Abstract:
Approximately 26% of postpartum women meet diagnostic criteria for a psychiatric disorder. Untreated psychopathology in the postpartum period is associated with a range of adverse outcomes for both infant and mother. Fortunately, the perinatal period provides an opportune time to intervene with and prevent postpartum psychopathology. Women have increased contact with health care providers during this time, providing an avenue through which access to prevention can be improved. Furthermore, with increased knowledge to identify high-risk women, preventive interventions can be delivered to assist both the woman and infant. Preventive efforts for postpartum psychopathology are aimed at modifying risk factors or protective factors to prevent the psychiatric disorder and primarily use three distinct approaches: universal, selective, and indicated. This chapter provides a review of the empirical research in the prevention of postpartum psychopathologies including postpartum depression, anxiety, bipolar disorder, psychosis, and addictions (tobacco, alcohol, and illicit drugs).

Environmental chemicals interact with the normal processes occurring in human bodies in many detrimental ways. Chapter 3 discusses the basic mechanisms by which exposures to environmental chemicals perturb normal homeostasis; interfere with the communication systems that coordinate proper functioning of cells, tissues, and organs; and modify gene expression by inducing epigenetic changes. Environmental chemicals disrupt homeostasis in adults, but fetuses, infants, children, and adolescents, whose homeostatic control systems are continuing to develop, are at the greatest risk for long-term adverse health outcomes due to even brief exposures during vulnerable periods in development.

Cardiovascular disease is a leading cause of maternal morbidity and mortality worldwide. Complex valvular heart disease accounts for approximately 30% to 50% of all cardiac diseases of pregnancy and presents significant challenges to the management of the parturient affected by it. Determination of disease severity and maternal risk assessment are especially important to development of appropriate plans of care for the labor, delivery, and immediate postpartum periods, when adverse events for both mother and fetus may occur. An understanding of the pathophysiology of the causative lesions and hemodynamic goals, thorough evaluation, and a multidisciplinary approach are key components to the successful management of these patients, allowing for appropriate selection of an anesthetic technique that balances the benefits and consequences to both mother and infant, thereby leading to optimal patient outcomes.

Syphilis, a chronic, sexually transmitted disease caused by the extracellular spirochete Treponema pallidum, has exhibited a remarkable resurgence in recent years. Despite the existence of inexpensive, easily administered, and highly effective antibiotic treatments, maternal and neonatal syphilis infections continue to be a major global public health problem. In addition to its potential to cause morbidity in the mother, untreated gestational syphilis (GS) can lead to serious adverse outcomes in the offspring, including stillbirth, prematurity, low birth weight, and neonatal death. Congenital syphilis (CS) is regarded as a missed opportunity during the antenatal care of the mother, resulting from socioeconomic, demographic, and behavioral factors that promote mother-to-child transmission (MTCT) of syphilis. This chapter emphasizes emerging concepts about screening aimed at controlling the ongoing epidemic, including serological screening of mother and infant, newer paradigms of “reverse screening,” clinical presentation, therapy, and long-term neurodevelopmental disabilities that must be a component of follow-up care.
