Abstract:
Background: Both dust inhalation and smoking combined, led to more deleterious effect on the lungs. The health of gold miners is not only eroded by excessive exposure to dust in the mines, but also by the habit of smoking. Smokers have a significantly higher risk of getting lung cancer than non-smokers. Objective: The objective of this study is to establish the relationship between smoking and lung abnormalities among ex-mineworkers in the Transkei region of South Africa. Patients and Methods: During a two years period (May 1997 to May 1999) 2080 former mineworkers were examined at the Benefit Examination Clinic at Umtata General Hospital, a tertiary hospital attached to the Walter Sisulu University in Eastern Cape Province. Radiological examinations were carried out on (466) former mineworkers, in the age group ranging from 30 years to 70-plus years. Results: Mineworkers who had smoked exhibited two to three times the number of gross lung abnormalities on radiological examination than those who had had no experience of smoking in their life. The readings taken indicated an odd ratio (OR) of 2.0 with a p value of <0.05, and Chi-square 8.3, indicative of statistically significant association between smoking and lung abnormality in the ex-mineworkers (i.e. ex-smokers, smokers, and non smokers). Conclusion: There is a strong correlation between lung abnormalities and smoking among ex-mineworkers in this study.

Abstract:
Liver cirrhosis is associated with cardiovascular dysfunction including decreased heart rate variability and impaired acceleration of the heart rate in response to sympathetic activation (chronotropic incompetence). In this thesis, the hypothesis that increased formation of reactive nitrogen species in cirrhosis causes nitration or S-nitrosation of cardiac proteins and leads to impaired chronotropic function was assessed in an experimental model of cirrhosis. Cardiac chronotropic responsiveness to fi-adrenergic stimulation was assessed in vitro using spontaneous beating rat isolated atria. A novel mass spectrometric method was developed for dynamic assessment of nitration reactions based on the nitration of deuterium-labelled pra-hydroxyphenyl acetic acid. Nitration of cardiac proteins was measured by mass spectrometry and located by immunogold electron microscopy. Marked impairment of chronotropic responses of isolated atria to isoproterenol was observed in rats with cirrhosis, which normalized after the administration of N-acetylcysteine (a scavenger of reactive oxygen and nitrogen species) or L-NAME (a nitric oxide synthase inhibitor). The levels of protein-bound nitrotyrosine in atrial tissue increased from 16 1 to 23 3 pg/g tyrosine in rats with cirrhosis, and decreased to 15 1 and 17 1 pg/g after treatment with L-NAME and N-acetylcysteine, respectively (P<0.05). Immunogold electron microscopy demonstrated increased nitration of filaments and mitochondria in the atria of rats with cirrhosis. A chemiluminescence- based method was developed to stabilise and measure S-nitrosothiols in tissues. There was no difference in cardiac S-nitrosothiols following induction of cirrhosis, and neither N-acetylcysteine nor L-NAME had any effect on the cardiac levels of S-nitrosothiols. Autonomic regulation of cardiac function was assessed by analysis of heart rate variability in anesthetized rats using Fast Fourier Transformation. Heart rate variability analysis showed impaired sympathovagal balance towards increase of cardiac sympathetic activity in rats with cirrhosis (P<0.05). However there was no change in the sympathovagal balance following N-acetylcysteine or L-NAME adminstration in cirrhotic rats. In conclusion, abnormal cardiac chronotropic function in cirrhosis is associated with increased nitration of cardiac proteins. Two independent treatments (N-acetylcysteine and L-NAME) that decrease nitration of cardiac proteins led to normalization of cardiac responses. Nitration of critical proteins in cardiac tissue may lead to abnormal cardiac function.

Abstract:
The accelerated rate of development of modern technology has greatly expanded the range of health hazards. Lead, a widely used industrial material, is a significant environmental pollutant that contaminates food, water, soil and air. Although much progress has been made in elucidating its adverse effects on various systems of the body like hepatic, CNS, renal etc., its effect on coagulation remains to be established. In view of this an experimental study was carried out in animals to understand how lead influences hemostasis.Male albino rats were exposed to lead either acutely by administering 20 mg lead acetate per kg body weight daily i.p. for 3 days or chronically by administering lead through drinking water containing 5 ppm lead for 150 days. Acute exposure to lead caused severe coagulopathy characterized by significant prolongation of plasma recalcification time, decrease in platelet count and decreased wall adherence of blood, decreased fibrinogen and euglobulin lysis time and significant increase in prothrombin time, thrombin time, and partial thromboplastin time. Similar observations were found in chronically exposed animals. It is concluded that exposure to heavy metals like lead may lead to a state of hypocoagulability.
