Abstract:
Heat pumps (HPs) are being developed with a new emphasis on cold climates. To lower the environmental impact of greenhouse gas (GHG) emissions, alternate low global warming potential (GWP) refrigerants must also replace the exclusive use of the refrigerant R410A, preferably without re-engineering the mechanical hardware. In this paper, we analyze the performance of four low-GWP alternative refrigerants (R32, R452B, R454B, and R466A) relative to the conventional R410A and draw conclusions on the relative performances for providing heating in cold climates based on the Air-Conditioning, Heating, and Refrigeration Institute (AHRI) 210/240 standard for two-speed heat pumps. The simulations are carried using the Department of Energy, Oak Ridge National Laboratory (DOE/ORNL) Heat Pump Design Model (HPDM), a well-known heating, ventilation, and air conditioning (HVAC) modeling and design tool in the public domain and the HVAC research and development community. The results of the simulation are further scrutinized using exergy analysis to identify sources of systemic inefficiency, the root cause of lost work. This rigorous approach provides an exhaustive analysis of alternate low-GWP refrigerants to replace R410A using available compressors and system components, without compromising performance.

Abstract:
An air conditioning system using water as refrigerant (R718) that compresses water vapor with multistage stage variable speed axial compressor with intercooling between stages by water injection is considered. Four stage compression with flash intercooling resulted in 50% improvement of coefficient of performance (COP) at full load compared to conventional refrigerants like R134a. The energy efficiency of an air conditioning unit is specified by seasonal energy efficiency ratio (SEER). SEER is defined as the ratio of cooling output of an air conditioner measured and electrical energy consumption as per AHRI 210/240 during cooling season. The SEER is computed after determining the evaporator cooling capacity and the electrical energy demand of the compressor at each bin temperature using assumed compressor isentropic efficiency, mechanical efficiency and electrical efficiency and multiplying by the weight of each bin temperature to determine the total for the cooling season. As a result of multistage compression, best part load performance of water as a refrigerant and operation of compressor near design point at part load due to variable speed drive, 50%–60% improvement in SEER is obtained compared to the best available in the market using conventional refrigerants such as R134a with single stage compression.

Abstract:
A valid diet quality assessment scale has not been investigated in hemodialysis patients. We aimed to adapt and validate the alternative healthy eating index in hemodialysis patients (AHEI-HD), and investigate its associations with all-cause mortality. A prospective study was conducted on 370 hemodialysis patients from seven hospital-based dialysis centers. Dietary data (using three independent 24-hour dietary records), clinical and laboratory parameters were collected. The construct and criterion validity of original AHEI-2010 with 11 items and the AHEI-HD with 16 items were examined. Both scales showed reasonable item-scale correlations and satisfactory discriminant validity. The AHEI-HD demonstrated a weaker correlation with energy intake compared with AHEI-2010. Principle component analysis yielded the plateau scree plot line in AHEI-HD but not in AHEI-2010. In comparison with patients in lowest diet quality (tertile 1), those in highest diet quality (tertile 3) had significantly lower risk for death, with a hazard ratio (HR) and 95% confidence intervals (95%CI) of HR: 0.40; 95%CI: 0.18 – 0.90; p = 0.028, as measured by AHEI-2010, and HR: 0.37; 95%CI: 0.17–0.82; p = 0.014 as measured by AHEI-HD, respectively. In conclusion, AHEI-HD was shown to have greater advantages than AHEI-2010. AHEI-HD was suggested for assessments of diet quality in hemodialysis patients.

Animasi 2D berbasis Motion Graphic dapat digunakan untuk mendukung proses mengajar yang lebih efektif, fleksibel, menarik serta mudah dipahami. Maka dari itu, dengan dibuatnya penelitian tentang Pembuatan Media Pembelajaran Animasi 2D berbasis Motion Graphic pada mata kuliah Praktikum Fotografi Dasar Materi Pengenalan Kamera, Teknik Exposure, dan Point of Interest ini, dapat membuat proses belajar mengajar menjadi lebih menarik, fleksibel dan mudah untuk dipahami mahasiswa. Pada peneilitan ini menggunakan metode Multimedia Development Life Cycle yang mana metode ini terbagi menjadi 6 tahapan. Teknik pengumpulan data pada penelitian ini dilakukan dengan wawancara, studi pustaka dan kuesioner. Kuesioner disebar kepada responden ahli dan pengguna, responden ahli merupakan seorang ahli di bidang animasi dan multimedia sedangkan responden pengguna merupakan mahasiswa D4 Jurusan Teknik Komputer Politeknik Negeri Sriwijaya. Analisis data pada penelitian ini dilakukan dengan menggunakan skala likert. Hasil dari penelitian ini diketahui nilai kelayakan animasi pada pengujian ahli yang terdiri dari 8 orang ahli diperoleh persentase sebesar 80,62% dengan kategori “Sangat Baik” dan pada pengujian pengguna yang terdiri dari 50 orang mendapatkan persentase sebesar 88,2% dengan kategori “Sangat Baik”. Abstract:
Purpose Alternative Healthy Eating Index (AHEI)-2010, an index that determines the quality of a diet, was created to predict the risk of chronic diseases. Nevertheless, it is unclear whether this index can predict the risk of prostate cancer, one of the most prevalent cancers among men around the world. The study aims to investigate the association between adherence to the AHEI-2010 and the risk of prostate cancer in Iranian men. Design/methodology/approach The case–control study was conducted in Kermanshah, Iran in the year 2016. The study included 50 cases of Iranian men with prostate cancer and 150 healthy controls. Anthropometric indices were measured by bioelectric impedance analysis (BIA). The AHEI-2010 included 11 food components that were assessed by using a 147-item food frequency questionnaire. A multivariate logistic regression was performed to analyze the association of the AHEI-2010 (expressed as a dichotomous variable) with prostate cancer. Findings As per the analysis, there were no significant differences in age, body mass index (BMI) and waist-to-hip ratio (WHR) between case and control groups, statistically. After adjustment for potential confounders, the higher AHEI-2010 scores were associated with a lower risk of prostate cancer (OR AHEI > 55 vs ≤ 55 = 0.26; 95% CI: 0.11-0.63). Moreover, the Mean of AHEI scores was higher in controls than in the cases (p < 0.001). Originality/value The authors’ findings suggest that adherence to the dietary patterns with high scores of AHEI-2010 is associated with a reduced risk of prostate cancer, so this index may be used as an effective measure to predict prostate cancer.

Abstract:
Most worldwide causes of disease and death are strongly associated with dietary factors and the application of eating indexes has proved to be a useful tool to determine diet quality in populations. The aim of this study was to evaluate the diet quality in Chile through the application of the Alternate Healthy Eating Index 2010 (AHEI-2010). A representative sample (n = 879) of Chilean urban population aged 15–65 years old from the Latin American Study of Nutrition and Health (Estudio Latinoamericano de Nutrición y Salud; ELANS) was used. Dietary intake data were obtained through two 24-hour food recalls and one beverage frequency questionnaire, which were used to calculate AHEI-2010 and its association with sociodemographic and anthropometric variables. In this Chilean sample, the AHEI-2010 score was 43.7 ± 7.8 points (mean ± SD). Trans fats and sodium intake were the highest scoring AHEI-2010 components whereas sugar-sweetened beverages and whole grains had the lowest score. Women, older subjects, and individuals in medium-high socioeconomic levels had significantly higher mean AHEI-2010 scores. No association was found between AHEI-2010 and body mass index (BMI), or nutritional status. Conclusions: Diet quality in the Chilean urban population aged 15–65 years old is far from optimal. Thus, there is room for significant improvement of diet quality in Chile through design and implementation of public health policies, particularly in high-risk groups for chronic diseases.

Background: The compliance of Alternate Healthy Eating Index (AHEI) 2010 influences their risk of complications in type two diabetes mellitus (T2DM). In Indonesia, AHEI-2010 has not been widely used in evaluating the diet quality in people after diagnosed T2DM.Objectives: To analyze changes in diet quality of adults patients with T2DM. Methods: This study was analyzed using a secondary data from “Cohort Study of Non-Communicable Diseases Risk Factors” by Indonesian Ministry of Health, on 105 adults newly diagnosed with T2DM. The diagnosis of T2DM was assessed based on the results laboratory tests of fasting blood glucose (FBG) ≥126 mg/dL and 2-hours post-75-g glucose load (2h-PG) ≥200 mg/dL. Dietary intake data was collected twice (at the beginning and the end of monitoring) using a 24-hour recall. The assessment of diet quality uses modified AHEI-2010 USA according to the Indonesians Dietary Guidelines, especially in the portion of the food components. Results: The total score for diet quality was higher at the beginning of monitoring 54.9 than the end of monitoring 53.3 and there was no statistical significance differences (p≥0.05). The total score from diet quality decreased 1.1 points, 53.4% of subjects showed score deterioration (deteriorating diet quality) and 46.7% showed score improvement (improved diet quality). There was a significant differences at the beginning and the end of monitoring only to components score of red/processed meat (p <0.05).Conclusions: Changes in diet quality that deteriorating over time during monitoring, characterized by a decrease in the total score for diet quality.

Osteoarthritis (OA) is a leading cause of immobility in the United States and is associated with older age, inflammation, and obesity. Prudent dietary patterns have been associated with disease prevention, yet little evidence exists describing diet quality (DQ) in older overweight or obese African American (AA) adults with OA and its relation to body composition. We conducted a secondary data analysis of a dataset containing alternate Healthy Eating Index-2010 (AHEI-2010), body composition, OA severity, and serum interleukin-6 (IL-6) data from 126 AA females (aged 60–87 years) with OA to examine the relationships between these variables. Our sample had poor DQ and reported having higher OA severity as measured by the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Interleukin-6 was negatively correlated with AHEI-2010, and AHEI-2010 and the WOMAC physical function subcategory (WOMACpf) were significant predictors of IL-6 (odds ratio (OR): 0.95, 95% confidence interval (CI) 0.92–0.99 and 1.04, 95% CI 1.01–1.07, respectively, p < 0.05) but not body composition. In conclusion, AHEI-2010 and WOMACpf were significant predictors of inflammation (IL-6) and AHEI-2010 accounted for ~16% of the variation of IL-6 (inflammation) in this sample.

Abstract:
IntroductionPsychological disorders have a major role in the incidence of chronic diseases and may result in reductions in the cost-effectiveness of the Armed Forces. Previous civilian studies have shown a protective association between healthy eating guidelines and mental disorders, but evidence to support this for a military population is limited. The aim of this study was to examine the association of Alternative Healthy Eating Index (AHEI-2010) with depression, stress and anxiety among Iranian military personnel.MethodA cross-sectional study was conducted on 246 male Army soldiers. Stress, anxiety, depression and dietary intakes were assessed. The association between variables was determined using multivariate logistic regression.ResultsThe prevalence of depression, stress and anxiety in study participants was 15.9%, 10.6% and 27.2% respectively. Participants with the highest adherence to the AHEI-2010 had an 80% lower odds of depression than those with the lowest adherence (OR: 0.20; 95% CI 0.04 to 0.78). Such an association was also found between adherences to the AHEI-2010 and anxiety (OR: 0.28; 95% CI 0.05 to 0.95). No significant association between adherence to the AHEI-2010 and stress was found.ConclusionAn inverse association between adherence to the AHEI-2010 and odds of depression and anxiety was found. Further studies are required to clarify this relationship.

ABSTRACT Background Diet quality may be an important area of focus for promoting cognitive health; however, the association between diet quality and cognitive function among Hispanics/Latinos remains largely unexamined. We hypothesized that a healthier diet quality will be associated with better cognitive function in middle-aged and older Hispanics/Latinos. Objective The objective of this study was to examine associations between the Alternate Healthy Eating Index (AHEI-2010), a measure of diet quality, and cognitive function in middle-aged and older Hispanics/Latinos. Methods Data from the Hispanic Community Health Study/Study of Latinos (HCHS/SOL) Visit 1 (2008–2011) were used (n = 8461; ages 45–74 y). Cognitive function was assessed with tests of verbal learning and memory, verbal fluency, and processing speed; a global cognition score was derived by summing the z scores of individual tests. Dietary intake was assessed via two 24-h recalls. Total AHEI-2010 score was categorized into quintiles (higher quintiles indicating healthier diet). Linear regression models were used to examine associations between AHEI-2010 quintiles and cognitive function adjusting for sociodemographic characteristics, daily energy intake, type 2 diabetes, smoking, and depressive symptoms. Results Compared with the lowest quintile, in the second to fourth AHEI-2010 quintiles, global cognition scores were significantly higher by 0.28, 0.52, and 0.48 units (P-trend = 0.042). In the second to fifth AHEI-2010 quintiles, verbal learning scores were significantly higher by 0.60, 0.62, 0.92, and 0.88 units, and verbal memory scores were higher by 0.33, 0.40, 0.52, and 0.46 units (P-trend = 0.020 and 0.007, respectively). No associations were observed between the AHEI-2010 and verbal fluency or processing speed (P-trend = 0.49 and 0.84, respectively). Among AHEI-2010 components, adequate consumption of vegetables, alcohol, and whole fruits were each associated with better cognitive function. Conclusions An overall healthier diet quality was associated with better global cognition, verbal learning, and verbal memory in middle-aged and older Hispanics/Latinos.

Abstract:
This study aimed to investigate the association between the alternative healthy eating index-2010 (AHEI-2010) with cognitive function and the mediating role of depressive symptoms in older adults using the data from the 2011–2014 National Health and Nutrition Examination Survey (NHANES). The AHEI-2010 was calculated from NHANES individual food data and Food Patterns Equivalents Database (FPED) diet data. Cognitive function was assessed by the Consortium to Establish a Registry for Alzheimer’s disease (CERAD, memory function indicator), Word Learning sub-test, digital symbol substitution test (DSST, processing speed indicator), animal fluency test (AFT, executive function indicator), and the comprehensive z-score (global cognition indicator). A weighted multiple linear regression model was used to explore the relationship between AHEI-2010 and cognitive function, and Karlson–Holm–Breen (KHB) method was used to explore the mediating effect of depressive symptoms. A total of 2644 participants were included in this study. Participants with higher AHEI-2010 scores were more likely to have higher scores in DSST, AFT, and comprehensive z-score compared with the lowest quartile. Depressive symptoms play a significant mediating role between AHEI-2010 and cognitive function. The proportion of mediating in CERAD, DSST, AFT, and comprehensive z-score was 14.14%, 9.10%, 9.15%, and 10.47%, respectively. This study found that higher dietary quality was associated with better cognitive function. In addition, depressive symptoms may be an important pathway linking diet and cognitive function.

Abstract Objectives Cystatin C (Cys C) is a promising biomarker for early-stage chronic kidney disease. Dietary intake plays an essential role in the prevention of kidney function decline, which has yet to be examined in relation to changes in Cys C among older adults. Our objective was to test whether scores on the Alternative Healthy Eating Index-2010 (AHEI-2010) were associated with change in Cys C from 2012–2016 and if this varied by race/ethnicity. Methods Observations were drawn from the Health and Retirement Study. Blood spot collection and examination occurred in 2012 and 2016, participant attributes were measured in 2012, and dietary assessment was conducted by a validated dietary frequency questionnaire in 2013. The sample was restricted to respondents aged ≥65 years who were White (n = 789), Black (n = 108), or Hispanic (n = 61) and had biomarkers measured in 2012 and 2016 (n = 958). Serum Cys C (mg/L) was constructed to be equivalent to the 1999–2002 NHANES scale. Dietary quality was measured using AHEI-2010. Autoregressive linear modeling adjusting for covariates and sampling design was used to examine the associations of interest. Results Mean serum Cys C was 1.20 ± 0.44 mg/L (SD) in 2012 and 1.25 ± 0.45 mg/L in 2016, and mean AHEI-2010 score was 58.11 ± 11.0. Greater AHEI-2010 was associated with lower serum Cys C level at baseline (b = −.004, SE = .013, P = .002) and less rapid increase in the Cys C level from 2012–2016 (b = −.003, SE = .012, P = .024). The association between AHEI-2010 and change in serum Cys C was significantly different for Whites and Hispanics (b = .128, SE = .031, P &lt; .001), but null when comparing Whites and Blacks. AHEI-2010 was negatively associated with change in Cys C for Whites, and positively associated with change in Cys C among Hispanics. Stratified analyses suggested that AHEI-2010 was not significantly different for Whites and Hispanics. Hispanics had significantly lower household income, assets, and educational attainment than Whites, and greater levels of food insecurity. Conclusions Our results indicate that dietary quality has a divergent association with change in serum Cys C for White and Hispanic older adults. These results suggest the need for examination of how disparities in socioeconomic status may influence the effect of dietary intake on kidney function for older adults from different racial/ethnic backgrounds. AbstractWe aimed to examine the association between the Alternative Healthy Eating Index updated in 2010 (AHEI-2010), the Dietary Inflammatory Index (DIITM) and risk of mortality in the Whitehall II study. We also conducted a meta-analysis on the DII-based results from previous studies to summarise the overall evidence. Data on dietary behaviour assessed by self-administered repeated FFQ and on mortality status were available for 7627 participants from the Whitehall II cohort. Cox proportional hazards regression models were performed to assess the association between cumulative average of AHEI-2010 and DII scores and mortality risk. During 22 years of follow-up, 1001 participants died (450 from cancer, 264 from CVD). Both AHEI-2010 (mean=48·7 (sd10·0)) and DII (mean=0·37 (sd1·41)) were associated with all-cause mortality. The fully adjusted hazard ratio (HR) persd, were 0·82; 95 % CI 0·76, 0·88 for AHEI-2010 and 1·18; 95 % CI 1·08, 1·29 for DII. Significant associations were also observed with cardiovascular and cancer mortality risk. For DII, a meta-analysis (using fixed effects) from this and four previous studies showed a positive association of DII score with all-cause (HR=1·04; 95 % CI 1·03, 1·05, 28 891deaths), cardiovascular (HR=1·05; 95 % CI 1·03, 1·07, 10 424 deaths) and cancer mortality (HR=1·05; 95 % CI 1·03, 1·07,n8269).The present study confirms the validity to assess overall diet through AHEI-2010 and DII in the Whitehall II cohort and highlights the importance of considering diet indices related to inflammation when evaluating all-cause, cardiovascular and cancer mortality risk.

Abstract Objectives The emergence of food insecurity as a primary nutrition-related health issue among older adults suggests a need to examine how nutritional assistance programs are related to food insecurity and dietary quality in aging populations. This project examines food insecurity and dietary quality in US adults age 65 and older and the impact of nutrition assistance programs. Methods The sample was drawn from the 2012 Health and Retirement Study and 2013 Health Care and Nutrition Study and included 3779 respondents representing a population of 37,217,566 adults aged 65 and older. Food insecurity was a binary measure based on the USDA six-item US Adult Food Security Survey Module. Two forms of nutritional assistance included receipt of supplemental food from sources such as food banks and Meals-on-Wheels (1 = yes, 0 = no) and reported receipt of SNAP benefits (1 = yes, 0 = no). Dietary quality was measured using the Alternative Healthy Eating Index-2010 based on a food frequency questionnaire. General linear modeling adjusting for covariates and complex sampling design was used to test if nutritional assistance moderated the association between food insecurity and AHEI-2010. Results Around 10% of the sample was food insecure, 14% reported receipt of supplemental food, and 6.4% were SNAP benefit recipients. In covariate-adjusted models, food insecurity and receipt of SNAP benefits were not associated with AHEI-2010, but receipt of supplemental food was (b = −1.39, SE = 0.67, P = 0.038). Receipt of supplemental food moderated the association between AHEI-2010 and food insecurity (P = 0.001). Simple effect estimates suggested that among those not receiving supplemental food, the food insecure had lower AHEI-2010 scores than the food secure (b = −2.15, SE = 0.88, P = 0.014). Among those receiving supplemental food, the food insecure had greater AHEI-2010 scores than the food secure (b = 2.62, SE = 1.25, P = 0.035) and similar AHEI-2010 scores as the food secure not receiving supplemental food. Conclusions Preliminary analysis suggests that receipt of supplemental food appears to be associated with better dietary quality among food-insecure older adults and confirms the importance of food assistance programs in combating the negative effect of food insecurity on dietary quality. Associations between asthma and the nutritional quality of diet remain poorly understood. We investigated the associations between overall quality of diet evaluated by three dietary scores (the Alternate Healthy Eating Index 2010 (AHEI-2010), the literature-based adherence score to Mediterranean diet (MEDI-LITE) and the modified Programme National Nutrition Santé Guideline Score (mPNNS-GS)) and the asthma symptom score and asthma control.In 2017, 34 766 participants from the NutriNet-Santé cohort answered a detailed respiratory questionnaire. Asthma was defined by the asthma symptom score (as the sum of five questions) and asthma control by the asthma control test (ACT).Roughly 25% of participants reported at least one asthma symptom and for the three dietary scores we observed a statistically significant negative association between a healthier diet and the asthma symptom score (for AHEI-2010, the adjusted OR was 0.79 (95% CI 0.75–0.84) for women and 0.67 (95% CI 0.60–0.75) for men). Among participants with asthma (n=2609) we also observed a negative association between a healthier diet and poorly controlled asthma. This was significant in men (OR 0.39 (95% CI 0.18–0.84) for the AHEI-2010) and borderline significant in women (OR 0.73 (95% CI 0.53–1.01) for the AHEI-2010; p=0.06 for trend).Healthier diet behaviours were associated with fewer asthma symptoms and greater asthma control.

Abstract Objectives Examine the impact of a multiple-goal (MG) vs single-goal (SG) lifestyle interventions on gestational diet quality and hyper-palatable food consumption. Methods Pregnant women were randomized in two pilot trials to MG (n = 21) or SG (n = 16) vs usual care (UC; combined trials, n = 18). The 18-week MG intervention included lifestyle modifications, and the 12-week SG intervention focused only on increasing fiber (≥30 g/day). Interventions included weekly instruction by a registered dietitian, and primary aims were appropriate weight gain. Diet recalls were performed at baseline and study end. Diet quality was scored with three versions of the alternative healthy eating index, including adaptations for pregnancy (AHEI 2010, AHEI-P, and AHEI-P 2010). Foods were classified as hyper-palatable based on fat, carbohydrate, sugar, and sodium content using previously defined criteria, and reported as percentage of total calories (%HPF). One-way ANOVA, paired t-tests, and Pearson's correlations were performed. Results At baseline, no between group differences for diet quality were found. By the end of the study, all diet quality scores increased for SG (P &lt; 0.009) and two increased for MG (AHEI 2010 & AHEI-P-2010, P &lt; 0.041). UC diet quality remained unchanged (P &gt; 0.05). At baseline, %HPF was different between groups (MG &gt; UC &gt; SG, 82%, 57%, and 26%, respectively; P &lt; 0.001). By study end, UC and SG increased %HPF (P &lt; 0.033), while MG remained unchanged (P &gt; 0.05), leaving MG and SG similar in %HPF (80% & 74%, respectively) and higher than UC (65%, P = 0.024). The %HPF was negatively correlated with all diet quality scores at baseline (r = −0.37 to −0.42; P &lt; 0.005) and two at the end of study (AHEI 2010 & AHEI-P 2010 r = −0.39, −0.30, respectively, P &lt; 0.040). Changes in diet quality and %HPF were not related (P &gt; 0.05). Conclusions Interventions aimed to prevent excessive gestational weight gain showed improvements in diet quality. Hyper-palatable food consumption was negatively related to diet quality, yet neither intervention decreased %HPF. Focusing on increasing dietary fiber may allow for a targeted message with broad implications for diet quality, but more research is needed. It is with growing sadness (but with expectations of time for some new activities in my life) that I announce that this is my final issue as editor of Australian Health Review (AHR). The new editor, Dr Gary Day from Griffith University in Queensland, is well suited to take over, with continued support from Dr Deborah Roberts, the Models of Care editor, and from the Editorial Board. Australian Health Review is over 30 years old and has achieved growing recognition both nationally and internationally. It has been a pleasure to have contributed to this excellent journal. The landscape of Australian health policy and management journals has changed over the past few years and further changes, to better meet the needs of authors and readers, are in store for AHR in 2010 and beyond. Over my tenure as editor, with much assistance from Professor Judith Dwyer, Dr Deborah Roberts, Dr Gary Day, Prue Power and the Editorial Board, and the publishing team at the Australasian Medical Publishing Company, we have achieved many milestones. Amalgamation of AHR with other journals will continue to strengthen the Australian presence in international scholarly publications. The online manuscript service has proved an efficient and effective mechanism for authors, reviewers and editors. The number of papers submitted for consideration has continued to grow, with over 100 papers submitted each year, of which about 60% are published. This large number of papers has meant that I am enormously grateful to the AHR reviewers. The 2009 reviewers are acknowledged in this issue (page 696). Thank you for volunteering your precious time to this most important task. The large number of papers has also meant that the page numbers of each issue have crept up to try to ensure authors do not have to wait too long to see their work in print. Mobility limitations were operationalized as a log-transformed count of 11 indicators of limitation in physical mobility. Dietary quality was measured using the Alternative Healthy Eating Index-2010 (AHEI-2010) based on responses to a food frequency questionnaire. Food insecurity was a binary measure based on the USDA six-item short form. Nutritional assistance included receipt of supplemental food from sources such as food banks and/or reporting receipt of SNAP benefits (1=yes, 0=no). Autoregressive multiple regression was used to test whether AHEI-2010 predicted change in mobility limitations from 2012-2014 and whether food insecurity or receipt of supplemental food moderated this relationship. Around 10.7% of older adults were food insecure, and around 17.5% reported receipt of nutritional assistance. AHEI-2010 was associated with a slower decline in mobility limitations over the 2-year observational window (b=-0.014, SE=.003, p&lt;.001), but food insecurity nor nutritional assistance were associated with changing mobility limitation, either directly or as moderators of the association between progressing limitations and AHEI-2010. These preliminary findings suggest dietary quality may be associated with disablement among older adults regardless of food environment.

ABSTRACT Background The frailty syndrome is associated with higher risk of disability and death after accounting for multimorbidity. Therefore, the determinants of frailty need to be identified to ensure older adults live not only longer but also healthier lives. However, the effect of diet quality on frailty is mostly unknown. Objectives We aimed to evaluate the alternate Mediterranean diet (AMED), the Dietary Approaches to Stop Hypertension (DASH) diet, and the alternate Healthy Eating Index-2010 (AHEI-2010) in association with frailty risk among older women. Methods We analyzed data from 71,941 women aged ≥60 y participating in the Nurses’ Health Study. The AMED, DASH, and AHEI-2010 were computed from validated FFQs in 1990 and repeated every 4 y until 2010. Frailty was defined as having ≥3 of the following 5 criteria from the FRAIL scale: fatigue, reduced resistance, reduced aerobic capacity, having ≥5 illnesses, and weight loss ≥5%. The occurrence of frailty was assessed every 4 y. Results During follow-up we identified 11,564 incident cases of frailty. After adjusting for potential confounders, the RRs (95% CIs) of frailty per 1-SD increase in the AMED, DASH, and AHEI-2010 scores were 0.87 (0.85, 0.90), 0.93 (0.91, 0.95), and 0.90 (0.88, 0.92), respectively. All diet quality scores were associated with lower risk of the individual frailty criteria fatigue, reduced resistance, reduced aerobic capacity, and weight loss. Lower consumption of red and processed meat, a lower sodium intake, a higher ratio of monounsaturated to saturated fat, vegetables, and moderate alcohol intake were components of the diet quality scores independently associated with lower risk of frailty. Conclusions Adherence to a healthy diet, as defined by the AMED, DASH, and AHEI-2010 scores, was associated with reduced risk of frailty in older women.

AbstractEarlier studies have shown a protective association between adherence to healthy eating guidelines and mental disorders in Western nations; however, data in this regard are limited from the understudied region of Middle East. We examined the association between adherence to healthy eating guidelines, as measured by Alternative Healthy Eating Index (AHEI)-2010, and prevalence of anxiety and depression in a large sample of Iranian adults. In this cross-sectional study, data on dietary intakes of 3363 adult participants were collected using a validated dish-based 106-item semi-quantitative FFQ. Adherence to healthy eating was quantified using AHEI-2010, as suggested by earlier publications. The Iranian validated version of Hospital Anxiety and Depression Scale was used to assess anxiety and depression in study participants. Data on other covariates were gathered using a pre-tested questionnaire. Overall, the prevalence of anxiety and depression was 15·2 % (males 10·8 % and females 18·3 %) and 30·0 % (males 22·9 % and females 35·1 %), respectively. After controlling for potential confounders, those in the top quartile of AHEI-2010 had a 49 % lower chance of anxiety (OR 0·51; 95 % CI 0·35, 0·72) and a 45 % lower odds of depression (OR 0·55; 95 % CI 0·42, 0·72), compared with those in the bottom quartile. Stratified analysis by sex revealed that women in the highest categories of AHEI-2010 had a 49 % lower odds of having anxiety and depression, after adjustment for confounders, but no significant association was found in men. In addition, among individuals who were 40 years old or younger, those with high adherence to AHEI-2010 were 58 and 51 % less likely to have anxiety and depression, compared with those with less adherence. Adherence to healthy eating was inversely associated with a lower chance of anxiety and depression in Iranian adults. Prospective studies are required to confirm these associations in Middle-Eastern populations.

Hasil kevalidan dan kepraktisan, produk di oleh ahli materi sebesar 96,87 persen dengan valid, ahli media produk sebesar 96,87 persen dan guru memberi nilai 100 persen dengan unsur sangat valid dan berkutat dengan baik tanpa revisi. Hasil angket respon peserta didik pada uji coba produk dan uji coba pakai cobaan dengan rata-rata sebesar 97 persen dengan sangat praktis dan menarik dan bisa digunakan. Dari mana penelitian dan pengembangan media komunikasi berbasis android pada materi materi proklamasi kemerdekaan kelas V valid ahli materi, ahli media dan guru, serta praktis dan menarik bagi peserta didik.

Model penelitian dan pengembangan yang dikembangkan diadopsi dari model pengembangan 4D (Four-D model) dari S. Thiagarajan, dkk (1974) tanpa menggunakan tahap disseminate. Kelayakan produk hasil pengembangan diperoleh dari uji validasi produk oleh ahli dan uji keterbacaan pada siswa. Data dalam penelitian ini adalah data kualitatif dan data kuantitatif. Data kualitatif didapatkan dari angket yang berisi tanggapan dan saran, sedangkan data kuantitatif didapatkan berdasarkan nilai rata-rata yang diperoleh dari skor angket validasi dengan menggunakan skala likert (skala bertingkat). Hasil uji validasi oleh ahli, menunjukkan bahwa produk sangat layak dengan persentase 97,3 persen untuk kelayakan materi dan 92 persen untuk kelayakan media. Berdasarkan uji keterbacaan pada siswa diperoleh persentase kelayakan bahan ajar digital sebesar 91,8 persen dan dapat dinyatakan bahwa bahan ajar digital yang dikembangkan sangat valid dan sangat layak. Dengan demikian bahan ajar digital pada materi sistem ekskresi manusia untuk siswa SMP Kelas VIII dinyatakan valid dan layak.

ABSTRACT Background Many dietary indexes exist for chronic disease prevention, but the optimal dietary pattern for colorectal cancer prevention is unknown. Objective We sought to determine associations between adherence to various dietary indexes and incident colorectal cancer in 2 prospective cohort studies. Design We followed 78,012 women in the Nurses’ Health Study and 46,695 men in the Health Professionals Follow-up Study from 1986 and 1988, respectively, until 2012. We created dietary index scores for the Dietary Approaches to Stop Hypertension (DASH) diet, Alternative Mediterranean Diet (AMED), and Alternative Healthy Eating Index-2010 (AHEI-2010) and used Cox regression to estimate HRs and 95% CIs for risk of colorectal cancer (CRC) and by anatomic subsite. We also conducted latency analyses to examine associations between diet and CRC risk during different windows of exposure. We conducted analyses in men and women separately, and subsequently pooled these results in a random-effects meta-analysis. Results We documented 2690 colorectal cancer cases. Pooled multivariable HRs for colorectal cancer risk comparing the highest to lowest quintile of diet scores were 0.89 (95% CI: 0.74, 1.08; P-trend = 0.10) for DASH, 0.89 (95% CI: 0.73, 1.10; P-trend = 0.31) for AMED, and 0.95 (95% CI: 0.83, 1.09; P-trend = 0.56) for AHEI-2010 (P-heterogeneity ≥ 0.07 for all). In sex-specific analyses, we observed stronger associations in men for all dietary indexes (DASH: multivariable HR = 0.81, 95% CI: 0.66, 0.98; P-trend = 0.003; AMED: multivariable HR = 0.80, 95% CI: 0.65, 0.98; P-trend = 0.02; AHEI-2010: multivariable HR = 0.88, 95% CI: 0.72, 1.07; P-trend = 0.04) than in women (multivariable HRs range from 0.98 to 1.01). Conclusions Adherence to the DASH, AMED, and AHEI-2010 diets was inversely associated with colorectal cancer risk in men. These diets were not associated with colorectal cancer risk in women. Non-communicable diseases (NCDs) and multimorbidity (≥two chronic conditions), are increasing globally. Diet is a risk factor for some NCDs. We aimed to investigate the association between diet quality (DQ) and incident NCDs. Participants were from the Australian Longitudinal Study on Women’s Health 1973–78 cohort with no NCD and completed dietary data at survey 3 (2003, aged 25–30 years) who responded to at least one survey between survey 4 (2006) and survey 8 (2018). DQ was measured by the Alternative Healthy Eating Index-2010 (AHEI-2010). Outcomes included coronary heart disease (CHD), hypertension (HT), asthma, cancer (excluding skin cancer), diabetes mellitus (DM), depression and/or anxiety, multimorbidity, and all-cause mortality. Repeated cross-sectional multivariate logistic regressions were performed to investigate the association between baseline DQ and NCDs over 15 years. The AHEI-2010 mean (±sd) for participants (n = 8017) was 51.6 ± 11.0 (range: 19–91). There was an inverse association between AHEI-2010 and incident asthma at survey 4 (ORQ5–Q1: 0.75, 95% CI: 0.57, 0.99). Baseline DQ did not predict the occurrence of any NCDs or multimorbidity between the ages of 25–45 years. Further well-planned, large prospective studies conducted in young women are needed to explore dietary risk factors before the establishment of NCDs.

Abstract Statins are 3-hydroxy-3-methylglutaryl coenzyme-A (HMG-CoA) reductase competitive inhibitors, that lower blood cholesterol, but also exert potent anti-inflammatory effects. The xenobiotic receptor, aryl hydrocarbon receptor (AhR), plays crucial role in the control of inflammatory responses. Evidences suggest that selected statins promote the transcription of the main AhR target gene, cytochrome P450 family 1 (Cyp1a1). Macrophages are essential components of innate and adaptive immunity, characterized by AhR expression and activity. Based on these evidences, we investigated potential AhR-mediated immune regulatory properties of statins in Bone Marrow Derived Macrophages (BMDM). We found that BMDM activation with the pro-inflammatory cytokine TNFa induced the inflammatory marker, inducible nitric oxide synthase (iNOS), which was completely prevented in BMDMs treated with TNFa in combination with specific statins, namely, atorvastatin and rosuvastatin. We confirmed high AhR expression in BMDM that was increased upon treatment with TNFa. We investigated the ability of different statins to activate AhR using a luciferase reporter assay cell lines and in BMDCs. We found that selected statins, such as atorvastatin and rosuvastatin, activated AhR in a dose dependent manner, an activation prevented in cells expressing selected AhR mutants. Notably, AhR deficient BMDMs, expressed undetectable levels of the main statin target protein HMG-CoA, which was instead highly expressed in wild type BMDMs. These data demonstrate that natural or synthetic statins can play regulatory roles in macrophages. Moreover, our results suggest a potential involvement of AhR in statin-mediated anti-inflammatory effects in macrophages.

The association between these scores and chronic kidney disease (CKD) is undetermined.ObjectiveWe aimed to estimate the association between the HEI, AHEI, and aMed scores and risk of incident CKD.MethodsWe conducted a prospective analysis in 12,155 participants aged 45–64 y from the Atherosclerosis Risk in Communities (ARIC) Study. We calculated HEI-2015, AHEI-2010, and aMed scores for each participant and categorized them into quintiles of each dietary score. Incident CKD was defined as an estimated glomerular filtration rate <60 mL/min/1.73 m2 accompanied by ≥25% decline in estimated glomerular filtration rate, a kidney disease–related hospitalization or death, or end-stage renal disease. We used cause-specific hazard models to estimate risk of CKD from the quintile of the dietary score through to 31 December 2017.ResultsThere were 3980 cases of incident CKD over a median follow-up of 24 y. Participants who had higher adherence to the HEI-2015, AHEI-2010, and aMed scores were more likely to be female, have higher educational attainment, higher income level, be nonsmokers, more physically active, and diabetic compared with participants who scored lower. All 3 dietary scores were associated with lower CKD risk (P-trend < 0.001). Participants who were in the highest quintile of HEI-2015 score had a 17% lower risk of CKD (HR: 0.83; 95% CI: 0.74, 0.92) compared with participants in the lowest quintile. Those in quintile 5 of AHEI-2010 and aMed scores, respectively, had a 20% and 13% lower risk of CKD compared with those in quintile 1.ConclusionHigher adherence to healthy dietary patterns during middle age was associated with lower risk of CKD.

Prevalensi infeksi terkait perawatan kesehatan (HCAI) saat ini adalah masalah kesehatan utama dan kontak pasien di OR dan PACU dapat menjadi faktor yang berkontribusi karena kontaminasi mikroba. Metode: Sebuah studi observasional dilakukan pada kepatuhan kebersihan tangan (HH) HCP yang bekerja di OR dan PACU dari fasilitas perawatan kesehatan tersier di Rumah Sakit X. Para peserta dalam penelitian ini terdiri dari ahli bedah, ahli anestesi, dan perawat yang beredar dan PACU. Pengamatan dan pengumpulan data dilakukan oleh dua perawat sumber daya klinis OR dan PACU, yang telah dilatih oleh departemen pengendalian dan pencegahan infeksi. Hasil : penelitian ini mencakup 2300 peluang untuk kepatuhan HH dan mengamati 200 perawat, 100 ahli bedah dan 100 ahli anestesi. Kepatuhan HH di OR dan PACU tertinggi di antara perawat >90% dan terendah di antara dokter, ahli bedah <70%, diikuti oleh ahli anestesi < 50%. Kesimpulan: Kepatuhan terhadap pedoman HH oleh perawat OR dan PACU diterima lebih tinggi daripada ahli anestesi dan ahli bedah.
