Abstract:
The article is devoted to the thorough study of the inaugural address as a genre of public speeches on the material of the Brazilian political discourse. The political discourse of Brazil, unlike the discourse in English, German or other languages, has so far not attracted due attention of researchers, and the present paper is intended to fill the lacuna. The work presents the results of analysis of speeches of this genre of four Brazilian presidents: J. Sarney, L. da Silva, D. Rousseff and J. Bolsonaro. The article describes typical features and elements inherent in the genre of inaugural adress as a whole, the fundamental characteristic of which is its ritual character. A comparison of the speeches of the politicians chosen by the author for the study, despite the similarity of their content and the identity of the thematic blocks, gives an idea of the linguistic features and speech strategies of each particular president.

Abstract:
Soft-X-ray angle-resolved photoelectron spectroscopy (ARPES) with photon energies around 1 keV combines the momentum space resolution with increasing probing depth. The concepts and technical realisation of the new soft-X-ray ARPES endstation at the ADRESS beamline of SLS are described. The experimental geometry of the endstation is characterized by grazing X-ray incidence on the sample to increase the photoyield and vertical orientation of the measurement plane. The vacuum chambers adopt a radial layout allowing most efficient sample transfer. High accuracy of the angular resolution is ensured by alignment strategies focused on precise matching of the X-ray beam and optical axis of the analyzer. The high photon flux of up to 1013 photons s−1(0.01% bandwidth)−1delivered by the beamline combined with the optimized experimental geometry break through the dramatic loss of the valence band photoexcitation cross section at soft-X-ray energies. ARPES images with energy resolution up to a few tens of meV are typically acquired on the time scale of minutes. A few application examples illustrate the power of our advanced soft-X-ray ARPES instrumentation to explore the electronic structure of bulk crystals with resolution in three-dimensional momentum, access buried heterostructures and study elemental composition of the valence states using resonant excitation.

Abstract:
This article explores how business activities have impacted on human rights in the energy industry by looking at the implications of the involuntary resettlements in hydro power dam projects in the affected communities in Viet Nam. It argues that despite the government’s consideration of hydropower as a main avenue to meet the need for energy demands with huge potential benefit for the national economy, the development of hydroelectric projects in Viet Nam has had certain human rights concerns. The risk for human rights violation may occur at any stage before, during and after the implementing of dam development projects. However, the most affected group is the displaced people as the result of the involuntary resettlement process. While the government has issued some policies, strategies and programmes to affected communities, these responses, however, were insufficient to adress concerns and lacked a human rights based approach. This article finds that the people and community being displaced due to the construction of the dam have been facing a number of human rights concerns, including the violation of particular rights such as the right to livelihood/food security, right to land, and the right to culture.

Abstract:
The forecasting and reconstruction of oceanic dynamics is a crucial challenge. While model driven strategies are still the state-of-the-art approaches in the reconstruction of spatio-temporal dynamics. The ever increasing availability of data collections in oceanography raised the relevance of data-driven approaches as computationally efficient representations of spatio-temporal fields reconstruction. This tools proved to outperform classical state-of-the-art interpolation techniques such as optimal interpolation and DINEOF in the retrievement of fine scale structures while still been computationally efficient comparing to model based data assimilation schemes. However, coupling this data-driven priors to classical filtering schemes limits their potential representativity. From this point of view, the recent advances in machine learning and especially neural networks and deep learning can provide a new infrastructure for dynamical modeling and interpolation within a data-driven framework. In this work we adress this challenge and develop a novel Neural-Network-based (NN-based) Kalman filter for spatio-temporal interpolation of sea surface dynamics. Based on a data-driven probabilistic representation of spatio-temporal fields, our approach can be regarded as an alternative to classical filtering schemes such as the ensemble Kalman filters (EnKF) in data assimilation. Overall, the key features of the proposed approach are two-fold: (i) we propose a novel architecture for the stochastic representation of two dimensional (2D) geophysical dynamics based on a neural networks, (ii) we derive the associated parametric Kalman-like filtering scheme for a computationally-efficient spatio-temporal interpolation of Sea Surface Temperature (SST) fields. We illustrate the relevance of our contribution for an OSSE (Observing System Simulation Experiment) in a case-study region off South Africa. Our numerical experiments report significant improvements in terms of reconstruction performance compared with operational and state-of-the-art schemes (e.g., optimal interpolation, Empirical Orthogonal Function (EOF) based interpolation and analog data assimilation).

Abstract:

Abstract:
"The increase in regional instability, generated by the current Russian-Ukrainian conflict as well as the effects of the Covid-19 pandemic have highlighted the need for effective governance through the application of tools aimed at providing security to the citizens. The development of technology, which has led to unrestricted access to information, has also created the emergence of new types of threats, such as fake-news, propaganda or disinformation, threats that require punctual, flexible and adaptable methods to counter, diminish or eliminate. A very good knowledge of the specifics of the Romanian people and their perception of security risks is required. Starting from the premise that the mentioned threats have already manifested, resulting in a wrong perception of the population on security risks, a tool that has proven its effectiveness since Antiquity and that can be used by the state, through its institutions, could be represented by strategic communication. This article first addresses the issue of the population’s perception of security risks, then issues related to strategic communication, and as a final research direction, the way in which risk perception can be changed through StratCom."

Abstract:

Abstract:
Soil degradation is a complex phenomenon, involving several factors that contribute to the loss of fertility. Among these factors, the salinization of rice-growing land has become a concern for producers in the commune of Enampore. The objective of this study is to contribute to a knowledge of the environmental problems induced by salinity. More specifically, it is to identify the different adaptation strategies of producers. The methodology used is based on a questionnaire sent to 237 households out of a total of 579 households in 5 villages. The results showed the complex effects of the climate footprint in Enampore. The main factors cited by farmers were declining rainfall (46.6%) and the advance of the salt tongue (47.2%). Moreover, the presence of salt directly affects the rice fields (96.7%). In addition, the presence of caterpillars and insects (49.8%) and acidification (41.4%) are also a central problem. The main environmental consequence of salt is the formation of a salt crust on the surface (54.4%). This manifests itself on the rice by chlorosis (47.1%) and leads to their mortality (48.2%). In response to these problems, rice farmers are implementing various sustainable adaptation strategies. These include the installation of anti-salt dams and the use of organic fertilizers to improve yields. Thus, for a better management of the identified constraints, it would be interesting to conduct local experiments with the use of organic amendments (biochar and cashew compost) in order to propose more sustainable practices.

Abstract:
Abstract Background A higher life expectancy for women in Germany coincides with shortages in health professionals. Besides gynecologists (Gyn), the main providers of ambulatory healthcare services for women 50+ are general practitioners (GPs), which are not providing any gynecological services. The project “Frauen 5.0” aims to identify strategies for improving health care provision for women 50+ in three north-east states of Germany by analyzing health system indicators and exploring the perspective of service providers, users, practitioners and policy makers. Methods Mixed-methods combined with participatory research including 1) secondary data analysis (Robert Koch-Institute and Zentralinstitut); 2) postal quantitative survey of all Gyn (n = 1.031) and of a randomized sample of 66% of GPs (n = 3.514); 3) qualitative interviews of women 50 + (n = 25); and 4) round tables with practitioners and policy makers. Results In Germany 25% of the population are women 50+, from which 41% do not use Gyn services. Users expressed the need that GPs/Gyn actively address women’s health issues during consultation. The survey (response rate 51% Gyn; 25% GPs) revealed a high willingness to collaborate among GPs and Gyn. Barriers and synergies in organizational, professional and user aspects were identified. Task-sharing between GP and Gyn, delegation to non-medical personnel and the introduction of financial incentives showed to be a key element to improve health care services for women 50+. Dissusion In the SDG era of “leaving no one behind” acknowledging and addressing the unmet needs of women 50+ is from a public health perspective an issue of utmost urgency as is closing the gap between research, policy and practice. Conclusions New strategies to improve health care services for women 50+ were developed by integrating the evidence from actual data in meaningfull dialogues between research, policy and practice. Key messages New models of health care provision are required to adress and improve the health of women 50+. Closer collaboration between research policy and practice can be achieved by participatory methods.

Abstract:
Abstract Background Missing school can lead to lower educational achievement, early school dropout and health problems. To address sickness absence in secondary schools the intervention Medical Advice for sick reported students (MASS) has been developed. Because of fundamental differences in relation to the children's age and in the schools' organisational structure, the original MASS is not suited to primary schools. The aim of this study was to adapt MASS to primary education. Methods The intervention mapping approach steps 1 through 4 were used to adapt MASS to primary schools (MASS-PS), including a literature search, stakeholder interviews and pre-testing. Results In step 1, a planning group was formed and a logic model of the problem was created based on a literature search and interviews with stakeholders. In step 2, a logic model of change was created to determine the behavioural and environmental factors that need to change to achieve the goals of the intervention. In step 3, a theoretical basis and practical strategies to adjust MASS to primary education were determined. In step 4, practical support materials were developed and two pre-tests of the materials were performed with stakeholders and end-users. Conclusions Intervention mapping was successfully used to adapt MASS to primary schools. The main changes were the lowering of the threshold for extensive sickness, consultations between teacher and absenteeism coordinator and the addition of a social worker and remedial educationalist as experts next to the child and youth healthcare physician. The second pre-test did not yield any adaptations, indicating good acceptance in practice. With MASS, extensive sickness absence among primary school pupils can be addressed as a ‘red flag' for underlying problems. Key messages Intervention mapping steps one through four were sucessfully used to adapt a secondary school intervention to adress sickness absence among students (MASS) to primary schools. With the adapted MASS intervention, extensive sickness absence among primary school pupils can be addressed as a ‘red flag’ for underlying problems.

Abstract:
Abstract Background Digital interventions in health services often fail due to an underappreciation of the complexity of the implementation. This study develops an approach to address complexity through an evidenced-based, theory-driven education and implementation program for an Electronic Medical Record (EMR) digital enhancement for acute stroke care. Methods An action research approach was used to design, develop, and execute the education and implementation program over several phases, with iterative changes over time. The study involved collaboration with multiple statewide and local key stakeholders and was conducted across two tertiary teaching hospitals and a regional hospital in Australia. Results Insights were gained over five phases. Phase 1 involved a review of evidence that supported blended learning strategies for the education and training of staff end-users. In Phase 2, contextual assessment was conducted via observation of study sites, providing awareness of local context variability and insight into key implementation considerations. The Non-adoption, Abandonment, Scale-Up, Spread and Sustainability (NASSS) framework assisted in Phase 3 to identify and manage the key domains of complexity. Phase 4 involved the design of the program which included group-based training and an e-learning package, endorsed and evaluated by key leaders. Throughout implementation in Phase 5, further barriers were identified, and iterative changes were tailored to each context. Conclusions The NASSS framework, combined with a multi-phased approach employing blended learning techniques, context evaluations, and iterative modifications, can serve as a model for generating theory-driven and evidence-based education strategies that adresss the complexity of the implementation process and context.

Abstract:
Case study On May 9th, 2023, a U.S. Border Patrol detained a family of five near Brownsville, TX. During processing, one of the family members, an eight-year-old girl, ADRA, was noted to have sickle cell anemia and a heart disease condition. Five days after they arrived at the Donna Facility, on May 14th, ADRA displayed symptoms, including abdominal pain and fever, and tested positive for Influenza A. She was administered medication and transferred to a designated isolation unit at the Harlingen Border Patrol Station. Despite her deteriorating condition and her mother’s urgent requests for medical intervention, there were no documented consultations with an on-call physician or considerations for her transfer to a local hospital. On May 17th, ADRA’s health critically declined, marked by multiple visits to the medical unit for vomiting and abdominal pain. An ambulance was dispatched only after ADRA experienced a seizure and became unresponsive, Fig. 1. Her subsequent death was deemed a “preventable tragedy” attributed to systemic failures in the Border Patrol’s medical care and decision-making processes in a juvenile care monitor’s report.1 Impact This article adds to the existing literature by: Summarizing the gap in age-specific guidelines for six chronic diseases that occur in children and adolescents held in custody. Identifying the lack of adequate intervention strategies for acute management of chronic diseases for youth held in custody and strategies for improving health equity.

Abstract:
PurposeAdrecizumab, a non-neutralizing antibody of adrenomedullin (ADM) was recently investigated regarding its potential to restore endothelial barrier function in septic shock patients with high plasma ADM levels. Circulating dipeptidyl peptidase 3 (cDPP3), a protease involved in the degradation of several cardiovascular mediators, represents another biological pathway strongly associated with outcome in septic shock, although unrelated to ADM. Therefore, the prognosis of patients with elevated cDPP3 may not be influenced by Adrecizumab. Also, time until initiation of treatment may influence efficacy.ObjectiveTo evaluate effects of cDPP3-based enrichment on treatment efficacy of Adrecizumab.Materials and MethodsPost-hoc analysis of AdrenOSS-2, a phase-II, double-blind, randomized, placebo-controlled biomarker-guided trial of Adrecizumab.ResultsCompared to the total study cohort [HR for 28-day mortality of 0.84 (95% CI 0.53;1.31), p = 0.439], therapeutic benefit of Adrecizumab tended to be more pronounced in the subgroup of 249 patients with low cDPP3 (<50 ng/mL); [HR of 0.61 (95% CI 0.34;1.08), p = 0.085]. Median duration to study drug infusion was 8.5 h. In the subgroup of 129 patients with cDPP3 <50 ng/mL and an early start of treatment (<8.5 h after septic shock diagnosis) HR for 28-day mortality vs. placebo was 0.49 (95% CI 0.21–1.18), p = 0.105. In multivariate interaction analyses corrected for baseline disease severity, both cDPP3, as well as the cDPP3 * treatment interaction term were associated with a reduced HR for 28-day mortality in the Adrecizumab treated group; p = 0.015 for cDPP3 in univariate analysis, p = 0.025 for the interaction term between cDPP3 and treatment group. In contrast, treatment timing was not significantly associated with 28-day mortality in multivariate interaction analyses.DiscussionIn septic shock patients with high ADM levels, a further post-hoc enrichment strategy based on cDPP3 may indicate (with all the caveats to be considered for post-hoc subgroup analyses) that therapeutic efficacy is most pronounced in patients with lower cDPP3 levels.

Abstract:
Warnings and Prohibitions as Means of Exerting Influence on the Addressee in Seventeenth– Eighteenth-Century Ukrainian SermonsIn religious communication, psychological influence – as a result of which a person should change their subjective features (value orientations, ways of conduct, etc.) – is aimed at fostering compliance with religious norms. The tasks of a priest include religious education and correcting people’s behaviour, warning them against acts which contradict Christian values, in other words: preventing people from committing sins. This task is best achieved by means of verbal persuasion used in sermons. This paper offers a diachronic analysis of speech acts of warning and prohibition (preventives and prohibitives) on the basis of written monuments of the Ukrainian language: Ukrainian sermons from the seventeenth and eighteenth centuries. In the analysed sermons, they are used in order to achieve the aim of preventing sinful conduct. They refer both to everyday situations and to moral attitudes in general. In the texts under consideration, the semantics of warning and prohibition is conveyed using means of expression from different levels. At the lexical level, they are verbs with the general meaning ‘to warn’, ‘to be afraid’, and verbs of action creating a distance between the individual and sinful feelings, thoughts and actions: ‘to escape’, ‘to reject’. Words and phrases denoting cognitive processes play the role of discourse markers: ‘to know’, ‘to be aware of’, ‘to remind’.The speech acts of warning and prohibition are most frequently expressed with verbs in the form of negative imperative. One specific aspect of the use of preventives is that they are supplemented with recommendations which the addressee may accept of his/her own will. The preacher uses various rhetorical strategies to enhance the convincing function of warnings, such as references to widely known cases from the past (precedential phenomena), quotations from the Holy Scripture, and preventive exhortations. In order to better convince the congregation and urge them to follow the model of proper conduct, the preacher uses various means of expression: epithets conveying negative valuation, and stylistic figures: amplification, gradation, pairs of synonyms. The material under consideration makes it possible to conclude that the Ukrainian language of the seventeenth–eighteenth centuries had a considerable potential in terms of verbal persuasion, and opens prospects for the study of its dynamics. Do zadań kapłana należy wychowanie i poprawa ludzkich zachowań, ostrzeganie przed czynami i działaniami sprzecznymi z wartościami chrześcijańskimi, a więc zapobieganie grzechowi. Niniejszy artykuł przedstawia analizę diachroniczną dwóch typów aktów mowy o charakterze dyrektywnym: ostrzeżeń i zakazów (prewentywów i prohibitywów), przeprowadzoną na materiale zabytków piśmiennictwa ukraińskiego – XVII–XVIII-wiecznych kazaniach ukraińskich. Rolę znaczników dyskursu pełnią słowa i frazy oznaczające procesy kognitywne: ‘znać’, ‘wiedzieć’, ‘przypominać’. Badany materiał skłania do wniosku, że język ukraiński XVII–XVIII wieku miał znaczny potencjał w zakresie perswazji słownej i otwiera perspektywy badań nad dynamiką jej rozwoju.

Introduction: We sought to determine if an online continuing medical education (CME) activity could improve knowledge and confidence of endocrinologists related to new/emerging treatment options for pediatric growth hormone deficiency (GHD). Methods: The CME intervention comprised a 30-min online video-based roundtable discussion among 3 expert faculty with downloadable slides. The effects of education were assessed for learners completing all 3 knowledge pre- and post-assessment questions using a matched pre-/post-assessment design, with participants serving as their own controls. A paired samples t-test was conducted for significance testing on overall average number of correct responses and for confidence rating, and a McNemar test was conducted at the question level (5% significance level, P <.05). Confidence was assessed in a 4th question using a 5-point Likert scale. The activity posted September 29, 2022, and data were collected through November 20, 2022. Results: Within the 2 months of data collection, 176 endocrinologists participated in the activity, with 36 completing all questions. (To date, 182 endocrinologists have participated as of December 12, 2022) Overall, 22% of endocrinologist learners improved their knowledge and 31% more answered ALL questions correctly after education. On a question-level: 8% of endocrinologists demonstrated improvement and 56% demonstrated reinforcement at recognizing gaps with once-daily pediatric GHD treatment options; 8% of endocrinologists demonstrated improvement and 58% demonstrated reinforcement at identifying efficacy comparisons between current daily and new/emerging once-weekly treatment options; 8% of endocrinologists demonstrated improvement and 56% demonstrated reinforcement at selecting the best strategies for team-based care to improve education and adherence in pediatric patients with GHD; Confidence: Overall, 28% of endocrinologists self-reported increased confidence in understanding the mechanism of once-weekly GHD treatment options, for an average confidence shift of +100% in those who increased confidence (P<.05). Continued educational gaps: 36% of endocrinologists need additional education to appreciate gaps with once-daily pediatric GHD treatments; 33% of endocrinologists need additional education to understand efficacy of once-weekly GHD treatment options; 17% of endocrinologists need additional education regarding team-based care of pediatric patients with GHD. Conclusion: This study demonstrates the success of an interactive, 30-min online CME on improving knowledge confidence of endocrinologists regarding once-weekly GHD treatment options, with gaps identified for future education.
