Dietary intolerance and poor oral intake account for a disproportionate number of emergency department visits and readmissions after bariatric surgery. Micronutrient, vitamin, and protein deficiencies can occur after both malabsorptive and restrictive weight-loss operations, and they are best mitigated against by conscientious preoperative counseling and vigilance in follow-up. Routine vitamin supplementation can prevent the need for unnecessary laboratory testing, while symptoms of dumping syndrome can frequently be managed with dietary and behavioral modification alone. Alternative enteral feeding access for alimentary supplementation can be safely performed surgically or with assistance by interventional radiology, and should be considered in the management of perforation, early anastomotic leak, surgical revision, or patients with refractory malnourishment.

Los textos e investigaciones sobre el acceso a la justicia revelan la polisemia que encierran estos términos. El rótulo de “acceso a la justicia” se aplica a diversas situaciones, lo que hace que su definición se haya vuelto no sólo necesaria, sino paulatinamente más elaborada, a partir de las contribuciones de las investigaciones sucesivas. Y esto es así, pues por un lado, tal como se aclara en uno de los artículos iniciales de esta colección, se ha producido una transformación histórica en el modo de entender el fenómeno. Por el otro, el mismo proceso de investigación sobre el acceso a la justicia ha enriquecido y ampliado los márgenes de estudio. Sin abundar en detalles, podría decirse que, al menos desde el punto de vista sociológico, en el tratamiento del tema, la discusión sobre los aspectos y soluciones institucionales se ve complementado con indagaciones sobre estrategias y propuestas que los mismos sectores excluidos de la justicia tienden a generar. Desde la perspectiva sociojurídica, lo que despierta interés y debate y lo que se cuestiona es el no acceso a la justicia del estado, o visto de otro modo, la injusticia del no acceso por parte de diversas categorías de personas definidas de varias maneras: “sectores vulnerables”, “desaventajados”, “excluidos”, “desafiliados sociales”, o“marginados”, los que, de algún modo, detentan como rasgo común la pobreza y precariedad económica y a partir de ello la falta de poder y la debilidad política. Cuando se habla de acceso a la justicia por lo general, entonces, se hace referencia a su ausencia y a la desprotección jurídico-política que ello genera. Dentro de esta línea de pensamiento, los artículos que integran este libro toman como tema central la alienación jurídica de sectores pobres periurbanos de la ciudad de La Plata y centran la atención en la percepción y las prácticas que los vecinos desarrollan para hacer frente, por un lado, al abandono sociojurídico del estado y por el otro, a las tensiones y peligros cotidianos generados por algunos actores con los que comparten el mismo ámbito de convivencia.

Memory and Attention Adaptation Training (MAAT) is a cognitive-behavioral therapy (CBT) designed to help cancer survivors self-manage and mitigate the late and long-term effects of cancer and cancer therapy on memory function. Cancer-related cognitive impairment (CRCI) is a set of mild to moderate memory and attention impairments that can have an adverse influence on quality of life. CRCI symptoms tend to present during active treatment, but for some individuals cognitive changes can persist for years. While the exact prevalence of CRCI is unknown, review of the literature estimates that nearly half of all survivors may experience some form of CRCI. Causes of CRCI are multiple and are the subject of continued research. Chemotherapy, genetic vulnerability, neurovascular damage, inflammation, and hormonal/endocrine disruption have all been identified as candidate mechanisms of persistent cognitive change. Given the multiple causal mechanisms, finding a biomedical treatment for CRCI remains elusive. MAAT was developed as a CBT to help cancer survivors make adaptive behavioral and cognitive changes to improve performance in the valued activities that CRCI hinders. MAAT consists of eight visits and has been designed for administration through telehealth technology, improving access to the survivorship care that so many cancer survivors may lack after the time and expense of cancer treatment. Survivors can use this workbook to reinforce their in-session learning and continue to build adaptive coping.

Fragile states are a menace. Their lawless environments spread instability across borders, provide havens for terrorists, threaten access to natural resources, and consign millions of people to poverty. But Western attempts to reform these benighted places have rarely made things better. Kaplan argues that to avoid revisiting the carnage and catastrophes seen in places like Iraq, Bosnia, and the Congo, the West needs to rethink its ideas on fragile states and start helping their peoples build governments and states that actually fit the local landscape. Fixing Fragile States lays bare the fatal flaws in current policies and explains why the only way to give these places a chance at peace and prosperity is to rethink how development really works. Flawed governance systems, not corrupt bureaucrats or armed militias, are the cancers that devour weak states. The cure, therefore, is not to send more aid or more peacekeepers but to redesign political, economic, and legal structures-to refashion them so they can leverage local traditions, overcome political fragmentation, expand governance capacities, and catalyze corporate investment. After dissecting the reasons why some states prosper and others sink into poverty and violence, Fixing Fragile States visits seven deeply dysfunctional places—including Pakistan, Bolivia, West Africa, and Syria—and explains how even the most desperate of them can be transformed.

Percutaneous drainage of an abscess is the standard of care and significantly improves patient outcomes. Typically, either trocar or Seldinger technique is used to access the abscess under ultrasound or computed tomography guidance. Pre-procedural planning is the most important step of the procedure, with the goal of using a safe, direct, short route to the collection. Vital anatomic structures such as sizeable arteries, solid organs (with a few exceptions), or a hollow viscus such as the stomach or colon should be avoided. Placement of a drain must balance effective positioning with a safe anatomic window for access. A multiloculated collection presents a special challenge because only one safe catheter approach may be available. In these cases, catheters with multiple side holes or multiple loops can be used to access the separate isolated components. Tissue plasminogen can also be utilized to break down the loculations and improve drainage.

Abstract:
Memory and Attention Adaptation Training (MAAT) is a cognitive-behavioral therapy (CBT) designed to help cancer survivors self-manage and mitigate the late and long-term effects of cancer and cancer therapy on memory function. Cancer-related cognitive impairment (CRCI) is a set of mild to moderate memory and attention impairments that can have an adverse influence on quality of life. CRCI symptoms tend to present during active treatment, but for some individuals cognitive changes can persist for years. While the exact prevalence of CRCI is unknown, review of the literature estimates that nearly half of all survivors may experience some form of CRCI. Causes of CRCI are multiple and are the subject of continued research. Chemotherapy, genetic vulnerability, neurovascular damage, inflammation, and hormonal/endocrine disruption have all been identified as candidate mechanisms of persistent cognitive change. Given the multiple causal mechanisms, finding a biomedical treatment for CRCI remains elusive. MAAT was developed as a CBT to help cancer survivors make adaptive behavioral and cognitive changes to improve performance in the valued activities that CRCI hinders. MAAT consists of eight visits and has been designed for administration through telehealth technology, improving access to survivorship care that so many cancer survivors may lack after the time and expense of cancer treatment. Survivors are provided a workbook they can use to work with their clinician and to reinforce learning and adaptive coping. This clinician manual guides the clinician step by step on MAAT administration and provides background on the theoretical underpinnings of CRCI and MAAT.

Abstract:
The introduction gives an overview of the book as a whole with a summary of all the chapters. The author positions herself in relation to the topic as the blind daughter of two visual artists, therefore both a potential consumer of museum access programs, while simultaneously critical of their shortcomings. She observes that museum access programs typically seem designed either for blind children or else for blind adults who have led such isolated lives that they are unfamiliar with terms associated with vision and visual art. The author also speculates on how this study of one minority—blind and visually impaired people—and one cultural site—the art museum—could serve as a model for future inquiry.
