Abstract:
The Minnesota Model oftreatmentfor alcohol addiction is a common intervention that is widely employed in the UK. Despite its popularity, outcome evaluations ofthis approach are scarce. This study describes the Minnesota Model alcohol addiction treatment programme that is delivered at a non-statutory organisation in the UK andpresentsfindings from an evaluation of its outcomes. A single-group quasi-experimental pre-post design is employed that includes a no treatment andpartial treatment control group. The study population comprised all clients . (n=478) who were assessed at the organisation between May 2001 and October 2005. All potential participants were contacted to obtain their informed consent. In total, 137participants agreed to take part in the study and baseline information was obtainedfrom archivedpretreatment assessment data. Ofthese participants, there are three groups ofclients who varied in their level ofengagement with the organisation. GROUP A were assessed but did not participate further (n=18); GROUP B began the programme but were dischargedfor various reasons (n=31); and, GROUP C completed the treatmentprogramme (n=55). Outcome measures include drinking and non-drinking outcomes. Results show a highly significant (p<0.0001) association between completion oftreatment andcontinued abstinence. Atpost-treatmentfollow-up ofone to three or mOore years, 58% ofparticipants completing the programme, reported continuous abstinence from all psychoactive substances. This constituted a meanfollow-up period of2.07 yearsfor participants completing treatment. These findings are exciting and unparalleled in the alcohol addiction treatment outcome literature.

Abstract:
Abstract Addiction to drugs and alcohol is a widespread, and ever-growing problem in American society today. Individuals who undergo treatment for their addiction often find it difficult to gain employment due to employers' negative perceptions of addiction. Previous research has found that many employers have a stigma of those in addiction recovery. However, little research has been done to determine if these stigmas affect hiring decisions. Drug and alcohol misuse are prominent in the Appalachian area, which presents an issue for employers in the area who maintain a drug-free work place or who have a stigma of those in addiction recovery. The proposed study will assess employers’ attitudes towards applicants who have a history of substance misuse and/or are in treatment for substance misuse, with specific focus on self-reported likelihood of hiring an applicant who is in recovery. We hypothesize that employers will report a decreased likelihood of hiring individuals who are in recovery for substance misuse. Participants who are at least eighteen years of age and English-speaking will complete a survey on the REDCap web platform that includes a subset of questions from the Addiction Attitudes and Beliefs Scale (AABS). Items that will be used to assess employers’ attitudes were adapted from the Substance Use Stigma Mechanisms Scale (SU-SMS) and the Perceived Stigma Addiction Scale (PSAS). The proposed study is part of a larger study that is assessing attitudes and beliefs toward addiction among employers and within faith communities, as well as perceived stigmas experienced by those who are living with addiction or have a history of substance misuse, with particular emphasis on attitudes within the Appalachian Highlands community. Possible limitations of this proposed study include the lack of generalizability since employers in the Appalachian area may not be representative of the overall population. Another possible limitation is the use of self-report measures. Participants may not be willing to report accurately due to the sensitivity of the topic. If results of the proposed study support our hypothesis, further research should look at ways to reduce stigma and support employers in hiring those in addiction recovery. Existing research suggests that employment is vital for addiction treatment success and is associated with a decreased likelihood of relapse, making the need for the amelioration of this stigma imperative in dealing with the addiction crisis.

Abstract:
This thesis aims to investigate and deconstruct the relationship between the narcotic, its narrative, and western modernity. To reveal the relationship, this thesis argues that it is possible to understand the philosophical, political, cultural and ethical dimensions of western modernity through the ulterior lens of the narcotic. As such, this thesis investigates western modernity's relationship to (a) cocaine as a specific narcotic, and (b) the concept of the narcotic with all its attendant connotations of addictions, illegitimacy, transgression, illegality, and so on. Accordingly, the thesis is both interpretive of the historical narrative of the narcotic of cocaine, and generative in its deconstruction of the relationship between western modernity and the concept of the narcotic. The deconstruction of this relationship ultimately reveals both prior narratives not as oppositional, but as supplementary. This has radical consequences for the manner in which we engage with narcotic use and the user - if the narcotic is supplement to the logic of western modernity, at each attempt to expel the use and user of the narcotic, rather then create difference, we self implicate ourselves in that expulsion and distance. To seek a new and more just means of dealing with the concept of the narcotic, and its use, therefore requires a new epistemological framework which can at once contemplate both narratives at the same time. To this end, the thesis suggests the use of critical complexity theory as one such methodological tool, if supplemented by the thoughts and strategies of Derridian deconstruction and Foucauldian discourse analysis.

Both substance abuse and suicidal behavior are global public health concerns. Much of the progress made in addressing problematic substance use and suicidal ideation and behavior stems from the notion of alleviating pathological factors. Positive psychological characteristics, such as forgiveness, have received much less attention from empirical investigators. We review the extant literature pertaining to the value and role of forgiveness as an effective resource for clinicians when treating individuals struggling with substance abuse and suicidal behavior. We discuss relevant theory and research (i.e., definitions, processes, and linkages) regarding similarities in models of forgiveness, substance abuse, and suicidal behavior and conclude with an overview of various means of using the process of forgiveness as a positive psychotherapy; whether through stand-alone forgiveness interventions, infusion with Twelve-Step Facilitation Therapy, or application through acceptance-based treatment modalities. In sum, forgiveness may be an important factor in the facilitation of change in the difficult often existangst-derived struggles (i.e., emotionally and philosophically driven psychological distress) inherent to substance abuse and suicidal behavior.

Abstract:
Several studies have argued that congregations are better at bonding internally rather than bridging social capital. The critique suggests that most congregations tend to internally orient relationship building activities within the confines of their membership to the detriment of promoting activities that increase social capital through expanding the connections within communities, regions, and common causes. While this critique ignores the substantial collaborative work done to promote issues such as racial equality and right to life, it is often the case that congregations prefer a “go it alone approach” or defer to parachurch organizations when addressing pressing social issues such as addiction, behavioral health, and homelessness. Collaborative efforts involving multiple congregations do occur but they are relatively rare and often occur within churches of the same denomination. Even more rare is collaboration between multiple congregations and community not for profits (e.g., United Way agencies), municipal departments (e.g., police, public housing) and both public and private universities.

Abstract:
As a part of the overall program in the Grandy laboratory at Oregon Health & Science University (OHSU), studying the underlying chemical biology of methamphetamine (Meth) addiction, this dissertation reports on the development of six new thyronamine analogs which were synthesized and assayed against trace amine associated receptor 1 (TAAR1), giving preliminary results consistent with the analogs being inverse agonists. Due to highly variable TAAR1 expression levels in the assays, based on inter-assay response to control Meth stimulation as well as other possible factors, kinetic models were developed to qualitatively explain the assay results. The models set approximate limits on the analogs' binding and disassociation rates relative to those of Meth. Analysis of the assays also provides more evidence of TAAR1's basal activity. Based on the models, the conversion rate of ligand-free inactive TAAR1 to ligand-free active TAAR1 is less than 6% of the binding rate of Meth to TAAR1. The models also suggest that the inverse agonists bind to the inactive ligand-free form of TAAR1 between 10 and 100 times faster than Meth binds to the inactive ligand-free form of TAAR1. Three of the new analogs, G5-110s8, G5-112s5, and G5-114s5, bind to the ligand-free active form of TAAR1 faster than they bind to the inactive ligand-free form of TAAR1. The models do not suggest an upper limit on the binding rate of those 3 analogs to the ligand-free active form of TAAR1. A control assay lacking TAAR1 revealed an electrophysiological off-target effect caused by G5-109s8. Also, a novel synthetic route was developed for ET-92, the lead compound for this project, which reduced the number of synthetic steps from 14 to 5 and improved the overall yield from 15.3% to 18.3% (77.4 mg) with the hope that further improvements in yield are possible.

Abstract:
Salivary cortisol has been measured extensively in developmental research over the last three decades. The purpose of this article is to summarize the contributions to and limitations of salivary cortisol measurement in developmental research and propose future directions for research that includes salivary cortisol measurement. The properties of cortisol, the history of its burgeoning popularity, and the utility and limitations of (a) cortisol as a biological indicator, (b) saliva as a source of cortisol, and (c) various saliva collection methodologies are described. The current state of understanding about what is and is not reliably predictable from cortisol is summarized and the value of salivary cortisol measurement in developmental research is discussed, addressing whether methodology could be driving research design. Recommendations are made for streamlining study design and reporting within developmental research.

Many instruments have been developed to determine whether someone claims to be religious, but they do not address the degree to which someone is satisfied with their religious commitment. Therefore, an instrument was revised to measure both a person’s level of religious commitment and satisfaction with that level of religious commitment. The instrument was named the Religious Surrender and Attendance Satisfaction Scale (RSASS). This study was conducted to determine initial validity for the satisfaction portion of the RSASS. Construct validity measures provided initial confirmation of the utility of RSASS as a measure of satisfaction with religious commitment that can be used by nurses in practice and research.

ENGLISH ABSTRACT: Much has been documented about the detrimental effects of adverse lifestyle factor exposure on the body. Exposure to factors, such as cigarette smoke, have proved to not only be a burden on global health and economy, but have also led to growing concerns about effects on systemic functions such as reproduction. The aim of the present study was to determine the effects of in utero and in vitro nicotine exposure on spermatozoal function and the antioxidant enzyme activity and lipid peroxidation (LPO) status of the male reproductive system. A better understanding of this process is necessary to combat the respective burdens of smoking and male infertility and for the prospective development of treatment strategies. Two experimental models were employed: Wistar rats were exposed to nicotine in utero while human and rat spermatozoa were exposed to nicotine in vitro. In utero studies were achieved by selecting healthy pregnant rats and treating them with 1 mg/kg-bodyweight/day nicotine or 1 ml/kg-bodyweight/day 0.85% physiologic saline throughout gestation and lactation. Male rat pups were selected and sacrificed at each of the following age groups (n=6): 42 days, 84 days and 168 days old. The pups were only exposed to the treatment/saline via placental uptake or lactation. Biochemical analyses of the tissue comprised of measurement of LPO and antioxidant enzyme activity. Results indicated a significant association of maternal nicotine exposure to decreased levels of primary antioxidant enzymes in rat testes. Of particular note was the observation that the treatment group, of which each of the respective antioxidant enzyme levels were significantly less than the control group, was the oldest (d168) rat group. In vitro studies were achieved by collecting sperm samples from healthy human donors (n=12), healthy rats (n=6) and obese rats (n=6). Samples were washed and exposed to different concentrations of high levels of nicotine (Control, 0.1mM, 1mM, 5mM, 10mM) in vitro. Semen parameters such as motility, viability and acrosome reaction were monitored at different time points (30min, 60min, 120min, 180min). Results revealed increasing in vitro nicotine concentrations were associated with decreased viability and acrosomal status of human spermatozoa and decreased progressive motility and viability of rat spermatozoa. Obesity was also associated with decreases in progressive motility and viability of rat spermatozoa. These results indicate that the acute in vitro exposure of spermatozoa to high levels of nicotine could adversely affect semen quality and may be an additive factor to the impediment of male fertility. In utero results reveal maternal nicotine exposure adversely affects male fertility in later life and seems to elicit more detrimental effects on the reproductive system than that of direct nicotine exposure to spermatozoa. Obesity also inhibits parameters of male fertility and these effects are exacerbated by nicotine exposure. The authors believe these adverse effects on the reproductive system to be related to an increased activation of leukocytes, excess production in reactive oxygen species (ROS) and consequent onset of oxidative stress (OS). Nevertheless this study agrees with other studies that nicotine exposure may be an additive factor to the impediment of male fertility.

Menslike blootstelling aan sulke faktore, soos sigaret rook, is wêreldwyd ‘n las vir gesondheid en ekonomie en het gelei tot geweldige kommer onder navorsers oor die moontlike komplikasies vir liggaamlike funksies soos voortplanting. Die doel van die betrokke projek was om die effekte van in utero en in vivo nikotien blootstelling op die antioksiderende ensiem aktiwiteit en lipied peroksidasie status van reproduktiewe weefsel en die funksionele parameters van spermatozoa te bepaal. ‘n Beter begrip van hierdie proses is noodsaaklik om die las van rook en vetsug teen te werk en vir die moontlike ontwikkeling van behandelingsstrategieë. Twee eksperimentele modelle is ontwerp: Wistar rotte is in utero blootgestel aan nikotien terwyl mens- en rot- spermatosoë ook in vitro aan nikotien blootgestel is. Vir die in utero studie is gesonde dragtige rotte gedurende swangerskap en laktasie met 1 mg/kgliggaamsgewig/ dag nikotien of 1 ml/kg-liggaamsgewig/dag 0.85% fisiologiese soutoplossing behandel. Manlike welpies is gekies en geoffer op elk van die volgende ouderdomme (n=6): 42 dae, 84 dae en 168 dae. Die welpies is slegs aan nikotien blootgestel deur plasentale opname en laktasie. Biochemiese analise van die testikulêre weefsel het ‘n beduidende assosiasie getoon tussen maternale nikotien blootstelling en verminderde vlakke van die primêre antioksiderende ensieme. Die 168 dag oue groep het ‘n merkbare vermindering getoon tussen kontrole en nikotien weefsel vir elk van die antioksiderende ensieme. Vir die in vitro studie is sperm monsters verkry vanaf gesonde mans (n=12), gesonde rotte (n=6) en vet rotte (n=6). Monsters is gewas en in vitro blootgestel aan verskeie hoë vlakke van nikotien (kontrole, 0.1mM, 1mM, 5mM, 10mM). Seminale parameters soos motiliteit, lewensvatbaarheid en akrosoom status is by verskei tydpunte gemeet (30min, 60min, 120min, 180min). Dit blyk dat verhoging in in vitro nikotien konsentrasies verband hou met verlaagde lewensvatbaarheid en akrosoom status van menslike spermatosoë en verlaagde progressiewe motilteit en lewensvatbaarheid van rot spermatosoë. Vetsug is ook geassosieer met verlagings in progressiewe beweeglikheid en lewensvatbaarheid van rot spermatosoë. In utero resultate openbaar dat maternale nikotien blootstelling manlike vrugbaarheid nadelig beïnvloed in latere lewe en blyk dat dit meer van ‘n nadelige uitwerking op die voortplantingstelsel het as dié van direkte nikotien blootstelling aan spermatosoë. In vitro blootstelling van spermatosoë aan hoë vlakke van nikotien, het wel ook semen kwaliteit nadelig beïnvloed. Vetsug inhibeer ook manlike vrugbaarheids parameters en hierdie effek word vererger deur nikotien blootstelling. Die outeure glo dat hierdie nadelige uitwerking op die voortplantingstelsel verband hou met 'n verhoogde aktivering van leukosiete, oortollige produksie van reaktiewe suurstof spesies en die gevolglike aanvang van oksidatiewe stres bevorder. Hierdie studie stem wel ooreen met ander studies wat nikotien blootstelling bestempel as ‘n bydraende faktor tot die struikelblok van manlike onvrugbaarheid.

Introduction: As the predominant social expression of grief, funerals have been purported to be waning in occurrence and/or transitioning to emergent, less conventional ceremonial forms. In this research, trends regarding the cost, nature (type), and prevalence of funeral services were examined relative to an extant data set from two funeral homes of shared ownership in northeast Tennessee. The purpose was to verify or refute purported change(s) in the frequency and/or ceremonial emphasis of funerals in the study area. Methods: Anonymized, archival data from two funeral homes in northeast Tennessee (N = 2,581), spanning five years (2008-2012), were evaluated for trends over time with respect to two outcome variables: (1) the presence/absence of a funeral, and (2) the degree of ceremonial emphasis. Results: A binary logistic regression model was fit to the data with annualized time bins (2008 -2012) as predictors of the dichotomous outcome variable (presence/absence of a funeral). An omnibus test of the fitted model against that of a constant/intercept-only model was not statistically significant, indicating that time alone (irrespective of covariates) did not reliably predict whether there would be a funeral ceremony or not [chi square (4) = 6.558, p = .161]. Ceremonial emphasis was then regressed on annualized time bins using a multinomial logistic model without covariates. Model fitting information indicated a statistically significant difference between the final model (including the predictors) and the intercept-only model, suggesting that –in the absence of covariates –annualized time bins did reliably predict degree of ceremonial emphasis [chi square (8) = 149.570, p < .001], with an apparent trend toward decreased ceremonial emphasis. With the covariates of geographic location and retail spending included in the model, and using dummy coding to compare the probability of each degree of ceremonial emphasis to all other categories of emphasis combined, subsequent binomial logistic regressions indicated that although some modest degree of incremental change is apparent, weaving together strands of statistical significance does not result in a meaningful overall explanation of how (and/or why) that change may be occurring. Conclusion: These results do not support the proposition that funerals are declining in their frequency of occurrence, but they do indicate some incremental alterations in the ceremonial emphasis of ritual response to death, as influenced by the passage of time (demarcated annually), economic impacts (relative to retail spending), and geographic location. This research highlights the need for additional empirical investigation into factors explaining possible shifts in mourning (e.g., funerals) as well asother aspects of loss response.

Abstract:
Criminal rehabilitation continues to be a significant social problem. Previous research suggests that the majority of offenders leave the penal system with little to no discharge planning (Placido, Simon, Gu, and Wong, 2005). Researchers have also found that rates of recidivism are often influenced by the degree of the offender’s motivation to succeedand their personal outlook on life (Allen, 2013). Thus, the combination of intrinsic and extrinsic factors can affect the offender’s chances of successful and persistent rehabilitation. This qualitative case study sought to examine and describe the experiences of formerly incarcerated adults (n=4, all male) and describe a rehabilitation program currently in place in the East Tennessee region. The Day Reporting Center (DRC) is a rehabilitation program where offenders are on house arrest and are required to attend scheduled program classes and counseling. The DRC is part of an $800,000 crime reduction grant funded by the Department of Justice. All respondents have had issues with substance abuse at some point which is a selection requirement of the program. All claim to be completely disassociated from past bad influences, and report having been in and out of jail. Participants indicated that the DRC has been extremely helpful for them and all four have support from their family, partner, or both. In addition,3 out of 4 participants have had parents who struggled or currently struggle with substance or drug abuse. 2 out of 4 participants experienced physical and emotional abuse growing up. Moreover, 3 out of 4 participants stated that being a convicted felon and getting a job outside of the program hours has been difficult. One participant has already received his GED and is currently working on a degree in Automotive Technology in addition to attending the DRC Program. Another participant aspires to attend trade school after graduating from the rehabilitation program. Although respondents reported challenges faced by those in the penal system, they are hopeful for their success in this program. For example, one participant brought up one of his most memorable moments from prison, where he watched his son take his first steps and watched him walk for the first time in prison through the glass. He was genuine and honest about this moment and said, “To be honest with you, I cried. It was so hard. You just want to give him a hug and praise him for his accomplishment, and you can’t, because of the glass.” This participant also recalled the first time he walked on grass after getting out of prison and discussed how this was “one of the greatest feelings in the world” for him. As a final point, remembering those types of moments in the darkest of times may in fact be enough reason for an offender to never want to resort back to that kind of lifestyle again. An abundance of evidence supports that high stress levels often predict poor health, and high levels of religiosity, broadly defined, predict good health. It is possible that one mechanism by which religiosity positively impacts health is by preventing or reducing stress response. Studies measuring Surrender (Surrender to God from a Christian Religious Tradition) have shown that religiosity measured in this way is consistently negatively related to stress levels in college samples and community samples of pregnant women. The next step toward investigating a possible causal mechanism of religiosity on stress level is to experimentally manipulate religiosity. If it is found that it is possible to experimentally increase religiosity, the stage is then set to endeavor to change health and influences on health, such as stress, by such an increase. Specifically religiosity could be manipulated to determine whether changes in religiosity actually affect stress levels. Although this is the normal progression when investigating the efficacy of medical treatments, there has been significant opposition to the manipulation of religiosity. It is understandable that attempting to reduce religiosity would be unethical, but if there are potential health benefits to increasing religiosity, this field of research would seem to be worthy of investigation. The research community has been resistant to fund or even to approve such studies. Even more surprisingly, the medical community, while embracing the use of pharmacological substances to improve health or reduce disease (even for pregnant patients), remains reluctant to even investigate the efficacy of prescribed increases in religiosity, which would represent less physiological risk. Why is there this specific prejudice against religiosity as an intervention? Is it due to the lack of an observable mechanism? There are medications for which the true mechanism of effect is unclear, yet they are used because of the value of the effect. Is it because of the potentially reduced need for medical or pharmacologic intervention? Is it because it is poorly understood by many health care providers? If the ethics regarding human subjects research are embraced, a study should illustrate the potential benefits for both participants and others that outweighs any potential harm to the participants. It seems that on that basis, such experimental studies of the effects of increased religiosity on health should be considered.

Prisons and jails have become the de facto psychiatric hospitals of the twenty-first century. In the wake of deinstitutionalization, as mental healthcare transitioned to a community-based model, many patients with mental illness found themselves incarcerated rather than hospitalized. Strict drug laws combined with the current opioid epidemic are now forcing the government to consider treatment over punishment, lest the prison populations continue to swell. It is time to strongly consider using the involuntary commitment for severe cases of substance abuse if the patient is unwilling to undergo rehabilitation. Refusing to wait for the federal government to act, cities and states around the nation have begun to experiment with novel solutions to these issues, working within the framework of the prison system to achieve better outcomes.
