Abstract:
Treatment and curative medical care often require medication. This chapter focuses on the provision of medications in impoverished settings and the challenges that inhibit access to life-saving drugs. It will review the failure of the for-profit market to increase drug access for the poor. The evolving concept of essential drugs will be explored by reviewing the history the WHO Essential Medicines List (EML) and the fight to expand the list to include new, and often patented medicines. The international treaties and policies that impact drug availability will be highlighted as will novel systems for drug development and distribution. Finally, the chapter will highlight the growing movement to decrease costs, increase supply, and advance development of drugs for neglected diseases affecting impoverished people.

Abstract:
The essential medicines concept developed by the World Health Organization (WHO), states that there is a list of minimum medicines for a basic health-care system, including the most efficacious, safe, and cost-effective ones for priority conditions. According to the WHO, essential medicines are those that satisfy the primary health-care needs of the population. Thus, they should always be available, affordable, and cost-effective. However, the vast majority of the global population does not have access to essential medicines. Many organizations have called on governments to adopt policies to improve availability of these essential medicines and developed programmes and projects to increase awareness and knowledge. This chapter describes two such projects developed by the International Association for Hospice and Palliative Care (IAHPC), based on the essential medicines concept to improve access to medicines and appropriate palliative care: the IAHPC List of Essential Medicines in Palliative Care and the Opioid Essential Prescription Package.

The narrow therapeutic index associated with most analgesic opioids creates a high degree of risk, obliging caution in patient selection; this chapter describes screening practices. More attention is due to the use of opioid therapy for chronic non-cancer pain as episodes of respiratory arrest increase. Universal precautions are recommended for all patients. Before and throughout treatment, selected tools assessing risk and misuse should be employed. Increased access to the opioid antagonist naloxone has followed the increase in opioid poisoning deaths; the Risk Index for Overdose or Serious Opioid-induced Respiratory Depression (RIOSORD) helps determine the likelihood of an opioid overdose, serving to select candidates for at-home naloxone access. Urine drug testing aids treatment plan adherence. Tables include: a comparison of screening tools for risk stratification; a comparison of tools for assessment of opioid misuse; the scoring procedure for the RIOSORD; and a list of risk classes with predicted probabilities.

In this chapter we illustrate the biological applications of small angle scattering. We cover examples of contrast matching has been used in the neutron case and with SAXS,. time-resolved studies that have helped reveal mechanism and how SAXS or SANS as a solution technique can access information missing from other studies, in particular residues that may be crystallographically undefined. We show examples of puting known structural information in context, and following large-scale functional changes or show when distinct populations co-exist in solution. The list of examples provided is not exhaustive and should be seen as a glimpse of the potential of biological mechanisms where SAXS or SANS can provide unique information complementary to other methods and help define function and mechanism.
