Abstract:
Introduction: L’objectif de cette étude vise à décrire la connaissance, l’attitude et la pratique des médecins de de Toamasina I sur la prévention du cancer du col utérin. Méthodes: Il s’agissait d’une enquête transversale descriptive du 1er Novembre 2020 au 31 Janvier 2021. La connaissance ou la pratique était supposée suffisante si 75% ou plus des médecins trouvent la bonne réponse à la question posée. Cette étude a obtenu l’autorisation des chefs hiérarchiques ainsi que le consentement des participants. Résultats : L’âge médian des 74 participants se situait à 40 ans. Ils étaient médecins généralistes, pratiquant à moins de 10ans, exerçant dans un centre public respectivement : 71,60% ; 45,90% ; 63,50%. Ils savaient dans 98,6% que l’HPV favorise le développement de cancer du col utérin. Les enquêtés ont réponses justes aux moyens de dépistage dans 78,10%. Ils estimaient dans 93,20% que le cancer du col peut être guéri s’il est découvert précocement. La proposition d’un dépistage aux femmes cibles, la prescription de frottis cervico-utérin et la pratique de l’IVA étaient évaluées respectivement : 93,20% ; 48,70% ; 25,70%. Discussion: Ces médecins avaient de bonnes connaissances mais ils manquaient de pratique probablement à cause de plateau technique insuffisante. Ces résultats ne reflètent pas la réalité des médecins malgaches. Conclusion: Les médecins possédaient des connaissances relativement suffisantes mais la mise en pratique était encore insuffisante. La formation post universitaire et la mise en place d’un plateau technique adéquat sont nécessaires. The proposal of screening to targetwomen, the prescription of cervicalsmear and the practice of VIA wereevaluatedrespectively: 93.20%; 48.70%; 25.70%. Discussion : Thesedoctorshad good knowledge but lacked practice, probably due to insufficienttechnicalequipment. Theseresults do not reflect the reality of Malagasy doctors. Conclusion: The doctorshadrelativelysufficientknowledge but the practical application wasstillinsufficient. Postgraduate training and the establishment of an adequatetechnicalplatform are necessary.

Abstract:
Hybrid scaffolds based on bioactive glass (BAG) particles (<38 µm), covalently linked to gelatin (G*) using 3-glycidoxypropyltrimethoxysilane (GPTMS), have been studied for bone bioengineering. In this study, two glass compositions (13-93 and 13-93B20 (where 20% of the SiO2 was replaced with B2O3)) were introduced in the gelatin matrix. The Cfactor (gelatin/GPTMS molar ratio) was kept constant at 500. The hybrids obtained were found to be stable at 37 °C in solution, the condition in which pure gelatin is liquid. All hybrids were characterized by in vitro dissolution in Tris(hydroxymethyl)aminomethane (TRIS) solution (for up to 4 weeks) and Simulated Body Fluid (SBF) (for up to 2 weeks). Samples processed with 13-93B20 exhibited faster initial dissolution and significantly faster precipitation of a hydroxyapatite (HA) layer. The faster ion release and HA precipitation recorded from the G*/13-93B20 samples are attributable to the higher reactivity of borosilicate compared to silicate glass. The MC3T3-E1 cell behavior in direct contact with the hybrids was investigated, showing that the cells were able to proliferate and spread on the developed biomaterials. Tailoring the glass composition allows us to better control the material’s dissolution, biodegradability, and bioactivity. Bioactive (especially with 13-93B20 BAG) and biocompatible, the hybrids are promising for bone application.

Abstract The class II major histocompatibility complex (MHCII) pathway in antigen-presenting cells (APCs) is the conduit by which extracellular proteins are processed by proteases, resulting in foreign peptides being presented to T cells. After depolymerization to a markedly decreased molecular size, zwitterionic polysaccharides (ZPSs) are also presented by this pathway. However, nothing is known about the chemistry of carbohydrate processing apart from its dependence on the presence of inducible nitric oxide synthase (iNOS). By examining the ability of known nitric oxide-derived species, peroxynitrite, nitroxyl, and nitric oxide itself, to degrade polysaccharide A (PSA), a ZPS from Bacteroides fragilis, we determined that deamination was the predominant mechanism of PSA processing in APCs. Structural characterization of the nitric oxide-derived product PSA-NO indicated that the depolymerization didn¡t disturb PSA¡s repeat units with exception of the deamination cleavage of 4-amino groups of 3-linked 2-acetamido-4-amino-2,4,6-trideoxy galactopyranosyl (AATp) residues. PSA-NO presented by APCs induced CD4+ T cell proliferation that was blocked by inhibitors of endosomal processing and by an antibody to HLA-DR. A novel mechanism (deaminative reaction) differentiates carbohydrate from protein processing during the oxidative burst.

Abstract To identify and characterize the genes involved in the functional regulation of hypertension and arteriosclerosis-specific genes, changes in gene expression profiles were examined by DNA microarray from the spontaneously hypertensive rats (SHR) and normal the Wistar-Kyoto (WKY) rats after administration with anti-hypertension drugs, including angiotensin I-converting enzyme (ACE) inhibitor, enalapril and the calcium channel blocker, nifedifine. Among the genes that were distinct and differentially expressed in brain, heart and liver of SHR rats administrated with the drugs, 113 were specifically up-regulated, and 102 were down-regulated genes compared to those of WKY or SHR rats. The differential gene expression profiles in DNA microarray were corroborated by semi-quantitative RT-PCR. Of these genes, expression of TNFSF4, 6, 10, IL-6 receptor, IL-18 receptor accessory protein and CD24 genes was extensively exhibited in a hypertension-specific manner, and patterns for hypertension-associated gene expression was regulated by changes of their expression in angiotensin ¥± or H2O2-treated vascular smooth muscle cells. These results will provide a fundamental basis to delineate the novel mechanisms, and a thorough investigation of the relationship between their expression and function will provide a better understanding in the development of new therapeutic drugs against cardiovascular diseases. Abstract:
Computer-Aided Language Learning (CALL) is growing nowadays because learning new languages is essential for communication with people of different linguistic backgrounds. Mispronunciation detection is an integral part of CALL, which is used for automatic pointing of errors for the non-native speaker. In this paper, we investigated the mispronunciation detection of Arabic words using deep Convolution Neural Network (CNN). For automated pronunciation error detection, we proposed CNN features-based model and extracted features from different layers of Alex Net (layers 6, 7, and 8) to train three machine learning classifiers; K-nearest neighbor (KNN), Support Vector Machine (SVM) and Random Forest (RF). We also used a transfer learning-based model in which feature extraction and classification are performed automatically. To evaluate the performance of the proposed method, a comprehensive evaluation is provided on these methods with a traditional machine learning-based method using Mel Frequency Cepstral Coefficients (MFCC) features. We used the same three classifiers KNN, SVM, and RF in the baseline method for mispronunciation detection. Experimental results show that with handcrafted features, transfer learning-based method and classification based on deep features extracted from Alex Net achieved an average accuracy of 73.67, 85 and 93.20 on Arabic words, respectively. Moreover, these results reveal that the proposed method with feature selection achieved the best average accuracy of 93.20% than all other methods.

Introduction: Migration is the result of interplay of many factors, which guide the individual’s decision to emigrate. Health workers tend to go where the working conditions are best. This study aims to identify the determinants and aspirants of nurse migration from Nepal. Methods: A descriptive cross sectional study was conducted using interview technique. A semi structured questionnaire was administered to all nurses working at a private hospital who met the inclusion criteria. Results: Majority of the participants were 20- 30 years old (93.20%), Hindu (86.5%), Chhetri (35.1%), unmarried (66.2%), had completed intermediate level (58.1%). Among the total respondents, 93.20% intended to migrate to abroad and more than half of the respondent (62.20%) preferred Australia as destination. The general factor provoking migration was bad nature of politician (98.6%) followed by political instability (90.5%) and non-availability of job (90.5%). The major working condition and social factors provoking migration were occupational security in other country (85.10%) and peer influence (91.9%) respectively. The reasons for migration as mentioned by respondents were education (59.50%) followed by better job opportunities (40.50%), better living standard (21.60%) and family and/or peer pressure (8.10%). Conclusions: The present study concluded that majority wanted to migrate and Australia was famous choices for nurses. Bad nature of politician, occupational security in other country and peer influence were factors for migration. The major purpose for migration was education.

Melalui tahapan pengembangan model ADDIE peneliti merancang suatu media pembelajaran menggunakan software Articulate Storyline pada materi Statistika dan Peluang. Kelayakan media dilihat oleh dua aspek yaitu aspek kevalidan dan kepraktisan. Hasil analisis menunjukkan validasi ahli media dan ahli materi memperoleh rata-rata persentase berturut-turut 86,25% dan 92,27% yang dikategorikan “sangat valid”. Selanjutnya dari hasil uji coba diperoleh respon positif dari guru dan siswa dengan rata-rata persentase sebesar 79,35% dan 93,20% yang dikategorikan “praktis dan sangat praktis”. Rata-rata persentase untuk kedua aspek ini sebesar 87,77% yakni kategori sangat efektif, sehingga media pembelajaran matematika ini layak untuk diimplementasikan di kelas.

Abstract Nucleic-acid sensing molecules provide a first line of defense against microbial infections, but recent studies have shown that their inappropriate activation could promote autoimmunity. MDA-5 senses virally-derived RNA in the cytoplasm, which leads to type one interferon (IFN) production, recently shown to be associated with systemic lupus erythematosus. Here, we have generated mice with multiple copies of Ifih1, the gene that encodes MDA-5, and crossed them to the lupus-prone FcgammaRIIB-/- mouse. Mice with enhanced MDA-5 expression and FcgammaRIIB deficiency have accelerated autoimmunity with splenomegaly and anti-nuclear antibodies in their serum compared to FcgammaRIIB-deficient mice alone. They also display elevated levels of splenic plasma and germinal center B cells, indicating possible early autoreactive B cell formation. Bone marrow and splenic macrophage/monocytes from MDA5 transgenic animals also display an activated phenotype, characterized by elevated Ly6A/E, PDCA-1 and CD80 expression. B cells from MDA-5-transgenic animals also express elevated levels of type one IFN signature genes. These results suggest that increased expression of MDA-5 could exacerbate lupus, possibly through the early production of type one IFN.

This paper studies inhibition of steel-rebar corrosion in concrete immersed in 0.5 M H2SO4, simulating industrial/microbial environment by the leaf extract ofMorinda lucida. Electrochemical monitoring methods were employed for testing different concentrations of the leaf extract admixed in duplicated specimens of steel-reinforced concrete slabs immersed in the acidic test-system. Statistical analyses as per ASTM G16-95 R04 of the experimental results showed that effectiveness ofMorinda lucidaat inhibiting concrete steel-rebar corrosion increased with the concentration of the admixture. 0.4167%Morinda lucida, per weight of cement, was identified with optimum inhibition efficiencyη= 98.78±0.34% followed in effectiveness by 0.3333%Morinda lucidawithη= 93.20±1.76% at inhibiting steel-rebar corrosion in the corrosive test-environment.

Artikkelissa kuvataan korkeakoulujen arviointineuvoston vuonna 1997 käynnistämää pilottihanketta, jonka tarkoituksena on ammattikorkeakoulujen laatujärjestelmien arvioiminen ja kehittäminen. Hankkeessa annettiin palautetta laatutyön toteutuksesta kullekin mukana olleelle oppilaitokselle ja jaettiin tietoa hyvistä käytännöistä oppilaitosten välillä. Ulkopuolinen arviointiryhmä havaitsi yhteiseksi piirteeksi ammattikorkeakouluissa sen, että opettajat ja opiskelijat arvostavat oppilaitostaan ja ovat innostuneita ammattikorkeakoulun tarjoamista uusista mahdollisuuksista. Laatutyö koko ammattikorkeakoulun organisaation kattavana toimintana hakee vielä muotoaan. Siksi on luonnollista, että laatutyöhön itseensä kohdistuva arviointi on alkutaipaleellaan. Laatuideologian soveltaminen koulutusorganisaatioissa ei ole kuitenkaan saavuttanut kaikkien hyväksyntää. Monien mielestä koulun kehittämisessä pitäisi lähteä liikkeelle syvällisemmän ymmärtämisen avulla.

Although echinococcal disease of the liver is common in some parts of the world, it is rarely seen in the United States. Even from endemic areas, few reports deal with its treatment during pregnancy. A 25 year old 18 week pregnant Italian woman presented with pruritis and right upper quadrant pain. Ultrasound revealed a cystic lesion in the left lobe of the liver. The cyst was treated by operative insertion of hypertonic saline and capsulorrhaphy. The patient and her fetus had no postoperative complications. We believe that the pregnant patient with symptomatic hepatic echinococcal disease should be treated operatively and that insertion of hypertonic saline and capsulorrhaphy is the safest and most effective technique for both the mother and fetus.

En las últimas décadas se ha incrementado la investigación en materia de evaluación de servicios ecosistémicos (SE), aunque en general se ha centrado en el análisis biofísico y económico sin prestar atención a la componente social. El propósito de este estudio es explorar cómo se valoran los SE desde un enfoque sociocultural, incluyendo la coparticipación de diferentes grupos de interés (stakeholders). Para ello se ha realizado un análisis sistemático de la literatura científica indexada en las bases de datos Scopus y Web of Science. A partir de la revisión bibliográfica, se han podido establecer las características y servicios que predominan en las evaluaciones socioculturales, y, por otro lado, se ha detectado una carencia de estudios en el marco de los parques naturales/protegidos y los bosques mediterráneos. Es por ello, que los hallazgos obtenidos podrían servir como referencia en el planteamiento de estudios de evolución, planificación y futuras actuaciones políticas sobre sostenibilidad y concienciación medioambiental.

This study is based on the scholars' critique of the Indonesian media reporting on disaster for focusing on vulnerable groups' suffering, sadness, and grief. The absence of comprehensive journalistic ethical guidelines for disaster published photographs in Indonesia, might contribute to the recurring choice and publication of such images in Indonesian print newspapers. Examining major disasters, including the 2004 Aceh Tsunami, the 2006 Yogyakarta and Central Java earthquake, the 2010 Mt. Merapi Eruption, and the 2015 Sumatra Forest fires, this research provides critical evidence for re-evaluating the Indonesian journalist’s codes of ethics, especially concerning the published photographs of disaster victims. Methodologically, this study draws on interviews with six Indonesian journalists and two media practitioners, exploring the ethical practices and dilemmas in photojournalism with a specific emphasis on disaster reporting in Indonesian print newspapers. The transcripts were analysed thematically, emphasising the repeating ideas or patterns present in the text. It also includes an analysis of three graphic/disturbing photographs from the 2010 Mt Merapi eruption coverage in Kompas and Kedaulatan Rakyat newspapers. The study highlights the complex balance between ensuring truthful reporting and honouring the dignity and privacy of those affected by disasters. It advocates for a nuanced method in disaster coverage that harmonizes journalistic integrity with a compassionate portrayal of victims. This research contributes to the discourse on ethical disaster photojournalism and has significant implications for policy development, journalist training programs, and the revision of journalistic codes of ethics, promoting a more empathetic and responsible approach in the field.

Shopee is e-commerce widely accessed and used in this era. Many people use Shopee because the products offered are cheaper and more affordable. Despite the fact that Shopee is a well-known e-commerce, it still requires responses and suggestions from the public to maintain or improve the features required. In this study, public sentiment analysis was carried out on Twitter social media related to the Shopee marketplace. This study collected data that contained tweets from predetermined keywords and used Word2Vec and Support Vector Machine classification methods. The use of Word2Vec influenced the level of accuracy so that it increased for each SVM kernel. Meanwhile, the best hyperparameter tuning was found in the polynomial kernel, with an accuracy rate of 93.20%.

This research studied the chemical compositions, antioxidant activities, and antimicrobial activities of kaffir lime (Citrus hystrix DC.) and key lime (Citrus aurantifolia (Christm.) Swingle.) peel essential oils extracted by hydrodistillation. GC-MS analysis of kaffir lime essential oil showed four major components as β-pinene (27.37%), β-phellandrene (22.69%), D-limonene (16.21%), and citronellal (12.43%), while in key lime essential oil the predominant component was D-limonene (45.91%), with two minor components as β-pinene (20.27%) and γ-terpinene (8.33%). Both essential oils scavenged free radicals, with amounts of sample required to decrease DPPH concentration by 50% (IC50) 93.20 and 57.90 mg/ml for kaffir lime and key lime essential oils, respectively. Both essential oils showed antibacterial activity against Staphylococcus aureus in the agar diffusion technique.

Penggunaan Antibiotik untuk anak-anak harus diawasi dan dikendalikan karena penggunaan antibiotik yang tidak terkontrol dan tidak tepat dapat berdampak pada kegagalan efek terapi yang diharapkan dan kemungkinan dapat meyebabkan resistensi antimikroba. Pemakaian antibiotik harus dianalisis untuk mengendalikan resistensi antibiotik. Tujuan penelitian ini untuk menilai rasionalitas terapi antibiotik pada pasien anak. Penelitian ini merupakan analisis deskriptif retrospektif dengan pengambilan data purposive sampling. Subjek dari penelitian adalah rekam medis dari bangsal anak RSUD Karawang. Rasionalitas penggunaan antibiotik adalah penilaian melalui metode gyssens. 147 data dianalisis untuk penelitian ini terdiri dari anak laki-laki (54,40%) dan perempuan (45,60%) dengan kasus demam tifoid (53,10), diare (24,5%) dan bronkopneumonia (22,4%). Antibiotik yang digunakan sefotaksim (71,43%), amoksisilin (5,44%) dan kloramfenikol (4,76%). Penilaian antibiotik melalui metode Gyssens adalah skala IVA (93,20%), skala IIB (6,12%) dan skala IIIA (0,68%). Kata Kunci: Antibiotik, Rasionalitas, Metode Gyssens. ABSTRACT Antibiotics prescription for pediatric patients is most widely used in therapy. Antibiotic use for pediatric must be watched and controlled because uncontrolled and inappropriate use of antibiotic can impact on failure of expected therapeutic effect and can also cause the possibility of antimicrobial resistance. Antibiotic consumption is must analyzed for controlling the antibiotics resistance. The aim of study The aim of this study was to assess the rationality of antibiotic therapy in pediatric patients. This research is a retrospective descriptive analysis with purposive data sampling. Subject of study are medical record from pediatric ward in RSUD Karawang. Rationality used of antibiotic is asses through Gyssens method. 147 data was analyzed for this study consist of boys (54,40%) and girls (45,60%) with case of typhoid fever (53,10), diarrhea (24,5%) and bronchopneumonia (22,4%). Antibiotic was use Cefotaxime (71,43%), Amoxicillin (5,44%), Chloramphenicol (4,76%)etc. Assessment antibiotic through Gyssens method are IVA scale (93,20%), IIB scale ( 6,12%) and IIIA scale (0,68%). High power LED is a kind of ideal green lighting source, which owns longer life, higher efficiency and lower electricity power consumption than incandescent lamps and fluorescent bulbs. Constant current driver is the most key factor for high power LED’s premium properties. Based on the specific chip LM3478, a novel Boost DC/DC converting circuit to drive LED was proposed. The whole circuit structure was simple, and owned high reliability with over current protection. The circuit operates continuous current mode (CCM), with normal supply voltage 12V. The constant output current is 700mA, which can drive two-row LED series, 5 LEDs at least each series. The test results show that the electricity efficiency is up to 93.20% and that the output current deviation is 7.71%. The operating temperature range is -40~+125°C.

In this paper we propose an input-to-state stability (ISS) criterion for continuous–time systems based on a finite–time decrease condition for a positive definite function of the norm of the state. This yields a so–called ISS finite–time Lyapunov function, which allows for easier choice of candidate functions compared to standard ISS Lyapunov functions. An alternative converse ISS theorem in terms of ISS finite– time Lyapunov functions is also provided. Moreover, we prove that ISS finite–time Lyapunov functions are equivalent with standard ISS Lyapunov functions using a Massera–type construction. The developed ISS framework can be utilized in combination with Sontag’s “universal” stabilisation formula to develop input–to–state stabilizing control laws for continuous–time nonlinear systems that are affine in the control and disturbance inputs, respectively. The purpose of this study was to investigate the relationships between Internet usage, social isolation, and Internet addiction among university students. The study used a quantitative method and questionnaires to measure the ‘problems of using the Internet’, ‘patterns of Internet usage’, ‘social isolation’, and ‘Internet addiction’. The study included 440 students from the University Putra Malaysia, chosen using a stratified random sampling method. The majority of the respondents were single (93.20%) and aged between 23 to 40 years old. The study found a significant relationship between ‘problems of using the Internet, social isolation, and Internet addiction’. However, there was no statistically significant relationship between the patterns of using the Internet and social isolation. The t-test result showed that no significant mean differences were found among marital status, field of study, social isolation, and Internet addiction. Implication of the study discussed.

Any relations and practices of debt within credit networks include not only pure economic exchange, such as barter or non-monetary exchange, but diverse kinds of social relations of debt. In this article, we consider case studies embedded in the particular context of early and late post-Soviet economic and political crisis, in which economic informalization was characterised by ‘wild capitalism’ or the absence of cash and livelihoods. We observe the coping strategies and mechanisms of economic survival adopted by ordinary people in response to these crises and to the absence of state social and economic institutions. We portray people’s concrete experiences of debt, solidary social relations, and economic exchange based on debt relationships in post-Soviet economies. The case studies are drawn from ethnographic material from Uzbekistan, Russia and Kazakhstan. Our case studies reveal that debt relations are not about two individuals who owe money but serve as the basis for debt-based trade, survival, and socializing networks, as well as part of moral economies. The article builds on scholarly works related to informal economies and survival mechanisms in post-Soviet space, to anthropology of debt, as well as discussion of (dis)trust.

Potable, or “sweet,” water was the foundation stone of maritime provisioning and, by implication, route planning on all but the shortest voyages in the premodern world. Without it, maritime trade and all other forms of seaborne exchange and circulation were effectively impossible. Yet water sources and technologies of transportation have been comparatively neglected in Indian Ocean history and archaeology. This paper rereads data from the ninth-century section of the Akhbār al-Ṣīn wa-l-Hind (Accounts of China and India) alongside recent evidence from two contemporary shipwrecks to examine the spacing of watering stops and the technologies of water transportation employed on long-distance sailings between the Gulf and Chinese ports. Working from the 2021 publication of the volumetric capacity of a group of so-called torpedo jars excavated in Thailand, this article proposes some preliminary quantitative estimates of the volume of freshwater, and thus the number of water jars, required on board vessels at the time. In so doing it raises important questions about the portability and handling of torpedo jars as well as the varied uses and reuses of such transport jars. Weaving passages from the Akhbār with information on ceramic remains from the Phanom Surin and Belitung wrecks, this article aims to start a conversation about the very real physical and physiological parameters that underlay Indian Ocean connectivity and the water transportation technologies that underpinned them.
