Abstract:
Background: The literature on Minorities’ Diminished Returns (MDRs) have shown worse than expected health of the members of racial and ethnic minority groups particularly Blacks. Theoretically, this effect can be in part due to weaker effects of educational attainment on preventive care and disease management in highly educated racial and ethnic minorities. Objectives: The current study explored the racial and ethnic differences in the effect of baseline educational attainment on % adherance to the routine physician visits among middle-aged and older adults in the US. Methods: This is a prospective study with 24 years of follow up. The Health and Retirement Study (HRS: 1992-2016) included 10880 middle-aged and older adults who were Hispanic, non-Hispanic, Black or White. The independent variable was educational attainment. The dependent variable was adherance to the routine physician visits (%). Age, gender, marital status, income, health behaviors (smoking and drinking) and health (depression, self-rated health, and chronic diseases) were the covariates. Race and ethnicity were the focal moderators. Linear regression was used for data analysis. Results: Overall, higher educational attainment was associated with higher % of adherance to the routine physician visits over the course of follow-up, net of all confounders. Race showed a significant statistical interaction with educational attainment suggesting of a smaller effect of high education attainment on % adherance to the routine physician visits for Black than White middle-aged and older adults. A similar interaction could not be found for the comparison of Hispanic and non-Hispanic middle-aged and older adults. Conclusion: Educational attainment is associated with a larger increase in preventive and disease management doctor visits for White than Black middle-aged and older adults. This is a missed opportunity to improve the health of highly educated middle-aged and older adults. It is not race/ethnicity or class that shapes health behaviors but race/ethnicity and class that shape people’s prohealth behaviors. At least some of the racial health disparities is not due to low SES but diminished returns of SES.

Abstract:
This case study outlines the use of acceptance and commitment therapy with a client struggling with uncomfortable thoughts and emotions concerning his injury, impacting adherence to his rehabilitation plan. The aims were to increase psychological flexibility and decrease cognitive fusion to allow for greater adherence to his rehabilitation plan and support well-being. The client engaged in a series of one-on-one sessions, discussing the triflex within acceptance and commitment therapy through the strategy of “recognize, release, refocus.” The intervention included practice engaging with the present moment, exploring defusion techniques, clarifying values, and committing to subsequent values-driven behaviors. The effectiveness of the intervention was assessed by monitoring psychological flexibility, cognitive fusion, and feedback from the client’s physiotherapists. The trainee sport and exercise psychologist then provided reflections on the case.

Adapting users need to fulfill their requirements and delivering products to be on time within the planned cost, is critical matter that all software project managers (SPM) put the highest priority for it while considering the users satisfaction at the same time. Agile methodology is one of the solutions provided by software engineers (SE), to get the customers involved in the system development life cycle (SDLC) to avoid the risk nonconformance cost. Yet SPM’s still facing the nonconformance costs and the dynamic changes, and the root cause of the issue is not pointed on to find a solution for it. This undertaking research aimed at determining whether software developers understand scrum rules. In addition, how does this knowledge gab affect the software projects success from the project management perspective. Furthermore, the engagement studied the impact of lack of enough knowledge on the topic to project delivery. The collected data from the qualitative and quantitative methods, which was conducted with scrum teams who worked in the <span>health information system</span> (HIS), Educational solutions, and Governmental solutions has showed deviations in organizational practices and team conflicting, competition, and pressure as well as declined product quality.

In 2004 Art Therapy Lab was created to investigate viability of team approach in art therapy in psychiatry. Five projects were carried out between 2004–2010. These projects were based on cooperation between medical professionals, professional artists and members of user organization. During these projects adherance to ethical regulations was assesed, ethical difficulties and dillemas were noted and discussed. This paper will discuss main ethical problems that were relevant in those projects and provide recomendations on how to solve and prevent them.

Patient noncompliance to treatment regimen is a common problem for health service providers. Five personality variable subscales of the Millon Behavioral Health Inventory were related to trainer-judged adherence to treatment plans of 50 injured athletes. Multiple regression analysis showed only Somatic Anxiety was a significant predictor of adherence (r = -.427); injured athletes with high scores are less likely to adhere to treatment recommendations. Results suggest the promise of the MBHI as a diagnostic instrument for evaluating injured athletes and enhancing their treatment, and provide evidence for construct validity of the Somatic Anxiety subscale using a previously unstudied population.

Sexual victimization has been associated with significant negative health outcomes as well as increased healthcare utilization (e.g., Breiding et al., 2013; Sickel et al., 2002). However, due to fear, embarrassment, or confidentiality concerns (Logan et al., 2004; Nasta et al., 2005), women may mistrust medical advice and not adhere to treatment recommendations, exacerbating development of negative health outcomes. Some research (Meade et al., 2009) has identified sexual victimization as a predictor of medical nonadherence, but to our knowledge no research has examined the role of medical mistrust in this relationship. A sample of 857 women was recruited via social media. Regression analyses in R Markdown revealed that sexual trauma was a significant predictor of both medical nonadherence (b = 0.4, SE = .08, p < .0001) and medical mistrust (b = 1.36, SE = .027 p < .0001), and further that medical mistrust predicted medical nonadherence (b = 0.06 SE = .01, p < .0001). Additionally, medical mistrust was found to significantly mediate the relationship between sexual trauma and medical nonadherence (b = .09, SD = .03, 95% CI =.04, .14). Results emphasize the importance of trauma-informed care and patient-provider rapport in bolstering resiliency and strengthening survivors’ adherence to medical recommendations. Suggestions for further application and intervention will be discussed.

Objectives: To identify the proportion of HIV-related deaths that occur among persons who have never accessed antiretroviral therapy; model the potential public health impact of improved access to antiretroviral therapy; determine the role of socio-economic status on access and as a predictor of HIV disease progression; evaluate the impact of adherence to HAART on survival rates; and to determine rates of therapy discontinuation and virological response among HIV-infected injection drug users. Methods: British Columbia's HIV/AIDS Drug Treatment Program provides antiretroviral therapy to all eligible HIV positive persons in British Columbia free of charge. Data from the Centre as well as mortality data from Vital Statistics, socioeconomic data from Statistics Canada, and prescription refill data from pharmacies were used to evaluate and model mortality patterns and antiretroviral therapy use among HIV-infected individuals. Results: Overall, 32.7% of persons who have died of HIV/AIDS during the period 1995 - 2001 died without accessing antiretroviral therapy. Several markers of socioeconomic status were associated with non-receipt of therapy, and socio-economic status was associated with the receipt of sub-optimal therapy among those who initiated treatment. Models developed also indicate that improved uptake of antiretroviral therapy among injection drug users could have a substantial impact on life expectancy and mortality in this setting. Among patients who initiated triple therapy, rates of sub-optimal adherence are common, and are associated with markedly higher rates of mortality. In particular, patients with a history of injection drug use may be at risk of treatment cessation and poor virologic response. Conclusions: Limited access to antiretroviral therapy, particularly among certain sub-populations, has contributed on to ongoing AIDS mortality. Among persons who initiate therapy, the problem of limited access has been compounded by suboptimal adherence and prescription patterns, as well as high rates of treatment discontinuation. These data suggest that novel strategies are required to improve access and adherence to antiretroviral therapy despite a universal healthcare system that provides antiretrovirals and HIV/AIDS care free of charge.
