Abstract:
An individual’s sodium to potassium intake ratio (Na:K) has been shown to be an important predictor of hypertension. The aim of this study was to estimate the mean 24 h urinary Na, K and Na:K of Irish adults and to identify the foods that determine Na:K in a nationally representative sample of Irish adults. This study was based on data from the Irish National Adult Nutrition Survey (2008–2010) (NANS), which collected spot urine samples and dietary data in a nationally representative sample of Irish adults aged 18+ years. The mean urinary molar Na:K of Irish men and women was 1.90 and 2.15, respectively, which exceed target molar ratios of ≤1.0 and ≤2.0. The mean estimated 24-h urinary excretion of Na was 4631 mg for men and 3525 mg for women, which exceed target maximum population intakes for all gender and age groups. The mean estimated 24-h urinary excretion of K was 3894 mg for men and 2686 mg for women, with intakes in women of all ages and older men (65+ years) below current recommendations. The key foods positively associated with a lower Na:K were fruits, vegetables, potatoes, breakfast cereals, milk, yogurt and fresh meat, while the foods negatively associated with a lower Na:K were breads, cured and processed meats and butters and fat spreads. Strategies to reduce sodium and increase potassium intakes are necessary to lower population Na:K, which may help to reduce the burden of hypertension-related diseases in the Irish population.

Abstract:
The incidence of varicose veins in the Irish adult population was studied. Using a questionnaire attached to the quarterly Economic Consumer Survey it was found that approximately 13% of Irish adults have varicose veins. Women out numbered men by 3:1 and the incidence in both sexes increased with age, 70% of sufferers being over 45 years. Also, those in standing employment and women who had had many pregnancies had a higher than average incidence for their age.

Abstract:
This article explores the evolution of Irish youth literature over the last four decades and these texts' engagement with cultural, political, and social transformations in Irish society. The adult desire to protect young people's ‘innocence’ from topics and experiences deemed dark or deviant tended to dominate late twentieth-century Irish youth literature. However, the turn of the millennium witnessed a growing capacity and willingness for Irish children's and young-adult authors to problematize hegemonic power systems, address social injustices, and present unsentimental, empowering narratives of youth agency. Post-Celtic Tiger youth writing by Irish women has advocated for the complexity of Irish girlhoods while Irish Gothic literature for teenagers has disrupted complacent narratives of Irish society in its anatomy of systemic violence, trauma, and adolescent girls' embodiment. Although queer identities and sexualities have been increasingly recognised and represented, Irish youth literature has yet to confront histories and practices of White privilege in past and present Irish culture and to inclusively represent the diverse, intersectional realities, identities, and experiences of twenty-first-century Irish youth.

Significant increments were also reported in waist and hip circumferences and waist:hip ratio between 2001 and 2011, with concurrent increases in the proportion of individuals at risk of developing CVD, particularly females aged 18–35 years. In 2011, 23·4 % of the Irish population was classified as obese; with the mean BMI increasing by 1·1 kg/m2between 1990 and 2001 and by 0·6 kg/m2between 2001 and 2011.ConclusionsThe present paper characterises obesity levels in Irish adults from 1990 to 2011. Absolute levels of overweight and obesity have increased between these time points. Of concern is the increase in the proportion of young women classified as at risk of CVD, using waist circumference and waist:hip ratio. Effective prevention strategies are needed to avoid further increases.

In Joyce’s novel A Portrait of the Artist as a Young Man the representation of the Irish nation is closely interwoven with that of Irish women. Two groups can be distinguished among the women and girl characters: the women who are a symbol of authority and those who embody desire. Stephen’s mother and Dante Riordan, a family relative and re ligious fanatic who closely surveyed and inf luenced his early childhood, symbolize those Irish who firmly supported the dogma that the Irish nation’s identity was not to be sepa rated from the nation’s necessity in being a Roman Catholic one, subdued to the domina tion of both Rome and London. Stephen, after having accepted this view as a child, refuses this standpoint as rigid and narrowminded; in one word, as a dangerous stereotype with disastrous consequences for the future of Ireland as he becomes an adolescent.As for the other group, the girl named (Stephen’s) desire, the one central and recur rent image which appears in its description is that of the “batlike soul”. The metaphor is deeply significant for the theme of this essay, as the girl characters are portrayed as unaware of themselves and only coming to consciousness, just as the Ireland of the epoch was seen and portrayed by young Stephen. The women, object of desire, are also seen as adulterous: but to betray, Stephen soon gets to understand, is the only way to be faithful – to himself and to his vision of what Ireland is yet to become.Thus the representation of the Irish nation is not only in connection with that of Irish women, but also in relation with a process of creation of Stephen’s own identity, as he slowly liberates himself from the public opinion and becomes a free minded and inde pendent adult, aware of the impact and importance his future artist vocation will have for him, as well as for his whole country.

LGBTQ+ youths in the Republic of Ireland report statistically higher levels of depression, anxiety and suicidal thoughts than their heteronormative peers, which can be attributed to bullying and homophobic rhetoric. Research indicates that community services, such as public libraries, can play a role in mitigating the mental health risks of this group. However, there is no formal policy within the Irish public library system directing the collection and provision of LGBTQ+ materials and services to anyone, let alone youths. Previous international studies have shown that, in the absence of a guiding intersectional collection development policy, LGBTQ+ library materials are overwhelmingly representative of the gay, white, adult male experience, to the detriment of other groups within the LGBTQ+ community. Conducting a quantitative content analysis of the Dublin City Council Public Library catalogue through the lens of intersectionality theory confirms that the Irish public library system is not an exception to this trend. Results indicate that catalogue materials containing LGBTQ+ metadata favor the adult, gay, male experience – as well as the youth, gay, male experience – over adult and young women. This trend is particularly noticeable among the eBook catalogue, an area that the Irish public library system has directly identified as a strategic target for collection development. Conclusions align with previous qualitative studies on LGBTQ+ provision in Irish libraries in that a comprehensive organizational policy document is needed to provide direction and enable funding for the development of the LGBTQ+ section of the library system’s catalogue.

With a focus on clothing, bodies, and emotions, this article examines girls’ First Holy Communions in twentieth-century Ireland (c. 1920–1970), demonstrating that Irish girls, even at an early age, embraced opportunities to become both the center of attention and central faith actors in their religious communities through the ritual of Communion. A careful study of First Holy Communion, including clothing, reveals the importance of the ritual. The occasion was indicative of much related to Catholic devotional life from independence through Vatican II, including the intersections of popular religion and consumerism, the feminization of devotion, the centrality of the body in Catholicism, and the role that religion played in forming and maintaining family ties, including cross-generational links. First Communion, and especially the material items that accompanied it, initiated Irish girls into a feminized devotional world managed by women and especially mothers. It taught them that purchasing, hospitality, and gift-giving were central responsibilities of adult Catholic women even as it affirmed the bonds between women family members who helped girls prepare for the occasion.

AbstractI is an important mineral for health, required for the production of key thyroid hormones, which are essential for cellular metabolism, growth and physical development. Hence, adequate I is crucial at all stages of life, but imperative during pregnancy for fetal brain development and during a child’s early life for neurodevelopment. Within Ireland, limited information exists on population I intakes and status. Therefore, the purposes of the present analysis were to estimate dietary I intakes and to analyse urinary iodine (UI) status using the cross-sectional National Adult Nutrition Survey 2008–2010 and the most recent Irish Total Diet Study. Median I intakes in the total population (n1106) were adequate with only 26 % of the population being classified as below the estimated average requirement (EAR). Milk consumption was the major source of I in the diet, contributing 45 % to total intake. Likewise, median UI concentrations (107 µg/l) indicated ‘optimal’ I nutrition according to the WHO cut-off points. In our cohort, 77 % of women of childbearing age (18–50 years) did not meet the EAR recommendation set for pregnant women. Although I is deemed to be sufficient in the majority of adult populations resident in Ireland, any changes to the current dairy practices could significantly impact intake and status. Continued monitoring should be of priority to ensure that all subgroups of the population are I sufficient.

AbstractObjectiveTo describe macronutrient intakes and food sources of the adult population in the Republic of Ireland and Northern Ireland and to assess adherence of this population to current dietary recommendations.DesignA cross-sectional food consumption survey collected food intake data using a 7-day food diary.SettingNorthern Ireland and the Republic of Ireland between October 1997 and October 1999.SubjectsOne thousand three hundred and seventy-nine adults aged 18–64 years (662 males and 717 females).ResultsMean daily energy intakes in men were 11 MJ per day, 15.5% was derived from protein, 34.8% from fat, 43.5% from carbohydrate and 5.9% from alcohol. Corresponding figures for women were 7.6 MJ per day, 15.6%, 35.6%, 45.1% and 3.5%. When alcohol energy was excluded the contribution of fat and carbohydrate to energy did not differ between men and women. When compared with existing dietary recommendations, 93% of men and 86% of women had protein intakes above the Population Reference Intake. Two approaches were used to assess adherence to the fat and carbohydrate dietary recommendations: (1) the proportion of individuals in the population attaining these dietary targets and (2) the proportion of the population that was included in a 'compliers' group which had a group mean equal to these dietary targets. Thirty-three per cent of men and 34% of women met the target of 35% of food energy from fat and 78% of men and 80% of women comprised the ‘compliers’ group having a group mean of 35% of food energy from fat. Twentythree per cent of men and 27% of women met the target of 50% of food energy from carbohydrate and 56% of men and 62% of women made up the 'compliers' group. Meat and meat products were the main source of fat (23%) and protein (37%), and bread and rolls (25%) were the main source of carbohydrate.ConclusionA reduction in dietary fat intake remains an important public health issue in the Republic of Ireland and Northern Ireland. An increase in carbohydrate intake and attention to the rise in alcohol intake is also warranted.

This study aimed to investigate the independent and synergistic effects of childhood and adult social class, as well as the effect of social mobility, on type 2 diabetes (T2D) risk in later life. Cross-sectional data from The Irish Longitudinal Study of Ageing (TILDA) (n = 4,998), a nationally representative probability sample of adults aged 50 and older, were analysed. Prevalent diabetes was defined using subjective (self-reported doctor’s diagnosis) and objective data (medications usage and glycated haemoglobin testing). Social class was classified as a three-level variable based on fathers’ occupation in childhood and respondents’ primary occupation in adulthood. A five-level social mobility variable was created from cross-classification of childhood and adulthood social class. Logistic regression was employed to assess the relationship between social class variables and T2D. Mean (SD) age of the sample was 63.8y (9.9) and 46.4% were male. Incidence of T2D was 11.6% of men and 7.7% of women. Some 57.4% of the sample were classified as Manual social class in childhood. Compared to those in Professional/Managerial occupations, belonging to the Manual social class in childhood was associated with an increased risk of T2D in men (Odds Ratio (OR): 1.36, 95% CI: 0.88, 2.10) and women (OR: 2.16, 95% CI: 1.21, 3.85). This association was attenuated in women when controlled for adulthood social class (OR: 1.84, 95% CI: 1.00–3.37), suggesting that the effect of childhood social class may be modified by improving social circumstance over the life course.

Abstract This on-going ethnographic study focuses on adult music learning by examining motivations and successful learning strategies favored by 6 players in one weekly Celtic pub session. There were 2 women and 4 men, the average age of participants was 45, and each had been learning Celtic Traditional Music (CTM) between six and 16years. Three had learned to play an instrument at school but had discontinued playing their school instruments after graduation. Participants were attracted to CTM and traditional instruments for various reasons, and learned through the medium ofitunesj rather than scales and exercises. Because CTM is an aural/oral culture, participants valued learning "by ear," but did regard written notation as a useful aid, particularly as a mnemonic strategy. We conducted session documentation through photo, video, cyberspace information and audio recording balanced against in-depth interviews. Research questions included: What were the various attitudes, beliefs, and perceptions of session participants regarding learning traditional Irish/Celtic music? What are the characteristics of learning that occurred in this informal setting? What are the implications for lifelong music learning and formal music education? Information about this kind of informal learning practice has implications for formal school instrumental learning.

Background: Reference intakes for iron are derived from physiological requirements, with an assumed value for dietary iron absorption. A new approach to estimate iron bioavailability, calculated from iron intake, status, and requirements was used to set European dietary reference values, but the values obtained cannot be used for low- and middle-income countries where diets are very different. Objective: We aimed to test the feasibility of using the model developed from United Kingdom and Irish data to derive a value for dietary iron bioavailability in an African country, using data collected from women of child-bearing age in Benin. We also compared the effect of using estimates of iron losses made in the 1960s with more recent data for whole body iron losses. Methods: Dietary iron intake and serum ferritin (SF), together with physiological requirements of iron, were entered into the predictive model to estimate percentage iron absorption from the diet at different levels of iron status. Results: The results obtained from the 2 different methods for calculating physiological iron requirements were similar, except at low SF concentrations. At a SF value of 30 µg/L predicted iron absorption from the African maize-based diet was 6%, compared with 18% from a Western diet, and it remained low until the SF fell below 25 µg/L. Conclusions: We used the model to estimate percentage dietary iron absorption in 30 Beninese women. The predicted values agreed with results from earlier single meal isotope studies; therefore, we conclude that the model has potential for estimating dietary iron bioavailability in men and nonpregnant women consuming different diets in other countries.

Because of the discretionary nature of voluntary food fortification in the European Union, there is a need to monitor fortification practices and consumption of fortified foods in order to assess the efficacy and safety of such additions on an ongoing basis. The present study aimed to investigate the nutritional impact of changes in voluntary fortification practices in adults aged 18–64 years using dietary intake data from two nationally representative cross-sectional food consumption surveys, the North/South Ireland Food Consumption Survey (NSIFCS) (1997–9) and the National Adult Nutrition Survey (NANS) (2008–10). The supply of fortified foods increased between 1997–9 and 2008–10, resulting in a higher proportion of adults consuming fortified foods (from 67 to 82 %) and a greater contribution to mean daily energy intake (from 4·6 to 8·4 %). The overall nutrient profile of fortified foods consumed remained favourable, i.e. higher in starch and dietary fibre and lower in fat and saturated fat, with polyunsaturated fat, sugars and Na in proportion to energy. Women, particularly those of childbearing age, remained the key beneficiaries of voluntary fortification practices in Ireland. Continued voluntary fortification of foods has increased protection against neural tube defect-affected pregnancy by folic acid and maintained the beneficial impact on the adequacy of Fe intake. Increased consumption of fortified foods did not contribute to an increased risk of intakes exceeding the tolerable upper intake level for any micronutrient. Recent increases in voluntary fortification of foods in Ireland have made a favourable nutritional impact on the diets of adults and have not contributed to an increased risk of adverse effects.

Breakfast is considered to be one of the most important meals of the day. Its omission has been reported to be associated with increased disease risk, such as obesity, diabetes, and coronary heart disease, as well as unhealthy lifestyle and lower dietary quality. Using data from the National Adult Nutrition Survey (NANS)—a food consumption survey conducted among 1500 Irish men and women over 18 years of age, residing in the Republic of Ireland at the time the survey was conducted—we aimed to characterize breakfast regularity, identify dietary patterns associated with regular breakfast consumption, and assess the nutritional quality of such dietary patterns, using the nutrient-rich food index score NRF9.3. We determined two breakfast regularity categories and assessed dietary quality, by means of adherence to the principal component analysis derived dietary patterns and the NRF9.3 dietary index. Regular breakfast consumers were identified as those who consumed breakfast 3–4 times out of the 4 days of the collection period; such consumers comprised the majority of the population (94.4%). They had the highest adherence to healthier dietary patterns, namely, the “vegetarian” (odds ratio (OR): 2.59: 95% Confidence Interval (CI): 1.40, 4.77), “fish and vegetables” (OR: 2.88: 95% CI: 1.63, 5.10), and “breakfast cereals” (OR: 4.62: 95% CI: 2.43, 8.79) dietary patterns. Breakfast significantly contributed to the daily micronutrient intake by providing, on average, 24% of dietary fiber, 32% of iron, 30% of calcium, 32% of folate, and 37% of riboflavin. The importance of regular breakfast consumption on those who skip breakfast should be highlighted, in order to improve compliance with nutritional recommendations and adherence to a healthy lifestyle.

BackgroundGypsies, Travellers and Roma (referred to as Travellers) are less likely to access health services, including immunisation. To improve immunisation rates, we need to understand what helps and hinders individuals in these communities in taking up immunisations.Aims(1) Investigate the barriers to and facilitators of acceptability and uptake of immunisations among six Traveller communities across four UK cities; and (2) identify possible interventions to increase uptake of immunisations in these Traveller communities that could be tested in a subsequent feasibility study.MethodsThree-phase qualitative study underpinned by the social ecological model. Phase 1: interviews with 174 Travellers from six communities: Romanian Roma (Bristol); English Gypsy/Irish Traveller (Bristol); English Gypsy (York); Romanian/Slovakian Roma (Glasgow); Scottish Showpeople (Glasgow); and Irish Traveller (London). Focus on childhood and adult vaccines. Phase 2: interviews with 39 service providers. Data were analysed using the framework approach. Interventions were identified using a modified intervention mapping approach. Phase 3: 51 Travellers and 25 service providers attended workshops and produced a prioritised list of potentially acceptable and feasible interventions.ResultsThere were many common accounts of barriers and facilitators across communities, particularly across the English-speaking communities. Scottish Showpeople were the most similar to the general population. Roma communities experienced additional barriers of language and being in a new country. Men, women and service providers described similar barriers and facilitators. There was widespread acceptance of childhood and adult immunisation, with current parents perceived as more positive than their elders. A minority of English-speaking Travellers worried about multiple/combined childhood vaccines, adult flu and whooping cough. Cultural concerns about vaccines offered during pregnancy and about human papillomavirus were most evident in the Bristol English Gypsy/Irish Traveller community. Language, literacy, discrimination, poor school attendance, poverty and housing were identified by Travellers and service providers as barriers for some. Trustful relationships with health professionals were important and continuity of care was valued. A few English-speaking Travellers described problems of booking and attending for immunisation. Service providers tailored their approach to Travellers, particularly the Roma. Funding cuts, NHS reforms and poor monitoring challenged their work. Five ‘top-priority’ interventions were agreed across communities and service providers to improve the immunisation among Travellers who are housed or settled on an authorised site: (1) cultural competence training for health professionals and frontline staff; (2) identification of Travellers in health records to tailor support and monitor uptake; (3) provision of a named frontline person in general practitioner practices to provide respectful and supportive service; (4) flexible and diverse systems for booking appointments, recall and reminders; and (5) protected funding for health visitors specialising in Traveller health, including immunisation.LimitationsNo Travellers living on the roadside or on unofficial encampments were interviewed. Previous research has continuously identified a need for a comprehensive model of working with first mothers in adoption. This gap in knowledge has hindered the development of services, to the detriment of the quality of life and well-being of this group. This study seeks to remedy this deficiency by exploring the lived experiences of Irish mothers who have lost a child through closed adoption. It aims to expand understanding of the impact of such loss, in particular exploring how it affects their journey of reconnection and reunion with their child in adulthood. Semi-structured interviews were undertaken with six women from Ireland who had lost a child in this way and their narratives were analysed using Interpretative Phenomenological Analysis (IPA). Two superordinate themes emerged from the data: ‘Motherhood concealed: the loss of autonomy and connection’ and ‘Motherhood revealed: a pathway towards autonomy and reconnection’. Within each superordinate theme, two subordinate ones were identified: ‘Dehumanised, disempowered and dismissed’, ‘Coping through disconnection’, ‘Breaking the silence’ and ‘Reunion and reconnection’. The first two of these encapsulate the loss of personal autonomy and connectedness resulting from the loss of a child and how participants coped with it. The two others describe the participants’ experiences of growth in personal autonomy and agency through the process of breaking the silence of their concealed motherhood and of reconnection and reunification with their now adult children. The findings indicate that engaging in therapeutic services and/or with adoption peer support enables participants to express their feelings of grief and move from coping strategies dominated by disconnection to ones marked by the autonomous processing of emotion and new coping patterns. The process of reunification with their children plays a significant role in this transition.

This article asks how anti-abortion discourses and dialogues engaged with ideas about motherhood, national identity, and women’s reproductive decision-making in 20th-century Ireland, particularly from 1967, when abortion was decriminalized in Britain, to 1983, when Ireland’s Eighth Amendment became the law of the land. It assesses the ways in which ‘pro-life’ advocates rejected the notion that women were independent adults capable of reproductive decision-making. Indeed, throughout the 1960s, 1970s, and 1980s, anti-choice activists defined all Irish women as innately innocent, moral, and naturally desirous of domesticity and motherhood. Abortion, they argued, was encouraged, coerced, and even forced by outsiders or ‘others’. The arguments of some anti-abortion activists utilized meaningful themes in Ireland’s colonial and nationalist history, including the historical notion of Irish sacrificial motherhood, the depiction of Irish women as young and vulnerable, and the explanation of abortion as foreign, anti-Irish, and reminiscent of British colonial repression.

AbstractDietary vitamin K1(phylloquinone) levels that are sufficient to maintain normal blood coagulation may be sub-optimal for bone, and habitual low dietary intakes of vitamin K may have an adverse effect on bone health. The objective of the present study was to measure the intake and adequacy of phylloquinone intake and the contribution of foods to phylloquinone intake in a nationally representative sample of Irish adults. The North/South Ireland Food Consumption Survey database was used, which contains data collected using a 7 d food diary in a randomly selected sample of Irish adults aged 18–64 years (n1379; 662 men and 717 women). Phylloquinone intakes were estimated using recently compiled food composition data for phylloquinone. The mean daily intake of phylloquinone from food sources was 79 (sd 44) μg. Intakes were significantly higher (P>0·001) in men than in women at levels of 84 and 75quest;μgsol;d. The main contributors to phylloquinone intakes were vegetables (48%), particularly green vegetables (26%). Potatoes (including chipped and fried potatoes), dairy products and fat spreads contributed 10% each and meat contributed 8%. In men, social class and smoking status influenced phylloquinone intakes. Of the population, 52% had phylloquinone intakes below 1 μg/kg body weight and only 17% of men and 27% of women met the US adequate intakes of 120 and 90 μg/d, respectively. The present study shows that habitual phylloquinone intakes in Irish adults are low, which may have implications for bone health.

In the present paper the prevalence of obesity (BMI 30 kg/m2) and current physical activity levels in Irish adults have been evaluated. The prevalence of obesity in Irish adults is currently 18 %, with men at 20 % and women at 16 %. A further 47 % of men and 33 % of women are overweight (BMI 25·0–29·9 kg/m2). Since 1990, obesity has more than doubled in men from 8 % to 20 %, and increased from 13 % to 16 % in women. The highest prevalence of obesity (30 %) was found in women aged 51–64 years. Defined waist circumference action levels identified 48 % of the population who are in need of weight management and who also are at a 1·5–4·5 times increased risk of having at least one cardiovascular disease risk factor. Physical activity levels were low overall. Men were more active in work and recreational pursuits than women, but women were more active in household activities. Walking was the most popular recreational pursuit. However, TV viewing occupied most of the leisure time of men and women. Higher levels of activity were associated with a lower BMI and waist circumference. The results indicate the need for sensitive and individualised strategies to promote physical activity and to achieve a healthy weight status.

The objective of this work was to quantify the impact of the voluntary fortification of foods on dietary intakes of vitamins and minerals of Irish adults. Foods that were voluntarily fortified were identified and pre- and post-fortification levels of micronutrients were determined from data supplied by manufacturers and food composition tables. Using food consumption data in 1379 adults aged 18–64 years, estimated using a 7-d food diary during the North/South Ireland Food Consumption Survey, intakes of micronutrients were determined, both including and excluding the fortification component in foods. Of approximately 3000 foods recorded as consumed, 1·9 % were fortified, mainly breakfast cereals and beverages. Median micronutrient content of fortified foods (FF) ranged from 18–33 % EC RDA per typical serving. Among consumers (65 % of men, 68 % of women), FF contributed, on average, 3·9 % (men) and 5·0 % (women) to mean daily intake (MDI) of energy. Relative to their contribution to MDI of energy, FF contribute a greater % MDI for Fe (men 16, women 19), folate (men 18, women 21), vitamins B1(men 14, women 16), B2(men 16, women 18), B6(men 12, women 15), D (men 5, women 11), B12(men 5, women 7) and niacin (men 10, women 12). Fortification significantly improved the adequacy of intake of some micronutrients, particularly of riboflavin, folate, vitamin D and Fe in women and did not contribute to an increased risk of adverse effects from excessive intake of any micronutrient.

Data for this analysis was based on the North/South Ireland Food Consumption Survey w7/15/2008hich used a 7 d food diary to collect food and beverage intake data in a representative sample of 1379 Irish adults (662 men and 717 women) aged 18–64 years. Foods contained in the database that are fortified were identified from the presence of vitamins and/or minerals in the ingredient list on the label. The results showed that an increased level of FF consumption was associated with lower intakes (percentage food energy) of total fat and saturated fat (women only) and higher intakes of total carbohydrate, total sugars (but not added sugars) and starch. Increased consumption was associated with a more micronutrient-dense diet and a reduced prevalence of dietary inadequacies of Ca, Fe, riboflavin and folate, particularly in women. Higher FF consumption was associated with higher intakes of fruit, lower intakes of alcohol and a lower likelihood of smoking in men and women. Thus it appears that FF consumption is a marker of both better dietary quality and healthy lifestyle behaviours.

Ireland used the Great Recession as an opportunity to pursue controversial reforms to lone parent social security payments while ignoring a significantly larger group of Irish women, qualified adults, the partners of coupled social welfare claimants. A review of the international literature about partners and recent policy trends is used to contextualise previously unpublished qualitative data capturing the practical experience of Irish partners and recent policy trends in relation to Irish partners. Despite the significant crisis-related rise in numbers of working aged qualified adults in Ireland, we see strong policy inertia in relation to this largely invisible group. The barriers to and lack of urgency for reform can be explained by several factors including fear of political backlash, ambiguous cultural norms about women’s roles, and practical capacity or lack of resources. The present policy architecture offers clear patriarchical dividends for male partners, employers and the policy elite, all of whom benefit from and remain attached to the concept of and the practice of a modified form of wifely labour. Policy avoidance cannot continue indefinitely, as reform of family-based welfare payments is central to resolving key policy problems including high participation rates and addressing child poverty. Options are briefly explored.

Objective: The objective of this study was to determine whether trajectories of older adults’ alcohol consumption are influenced by the following life course transitions, relationship status, employment status, and self-rated health. Method: Volume and frequency of drinking were harmonized across first three waves of The Irish Longitudinal Study on Ageing (TILDA; N = 4,295). Multilevel regression models were used to model frequency, average weekly consumption, and heavy episodic drinking. Results: Men and women drank more frequently over time, with frequency decreasing with age for women. Average weekly consumption decreased over time and with increasing age. Transitions in self-rated health, particularly those reflecting poorer health, were associated with lower frequency and weekly consumption. Heavy episodic drinking decreased with age. Men who were retired across all waves were more likely to engage in heavy episodic drinking at baseline. Discussion: Despite the decline in average weekly consumption and heavy episodic drinking, the observed quantities consumed and the increase in frequency of consumption suggest that older Irish adults remain vulnerable to alcohol-related harms.

ABSTRACTBackground: Hospital-treated deliberate self harm and suicide among older adults have rarely been examined at a national level.Methods: The Irish Central Statistics Office provided suicide and undetermined death data for 1980–2006. The National Registry of Deliberate Self Harm collected data relating to deliberate self harm presentations made in 2006–2008 to all 40 Irish hospital emergency departments.Results: Rates of female suicide among older adults (over 55 years) were relatively stable in Ireland during 1980–2006 whereas male rates increased in the 1980s and decreased in more recent decades. Respectively, the annual male and female suicide and undetermined death rate was 22.1 and 7.6 per 100,000 in 1997–2006. Male and female deliberate self harm was 3.0 and 11.0 times higher at 67.4 and 83.4 per 100,000, respectively. Deliberate self harm and suicide decreased in incidence with increasing age. Deliberate self harm generally involved drug overdose (male: 72%; female 85%) or self-cutting (male: 15%; female 9%). The most common methods of suicide were hanging (41%) and drowning (29%) for men and drowning (39%) and drug overdose (24%) for women. City and urban district populations had the highest rates of hospital-treated self harm. The highest suicide rates were in urban districts.Conclusions: Older Irish adults have high rates of hospital-treated deliberate self harm but below average rates of suicide. Drowning was relatively common as a method of suicide. Restricting availability of specific medications may reduce both forms of suicidal behavior.

Background. There is little population-based evidence on ethnic variation in the most common mental disorders (CMD), anxiety and depression. We compared the prevalence of CMD among representative samples of White, Irish, Black Caribbean, Bangladeshi, Indian and Pakistani individuals living in England using a standardized clinical interview.Method. Cross-sectional survey of 4281 adults aged 16–74 years living in private households in England. CMD were assessed using the Revised Clinical Interview Schedule (CIS-R), a standardized clinical interview.Results. Ethnic differences in the prevalence of CMD were modest, and some variation with age and sex was noted. Compared to White counterparts, the prevalence of CMD was higher to a statistically significant degree among Irish [adjusted rate ratios (RR) 2·09, 95% CI 1·16–2·95, p=0·02] and Pakistani (adjusted RR 2·38, 95% CI 1·25–3·53, p=0·02) men aged 35–54 years, even after adjusting for differences in socio-economic status. Higher rates of CMD were also observed among Indian and Pakistani women aged 55–74 years, compared to White women of similar age. The prevalence of CMD among Bangladeshi women was lower than among White women, although this was restricted to those not interviewed in English. There were no differences in rates between Black Caribbean and White samples.Conclusions. Middle-aged Irish and Pakistani men, and older Indian and Pakistani women, had significantly higher rates of CMD than their White counterparts. The very low prevalence of CMD among Bangladeshi women contrasted with high levels of socio-economic deprivation among this group. Further study is needed to explore reasons for this variation.

The contribution of food groups to DF and NSP intake is reported and fibre intakes are compared with dietary recommendations and with intakes in some European countries.DesignFood consumption was estimated using 7-day food diaries for a representative sample (n = 1379; 662 men, 717 women) of 18–64 year old adults in the Republic of Ireland and Northern Ireland selected from the electoral register. DF and NSP intakes were estimated from tables of food composition.ResultsThe mean daily intake of DF in the total sample was 20.2 g (standard deviation (SD) 7.8) [23-2 g (SD 8.5) in men, 17.4 g (SD 5.9) in women] or 2.24 g MJ−1 (SD 0.7) [2.16 g MJ−1 (SD 0.7) in men, 2.33 g MJ−1 (SD 0.7) in women]. Mean daily intake of NSP was 14.8 g (SD 6.2) [16.7 g (SD 6.8) in men, 13.0 g (SD 5.0) in women] or 1.65 g MJ−1 (SD 0.6) [1.56 g MJ−1 (SD 0.6) in men, 1.74 g MJ−1 (SD 0.6) in women]. The main food groups that contributed to mean daily intake of DF (NSP) in the sample were breads 31% (23%), potatoes 19% (23%), and vegetables 17% (19%). Absolute intakes of DF and NSP were higher (P < 0.001) in men than women; however, women overall consumed more (P < 0.001) fibre-dense diets than men. Women aged 18-35 years consumed less (P < 0.01) DF and NSP (g) than women aged 36-64 years. Both men and women aged 18–35 years consumed less (P < 0.01) fibre-dense diets than men and women aged 36–64 years. The NSP intake was below the nutritional goal of 18 g day−1 in 77% of adults and below the minimum of the recommended range (12 g day−1) in 37% of the total sample. Compliance (i.e. the maximum number of individuals whose collective mean daily intake corresponded to the population goal) with the (UK) population goal for an average intake of 18 g day−1 NSP was achieved by a greater proportion of the population (63%) than compliance with the (German) dietary fibre recommendation of 30 g day−1 (27%) or the (Nordic) recommendation of 3 g MJ−1 day−1 (33%).ConclusionDF (NSP) intakes were lower than dietary recommendations in a substantial proportion of the population. This is likely to contribute to impaired bowel function and constipation, which in turn may contribute to increased risk of chronic gastrointestinal disease.

Anthropometric screening has been recommended for the detection of undernutrition as it is simple, inexpensive and non-invasive. However, a recent study estimating the prevalence of undernutrition on admission to hospital in Dublin, Republic of Ireland, highlighted that the anthropometric reference data currently available in the UK and Republic of Ireland are inadequate to accurately determine nutritional status. In order to provide current anthropometric data, we carried out a cross-sectional study of 874 free-living, apparently healthy Irish-born elderly individuals aged over 65 years. Height, weight, triceps skinfold thickness, mid-arm and calf circumference were measured, values for BMI, mid-arm muscle circumference and arm muscle area were calculated and smoothed centile data derived for each variable. One-third of these elderly individuals had a BMI between 20–25 k/2, approximately two-thirds (68·5 % of males and 61 % of females) were classified as overweight or obese, almost one-fifth having a BMI over 30 k/2(17 % of men and 20 % of women). Very few were underweight, only 3 % having a BMI below 20 k/2. Height, weight, BMI and muscle reserves decreased with increasing age. The reduction in muscle size was associated with lower handgrip strength. Fat reserves declined with age in females only. Just over half of elderly Irish women reported participating in active leisure of 20 min duration four or more time/eek, although 13 % reported having no involvement in active leisure. These data for the Irish elderly extend the data generated from a recent countrywide survey of Irish adults aged 18–64 years, thus providing suitable reference standards for nutritional assessment of elderly Irish individuals.

Low phylloquinone (vitamin K1) intakes have been associated with low bone mineral density in older adults. Phylloquinone intakes and serum undercarboxylated osteocalcin (ucOC) levels were assessed in ninety-seven apparently healthy, free-living Irish women aged 50–75 years. Phylloquinone intakes were estimated using a detailed dietary history, which measured habitual food intakes from a typical 14 d period, and recently published food composition data for phylloquinone. Fasting serum ucOC was measured using an enzyme immunoassay. The median daily intake of phylloquinone in the group from all sources was 108·8 mg and from food sources only was 106·6 mg, indicating that approximately 99% of the phylloquinone came from food. Vegetables and vegetable dishes contributed 67% of the total phylloquinone intake, but further analysis showed that broccoli, cabbage and lettuce were the primary sources, making a total contribution of 44 %. Twenty per cent of the women had a phylloquinone intake below the UK recommendation of 1μg/kg body weight per day and 34% failed to meet the US Adequate Intake value of 90mg/day. Mean serum ucOC levels in the women were 6·2 (SD 1·7) ng/ml and were predicted by phylloquinone intake (β— 22·20, generated from log-transformed phylloquinone intake data; P=4 0·04). On the basis of comparisons with both UK recommendations and US A equate Intakes for phylloquinone, the habitual intakes of phylloquinone in a high proportion of Irish postmenopausal women may not be adequate.

The current use of nutritional supplements was assessed using a selfadministered questionnaire and respondents entered each supplement as it was consumed into the food diary.ResultsTwenty-three per cent of respondents regularly used nutritional supplements. Twice as many women used supplements as men. The intakes of micronutrients were significantly higher (P < 0.001) in supplement users than in non-users. Micronutrient intakes from food sources were similar in male users and non-users of supplements, but were significantly higher (P < 0.01) in female users, by 3 to 13%, for Fe, Mg, Mn, vitamins C and E and niacin than in non-users. The percentage of female users between 18 and 50 years who had mean Fe intakes below the average requirement (AR) (10 mg) decreased from 50 to 25 when the contribution from supplements was included. The use of supplements reduced the percentage of men who had mean intakes below the AR for Zn from 19 to 13, for riboflavin from 14 to 6 and for vitamin A from 20 to 5, and reduced the percentage of women with intakes below the AR for Ca from 23 to 16 and for riboflavin from 23 to 14. Twenty-one women out of 80 aged between 18 and 50 years, who consumed supplemental folate, achieved the intake of 600 |xg recommended to prevent neural tube defects. Twenty-two per cent of the women who took iron and 15% of the women who took vitamin B$ in supplemental form had mean daily intakes that exceeded that UL for these nutrients. Supplement users did not exceed the UL for the other micronutrients.ConclusionsSupplementation appears to be beneficial in promoting adequate intakes of some micronutrients, particularly Fe and folate in women aged 18–50 years and vitamin A in men. There appears to be little risk to supplement users of experiencing adverse side effects due to excessive intakes of micronutrients.

Weight, height, waist circumference, hip circumference and body composition were measured according to standard procedures.SettingNorthern Ireland and the Republic of Ireland, 1997–1999.SubjectsRandom representative sample of 1379 adults aged 18–64 years. Results: With the exception of body fat, all anthropometric values were significantly higher for men than women (P < 0.001). All measurements were significantly higher in the 36–50-year-old age group compared with 18–35 year olds. Height was the exception, which decreased significantly with age (P < 0.05). Weight, height and body mass index (BMI) have increased in Ireland since last measured in 1988 and in 1990. Over the last decade, obesity has increased in men 2.5 fold from 8% to 20% and in women by 1.25 fold from 13% to 16%. Significantly more women have a normal BMI than men (50.4% vs. 33.3%; P < 0.05). Cut-off points for a high waist circumference and high waist-to-hip ratio identified 47% and 33% of the population, respectively, to be at an increased risk for cardiovascular disease. Social class did not have any significant effect on mean BMI. Location of residence influenced BMI but not in any consistent manner. Ex-smokers had a significantly higher BMI than nonsmokers and smokers (P < 0.05).ConclusionsA revision of current recommendations for combating obesity is warranted to improve the health of the Irish population. Further research is needed to identify the factors that have contributed to the dramatic increase in the prevalence of obesity in men over the last decade and have resulted in a higher prevalence of obesity in men than in women.

A cross-sectional study of subjects, randomly selected using the electoral register, estimated habitual food intakes using a 7 d food diary in a nationally representative sample of 662 men and 717 women (not pregnant or lactating) aged 18–64 years. Consumers were classified into thirds, based on the distribution of mean daily intakes for red meat, white meat and processed meat. The mean intakes of red meat, white meat and processed meat were 51, 33 and 26 g/d respectively, and men consumed significantly more (P<0·001) than women for all meat types. In men, red meat consumption was associated with lower (P<0·001) prevalence of inadequacy for Zn, riboflavin and vitamin C intakes. Increasing processed meat intake was associated with a lower (P<0·01) level of compliance with dietary recommendations for fat, carbohydrate and fibre in men. Increasing processed meat consumption was associated with lower (P<0·01) wholemeal bread, vegetables, fruit and fish intakes in men and women. Managerial occupations were associated with lower processed meat intakes. It is important to distinguish between meat groups, as there was a large variation between the dietary quality in consumers of red meat, white meat and processed meat. Processed meat consumption is negatively associated with dietary quality and might therefore be a dietary indicator of poor dietary quality. This has important implications in nutritional epidemiological studies and for the development of food-based dietary guidelines.

Nationally representative data on 6105 community-dwelling adults aged > 50 years were analyzed. Follow-up data was obtained two years after cohort inception. Multivariable linear regressions and mediation analyses were used to assess the associations. Analyses were stratified by gender.ResultsBetter spousal relationship quality was protective against depressive symptoms and worry for men. For both genders, support from friends was protective against depressive symptoms, and better relationship quality with children was protective against depressive symptoms and worry. Social network integration was inversely related to depressive symptoms for men. Loneliness significantly mediated most associations (Tables 1–3).ConclusionsHigh quality spousal relationships and social integration appear to play a more central role for mental health among men than for women. For both genders, poor social relationships increase feelings of loneliness, which in turn worsens mental health. The iridocorneal endothelial (ICE) syndrome is a rare ocular disorder that includes a group of conditions characterized by structural and proliferative abnormalities of the corneal endothelium, the anterior chamber angle, and the iris. Common clinical features include corneal edema, secondary glaucoma, iris atrophy, and pupillary anomalies, ranging from distortion to polycoria. The main subtypes of this syndrome are the progressive iris atrophy, the Cogan-Reese syndrome, and the Chandler syndrome. ICE syndrome is usually diagnosed in women in the adult age. Clinical history and complete eye examination including tonometry and gonioscopy are necessary to reach a diagnosis. Imaging techniques, such as in vivo confocal microscopy and ultrasound biomicroscopy, are used to confirm the diagnosis by revealing the presence of “ICE-cells” on the corneal endothelium and the structural changes of the anterior chamber angle. An early diagnosis is helpful to better manage the most challenging complications such as secondary glaucoma and corneal edema. Treatment of ICE-related glaucoma often requires glaucoma filtering surgery with antifibrotic agents and the use of glaucoma drainage implants should be considered early in the management of these patients. Visual impairment and pain associated with corneal edema can be successfully managed with endothelial keratoplasty.

Abstract Background Food is an important part of older people’s life contributing to their physiological and mental well-being and to their satisfaction with life. The aim of this study was to investigate the factors that contribute to the satisfaction with food-related life of Irish community-dwelling adults older than 55 years. Methods A mixed methods approach was employed. Quantitative data were collected through a survey which tested how socio-demographic characteristics, food habits, mood and self-perceived health were correlated (Spearman correlation) with the 5 items from the satisfaction with food-related life (SWFL) questionnaire. Qualitative data were collected through open-ended questions which investigated the factors determining food satisfaction levels of the 81 adults (54 women) older than 55 years who participated in the study. Results The highest score (4.3/5) was given to the item from the SWFL questionnaire: “Food and meals are very important elements in my life”, and was regardless of gender, age and marital status of the participants. In contrast, the lowest score (3.9/5) was given to the item “My life in relation to food and meals is ideal”. Spearman’s correlation revealed that fruit consumption was positively correlated “With regard to food, the conditions of my life are excellent” (rho=0.29, p<0.05). The open-ended questions showed that sensory appeal was the most popular term related to food satisfaction levels. Conclusion Meals and food play an important role in older adults’ life; however, participants did not consider their life in relation to meals and food as ideal. Therefore, strategies need to be employed to improve food and meal experience for community-dwelling older adults to keep them healthy for longer. Food industry should focus on developing nutritious and sensory-appealing food for the older consumers. Finally, the finding that consumption of fruit leads to satisfaction with food-related life could be used as a health promotion message to change eating behaviour.

The present study used data from two consecutive waves of the Irish Longitudinal Study on Ageing (TILDA) survey. Multimorbidity was assessed at Wave 1 (2009–2011) and was defined as the presence of at least two chronic conditions. Falls occurring at Wave 2 (2012–2013) were self-reported. Mediating variables considered were polypharmacy, cognitive impairment, sleep problems, pain, low handgrip strength, difficulty in activities of daily living (ADL), obesity, and underweight. Multivariable binary logistic regression and mediation analysis using the Karlson Holm Breen method were conducted. This study included 6900 adults aged ≥50 years (51.6% women; mean [SD] age 63.1 [8.9] years). Compared to no chronic conditions at baseline, there was a positive and significant association between multimorbidity and falls at follow-up, with ORs ranging from 1.32 (95% CI = 1.06–1.64) for 2 conditions to 1.92 (95% CI = 1.54–2.38) for ≥4 conditions. Pain (23.5%), polypharmacy (13.3%), and difficulty in ADL (10.7%) explained the largest proportion of the multimorbidity-fall relationship. Multimorbidity increased risk for incident falls in older adults from Ireland. Interventions should be implemented to reduce fall risk in people with multimorbidity, especially targeting the identified mediators.

Of 742 food codes that contained meat, 320 were codes for meat consumed as an individual portion and 422 were composite foods and were disaggregated to estimate the meat content.SubjectsA nationally representative sample of 475 men and 483 women (not pregnant or lactating) from the Republic of Ireland aged 18–64 years.ResultsThe mean intake of meat was 134 g day−1in consumers (98.5%) and men (168 g day−1) consumed significantly more (P< 0.001) than women (102 g day−1). Mean intakes of meat were higher in subjects with manual skilled occupations (P< 0.01) and lower in those with third-level educational qualifications (P< 0.05). Without disaggregating meat from composite foods, meat intake was overestimated by 43% (57 g day−1) and varied widely by meat category. Meat disaggregated from composite foods contributed 25% of meat intake. The contribution meat made to nutrient intakes ranged from 29% for protein, vitamin B12, zinc and niacin to 20% for vitamin D, 16% for vitamin B6, 15% for thiamine and 14% for iron.ConclusionsFailure to disaggregat meat from composite foods substantially overestimates meat intake, with a large variation between meat categories. This has important implications for estimates of meat intakes in nutritional epidemiological studies and for food safety purposes.

The mean daily intakes of vegetables, fruit and potatoes from composite foods were 37 g (26%), 6 g (5%) and 17 g (7%), respectively. The mean intake of vegetables from composite foods was unrelated to age or gender, but increased with increasing social class and level of education attained. The proportions of men and women meeting the recommendation for ≥400g day−1(5 servings of 80 g per day) of vegetables and fruit were 21% (15% excluding composite foods) and 19% (12% excluding composite foods), respectively. Compliance with the dietary recommendation decreased with decreasing levels of educational attainment and social class.Conclusion:Intakes of vegetables and fruit are low compared with current dietary recommendations, particularly in those of lower levels of educational attainment and social class. Composite foods are an important source of vegetables (less importantly of fruit) and should be included when estimating vegetable intakes. Failure to do so may result in bias in estimates of intake and of compliance with dietary guidelines for population groups, as well as misclassification of individuals by level of intake.

PurposeThe purpose of the Irish dual-energy X-ray absorptiometry (DXA) Health Informatics Prediction (HIP) for Osteoporosis Project is to create a large retrospective cohort of adults in Ireland to examine the validity of DXA diagnostic classification, risk assessment tools and management strategies for osteoporosis and osteoporotic fractures for our population.ParticipantsThe cohort includes 36 590 men and women aged 4–104 years who had a DXA scan between January 2000 and November 2018 at one of 3 centres in the West of Ireland.Findings to date36 590 patients had at least 1 DXA scan, 6868 (18.77%) had 2 scans and 3823 (10.45%) had 3 or more scans. There are 364 unique medical disorders, 186 unique medications and 46 DXA variables identified and available for analysis. The cohort includes 10 349 (28.3%) individuals who underwent a screening DXA scan without a clear fracture risk factor (other than age), and 9947 (27.2%) with prevalent fractures at 1 of 44 skeletal sites.Future plansThe Irish DXA HIP Project plans to assess current diagnostic classification and risk prediction algorithms for osteoporosis and fractures, identify the risk predictors for osteoporosis and develop novel, accurate and personalised risk prediction tools, by using the large multicentre longitudinal follow-up cohort. Furthermore, the dataset may be used to assess, and possibly support, multimorbidity management due to the large number of variables collected in this project.

With surfing debuting at the 2021 Tokyo Olympics (postponed from summer 2020 due to the COVID 19 global pandemic) it is timely to consider surfing and the national identifications women in Ireland may have with this sport. As Lee Bush states, ‘with so little scholarship on surfing women, descriptive studies are needed as a foundation for launching future interpretive and critical studies.’[1] Twelve women who surf in Ireland spoke about the links their surfing may or may not have with their national identity. Previous academic inquiry on links between national identity and sport on the island of Ireland has almost exclusively focused on men’s experiences of team sports and issues of ‘Irishness’.[2] ‘Irishness’ is globally recognised and stereotypically linked to traditional and indigenous Irish sports such as Gaelic football and a range of other cultural activities. Research into women’s sport participation has largely been restricted to the study of soccer in the Republic of Ireland,[3] and gendered evaluations of various lifestyle and health surveys.[4] Katie Liston, a key researcher in sport and gender relations in Ireland, highlights that ‘there seems to be an increasing diversity in the kinds of activities in which people participate in’,[5] and that there is a shift towards ‘lifestyle’ activities for adults as diversity increases in young people’s participation in sports and leisure activities. In Caitriona Lally's debut novel Eggshells (2015), the narrator Vivian Lawlor is an adult woman with a quirky personality living in North Dublin, who believes herself to be a changeling. Throughout the novel, Vivian travels various paths in Dublin looking for specific ‘thin places,’ creating ‘an alternative map of Dublin’, as Claire Kilroy's review puts it. Folklore is often used as a code for hiding aspects of Irish life that are unspeakable, and in Eggshells the changeling story is a coded testimony of family violence in which the changeling figure is labelled as nonhuman. Rejected by family, she looks to queer models of kinship as outlined by Judith Butler, through transformative portals and a companion who is a fellow trauma survivor. This essay argues that, while her experience of traumatic family violence is silently coded within the changeling story, Vivian strategically deploys changeling legend to embody a nonconforming gender presentation.

Method: Analyses were conducted on participants who completed Wave 1 of The Irish Longitudinal Study on Ageing (TILDA), a probability sample of individuals age &gt; 50 years. PS was measured by self-report and EF was measured by Color Trails 2 completion time and Color Trails 2 – Color Trails 1 difference in completion time. Participants with MMSE scores&lt;18 were excluded; the final sample included 5130 adults (54.3% women) with a mean age of 62.6 years. Results: Linear regression analyses indicated PS was significantly and positively associated with (a) Color Trails 2 completion time (b = 8.31, p &lt; 0.001); and (b) Color Trails difference in completion time (b = 5.03, p &lt; 0.001). These associations remained significant when adjusting for demographic covariates and depressive symptoms. Age moderated the association between PS and EF (ps &lt; 0.001), such that the strength of the association between PS and EF was stronger for older adults; the interaction remained significant when adjusting for demographics and depressive symptoms. Conclusions: Results suggest that (a) PS may negatively impact EF; (b) the association is independent of demographic characteristics and depressive symptoms; and (c) the negative impact of PS on EF may be more pronounced for older adults. Clinicians may consider assessing severity of PS when administering EF measures to account for psychological factors contributing to cognitive functioning.

AbstractThis study provides real-time support for the hypothesis, previously inferred from apparent time studies, that stable sociolinguistic variables are age-graded. Stable variables have been shown to exhibit a curvilinear pattern with age in which adolescents use nonstandard variants at a higher rate than adults do. An analysis of the morphophonological variable (ing) was carried out using recordings and ethnographic observations of 13 young American women during and after their final years of high school. Offering a detailed look at the late adolescent life stage, the study also explores speakers’ motivations for retaining or retreating from nonstandard variants as they prepare to enter adulthood. These are examined at both the group and the individual level. The results indicate that the degree of retreat from nonstandard variants is socially differentiated, in line with apparent time findings. Future enrollment in a locally oriented college, and alignment to a local ethnic network (Irish or Italian)—not social class—were the predictors of retention in high school.

Background: Socioeconomic position (SEP) is an important determinant of health and it is dynamic across the entire lifespan. We sought to investigate the relationship between life-course SEP and chronic kidney disease (CKD) using 3 conceptual models: critical period, pathway and accumulation. Methods: Cross-sectional analysis of 4,996 participants from The Irish Longitudinal Study on Ageing, a nationally representative cohort of community-dwelling adults aged ≥50 years. We defined childhood and adulthood SEP according to father’s and respondent’s occupation respectively. SEP was categorised as high (reference), intermediate, low and never worked. CKD was defined as a glomerular filtration rate < 60 mL/min/1.73 m2 estimated from the combination of creatinine and cystatin C. We used logistic regression to estimate the age-adjusted association between SEP and CKD separately in men and women. Results: Low childhood SEP was strongly associated with CKD in women, after adjusting for adulthood SEP (OR 1.90 [95% CI 1.24–2.92]), supporting the critical period hypothesis. This association was not explained by traditional CKD risk factors. Women who experienced low childhood SEP and whose circumstances improved in adulthood also had increased odds of CKD, further supporting a critical period effect in childhood. There was comparatively less evidence in support of the pathway or accumulation models. We did not observe a statistically significant association between SEP and CKD in men. Conclusions: Our findings suggest that women exposed to disadvantaged SEP in childhood represent an at-risk group in whom there may be opportunities for identification of CKD and facilitation of health-promoting behaviours from an early age.

AbstractObjective:To describe the consumption of ready-to-eat-breakfast cereals (RTEBCs) in Irish adults and its impact on adequacy and safety of micronutrient intakes and compliance with dietary recommendations.Design:Analysis for this paper used data from the North/South Ireland Food Consumption Survey that estimated habitual food intake using a 7-day food diary in a representative sample of adults aged 18–64 years (n=1379; 662 men, 717 women).Results:Despite the small quantity consumed (mean 28.6gday-1or 4.7% of total energy intake), RTEBCs made an important contribution to the mean daily intake of carbohydrate (8.1%), starch (10.8%), dietary fibre (9.8%) and non-starch polysaccharides (NSP) (10.8%) in consumers. Increased consumption was associated with a more fibre-dense diet and with greater compliance with dietary recommendations for fat, carbohydrate and NSP. Fortified RTEBCs contributed significantly to mean daily intakes of iron (18%), thiamin (14%), riboflavin (17%), niacin (15%), vitamin B6(13%), total folate (18%) and vitamin D (10%) and most of the contribution was from micronutrients added to RTEBCs. Increased consumption of fortified RTEBCs was associated with an increased nutrient density for a number of micronutrients and with a lower prevalence of dietary inadequacy of calcium, iron, riboflavin and folate, particularly in women. However, it was not associated with intakes in excess of the Tolerable Upper Intake Level for any micronutrient.Conclusions:The consumption of RTEBCs is associated with improved compliance with dietary recommendations for fat, carbohydrate and fibre, with a more micronutrient-dense diet and a reduced risk of dietary inadequacy for calcium, iron, riboflavin and folate, without increasing the risk of excessive intakes of micronutrients.

Diets were assessed with a standard FFQ. LCA, under the assumption of conditional independence, was used to identify mutually exclusive subgroups with different dietary patterns, based on food group consumption.SettingRepublic of Ireland.SubjectsMen and women aged 50–69 years at baseline (n 923) and at 10-year follow-up (n 320).ResultsThree dietary classes emerged: Western, Healthy and Low-Energy. Significant differences in demographic, lifestyle and health outcomes were associated with class membership. Between baseline and follow-up most people remained ‘stable’ in their dietary class. Most of those who changed class moved to the Healthy class. Higher education was associated with transition to a healthy diet; lower education was associated with stability in an unhealthy pattern. Transition to a healthy diet was associated with higher CVD risk factors at baseline: respondents were significantly more likely to be smokers, centrally obese and to have hypertension (non-significant).ConclusionsLCA is useful for exploring dietary patterns transitions. Understanding the predictors of longitudinal stability/transitions in dietary patterns will help target public health initiatives by identifying subgroups most/least likely to change and most/least likely to sustain a change.

Cystic fibrosis (CF) is the most common life-limiting autosomal recessive genetic disorder among Caucasian populations. The majority of CF cases are diagnosed in childhood; however, increasing numbers of adults are being diagnosed with the condition. We present the case of a 65-year-old Irish woman presenting with a chronic cough and a history of recurrent respiratory tract infections. Staphylococcus aureus, Scedosporium apiospermum and Stenotrophomonas maltophilia were grown from bronchoalveolar lavage raising suspicion for CF. Sweat testing was negative; however, genetic testing revealed the presence of ∆F508 and R117H CF mutations, the latter mutation conferring a milder form of CF. The patient commenced treatment with the cystic fibrosis transmembrane conductance regulator (CFTR) potentiator medication ivacaftor to good effect. Novel CFTR potentiators and modulators have significant potential to benefit morbidity and mortality in this group. In this case, the microbiological results were key in pursuing genetic testing and diagnosing CF.

ABSTRACTThe authors use Structural Equation Modelling techniques to analyse the determinants of wellbeing amongst older adults using data from the first wave of the Irish Longitudinal Study on Ageing (TILDA), a rich source of data on people aged over 50 and living in private households. The analysis uses a two-group linear statistical model to explore the influence of socio-economic position on the wellbeing of men and women, with Full Information Maximum Likelihood estimation to handle missing data. The fit indices for the final model are highly satisfactory and the measurement structure is invariant by gender and age. The results indicate that socio-economic position has a significant direct influence on wellbeing and a strong indirect influence which is mediated by health status and lifestyle. The total standardised effect of Socio-economic Position on Socio-emotional Wellbeing is statistically significant (p⩽ 0.05) and equal to 0.32 (men) and 0.43 (women), a very strong influence which risks being underestimated in standard multivariate models. The authors conclude that health, cognitive functioning and wellbeing reflect not just the ageing process, but also the impact of social inequalities across the lifecourse and how they are transmitted across different life spheres. These results can help to orient future research on factors which mediate between socio-economic position and wellbeing, an important policy-related issue.

Abstract Background Older adults with access to a car report increased social participation, better quality of life and better mental health. Existing research often compares the car to public transport, however we hypothesise that individuals who drive themselves or are driven by spouses have better outcomes compared to those driven by family or friends or taking public transport. This study examined how differences in transport mode affect psychosocial wellbeing in community-dwelling adults aged ≥50 years in Ireland. Methods Data were collected from 8092 adults during wave 1 of The Irish Longitudinal Study on Ageing (TILDA), a nationally representative cohort study. Participants indicated their most frequent mode of transport: car, car (driven by partner/spouse), car (driven by family/friends/taxi), public transport. Regression analysis was used to examine associations between transport mode with depressive symptoms, quality of life and loneliness. Results 89.8% of adults travel most frequently by car with 72.2% driving themselves. Driving was independently associated with lower depressive symptoms (beta=-1.9, 95% CI: -2.59, -1.30, p<0.001) and loneliness (beta =-0.45, 95% CI: -0.70, -0.21, p<0.001) and better quality of life OR=4.11, 95% CI: 3.13, 5.08, p<0.001) compared to relying on lifts from family/friends/taxi. Being driven by a spouse/partner or taking public transport were also associated with more positive effects although to a lesser extent. Interaction analysis showed that men who regularly travelled by public transport had higher loneliness scores than women (beta=0.67, 95% CI: 0.16, 1.18, p<0.05). Conclusion Most adults aged ≥50 years in Ireland rely on the car for transport. Driving, being driven by a spouse/partner or taking public transport are associated with better psychosocial wellbeing compared to being driven by others, highlighting the importance or independent travel, whether by car or public transport. Opportunities to support this should be considered when planning the future transport needs of ageing populations.

ObjectivesThe aim of this study is to estimate the prevalence and incidence of type 1 diabetes in the Irish population using a national pharmacy claims database in the absence of a national diabetes register.DesignNational, population-based, retrospective, cross-sectional study.SettingCommunity care with data available through the Health Service Executive Pharmacy Claims Reimbursement Scheme from 2011 to 2016.ParticipantsIndividuals with type 1 diabetes were identified by coprescription of insulin and glucometer test strips without any prolonged course (>12 months) of oral hypoglycaemic agents prior to commencing insulin. Those claiming prescriptions for long-acting insulin only, without any prandial insulin, were excluded from the analysis. Incidence was estimated based on the first claim for insulin in 2016, with no insulin use in the preceding 12 months.Main outcome measuresPrevalence of type 1 diabetes in children (<18 years) and adults (≥18 years); incidence of type 1 diabetes in children (≤14 years) and adolescents and adults (>14 years).ResultsThere were 20 081 prevalent cases of type 1 diabetes in 2016. The crude prevalence was 0.42% (95% CI 0.42% to 0.43%). Most prevalent cases (n=17 053, 85%) were in adults with a prevalence of 0.48% (95% CI 0.47% to 0.48%). There were 1527 new cases of type 1 diabetes in 2016, giving an incidence rate of 32 per 100 000 population/year (95% CI 30.5 to 33.7). There was a significant positive linear trend for age, for prevalence (p<0.0001) and incidence (p=0.014). The prevalence and incidence were 1.2-fold and 1.3-fold higher in men than women, respectively. Significant variations in prevalence (p<0.0001) and incidence (p<0.001) between the different geographical regions were observed.ConclusionsThis study provides epidemiological estimates of type 1 diabetes across age groups in Ireland, with the majority of prevalent cases in adults. Establishing a national diabetes register is essential to enable updated epidemiological estimates of diabetes and for planning of services in Ireland.
