Abstract:
The novel coronavirus (SARS-CoV-2) is posing a serious threat to the mankind with its massive infection rate and potentially fatality. A total of 212 countries have been infected within the 112 days of first report causing 2 314 621 confirmed cases and 157 847 deaths worldwide. India, the country which is already battling with poverty, malnutrition and high population density is also at the second stage of coronavirus transmission. The situation is worsening and the attention has focused on the prevalence and preventive measures to be taken to protect 1.35 billion people of the largest democratic country of the world. In this review, a study has been designed to evaluate the prevalence, transmission, clinical symptoms, and preventive measures to control the community transmission of this fatal disease. The initial impact of coronavirus disease (COVID-19) outbreak on Indian economy has also been dealt with. This study reviews and summarizes the main points of the epidemic in India until the end of April 2020.

Abstract:
PURPOSE NALA (ClinicalTrials.gov identifier: NCT01808573 ) is a randomized, active-controlled, phase III trial comparing neratinib, an irreversible pan-HER tyrosine kinase inhibitor (TKI), plus capecitabine (N+C) against lapatinib, a reversible dual TKI, plus capecitabine (L+C) in patients with centrally confirmed HER2-positive, metastatic breast cancer (MBC) with ≥ 2 previous HER2-directed MBC regimens. METHODS Patients, including those with stable, asymptomatic CNS disease, were randomly assigned 1:1 to neratinib (240 mg once every day) plus capecitabine (750 mg/m2 twice a day 14 d/21 d) with loperamide prophylaxis, or to lapatinib (1,250 mg once every day) plus capecitabine (1,000 mg/m2 twice a day 14 d/21 d). Coprimary end points were centrally confirmed progression-free survival (PFS) and overall survival (OS). NALA was considered positive if either primary end point was met (α split between end points). Secondary end points were time to CNS disease intervention, investigator-assessed PFS, objective response rate (ORR), duration of response (DoR), clinical benefit rate, safety, and health-related quality of life (HRQoL). RESULTS A total of 621 patients from 28 countries were randomly assigned (N+C, n = 307; L+C, n = 314). Centrally reviewed PFS was improved with N+C (hazard ratio [HR], 0.76; 95% CI, 0.63 to 0.93; stratified log-rank P = .0059). The OS HR was 0.88 (95% CI, 0.72 to 1.07; P = .2098). Fewer interventions for CNS disease occurred with N+C versus L+C (cumulative incidence, 22.8% v 29.2%; P = .043). ORRs were N+C 32.8% (95% CI, 27.1 to 38.9) and L+C 26.7% (95% CI, 21.5 to 32.4; P = .1201); median DoR was 8.5 versus 5.6 months, respectively (HR, 0.50; 95% CI, 0.33 to 0.74; P = .0004). The most common all-grade adverse events were diarrhea (N+C 83% v L+C 66%) and nausea (53% v 42%). Discontinuation rates and HRQoL were similar between groups. CONCLUSION N+C significantly improved PFS and time to intervention for CNS disease versus L+C. No new N+C safety signals were observed.

1002 Background: NALA (ClinicalTrials.gov NCT01808573) is a multinational, randomized, open-label, phase III trial of neratinib (an irreversible pan-HER tyrosine kinase inhibitor [TKI]) + capecitabine (N+C) vs lapatinib (a reversible dual TKI) + capecitabine (L+C) in patients with stage IV HER2+ metastatic breast cancer (MBC) who had received ≥2 prior HER2-directed regimens for MBC. Methods: Patients were randomized 1:1 to N (240 mg qd po) + C (750 mg/m2 bid po) or L (1250 mg qd po) + C (1000 mg/m2 bid po). Co-primary endpoints were centrally assessed progression-free survival (PFS) and overall survival (OS). Secondary endpoints were investigator-assessed PFS; objective response rate (ORR); duration of response (DoR); clinical benefit rate (CBR); time to intervention for symptomatic metastatic central nervous system (CNS) disease; safety; and patient-reported health outcomes. Results: 621 patients were randomized (307 to N+C; 314 to L+C). The risk of disease progression or death was reduced by 24% with N+C vs L+C (HR = 0.76; 95% CI 0.63–0.93; p = 0.006); 6- and 12-month PFS rates were 47.2% vs 37.8% and 28.8% vs 14.8% for N+C vs L+C, respectively. OS rates at 6 and 12 months were 90.2% vs 87.5% and 72.5% vs 66.7% for N+C vs L+C, respectively (HR = 0.88; 95% CI 0.72–1.07; p = 0.2086). ORR in patients with measurable disease at screening was improved with N+C vs L+C (32.8% vs 26.7%; p = 0.1201), as was CBR (44.5% vs 35.6%; p = 0.0328) and DoR (HR = 0.50; 95% CI 0.33–0.74; p = 0.0004). Time to intervention for symptomatic CNS disease (overall cumulative incidence 22.8% vs 29.2%; p = 0.043) was delayed with N+C vs L+C. Treatment-emergent adverse events (TEAEs) were similar between arms, but there was a higher rate of grade 3 diarrhea with N+C vs L+C (24.4% vs 12.5%). TEAEs leading to neratinib/lapatinib discontinuation were lower with neratinib (10.9%) than with lapatinib (14.5%). Conclusions: N+C significantly improved PFS with a trend towards improved OS vs L+C. N+C also resulted in a delayed time to intervention for symptomatic CNS disease. Tolerability was similar between the two arms, with no new safety signals observed. Clinical trial information: NCT01808573.

The aim of the study was to determine the factor structure of the Religious Problem-Solving Scale in an African population. The sample included 624 students: 314 Grade 12, secondary school students and 310 third year social science university students in South Africa. The principal component analysis with varimax rotation yielded three components accounting for 64% of the total variance. The first factor contained 12 items from the Deferring scale, the second 12 items of the Self-Directing scale and the third factor 12 items from the Collaborative scale. The Deferring rather than the Collaborative religious problem-solving style seems to be more prevalent in this African sample than among western subjects.

Abstract:
Optimized fixed-time embryo transfer (FTET) protocols for synchronization of recipients have the potential to improve the overall efficiency and profitability of embryo transfer (ET) programs. The objective of the present study was to evaluate the effect of dose of gonadotrophin-releasing hormone (GnRH) at initiation of a 5-day synchronization protocol for FTET. Holstein heifers (n=2689) at two locations were synchronized using a 5-day CO-Synch protocol as follows: Day 0: CIDR inserted, Day 5: CIDR removed, prostaglandin (PG)F2α treatment (500μg cloprostenol), Day 6: PGF2α treatment, Day 8: GnRH (100μg of gonadorelin). On Day 0, at the time of CIDR insertion, heifers were assigned in a completely randomised design to the following groups: Single (a single dose of GnRH; 100μg of gonadorelin), Double (200μg of gonadorelin) or No GnRH (control). All heifers received an Estrotect patch placed on Day 5 and evaluated for signs of oestrus on Day 8. At location A, heifers were evaluated by ultrasonography 5 days after GnRH to determine presence and size of corpus luteum (CL), whereas at location B presence and location of CL were determined by transrectal palpation at the time of transfer. Heifers with a CL received an embryo 7±1 days after GnRH administration, and pregnancy was determined by ultrasonography 41 and 63 days after GnRH. Data were analysed by generalized linear mixed models. Oestrus expression was greater in heifers that received Single and Double GnRH than in the No GnRH group (P=0.001). Similarly, utilisation rate (number transferred per number treated) was greater for heifers in the Single and Double GnRH group than for those in the No GnRH group (P=0.02). Pregnancy data were analysed for a subset of recipients using data from Day 41 (n=2267) and Day 63 (n=2042). The analysis of fertility outcomes included as covariates the type of embryo (invitro fresh or frozen and invivo fresh or frozen), embryo stage, embryo quality, interval from GnRH to transfer, and oestrus expression. Pregnancies per embryo transfer (P/ET) at Days 41 and 63 were not different between treatment groups (P=0.86), and there was no interaction between type of embryo and treatment (P>0.15). Pregnancy loss between Days 41 and 63 was not different (P=0.49) between treatments groups. In conclusion, the removal of the initial GnRH from a 5-day FTET protocol resulted in a slight but significant reduction in the utilisation rate and the percentage of heifers showing oestrus. However, there was no detrimental effect on fertility. As a result, the overall cost of the FTET program can be reduced by eliminating the need for the initial GnRH treatment without compromising fertility. In the two years (625-626 AH) (1227-1228 AD), Ibn Almugawer gave a precise description of Mecca Almukaramah in his era, pointing to the importance of Mecca Almukaramah and its religious rank. The material presented by him is of great importance because it filled a gap in the traveling and geographical information of Mecca Almukaramah at the beginning of the seventh century AH (13 AD), especially since Ibn Almugawer was not just a chatty and a historian, but also a traveler with accurate observation. In view of the importance of the book from the scientific and historical point of view, we have decided to re-consult it, especially to its most documented editions (Oscar Lofgren Verification), to explore its material about Makkah Almukaramah and to vet some of its details and references that reflect an important phase in the history of Mecca Almukaramah, for the paucity of the writing about and the disturbance of the available historical material around it.

Abstract:
Introduction: In high-income countries, vomiting often impedes oral rehydration therapy, leading to intravenous rehydration fluid administration to children with acute gastroenteritis. Ondansetron administration reduces vomiting and intravenous fluid administration in this population. We evaluated whether ondansetron is similarly effective when employed in Pakistan. Methods: In this 2-hospital, double-blind, placebo-controlled, emergency department-based, randomized trial, we recruited children aged 0·5 to 5·0 years, without dehydration, who had diarrhea and 1 episode of vomiting within 4 hours of arrival. Patients were randomly assigned (1:1), via an internet-based randomization service, using a stratified, variable block randomization scheme, to receive a single dose of oral ondansetron or placebo. The primary endpoint was intravenous rehydration (administration of 20 ml/kg over 4 hours of an isotonic fluid) within 72 hours of randomization. All randomized children were analysed. Results: From July 3, 2014, to January 12, 2017, 626 children were randomized. Intravenous rehydration was provided to 10.8% (34/314) and 10.3% (27/312) of children administered placebo and ondansetron, respectively (OR: 0.946; 95% CI: 0.564, 1.587; P=0.834). A regression model fitted with treatment group and adjusted for antiemetic administration and vomiting frequency in the preceding 24 hours, yielded similar results; OR=0.952; 95% CI: 0.570, 1.589; P=0.850. There was no evidence of interaction between treatment group and age (P=0.974), 3 diarrheal stools in the preceding 24 hours (P=0.983) or 3 vomits in the preceding 24 hours (P=0.554). During the 4-hour study observation period, 24.0% (75/314) and 19.6% (61/312) of children in the placebo and ondansetron groups vomited, respectively; OR: 0.774; 95%CI: 0.528, 1.133; P=0.187. Conclusion: Ondansetron administration did not significantly reduce intravenous rehydration use, suggesting that in children without dehydration, ondansetron administration does not significantly alter the disease course and should not be administered to this group of children.

We assessed the different sets of diagnostic criteria for primary progressive multiple sclerosis (PPMS), in order to determine their sensitivity when applied to a cohort of 261 PPMS patients. According to the Thompson criteria, 168 patients (64.4%) had definite PPMS, 84 patients (32.2%) had probable PPMS, and nine patients (3.4%) had possible PPMS; according to the McDonald criteria, 180 patients (69%) had PPMS; according to the revised McDonald criteria, 194 patients (74.3%) had PPMS. Our findings indicate that the revised McDonald criteria are more sensitive than the original McDonald criteria, but less sensitive than the Thompson criteria. Accreditation suggests an organisation has met standards of quality through extensive self-study and external review. This study examines the influence of Commission on Accreditation for Law Enforcement Agencies (CALEA) accreditation on citizen complaints. We identified the accredited status of the agencies included in the 2007 LEMAS report as CALEA-accredited or non-accredited resulting in a final sample size for this study of 628 agencies (CALEA accredited = 314, non-accredited = 314). Findings suggest that no difference exists between CALEA-accredited agencies and non-accredited agencies in: (1) the total number of complaints received; (2) the number of sustained citizen complaints.

To determine the role of endothelin-1 (ET-1) and its receptors in the regulation of calcitonin gene-related peptide (CGRP) release, male Wistar rats were divided into six groups and subjected to the following treatments for 1 wk with or without ABT-627 (an ETA receptor antagonist, 5 mg·kg−1·day−1 in drinking water) or A-192621 (an ETB-receptor antagonist, 30 mg·kg−1·day−1 by oral gavage): control (Con), ET-1 (5 ng·kg−1·min−1 iv), Con + ABT-627, Con + A-192621, ET-1 + ABT-627, and ET-1 + A-192621. Baseline mean arterial pressure (MAP, mmHg) was higher ( P < 0.05) in Con + A-192621 (122 ± 4) and ET-1 + A-192621 (119 ± 4) groups compared with Con (104 ± 6), ET1 (106 ± 3), Con + ABT-627 (104 ± 3), and ET1 + ABT-627 (100 ± 3) groups. Intravenous administration of CGRP8–37 (a CGRP receptor antagonist, 1 mg/kg) increased MAP ( P < 0.05) in ET-1 (13 ± 1), Con + A-192621 (12 ± 1), and ET-1 + A-192621 (15 ± 3) groups compared with Con (4 ± 1), Con-ABT-627 (4 ± 1), and ET-1 + ABT-627 (5 ± 1) groups. Plasma CGRP levels (in pg/ml) were increased ( P < 0.05) in ET-1 (57.5 ± 6.1), Con + A-192621 (53.9 ± 3.4), and ET-1 + A-192621 (60.4 ± 3.0) groups compared with Con (40.4 ± 1.6), Con + ABT-627 (40.0 ± 2.9), and ET-1 + ABT-627 (42.6 ± 1.9) groups. Plasma ET-1 levels (in pg/ml) were higher ( P < 0.05) in ET-1 (2.8 ± 0.2), ET-1 + ABT-627 (3.2 ± 0.4), Con + A-192621 (3.3 ± 0.4), and ET-1 + A-192621 (4.6 ± 0.3) groups compared with Con (1.1 ± 0.2) and Con-ABT-627 (1.3 ± 0.2) groups. Therefore, our data show that ET-1 infusion leads to increased CGRP release via activation of the ETA receptor, which plays a compensatory role in preventing ET-1-induced elevation in blood pressure.

PurposeThis study aims to use nanofluid-based minimum quantity lubrication (NMQL) technique to minimize the use of cutting fluids in machining of Inconel-625 and Stainless Steel 304 (SS-304) (Ni-Cr alloys).Design/methodology/approachMachining of Ni-Cr-based alloys is very challenging as these exhibit lower thermal conductivity and rapid work hardening. So, these cannot be machined dry, and a suitable cutting fluid has to be used. To improve the thermal conductivity of cutting fluid, multi-walled carbon nanotubes (MWCNTs) were added to the soybean oil and used with MQL. This study attempts to compare tool wear of coated carbide inserts during face milling of Inconel-625 and SS-304 under dry, flooded and NMQL conditions. The machining performance of both materials, i.e. Inconel-625 and SS-304, has been compared on the basis of tool wear behavior evaluated using scanning electron microscopy-energy dispersive spectroscopy.FindingsThe results indicate higher tool wear and lower tool life during machining of Inconel-625 as compared to SS-304. Machining of Inconel-625 exhibited non-consistent tool wear behavior. The tool failure modes experienced during dry machining are discrete fracture, cracks, etc., which are completely eliminated with the use of NMQL machining. In addition, less adhesion wear and abrasion marks are noticed as compared to dry and flooded machining, thereby enhancing the tool life.Research limitations/implicationsInconel-625 and SS-304 have specific applications in aircraft and aerospace industry, where sculptured surfaces of the turbine blades are machined. The results of current investigation will provide a rich data base for effective machining of both materials under variety of machining conditions.Originality/valueThe literature review indicated that majority of research work on MQL machining has been carried out to explore machining of Ni-Cr alloys such as Inconel 718, Inconel 800, AISI4340, AISI316, AISI1040, AISI430, titanium alloys, hardened steel alloys and Al alloys. Few researchers have explored the suitability of nanofluids and vegetable oil-based cutting fluids in metal cutting operation. However, no literature is available on face milling using nanoparticle-based MQL during machining Inconel-625 and SS-304. Therefore, experimental investigation was conducted to examine the machining performance of NMQL during face milling of Inconel-625 and SS-304 by using soybean oil (vegetable oil) with MWCNTs to achieve ecofriendly machining.

X-ray diffraction and scanning electron microscopy/energy dispersive spectroscopy were used to analyze the phases and microstructures of the corrosion products. Inconel 625 alloy and 316 ss exhibited high corrosion rates in molten chlorides, and the corrosion rates of these two alloys accelerated when the temperature increased from 700°C to 900°C. The results of the electrochemical tests showed that both alloys exhibited active corrosion in chloride molten salt, and the current density of 316 ss in chloride molten salt at 700°C was 2.756 mA/cm−2, which is about three times the value for Inconel 625 alloy; and the values of the charge transfer resistance (Rt) for Inconel 625 were larger than those for 316 ss. The corrosion of these two alloys is owing to the preferred oxidation of Cr in chloride molten salt, and the corrosion layer was mainly ZnCr2O4 which was loose and porous and showed poor adherence to metal.

The aim of the study was to examine the proposition that religiosity and the related factor, spirituality, contribute to lower personal distress among students. The sample comprised 624 students: 314 Grade 12 secondary school students and 310 third-year social science university students in South Africa. Results indicate that the majority of the students show a high degree of religious belief and religious involvement. Some religious variables – such as being a born-again Christian and considering religion as important – were associated with decreased Perceived Stress. Minor psychiatric morbidity as measured with the Self-reporting Questionnaire (SRQ; WHO, 1994) was positively associated with some religious beliefs and involvement, while meaning and direction in life was inversely associated with the SRQ score. All three religious coping styles were inversely associated with perceived stress. Contrary to expectations it was found that some of the religious coping styles were positively associated with minor psychiatric morbidity, especially the depression scores of the SRQ. Findings show that some religious variables were positively associated with mental health while others were inversely or not related, thus only partially supporting the religion-mental health link.

This study examined the effectiveness of solution-focused brief therapy (SFBT) on child well-being and family functioning outcomes for child welfare involved parents. A randomized controlled trial design was used to evaluate the effectiveness of SFBT as compared to treatment-as-usual in an outpatient substance abuse treatment center. Mixed linear models tested within and between-group changes using intent-to-treat analysis ( N = 180). Hedges’s g effect sizes examined the magnitude of treatment effects. Both conditions reported improvements on the child well-being measures (Behavior Rating Inventory of Executive Function [BRIEF]-Parent Report and Child Behavior Checklist-School Age Form [CBCL-SA]) and family functioning measures (Adult-Adolescent Parenting Inventory [AAPI-2] and Center for Epidemiologic Studies-Depression [CES-D] Short Form) at posttest. While none of the between group analyses were statistically significant on either outcome domains, effect sizes did show improvements in the small to medium range for both groups. SFBT effect sizes for BRIEF subscales ranged from .024 to .267 and for control group ranged from .136 to .363. SFBT effect sizes on CBCL-SA subscales ranged from .059 to .321 and for control group ranged from .101 to .313. SFBT effect sizes on AAPI-2 subscales ranged from .006 to .620 and control group ranged from .023 to .624. SFBT effect sizes on CES-D measure were .428 and for control group were .317. Results show SFBT to be an effective intervention for helping parents around child well-being and family functioning outcomes similar to current empirically-supported therapies. SFBT provides a more strengths-based approach to help families improve family well-being and thus help improve their child’s well-being.

The alkali volcanics and intrusive rocks, dated at around 35-33Ma, are cut by mineralised northeast and east trending faults showing predominant evidence for strike-slip. Mineralisation includes haematite-Au-Cu and is accompanied by iron-rich alteration of the volcanic rocks. Detailed assessment of the geometry of the fault system at Pu Sam Cap suggests that the faults formed as a Riedel shear system during left-lateral slip within the Song Hong-Song Chay shear zone and the numerous contemporaneous northwest trending faults to the south; the northeast trending faults are interpreted as dextral “book-end’’ faults between major northwest trending faults enclosing the Pu Sam Cap massif. As mineralisation is hosted within these faults and is also associated with lamprohyric dykes it confirms a thermal event younger than the alkaline volcanics and syenitic intrusives at Pu Sam Cap, suggesting a hidden, young porphyry system. Beden algısı, bireyin kendini zihninde canlandırması olarak tanımlanabilmektedir. Geçtiğimiz yıllarda ülkemizde obezite ve fazla kiloluluk prevelansı gençlerde de hızlı bir artış göstermektedir. Kilo artışı erkeklerin ve kadınların beden algısını negatif olarak etkilemekte ve bu durum medyada zayıflığın idealleştirilmesiyle ilişkilendirilmektedir. Araştırmanın temel amacı, üniversite öğrencilerin kendi zihinlerinde canlandırdıkları beden algı düzeyleri ile boy, ağırlık ve beden kitle endisleri (BKE) arasındaki ilişkinin belirlenmesidir. Araştırmanın örneklemini Ankara ve Sivas'ta öğrenim gören, 17-30 yaş arası 617 (304 erkek ve 313 kadın) üniversite öğrencisi oluşturmaktadır. Araştırma sonucunda BKE değerlerine göre erkeklerin %0.3'ü ve kadınların %6.1'i zayıf, erkeklerin %9.9'u ve kadınların %6.4'ü obez olarak saptanmıştır. Beden ağırlığı algısı durumuna göre erkeklerin %6.1'i ve kadınların %29.6'sı kendilerini zayıf, erkeklerin %47.7'si ve kadınların %20.3'ü kendilerini fazla kilolu kabul etmektedir. Vücut algısı ölçeği ortalama puanı her iki cinsiyet arasında istatistiki açıdan anlamlı bulunmuştur (p<0.05). Her iki kentte eğitim gören erkeklerin kendilerini daha zayıf, kadınların daha kilolu algıladıkları belirlenmiştir. Kadınların görsel kaygılarla, erkeklerin ise sağlıklı olmak için kilo vermek istedikleri ve son bir ayda diyet yapan bireylerde kadınların daha yüksek oranda olduğu saptanmıştır. Araştırma sonuçları bireylerin fiziksel durumunun değerlendirilmesi ve beden hoşnutsuzluğunun rolü açısından da tartışılmıştır.

Background: The Oxford–AstraZeneca vaccine (Covishield) was the first to be introduced in Bangladesh to fight the ongoing global COVID-19 pandemic. As this vaccine had shown some side-effects in its clinical trial, we aimed to conduct a study assessing short-term adverse events following immunization (AEFIs) in Bangladesh. Method: A cross-sectional study was conducted on social and electronic media platforms by delivering an online questionnaire among people who had taken at least one dose of the COVID-19 vaccine. The collected data were then analysed to evaluate various parameters related to the AEFIs of the respondents. Results: A total of 626 responses were collected. Of these, 623 were selected based on complete answers and used for the analysis. Most of the respondents were between 30–60 years of age, and 40.4% were female. We found that a total of 8.5% of the total respondents had been infected with the SARS-CoV-2 virus. Our survey revealed that out of 623 volunteers, 317 reported various side-effects after taking the vaccine, which is about 50.88% of the total participants. The majority of participants (37.07%, 231/623) reported swelling and pain at the injection site and fever (25.84%, 162/623); these were some of the common localized and generalized symptoms after the COVID-19 vaccine administration. Conclusion: The side-effects reported after receiving the Oxford–AstraZeneca vaccine (Covishield) are similar to those reported in clinical trials, demonstrating that the vaccines have a safe therapeutic window. Moreover, further research is needed to determine the efficacy of existing vaccines in preventing SARS-CoV-2 infections or after-infection hospitalization.

490 Background: LEN has demonstrated the efficacy and safety in pts with aHCC as first-line treatment in the pivotal REFLECT trial. Further evaluation in real-world setting is necessary to measure the clinical outcomes of LEN in daily practice. Methods: This is a multicenter retrospective analysis from 3 Korean referral cancer institutions. Between September 2018 and August 2019, a total of 74 pts received LEN for the management of BCLC B or C aHCC, and 66 pts who had at least one follow-up visit after the start of LEN were included in this analysis. Results: Median age was 58 years (range, 19-81), and 46 pts (69.7%) were male. Baseline characteristics were as follows; Child-Pugh class A/B/C in 46 (69.7%)/14 (21.2%)/6 (9.1%), BCLC B/C/D in 1 (1.5%)/63 (95.5%)/2 (3.0%), prior systemic therapy in 25 (37.9%) including 14 (21.2%) with prior immune checkpoint inhibitors (ICIs). LEN was used as first/second/third to fourth lines of therapy in 41 (62.1%)/13 (19.7%)/12 (18.2%) pts, and 27 (40.9%) had extensive disease extent excluded in the REFLECT trial. With a median follow-up duration of 4.8 months (95% CI, 3.4–6.1), the median PFS and OS were 4.6 (95% CI, 3.2-6.0) and 7.5 months (mo) (95% CI, 3.7–11.2), respectively, in overall pts: first-line setting, 4.2 (95% CI, 3.2-5.2) and 6.5 mo (95% CI, 5.0-8.1), respectively; ≥ second-line setting, 6.1 mo (95% CI, 3.6–8.5) and not reached, respectively. In pts with prior ICIs, median PFS was 6.1 mo (95% CI, 1.8-8.4) and median OS was not reached. According to the RECIST v 1.1, response rates and disease control rate were 12.1% and 71.2%, respectively, in overall pts. The most common grade 3-4 toxicities were hyperbilirubinemia (n=9, 13.6%), AST elevation (n=5, 7.6%), diarrhea (n=4, 6.1%) and fatigue (n=4, 6.1%). Conclusions: LEN was effective and well tolerated in pts with aHCC in Korean real-life setting.

Abstract Aging attitudes have important consequences on functioning in later-life. A critical question concerns whether such attitudes may bias perceptions of one’s own aging, with potentially negative effects on important outcomes. Using data from adults aged 30 – 85 in the US (n=315), Hong Kong (n=317), and Germany (n=623), we examined the impact of age and aging attitudes on accuracy of perceptions of change in well-being over five years in different domains of functioning. Across contexts, comparisons revealed good correspondence between retrospective reports and actual change. However, older adults and those with negative attitudes retrospectively reported less positive change over this period. Accuracy of perceived change was affected by aging attitudes, with positive attitudes being associated with greater accuracy across most domains, although culture moderated these effects. The results highlight the complex relationship between culture and perceptions of well-being, as well as the potentially insidious effects of attitudes on their accuracy.

Abstract:
Objective: To determine the rate and predictors of palliative care referral (PCR) in hospitalized patients with acute heart failure (AHF). Introduction: The PCR is commonly utilized in terminal conditions such as metastatic cancers. There is no data on trends and predictors from large-scale registry of general population regarding PCR in patients with AHF. Methods: For this retrospective study, data were obtained from National Inpatient Sample Database from 2010 to 2014. We used International Classification of Diseases, Ninth Revision diagnosis codes to identify cases with a principle diagnosis of AHF. These patients were divided into 2 groups: (1) PCR, (2) no PCR groups. We performed multivariate analysis to identify predictors of PCRs, as well as reported PCR trends from 2010 to 2014. Results: From the database, out of 37 312 324 hospitalizations, 621 947 unweighted cases with primary diagnosis of AHF were selected for further analysis. About 2.8% received PCR. From 2010 to 2014, there was an uptrend from 2.0% to 3.6% for PCR. Metastatic cancer, ventilator-dependent respiratory failure, and cardiogenic shock were strongly associated with PCR. Those who underwent percutaneous coronary intervention and African American or other races were negative predictors for PCR. In the PCR group, 31.4% of patients died during hospitalization. Conclusion: Palliative care referrals were made in a very small proportion of patients with AHF. We observed steady rise in the PCR utilization. Chronic conditions, advancing age, and high-risk patients were major predictors of PCR.

314 Background: The mammalian target of rapamycin (mTOR) inhibitor everolimus is the first agent to show efficacy in a randomized, controlled phase III trial in patients with mRCC after progression on VEGFr-TKIs (RECORD-1). Progression-free survival (PFS) was significantly improved (4.9 vs 1.9 months) and the risk of disease progression was reduced by 67% with everolimus compared with placebo. To fulfill an unmet medical need, everolimus was offered globally in this EAP. Presented here are preliminary results on 605 patients. Methods: The program began in July 2008 (ClinicalTrials.gov: NCT00655252 ; EudraCT: 2007-005460-28), and since then over 1,000 patients in 34 countries have been enrolled. Patients with clear cell and non–clear cell mRCC who failed or became intolerant of VEGFr-TKIs received daily oral doses of everolimus with investigator assessment every 3 months. Results: Data were collected for 605 patients who had discontinued treatment as of January 15, 2010. Evaluable patients had a mean age of 63 years, and most (94%) had progressed after prior VEGFr-TKI therapy. The adverse event (AE) profile did not differ significantly from that reported in the RECORD-1 trial. Most frequently reported grade 3–4 AEs were anemia (6.1%), stomatitis (4.6%), fatigue (4.6%), hyperglycemia (4.0%), and infection (3.6%). Grade 3–4 noninfectious pneumonitis was reported in 2.8%. Best overall response was stable disease, which was evident in 42% of patients. Conclusions: The EAP has allowed patients with mRCC access to everolimus before marketing approval. The rapid enrollment rate of this EAP confirms the unmet medical need after failure of VEGFr-TKIs. Everolimus has shown good tolerability, and no new safety issues have been identified. The investigator-assessed response rate is consistent with that reported in the RECORD-1 trial. The EAP provides an efficient framework for the development of other programs for innovative anticancer agents in patients without satisfactory therapeutic options. [Table: see text]

ABSTRACTRigid pavement for highways in Indonesia started as an alternative for road pavement. Roller compacted concrete (RCC) is one type of dry concrete mixture with low water cement ratio (slump <5 cm). The RCC spreading is the same as the asphalt concrete pavement that uses the finisher and is compacted using a roller. This research uses RCC mix design with cement water factor value 0,25. Additional silicafume materials of 10%, superplasticizer 2.4%, and steel fiber (dramix) with variations of 0.5% and 1%. RCC Compaction using standard proctor tools. Testing of bending forces and compression of concrete in 1 day and 28 days. The result of research of RCC variation of 0.5% steel fiber and 1 day old concrete obtained Flexural Strength of 4,2 MPa while 28 days concrete gained Flexural Strength of 5,7 MPa. Penghamparan RCC sama seperti penghamparan perkerasan beton aspal yang menggunakan finisher dan dipadatkan menggunakan roller. Kajian ini menggunakan mix design RCC dengan nilai faktor air semen 0,25. Bahan tambah silicafume sebesar 10%, superplasticizer 2,4%, dan serat baja (dramix) dengan variasi 0,5% dan 1%. Pemadatan RCC menggunakan alat standar proctor. Pengujian kuat lentur pada beton umur 1 hari dan 28 hari. Hasil kajian RCC variasi serat baja 0,5% beton umur 1 hari diperoleh kuat lentur 4,2 MPa, sedangkan beton umur 28 hari diperoleh kuat lentur 5,7 MPa. Four copolymers (P(tCz (tris(4-carbazoyl-9-ylphenyl)amine)-co-bTP (2,2'-bithiophene)), P(tCz-co-CPDT (4H-cyclopenta[2,1-b:3,4-b’]dithiophene)), P(tCz-co-DTC (3,6-di(2-thienyl)carbazole)), and P(tCz-co-CPDTK (cyclopentadithiophene ketone))) are deposited on indium tin oxide (ITO) surfaces using electrochemical polymerization. Spectroelectrochemical properties of copolymer electrodes reveal that the colors of P(tCz-co-bTP) film are pinkish-orange, light olive green, light grayish blue, and dark blue at 0.0, 0.8, 1.2, and 1.6 V, respectively, whereas the color variations of P(tCz-co-CPDTK) film are light yellow, yellow, and blue at 0.0 V, 0.8 V, and 1.5 V, respectively. The ΔT of P(tCz-co-bTP), P(tCz-co-CPDT), P(tCz-co-DTC), and P(tCz-co-CPDTK) films are estimated to be 43.0% at 967 nm, 28.7% at 864 nm, 43.6% at 870 nm, and 24.5% at 984 nm, respectively. Five electrochromic devices (ECDs) are assembled using the tCz-based homopolymer and copolymers as coloring electrodes, and poly(2,2-dimethyl-3,4-propylenedioxythiophene) (PProDOT-Me2) as the complementary electrode. P(tCz-co-DTC)/PProDOT-Me2 ECD reveals high transmittance change (45.9% at 624 nm), P(tCz-co-CPDTK)/PProDOT-Me2 ECD shows high η (513.0 cm2 C−1 at 582 nm), and P(tCz-co-bTP)/PProDOT-Me2 ECD presents short switching time (less than 0.4 s) at 628 nm. Moreover, these ECDs show satisfactory redox stability and open circuit stability.

The &ldquo;Breme&rdquo; onion is a red-skinned cultivar growing in the northwest Italy. To date, its nutrient composition has not been described. In this study, we quantified the contents of quercetin (Q) and its glycosides and we studied their stability in the dependence on the local home-storage methods storage at 4&deg;C and freezing. Quercetin-3,4'-O-diglycoside (3,4'-Qdg) was the most abundant form, followed by quercetin-4'-O-diglycoside (4'-Qmg ) and Q. We observed the reduction in the contents of all the analysed flavonols after storage at 4&deg;C and after storage in frozen state. No changes have been observed in the ratio Q/3,4'-Qdg + 4'-Qmg, as well as in 3,4'-Qdg /4'-Qmg between the fresh, stored at 4&deg;C, and frozen onions. This could suggest an overall condition of instability, not the activation of a selective deglycosylation pathway. In conclusion, our study shows that the &ldquo;Breme&rdquo; onion is mainly rich in 3,4'-Qdg and that home-storage methods do not preserve the stability of some important health-promoting molecules.

7 Background: S-1/cisplatin is regarded as one of the standard regimens in Japan by the SPIRITS trial, but cisplatin needs in-hospital treatment for hydration. Phase II data of S-1/DOC showed long survival by outpatient basis. Then we hypothesized that S-1/DOC can be the standard regimen for AGC as outpatient basis. Methods: This is a prospective, multinational randomized phase III study in pts with AGC. Arm A: pts receive S-1 (40mg/m2) twice daily 14 days followed by 7 days rest plus DOC (40 mg/m2) iv on day 1. Arm B: pts receive S-1 28days followed by 14 days rest. Eligibility criteria included measurable/nonmeasurable AGC, age 20–79 and no prior chemotherapy. Primaryendpoint was OS, secondary endpoints were TTP, RR and safety. Based on planned sample size of 628 pts, the trial was designed tohave 90% power to detect an improvement in OS from 300 to 400 days. The accrual period was 3 years with 2 years follow up. Results: 639 pts (Arm A/B, 316/323) were registered and the eligible pts were 635 (314/321).The MST for Arm A was 386 and for Arm B was 327days. The OS for Arm A was not superior to Arm B (log-rank p = 0.1595, HR: 0.88). The TTP for Arm A was 160 and for Arm B was 126 days. The TTP for Arm A was significantly superior to Arm B (log-rank p < 0.0003, HR: 0.74). RR of Arm A was significantly higher than Arm B (36.0%/ 24.4%, p = 0.01). By subset analysis of OS in pre-stratified nonmeasurable AGC, the MST for Arm A was significantly superior to for Arm B (523/335, log-lank p = 0.018). Most common grade 3/4 toxicities were: neutropenia, 31.4% vs. 4.4%; leucopenia, 22.1% vs. 2.5% and anorexia; 15.4% vs. 12.1%. Conclusions: The combination ofS-1/DOC did not meet primary endpoint of OS, but of OS in nonmeasurable group and TTP of the S-1/DOC was significantly superior to that of the S-1 alone. We supposed that recent development of the second-line chemotherapy was influenced to our results. This regimen can be regarded as one of the standard treatments for AGC as outpatient basis. On December 22, a preliminary count for the Harvard Graduate Students Union-UAW was not conclusive, with 314 challenge ballots exceeding the margin between 1,272 yes and 1,456 no votes. Both elections were possible because the National Labor Relations Board, ruling on a suit brought by Columbia students, overturned a 2004 decision that prohibited the formation of graduate unions in private colleges and universities.

The clinical chart of 621 patients with III-IV haemorrhoids undergoing Stapled Hemorrhoidopexy (SH) with CPH34 HV in 2012–2014 was consecutively reviewed to assess its safety and efficacy after at least 12 months of follow-up. Mean volume of prolapsectomy was significantly higher (13.0 mL; SD, 1.4) in larger prolapse (9.3 mL; SD, 1.2) (p<0.001). Residual or recurrent haemorrhoids occurred in 11 of 621 patients (1.8%) and in 12 of 581 patients (1.9%), respectively. Relapse was correlated with higher preoperative Constipation Scoring System (CSS) (p=0.000),Pescatori’s degree (p=0.000),Goligher’s grade (p=0.003), prolapse exceeding half of the length of the Circular Anal Dilator (CAD) (p=0.000), and higher volume of prolapsectomy (p=0.000). At regression analysis, only the preoperative CSS,Pescatori’s degree,Goligher’s grade, and volume of resection were significantly predictive of relapse. A high level of satisfaction (VAS = 8.6; SD, 1.0) coupled with a reduction of 12-month CSS (Δ preoperative CSS/12 mo CSS = 3.4, SD, 2.0;p<0.001) was observed. The wider prolapsectomy achievable with CPH34 HV determined an overall 3.7% relapse rate in patients with high prevalence of large internal rectal prolapse, coupled with high satisfaction index, significant reduction of CSS, and very low complication rates.

Objetivo: evaluar los progresos del crecimiento y del desarrollo durante el primer trimestre de vida según lo estipulado en la resolución 0412/00 del ministerio de la protección social de la república de colombia, y la adherencia a la 1° y 2° consulta posnatal por parte de las madres. Materiales y métodos: estudio de corte transversal analítico, en una población de 1 229 recién nacidos, cuyo parto fue atendido en las clínicas de assbsalud e.s.e (manizales-colombia) sede san cayetano entre los años 2011 y 2012. se analizaron las historias clínicas para extraer de allí los variables necesarias para el estudio. Resultados: al primer control posnatal asistieron el 83,2% de los recién nacidos, con una edad promedio de 3,4 días, al 2° el 44,6%, con una edad promedio de 3,4 meses, el 76,7% de las madres dieron lactancia materna exclusiva a sus bebés, el 6,1% no tenían plan de inmunización completo, 10% con patologías congénitas, osteo-muscular en el 28,3%, 44,3% de patología adquirida, en el 53,5% respiratoria, sífilis congénita en el o,2%, no se encontraron casos de ictericia. básicamente asistió a controles la población urbana, la que había asistido a más controles prenatales, y cuando el bebé tenía alguna patología, congénita o adquirida. Conclusiones:la alta inasistencia, sobre todo en la 2° consulta posnacimiento, dificulta el seguimiento del progreso del crecimiento y desarrollo del bebé durante el primer trimestre de vida en esta población Objective: To determine the standards of growth and development of infants throughouttheir first appointments during their first trimester of life and to evaluate the characteristicsthat may influence, directly or indirectly, the proper development of the newborn’shealth. Materials and methods: Cross sectional study, including a population of1229 infants delivered between 2011 and 2012 in ASSBASALUD E.S.E. clinic in SanCayetano. The medical records where analyzed to obtain the necessary variables.Results: To the first appointment 83.2% of the newborns assisted, having an averageage of 3.4 days. To the second control 44.6% assisted having an average age of 3.4months. 76.7% of the mothers gave exclusively breast milk to the babies, 6.1% didn’thad any complete vaccination plan, 10% had congenital pathologies, 28,3% in theskeletal and muscular system, 44.3% acquired pathology, 53.3% respiratory systemand 0.2% congenital syphilis, there were no cases found of jaundice. Basically theurban population assisted the most to the appointments and when the baby has anyacquired or congenital pathology. Conclusions: The high absence rate, mostly in thesecond appointment after birth makes it difficult to follow the growth and developmentof the baby during the first trimester of life.

ABSTRACT Background Information regarding the tobacco habits in patients visiting Mahatma Gandhi Mission's (MGM) Dental College and Hospital is scarce. Therefore, development and better implementation of the tobacco cessation initiatives can be undertaken with the knowledge of the tobacco use among patients. Materials and Methods A cross-sectional, questionnairebased study was conducted among the outpatients of MGM Dental College and Hospital from July to September, 2012. Questions regarding the form of tobacco used, frequency, duration and attitude toward stopping its consumption were asked. Statistical analysis was carried out using descriptive statistics and chi-square tests using Statistical Software package (SPSS) version 17. Results 314/505 (62.1%) of the patients reported of tobacco use. Majority 162/314 (51.6%) of the patients used smokeless tobacco in which mishri and gutkha were mostly consumed. Work pressure was the common 100/314 (31.8%) cause of tobacco use. More than half the patients had tried to quit the habit and showed awareness toward its adverse effects on oral and general health. Conclusion Tobacco use has become an important concern due to its growing addiction among all age groups and gender. It is imperative to understand the psychosocial ideology and pattern of tobacco use among the patients in order to effectively counsel and motivate them to stop the habit. Secondary outcomes were progression-free survival (PFS), overall response rate, and toxicity.ResultsFrom 2001 to 2004, 626 patients were randomly assigned; 312 patients were assigned to PCG, and 314 patients were assigned to GC. After a median follow-up of 4.6 years, the median OS was 15.8 months on PCG versus 12.7 months on GC (hazard ratio [HR], 0.85; P = .075). OS in the subgroup of all eligible patients was significantly longer on PCG (3.2 months; HR, 0.82; P = .03), as was the case in patients with bladder primary tumors. PFS was not significantly longer on PCG (HR, 0.87; P = .11). Overall response rate was 55.5% on PCG and 43.6% on GC (P = .0031). Both treatments were well tolerated, with more thrombocytopenia and bleeding on GC than PCG (11.4% v 6.8%, respectively; P = .05) and more febrile neutropenia on PCG than GC (13.2% v 4.3%, respectively; P < .001).ConclusionThe addition of paclitaxel to GC provides a higher response rate and a 3.1-month survival benefit that did not reach statistical significance. Novel approaches will be required to obtain major improvements in survival of incurable urothelial cancer.

Abstract:
HVOF coating finds application in industry to protect the surface from the harsh environments such as high temperature, corrosion, and abrasion. In the present study, HVOF coating consisting of Inconel 625 powders blended with WC particles and sprayed on to 304 steel is carried out. The mechanical properties, such as elastic modules and fracture toughness, of the resulting coating are determined using the indentation tests. The influence of the mass fraction of WC on the fracture toughness and elastic modulus of the coating are also examined. It is found that addition of WC particles in Inconel 625 powder enhances the fracture toughness of the resulting surface. This is attributed to increased elastic modules and hardness.

Abstract:
The combinations of organic liquid fertilizer material obtained from sea lettuce (Ulva lactuca) and different concentrations of zeolite were applied to the cucumber (Cucumis sativus) plant in six treatments with three replicates each. The combinations of the experimental groups were as follows; control group, G1 group 180 g Z (Zeolite), G2 group 90 g Z + 1140 ml Liquid Fertilizer (LF), G3 group 180 g Z + 620 ml LF, G4 group 270 g Z + 310 ml LF, G5 group 620 ml LF. Except the control group, all zeolites were applied with the first dose of liquid fertilizer, and other liquid fertilizer doses were given in three periods, after germination, flowering and yield. While the lowest germination rate was in control group, the highest rate was in group with 60 g zeolite. While the lowest seedling weight was observed in group with 90 g zeolite + 1140 ml liquid fertilizer, the highest rate was in group with 180 g zeolite + 620 ml liquid fertilizer. While the highest cucumber weight was observed in group with 270 g zeolite + 310 ml liquid fertilizer, the lowest was in group with 620 ml liquid fertilizer. Therefore, it may be said that zeolite has positive effect on germination. On the other hand, liquid fertilizer and zeolite combinations may have positive effects on the weight of the cucumber seedling and yield.
