Abstract The de novo pyrimidine biosynthetic pathway is an inducible cellular program which allows the rapid synthesis of pyrimidine nucleotides in proliferating cells, and the final rate-limiting step is catalyzed by cytidine triphosphate synthase (CTPS1 or CTPS2). A CTPS1 loss-of-function mutation in humans has illustrated the essential and non-redundant role of CTPS1 in activated lymphocyte proliferation. Therefore, selective CTPS1 inhibition is an attractive approach to specifically target pathological immune activity in autoimmune and inflammatory disease. Using cryo-EM and structure-based drug design, we have identified a series of potent, orally bioavailable, CTPS1-specific inhibitors, exemplified by the compound NTX-871. In-vitro, NTX-871 significantly inhibits the proliferation of T-cell derived lines and activated primary T-cells. These inhibitory effects are reversed by the addition of exogenous cytidine, illustrating the specificity of NTX-871 for CTPS1 and the de novo pyrimidine synthesis program. NTX-871 was also tested in several mouse in-vivo PD and autoimmune disease models. Oral dosing of NTX-871 significantly reduced T-cell proliferation in an ovalbumin-induced T-cell proliferation model, and QD or BID dosing of NTX-871 significantly improved disease phenotypes in models of delayed-type hypersensitivity, collagen-induced arthritis, and experimental autoimmune encephalomyelitis.

The word 'competition' in economic terminology means the independence of business actions opted by different sellers of the same product. However, in context of the telecom industry, the same word has opposite connotation-interdependence among competing service providers. The reason is that "the telecom system must work as a single system [because] users desire end to end services within an apparently 'seamless' communication network. They want connectedness and connectability" [Melody (1997), p. 53]. Therefore, to attract users, a new entrant in the industry, while intending to compete with the incumbent monopoly operator, has to ensure interconnection arrangements with the latter.

This study seeks to determine the effectiveness of implementing OMR-Tele/RBI in the remote areas of the Municipality of Malungon. The research employed a qualitative case study method that utilized key informant interviews with the Junior High School learners and Focus Group Discussions (FGD) with home learning partners in Malungon, Sarangani Province. According to the findings, OMR-Tele/RBI benefits some learners, particularly junior high school learners, because they can study anywhere, anytime, and at their discretion. However, due to their short concentration span and inability to study independently, primary learners are unsuitable for OMR-Tele/RBI. Problems with OMR-Tele/RBI have also hindered remote learning. Most learners living in mountain areas have slow or even no access to radio or the internet. Moreover, the majority of learners lived in small houses with no subdivisions. Therefore, students were unable to find a peaceful place to study. As a consequence, students cannot concentrate while listening to or viewing the lessons. In addition, it was recommended that the teacher models and facilitators be equipped to construct age- and level-appropriate audio or video lessons. Additionally, the audio and video lessons should be quality-assured to ensure the program’s effectiveness.

Abstract Introduction/Objective Comparisons of diagnostic testing across multiple health systems over multiple years are uncommon. Such comparisons would quantify variations in test use between health systems. Methods/Case Report Using the Vizient Clinical Database, we compared clinical laboratory and imaging utilization for hospitalized adult sepsis patients (N = 69,035) over three years (2017-2019) at 19 large academic-affiliated health systems across the United States. We used Medicare Severity Diagnosis Related Groups (MSDRG), employed by the US Centers for Medicare and Medicaid Services (CMS), and identified sepsis patients (MSDRG triplet 870/871/872). We stratified hospitalized sepsis patients by severity of illness (SOI) into high severity (MSDRG 870), moderate severity (MSDRG 871), and low severity (MSDRG 872) groups. SOI further categorizes patients within a diagnostic group, quantifying the extent of comorbid conditions and complications. We measured hospital length of stay (LOS), number of laboratory tests (CPT codes 80000-89999), and number of imaging studies (CPT codes 70000-79999). We divided the number of laboratory tests and radiology studies by mean hospital LOS (in days) to calculate laboratory tests and imaging studies per hospital day. Results (if a Case Study enter NA) Between health systems, lowest and highest values for laboratory and imaging utilization ranged from 50.1 to 141.3 tests per hospitalization and 1.4 to 7.2 studies per hospitalization, respectively. Differences in laboratory tests between health systems persisted after adjusting for SOI with low to high laboratory tests per hospitalization ranging from 127.8 to 405.7, 51.0 to 144.7, and 31.9 to 78.2, for high, moderate, and low SOI groups. Utilization ranges were smaller for imaging studies. After adjusting for hospital LOS, laboratory testing differences between health systems were more pronounced and with low and high laboratory test per day utilization of 6.5 to 24.3, 6.1 to 18.5, and 6.0 to 17.1, for high, moderate, and low SOI groups. Differences in radiology studies were not as pronounced after adjusting for LOS. Conclusion There is considerable variation among health systems in laboratory and radiology resource utilization for hospitalized sepsis patients. This variation persists, especially for laboratory testing, after adjusting for SOI and LOS.

In their pioneering study of the Trent codices, Adler and Koller divided the six manuscripts then known to them into two distinct and separate groups: a younger group, Tr88–91, which they believed to have been compiled ‘in or for Trent’ by the scribe Johannes Wiser; and an older group, Tr87 and Tr92, which they described less specifically as ‘unquestionably north Italian’ in origin. Recent findings have tended to confirm Trent as the place of origin of the former group, while at the same time indicating that the blanket description ‘north Italian’ can no longer suffice for the latter. This group comprises in effect three sources which once existed independently of each other: a principal source divided into two parts, Tr871 and Tr922, here referred to jointly as TR; and two smaller sources, Tr872 and Tr921. The few attempts that have been made to determine the individual provenances of these manuscripts rest more on informed hypothesis than hard fact, with the result that their precise origin and the reason for their association with the later codices have remained as much of a puzzle as they were when the first six manuscripts were rediscovered just over a century ago.

Abstract:
The surveillance and virological characterization of H5N8 avian influenza viruses are important in order to assess their zoonotic potential. The genetic analyses of the Egyptian H5N8 viruses isolated through active surveillance in wild birds and domestic poultry in the winter of 2016/2017 showed multiple introductions of reassortant viruses. In this study, we investigated and compared the growth kinetics, infectivity, and pathogenicity of the three reassortant forms of H5N8 viruses detected in wild birds and domestic poultry in Egypt during the first introduction wave in the winter of 2016/2017. Three representative H5N8 viruses (abbreviated as 813, 871, and 13666) were selected. The 871/H5N8 virus showed enhanced growth properties in vitro in Madin Darby canine kidney (MDCK) and A549 cells. Interestingly, all viruses replicated well in mice without prior adaptation. Infected C57BL/6 mice showed 20% mortality for 813/H5N8 and 60% mortality for 871/H5N8 and 13666/H5N8, which could be attributed to the genetic differences among the viruses. Studies on the pathogenicity in experimentally infected ducks revealed a range of pathogenic effects, with mortality rate ranging from 0% for 813/H5N8 and 13666/H5N8 to 28% for 871/H5N8. No significant differences were observed among the three compared viruses in infected chickens. Overall, different H5N8 viruses had variable biological characteristics, indicating a continuous need for surveillance and virus characterization efforts.
