Disinvestment by states has driven up tuition prices at public colleges, and student debt has reached a startling record-high of one trillion dollars. Cost-minded students and their families--and the public at large--are questioning the worth of a college education, even as study after study shows how important it is to economic and social mobility. And as elite institutions trim financial aid and change other business practices in search of more sustainable business models, racial and economic stratification in American higher education is only growing. In American Higher Education in Crisis?: What Everyone Needs to Know, Goldie Blumenstyk, who has been reporting on higher education trends for 25 years, guides readers through the forces and trends that have brought the education system to this point, and highlights some of the ways they will reshape America's colleges in the years to come. At the same time, it is vital to secure adequate quality in TVET programmes which depend so much on the competence and commitment of college lecturers.

Those threats include rising costs endemic to personal services like higher education, growing income inequality in the United States that affects how much families can pay, demographic changes that will affect demand, and labor market changes that could affect the value of a degree. The book also evaluates changing patterns of state and federal support for higher education, and new digital technologies rippling through the entire economy. Although there will be great challenges ahead for America’s complex mix of colleges and universities, this book’s analysis is an antidote to the language of crisis that dominates contemporary public discourse. The bundle of services that four-year colleges and universities provide likely will retain their value for the traditional age range of college students. The division between in-person education for most younger students and online coursework for older and returning students appears quite stable. This book provides a view that is less pessimistic about the present, but more worried about the future. The diverse American system of four-year institutions is resilient and adaptable. But the threats this book identifies will weigh most heavily on the schools that disproportionately serve America’s most at-risk students. The future could cement in place a bifurcated higher education system, one for the children of privilege and great potential and one for the riskier social investment in the children of disadvantage.

That part of the clinical interface between neurology and general medicine occupied by inflammatory and immunological diseases is neither small nor medically trivial. Neurologists readily accept the challenges of ‘primary’ immune diseases of the nervous system: these tend to be focussed on one particular target such as oligodendrocytes or the neuro-muscular junction present in predictable ways, and are amenable as a rule to rational, methodological diagnosis, and occasionally even treatment. This is proper neurology.‘Secondary’ neurological involvement in diseases mainly considered systemic inflammatory conditions—for example, SLE, sarcoidosis, vasculitis, and Behçet’s—is a rather different matter. It may be difficult enough to secure such a diagnosis even when systemic disease has previously been diagnosed and new neurological features need to be differentiated from iatrogenic disease, particularly drug side effects or the consequences of immune suppression. But all the diseases mentioned may present with and confine themselves wholly to the nervous system; they may mimic one another, and pursue erratic and unpredictable clinical courses. In central nervous system disease, diagnosis by tissue biopsy is potentially hazardous and unattractive. Few neurologists enjoy excesses of confidence or expertise when faced with such clinical problems: the cautious diagnostician is perplexed, and the evidence-based neuroprescriber confounded. Unsurprisingly, great variations in approaches to diagnosis and management are seen (Scolding et al. 2002b).But rheumatologically inclined general, renal or respiratory physicians, comfortable when managing inflammation affecting their system or indeed other parts of the body designed to support the nervous system, are generally also ill at ease when faced with neurological features whose differential diagnosis may be large, particularly given the near universal diagnostic non-specificity of either imaging or CSF analysis.Here then is the subject material for this chapter: the diagnosis and management of central nervous system involvement in inflammatory and immunological systemic diseases (Scolding 1999a). In not one of these neurological conditions has a single controlled therapeutic trial been reported, and much that is published on these conditions is misleading or inaccurate. And yet the frequency with which the diagnosis is only confirmed or even first emerges at autopsy bears stark witness to both the severity and evasiveness of these disorders.

In the United States, approximately one in five women experiences rape during college, and lesbian, gay, bisexual, transgender, and queer (LGBTQ) students experience sexual violence at higher rates than their peers. Given this context, many colleges are working to better prevent and address these assaults. This book takes up this social problem—how organizations talk about and respond to sexual violence—and considers it in proximity to a persistent theoretical dilemma in the academic field of organizational communication: How are organization and violence related, and what does that relationship have to do with communication? Guided by feminist new materialist and intersectional theories, the book examines one public U.S. university known for responding well to sexual violence. It focuses on the processes and policies that require most faculty and administrators, along with student–employees, to report sexual violence to designated campus offices, per federal laws Title IX, the Clery Act, and the Violence Against Women Act. Unfortunately, the university’s interventions in sexual violence reinforce other violent systems. The book illustrates the negative consequences of considering communication to be either separate from the physical world or indistinguishable from it. It also details problems with the notion that only individuals enact violence. Through its focus on two core ideas—communication and agency—the book encourages scholars to avoid wholly constructivist or realist arguments, and it shows the importance of questions about power and difference in organizational scholarship on posthumanism and materiality. The book concludes with suggestions for how U.S. universities can look “beyond the rapist” to generate more robust interventions in sexual violence.

This chapter examines the fullest formulation of probabilism as it was articulated by the Jesuit theorists and professors of theology at the Roman College, which by the end of the sixteenth century had become the center of Jesuit knowledge. This chapter focuses in particular on Francisco de Toledo, Gregorio de Valencia, Francisco Suárez, and Gabriel Vázquez. Through an analysis of a substantial number of these authors’ manuscripts alongside their printed work, this chapter shows how probabilism evolved into a coherent and wide-ranging intellectual system for dealing with human uncertainty. Furthermore, tracing the development of probabilism through the lecture notes of the Jesuit theologians who taught at the Roman College and moved within a larger, pan-European, educational and intellectual milieu allows us to understand how a distinctive, complex, and hugely influential school of theological thought took its shape.

During the civil rights era, Mississippi was cloaked in the hateful embrace of the Closed Society, historian James Silver’s description of the white caste system that enforced segregation and promoted the subservient treatment of blacks. Surprisingly, challenges from Mississippi’s college basketball courts brought into question the validity of the Closed Society and its unwritten law, a gentleman’s agreement that prevented college teams in the Magnolia State from playing against integrated foes. Mississippi State University was at the forefront of the battle for equality in the state with the school’s successful college basketball program. From 1959 through 1963, the Maroons won four Southeastern Conference basketball championships and created a championship dynasty in the South’s preeminent college athletic conference. However, in all four title-winning seasons, the press feverishly debated the merits of an NCAA appearance for the Maroons, culminating in Mississippi State University’s participation in the integrated 1963 National Collegiate Athletic Association’s National Championship basketball tournament. Full Court Press examines news articles, editorials, and columns published in Mississippi’s newspapers during the eight-year existence of the gentleman’s agreement, the challenges posed by Mississippi State University, and the subsequent integration of college basketball within the state. While the majority of reporters opposed any effort to integrate athletics, a segment of sports journalists, led by the charismatic Jimmie McDowell of the Jackson State Times, emerged as bold and progressive advocates for equality. Full Court Press highlights an ideological metamorphosis within the press during the Civil Rights Movement, slowly transforming from an organ that minimized the rights of blacks to an industry that weighted the plight of blacks on equal footing with their white brethren.

How did Catholic colleges and universities deal with the modernization of education and the rise of research universities? In this book, Philip Gleason offers the first comprehensive study of Catholic higher education in the twentieth century, tracing the evolution of responses to an increasingly secular educational system. At the beginning of the century, Catholics accepted modernization in the organizational sphere while resisting it ideologically. Convinced of the truth of their religious and intellectual position, the restructured Catholic colleges grew rapidly after World War I, committed to educating for a "Catholic Renaissance." This spirit of militance carried over into the post-World War II era, but new currents were also stirring as Catholics began to look more favorably on modernity in its American form. Meanwhile, their colleges and universities were being transformed by continuing growth and professionalization. By the 1960's, changes in church teaching and cultural upheaval in American society reinforced the internal transformation already under way, creating an "identity crisis" which left Catholic educators uncertain of their purpose. Osteogenesis imperfecta (OI) is a genetic disorder that affects collagen formation and ultimately leads to increased bone fragility. The fragile nature of the bones leads to fractures, even from seemingly normal patient care. Affected patients are at the highest risk for unintentional fractures during perioperative care. There are several different types of OI. Type I is the most common. With the different types come varying degrees of severity. Types II and III are the more severe forms. The classic clinical triad seen in OI is blue sclerae, multiple fractures, and conductive hearing loss. The patient may have other systemic involvement beyond the fragile musculoskeletal system. It is imperative that the anesthesiologist be well-versed in the natural history and perioperative management of patients with OI in order to optimize care and minimize complications.

We then describe the communication skills training programme for residents, fellows, and practising physicians at Hamad Medical Corporation, the public healthcare system. Challenges in Arab countries for healthcare communication result predominantly from multicultural populations and from cultural differences with the West. This chapter explores how these clinicians can face and transcend these challenges via communication skills training.

This chapter draws attention to Wilde’s methods of studying for his final examinations in Literæ Humaniores (or Greats) between 1876 and 1878, when he was an undergraduate at Magdalen College, Oxford. It looks closely at the substantial document that scholars usually refer to as the ‘Philosophy’ notebook: a 350-page set of notes that range across many of the modern areas of disciplinary inquiry that the Greats student was expected to know in addition to classical history, philosophy, and literature. Using as an example Wilde’s notes on the concept of abstraction, the chapter traces his engagement with this idea in relation to such works as Aristotle’s Posterior Analytics, Mill’s System of Logic, Bain’s Mental and Moral Science, and Cairnes’s Essays in Political Economy. Throughout Wilde displays a truly dialectical turn of mind as he adopts different approaches to the logical process of abstraction: the condensing of complex particulars into general definitions.

Rheumatoid arthritis (RA) is among the most disabling form of chronic inflammatory joint disease. Not all forms of arthritis develop into RA; on the contrary, it may be very challenging to differentiate RA from cases of arthritis that are self-limiting or caused by another disease. Evaluation of early arthritis includes some basic steps, such as excluding trauma, crystal, or infectious-related disease, as well as considering additional features that may guide towards a specific diagnosis. If no specific diagnosis can then be made, the presentation can be labelled as undifferentiated arthritis. Typical differential diagnoses of RA include viral polyarthritis, seronegative spondylarthropathies, polymyalgia rheumatic, and other systemic rheumatic diseases. In 2010, new classification criteria were published that led to a change in the approach to RA. Compared to the previous criteria, the American College of Rheumatology (ACR) 1987 criteria, a scoring system was devised, appreciating the type and number of affected joints (up to 5 points), as well as serology (up to 3 points), elevated acute-phase reactants (1 point), and a symptom persistence of 6 weeks or longer (1 point). If 6 or more points are reached, then classifiable RA is present. Importantly, classification status, which is used for study purposes, is not always identical to the diagnostic status, which often leads to clinical treatment.

State budgets have become more volatile in recent years, and this has increased budget uncertainty for public institutions. In addition, the real value of state appropriation has trended downward in most states for many years. Public universities continue to fall further behind selective private competitors in spending per student. Falling quality at many public universities affects time to graduation and graduation rates, and the burden falls disproportionately on less-well-off students. We risk a bifurcation of our higher education system into well-funded selective private colleges and a less selective underfunded public sector that serves the bulk of the nation’s most vulnerable students.

This book explores the societal, technological, and user-related factors in understanding why and how digitally savvy college students in Asia’s most mobile cities—Shanghai, Hong Kong, Singapore, and Taipei—seek news via the mobile phone, how they evaluate mobile news for credibility and usefulness, and the consequences of this practice: becoming engaged with mobile news, which then teaches them about current affairs. The analyses are situated at the intersection of technological advances from 3G to 4G and marked differences in political and media systems across the four cities, which jointly shape Asia’s new generations of citizens. Technologically, the deeply diffused mobile phone motivates civic-minded millennials and centennials in Asia to access news with their phones and engage with the news for civic learning. However, sociopolitical factors impede potential positive outcomes of mobile news consumption. Cross-societal comparisons of survey data collected from two time periods reveal new insights into the interplay of technology and society in consuming mobile news.

A statue of Horace Mann, erected in front of the Boston state house in 1863, declares him the “Father of the American Public School System.” For over a century and a half, most narratives about early American education have proceeded as if this epithet were true. It has been etched into the general American consciousness as surely as it has been etched into the stone pedestal on which Mann stands. As Mann looms over the Boston Common, so he has loomed over discussions of early American schooling. The Common School Awakening offers a new narrative about the rise of public schools in America. The story begins before Horace Mann ever entered the scene as the first secretary of the Massachusetts Board of Education. In the first half of the nineteenth century a broad and distinctly American religious consensus emerged, allowing people from across the religious spectrum to cooperate in systematizing and professionalizing America’s schools, all in an effort to Christianize the country. At the height of this movement, several states introduced state-sponsored teacher training colleges and concentrated government oversight of schools in offices such as the one held by Mann. Shortly thereafter, the religious consensus that had served as the foundation for this common school system disintegrated. But the system itself remained, the legacy not just of one man, but of a whole network of reformers who put into motion a transatlantic and transdenominational religious movement—the “Common School Awakening.”

The adoption of new technologies in instruction will change the nature of instruction itself. There are four broad categories of the potential benefits of technology in higher education: off-loading; enhanced resources; enriched conventional class lecture/discussion; and outreach through distance education. Other college and university administrators have seen technology as either a money-saving or money-making tool for their institutions. The technologies most commonly associated with pedagogy include desktop software, internet-mediated communications, World Wide Web pages, distance education courseware, internet access to statistical databases, Geographic Information Systems (GIS), cellphone and personal digital assistant applications, and classroom response systems (CRS). There has been a modest and somewhat sporadic literature on teaching with technology in international studies, much of which follows the development of new technologies, such as personal computers, the World Wide Web, and courseware development. The three major themes in the scholarship on technology in teaching and learning in international studies include technology-based enthusiasm/experimentation, comparative studies, and skepticism. However, some of the challenges to scholarship in teaching and learning with technology: the use of technology has become so pervasive, accepted, and easy that few teacher-scholars bother to write in scholarly journals about the act; weak structure of incentives for studying the use of technology in teaching and learning; and technological instability and discontinuity. Nevertheless, there are some technologies and trends that may appear in the future international relations course. These include podcasting, Real Simple Syndication (RSS) Feeds, Twittering, and Wikipeda and Google Books.

One of the goals to be achieved by 2030 of the Global Health Sector Strategy on Viral Hepatitis 2016-2021 is to reduce mortality from hepatitis viruses B (HBV) and C (HCV). To measure and monitor it, countries need to implement a systematic process to generate national estimates of mortality from viral hepatitis, which many lack. This document is aimed at the institutions and/or ministries in charge of monitoring progress in each country. The main objective of this protocol is to present simple methods to estimate the proportion of patients with cirrhosis and hepatocellular carcinoma who have HBV and HCV infection, and then calculate the national mortality due to these sequelae attributable to viral hepatitis, preferably within a surveillance system. In addition, a general framework is provided on how the surveillance system should function, how to collect the data, and ethical considerations. The surveillance system will be based on sentinel centers where information will be collected from patients with cirrhosis and hepatocellular carcinoma. These data will be used to estimate the fraction of cirrhosis and hepatocellular carcinoma attributable to HBV and HCV. On the other hand, data will be collected on the number of deaths nationwide from cirrhosis and hepatocellular carcinoma. With this information, mortality from cirrhosis and hepatocellular carcinoma attributable to HBV and HCV will be estimated.

The town of Mondragón in the Basque Country is home to one of the largest and most significant experiences of co-operative organization and workers’ self-management anywhere in the world. The Mondragón co-operative movement, born in the 1950s around the local technical training college and a handful of small industrial firms, encompasses today more than one hundred co-operative firms operating in ninety-seven countries and generating an aggregate revenue of €12bn. In this chapter we review the historical origins of the Mondragón experience and the goals that guided the first co-operative projects. After describing the key organizational principles and governance mechanisms of individual co-operatives and of the Mondragón group as a whole, we will examine the rapid expansion and internationalization of some of the most emblematic Mondragón firms—a process that has led to a difficult balance between the maintenance of the original co-operative principles at home and an increasing reliance on capitalist forms of ownership and production abroad. We conclude by discussing the impact of the recent economic downturn on the Mondragón group and its system of inter-co-operative solidarity, and by reflecting on the future prospects for this far-reaching experiment in collective ownership and democratic governance.

This chapter examines the accuracy of the quote “behind every great fortune is an equally great crime”, attributed to Balzac. In our times great individual fortunes are generally generated via the instrument of the business firm. The question then becomes when are firm profits a crime? Firm profits are, in general, explained by one or more of three factors: luck, efficiency, and collusion. While it is difficult to regard luck as a crime, luck does not reflect merit. Profits due to efficiency seem like the least problematic case, and collusion seems the clearest case of when fortune coincides with crime. A variety of cases lie, at least in a dynamic sense, at the intersection of the three conditions. Unfortunately, history suggests that big business and big government can collude to keep profits flowing to the former and contributions flowing to the latter. While luck and efficiency may help initiate wealth, the system tolerates connivance, making Balzac’s statement more plausible than it ought to be.

The ability and privilege to handle Isotopes for medical use is regulated by NRC with federal laws on training and experience (10CFR 35) incorporated into clinical training guidelines. Core Cardiology Training Symposium (COCATS) and documents sponsored (and updated) by the American College of Cardiology (ACC) specify the clinical training necessary to independently practice nuclear cardiology. Certification Board of Nuclear Cardiology (CBNC) collects appropriate prerequisite statements of training that match guidelines and administers a well- constructed examination leading to physician certification in recognition of meeting national standards to practice Nuclear Cardiology. Policies and procedures of NRC/COCATS/CBNC are reviewed in this chapter and near term challenges to evolving physician certification in a complex healthcare system are explored. Lab Accreditation is a quality process that goes beyond physicians to assess equipment, staff, policies and procedures, image quality, and reporting. Lab accreditation by one of three recognized accrediting bodies is now mandated for reimbursement of advanced imaging in Medicare program. Intersociety Accreditation Commission (IAC, Nuclear/PET division) is a recognized body for lab accreditation and has placed special emphasis on clinical reporting/outcomes and meaningful quality improvement activities. IAC Nuclear cardiology Standards are reviewed as opportunities to externally be recognized for meeting quality standards within a protected peer review climate. As with certification, near term challenges exist for evolving lab accreditation and these are explored as well in this chapter.

This chapter discusses the law on creditor claims in Mexico. Insolvency proceedings in Mexico aim for an orderly liquidation procedure of all of the bankrupt’s assets to allow an equally orderly distribution of proceeds among creditors under the principle of par conditio creditorum (equal treatment for all creditors). Ideally, the body of law regulating this procedure should match the insolvency law of each country, so that the process can be effected congruently and logically. However, social policies and other commercial considerations have created systems of priorities in asset distribution that vary from country to country as a function of the policies of each. In many cases, the only commonality in all systems is that ordinary unsecured creditors collect last, if at all. Mexico is no exception, specifically in the treatment of claims arising out of a labour relationship, which in turn is derived from the overly protective and outdated regime established by the federal constitution for workers. The remainder of the chapter deals with insolvency claims, administration claims, and non-enforceable claims in turn. Each section examines: the definition and scope of the claim; rules for submission, verification, and satisfaction or admission of claims; ranking of claims; and voting and other participation rights in insolvency proceedings.

The optical interface between a light source and a detector is important. In most practical realizations the aim should be to collect the maximum light possible. Lens systems seldom do this efficiently, especially where the light source is diffuse. Underlying any attempt at concentrating or guiding light is subject to a fundamental limitation referred to as étendue (phase space cannot be squeezed). Light collection from small volume scintillators of high refractive index may approach 50 %, while collection from large-area scintillators is typically less than a few per cent. Incorporation of wavelength-shifting light guides and fibres leads to enhanced performance. Efficiency measurements by the author in terms of photoelectrons per keV are presented for selected configurations. Optical recycling derived from total internal reflection provides enhancement in effective quantum efficiency by a factor of up to 10. Concepts such as escape cones, adiabatic light guides, and trapped light are covered in detail.

Tensions in South African universities have traditionally centred around equity (particularly access and affordability), historical legacies (such as apartheid and colonialism), and the shape and structure of the higher education system. What has not received sufficient attention, is the contribution of the university to place-based development. This volume is the first in South Africa to engage seriously with the place-based developmental role of universities. In the international literature and policy there has been an increasing integration of the university with place-based development, especially in cities. This volume weighs in on the debate by drawing attention to the place-based roles and agency of South African universities in their local towns and cities. It acknowledges that universities were given specific development roles in regions, homelands and towns under apartheid, and comments on why sub-national, place-based development has not been a key theme in post-apartheid, higher education planning. Given the developmental crisis in the country, universities could be expected to play a more constructive and meaningful role in the development of their own precincts, cities and regions. But what should that role be? Is there evidence that this is already occurring in South Africa, despite the lack of a national policy framework? What plans and programmes are in place, and what is needed to expand the development agency of universities at the local level? Who and what might be involved? Where should the focus lie, and who might benefit most, and why? Is there a need perhaps to approach the challenges of college towns, secondary cities and metropolitan centers differently? This book poses some of these questions as it considers the experiences of a number of South African universities, including Wits, Pretoria, Nelson Mandela University and especially Fort Hare as one of its post-centenary challenges.

In 2010, teachers from the B.A. in English Teaching at Universidad Católica Luis Amigó formed CILEX (Construcciones Investigativas en Lenguas Extranjeras). Research and teaching in the program have grown synergistically ever since, but ten years down the road it was time to take stock of our research to project the direction in which we wanted to move forward. This book is the result of that effort to recognize our shared history and thus propel our upcoming academic endeavors. The book starts out by presenting the epistemological foundations of CILEX, which is based on the threefold notion of the language teacher as an intellectual, an academic, and an educator. It thereon explains the system that arranges our academic production within five thematic nodes: cultural studies, language policy, literacies, language teacher education, and language assessment. Each chapter reports on one or two studies in which the authors participated as leading researchers or advisors. Hence, the book also reflects the formative research tradition that characterizes most of our practice. Having language teacher education as a binding thread that cuts across the entire volume, authors present their particular perspective on topics as varied as college academic performance, early childhood literacy, language policy appropriation, teacher educators’ assessment literacy, student teachers’ practicum identity crisis, research training in teacher education, and critical reading instruction. This book condenses the work of a group of teacher educators who believe in the power of research to galvanize teaching and inspire positive educational change. As readers go through its pages, it is our hope they will be able to recognize not only the singular value of each individual chapter but also the richness of our collaboration, which constitutes the fabric of our identity as an academic community.
