Abstract:
ABSTRACTBackground:Dementia is an emerging health priority in Australian Aboriginal communities, but substantial gaps remain in our understanding of this issue, particularly for the large urban section of the population. In remote Aboriginal communities, high prevalence rates of dementia at relatively young ages have been reported. The current study is investigating aging, cognitive decline, and dementia in older urban/regional Aboriginal Australians.Methods:We partnered with five Aboriginal communities across the eastern Australian state of New South Wales, to undertake a census of all Aboriginal men and women aged 60 years and over residing in these communities. This was followed by a survey of the health, well-being, and life history of all consenting participants. Participants were also screened using three cognitive instruments. Those scoring below designated cut-offs, and a 20% random sample of those scoring above (i.e. “normal” range), completed a contact person interview (with a nominated family member) and medical assessment (blind to initial screening results), which formed the basis of “gold standard” clinical consensus determinations of cognitive impairment and dementia.Conclusion:This paper details our protocol for a population-based study in collaboration with local Aboriginal community organizations. The study will provide the first available prevalence rates for dementia and cognitive impairment in a representative sample of urban Aboriginal people, across city and rural communities, where the majority of Aboriginal Australians live. It will also contribute to improved assessment of dementia and cognitive impairment and to the understanding of social determinants of successful aging, of international significance.

Abstract:
This article contributes to an expanding literature concerned with the instrumentality of accounting and the consequences of its use within government—Indigenous relations. It examines a single case of how accounting was employed within the Australian state of New South Wales to manipulate the income and spending of Aboriginal women. The article explores how ccounting was integral to the control and administration of the New South Wales Family Endowment Payments; a policy intended to reconstitute Aboriginal women according to particular norms of citizenship. The article not only allows us to better understand the roles of accounting in such historical practices of social engineering, but also illustrates that the objectives for such programmes are not simple and that often they attempt to satisfy the competing interests of the social and the economic.

Abstract:
John White, Surgeon-General of New South Wales, is best remembered for his handsome book Journal of a voyage to new South Wales published in London during 1790. He was a native of County Fermanagh in northwestern Ireland. He became a naval surgeon and in this capacity was appointed to serve as surgeon on the First Fleet which left England for New South Wales (Australia) in 1787. While living in New South Wales, White adopted Nanberree, an aboriginal boy, and fathered a son by Rachel Turner, a convict, who later married Thomas Moore. John White returned to England in 1795, became a Fellow of the Linnean Society of London and was granted the degrees of Doctor of Medicine and Master of Arts by the University of St Andrews. White was married twice, and was survived by his second wife and his four children, including his illegitimate, Australian-born son, Captain Andrew Douglas White. Dr John White died in 1832 aged 75 and is buried in Worthing, Sussex, England.While serving as Surgeon-General at Sydney Cove, New South Wales, between 1788 and 1794 John White collected natural history specimens and assembled a series of paintings of plants and animals. After returning to England, White lent these paintings to botanists and zoologists, and permitted copies to be made. Thus, he contributed substantially to European knowledge of the indigenous flora and fauna of Australia.

Abstract:
This paper brings to the reader‟s attention a history of tobacco smoking that arguably had a negative effect on the health of Aboriginal communities in the Hunter region of central eastern New South Wales during the early colonial contact period from 1800 to 1850. Furthermore, it will also be shown that tobacco was used by colonists to engage the services of Aboriginal people, not only in Aboriginal communities in the Hunter region, but further afield across many other frontiers of colonial expansion in Australia in the 19th century. It will be demonstrated through primary archival and secondary sources that colonists utilised tobacco as a coercive agent to appease, befriend, pacify, coerce and remunerate Aboriginal People, resulting in widespread addiction. It is argued that tobacco smoking not only undermined the health of traditional communities, but also this unhealthy habit has been largely overlooked in measurements of the impact of colonization on the health of Indigenous people. While historians widely acknowledge that exotic diseases such as smallpox had a negative effect on the health of Aboriginal People, it is rarely considered in contemporary historical accounts that tobacco had an even more insidious effect on the well being of Aboriginal societies during the early colonial contact period. Furthermore, while diseases such as smallpox have hopefully disappeared forever, health destroyers like tobacco have endured and continue to impact on Aboriginal health. Finally, this paper recognises the enormity of challenges faced by health authorities, and indeed Indigenous Australians, in contemporary society in combating a chronic problem that has been embedded in Aboriginal post-colonial culture during the long course of European occupation.

In 2014 Indigenous theatre director Wesley Enoch announced in an interview that ‘the aim of Indigenous theatre is to write into the public record neglected or forgotten stories’. He also spoke about the aims of a new Australian play, Black Diggers, as ‘honouring and preserving’ these stories. For Enoch, Black Diggers (re)addresses a great silence in Australia’s history, that of the Aboriginal experience of war. Also in 2014, the memorial sculpture Yininmadyemi Thou Didst Let Fall, commissioned by the City of Sydney Council, aimed to place in memoriam the story of forgotten Aboriginal soldiers who served during international conflicts, notably the two world wars. Both Black Diggers and the Yininmadyemi memorial sculpture are counter-hegemonic artefacts and a powerful commentary of a time of pseudo-nationalist memorialization. Both challenge the validity of many of Australia’s socio-political and historical accounts of war, including the frontier wars that took place between Aboriginal people and European settlers. Both unsettle Australia’s fascination with a memorialized past constructed from a culture of silence and forgetfulness. Liza-Mare Syron is a descendant of the Birripi people of the mid-north coast of New South Wales in Australia. An actor, director, dramaturg, and founding member of Moogahlin Performing Arts, a Sydney-based Aboriginal company, she is currently the Indigenous Research Fellow at the Department of Media, Music, Communication, and Cultural Studies at Macquarie University, Sydney. She has published widely on actor training, indigenous theatre practice, inter-cultural performance, and theatre and community development.

A general theme in Australian prehistory is the development of the distinctive social, economic and technological systems observed in recent Aboriginal societies. Research has demonstrated significant change in the Australian archaeological sequence and general trends of such are shared by numerous regions. Most that have been investigated indicate low density occupation during the Pleistocene and early Holocene with significant increases in site numbers, increased artefact discard rates and dissemination of new technologies and artefact types in mid-to-late Holocene times (e.g. Lourandos 1985). On the other hand, each region has a unique prehistory, range of material evidence and research potential. Our knowledge of Holocene developments in aboriginal subsistence systems, for instance, is largely based upon the history of cycad exploitation in the central Queensland Highlands (Beaton 1982), the appearance of seed grindstones in arid and semi-arid zones (Smith 1986) and evidence for increased emphasis on small-bodied animals in N.E. New South Wales and S.E. Queensland (McBryde 1977:233; Morwood 1987:347).

AbstractThe last decade or so has seen a fundamental shift in Aboriginal cultural heritage law in Australia. A number of subnational jurisdictions in Australia have undergone major reforms to their Aboriginal heritage legislation. Other subnational jurisdictions are currently in the reform process or have promised reform in coming years. We use the latest (and, at the time of writing, ongoing) process to reform Aboriginal heritage legislation in the state of New South Wales (NSW) to explore some of the legal issues and themes emanating from the Australian experience. The NSW example is a useful case study for thinking about how minority heritage regulation can not only serve broader social movements but also undercut some of its own possibilities. We argue that even law that is ostensibly in place to promote the control of communities over their own heritage can cause difficult balancing acts that may default to a dependency path and effectively detract from its own projected goals.

In Australia, Aboriginal peoples have sought to exploit and challenge settler colonial schooling to meet their own goals and needs, engaging in strategic, diverse and creative ways closely tied to labour markets and the labour movement. Here, we bring together two case studies to illustrate the interplay of negotiation, resistance and compulsion that we argue has characterised Aboriginal engagements with school as a structure within settler colonial capitalism. Our first case study explains how Aboriginal families in Victoria and New South Wales deliberately exploited gaps in school record collecting to maintain mobility during the mid-twentieth century and engaged with labour markets that enabled visits to country. Our second case study explores the Strelley mob’s establishment of independent, Aboriginal-controlled bilingual schools in the 1970s to maintain control of their labour and their futures. Techniques of survival developed in and around schooling have been neglected by historians, yet they demonstrate how schooling has been a strategic political project, both for Aboriginal peoples and the Australian settler colonial state.

This is a persona] account of an Aboriginal woman who went through the education system in Australia to obtain finally her law degree. Aboriginal people experience many hurdles in the education system. Many Aboriginal children feel alienated within the legal system which until recently focused on a colonial history of Australia, ignoring the experiences, indeed the presence, of indigenous people in Australia. The Australian government had a policy of not educating Aboriginal people past the age of 14. The author was one of the first generation that could go straight from high school to university. She speaks of the debt she feels towards the generations of her people that fought for her right to access to higher education. The author went on to become the first Aboriginal person to be accepted into Harvard Law School which brought different personal challenges and allowed for reflection on comparisons of the sensitivity towards race in both education systems. When the author returned to Australia, she took a position teaching at the University of New South Wales. She had to come to terms with working within a system that she had felt alienated within as a student. Her position at the front of the class has created a sense of empowerment that she can pass on to her Aboriginal and female students.

Human awareness of plants in Australia goes back 50,000 years when the aboriginal first began using plants to treat, clothe and feed themselves. The European influence came in 1778 with the First Fleet landing in New South Wales. Australia's earliest records of using horticulture for therapy and rehabilitation were in institutions for people with intellectual disabilities or who were incarcerated. Eventually, legislation created greater awareness in the government and community for the needs of persons with disabilities, and many worthwhile projects, programs and organizations were established or gained greater recognition. Horticultural therapy programs may be found in nursing homes, rehabilitation centers, adult training support services, hospitals, day centers, community centers and gardens, educational institutions, supported employment, and the prisons system. This article reviews the history and development of Australian horticulture as a therapy in the treatment of disabilities and social disadvantaged groups, and includes an overview of programs offered for special populations and of Australia's horticultural therapy associations. It also discusses opportunities for research, teaching and extension for horticultural therapy in Australia.

In 2009, the State Library of New South Wales acquired a collection of 201 letters written by the Royal Navy officer John Septimus Roe. Dating between 1807 and 1829, these letters cover Roe's time serving with Phillip Parker King on the Australian coastal survey voyages of the Mermaid and Bathurst (1817–1823), and later with James Bremer on the Tamar on the Australian north coast and in Southeast Asian waters (1824–1827). This article, based on a close study of the letters, explores how Roe's interest in natural history and ethnography developed during this time, leading to the establishment of an extensive private museum, with the particular encouragement of his brother William Roe, at the family home, the rectory of the church of St Nicolas, Newbury, Berkshire. Roe took advantage of his time, while surveying areas of Australia largely unknown to Europeans, to make a collection of some scientific importance, but the museum was sold and dispersed in 1842, so that the close reading of the letters provides the only substantive account of its contents. The letters also provide an opportunity to make a case study of the web of connections – and opportunities for promotion – that collecting provided for a then quite junior British naval officer. Although no item with a confirmed provenance to the museum is recorded, it is hoped that this article may provide clues that will lead to the unearthing of specimens acquired by Roe which formed part of his enormous natural history collection, and also to the Aboriginal spears, weapons and other implements collected from the remoter stretches of the Australian coast.

Environmental history, as it has emerged in recent years, is most distinctive in the way it illustrates a serious engagement between the disciplines of ecology and history. This article begins with an exploration of the lineage and promise of environmental history, particularly in the Australian setting. It then analyses a number of the cultural debates about vegetation change in Australia—about clearing, open landscapes, scrub encroachment and burning practices—and draws attention to the way that morals, politics and aesthetics shaped environmental perception and still do. Clearing was the dominant discourse in the history of landscape change and a legislative requirement for secure settlement. At the same time, criticism of clearing and its effects represented an early conservationist sensibility, but the heroic pioneering labour of clearing, the political imperatives associated with it and the escalating ecological legacy it generated, have sometimes made us forget how open was much of the Australian landscape when Europeans first arrived. The morality of clearing—the arguments for and against—focused the minds of settlers on the trees and the loss of them, while the aesthetics of pastoralism attracted their eyes to the grasslands and made them rejoice in the curious legacy of 'open' landscapes. In the early nineteenth century, the most common usage of the word 'forest' was to describe land fit to graze: 'according to the local distinction, the grass is the discriminating character [of forest land] and not the Trees'. At the same time, pastoralists were unwilling to recognise the role of Aboriginal people in creating such open landscapes and this reticence to acknowledge the Aboriginality of the pastoral economy persists today. This in turn affected the way settlers perceived the new forests that appeared after European invasion. The fate of the vegetation Europeans found has understandably been so much the focus of science and history—its removal, replacement, utilisation, modification and conservation—that 'new forests' easily escape scholarly attention; and being new, they seem far less valuable and threatened. They have generally been perceived as a nuisance, as enclosing and encroaching, as 'scrub', as 'woody weeds'. The politics of understanding regrowth are related not only to the issues of clearing and density, but especially to the culture of burning in Aboriginal and settler society and its implications for management and biodiversity. If the coming together of ecology and history best defines the new 'environmental history', then the most illuminating confluences are those where each discipline helps the other to identify what constitutes a unique 'event', both ecologically and historically. The article therefore finishes with examples of events in two landscapes—the long drought of the 1890s in western New South Wales and the Black Friday bushfires of 1939 in the mountain ash forests of Victoria—to illustrate how each emerges as an intriguing artefact of nature and history, a cultural exaggeration of a natural rhythm. Even as we discover the ecological depth of each apparently 'natural' event, we are reminded of its historical specificity.

Exactly a hundred years before this conference, in August 1900, a Royal Commission was appointed which 'deserves a prominent, if not defining, place in Australian environmental history' (Quinn 1995). This paper explores the social, political and environmental context of this very significant inquiry. Beginning with six edited extracts from the Commission's transcript of evidence, the paper reflects upon the enduring relevance of the inquiry today. It describes the nature of European occupation of the western lands of New South Wales in the 1860s and 70s — a period when there appeared no physical limit to pastoral expansion — and then summarises the environmental crisis of the final 20 years of the century. Nineteenth-century debates about land reform were dominated by the class struggle between squatters and selectors and by the imperative to occupy, for strategic and moral purposes, what were regarded as vacant lands. The 1901 Royal Commission gave early voice to environmental arguments for occupation, and not just cultural ones, and there was a recognition that European settlers had disrupted earlier, Aboriginal systems of habitation and management and tipped the land into an escalating instability. Legislators began to argue that the land needed people as much as people needed the land. The paper concludes with the reflection that it is not just the formality of a centenary that makes us want to listen carefully to the voices unearthed by the 1901 Royal Commission. Science is now more integrative of the social and humanist perspective than it was in the middle of the 20th century; it is more receptive to the testimony of people living on and working the land, and more eager to enter into a dialogue with them and their history.

This article offers a critical exploration of social studies textbooks and allied curriculum materials used in New South Wales primary schools between 1930 and 1960, and of the way in which these texts positioned, discussed, and assessed Aboriginal Australians. With reference to European commitments to Enlightenment philosophies and social Darwinian views of race and culture, the author argues that Aboriginal peoples were essentialized via a discourse of paternalism and cultural and biological inferiority. Thus othered in narratives of Australian identity and national progress, Aboriginal Australians were ascribed a role as marginalized spectators or as a primitive and disappearing anachronism.

Background This paper examines condom use in a sample of Aboriginal young people in New South Wales (NSW) aged 16–30 years. Methods: Cross-sectional data were collected using hand-held computer devices from 293 Aboriginal people attending two Aboriginal events in NSW. Results: Almost two-thirds of respondents reported having had a casual sex partner in the previous 6 months. Of these, 39.2% reported always using a condom with casual partners. Having always used a condom with casual partners varied among respondents, and was more likely among younger respondents (adjusted odds ratio (AOR): 2.7, 95% confidence interval (CI): 1.2–6.1) and less likely among those who used illicit drugs (AOR: 0.2, 95% CI: 0.1–0.7). Conclusions: In comparison to published studies of other Australians, casual sex appears to be more common among this sample of Aboriginal young people; however, the proportion who report having always used condoms with casual partners is very similar. This suggests that although casual sex is more common, Aboriginal young people do not engage in risky behaviour any more often than other young Australians. Further work should be conducted with those who do not always use condoms, such as those who are older and who use illicit drugs, particularly with regards to how abstinence from drug use supports protective behaviours such as condom use among this population of Aboriginal young people.

Background: Australia’s healthcare system is complex and fragmented which can create challenges in healthcare, particularly in rural and remote areas. Aboriginal people experience inequalities in healthcare treatment and outcomes. This study aimed to investigate barriers and enablers to accessing healthcare services for Aboriginal people living in regional and remote Australia. Methods: Semi-structured interviews were conducted with healthcare delivery staff and stakeholders recruited through snowball sampling. Three communities were selected for their high proportion of Aboriginal people and diverse regional and remote locations. Thematic analysis identified barriers and enablers. Results: Thirty-one interviews were conducted in the three communities (n = 5 coastal, n = 13 remote, and n = 13 border) and six themes identified: (1) Improved coordination of healthcare services; (2) Better communication between services and patients; (3) Trust in services and cultural safety; (4) Importance of prioritizing health services by Aboriginal people; (5) Importance of reliable, affordable and sustainable services; (6) Distance and transport availability. These themes were often present as both barriers and enablers to healthcare access for Aboriginal people. They were also present across the healthcare system and within all three communities. Conclusions: This study describes a pathway to better healthcare outcomes for Aboriginal Australians by providing insights into ways to improve access.

During the early 1970s, large numbers of Aboriginal people became tenants of the Housing Commission of New South Wales under the Housing for Aborigines program. Most moved from government reserves or dilapidated and overcrowded private rental dwellings to broadacre suburban estates. As public housing tenants, they encountered considerable pressures to become 'respectable' citizens, to build their lives around privacy, sobriety, moral restraint, the nuclear family, conventional gender roles and wage labour. For many indigenous Australians, these expectations-which were based as much on class relations as on colonialism— represented a threat to their conventional ways of life and their obligations to extended family and community. This paper explores the patterns of conformity and resistance amongst Aboriginal tenants. It draws on the sociological and cultural studies literature on youth subcultural resistance and compares anthropological theory about indigenous responses to the pressures of modernity.

Self-reported health is a holistic measure and may fit well with Aboriginal views of health and well-being. This study aimed to identify predictors of self-reported health in older Aboriginal Australians and determine acceptable research methodologies for future aging research.Design:Longitudinal, population-based study.Setting:Five communities across New South Wales, Australia (two urban and three regional sites).Participants:Aboriginal and Torres Strait Islander people (n = 227; 60–88 years, M = 66.06, SD = 5.85; 145 female).Measurements:Participants completed baseline (demographic, medical, cognitive, mental health, and social factors) and follow-up assessments (self-reported health quantified with 5-point scale; sharing thoughts on areas important for future research). Predictors of self-reported health were examined using logistic regression analyses.Results:Self-reported health was associated with sex, activities of daily living, social activity participation, resilience, alcohol use, kidney problems, arthritis, falls, and recent hospitalization. Arthritis, kidney problems, and resilience remained significant in multiple logistic regression models.Conclusions:Perceived resilience and the absence of certain chronic age-related conditions predict older Aboriginal peoples’ self-reported health. Understanding these factors could inform interventions to improve well-being. Findings on acceptable research methodologies suggest that many older Aboriginal people would embrace a range of methodologies within long-standing research partnerships, which is an important consideration for Indigenous population research internationally.

The Prime Minister blessed the occasion with his presence and a short speech. Three of the former Chairs of the the National Aboriginal Education Committee were there, as were Aboriginal and Torres Strait Islander educationists from most Australian states. Only New South Wales, which decided to boycott the launch, wasn’t officially represented.There are two reasons for calling the policy that the Minister launched the Aboriginal Education Policy. Firstly, because it is the first policy formally endorsed by any National government; and secondly, because it responds to the call made in the 1988 Report of the Aboriginal Education Policy Task Force, for a concerted national effort – to achieve broad equity between Aboriginal people and other Australians in access, participation and outcomes at all stages of education. Alcohol use and related injuries are a leading risk factor for deaths and disabilities in Australia, particularly for Aboriginal and Torres Strait Islander people. An improved understanding of individual and geographical community characteristics that are significantly associated with higher rates of alcohol-related injuries for specific populations can contribute to more effective efforts aimed at reducing alcohol-related injuries. For Aboriginal and non-Aboriginal Australians in New South Wales, this study used emergency department (ED) data to investigate rates of alcohol-related injuries, whether differences in rates vary between communities, and individual and community characteristics significantly associated with alcohol-related injuries. Differences in rates of alcohol-related injuries between Aboriginal and non-Aboriginal people varied significantly between communities. Being younger than 38 years old was significantly associated with increased risk of alcohol-related injuries, independent of Aboriginal status and gender. Increased disadvantage of the geographical community inhabited was associated with increased alcohol-related injuries for males. For Aboriginal males, living in a regional community was significantly associated with increased alcohol-related injuries, compared to living in major cities. Conversely, for non-Aboriginal people, living in regional communities was significantly associated with fewer alcohol-related injuries. It is therefore likely that an explanation for between-community differences can be found in regional communities.

In 1852, the naturalist and writer Louisa Meredith observed in her book My Home in Tasmania: “I know of no place where greater order and decorum is observed by the motley crowds assembled on any public occasion than in this most shamefully slandered country: not even in an English country village can a lady walk alone with less fear of harm or insult than in this capital of Van Diemen's Land, commonly believed at home to be a pest-house, where every crime that can disgrace and degrade humanity stalks abroad with unblushing front.”Meredith's paean to life in the notorious Australian penal colony of Hobart was in stark contrast to her earlier, highly unfavourable account of colonial Sydney. It papered over the years of personal hardship she had endured in Australia, as well as avoiding mention of the racial warfare against Tasmania's Aborigines that had afforded her such a genteel European existence.Such intra-Australian complexities, however, were lost when Meredith's account was superimposed onto German debates about the desirability of penal colonies for Germany. Instead, Meredith's portrait of a cultivated city emerging from the most notorious penal colony in Australia was presented as proof that the deportation of criminals was an important dimension of the civilising mission of Europe in the extra-European world. It was also presented as a vindication of those in Germany who wished to rid Germany of its lumpen criminal class through deportation. The exact paragraph of Meredith's account cited above was quoted in German debates on deportation for almost half a century; first in 1859 by the jurist Franz von Holtzendorff, and thereafter by Friedrich Freund when advocating the establishment of a penal colony in the Preußische Jahrbücher in September 1895.

OBJECTIVE: To estimate the incidence and outcomes of sepsis hospitalisations in Aboriginal and Torres Strait Islander and non-Indigenous residents of New South Wales. DESIGN AND PARTICIPANTS: Prospective cohort study of residents aged 45 years and older, recruited between 2006 and 2009, and followed for hospitalisation for sepsis. MAIN OUTCOME MEASURES: Incidence and hazard ratio (HR) of sepsis hospitalisation and intensive care unit (ICU) admission identified using International Classification of Diseases (10th revision) coding on discharge data. Length of stay, readmission and mortality in those admitted for sepsis. RESULTS: Of 264 678 participants, 1928 (0.7%) identified as Aboriginal and/or Torres Strait Islander. Sepsis hospitalisation was higher in Aboriginal and Torres Strait Islander participants (8.67 v 6.12 per 1000 person-years; age- and sex-adjusted HR, 2.35; 95% CI, 1.98–2.80) but was attenuated after adjusting for sociodemographic factors, health behaviour and comorbidities (adjusted HR, 1.56; 95% CI, 1.31–1.86). Among those hospitalised for sepsis, after adjusting for age and sex, there were no differences between the proportions of Aboriginal and Torres Strait Islander and non-Indigenous participants admitted to an ICU (18.0% v 16.1%; P = 0.42) or deceased at 1 year (36.1% v 36.8%; P = 0.92). Aboriginal and Torres Strait Islander participants had shorter lengths of hospital stay (9.98 v 11.72 days; P < 0.001) and ICU stay (4.38 v 6.35 days; P < 0.001) than non-Indigenous participants. Overall, more than 70% of participants were readmitted to hospital within 1 year. CONCLUSION: We found that the rate of sepsis hospitalisation in NSW was higher for Aboriginal and Torres Strait Islander adults. Culturally appropriate, community-led strategies targeting chronic disease prevention and the social determinants of health may reduce this gap. Preventing readmission following sepsis is a priority for all Australians.

Despite 42 years progress since the 1967 referendum enabling laws to be made covering Aboriginal Australians their poor health status remains and is extensively documented. This paper presents results of a study into Cultural Awareness Training (CAT) in New South Wales and specifically South West Sydney Area Health Service (SWSAHS) with the aim of improving long-term health gains. The evidence demonstrates poor definition and coordination of CAT with a lack of clear policy direction and accountability for improving cultural awareness at government level. In SWSAHS staff attendance at training is poor and training is fragmented across the Area. The paper proposes actions to improve Aboriginal cultural awareness for health professionals including incorporating Aboriginal CAT into broader based Cross Cultural Training (CCT). What is known about the topic? Cross-cultural education programs for both Aboriginal and non-Aboriginal health industry staff are poorly coordinated, delivered and evaluated. There is recognition that improvements in this area could bring real enhancements in service delivery and health outcomes. What does this paper add? The deficiencies in Aboriginal CAT programs in general are explored and specifically identified in one large NSW health area with a major urban Aboriginal population. This paper reviews CAT themes in the literature and evaluates the effectiveness of known programs. What are the implications for practitioners? The authors list a series of recommendations that have the potential to improve awareness of Aboriginal cultural issues to provide a basis for development of effective and comprehensive CAT programs to bring real improvements in service delivery.

Purpose The purpose of this paper is to examine correlates and predictors of hazardous drinking behaviour, that may be considered evidence of generalised strain, in a sample of incarcerated non-Aboriginal males in New South Wales, Australia. Design/methodology/approach Data were collected from 283 non-Aboriginal male inmates as part of a larger epidemiological survey of inmates in NSW undertaken in 2015 by the Justice Health and Forensic Mental Health Network. Data relating to a range of social factors were selected with reference to relevant literature and assessed with regards their predictive value for scores from the Alcohol Use Disorders Identification Test (AUDIT). To facilitate regression analysis, variables were logically organised into historical factors or adult factors. Findings Almost all participants reported some history of alcohol consumption. Hazardous drinking was common among participants. While parental alcohol problems and adult drug use were the only correlates of AUDIT scores, parental misuse of alcohol was shown to be an important predictor of AUDIT scores in regression analysis. The role of parent gender was inconclusive. Previous incarceration as an adult, employment status, and drug use as an adult also predicted AUDIT scores. Originality/value Alcohol abuse is common among inmates and the use of alcohol is implicated in the commission of many offences. A better understanding of its genesis may inspire novel approaches to treatment, leading to improved health outcomes for inmates.

Recent erosion in arid regions of western NSW has exposed large areas that are scattered with stone artefacts manufactured by Aboriginal people in prehistory. These exposures offer an opportunity for archaeologists to study the artefacts abandoned by Aboriginal people through time and to compare those artefacts that accumulate in different parts of the landscape. To reconstruct the nature of prehistoric behaviour in the rangelands, two approaches are needed. First, the geomorphological context of the artefacts needs to be considered since exposure of the artefacts is a function of landscape history. Second, large areas (measured in thousands of square metres) and large numbers of artefacts need to be considered if patterns reflecting long-term abandonment behaviour by Aboriginal people are to be identified. This paper reports on the Western New South Wales Archaeological Program (WNSWAP) which was initiated in 1995 to study surface archaeology in the rangelands. Geomorphological studies are combined with artefact analysis using geographic information system software to investigate Aboriginal stone artefact scatters and associated features such as heat retainer hearths, in a landscape context. Results suggest that apparently random scatters of stone artefacts are in fact patterned in ways which inform on prehistoric Aboriginal settlement of the rangelands. Objective Government expenditure on and the number of aged care facilities in Australia have increased consistently since 1995. As a result, a range of aged care policy changes have been implemented. Data on demographics and utilisation are important in determining the effects of policy on residential aged care services. Yet, there are surprisingly few statistical summaries in the peer-reviewed literature on the profile of Australian aged care residents or trends in service utilisation. Therefore, the aim of the present study was to characterise the demographic profile and utilisation of a large cohort of residential aged care residents, including trends over a 3-year period. Methods We collected 3 years of data (2011–14) from 77 residential aged care facilities and assessed trends and differences across five demographic and three service utilisation variables. Results The median age at admission over the 3-year period remained constant at 86 years. There were statistically significant decreases in separations to home (z = 2.62, P = 0.009) and a 1.35% increase in low care admissions. Widowed females made up the majority (44.75%) of permanent residents, were the oldest and had the longest lengths of stay. One-third of permanent residents had resided in aged care for 3 years or longer. Approximately 30% of residents were not born in Australia. Aboriginal residents made up less than 1% of the studied population, were younger and had shorter stays than non-Aboriginal residents. Conclusion The analyses revealed a clear demographic profile and consistent pattern of utilisation of aged care facilities. There have been several changes in aged care policy over the decades. The analyses outlined herein illustrate how community, health services and public health data can be used to inform policy, monitor progress and assess whether intended policy has had the desired effects on aged care services. What is known about the topic? Characterisation of permanent residents and their utilisation of residential aged care facilities is poorly described in the peer-reviewed literature. Further, publicly available government reports are incomplete or characterised using incomplete methods. What does this paper add? The analyses in the present study revealed a clear demographic profile and consistent pattern of utilisation of aged care facilities. The most significant finding of the study is that one-third of permanent residents had resided in an aged care facility for ≥3 years. These findings add to the overall picture of residential aged care utilisation in Australia. What are the implications for practitioners? Attendance at the Medicare Benefits Scheme (MBS) supported indigenous health checks are crucial to improve prevention and management of chronic health conditions. However, in conjunction with lifestyle and environmental factors, attendance rates at primary healthcare services for screening and treatment have fallen in Australia during the COVID-19 pandemic. This study aims to explore the influence of the COVID-19 pandemic on preventive health behaviours of indigenous Australians and the associated barriers to, and enablers of, engagement with health services to formulate a targeted intervention strategy.Methods and analysisA concurrent mixed-methods study (comprising quantitative and qualitative data collection methods) will be employed. Descriptive analysis of MBS data about the characteristics of indigenous peoples of Australia claiming health assessment services will be performed. Generalised estimating equation regression models will be used to examine the use of health assessment services over time. Qualitative interviews informed by indigenous research methods will be conducted. Interviews will investigate barriers to, and enablers of, engagement with health services. Thematic approach guided by the principles of indigenist praxis, storytelling and collaborative research will be used to analyse the interview data. The project commenced in July 2020 and will be completed by July 2022.Ethics and disseminationThe project received ethics approval from the Aboriginal Health and Medical Research Council of New South Wales and the University of New England Human Research Ethics Committee. AbstractThis article examines the extent to which a recent law reform initiative in New South Wales (NSW), Australia—the draft Aboriginal Cultural Heritage Bill 2018 (NSW)—advances the general principles outlined in the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP). The examination reveals some improvements on the current legal framework and some concerning proposals that distance the NSW government from the UNDRIP principles. Key concerns include a proposed transfer of administrative responsibility to Aboriginal bodies with no corresponding guarantee of funding; the continued vesting of key decision-making powers in government; inept provisions for the protection of secret knowledge; and lower penalties for harming cultural heritage than for related offences in existing environmental and planning legislation. Given the bill’s weaknesses, the article explores pragmatic alternatives to better advance the UNDRIP principles.

Aboriginal Protectorates operated in the late 1830s and 1840s in the Port Phillip District of New South Wales (later to become the colony of Victoria) in Australia and New Zealand. This article examines a small selection of the extensive archive of Port Phillip and New Zealand Protectorates to illustrate the ways that language and communication work within colonial projects to support and extend colonial authority. Examining language acquisition by Protectors, it places attitudes to and use of Indigenous languages within the context of colonialism in each site, arguing that Indigenous voices in New Zealand were co-opted, and in Port Phillip were marginalised, in the service of divergent approaches to dispossessing Indigenous peoples from their land. The article also explores glimpses of Māori or Aboriginal experiences of humanitarianism, colonisation, and dispossession captured in this archive.

We have been talking together about concepts of place for many years, and both jointly and separately we have come up against many horrific facts of damage, trauma, loss and irretrievable devastation. Debbie encountered this first and foremost in her work with Aboriginal Australians: What happens when sacred sites are destroyed? What are the effects of being dispossessed and having one’s own existence denied? What are the consequences of extinction when the extinct ones are kin—members of the one totemic family? Debbie’s engagements with ecological loss pressed her to consider extinctions and then to think of life itself in the context of desecration. Pete encountered these facts in his wide-ranging work with loss of place: What is the impact of destroyed homes and lost country? How do people engage with deliberate erasure of the sacred in recent war zones, with the vandalised cemeteries in Havana, or with a church in New South Wales where a black mass was conducted?

Abstract:
IntroductionThe age-adjusted rate of potentially preventable hospitalisations for Aboriginal and Torres Strait Islander people is almost five times the rate of other Australians. Quality use of medicines has an important role in alleviating these differences. This requires strengthening existing medication reviewing services through collaboration between community pharmacists and health workers, and ensuring services are culturally appropriate. This Indigenous Medication Review Service (IMeRSe) study aims to develop and evaluate the feasibility of a culturally appropriate medication management service delivered by community pharmacists in collaboration with Aboriginal health workers.Methods and analysisThis study will be conducted in nine Aboriginal health services (AHSs) and their associated community pharmacies in three Australian states over 12 months. Community pharmacists will be trained to improve their awareness and understanding of Indigenous health and cultural issues, to communicate the quality use of medicines effectively, and to strengthen interprofessional relationships with AHSs and their staff. Sixty consumers (with a chronic condition/pregnant/within 2 years post partum and at risk of medication-related problems (MRPs) per site will be recruited, with data collection at baseline and 6 months. The primary outcome is the difference in cumulative incidence of serious MRPs in the 6 months after IMeRSe introduction compared with the 6 months prior. Secondary outcomes include potentially preventable medication-related hospitalisations, medication adherence, total MRPs, psychological and social empowerment, beliefs about medication, treatment satisfaction and health expenditure.Ethics and disseminationThe protocol received approval from Griffith University (HREC/2018/251), Queensland Health Metro South (HREC/18/QPAH/109), Aboriginal Health and Medical Research Council of New South Wales (1381/18), Far North Queensland (HREC/18/QCH/86-1256) and the Central Australian HREC (CA-18-3090). Dissemination to Indigenous people and communities will be a priority. Comparison of Aboriginal interests in rangelands in western New South Wales with those in north- western Northern Temtory and the Kimberley of Western Australia reveals little difference in their history, aspirations for land acquisition, or plans for multiple use management. Throughout Australia, Aboriginal people who are traditional owners of rangelands continue to live on or near the land they regard as traditionally theirs. This is true of the more closely settled rangelands as well as remote regions. In all the rangeland areas Aboriginal people now wish (and following the Mabo decision may more realistically expect to gain) some form of freehold title to at least some of their land. Aboriginal people whose traditional lands are located in western New South Wales have access to very little of their land but have maintained their connection to it. They have aspirations of obtaining access to and control over portions of it, with plans to manage it under a multiple use regime that would include small-scale sustainable pastoralism and agriculture, while living in dispersed family groups on the land. Aboriginal people's desire to retain access to their traditional land for non-economic reasons (spiritual, social, historical) is paramount. Should the Commonwealth Land Fund legislation be enacted, cultural imperatives as well as economic viability will need to be taken into account in the purchase of land. Planning for future management should incorporate traditional ecological knowledge and should involve Aboriginal traditional owners and their organisations, such as land councils and resource agencies, in local and regional planning.

Many studies document the relationship between housing quality and health status. Poor housing in Aboriginal communities continues to be linked to the compromised health status of Aboriginal Australians. The New South Wales (NSW) Housing for Health (HfH) program has been assessing and repairing Aboriginal community housing across the state for 20 years using a standardised intervention methodology that aims to improve the health of Aboriginal people in NSW by improving their living environments. Items are tested and repairs are prioritised to maximise safety and health benefits and measured against 11 Critical Healthy Living Priorities (e.g., safety, facilities for washing people and clothes, removing waste and preparing food). Descriptive analysis of data collected pre- and post-intervention from 3670 houses was conducted to determine the effectiveness of the program. Analysis demonstrated statistically significant improvements in the ability of the houses to support safe and healthy living for all critical healthy living priorities post-interventions. Trend analysis demonstrated the magnitude of these improvements increased over 20 years. In 24 communities (n = 802 houses) where projects were repeated (5–17 years later), results indicate sustainability of improvements for 9 of 11 priorities. However, the overall condition of health-related hardware in Aboriginal community housing across NSW pre-intervention has not significantly changed during the program’s 20 years. Results suggest a systematic lack of routine maintenance and quality control continues to be the overwhelming cause for this lack of improvement pre-intervention. Our evaluation of the HfH program demonstrated that fidelity to a standardised housing testing and repair methodology to improve residents’ safety and health can have sustainable effects on housing infrastructure and associated health benefits, such as a 40% reduction in infectious disease hospital separations. Housing and health agencies should collaborate more closely on social housing programs and ensure programs are adequately resourced to address safety and health issues.

The ethnographic recordings of Robert Hamilton Mathews in the late nineteeth and early twentieth centuries have preserved data about Aboriginal language, beliefs, customs, technology and social and family networks. This data has steadily gained increasing respect over more recent decades, and its quality is often attributed to Mathews’ previous career as a property surveyor. Nevertheless, detailed research in the records of the Lands Department of New South Wales held by State Archives of New South Wales reveals a different story—of a man driven to maximise his income using illegal and unethical methods. Official inquiries into Mathews’ conduct by experienced surveyors showed that he often flaunted professional guidelines and practices to such an extent that the influence of Mathew’s surveying career on his ethnographical work should be reconsidered.

Sexual abuse and family violence are widespread and under-reported phenomena for which Aboriginal victims face even greater barriers to asking for and receiving assistance than do others in the community. There is a need for strategies to address abuse without disempowering and alienating Aboriginal people. A program developed by the New South Wales Health Education Centre Against Violence is addressing this issue at the same time as contributing towards a strengthened Aboriginal health workforce. The training program which is a 1-year qualification course has grown from a 52% rate of graduation in its first 6 years to 92%. Three practices in the classroom have contributed to this success. These are: (i) recognition of the emotional impact of the training and its links to participants own histories; (ii) providing space to address participants negative prior educational experiences; and (iii) further developing content on the recent sociopolitical history of Aboriginal people. These practices have strengthened this successful course, which is building a skilled workforce to provide accessible, culturally sensitive services for Aboriginal people experiencing abuse.

This study aimed to identify the factors associated with timely (within four weeks) HCV RNA testing and timely (within six months) DAA initiation following HCV notification in the DAA era. We conducted a cohort study of people with an HCV notification in NSW, Australia. Notifications of positive HCV serology were linked to administrative datasets. Weights were applied to account for spontaneous clearance. Logistic regression analyses were performed. Among 5582 people with an HCV notification during 2016–2017, 3867 (69%) were tested for HCV RNA, including 2770 (50%) who received timely testing. Among an estimated 3925 people with chronic HCV infection, 2372 (60%) initiated DAA therapy, including 1370 (35%) who received timely treatment. Factors associated with timely HCV RNA testing included age (≥30 years), female sex, non-Aboriginal ethnicity, country of birth being Australia, and no history of drug dependence. Factors associated with timely treatment were age (≥30 years), male sex, non-Aboriginal ethnicity, country of birth being Australia, no history of drug dependence, and HCV/HIV co-infection. In the DAA era, 50% of people with an HCV notification did not receive timely HCV RNA testing. Most people with an HCV infection received therapy; however, DAA initiation was delayed among many.

IntroductionAchieving parenthood is challenging in individuals receiving renal replacement therapy (RRT; dialysis or kidney transplantation) for end-stage kidney disease. Decision-making regarding parenthood in RRT recipients should be underpinned by robust data, yet there is limited data on parental factors that drive adverse health outcomes. Therefore, we aim to investigate the perinatal risks and outcomes in parents receiving RRT.Methods and analysisThis is a multijurisdictional probabilistic data linkage study of perinatal, hospital, birth, death and renal registers from 1991 to 2013 from New South Wales, Western Australia, South Australia and the Australian Capital Territory. This study includes all babies born ≥20 weeks’ gestation or 400 g birth weight captured through mandated data collection in the perinatal data sets. Through linkage with the Australian and New Zealand Dialysis and Transplant (ANZDATA) registry, babies exposed to RRT (and their parents) will be compared with babies who have not been exposed to RRT (and their parents) to determine obstetric and fetal outcomes, birth rates and fertility rates. One of the novel aspects of this study is the method that will be used to link fathers receiving RRT to the mothers and their babies within the perinatal data sets, using the birth register, enabling the identification of family units. The linked data set will be used to validate the parenthood events directly reported to ANZDATA.Ethics and disseminationEthics approval was obtained from Human Research Ethics Committees (HREC) and Aboriginal HREC in each jurisdiction. Findings of this study will be disseminated at scientific conferences and in peer-reviewed journals in tabular and aggregated forms. De-identified data will be presented and individual patients will not be identified. We will aim to present findings to relevant stakeholders (eg, patients, clinicians and policymakers) to maximise translational impact of research findings.
