Abstract:
Background: The literature on Minorities’ Diminished Returns (MDRs) have shown worse than expected health of the members of racial and ethnic minority groups particularly Blacks. Theoretically, this effect can be in part due to weaker effects of educational attainment on preventive care and disease management in highly educated racial and ethnic minorities. Objectives: The current study explored the racial and ethnic differences in the effect of baseline educational attainment on % adherance to the routine physician visits among middle-aged and older adults in the US. Methods: This is a prospective study with 24 years of follow up. The Health and Retirement Study (HRS: 1992-2016) included 10880 middle-aged and older adults who were Hispanic, non-Hispanic, Black or White. The independent variable was educational attainment. The dependent variable was adherance to the routine physician visits (%). Age, gender, marital status, income, health behaviors (smoking and drinking) and health (depression, self-rated health, and chronic diseases) were the covariates. Race and ethnicity were the focal moderators. Linear regression was used for data analysis. Results: Overall, higher educational attainment was associated with higher % of adherance to the routine physician visits over the course of follow-up, net of all confounders. Race showed a significant statistical interaction with educational attainment suggesting of a smaller effect of high education attainment on % adherance to the routine physician visits for Black than White middle-aged and older adults. A similar interaction could not be found for the comparison of Hispanic and non-Hispanic middle-aged and older adults. Conclusion: Educational attainment is associated with a larger increase in preventive and disease management doctor visits for White than Black middle-aged and older adults. This is a missed opportunity to improve the health of highly educated middle-aged and older adults. It is not race/ethnicity or class that shapes health behaviors but race/ethnicity and class that shape people’s prohealth behaviors. At least some of the racial health disparities is not due to low SES but diminished returns of SES.

This case study outlines the use of acceptance and commitment therapy with a client struggling with uncomfortable thoughts and emotions concerning his injury, impacting adherence to his rehabilitation plan. The aims were to increase psychological flexibility and decrease cognitive fusion to allow for greater adherence to his rehabilitation plan and support well-being. The client engaged in a series of one-on-one sessions, discussing the triflex within acceptance and commitment therapy through the strategy of “recognize, release, refocus.” The intervention included practice engaging with the present moment, exploring defusion techniques, clarifying values, and committing to subsequent values-driven behaviors. The effectiveness of the intervention was assessed by monitoring psychological flexibility, cognitive fusion, and feedback from the client’s physiotherapists. The trainee sport and exercise psychologist then provided reflections on the case.

Adapting users need to fulfill their requirements and delivering products to be on time within the planned cost, is critical matter that all software project managers (SPM) put the highest priority for it while considering the users satisfaction at the same time. Agile methodology is one of the solutions provided by software engineers (SE), to get the customers involved in the system development life cycle (SDLC) to avoid the risk nonconformance cost. Yet SPM’s still facing the nonconformance costs and the dynamic changes, and the root cause of the issue is not pointed on to find a solution for it. This undertaking research aimed at determining whether software developers understand scrum rules. In addition, how does this knowledge gab affect the software projects success from the project management perspective. Furthermore, the engagement studied the impact of lack of enough knowledge on the topic to project delivery. The collected data from the qualitative and quantitative methods, which was conducted with scrum teams who worked in the <span>health information system</span> (HIS), Educational solutions, and Governmental solutions has showed deviations in organizational practices and team conflicting, competition, and pressure as well as declined product quality.

Placenta accreta spectrum is very rarely encountered with ruptured uterus and is commonly seen in third trimester of pregnancy. Hereby, a case of placenta percreta with uterine rupture in second trimester of pregnancy is presented. 40 year old women with previous 2 LSCS presented in emergency department with ninteen weeks pregnancy and massive haemoperitoneum. Emergency laprotomy revealed uterine rupture alnong with placenta percreta for which obstetric hysterectomy was done. Although, a rare occurrence, obstetricians should consider patients placenta accreta spectrum in patients with previous surgeries presenting with haemoperitoneum and signs of hypovolemic shock in second trimester of pregnancy, considering the possibility of rupture at the site of placenta adherance.

Growth, yield, and fruit quality of 2 non- astringent persimmon (Diospyros Inaki) cultivars, Izu and Fuyu, were evaluated under subtropical conditions of coastal Australia. Although maximum tree yields for Izu and Fuyu were high (24 and 37 t/ha, respectively) the percentage of fruit suitable for export was low and variable (range: Izu 8.547%; Fuyu 2844%). Main fruit quality defects were wind rub, petal adherance marks, fruitfly stings, calyx cavity, sunburn, and bird damage. Without fruit thinning, both cultivars were susceptible to overcropping in some seasons, which resulted in a high percentage (>40%) of small fruit (<150 g). Most fruit (80% for Izu and 90% Fuyu) had Brix levels higher than the recommended export standard of 14�.

How comprehensive, however, should collective remedies under EU law be? This introductory article - as well as the other articles of this volume - explores the feasibility of a horizontal approach to the collective enforcement of EU law. While the hope for such an approach has been bolstered by the engagement of the EU institutions, the Commission's most recent 2013 Recommendation fails to significantly advance the development of collective remedies at EU level. Introduction: Chronic Kidney Disease causes sudden changes in the daily lives of patients and creates a great impact on their quality of life. Failure of adherence in Hemodialysis can lead to increased morbidity, mortality, cost, and burden on health care system. Objectives: This study was conducted to assess the quality of life and adherence to treatment of patients undergoing Hemodialysis. Methodology: A descriptive correlational study was carried out at Nobel Medical College Teaching Hospital, Biratnagar, among 96 patients who were under HD using convenience sampling technique. The data was collected through face to face interview for the period of 5 months. Quality of life and adherence to treatment were assessed using Kidney Disease Quality of Life Questionnaire (KDQOL) and End Stage Renal Disease Adherence Questionnaire (ESRD – AQ) respectively. The data was analyzed using t test, Spearman correlation statistics. Results In this study, the overall mean score of quality of life (QOL) of patients under HD was 48.9±13.7. More than half (54.1%) of the patients had moderate adherence to treatment while 31.2% had good adherence. Domains with the highest and lowest scores on quality of life and adherence to treatment were social domain and illness impact and adherence to HD treatment and diet restriction, respectively. There was statistically significant association between total score of quality of life and duration of HD (p≤0.05). Likewise, there was statistically significant association between adherence to treatment and marital status (p≤0.05), educational status (p≤0.05), ethnicity (p≤0.05), duration of illness (p≤0.05). Poor correlation (r= 0.273) was observed between quality of life and adherence to treatment. Conclusion: On average, the kidney disease component dimension of quality of life of patients under HD is higher and the overall level of adherence to treatment was moderate. Thus proper periodic counseling on illness and its long term impact, regular follow up as well as information on adherance to dietary measures are important to improve the quality of life and treatment adherance of patients under hemodialysis.

Abstract:
The aim of the study was to assess adherence to breast and cervical cancer screening ensuring compliance with ethical principles.Material and methods. A total of 1,015 women of target age groups, living in Almaty, the Republic of Kazakhstan, participated in breast and cervical cancer screenings. A structured questionnaire was used to collect data from each participant (1,239 questionnaires: 674 for breast cancer screening and 565 for cervical cancer screening). Two hundred and twenty-four women participated in two screenings. The survey was conducted immediately after the respondents had had a mammography and / or Pap test. The questionnaire included the socio-demographic characteristics of women, questions on awareness of screening, level of knowledge before and after screening, sources of information, feelings before screening, after screening and and also after the receipt of test results. Binary logistic regression was used for data analysis.Results. Sixty-one percent (95 % CI 57.7; 63.1) of women participated in screening for the first time, 39 % (95 % CI 36.4; 41.8) of respondents visited outpatient clinic for the second and the third time. Multivariable analysis revealed that the participation in screening was associated with the purpose of visit, awareness and confidentiality. Thus, women, who arrived at the outpatient clinic with purposes other than screening were 2.3 times less (OR=2 95 % CI 1.6; 3.1) likely to participate in next screening compared to those, who came to the clinic with the purpose of screening as well as those, who were referred by employers (OR=2 95 % CI 1.2; 3.2) or friend/family (OR=2 95 % CI 1.0; 4.3). Women, who had little information about screening as a whole and about screening test before it was taken, were 3 and 2 times less likely to come to screening again (OR=3 95 % CI 1.6; 5.9 and OR=2 95 % CI 0.8; 3.7, respectively). The confidentiality violation led to a decrease in chance (OR=3.5 95 % CI 2.2; 4.9) of being screened again for those women, who experienced nervousness and shame during the screening, and also indicated the presence of nonmedical people during the test.Conclusion. Measures aimed to ensure compliance to ethics in screening for breast and cervical cancer will improve the quality of screening programs in accordance with international standards, as well as increase the personal responsibility for active participation in screening.

Cardiac complications may present a particular problem following radiation treatment applied to the mediastinum and thoracic wall (and especially to the left breast). Exposure of the heart during radiotherapy increases the risk of ischemic heart disease occurring generally years after the treatment. The incidence of radiation cardiotoxicity depends on various factors related to oncological therapies and the patient (details of radiotherapy, age, gender, comorbidies, smoking habits, etc.). Until recently the majority of clinical studies reported increased cardiac morbidity in patients receiving radiation treatment of the chest wall and the breast. Due to modern methods, however, postoperative chest wall and left breast irradiation is much safer today than previously. In order to avoid cardiotoxity, adherance to clinical practice guidelines for chemo- and targeted therapy of breast cancer, use of the most advanced irradiation procedures, regular monitoring of patients, and close cooperation between cardiologists and ocologists are all recommended. ntibiotic sensitivity of Salmonella isolates was investigated, the isolates were sensitive to some antibiotics such as chloramphenicol, imipenem and most of them showed resistance to amoxicillin and naldixic acid. The hydrophobicity of Salmonella was investigated by applying BATH test (Bacterial Adherence To Hydrocarbons) using xylene. The range was between (21-90) %, the results showed that five isolates were hydrophobic including sal10 isolate. The surface layer proteins (SLP)of sal10 showed the ability to act as agglutinins by applying haemagglutination test with and without 0.5% mannose, also the bacteria seemed to possess pili -type1 because of its’ ability to agglutinate with human blood cells and yeast cells. The agglutination of the bacteria was inhibited in the presence of mannose.The isolate sal10 was adhesive to buccal epithelial cells in an average 52 cell/epithelial cell, while this average decreased to reach 24 cell/ epithelial cell in the presence of Saccharomyces boulardii which was able to inhibit 54% of the bacterial adherance.

Abstract:
A previously unknown gas-solid interacted power generation is developed using triboelectric effect. We designed an adhesive, gas-tight, and self-healing supramolecular polysiloxane-dimethylglyoxime–based polyurethane (PDPU) porous elastomer based on segmented oxime-carbamate-urea. It is an intrinsically triboelectric negative material with trapped air within closed voids, exhibiting ultrahigh static surface potential and excellent compressibility. This porous PDPU generates electricity from interactions between the trapped air and the elastomeric matrix under periodical compression. The positively charged trapped air (or other gas) dominates the tribo-electrification with PDPU, inducing electron transfer from gas to the solid polymer for electricity generation. The self-healable elastomer renders gas-solid interacted triboelectric nanogenerator, GS-TENG, with high stretchability (~1200%). The inherently adhesive surface enables adherance to other substrates, allowing mechanical energy harvesting from deformations such as bending, twisting, and stretching. GS-TENG promises a freestanding wearable functional tactile skin for self-powered sensing of touch pressure, human motions, and Parkinsonian gait.

Vapour-deposited and sputtered thin films are widely used in optical and microelectronic components. The optical, electrical and mechanical properties of such films depend primarily on microstructural features such as grain size, shape and crystallographic orientation, defect and impurity content, and substrate surface topology. The microstructure is determined by deposition conditions and especially substrate temperature andinclination relative to the source, metal deposition rate, and residual gaspressure in the chamber. These parameters affect the transition fromirregular, defect-laden, fibrous grains and rough film surfaces to smooth, highly reflecting surfaces and well-formed columns with little void space between them.Factors affecting the adherance of thin films to substrates are under study as part of a broad program on interfacial adhesion. Important parameters are internal stresses that drive spontaneous delamination and the fracture energy of the interface that resists crack growth. Internal stress levels in Cr films can be exceptionally high, so this metal is being studied extensively to determine the origin of tensile stresses in relation to microstructure.

Despite their low performance quality and aesthetics, non-standard bricks are commonly used in the construction of low-rise buildings and rural houses in Indonesia. These bricks are produced traditionnally in home run plants with little adherance to appropriate mix designs. These unfired soil lime bricks enter the market within 2 weeks of production, which inflicts potential loss to the buyers due to their low performance quality. The paper discusses strength improvements of these early age bricks through 2 methods. The first method is by adding the optimal amount of mixing water to the soil and lime mix. The second method is by adding untreated coconut fibers with a 2.5 cm fiber length as additional reinforcements. The effect of 3%, 5% and 7% fiber addition to the mechanical properties of bricks were investigated. Laboratory tests showed that higher compressive strengths, compared to that of traditional unfired soil lime bricks, could be achieved through the 2 proposed methods. The bricks reinforced with higher percentage of untreated coconut fibers have better modulus of ruptures.

Digital signal processing offers various approaches of sig-nal evaluation each having individual advantages and disadvantages regarding different power qualityvariables. As a result of incresing decentralized feed in of regenerative plats using inverters, the si-nusoidal fundamental of the power system is distorted and harmonics occur. For elimination of thoseunwanted signal components, a variety of methods is available. Supple- menting current research ac-tivites a laboratory model of an active filter using low budget prototyping hardware is developed andevaluated. Therefore, an experimental circuit containing a band pass filter for signal adjustment aswell as a PWM with amplification circuit for signal correction have been elaborated. Necessary cal-culations are performed by a standard Atmel microprocessor as used by Arduino Uno, including an8-bit analog to digital converter (ADC).

Palladium appears to be an important component in contact metallizations to p-type III-V semiconductors. Several successful ohmic contacts containing Pd have been fabricated to p-type GaAs; e.g. Au/Pd/Zn/Pd to GaAs. Palladium is easily deposited on and reacts readily with GaAs, at relatively low temperatures, to form binary and ternary compounds. The formation of these compounds helps generate Ga vacancies in GaAs, necessary for dopant incorporation. Palladium also aids in the wetting of metals, such as Zn to GaAs, improving their adherance to the semiconductor surface. For the same reasons expressed above, Pd may also prove to be a valuable component in contact metallizations to p- type InP. Very little work has been done on the interaction between Pd and InP, however. Krishnan et al have reported on the formation of a Pd InP ternary phase, which is predominantly amorphous with some signs of crystallinity. The interaction between Pd and InP, under annealing conditions usually encountered in contact fabrication, is discussed in this paper. Particular emphasis is put on one ternary phase that forms during annealing, i.e Pd2InP.

Pencarian literatur dengan menggunakan mesin pencarian online Sciencedirect, EBSCO, Proquest, google scholar dari tahun 2013-2019. Hasil:Penggunaan mHealth terdiri dari sms otomatis, panggilan telfon, dan video edukasi. Pemanfaatan dari berbagai jenis mHealth ini digunakan sesuai dengan kebutuhan dari pasien. Manfaat mHealth sms otomatis dan telfon yaitu sebagai pengingat minum obat dan jadwal kunjungan rutin. Video edukasi memberikan informasi mengenai TB dan motivasi dari kelompok sebaya. Kesimpulan: Penggunaan teknologi mHealthdapat meningkatkan kepatuhan pasien TB dalam pengobatan dengan metode pengingat minum obat, jadwal kunjungan rutin, edukasi, dan dukungan sosial. Rekomendasi: Beberapa manfaat penggunaan mHealth telah dibuktikan oleh beberapa studi di luar negeri. Untuk itu diperlukan studi lebih lanjut tentang intervensi pelayanan kesehatan seperti DOTS pada pasien TB menggunakan aplikasi mHealth di Indonesia.

Scavenger receptors are multifunctional integral membrane proteins that mediate the endocytosis of many different macromolecular polyanions and also participate in host defence reactions and cell adherance. In Atlantic cod (Gadus morhua L.), two intravenously injected scavenger receptor ligands, [125I]tyramine-cellobiose-labelled formaldehyde-treated serum albumin (125I-TC-FSA) and 125I-labelled N-terminal propeptide of type I procollagen (125I-PINP), distributed mainly to the heart. Cellular uptake was visualized by injections of fluorescently labelled FSA (FITC-FSA), which was recovered in discrete vesicles in endocardial endothelial cells of both heart chambers. Studies in vitro showed that radioiodinated FSA and PINP were endocytosed and degraded very efficiently by cultured atrial endocardial endothelial cells. Moreover, uptake of 125I-FSA was Ca2+-independent. Out of a range of unlabelled ligands, only the scavenger receptor ligands FITC-FSA, polyinosinic acid and, to a varying extent, FSA, acetylated low-density lipoprotein (AcLDL) and PINP, were able to compete with radioiodinated FSA, PINP or AcLDL for uptake in isolated endocardial cells. From our findings, we conclude that the endocardial endothelial cells are major carriers of scavenger receptors in cod. In addition, our results strengthen the hypothesis that these cells in cod play the same important function as that established for the scavenger endothelial cells of the mammalian liver.

Lactic acid bacteria (LAB) are the main group of probiotic bacteria that widely used in fruit juice fermentation products. Fruit juice contains antioxidants that can prevent free radicals. The objective of this study was to identify LAB isolated from pineapple juice that possess probiotic properties and to analyze the antioxidant activity of pineapple juice after being fermented with the LAB. LAB isolates from pineapple juice were evaluated for their probiotic properties (tolerance to acidic conditions and bile salts, antimicrobial activity against pathogenic bacteria, and sensitivity to antibiotics) and their adherence properties (autoaggregation, coaggregation, and hydrophobicity). The best isolates obtained are then inoculated into pineapple juice. From 5 types of pineapple juice, 5 isolates with LAB characteristics were obtained, i.e NHC6, NHC7, NHC8, NHC9, and NPC1. Isolate NHC6 had a good tolerance to acidic conditions (pH 2) and 0,5% bile salt, had antimicrobial activity, was sensitive to ampicillin, showed the best adherance properties and was identified as Lactobacillus plantarum. L. plantarum NHC6 was then added to pineapple juice. Addition of L. plantarum NHC6 to pineapple juice reduced the ᵒBrix and pH values during incubation. After 24 hours of incubation, the antioxidant activity was 89,05%, with LAB growth of 8,33 log CFU/mL, thus 24 hours incubation was considered the best fermentation time for pineapple juice.

Background: Treatment of tuberculosis involves giving multiple drugs to the patient which is done to prevent development of resistance besides targeting all bacterial sub-populations. The objective of the present study was to find out the adverse drug reactions (ADRs) of the first line anti-tubercular drugs and to calculate prevalence and causality of these drugs.Methods: The study was conducted by the Department of Pharmacology, Government Medical College, Srinagar in association with the department of Chest medicine. All patients of tuberculosis on directly observed treatment-short course (DOTS) of either sex or age group were enrolled. An assessment of the causality and allocation of ADRs was done using Naranjo’s monitoring scale and WHO-UMC scale.Results: A total of 57 ADRs with 13 different types were observed in 164 patients, with an overall prevalence of about 34.7%. Thirty seven (64.9%) ADRs were categorized as having a ‘probable’ causal relationship while 17 (29.8%) was categorized as ‘possible’ as per the Naranjo’s scale. As per the WHO-UMC monitoring scale fifty two (91.2%) were categorized as ‘possible’ while three (5.3%) were categorized as ‘certain’.Conclusions: Occurrence of ADRs generally results in non-adherance. Timely detection of ADRs through an effective pharmacovigilance programme is the need of hour. The establishment of an active pharmacovigilance centre which was set up in our institution has paved the way to improve the quality of patient care by ensuring safer use of drugs and has helped us to identify and report the various ADR’s encountered.

The effects of 2%, 4% and 6% fiber addition to the strength of bricks were investigated. The bricks reinforced with 4% fiber content resulted in better strengths compared to those reinforced with other percentage of fibers. The effect of fiber length uniformity to the bricks strength was also evaluated. A 4% non-uniform fiber addition, which constitutes 1/3 part each of 1 cm, 2.5 cm and 4 cm average fiber length, was investigated. The results show that unfired bricks added with uniform fiber resulted in better performances compared to those added with non-uniform fibers. Higher compressive and bending strengths, compared to those strengths of unfired plain soil lime bricks and traditional soil lime bricks, could be achieved through fiber addition.

Pasien dengan diagnosis hipertensi memerlukan kombinasi obat antihipertensi untuk mencapai target tekanah darah seperti Angiotensin Converting Enzym Inhibitor (ACEI), Angiotensin Reseptor Blocker (ARB), Calcium Channel Bloker (CCB), dan diuretik sehingga pasien harus minum lebih dari 2 macam obat (polifarmasi) hal ini merupakan salah satu faktor penyebab pasien cenderung tidak patuh dalam minum obat. Kepatuhan merupakan salah satu Drug Related Problems (DRPs) yang paling sering terjadi sehingga mengakibatkan kegagalan efek terapi. Kepatuhan minum obat bagi pasien penyakit kronis seperti hipertensi sangat penting karena dengan minum obat secara teratur dapat mengkontrol tekanan darah pasien, mengurangi resiko kerusakan organ akibat peningkatan tekanan darah dan munculnya penyakit komplikasi yang lain. Penelitian ini bertujuan untuk mengetahui tingkat kepatuhan pasien hipertensi serta hubungan antara tingkat kepatuhan minum obat terhadap efek terapi di RSUD dr.R. Soetrasno Rembang. Penelitian ini menggunakan metode deskriptif cross sectional. Data diperoleh dari rekam medik pasien meliputi data demografi pasien, data pengobatan pasien dan kepatuhan berdasarkan kuisioner MMAS-8(Morisky Medication Adherance Scale-8 ). Hasil yang diperoleh selanjutnya diolah menggunakan Chi square test SPSS.16. Hasil penelitian menunjukkan pasien yang mengalami interaksi obat dengan obat sebanyak 75,6%. Pasien yang termasuk kategori tidak patuh 34,1%, patuh rendah 34,1%, patuh sedang 21,9% dan patuh tinggi 9,9%. Berdasarkan hasil statistik di peroleh hasil P = 0.021 (P ? 0.05) maka dapat di simpulkan bahwa ada hubungan yang signifikan antara tingkat kepatuhan minum obat dengan pencapaian efek terapi yang di harapkan.

AbstractRelapse is a condition of getting back to illness after the recovery from illness. It results in stigma, high cost of treatments, and a decline in functionality both for patients and their families. It is common in mentally ill patients therefore this study attempted to assess lifetime prevalence and factors associated with relapse in a patient with mental illness. Institutional based cross-sectional study design was employed and simple random sampling techniques were used to select 178 study participants. Data were collected through face to face interviews by using a single item question to measure relapse. Data were entered by using Epi-data 3.1 software and exported to the statistical package for social science 22.0 software for analysis. Bivariate logistic regression analysis was conducted to see the association between dependent and independent variables and to identify significant variables associated with the outcome variable at P < 0.05 multivariable logistic regression analysis was done. One hundred seventy-eight mentally ill patients have participated in the study that yields a response rate of 100%. From all 125 (70.2%) of them had relapsed. Non adherance (AOR = 6.35, 95% CI, 2.524–15.852, P < 0.001), high disablity score (AOR = 3.728, 95% CI, 1.434–9.687, P = 0.007) and having single admission history (AOR = 0.196, 95% CI, 0.050–0.761 P = 0.019) were factors which indepedently associated with relapse of mentally ill patients. This study found more than two-thirds of patients had relapsed. Adherence to medication, functional disability, and having single admission were variables that predict the relapse of mentally ill patients.

AimsWe aimed to investigate the adherence to the Mental Health Act Code of Practice and the adequate documentation of consent to treatment across three forensic low secure inpatient units.MethodOur sample included all inpatients detained on three forensic wards at The Newsam Centre. This included a total of 31 patients with an age range of 25 to 59 years. The Mental Health Act Code of Practice was used as criteria for audit standards. Data were collected using Microsoft Excel and analysed using descriptive methods.ResultWe found that 28 patients out of 31 had been admitted for over three months and of these patients 12 were subject to a T2 and 16 subject to a T3. A total of 24 patients had their CTT medication list documented on the online drug chart; with a remaining seven patients who did not. As per guidelines, 27 patients had the appropriate medications prescribed as per their CTT, however one patient did not. The audit revealed a total of two patients currently on a Section 62. Of the qualifying T3 forms, four patients had this reviewed every two years whilst there was one patient who had not.ConclusionWe found that the adherence to Mental Health Act Code of Practice was overall positive with the majority of service users being reviewed appropriately and documented as per guidance. However, areas identified for improvement included the recording of CTT on online drug charts as well as reviewing T3 every two years. This audit highlights the need for easy access to guidance, appropriate documentation as well as frequent checking of adherence. A leaflet has been created outlining the guidelines and will be distributed to all staff working within the forensic settings and placed in easily accessible locations. As further recommendations from this audit we advise all wards to plan weekly checks during team meetings to ensure information is up to date and that all staff are aware of any discrepencies. A re-audit is planned in the coming months to re-assess adherance after implementation of the interventions.

The study was conducted at four villages of Sadar Upazila of Mymensingh district to study the management system and identify the probable causes of genetic draw off of indigenous cattle. The data were collected through personal interview of 151 randomly selected farmers by using a pre-tested structured interview schedule during the period from January to April, 2012. It was observed that the cattle per household was 2.98 in number. Among the farmers, 48% of them provided soft (soiled floor with bamboo fence), 48% semi-concrete (concrete floor with half of concrete wall and half of bamboo fence), 3% of concrete house (concrete floor and wall) and other 1% had no house for their cattle. About 80%, 17% and 3% farmers reared cattle by semi-intensive, intensive and extensive or free grazing system, respectively. More than 84% of the farmers managed feed from both sources (own and purchase) for their cattle even though 15% fully depended on purchasing feed from local market, but a little (1%) of them from their own sources. Among the cow genotypes, more than half (61.16%) was of the indigenous which was significantly (p<0.01) higher than the crossbreds (38.84%). It was observed that semen of Holstein were used significantly (p<0.05) higher than that of the semen of local or any other crossbred bulls. Most of the farmers (76%) inseminated their cows artificially rather than naturally (19%) and merely of 5% did both. A small number of farmers (11%) followed the existing breeding policy while maximum of them (86%) ignored or overlooked it. The main cause of genetic erosion of indigenous cattle in the study areas, might be due to lack of adherance to breeding policy and practice of insiariminate crosses with exotoc breeds. Nevertheless, lack of farmers’ awareness, changes in cattle management system along with agricultural practices might have also contributed to this issue as well. It needs to develop sustainable guidelines by the relevant authorities for proper breeding practices to minimize the uncontrolled and misdirected crossing between exotic and local cattle for conserving and protecting the valuable indigenous cattle in the region. 493 Background: Treatment paradigms for borderline resectable pancreatic cancer (BRPC) continue to evolve with increasing use of neoadjuvant chemotherapy (NAC) and chemoradiation (NACR). Variations in the definition of BRPC and in neoadjuvant approaches have made standardization of care for BRPC difficult. We report an effort to standardize management of BRPC throughout Sanford Health, a largely rural community oncology (CO) network in the Upper Midwest. Methods: Starting in February 2014, cases of PC without evidence of metastatic disease were categorized as BRPC if they met one or more of the following criteria: 1) abutment of superior mesenteric, common hepatic or celiac arteries with < 180o involvement 2) venous involvement deemed potentially reconstructable and/or 3) biopsy proven regional lymph node involvement. Patients with BRPC were treated with NAC followed by reimaging and surgery if resectable; if disease remained BRPC, patients underwent NACR and surgical re-evaluation. NAC was administered at 5 CO sites in the Sanford network; NACR and surgery were performed at 2 high volume centers. Results: Forty-three patients from 2/2014-4/2017 were diagnosed with BRPC. Median age of patients was 64 (range 39-83); 56% were male. 42/43 patients received NAC as initial therapy, with 1 receiving NACR due to patient preference. 90% of patients receiving NAC received mFOLFIRINOX (38/42), with FOLFIRI (1) and gemcitabine/nab-paclitaxel (3) also utilized. 11/42 (26%) patients had resectable disease after NAC and underwent surgery; 23 (55%) proceeded to NACR. Overall, 26/43 (61%) went to surgery with 18 (42%) able to undergo resection. Of those, 12/18 (67%) attained R0 resection and 10/18 (56%) were pathologic N0. Primary tumor location (head v. body/tail), age, pretreatment CA19-9, change in CA19-9 with treatment and NACR were not associated with a higher likelihood of resection. Conclusions: This report demonstrates the feasibility of implementing a standardized approach to BRPC across multiple CO sites over a wide geographic area. Adherance to protocol therapies was good and surgical outcomes are similar to many reported series; survival data is maturing and will be presented at the meeting.

Background:Mobile health applications are frequently used to increase exercise adherence in patients with musculoskeletal problems. However, the usability of these health mobile applications mostly has not been proven. In our previous study, the usability of the mobile app, which includes postural correction and neck spinal stabilization exercises, has been proven. (M.U-NeckExercise application)Objectives:The aim of this study is to evaluate the effectiveness of the mobile application-based home exercise program for patients with chronic neck pain.Methods:60 people with chronic neck pain were participated in the our study. They were randomized into two groups. The first group (n:30) received home exercises via the novel mobile app, and the second group (n:30) received the same home exercises via the brochure. The exercise program consisted of neck and thoracic postural correction and neck spinal stabilization exercises. Participants has been requested to do the exercises 3 days in a week for 6 weeks. Participants’ pain levels were assessed by using the Visual Analogue Scale (VAS), their neck-related functional limitations were evaluated by Neck Pain and Disability Index (BADI). Exercise adherence was meausered with a ratio of total participated sessions compared with the target defined by patient activation monitor and exercise adherance questionnaire. Targeted participation were %60 of total sessions.Results:In both groups, the improvement in VAS and BADI scores was statistically significant (p<0.05). It was found that the pain parameters of VAS score decreased statistically more in the mobile application-based exercise group (p<0.05). While there was no statistically significant difference between the groups in the BADI score, the effect size results was higher in the mobile app group (effect size= 0.411). Our exercise commitment target in the mobile app group was an average of 10.5 sessions. The target session number has been reached 11.25 sessions.Conclusion:It has been observed that the mobile application-based exercise program is effective in reducing pain and increasing exercise adherence in people with chronic neck pain. Abstract:
<p><strong><em>Background:</em></strong><em> Enterococcus faecalis and Fusobacterium nucleatum are the most common bacteria found in infected root canal teeth and most of them often caused failure in endodontic treatments. These bacteria can form biofilm which makes them more resistant against antibacterial agents. Biofilm formation also causes a decrease in antibiotics and antimicrobials sensitivity. Pluchea indica Less leaves is a species of plants that has several chemical properties. It consists of flavonoids and polyphenols which have benefits to inhibit biofilm formation. Because of its benefits, the extract of Pluchea indica Less leaves can be potentially developed as one of sterilization dressing in root canal teeth. <strong>Purpose: </strong>The aim of this study was to determine biofilm formation inhibition of Pluchea indica Less leaves extract against Enterococcus faecalis and Fusobacterium nucleatum. <strong>Materials and Methods: </strong>The dilution method was done first to show the Minimum Inhibitory Concentration (MIC) of the extract. The inhibition biolfilm formation was tested using microtitter plate assay by measuring the bacterial biofilm Optical Density (OD) from ELISA reader’s results and using autoagregation assay to show the inhibition of adherance bacteria. The Pluchea indica Less leaves extract concentration used for inhibition of biofilm formation were 100%, 50%, 25%, 12,5%, and 6,25%. <strong>Results:</strong> The result of biofilm formation inhibition showed that Pluchea indica Less leaves extract were able to inhibit Enterococcus faecalis and Fusobacterium nucleatum’ biofilm formation with strong moderate effect.
