Full textAbstract part 1 Acute exacerbations of chronic obstructive pulmonary disease (AECOPDs) frequently cause patients with COPD to access the emergency department and have a negative impact on the course of the disease. The objectives of our study were: 1) describing the socio-demographic and clinical characteristics, and the clinical management, of patients with AECOPD, when they present to the emergency department; and 2) estimating the costs related to the management of these patients. We conducted a retrospective cohort study in Italy, collecting data on 4,396 patients, from 34 centres. Patients had a mean (SD) age of 76,6 (10.6) years, and 61.2 % of them where males. More than 70 % of the patients had a moderate to very high comorbidity burden, and heart failure was present in 26.4 % of the cohort. The 64.6 % of patients were admitted to hospital wards, with a mean (SD) length of stay of 10.8 (9.8) days. The estimated cost per patient was 2617 €. Conclusions: Patients attending the ED for an AECOPD are old and present important comorbidities. The rate of admission is high, and costs are remarkable. Abstract part 2 Exacerbations of chronic obstructive pulmonary disease (AECOPD) frequently require hospitalizations, may necessitate of invasive mechanical ventilation (IMV), and are associated with a remarkable in-hospital mortality. The BAP-65 score is a risk assessment model (RAM) based on simple variables, that has been proposed for the prediction of these adverse outcomes in patients with AECOPD. If showed to be accurate, the BAP-65 RAM might be used to guide the patients management, in terms of destination and treatment. We conducted a retrospective, multicentre, chart-review study, on patients attending the ED for an AECOPD during 2014. The aim of the study was the validation of the BAP-65 RAM for the prediction of in-hospital death or use of IMV (composite primary outcome). We assessed the discrimination and the prognostic performance of the BAP-65 RAM. We enrolled 2908 patients from 20 centres across Italy. The mean (standard deviation) age was 76 (11) years, and 38% of patients were female. The composite outcome occurred in 5.3% of patients. The AUROC of BAP-65 for the composite outcome was 0.64 (95%CI 0.59-0.68). The sensitivity of BAP-65 score ≥ 4 to predict in-hospital mortality was 44% (95% CI 34%-55%), the specificity was 84% (95% CI 82%-85%), the positive predictive value was 9% (95% CI 6%-12%), and the negative predictive value was 98% (95% CI 97%-98%). Conclusions: In patients attending Italian EDs with an AECOPD, we found that the BAP-65 score did not have sufficient accuracy to stratify patients upon their risk of severe in-hospital outcomes. Full textEsta dissertação analisa o gerenciamento cultural ao longo do ciclo de vida de uma organização. Para tanto, foi feito um estudo de caso da ENSR Brasil - ENSR International Brasil Ltda., empresa do mercado de consultoria ambiental, presente no Brasil desde 1998. Com base em uma análise qualitativa, neste trabalho foi avaliada a evolução da cultura organizacional da ENSR Brasil em seus diferentes estágios evolutivos e, com base em modelos e tipologias culturais identificados na literatura, analisado o processo de mudança cultural instituído na empresa a partir de 2006, depois de instituída influência mais direta e efetiva da matriz americana. O resultado deste estudo reforça a literatura sobre o tema referente à questão da forte correlação entre cultura e ciclo de vida, evidenciando a importância de variáveis como a estrutura de poder e o processo decisório no gerenciamento da mudança organizacional.
This essay analyses the cultural management along the organization life cycle. With this purpose, a case study was developed based on the research of the company ENSR Brasil - ENSR International Brasil Ltda., which was established in Brazil in 1998. Based on a qualitative analysis, this works evaluates the evolution of ENSR Brasil`s organizational cultural in its different evolution stages. Additionally, by applying models and typologies identified in the literature, it analyses the process of cultural change established in the company from 2006 on, after a more direct and effective influence of the American head office was instituted. The result of this study reinforces the literature about the subject related to the strong correlation between culture and life cycle, pointing out the importance of variables such as power structure and the decision process in the management of the organizational change.

Full textNowadays, more and more responsibility is being handed over to local authorities in the setting up and running of publicly-owned broadband networks. The way in which this growing responsibility is implemented has to be studied from the dual aspect of public and private intervention.For although, as planners of local development, local authorities are responsible for the coordination of public service broadband activities, they nevertheless call upon the expertise of private enterprise. This thesis analyses how cooperation between public and private sectors works and seems to be driven by a logic of functional subsidiarity. Local authorities, as economic operators subject to the competition rules of the Treaty, now acquire the status of telecommunications service provider and are thus governed by specificliberalised sector regulations. Full textIn this PhD thesis, an analysis of success conditions of an educational improvement program was performed. This study was focused in factors determining change in schools and how are shown them in public schools at Coyhaique, Chile. This study was supported in framework of project “Apoyo y Fortalecimiento Educacional en gestion Directiva y Competencias Profesionales Docentes” in collaboration with Agencia Española de Cooperacion Internacional para el Desarrollo, Universidad Autónoma de Barcelona and Municipalidad de Coyhaique. The main goal of this study was detected if change conditions in Coyhaique schools can be feasible. In this way was necessary a) to determine conditions that can influence the success of an educational program, by designing a comprehensive evaluation model. This model have allowed to know the study context and identify the main change conditions; b) to carry out a bibliographic study to establish institutional and territorial conditions, which can determine changes in the education and c)to analyse qualitatively change conditions through the experiencies of principals. In order to better conduce change processes, a theoretical and practical study was performed. In order to better promote educational changes, factors that inhibit and facilitate changes in specific educational contexts were determined. The methodology includes guidelines and procedures adjusted to the reality of schools and improvement programs. The main results have allowed recognize factors that favor and inhibit change processes in the school. Schools are aware in the theory of the conditions that help your development. But in practice there are limitations that avoid the improvement. These limitations are recognized in different aspects or areas of education. Some limitations are competences and professional deficiencies of teachers, the educational policies and administrative structures, the student's family background, geographic location and regional development. But is very important highlight the participation of entire school in the dynamization of the educational change.

De uma forma geral é difícil associar recursos tão avançados da tecnologia à forma que se conduz a operação e a manutenção nas edificações na atualidade. Quando se refere à adoção de recursos tecnológicos nesta etapa do ciclo de vida da edificação, é comum relacioná-los a métodos tradicionais de operação, uma grande quantidade de documentos, que geralmente não são achados quando necessário, ou recursos utilizados para desenvolver a manutenção preditiva. Este cenário não é diferente na Faculdade de Arquitetura da UFBA (FAUFBA), onde os processos de operação e manutenção seguem padrões tradicionais, de pouca eficiência e eficácia, que dependem de departamentos como a Superintendência de Meio Ambiente e Infraestrutura da UFBA (SUMAI), responsável pela manutenção e conservação dos campi de Salvador: campus Federação / Ondina, campus do Canela, das unidades dispersas e também do campus Anísio Teixeira em Vitória da Conquista. O presente trabalho aborda a utilização da tecnologia CAFM em Instituições de Ensino Superior. Para tanto, foram analisados estudos de caso sobre a aplicação de CAFM na operação e na manutenção de Universidades nacionais e no exterior. Foram identificadas as dificuldades e a metodologia adotada para a implementação da tecnologia, que propõe a concentração de informações essenciais ao processo de gestão da edificação. Diante de recursos promissores, desde a gestão de instalações físicas à gestão de pessoas, a FAUFBA foi utilizada como objeto de trabalho para a implementação do Sistema de Alocação de Espaços em uma aplicação de Facilities Management. Para este propósito, foram utilizados modelos BIM das edificações que compõem a FAUFBA para a realização de testes. Ressaltase a importância da precisão dos dados alfanuméricos ou geométricos, visto que, este é um sistema de banco de dados espacialmente referenciado, ou seja, um repositório de informações, que é de grande valor durante a etapa de operação e manutenção da edificação.

In general, it is difficult to associate such advanced technology resources with the way that operation and maintenance in today's buildings is conducted. Regarding the adoption of technological resources at this stage of edification life cycle, it is common to relate them to traditional methods of operation. Usually a large number of documents are not found when necessary, or resources used to develop predictive maintenance. This scenario is no different in the Faculty of Architecture of UFBA (FAUFBA), where operation and maintenance processes follow traditional, lowefficiency and efficacious standards, which depend on departments such as the UFBA Superintendence of the Environment and Infrastructure (SUMAI), responsible for the maintenance and conservation of the campuses of Salvador: Federação / Ondina campus, Canela campus, dispersed units and also the Anísio Teixeira campus in Vitória da Conquista. The present work deals with the use of CAFM technology in Higher Education Institutions. Therefore, we analyzed case studies on the application of CAFM in the operation and maintenance of national universities and abroad. We identified the difficulties and the methodology adopted for the implementation of the technology, which proposes the concentration of essential information to the process of building management. Considering the necessities of the university with regards of management of physical facilities and personnel, it was used as a work object for the implementation of the Space Allocation System, using Facilities Management. For this purpose, BIM models of the buildings that make up the FAUFBA were used to carry out tests. The importance of the precision of the alphanumeric or geometric data is emphasized, since this is a spatially referenced database system - a repository of information, which is of great value during the stage of operation and maintenance of the building.

Full textO Building Information Modeling é uma metodologia de trabalho aplicável de forma transversal em toda a indústria da construção que vem sistematizar conjunto de políticas, processos e tecnologias inter-relacionadas para ter uma visão abragente do projeto e construção, contendo os seus dados num formato digital para poder acompanhar todas as fases do ciclo de vida das construções. O desenvolvimento deste trabalho numa primeira fase passa pela demonstração das vantagens do uso desta metodologia, faz resenha dos países que já a implementaram bem como apresenta a análise dos resultados obtidos a realização de um inquérito aos principais intervenientes a área de Arquitectura, Engenharia e Construção, tendo-se obtido 105 respostas válidas e os dados do estudo demonstram que o estado de conhecimento e implementação do BIM em Angola está na fase inicial. Na fase final fez-se a modelação partindo-se dos projetos arquitectónicos dos novos Edifícios do ISTM para a modelação piloto da CAFETARIA/BAR ISTM nas especialidades de estrutura, abastecimento de águas, drenagem e electricidade, e com exploração e demonstração de viabilidade de vários usos BIM. Espera-se que os resultados obtidos nesta pesquisa possam contribuir para conhecimento do estado da metodologia BIM em Angola bem como adoptar o plano apresentado para uso na fase de projeto de empreendimentos dessa natureza.
Building Information Modeling is a methodology that systematizes a set of interrelated policies, processes and technologies to have a comprehensive view of projects and the data in a digital format to be able to monitor all phases of throughout the cycle of life of buildings. This dissertation initially demonstrates the advantages of using this methodology, reviews the countries that have already implemented and presents the analysis of the results obtained by conducting a survey of the main actors in the area of Architecture, Engineering and Construction in Angola. A total of 105 valid answers was obtained and the study data show that the state of knowledge and implementation of BIM in Angola is in the early stage. In the final phase, the modelling was made starting from the architectural projects of the new ISTM Buildings for the pilot modelling of Cafeteria/BAR ISTM in the specialties of structure, water supply, drainage and electricity, with exploration and demonstration of viability of several BIM uses. It is expected that the results obtained in this research may contribute to the knowledge of the state of BIM methodology in Angola as well as to adopt the plan presented for use in the project phase of such projects of this nature.

Full textThe construction industry is changing, the lack of man labour is a sign of the changes that are affecting the world. Since the financial crisis of 2008, the construction suffers and eagers to find solutions to face the new demand for housing and the lack of response to the current market gap. The shift for off-site construction is being talked recently with new development in technology with Building Information Modelling (BIM) leading this new transition. Design for Manufacturing and Assembly (D is the idea in this process, enhancing the design and production of the building parts in-house to be later assemble and transported to the site, providing a viable solution to the challenge presented. This dissertation result from a collaboration with Grupo Casais, a Portuguese construction company based in the city of Braga. The aim was to develop sustainable prefabrication processes, to be applied in current and future projects. Namely, the integration of BIM tools for the design and manufacturing of partitions walls system with a high degree of combinatorial application. The goal of the process contemplates the possibility of maximizing the repetition of prefabricated elements to be used and systematize their assembly on site. Therefore, by following the principles of DfMA to incorporate the maximization of resources and time, without compromising stakeholders and benefiting from the advantages of offsite construction. The solution developed uses Visual Programming to generate a process to divide and sort rooms by their typology, to later export as data. Additionally, through the research it used parameters with the aim of helping in grouping and sorting (within specific constrains) the building parts. This method allows to systematize the knowledge needed to define a computational tool for the manufacturing optimization of the rooms of a given project. With this goal in mind, the research was focused on the development of computational models that maximize the output of all the information. This research aims to bridge the gap of prefabrication between architecture and engineer by applying DfMA through BIM tools. The desirable outcome can be applied to a project that was not prepared to be prefabricated, hence making it a reliable solution to make the transition. With the systematic definition of a set of computational models in a BIM Application Programming Interface (API), the aim was to coordinate and cooperate all the information available inside the model to optimize and generate information that can further help in the prefabrication and planning of building components.
A construção civil está a passar por mudanças, a falta de mão de obra é um sinal das alterações que estão afetando o mundo. Desde a crise financeira de 2008, a construção sofre e anseia por encontrar soluções para fazer face à nova procura de habitações e à falta de resposta à atual procura de mercado. A mudança para construção em fábrica tem vindo a ser debatida recentemente com o desenvolvimento na tecnologia, onde Building Information Modeling (BIM) lidera esta nova transição. Projeto para Manufatura e Montagem (DfMA) é a ideia neste contexto, potencializando o projeto e a produção de componentes pré-fabricados (Off-site construction), para serem posteriormente montadas e transportadas para o local, proporcionando uma solução viável para o problema apresentado. Esta dissertação resulta de uma colaboração com o Grupo Casais, uma empresa de construção portuguesa sediada na cidade de Braga. O objetivo era desenvolver processos de pré-fabricação sustentáveis, para serem aplicados em atuais e futuros projetos. Neste contexto, a integração de ferramentas BIM em projeto e fabricação de sistema de paredes divisórias, com várias aplicações e combinações. O objetivo deste processo desenvolve-se na possibilidade de maximizar a repetição dos elementos pré-fabricados a serem utilizados e sistematizar a sua montagem no local. Portanto, beneficiada implementação de DfMA, maximiza-se recursos e tempo, sem comprometer todas as especialidades envolvidas, aproveitando as vantagens da construção em fábrica. A solução desenvolvida utiliza a Programação Visual para gerar um processo de divisão e ordenação de espaços pela sua disposição, para posteriormente transformar em dados. Para além disso, através da investigação utilizou-se parâmetros com o objetivo de ajudar a organização e classificação (dentro de limites específicos) das componentes da construção. Este método permite sistematizar os conhecimentos necessários para definir uma ferramenta computacional para a otimização da fabricação dos espaços de um determinado projeto. Com este objetivo em mente, a investigação teve como foco o desenvolvimento de modelos computacionais que maximizem a saída de todas as informações. Esta investigação visa completar a ligação da pré-fabricação entre arquitetura e engenharia aplicando DfMA por meio de ferramentas BIM. O resultado visado permite a aplicação num projeto que não foi preparado para ser pré-fabricado, tornando-o uma solução que permite a transição. Com a sistematização de um conjunto de modelos computacionais em uma Interface de Programação de Aplicativos (API) BIM, o objetivo foi coordenar e incorporar todas as informações que constam no modelo para otimizar e gerar dados que possam auxiliar na pré-fabricação e planeamento da fabricação das componentes.

Full textO Building Information Modelling (BIM) é muito mais do que um software usado para a construção em 3D. Esta metodologia surge no contexto da digitalização e centra-se na comunicação de informação entre os diversos intervenientes, incluindo as áreas de projeto, engenharia, construção e manutenção de infraestruturas. Constitui uma grande oportunidade para o setor da construção e para os seus profissionais baseada na integração e colaboração entre as várias fases de projeto. Em Portugal, e considerando o grau de adoção do BIM noutros países, a metodologia ainda não é amplamente aplicada. Verificando-se que na maior parte dos casos, a sua implementação nas empresas de construção é efetuada segundo a iniciativa dos colaboradores que tenham interesse nesta temática (abordagem bottom-up) ou pelos administradores que vêem o BIM como uma oportunidade de negócio a longo prazo (abordagem top-down). Alguns intervenientes com largos conhecimentos do BIM e com forte presença na indústria da construção afirmam que para a implementação do BIM em Portugal seja agilizada, é necessário envolver no processo o Estado, que deverá constituir-se como um agente de mudança dando o exemplo ao usar o BIM nos procedimentos de aquisição, tanto na fase de conceção como na de execução. Assim, é importante reter que o Plano de Execução BIM (BEP) deve definir os usos BIM a implementar ao longo do projeto de construção, desde a conceção até à sua operação. Nesse sentido, este trabalho tem como objetivo auxiliar as empresas do setor AECO (Arquitetura, Engenharia, Construção e Operação) a efetuar contratações de serviços em BIM. O principal foco do presente trabalho é a elaboração de uma proposta para um BEP para empresas de construção que operem quer no mercado nacional como no internacional, complementado por uma proposta para pagamentos de serviços contratados segundo a metodologia BIM. Esta proposta concentra-se em auxiliar as equipas de projeto na elaboração do melhor plano para a implementação da metodologia, durante todas as etapas de um projeto. Para a elaboração da proposta do BEP foi necessário efetuar um aprofundado e rigoroso estudo das normas de BIM/BEP existentes internacionalmente. Posteriormente, foi realizado um caso de estudo onde se analisou a viabilidade da proposta efetuada para o BEP em contexto empresarial.
The Building Information Modelling (BIM) is much more than a software used for 3D construction. This approach arises in the context of digitization and focuses on communication of information between the various players, including the areas of design, engineering, construction and maintenance of infrastructure. Is a great opportunity for the construction industry and its professionals based on integration and collaboration between the various project phases. In Portugal, and considering the degree of adoption of BIM in other countries, the methodology is still not widely applied. It is verified that in most cases, its implementation in construction companies is carried out according to the initiative of employees who are interested in this subject (bottom-up approach) or by the managers who see BIM as a long-term business opportunity (top-down approach). Some stakeholders with a broad knowledge of BIM and with a strong presence in the construction industry say that for the implementation of BIM in Portugal to be streamlined, it is necessary to involve in the process the State, which should constitute itself as a change agent setting the example when using BIM in procurement procedures, both at the design and execution stages. Thus, it is important to keep in mind that the BIM Execution Plan (BEP) should define the BIM uses to be implemented throughout the construction project, from conception to operation. In this sense, this work aims to assist the companies in the AECO (Architecture, Engineering, Construction and Operation) sector in contracting services in BIM. The main focus of the prsent work is the elaboration of a proposal for a BEP for construction companies operating both in the national and international market, complemented by a proposal for payments of contracted services according to BIM methodology. This proposal focuses on assisting the project teams in designing the best plan for implementing the methodology during all stages of a project. In order to prepare the BEP proposal, it was necessary to carry out a thorough and rigorous study of the international BIM/BEP standards. Subsequently, a case study was carried out to analyze the feasibility of the proposal made to the BEP in a business context.
