Abstract:
This paper focuses on the nature of saintly or heroic acts, which, according to J.O. Urmson, exist as a fourth, less traditional category of moral actions. According to this division, heroic acts are those, which have positive moral value, but cannot be demanded of an individual as their duty; however, this paper argues that Urmson is mistaken in his claim that a consequentialist ethical framework is the most capable of accounting for heroic acts. Furthermore, this paper claims that an Aristotelian account is the most appropriate ethical theory to consider, which could better countenance the existence of heroic acts.

Abstract:
A laser-dye technique is known to injure selectively microvascular endothelium in situ. With the use of the arterioles on the surface of the mouse brain (pial arterioles), the endothelial injury is manifest by loss of several endothelium-dependent responses, including the dilation produced by topical application of acetylcholine (ACh), bradykinin (BK), and calcium ionophore A-23187. The responses normally recover by 60 min following injury. The present study shows that inhibitors of protein synthesis prevent the recovery. Either actinomycin D (Act-D; 1 mg/kg) or cycloheximide (CX; 10 mg/kg) was injected intraperitoneally 35 min before the light-dye injury. The CX was used in separate studies with ACh, BK, and ionophore. In each study, recovery of the endothelium-dependent dilation failed to occur 60 min after injury, while recovery occurred in contemporary vehicle-treated controls. Act-D was used in separate studies of ACh and BK. Again, recovery of endothelium-dependent dilation was prevented. Neither CX nor Act-D inhibited the response to ACh in uninjured vessels. Thus response to ACh was intact in such arterioles 90 min after the injection of CX or Act-D. CX was also given to uninjured mice and was found to have no inhibitory action on the response to ionophore 90 min later. CX and Act-D inhibit protein synthesis by very different mechanisms, inhibition of translation and of transcription, respectively. Because both prevented recovery of the endothelium-dependent responses, we conclude that one or more rapidly synthesized proteins are required for the recovery.

Abstract:
The Basic Law, whose 60th anniversary we celebrate today, is not identical with the Constitution that was enacted on 23 May 1949. In the sixty years of its existence, it has been amended fifty-four times. Further amendments are under way. Many amendments concerned more than just one article. A little more than half of the 146 articles of the original text still read as they were framed in 1949. However, twenty-six of these unchanged articles are part of the “Transitional and Concluding Provisions.” A number of them were limited to one singular act of application, such as Art. 136 I (“The Bundesrat [Federal Council] shall convene for the first time on the day the Bundestag [national parliament] first convenes.”), and lost their normative value with the completion of the given act. A little less than half of the original articles, and the Preamble, have been subject to amendments; some of them several times. The forerunner is Art. 106 (apportionment of tax revenue), with six amendments. In addition, forty-seven articles have been added to the original text, some of which were repealed later.

Abstract:
The credit of bank has the potential of misusing or abusing by parties whose not responsible for getting profit or benefit, unlawfully. The parties can be internal or external persons such as bank officers, board members of bank, commissioners, stakeholders and bank customers. Misuses in bank loaning can be a criminal offense of banking if the board of members or officers do not obey the rule of the banking concerning the principles of prudential and appraisal, carefully. Lately, in practice, it can be a criminal offense or criminal act. It is a question, do a sentence of corruption criminal act into jail to the board of members and bank consumer is right measures? ?. Is criminal punishment against customers effective in handling bad debts ? Criminalization of corruption to them by engaging the Act of Corruption Criminal Act against to the Act of Banking that has ruled board of members in article 50 with imprisonment. It can be double standards in a completion of bad credit that funded by state finances and privates. Therefore, the Corruption Act may not be used in the settlement of bad loans to private banks. The settlement of bad loans based on The Act of Mortgage Right which guarantee the land rights of the debtor as debt repayment. The law should be bound up to avoid uncertain of law. Keywords: credit loan, corruption and completin of bad debts.

Abstract:
ABSTRACTWhooping cough results from infection of the respiratory tract withBordetella pertussis, and the secreted adenylate cyclase toxin (ACT) is essential for the bacterium to establish infection. Despite extensive study of the mechanism of ACT cytotoxicity and its effects over a range of concentrationsin vitro, ACT has not been observed or quantifiedin vivo, and thus the concentration of ACT at the site of infection is unknown. The recently developed baboon model of infection mimics the prolonged cough and transmissibility of pertussis, and we hypothesized that measurement of ACT in nasopharyngeal washes (NPW) from baboons, combined with human andin vitrodata, would provide an estimate of the ACT concentration in the airway during infection. NPW contained up to ∼108CFU/mlB. pertussisand 1 to 5 ng/ml ACT at the peak of infection. Nasal aspirate specimens from two human infants with pertussis contained bacterial concentrations similar to those in the baboons, with 12 to 20 ng/ml ACT. When ∼108CFU/ml of a laboratory strain ofB. pertussiswas culturedin vitro, ACT production was detected in 60 min and reached a plateau of ∼60 ng/ml in 6 h. Furthermore, when bacteria were brought into close proximity to target cells by centrifugation, intoxication was increased 4-fold. Collectively, these data suggest that at the bacterium-target cell interface during infection of the respiratory tract, the concentration of ACT can exceed 100 ng/ml, providing a reference point for future studies of ACT and pertussis pathogenesis.

Abstract:
ABSTRACT The Aptima Combo 2 (AC2) and Aptima CT (ACT) (Hologic Inc., San Diego, CA) are nucleic acid amplification tests (NAATs) that detect Chlamydia trachomatis . AC2 also detects Neisseria gonorrhoeae . Storage and temperature conditions may impact the utility of NAATs in some settings and screening programs. We evaluated specimen stability for use beyond the Aptima package insert specifications for temperature and duration of storage (between 2°C and 30°C and 60 days, respectively) in two studies: (i) dry C. trachomatis -seeded swabs were used with ACT after storage at 4°C, 23°C, or 36°C for up to 84 days and (ii) swabs seeded with C. trachomatis and N. gonorrhoeae and then placed in transport medium were tested with AC2, after being mailed via the U.S. Postal Service to three different sites. Prolonged storage of samples had no effect, and samples stored at 4°C, 23°C, and 36°C for up to 84 days yielded comparable ACT positivities, although there was a drop in signal intensity for virtually all specimens under all storage/shipping conditions after day 21. In the mailing study, 80%, 52% and 29% of seeded swabs were exposed to temperatures of >30°C during three rounds in transit, and 2% reached temperatures of >40°C. No evidence of signal degradation in the AC2 assay for detection of C. trachomatis or N. gonorrhoeae was observed, although some mailed swabs took more than 5 weeks to reach the laboratory site. These two studies support the potential use of swabs at temperatures above 36°C and storage beyond 60 days and provide confidence regarding this commercially available NAAT for testing of specimens after mailing.

Applied Physiology, Nutrition, and Metabolism 44, no. 4 (April 2019): 434–42. http://dx.doi.org/10.1139/apnm-2018-0506.

Abstract:
Acute acetaminophen (ACT) ingestion has been shown to enhance cycling time-trial performance. The purpose of this study was to assess whether ACT ingestion enhances muscle activation and critical power (CP) during maximal cycling exercise. Sixteen active male participants completed two 3-min all-out tests against a fixed resistance on an electronically braked cycle ergometer 60 min after ingestion of 1 g of ACT or placebo (maltodextrin, PL). CP was estimated as the mean power output over the final 30 s of the test and W′ (the curvature constant of the power–duration relationship) was estimated as the work done above CP. The femoral nerve was stimulated every 30 s to measure membrane excitability (M-wave) and surface electromyography (EMGRMS) was recorded continuously to infer muscle activation. Compared with PL, ACT ingestion increased CP (ACT: 297 ± 32 W vs. PL: 288 ± 31 W, P < 0.001) and total work done (ACT: 66.4 ± 6.5 kJ vs. PL: 65.4 ± 6.4 kJ, P = 0.03) without impacting W′ (ACT: 13.1 ± 2.9 kJ vs. PL: 13.6 ± 2.4 kJ, P = 0.19) or the M-wave amplitude (P = 0.66) during the 3-min all-out cycling test. Normalised EMGRMS amplitude declined throughout the 3-min protocol in both PL and ACT conditions; however, the decline in EMGRMS amplitude was attenuated in the ACT condition, such that the EMGRMS amplitude was greater in ACT compared with PL over the last 60 s of the test (P = 0.04). These findings indicate that acute ACT ingestion might increase performance and CP during maximal cycling exercise by enhancing muscle activation.

Abstract:
As most development planning and rendering of public services happens at a place or in a space, geospatial data is required. This geospatial data is best managed through a spatial data infrastructure, which has as a key objective to share geospatial data. The collection and maintenance of geospatial data is expensive and time consuming and so the principle of “collect once &amp;ndash; use many times” should apply. It is best to obtain the geospatial data from the authoritative source &amp;ndash; the appointed data custodian. In South Africa the South African Spatial Data Infrastructure (SASDI) is the means to achieve the requirement for geospatial data sharing. This requires geospatial data sharing to take place between the data custodian and the user. All data custodians are expected to comply with the Spatial Data Infrastructure Act (SDI Act) in terms of geo-spatial data sharing. Currently data custodians are experiencing challenges with regard to the sharing of geospatial data.<br> This research is based on the current ten data themes selected by the Committee for Spatial Information and the organisations identified as the data custodians for these ten data themes. The objectives are to determine whether the identified data custodians comply with the SDI Act with respect to geospatial data sharing, and if not what are the reasons for this. Through an international comparative assessment it then determines if the compliance with the SDI Act is not too onerous on the data custodians.<br> The research concludes that there are challenges with geospatial data sharing in South Africa and that the data custodians only partially comply with the SDI Act in terms of geospatial data sharing. However, it is shown that the South African legislation is not too onerous on the data custodians.

Abstract:
We chronologically determined whole blood coagulation time during continuous heparin administration, and investigated optimal doses of heparin in thirty-seven vascular surgery cases. ACT was determined with Hemochron 401. Heparin (2000 IU) was administered by bolus injection at the beginning of intravascular surgery, which was followed by continuous injection of 20 to 160 IU/kg/h. ACT determined before and 30 minutes after heparin administration were compared. There were no complications. ACT was maintained at a nearly constant level by continuous heparin administration. The dose of heparin required to maintain ACT at a level 1.5 to 2 times the initial level was 20 to 60 IU/kg/h. ACT was prolonged by more than three times in two of the ten subjects who were given heparin at a dose of 70 IU/kg/h or more. Continuous administration of heparin allowed maintenance of ACT at a nearly constant level during intravascular surgery. ACT was maintained within the range which is believed to be effective for prevention of thrombus formation (approximately 1.5 to 2.0 times larger than the initial level) by continuous administration of 20 to 60 IU/kg/h of heparin. ACT was, however, prolonged to more than three times the initial level in some subjects who were given 70 IU/kg/h or higher doses, suggesting the risk of a bleeding tendency. Accordingly, it is ideal to continue heparin administration at appropriate doses, while measuring ACT. The results of our study should serve as a useful standard for meeting this goal.

Abstract:
Abstract Background While many people enter residential care of their own free will and because it is their preference, the evidence tells us that there are also many who if they had the choice would remain in their own homes. Lack of appropriate community supports may provide some impetus to enter residential care. According to Care Alliance Ireland, an additional four million hours of homecare needs to be provided to cope with the successful ageing demographics, at a cost of €110 million. There is no statutory or common-law power to detain a patient in a Healthcare Facility outside of the application of the Mental Health Act 2001. This presents legal, ethical and moral dilemmas for Healthcare Providers when caring for a person who lacks capacity wishes to self - discharge. A duty of care obligates healthcare professionals to act in the best interest of the individual. Under the Health Act 2007 the requirement is to provide for a ‘safe discharge’. Pending advancement of the Assisted Decision Making (Capacity) Act 2015 which provides a statutory framework to assist and support individuals to make legally-binding agreements about their welfare, their property and affairs we are currently acting under the Lunacy Regulations (1871). Methods We developed a ‘Deprivation of Liberty’ form which enable comprehensive Interdisciplinary Team discussion and direction of care. Presumption of capacity, respect for the resident’s wishes and consideration of all possible supportive actions up to and including sourcing community support services were considered. Results This format has enabled comprehensive discussion and robust adherence to human rights for three residents thus far Conclusion The situation remains that there is no legal framework to guide healthcare providers currently. Using a Human Rights based approach is imperative to guide us while awaiting advancement of the ADMA (2015) and Deprivation of Liberty legislation to be included in this act.

Over recent years, the EU has increasingly looked at the regulation of various forms of automation and the use of algorithms. For recommender systems specifically, two recent legislative proposals by the European Commission, the Digital Services Act from December 2020 and the Artificial Intelligence Act from April 2021, are of interest. This article analyses the recent legislative proposals with a view to identify the regulatory trajectory. Whereas the instruments differ in scope, it argues that both may -directly and indirectly- regulate various aspects of recommender systems and thereby influence the debate on how to ensure responsible, not opaque, machines that recommend information to humans.

It is highly expressed on the surfaces of many cancers and myeloid derived suppressor cells (MDSCs) but absent on normal tissues. By conjugating a novel maytansinoid toxin to a recombinant form of human AFP (ACT-101), we can selectively deliver the toxin to cancer and MDSC cells while sparing normal cells, thereby enabling a combination of targeted and immune activating approaches against the tumor. Methods: Four ACT-101-maytansinoid conjugates with different protein-toxin ratios and different linker chemistries were initially investigated in a mouse COLO-205 xenograft model, resulting in the selection of ACT-903 as the lead candidate for further development. A second study performed in the same tumor model compared the effects of single intravenous doses of ACT-903 (10-50 mg/kg) to control groups receiving either vehicle or the unconjugated protein, ACT-101 (25 mg/kg). Tumor growth, survival and clinical observations were assessed for 60 days post tumor implantation. Both serum ACT-101 and maytansinoid levels were measured following the single dose. Results: In the first study, a statistically significant reduction in tumor volume was seen for all four conjugates compared to vehicle control (p < 0.05). For ACT-903, tumors continued shrinking even after treatment completion, becoming undetectable in 9 of 10 animals. All 10 mice in this group survived through Day 60 with no obvious signs of toxicity. In contrast, there were no survivors in the control group. In the single dose study of ACT-903, a significant reduction of tumor burden compared to vehicle was achieved by Day 14 (p < 0.05) in both the 40 and 50 mg/kg dose groups. Mice in these 2 groups received a second dose 15 days after the first dose, based on observed tumor re-growth. Survival was significantly prolonged in the 50 mg/kg (p = 0.0037) and 40 mg/kg group (p < 0.0001) with 7 of 10 in the 50 mg/kg group and 9 of 10 animals in the 40 mg/kg group surviving to Day 60. Similar doses of ACT-101 and ACT-903 produced comparable ACT-101 serum levels, suggesting that the pharmacokinetic profile of the conjugate is driven primarily by the protein component. Free maytansinoid levels at 4 hours were less than 0.01% of the injected dose of the conjugate, indicating that the conjugate is stable in circulation. Conclusions: The efficacy and tolerability of ACT-903 in an animal tumor model supports advancing this conjugate toward clinical use with a regimen of either once a week or once every other week administration.

Abstract:
Objective: This study tested the efficacy of Acceptance and Commitment Therapy (ACT), compared with Cognitive–Behavioral Therapy (CBT), in alleviating academic procrastination. Method: A total of 60 (53.3% male) undergraduates suffering from academic procrastination were randomly assigned to two treatment groups (ACT and CBT) and a control group. The procrastination symptoms and related psychological mechanisms were assessed immediately after the closure of treatment and at 3-month follow-up (FU). Results: Both therapies showed remarkable short-term effects in decreasing procrastination, but ACT had a better long-term effect. Participants achieved self-esteem enhancement through treatment of both ACT and CBT. While ACT significantly decreased negative affect and improved neuroticism, CBT had a stronger effect on time management. Conclusion: The findings suggest that both CBT and ACT are effective interventions for procrastinators but may have different therapeutic mechanisms.

Abstract:
The absence of the doctrine of fair use from Australian copyright law has been a bone of contention in Australia after the Australia-United States Free Trade Agreement (FTA). As the Australian government reformed the Copyright Act 1968 (Cth) in the aftermath of the FTA it eschewed the option of adopting fair use. Instead, Australia chose to incorporate a version of fair use into its existing fair dealing framework. Accordingly, the Copyright Amendment Act 2006 (Cth) inserted ss 41A and 103AA into the Copyright Act. These provisions provide that a fair dealing with a copyright protected work does not constitute an infringement if it is done for the purposes of parody or satire. These provisions codify part of the ratio of the United States Supreme Court in the seminal case of Campbell v Acuff Rose Music. However, the parameters of these new provisions are unexplored and the sparse nature of fair dealing jurisprudence means that the true meaning of the provisions is unclear. Moreover, two cases from the United States, SunTrust Bank v Houghton Mifflin and Salinger v Colting, underline just how important it is to have legal rules that protect literary ‘re-writes’. Both cases involved authors using an original novel to ‘write back’ to the original author and the broader culture. ‘Writing back’ or the ‘re-write’ has a firm basis in literature. It adds something invaluable to our culture. The key question is whether our legal landscape can allow it to flourish. This paper examines the interaction between fair use and literary re-writes.

Abstract:
1583 Background: ACT for NSCLC is associated with improved survival in the general population but may be underutilized. Underutilization may relate to lack of referral from surgeon to MO, MO not offering ACT, or patient declining ACT. Here we explore factors associated with referral to MO and use of ACT among patients with resected NSCLC in Ontario Canada. Methods: The Ontario Cancer Registry was used to identify all incident cases of NSCLC diagnosed in Ontario 2004-2006. We linked electronic records of treatment to identify surgery, ACT, and MO consultation. Co-morbidity was classified using the Charlson Comorbidity Index modified for administrative data. A multivariate logistic regression model was used to evaluate factors associated with referral to MO and use of ACT. Results: 3354 cases of NSCLC were resected in Ontario 2004-2006, 1830 (55%) were seen post-operatively by MO and 1032 (31%) were treated with ACT. Cases younger than 70 were more likely to have MO consultation (age 60-69 OR 1.6; 50-59 OR 2.3; 20-49 OR 2.2, p<0.001) as were cases with stage II/III (ORs 2.7 and 2.0, p<0.01) compared to stage I disease. There was substantial geographic variation in the proportion of surgical cases referred to MO (range 32-88%, p<0.001). Among cases seen by MO, patients younger than 70 were more likely to have ACT (age 60-69 OR 3.1; 50-59 OR 4.7; 20-49 OR 6.7, p<0.001) as were cases with stage II/III (ORs 2.7 and 3.0, p<0.001) compared to stage I disease. Less co-morbidity (OR 2.1, p=0.02) and shorter post-operative stay (OR 1.4, p=0.001) were also associated with use of ACT. Among cases seen by MO, there was some geographic variation (range 46-63%, p<0.001) in rates of ACT utilization. Conclusions: The upstream decision to refer to MO is associated with age and stage of disease and these factors have an even greater effect on ACT utilization once patients are seen by MO. While co-morbidity and post-operative length of stay are not associated with referral to MO, they are associated with use of ACT among cases seen by MO. There is substantial geographic variation in referral patterns to MO and less variation in ACT utilization once patients are seen by MO.

Abstract:
The Tax Reform Act of 1986 was studied to determine its short-run impact on Pennsylvania farmers. For the 3,059 farms studied, the average increase in total tax obligation resulting from the TRA of 1986 was $446 per farm, or a 48 percent increase in federal income taxes actually paid in 1984. Differences in impact were found across farms categorized by commodity type. Egg producers were most severely impacted, while beef cattle producers least affected. In general, the increase in adjusted gross income was due to the loss of 60 percent exclusion on capital gain income.

Abstract:
The Worker Adjustment and Retraining Notification Act (WARN) of 1988 requires that covered firms provide affected employees with 60 days' advance notice of plant closings and large-scale layoffs. The authors use data from the three most recent Displaced Worker Surveys to compare the extent of notice under voluntary (1983–88) and mandatory (1989–91) notice regimes. They find that the legislation has had at most a small impact on the provision of notice of the length stipulated under the Act. This non-event is at least partially due to the liberal firm-size and layoff thresholds for determining coverage under WARN.

Abstract:
This review reports on methods used to evaluate airway clearance techniques (ACT) in adults with CF and examined data for evidence of any effect. Sixty-eight studies described ACT in adequate detail and were included in this review. Frequently reported outcomes were sputum expectoration (72%) and spirometric lung function (60%). Compared with cough alone, following any ACT, there was a trend for greater sputum wet weight, however FEV1 was not different. The mean (95% CI) within-group effect for sputum wet weight following any ACT was 12.43 g (9.28 to 15.58) (n = 30 studies) and for FEV1 was 0.03 L (−0.17 to 0.24) (n = 14 studies). Meta-regression demonstrated that, when compared with cough alone, greater sputum wet weight was reported in groups that received additional ACT by between 2.45 and 3.94 g (F3,66 = 2.97, p = 0.04). These data suggest the addition of ACT to cough alone may optimise sputum clearance; however, FEV1 lacked sensitivity to detect this change. Importantly, this review highlights the lack of appropriate measures to assess ACT efficacy.

Abstract:
Previous results have shown that the human promyelocytic leukemia HL-60 cell line responds to either proliferating or differentiating stimuli. When these cells are induced to proliferate, protein kinase C (PKC)-βII migrates toward the nucleus, whereas when they are exposed to differentiating agents, there is a nuclear translocation of the α isoform of PKC. As a step toward the elucidation of the early intranuclear events that regulate the proliferation or the differentiation process, we show that in the HL-60 cells, a proliferating stimulus (i.e., insulin-like growth factor-I [IGF-I]) increased nuclear diacylglycerol (DAG) production derived from phosphatidylinositol (4,5) bisphosphate, as indicated by the inhibition exerted by 1-O-octadeyl-2-O-methyl-sn-glycero-3-phosphocholine and U-73122 (1-[6((17β-3-methoxyestra-1,3,5(10)-trien-17-yl)amino)hexyl]-1H-pyrrole-2,5-dione), which are pharmacological inhibitors of phosphoinositide-specific phospholipase C. In contrast, when HL-60 cells were induced to differentiate along the granulocytic lineage by dimethyl sulfoxide, we observed a rise in the nuclear DAG mass, which was sensitive to either neomycin or propranolol, two compounds with inhibitory effect on phospholipase D (PLD)-mediated DAG generation. In nuclei of dimethyl sulfoxide-treated HL-60 cells, we observed a rise in the amount of a 90-kDa PLD, distinct from PLD1 or PLD2. When a phosphatidylinositol (4,5) bisphosphate-derived DAG pool was generated in the nucleus, a selective translocation of PKC-βII occurred. On the other hand, nuclear DAG derived through PLD, recruited PKC-α to the nucleus. Both of these PKC isoforms were phosphorylated on serine residues. These results provide support for the proposal that in the HL-60 cell nucleus there are two independently regulated sources of DAG, both of which are capable of acting as the driving force that attracts to this organelle distinct, DAG-dependent PKC isozymes. Our results assume a particular significance in light of the proposed use of pharmacological inhibitors of PKC-dependent biochemical pathways for the therapy of cancer disease.

Abstract:
ABSTRACT Human-monocyte-derived dendritic cells (MoDC) are very efficient in the uptake of Listeria monocytogenes, a gram-positive bacterium which is an important pathogen in humans and animals causing systemic infections with symptoms such as septicemia and meningitis. In this work, we analyzed the influence of blood plasma on the internalization of L. monocytogenes into human MoDC and compared the uptake of L. monocytogenes with that ofSalmonella enterica serovar Typhimurium and Yersinia enterocolitica. While human plasma did not significantly influence the uptake of serovar Typhimurium and Y. enterocolitica by human MoDC, the efficiency of the uptake ofL. monocytogenes by these phagocytes was strongly enhanced by human plasma. In plasma-free medium the internalization of L. monocytogenes was very low, whereas the addition of pooled human immunoglobulins resulted in the internalization of these bacteria to a degree comparable to the highly efficient uptake observed with human plasma. All human plasma tested contained antibodies against the 60-kDa extracellular protein of L. monocytogenes (p60), and anti-p60 antibodies were also found in the commercially available pooled immunoglobulins. Strikingly, in contrast to L. monocytogenes wild type, an iap deletion mutant (totally deficient in p60) showed only a minor difference in the uptake by human MoDC in the presence or the absence of human plasma. These results support the assumption that antibodies against the listerial p60 protein may play an important role in Fc-receptor-mediated uptake of L. monocytogenes by human MoDC via opsonization of the bacteria. This process may have a major impact in preventing systemic infection in L. monocytogenes in immunocompetent humans.

Abstract:
After decades of successful artemisinin regimen in combating malaria, its effectiveness has decreased since parasite resistance to the treatment regimen has begun to appear. Adherence to artemisinin combination therapy (ACT) in a population is considered to be the key factor contributing to such resistance phenomenon. Although several studies have tried to demonstrate adherence to several ACT types in a population, only a limited number of studies demonstrated adherence to dihyrdroartemisinin + piperaquine (DHP) regimen. The present study was conducted in two localities representing low and high endemic areas in Indonesia. Active case detection (ACD) and passive case detection (PCD) have been applied to screen for malaria case in the localities. At day 3, patients were visited in the house to be interviewed using structured questionnaire. Capillary sample of each patient was also collected on Whatman® filter paper at day 60 to observe the piperaquine metabolite of the patients. 47 and 91 (out of 62 and 138) patients from Jambi and Sumba, respectively, were successfully enrolled in this study. In Jambi, the level of adherence was 66%, while that in Sumba was 79.1%. The associated factors of adherence in our study settings are patient age group (OR = 1.65 [CI: 0.73–3.73]) and patients’ knowledge of malaria prevention measure (OR = 0.29 [CI: 0.09–0.9]). Our study suggested that the adherence to ACT medication among population in our study setting is considered to be less than 80%, which needs to be elevated to avoid the growing trend of treatment failure as seen globally. Additionally, our study found that metabolite at day 60 after prescription of piperaquine could be a potential marker for monitoring adherence to piperaquine drug in a population.

Abstract The current study aimed to examine how university teachers advise their fellow teachers with a focus on solicited advice. To this end, 60 university teachers who use English as a Lingua Franca (ELF) as the medium of their everyday communication completed 8 role-plays in which newly hired teachers sought the advice of their colleagues regarding work issues. The role-plays were recorded and transcribed in preparation for coding, which was conducted with the use of Martínez-Flor’s (2003) taxonomy of advising and Trosborg’s (1995) taxonomy of internal modifiers. External modifiers were also examined as they emerged in the data. The results showed that the participants mainly preferred the use of direct advising strategies while simultaneously attempting to cater for the interlocutors’ negative face. While the advising strategies were situation dependent, the effect of the social variables of the advisor’s gender/teaching experience and the advisee’s academic rank was minimal. The results are interpreted in light of the theoretical model of politeness and the literature on the speech act of advising and the use of ELF.

The aim of the study was to determine the prevalence of atrial fibrillation (AF) in patients hospitalized in the therapeutic and cardiology departments of the St. Petersburg hospital, and to analyze the prevalence of the number of components of the metabolic syndrome (MS) in patients with AF and anticoagulant therapy (ACT) assigned to outpatient stage to hospitalization. A retrospective analysis of 1760 case histories of patients hospitalized in the therapeutic and cardiology departments of the Therapy Clinic of the Faculty of Pavlov State Medical University named after IP Pavlov for 2017 was performed. It was established that AF was found in 16.5% (286) patients hospitalized in 2017. Among the causes of AF in patients, the number of components of MS was analyzed: 0 – 2.1% (6), 1 – 4.5% (13), 2 – 19.2% (55), 3 – 34.3% (98), 4 – 31.5% (90), 5 – 8.4% (24), thus MS with 3 or more components installed in 74.1% (212) surveyed in 2017. 58.7% (168) received ACT at the time of hospitalization, and 41.3% did not receive it (118). In 32,9% of patients with AF did not receive anticoagulant therapy at the prehospital stage, with no contraindications for administration. Among patients receiving ACT at the time of hospitalization, direct oral anticoagulants were prescribed 2 times more often than warfarin (69% and 31%, р = 0,01).

Abstract Background and objectives The DNA methyltransferase inhibitors decitabine represent archetypal drugs for epigenetic cancer therapy. One alternative approach for the treatment of (Acute Myelocytic Leukemia) AML is the use of hypomethylating agents, including the 5-aza-2-deoxycytidine (decitabine; DAC) and other epigenetic regulators. However, the exact mechanism of DAC for drug resistance AML remains poorly understood. We conducted this study to explore how DAC regulated the sensitivity to adriamycin and aclacinomycin of AML cell line HL60/ADR and assessed the efficacy and safety of decitabine-based induction treatment patients with refractory acute myeloid leukemia. Methods In this study, we analyzing cell proliferation of HL60, HL60/ADR and KG1-¦Á cells treated by DAC against control group detected with MTS assay. Then the changes of genes methylation were determined using Methylation specific PCR (MSP), stained with anti-CD11b, before being analyzed for cell markers with flow cytometric analysis. The cell protein levels were checked with Western blotting as described elsewhere. Data were collected from refractory acute myelogenous leukemia (AML) patients treated with homoharringtonine, cytarabine, and aclarubicin after decitabine (D-HAA). Control group consisted of fludarabine, cytarabine with or without Granulocyte Colony-Stimulating Factor between 2012 and 2014. Results Different dose of DAC had proliferative inhibition effect on HL-60 cells, with time-dose dependent relationship. But the proliferative inhibition of DAC in KG1-¦Á and HL-60/ADR cells was not so effective. After been treated with 1¦ÌM DAC for 72 hours, HL-60/ADR cells were cultured with 0.5µg/ml adriamycin 1 hour. Adriamycin positive cells in DAC group were higher than control group, and the proliferative inhibition effect of adriamycin (P<0.001) and aclacinomycin (P<0.001) on HL60/ADR cells was increased (Figure 1). 1¦ÌM DAC significantly induced expression of CD11b on HL-60/ADR and KG1-¦Á cells, accompanied by cellular changes of differentiation (decreased nuclear¨Ccytoplasmic ratio, nuclear condensation, segmentation or lobulation, cytoplasmic granulation or vacuolization). Followed the increased the sensitivity of adriamycin, DNMT1 and SPRPs gene methylation was down-regulated. And ¦Â-catenin protein in nuclear was depressed after treatment of DAC. We enrolled 75 patients, of whom 71 were included in the intention-to-treat analysis. 65.2% patients in the D-HAA group achieved complete remission versus 29.2% in the FA/FLAG group (P=0.031). 2-year disease-free survival and overall survival were better in D-HAA group than FA/FLAG group (P=0.020, P=0.025; Figure 2). Conclusion These findings provide evidence for clinic protocols using DAC to reversed drug resistance in refractory AML cell. Disclosures No relevant conflicts of interest to declare.

Abstract:
The Act of 21 November 1996 on museums, which has been in force for over 20 years (Journal of Laws of 2017, item 972, as amended), has been amended dozen or more times. Seven of these amendments entered into force in the last two years (2016–2017). They were to a large extent of adjustment character, and concerned inter alia the competence requirements for museum professionals (Journal of Laws of 2017, item 60), removals of museum exhibits from museum inventory (Journal of Laws of 2016, item 1330, and of 2017, item 1086) and the Council of Museums being replaced by the Council of Museums and Memorial Sites (Journal of Laws of 2016, item 749). Amendments concerning the admission charges in museums were of systemic character (Journal of Laws of 2017, item 132) as well as the restitution amendments introduced in Article 57 of the Act of 25 May 2017 on restitution of the national cultural goods, including a new penal provision added to the Act on museums (Art. 34.a in a new chapter 5.a – Journal of Laws of 2017, item 1086). Apart from amendments described in this article, others ought to be mentioned – related to an informative aspect. In this context, the amendments to the Act on museums being in force since 16 June 2016, provided for in Article 29 of the Act of 25 February 2016 on the re-use of public sector information (Journal of Laws of 2016, item 352, as amended) are of great significance. References to this Act can be found in section 4 of Art. 25 and section 4 of Art. 25.a of the Act on museums, added by this amendment, which regulate introducing and charging fees for museum exhibits being prepared and made accessible for different from usual purposes, and for permitting to use their images. Article 31.a of the Act on museums added by this amendment has been repealed by the Art. 34 of the Act of 10 June 2016 on delegating customer services to employees (Journal of Laws of 2017, item 132). The latter Act added Article 30.a of the Act on museums, related in its contents to this regulation. It states that the access to information for safeguarding museum exhibits is limited for the sake of protection from fire, theft and other type of danger which could bring damage or loss of the museum collection.

Abstract:
Dose adjustment of unfractionated heparin (UFH) anticoagulation is an important factor to reduce hemorrhagic events. High doses of heparin can be monitored by Activated Clotting Time (ACT). Because of limited information about the monitoring of low-dose heparin we assessed monitoring by ACT, aPTT and anti-Xa. Blood samples from healthy volunteers (n = 54) were treated ex vivo with increasing UFH doses (0-0.4 IU/ml). Samples from ICU-patients (n = 60), were drawn during continuous UFH infusion. Simultaneous ACT measurements were performed using iSTAT and Hemochron. In UFH treated blood, iSTAT and Hemochron showed a significant change of ACT at ≥0.075 IU/ml and ≥0.1 IU/ml UFH, respectively. In ICU-patients no relationship between ACT and either UFH dose, aPTT and anti-Xa was observed. Hemochron was affected by antithrombin and platelet count. iSTAT was sensitive to CRP and hematocrit. A moderate correlation was identified between UFH dose and aPTT (R2 = 0.196) or anti-Xa (R2 = 0.162). In heparin-spiked blood, ACT is sensitive to heparin at levels of ≥0.1 IU/ml heparin. In ICU-patients, ACT did not correlate with UFH dose or other established methods. Both systems were differently influenced by certain parameters.

Abstract:
IntroductionSexual offenders are classified in terms of the act they have committed, diagnosis of sexual preference disorder (paraphilic disorder), and the potential motives behind the act. The typology that is often used in forensic-sexological practice is the division into preferential and non-preferential perpetrators, i.e. perpetrators showing or not showing a sexual preference disorder.ObjectivesThe aim of the study was to assess whether psychosocial and personality variables significantly differ between the group of preferential and non-preferential sexual offenders.MethodsThe study involved 120 persons, including 60 preferential and 60 non-preferential sexual offenders. The participants were presented with selected, standardized psychological tools to personality traits, self-esteem, life satisfaction, capacity to understand emotions, attachment style.ResultsThe study involved 120 persons, including 60 preferential and 60 non-preferential sexual offenders. The participants were presented with selected, standardized psychological tools to personality traits, self-esteem, life satisfaction, capacity to understand emotions, attachment style.ConclusionsDifferences between the both study groups and the male standardization sample suggest worse psychosocial functioning of sexual offenders. A critical analysis of the methodological limitations of this study have been presented.Conflict of interestScientific work was financed from the budget for science in the years 2017-2021, as a research project DI 16/003046 under the programme „Diamond Grant”.
