Association of various types of ECG changes were identified and observed. Results: - Among 100 patients,55% had ischaemic stroke and 45% had haemorrhagic stroke. 43.63% aged between 61-70 years had ischaemic stroke and 28.28% aged between 61-80 years had haemorrhagic stroke. Female had higher frequency of ischaemic stroke 52.72% and male had higher frequency of haemorrhagic stroke 75.56%. Abnormal ECG found 84.44% in haemorrhagic stroke and 54.54% in ischaemic stroke. Of all abnormal ECG, ST depression is most frequent and 42.22% in haemorrhagic stroke, T inversion is next 20% in ischaemic stroke, AF is present in 18.18% in ischaemic stroke and QT prolongation is found in 17.77% in hemorrhagic stroke. There considerable variation of ECG changes according to CT scan evidence of particular area of brain involvement like ST depression (28.88%) found in gangliothalamic bleed in haemorrhagic stroke, AF is found (16.36%) in insular area involvement in ischaemic stroke, T inversion is more (12.72%) in large MCA territory infarctive stroke and QT prolongation (11.11%) is found in intracerebral haemorrhage including SAH. Predictable early in hospital mortality is associated with AF 37.5% and with QT prolongation 31.25%. Conclusion:- In haemorrhagic stroke the ECG abnormalities were more frequent then in ischaemic stroke. The most common abnormalities were ST depression, T wave inversion, AF and QT prolongation. SynopsisUntil recently, life events were considered as chance occurrences. However, there is now increasing evidence that reported life events, at least in adult life are not random. Life events not only tend to cluster in families but also appear to be influenced by genetic factors. The aim of this study was to investigate whether genetic factors also influence reported life events in childhood using a systematically ascertained sample of 376 twin pairs aged 8 to 17. Overall, reported life events in this younger population were found to be heritable. However, the degree of genetic and environmental influence appeared to vary across the sexes, at least for parent-rated life events and according to whether life events were rated by the parents or the children themselves. Genetic influences appeared to be more important for self reports, suggesting that individual differences in cognition play a role in reporting life events.

Aim: The aim of this study was to assess the association between PEAR1 polymorphisms and ischemic clinical outcomes. Materials & methods: We searched the electronic database for articles on the relationship of PEAR1 SNPs and ischemic events in patients with coronary artery disease (CAD) up to October 2020. Results: A total of 9914 patients with CAD from six studies focusing on 12 SNPs of PEAR1 were included in this study. The A allele of rs12041331 were associated with ischemic events (odds ratio: 1.40; 95% CI: 1.04–1.88; p = 0.03). The AA homozygotes of rs2768759 was related to a higher risk of ischemic events than carriers of the C allele (odds ratio: 2.08; 95% CI: 1.09–3.97; p = 0.03). Conclusion: PEAR1 rs12041331 and rs2768759 are significantly associated with ischemic events in patients with CAD.

SenseCam is an effective memory-aid device that can automatically record images and other data from the wearer's whole day. The main issue is that, while SenseCam produces a sizeable collection of images over the time period, the vast quantity of captured data contains a large percentage of routine events, which are of little interest to review. In this article, the aim is to detect "Significant Events" for the wearers. We use several time series analysis methods such as Detrended Fluctuation Analysis (DFA), Eigenvalue dynamics and Wavelet Correlations to analyse the multiple time series generated by the SenseCam. We show that Detrended Fluctuation Analysis exposes a strong long-range correlation relationship in SenseCam collections. Maximum Overlap Discrete Wavelet Transform (MODWT) was used to calculate equal-time Correlation Matrices over different time scales and then explore the granularity of the largest eigenvalue and changes of the ratio of the sub-dominant eigenvalue spectrum dynamics over sliding time windows. By examination of the eigenspectrum, we show that these approaches enable detection of major events in the time SenseCam recording, with MODWT also providing useful insight on details of major events. We suggest that some wavelet scales (e.g., 8 minutes–16 minutes) have the potential to identify distinct events or activities.

Abstract:
Collaborative drug therapy management in primary health care involves communication among the physician, pharmacist and user of simvastatin and can result in safer results regarding patient wellbeing. The aim of the study was to investigate muscle adverse events and risk factors related to simvastatin. For patients who developed muscle adverse events, collaborative drug therapy management was performed in an attempt to resolve the symptoms. A non-randomized case study was conducted at the single basic health unit in the city of Peabiru, Parana, Brazil, for a period of one year. Patients were interviewed using a structured form. To confirm muscle adverse events, the patient was referred to a physician and submitted to the suspension and return to treatment. Thyroid-stimulating hormone, creatine kinase and alanine aminotransferase exams were performed. A sample of 148 users of simvastatin was selected. Eleven patients had some type of simvastatin-associated muscle adverse event (myopathy), among whom seven had muscle symptoms (myalgia) and four had elevated creatine kinase, but were asymptomatic (asymptomatic myopathy). Collaborative drug therapy management focused on simvastatin for five patients with myalgia led to improvements in the quality of life of two patients. EVENTOS ADVERSOS MUSCULARES ENTRE USUÁRIOS DE SINVASTATINA NA ATENÇÃO PRIMÁRIA EM SAÚDE: MANEJO COLABORATIVO DE TERAPIA MEDICAMENTOSA Resumo: O manejo colaborativo de tratamento medicamentoso em atenção primária envolve a comunicação entre o médico, farmacêutico e o usuário de sinvastatina e pode levar a resultados mais seguros, favorecendo o bem-estar do paciente. O objetivo do estudo foi investigar eventos adversos musculares e fatores de risco para tais eventos, relacionados à sinvastatina. Para os pacientes que desenvolveram eventos adversos musculares, o manejo colaborativo foi realizada de forma a resolver os sintomas. Um estudo de caso não randomizado foi realizado na única unidade básica de saúde na cidade de Peabiru, Paraná, Brasil, por um período de um ano. Os pacientes foram entrevistados por meio de um formulário estruturado. Para confirmar os eventos adversos musculares, o paciente era encaminhado ao médico, sendo submetido à suspensão e retorno da sinvastatina. Foram realizadas dosagens do hormônio tireoestimulante, creatina quinase e alanina aminotransferase. Uma amostra de 148 usuários de simvastatina foi selecionada. Do grupo estudado, 11 pacientes tiveram algum tipo de evento adverso muscular (miopatia) associada à sinvastatina, entre os quais sete tiveram sintomas musculares (mialgia) e quatro apresentaram elevação da creatina quinase, mas eram assintomáticos (miopatia assintomática). O manejo colaborativo de terapia medicamentosa focada na sinvastatina para cinco pacientes com mialgia levou a melhoria na qualidade de vida de dois pacientes.
