Abstract:
Abstract Heliconias have great commercial importance due to their durability of inflorescences, sizes, forms and bright colors. Regarding its production, the mineral nutrition of the crop is one of the most relevant aspects to obtain quality floral stems. The objective was to determine the influence of N-P-K on the growth and nutritional content of Heliconia stricta Dwarf Jamaican. A completely randomized block design was used with four treatments and five repetitions. The treatments consisted in fertilizing the soil with N-P-K. The levels of fertilization expressed in kg ha-1 of NPK were: control without application 00-00-00 (T100), 31-31-31 (T231), 62-62-62 (T362) and 93-93-93 (T493). The plants received the treatments for six months. Growth variables and nutrient contents of heliconia biomass were evaluated. The analysis of variance showed significant statistical differences (P ≤ 0.05) in the vigor and color of the plant, as well as the phosphorus, magnesium and copper contents. The significant increases in vigor and color were with doses 31-31-31 and 62-62-62 of NPK. As for the contents of P, Mg and Cu were influenced by doses 31-31-31, 62-62-62 and 93-93-93 of NPK. This work shows important findings in relation to the nutritional diagnosis in the foliar content of nutrients.

Aim. The goal of the study was determine whether, from a molecular point of view, inhibitors KN-93 and KN-62 are capable to disrupt the functioning of plant homologs of CaMK2 and being used as tools for the experimental study of Ca2+-dependent phosphorylation in higher plants. Methods. Selected calmodulin 1 H.sapiens reference structure and reconstructed spatial structure of calcium-binding domain of CPK1 A.thaliana. We have conducted the molecular docking of calmodulin-mediated inhibitors to CaMK2: KN-93 and KN-62 for CALM1 H. sapiens and calcium-binding domain of CPK1 A. thaliana with full ligand mobility and static amino acid residues with the use of CCDC GOLD Suite. Results. We have established the presence of spatially homologous structures within CALM1 and CPK1 that might be the binding sites for KN-93 and KN-62. Performing the molecular docking we have shown the utility of those pockets from the point of binding energy for KN-93 and KN-62. We have conducted comparative analysis basing on the results of the CCDC GOLD Suite score functions (GoldScore and ASPScore). Conclusions. It has been shown that the inhibitors of animal CaMK2 - KN93 and KN62 are capable of interacting with the site of the CaM-like domain of the plant protein kinase CPK1 homologous to the similar site of animal calmodulin, which may impair its functionality. Abstract:
RESUMO Klebsiella pneumoniae é um patógeno oportunista, responsável por diversos tipos de infecções nosocomiais, e é considerado um microrganismo multirresistente. Dados na literatura que forneçam informações a respeito da resistência desse microrganismo a antimicrobianos em amostras de animais são escassos. Dessa forma, o objetivo deste trabalho foi avaliar o perfil e o seu aumento das resistências a antimicrobianos dentro da medicina veterinária. Um total de 67 isolados de K. pneumoniae, provenientes de diferentes sítios de isolamento de animais domésticos (39/67) e silvestres (28/67), foi confirmado por sequenciamento do gene 16S rRNA. O maior percentual de isolamento de K. pneumoniae foi de amostras de urina, com 16% (11/67), fezes, com 15% (10/67), e pulmão, com 13,5% (09/67). No perfil de resistência, foram testadas 11 categorias de antibióticos, sendo a maior taxa de resistência ao metronidazol 97% (65/67), à ampicilina 94% (63/67), à amoxicilina 93% (62/67), às sulfonamidas 93% (62/67), à colistina 93% (62/67) e à nitrofurantoína 88% (59/67). Aqueles que apresentaram menor taxa de resistência foram: meropenem 3% (2/67), imipenem 6% (4/67) e amicacina 16% (11/67). Todos os isolados foram considerados bactérias multirresistentes (MRD), com o índice de resistência múltipla aos antibióticos (IRMA) variando de 0,15 a 0,85 e com 60 tipos de padrões de resistência. O resultado deste estudo reforça que os animais são reservatórios de K. pneumoniae multirresistentes.

Abstract:
The influence of intravenous (iv) bombesin on multiunit activities recorded from the ventral gastric branch of the vagus was investigated in urethan-anesthetized rats. Consecutive injections of bombesin (0.062, 0.62, 6.2, 62, and 620 pmol iv) decreased dose dependently gastric vagal efferent discharges to 79.8 +/- 4.9, 68.3 +/- 10.2, 47.0 +/- 6.7, 41.6 +/- 4.7, and 36.5 +/- 8.9%, respectively, from preinjection levels. Saline injection had no effect. Bombesin (62 pmol iv) reduced cervical vagal efferent discharges to 25 +/- 6% before and 67 +/- 5% after bilateral cervical vagotomy distal to the recording site. Bombesin (62 and 620 pmol iv) increased gastric vagal afferent discharges by 45 and 93%, respectively. These data show that systemic injection of bombesin potently decreases gastric and cervical vagal efferent activity in part through vagal-dependent mechanisms that may involve the increase in gastric vagal afferent activity.

The Talk Test (TT) is a simple technique for prescribing exercise training intensity, based on the ability to ‘speak comfortably’ after reciting a standard speech provoking stimulus. This study compares the length of the speech provoking stimulus on power output (PO) at standard TT speech comfort markers: last positive (LP), equivocal (EQ), negative (NEG), in relation to objective markers of exercise intensity, the ventilatory (VT) and respiratory compensation (RCT) thresholds. Eighteen healthy subjects performed incremental (25W/2 min) exercise with concomitant gas exchange to measure VT and RCT. They also performed (random order) incremental exercise without gas exchange while repeating standard speech provoking stimuli of 31, 62 and 93 words to allow identification of the LP, EQ and NEG stages of the TT. The mean (±SD) PO at LP (139±38, 117±39 and 103±38 W), EQ (164±38, 142±38 and 128±33 W) and NEG (196±42, 189±43, and 174±39 W) stages of the TT were analyzed for 31, 62 and 93 word passages, in relation to the PO at VT (128±43 W) and RCT (175±39W). PO at EQ and PO at NEG stages of the TT with the 93 word speech provoking stimulus were not significantly different than the PO at VT and PO at RCT, respectively. The mean error for predicting PO at VT approximated zero with the longer (93 word) speech passage duration for EQ (0.1±37 W) and for predicting the PO at RCT for NEG (0.1±29 W). The results suggest that a longer duration speech provoking passage optimizes accuracy of the TT estimation of VT and RCT

Based on a 1999 needs assessment a pediatric community-based outpatient dietitian counselling service was created. By 2010 annual referrals had grown to almost 1500 (62% from physicians; 38% from public health nurses). An evaluation was undertaken to gather perspectives of practitioners and parents about access, satisfaction, referral practices, and changes in knowledge, attitudes, behaviour, and child well-being. Health professionals surveyed via email were 62 pediatricians (response rate 71%), 25 family physicians (21%), 87 public health nurses (31%), and 7 dietitian providers (100%). Parents (n = 93, response rate 75% of those contacted) were interviewed by telephone. Pediatricians reported a significantly lower rate of 7% (95% confidence interval (CI), 0.8%–23%) for admitting children to hospital to access a dietitian, compared to 1999 of 39% (95% CI, 22%–59%) (P = 0.005). Health professionals reported a high degree of agreement on benefits of the service to their practice and on child health problems and a high degree of satisfaction with the service. Parents reported gaining knowledge (76%), confidence (93%), and making behaviour changes in foods offered (77%). Our evaluation demonstrated health practitioners saw a need for access to dietitians for pediatric dietitian counselling and parents reported more confidence and improved child feeding practices after dietitian counselling.

SummaryAn enzyme linked immunosorbent assay (ELISA), based on sheep hydatid cyst fluid antigen was used for the detection of specific antibodies of IgG, IgM, IgE and IgG subclass in the serum samples of 62 clinically and radiologically diagnosed cystic echinococcosis (CE) patients, 8 clinically suspected cases of CE, 25 other parasitic disease controls and 25 healthy controls. The diagnostic sensitivity in the clinically and radiologically suggestive cases (n = 62) for IgG antibody detection was highest (93 %), followed by IgE, IgG4, IgG1, IgG2, IgM and IgG3 with 89 %, 87 %, 85 %, 76 %, 70 % and 55 % respectively. The detection of specific IgE, IgG1 and IgG4 antibody showed the higher diagnostic sensitivity and specificity to the extent that they can be safely used as better substitute to IgG. Even though, the diagnostic sensitivity of IgG was highest (93%) but was less specific (88 %) due to the frequent non-specific reactions in the sera of patients with other parasitic infections and healthy controls. None of the sera samples from healthy controls gave non-specific reaction with IgE, IgG1 and IgG4 and there was a considerably reduced cross-reaction with these antibodies. The most discriminatory and specific antibodies found in this study belonged to IgE, IgG1 and IgG4; therefore, these antibodies may serve as useful diagnostic markers for CE.

Cucumber (Cucumis sativus L) is mostly cultivated by family-based farmers worldwide and processed to pickles by small to mid-sized industries generating employment and income. But irrigation management needs better investigation for adapting adequate sustainable practices. The rational use of irrigation water still has been neglected nowadays, but can improve cucumber production. The objective was to evaluate different irrigation levels on growth and yield parameters of the Amour F1 cucumber hybrid through time under greenhouse. Treatments consisted of five irrigation levels (amounts of water applied of 62, 93, 124, 155 and 186 mm), ranging from water stress to excess water. The experimental design was randomized blocks, with four replications. Vegetative growth and yield showed different responses to the irrigation levels. Plant height and internode length were less influenced by the irrigation levels, throughout the time, than stem diameter and root length. Cucumber yield was low with 62 mm and 93 mm, higher with 124 mm irrigation, and delayed in time at 155 mm and 186 mm. Water excess (186 mm) was not beneficial for pickling cucumber plants, and the amount of 124 mm was satisfactory for their development and yield. The results of this study may allow adoption of sustainable irrigation practices with no waste of agricultural water, a scarce resource worldwide

Abstract Abstract 3509 Allogeneic hematopoietic cell transplantation (alloHCT) has become standard therapy for hematologic disorders and malignancies. We assessed whether overall survival (OS) at 100 days, which represents early transplant-related mortality (TRM), and at one year, which represents disease-related mortality and later TRM, had changed over time. The study population was derived from patients undergoing 38,060 first alloHCTs between 1994–2005 in US and Canadian centers and reported to the CIBMTR. Donor lymphocyte infusions were excluded. Statistical significance was measured using Ptrend over 6 time cohorts to test whether the OS estimates were stable (slope = 0), increasing (slope>0) or decreasing (slope<0) over time. The Day 100 and 1 year OS estimates are shown in the Table. Disease and disease status subgroups were defined a priori, and the OS estimates are not adjusted for any covariates such as age, Karnofsky status, etc. Marked improvements in 100-day survival were seen for all disease and disease status groups examined. Day 100 mortality rates in HLA-matched sibling alloHCT recipients during the most recent period (2004-5) were as low as 2% for CML treated in CP1, 6–8% for AML in CR1 and ALL in CR2, and a modest 12% for MDS. Even in alloHCT recipients with unrelated donors treated in 2004-5, the Day 100 mortality rates ranged from 9–22%, a significant decrease from historical mortality rates of 29–37%. Significant improvements in 1-year OS were noted for all groups undergoing unrelated-donor alloHCT; however, among those undergoing HLA-matched sibling alloHCT, significant improvements in 1-year OS were only seen in patients with CML in CP1 or MDS. OS has improved for many patients undergoing myeloablative alloHCT, which likely reflects improvement in supportive care and better patient/donor selection. Day 100 OS has significantly improved in all patients who received a myeloablative HLA-matched related or unrelated donor alloHCT. Significant improvement in 1-year OS was also experienced by most patients. Abstract:
The metabotropic receptor mGluR6 is localized to the dendrites of On bipolar cells and mediates synaptic input from photoreceptors. The binding of glutamate to the receptor activates a phosphodiesterase (PDE), which then hydrolyzes cGMP. A nonselective cationic conductance, believed to be gated directly by cGMP, is turned off as a result of the fall in cGMP levels, and the cell hyperpolarizes. Here we present evidence for regulation of the conductance by an additional mechanism that it is independent of cGMP. Whole-cell recordings were obtained from On bipolar cells in slices of tiger salamander retina. Dialysis of cells with 1 μM KN-62 or 10 μM KN-93, two inhibitors of type II calmodulin-dependent protein kinase (CaMKII), depressed cGMP-dependent currents. This depression persisted when hydrolysis of cGMP was prevented with IBMX, a broad-spectrum PDE inhibitor, suggesting that CaMKII acts downstream from the PDE in the cascade. The depression of cGMP-dependent currents was probably not due to a direct interaction of the inhibitors with the channels as neither 1 μM KN-62 or 10 μM KN-93 was found to have any effect on cyclic nucleotide-gated channels when applied directly to excised patches of rod outer segments. We propose that phosphorylation by CaMKII may be an important mechanism for regulating the cGMP-dependent conductance of On bipolar cells.

Abstract:
<p><strong>Objetivo: </strong>analizar la calidad del registro y grado de control de los factores de riesgo cardiovascular en pacientes con antecedentes de cardiopatía isquémica, atendidos en consulta de atención primaria mediante historia clínica informatizada. <strong>Métodos: </strong>estudio transversal. Se utilizaron los registros en omi-ap y selene (programas de historia clínica informatizada utilizados en el Servicio Murciano de Salud) para una muestra de 150 pacientes. Las variables analizadas fueron: antropométricas (índice de masa corporal imc), bioquímicas (perfil lipídico), consumo de alcohol, factores de riesgo cardiovascular registrados (tabaco, hipertensión arterial, diabetes mellitus, enfermedad cerebrovascular, arteriopatía periférica), así como el tratamiento registrado. A mayor número de variables registradas se consideró el registro de mejor calidad. <strong>Resultados:</strong> 62 pacientes tenían diabetes mellitus (41.33%), 93 eran hipertensos (62%), 17 presentaban enfermedad cerebrovascular (11.33%) y 16, arteriopatía periférica (10.67%). Se registró tabaquismo en 94 pacientes (62.67%), de los cuales, 22 eran fumadores (23.40%). El consumo de alcohol solo estaba registrado en 93 pacientes de la muestra (62%) y, de ellos, 23 presentaban un patrón de consumo de alcohol elevado (15.33%). El colesterol total y los triglicéridos constaban en la historia clínica de 114 pacientes (76%), con hdl y ldl en 101 (67.33%). The effects of inhibitors of CaMKII on intracellular Ca2+ signaling were examined in single calf pulmonary artery endothelial (CPAE) cells using indo-1 microfluorometry to measure cytoplasmic Ca2+ concentration ([Ca2+]i). The three CaMKII inhibitors, KN-93, KN-62, and autocamtide-2-related inhibitory peptide (AIP), all reduced the plateau phase of the [Ca2+]i transient evoked by stimulation with extracellular ATP. Exposure to KN-93 or AIP alone in the presence of 2 mM extracellular Ca2+ resulted in a dose-dependent increase of [Ca2+]i consisting of a rapid and transient Ca2+ spike followed by a small sustained plateau phase of elevated [Ca2+]i. Exposure to KN-93 in the absence of extracellular Ca2+ caused a transient rise of [Ca2+]i, suggesting that exposure to CaMKII inhibitors directly triggered release of Ca2+ from intracellular endoplasmic reticulum (ER) Ca2+ stores. Repetitive stimulation with KN-93 and ATP, respectively, revealed that both components released Ca2+ largely from the same store. Pretreatment of CPAE cells with the membrane-permeable inositol 1,4,5-trisphosphate (IP3) receptor blocker 2-aminoethoxydiphenyl borate caused a significant inhibition of the KN-93-induced Ca2+ response, suggesting that exposure to KN-93 affects Ca2+ release from an IP3-sensitive store. Depletion of Ca2+ stores by exposure to ATP or to the ER Ca2+ pump inhibitor thapsigargin triggered robust capacitative Ca2+ entry (CCE) signals in CPAE cells that could be blocked effectively with KN-93. The data suggest that in CPAE cells, CaMKII modulates Ca2+ handling at different levels. The use of CaMKII inhibitors revealed that in CPAE cells, the most profound effects of CaMKII are inhibition of release of Ca2+ from intracellular stores and activation of CCE.

A study was carried out on 93 consecutive unilateral below-knee (BK) and 62 above-knee (AK) amputees. The dimensions of the amputation stumps were measured and the general condition and contralateral limb assessed at the time of prosthetic fitting. After one postoperative year, follow-up information for 124 (89%) of the surviving patients was obtained by personal contact. The observations were based on the standard formula for stump classification constructed by the International Society for Prosthetics and Orthotics. The 93 BK stumps had a mean length of 16.0 cm and the 62 AK stumps a mean length of 28.0 cm. The scar on the stump was adherent in 13% of BK and 2% of AK stumps. The scar was deeply wrinkled in 7% of BK stumps and 10% of AK stumps. The scar on the stump was most frequently adherent or deeply wrinkled in trauma patients (33%). The skin was undamaged in 93% of all the patients at the first visit and in 94% at the time of follow-up. The mobility of the stump in the proximal joint was limited at the time of prosthetic fitting in 15% of cases. Phantom pain was reported by 59% and stump pain by 5% of patients at this time. Although the phantom pain was mild in most cases, it was usually still present after one year, and 53% of the surviving patients suffered from phantom pain. At the first visit, 20% of patients had problems in their contralateral leg. During the first postoperative year, 6 contralateral BK amputations were performed in the BK group and one contralateral AK amputation in the AK group. Thus, along with examination of the stump, attention must be paid to the contralateral limb with a view to preserving it. The study supports the usefulness of the standard form and classification of amputation stumps.

Fructosamine was measured in the serum of 62 patients with insulin-dependent diabetes (IDD) and 32 non-diabetics and the results compared with glycated albumin levels (GSA) measured using the affinity medium Cibarcron blue F3GA. Good correlations were found both for the IDD patients ( r=0·93) and the combined group of IDD plus non-diabetics ( r=0·95). We conclude, that fructosamine measurements accurately reflect GSA concentrations, and, therefore, provide a practical method for assessing intermediate term glycaemia in IDD.

A field experiment was conducted in a newly reclaimed soil at El-Saff region, El-Giza Governorate, Egypt to study the effects of different rates of nitrogen (N: 62 to 248 kg ha-1) with or without soil inoculation of sulfur- (S-) oxidizing bacteria (SoxB) and combined inoculation of SoxB and N-fixing bacteria (NFxB) on yield, quality and nutritional status of onion (Allium cepa L., “Giza 20”). Elemental S at 620 kg ha-1was applied to all treatments. Application of N at 62, 124, and 248 kg ha-1rates increased onion yield, plant height, and N uptake by 28 to 76%, 32 to 53%, and 61 to 145%, as compared to those of the plants that received no N. Inoculation of SoxB at various N rates increased onion yields by 47 to 69% and N uptake by 76 to 93%, as compared to those of the plants which received the respective rates of N but no SoxB inoculation. Inoculation with SoxB and NFxB increased onion yield by 221%, plant height by 62%, and N uptake by 629%, as compared to those of the plants grown without inoculation and no N applied.

Abstract Study AML-BFM 98 aimed to improve prognosis by intensification and optimization of the initial therapy elements. Between 7/1998 and 6/2003, 473 children and adolescents &lt; 18 years with de novo AML entered the multicenter AML-BFM 98 trial. Treatment was based on the results of study AML-BFM 93 and was similar in standard risk (SR) and high-risk (HR) patients (definition for SR [n=192]: FABM1/M2 with Auer rods, M4eo with ≤ 5 % blasts in the day-15 bone marrow, all patients with M3; HR: all others [n=281]). Treatment: After induction with AIE (cytarabine, idarubicin and etoposide) all patients (excluding FAB M3) were treated with HAM (high-dose cytarabine 3g/m2/12h x3 days and mitoxantrone 10mg/m2/day x2 days), which, in the previous study 93, was given for HR patients only. Subsequently all patients were randomly assigned to receive either the 6-week consolidation as in the previous studies or two short therapy cycles including higher doses of cytarabine but the same cumulative dose of anthracyclines. The following therapy elements, intensification with high-dose cytarabine/etoposide and maintenance, were similar in studies 93 and 98. Allogeneic stem cell transplantation from a family donor was restricted to HR patients only. Overall results: 418 of 473 (88%) patients achieved remission. Early death rate could be reduced compared to study 93: 3.2% vs.7.4%. Five-year survival, event-free survival (EFS) and disease-free survival (DFS) were 62%±3%, 49%±3% and 57%±3%, respectively. Estimated survival improved (study 93: 58%±2%, plogrank .03), pEFS and pDFS were similar to study 93 (50%±2% and 61%±2%, n.s.). Randomization results of the 6-week consolidation arm (n=191) vs. the 2-short-cycle arm (n=199) were similar (pEFS 51%±4 and 50%±4%, n.s.). However, total treatment duration was shorter (median 15 days) and morbidity was lower in the short-cycle arm (5 vs. 9 deaths in CCR). Five-year pSurvival in HR patients (surviving &gt;.44years, median time to SCT) with or without SCT in 1. CR was similar: 69%±14% vs. 64%±4%; plogrank .35. There was no improvement of prognosis in SR patients (FAB M3 excluded) compared to the previous study 93 despite additional application of HAM (CR rate 93% vs. 89%, p(chi) .24, 5-years pEFS 62%±4% vs. 67%±4%; n.s.). Conclusion: The estimated survival is now in the range of 65%–70% for those patients who achieve remission. Results of study 98 show, that intensification of chemotherapy with HAM does not improve survival in SR patients. There is no difference in survival of HR patients receiving SCT in 1st CR compared to those with chemotherapy alone. Because improvement of prognosis can not be achieved by intensivication of therapy alone, optimizing the chemotherapeutic regimen by introducing less toxic, but similar effective therapy elements together with better supportive care strategies will reduce treatment related mortality and thereby improve survival in small steps.

Goals: To assess the long-term results of transoral incisionless fundoplication (TIF 2). Background: TIF with the EsophyX2 is an accepted procedure to treat gastroesophageal reflux disease (GERD). Long-term data have been limited. We report clinical outcomes of 151 patients followed up to 9 years. Study: A single institution prospective registry of patients undergoing TIF 2 between 11/2008 and 7/2015. Outcomes were assessed by complications, re-interventions, and a mixed effect model of clinical response over time. Results: A total of 151 patients (87 women), mean age 62 years (30–91), mean body mass index (BMI) 26.6 (20–36.1), 93% on daily proton pump inhibitor (PPI), underwent TIF 2 without hiatal hernia repair; 131 of the 151 patients (86%) were available for follow-up at a median of 4.92 years (0.7–9.7 years). Of 120 patients ⩾5 years post-TIF, 62 (51%) were followed for a median 6.8 years. Median GERD-health-related quality of life (HRQL) scores decreased from 21 (interquartile range (IQR) 9.5–30) off PPI and 14 (4–24) on PPI at baseline to 4 (2–8) at 4.92 years and remained at 5 (2–9) in the 62 patients 5–9 years post-TIF. Sixty-four per cent had successful (>50%) reductions in GERD-HRQL scores at 4.92 years and 68% of patients followed ⩾5 years. Median regurgitation decreased from 15 (8–20) off PPI and 11 (5–20) on PPI at baseline to 0 (0–4) at 4.92 years, remaining at 1 (0–3) in 62 patients 5–9 years post-TIF. Mixed model analyses confirmed significant and stable improvements in GERD-HRQL and regurgitation scores at all annual follow-up time points after TIF. Daily PPI use decreased from 93% to 32% at 4.92, and 22% at ⩾5 years post-TIF. Revision to laparoscopic fundoplication in 33(22%) showed comparable outcomes. Two patients recovered uneventfully after laparoscopic surgery for localized perforation. Conclusions: TIF 2 provides durable relief of GERD symptoms at up to 9 years with 69–80% of patients having a successful outcome by symptom response and PPI use.

Background Despite the potential and interest of using technology for delivering specialized psychiatric services to young adults, surprisingly limited attention has been paid to systematically assess their perspectives in this regard. For example, limited knowledge exists on the extent to which young people receiving specialized services for a first-episode psychosis (FEP) are receptive to using new technologies as part of mental health care, and to which types of technology-enabled mental health interventions they are amenable to. Objective The purpose of this study is to assess the interest of young adults with FEP in using technology to receive mental health information, services, and supports. Methods This study uses a cross-sectional, descriptive survey design. A convenience sample of 67 participants between the ages of 18 and 35 were recruited from two specialized early intervention programs for psychosis. Interviewer-administered surveys were conducted between December 2013 and October 2014. Descriptive statistics are reported. Results Among the 67 respondents who completed the survey, the majority (85%, 57/67) agreed or strongly agreed with YouTube as a platform for mental health-related services and supports. The top five technology-enabled services that participants were amenable to were (1) information on medication (96%, 64/67); (2) information on education, career, and employment (93%, 62/67); (3) decision-making tools pertaining to treatment and recovery (93%, 62/67); (4) reminders for appointments via text messaging (93%, 62/67); and (5) information about mental health, psychosis, and recovery in general (91%, 61/67). The top self-reported barriers to seeking mental health information online were lack of knowledge on how to perform an Internet search (31%, 21/67) and the way information is presented online (27%, 18/67). Two thirds (67%; 45/67) reported being comfortable in online settings, and almost half (48%; 32/67) reported a preference for mixed formats when viewing mental health information online (eg, text, video, visual graphics). Conclusions Young people diagnosed with FEP express interest in using the Internet, social media, and mobile technologies for receiving mental health-related services. Increasing the awareness of young people in relation to various forms of technology-enabled mental health care warrants further attention. A consideration for future research is to obtain more in-depth knowledge on young people’s perspectives, which can help improve the design, development, and implementation of integrated technological health innovations within the delivery of specialized mental health care.

To determine the prevalence of amino acid substitutions in hepatitis C virus NS5a and core proteins which areassociated with resistance to direct-acting antivirals and interferon in genotype 1b (HCV-1b) strains circulating in Russia. Materials and methods. Nucleotide sequences of NS5a (n=93) and core (n=30) of HCV-1b were obtained using direct sequencing of respective amplified genome fragments. The search for resistance associated substitutions was performed for amino acid positions 28, 29, 30, 31, 32, 58, 62, 92, 93 of NS5a, and 70 and 91 amino acid positions of core proteins, respectively. Results. The total proportion of HCV-1b strains carrying resistance associated substitutions in NS5a was 22,6% (21/93). The total detection rate of L31M and Y93H substitutions that are associated with resistance to the majority of NS5a inhibitors was 10,8%. Less clinically significant substitutions L28M, R30Q, P58S/T, A92T were detected too. The proportion of infections caused by HCV-1b strains that are potentially resistant both to interferon and NS5a inhibitors was 10% (10/30). Conclusion. Testing of HCV-1b infected patients for background resistance profile could be a useful tool to prevent the choosing of initially ineffective treatment regimen.

Abstract. Die vorliegende Studie geht von der These aus, dass die Entvölkerung des Berggebietes in Friaul innerhalb der gesamten Ostalpen am weitesten fortgeschritten ist. Um 1990 zählten die meisten Subregionen Nordfriauls zu jenen Gebieten Italiens, die seit dem Zweiten Weltkrieg die stärksten Bevölkerungsverluste hinnehmen mussten. Ausschlaggebend dafür waren mehrere Auswanderungsschübe, deren negative Auswirkungen auf die aktuelle Bevölkerungsstruktur sich in steigenden Sterbe- und sinkenden Geburtenziffern widerspiegeln. Eine Abschwächung der Bevölkerungsabnahme in jüngster Vergangenheit ist lediglich auf wenige Zuwanderungen zurückzuführen, die statistisch zwar in Erscheinung treten, insgesamt gesehen jedoch zu vernachlässigen sind. Verglichen mit dem übrigen Berggebiet Friauls häufen sich im Canale di Ferro trotz periodisch anwesender Freizeitbevölkerung total entsiedelte geschlossene Ortschaften (ghost towns) wie sonst nirgendwo im Ostalpenraum. Da die bisherigen raumwirtschaftlichen Entwicklungsmaßnahmen die sozioökonomischen und demographischen Probleme nicht lösen konnten und eine Umsetzung unkonventioneller Förderungsstrategien wenig realistisch erscheint, ist davon auszugehen, dass die Entvölkerung des friulanischen Gebirgsraumes in Zukunft weiter anhalten wird.

It presupposes that symptoms cluster in two dimensions, fitting together not only theoretically but empirically. Factor analysis of three published studies of 93, 62 and 52 schizophrenic patients and a large pooled sample showed that more than two distinct dimensions are required to categorise symptoms in schizophrenia. This result is consistent across methods and samples, and with previous literature. The added dimensionality resulted from a splitting of the positive symptom domain into more distinct factors.

Abstract Background Endemic mycoses are caused by the dimorphic fungi Histoplasma capsulatum, Blastomyces dermatitidis, and Coccidiodes species. Histoplasmosis and blastomycosis are endemic in Minnesota, with travel to coccidioidomycosis endemic areas being common. Diagnosis is challenging, in part due to confusion regarding laboratory testing. In our institution, we have observed that fungal serologies are often ordered when such infections are suspected, but results rarely seem to affect management. We reviewed the impact of serologic testing on the management of a Midwest veteran population. Methods Retrospective, observational study of patients with any serologic testing for endemic mycoses performed from January2014 to December 2018 at the Minneapolis VA Health Care System. To focus evaluation on the utility of serologic testing, we excluded patients with fungal antigen testing. Results Of 127 patients tested, 62 (49%) had only serologic testing. Patients were predominantly males (95%) with a median age of 66 years (range 27–93). Nineteen (31%) were tested in the hospital and had a median stay of 8 days (range 1–48). Median Charlson score was 4 (range 0–12). Travel to an endemic area for coccidioidomycosis was frequent (27/62: 44%), with Arizona being the most common destination (20/27: 74%). Median illness duration was 30 days (range 1–720). Respiratory symptoms predominated (43/62: 69%), followed by nonspecific (6/62: 10%), neurologic (5/62: 8%), and musculoskeletal (2/62: 3%) symptoms. Five (8%) were asymptomatic. Abnormal imaging was common, with 27/62 (44%) patients having an abnormal chest radiograph (consolidation 15%, nodules 11%, and interstitial pattern 8%), and 44/62 (71%) having abnormal CT findings (nodules 55%, ground glass opacities 18%, and consolidation 15%). Six patients (10%) had positive serology, but antifungals were started only in one case. Fungal serology results impacted management in 19/43 (44%) patients seen in clinic, but in 0/19 tested as inpatients (P < 0.001). The most common action (16/19: 84%) was to cease diagnostic workup (table). Conclusion Fungal serologies can be useful in patients evaluated in clinic who present with respiratory symptoms, abnormal imaging, and potential fungal exposure. It shows a statistical significance as 0.007 from Independent Sample T-test. Conclusion: The RNN-LSTM performs better in finding accuracy and precision when compared to CNN.

We recently developed a mass spectrometry (MS) procedure based on the detection of a serum disaccharide (MS-DS) in patients with invasive candidiasis (IC). Here, we compare the performance of MS-DS for the diagnosis of IC, invasive aspergillosis (IA), and mucormycosis (MM) with those of commercially available antigen detection tests. This retrospective study included 48 patients (23 IC patients [74 serum samples], 15 IA patients [40 serum samples], and 10 MM patients [15 serum samples]) and 49 appropriate controls (102 serum samples). MS-DS, mannan (Mnn), galactomannan (GM), and (1,3)-β-d-glucan (BDG) were detected by matrix-assisted laser desorption ionization–time of flight (MALDI-TOF) MS, Platelia, and Fungitell assays, respectively. For IC, the sensitivity and specificity of the MS-DS index, BDG detection, and Mnn detection were 62% and 84%, 82% and 60%, and 33% and 94% per serum sample and 83% and 69%, 96% and 31%, and 39% and 86% per patient, respectively. For IA, the corresponding values in comparison to BDG and GM detection were 83% and 81%, 62% and 95%, and 62% and 100% per serum sample and 93% and 76%, 87% and 90%, and 93% and 100% per patient, respectively. Nine of the 10 MM patients had a positive MS-DS result. MS-DS gave an early diagnosis in IC (73% positivity before blood culture), IA (positive before GM detection in six patients), and MM (positivity mainly preceded the date of diagnosis) patients. For IC, persisting MS-DS was associated with a poor prognosis. The different biomarkers were rarely detected simultaneously, suggesting different kinetics of release and clearance. For IA, MS-DS provided better complementation to GM monitoring than BDG monitoring. MS-DS detects panfungal molecules circulating during invasive fungal infections. The performance of MS-DS compared favorably with those of biological tests currently recommended for monitoring at-risk patients. Further validation of this test in multicenter studies is required.

Os tratamentos consistiram de 00; 31; 62; 93 e 124 g de NPK, na proporção de 20:05:20, aplicados em três parcelamentos de 20%, 40% e 40% da dose aos 45, 120 e 240 dias, após o plantio. O delineamento experimental utilizado foi o de blocos casualizados, com quatro repetições. O peso médio do fruto foi influenciado pela dose de adubo, com valor significativamente maior para peso do fruto sem coroa, na dosagem de 93 g de adubo. Com a elevação das doses de adubo os teores de brix e acidez titulável aumentaram, enquanto a relação brix/acidez titulável apresentou redução.

Individualized dosage regimens have recently been recommended for patients treated with aminoglycoside antibiotics. We have developed a calculator-based program for our patients with cystic fibrosis and have studied 93 courses of intravenous aminoglycoside treatment, comparing predicted and measured values in 45 courses. Pharmacokinetic parameters differed notably among subjects: This was reflected by widely variable total daily aminglycoside dosage requirements. The mean daily dosage requirements (± SD) for tobramycin (62 treatment courses) was 13.0 ± 3.74 mg/kg, and for gentamicin (26 treatment courses) was 11.5 ± 2.6 mg/kg. The accuracy of the program was evaluated by its ability to predict peak and trough values in individuals: 84 percent of measured peaks were within 2 μg/ml of predicted peaks and 93 percent of trough levels were within 1 μg/ml of the predicted level. Nephrotoxicity was observed in one patient, ototoxicity in three. This program provides a simple, safe, and effective method of tailoring an aminoglycoside regimen to the patient's needs.

Patient pathways to diagnosis were mapped and routes for different cancers were compared.ResultsOnly 2/93 cases (2.1%) in 10 years were referred via the 2-week pathway. Most presentations were acute via immediate general practitioner (GP) referral or self-referral to the emergency department 62/93 (67%). Leukaemia presented acutely and via the GP more often than via self-presentation to the emergency department 21/28 (75% vs 25%), while solid tumours were self-referred to the emergency department 21/34 (62% vs 38%) more often than via the GP. TTP and TTD were calculated for 87 patients. Wilms’ tumour demonstrated the shortest median TTP of 7 days and TTD of 16 days. Lymphoma had the longest TTD, with TTP 107 days and TTD 120 days. Pathways to diagnosis via other specialties were longer.ConclusionThe majority of children diagnosed with cancer present via acute services, with the route varying between tumour types. Only two cases in 10 years were referred via the 2-week pathway, thus challenging its relevance in the paediatric population.

Abstract The prevalence of depressive symptoms among medical students is known to be higher because they have been faced with variety of stressors. The purpose of this study was to determine the depressive symptoms among undergraduate medical students of Mulawarman University. This was a quantitative descriptive research with univariate analysis. Beck Depression Inventory (BDI) was used to collect the primary data. A cross-sectional study was conducted on a total of 364 sampled students selected by purposive sampling technique. The obtained data were described using descriptive statistics. The results of this study showed that 177 students (48,6%) had low (minimal) levels of depression, 93 students (25,6%) had mild depression, 62 students (17,0%) had moderate depression, and 32 students (8,8%) had severe depression. In summary, most students in the Medical Study Program of Mulawarman University had minimal depressive symptoms. Abstrak Prevalensi gejala depresi pada mahasiswa kedokteran diketahui lebih tinggi karena mereka dihadapkan dengan stresor yang bervariasi. Tujuan penelitian ini yaitu untuk mengetahui gejala depresi pada mahasiswa kedokteran preklinik di Universitas Mulawarman. Penelitian ini merupakan penelitian deskriptif kuantitatif dengan analisis univariat. Beck Depression Inventory (BDI) digunakan untuk mengumpulkan data primer. Studi potong lintang dilakukan pada 364 mahasiswa menggunakan teknik purposive sampling. Data yang telah diperoleh dideskripsikan menggunakan statistik deskriptif. Hasil penelitian menunjukkan sebanyak 177 (48,6%) mahasiswa memiliki tingkat depresi rendah (minimal), 93 mahasiswa (25,6%) memiliki gejala depresi ringan, 62 mahasiswa (17,0%) memiliki gejala depresi sedang, dan 32 mahasiswa (8,8%) memiliki gejala depresi berat. Background: The Pentagon Drawing Test (PDT) is a common assessment for visuospatial function. Evaluating the PDT by artificial intelligence can improve efficiency and reliability in the big data era. This study aimed to develop a deep learning (DL) framework for automatic scoring of the PDT based on image data. Methods: A total of 823 PDT photos were retrospectively collected and preprocessed into black-and-white, square-shape images. Stratified fivefold cross-validation was applied for training and testing. Two strategies based on convolutional neural networks were compared. The first strategy was to perform an image classification task using supervised transfer learning. The second strategy was designed with an object detection model for recognizing the geometric shapes in the figure, followed by a predetermined algorithm to score based on their classes and positions. Results: On average, the first framework demonstrated 62%accuracy, 62%recall, 65%precision, 63%specificity, and 0.72 area under the receiver operating characteristic curve. This performance was substantially outperformed by the second framework, with averages of 94%, 95%, 93%, 93%, and 0.95, respectively. Conclusion: An image-based DL framework based on the object detection approach may be clinically applicable for automatic scoring of the PDT with high efficiency and reliability. With a limited sample size, transfer learning should be used with caution if the new images are distinct from the previous training data. Partitioning the problem-solving workflow into multiple simple tasks should facilitate model selection, improve performance, and allow comprehensible logic of the DL framework.

ABSTRACT Isolates of Salmonella enterica serovar Typhi that are multidrug resistant (MDR, resistant to chloramphenicol, ampicillin, and trimethoprim-sulfamethoxazole) and have reduced susceptibility to fluoroquinolones (nalidixic acid resistant, Nar) are common in Asia. The optimum treatment for infections caused by such isolates is not established. This study compared different antimicrobial regimens for the treatment of MDR/Nar typhoid fever. Vietnamese children and adults with uncomplicated typhoid fever were entered into an open randomized controlled trial. Ofloxacin (20 mg/kg of body weight/day for 7 days), azithromycin (10 mg/kg/day for 7 days), and ofloxacin (15 mg/kg/day for 7 days) combined with azithromycin (10 mg/kg/day for the first 3 days) were compared. Of the 241 enrolled patients, 187 were eligible for analysis (186 S. enterica serovar Typhi, 1 Salmonella enterica serovar Paratyphi A). Eighty-seven percent (163/187) of the patients were children; of the S. enterica serovar Typhi isolates, 88% (165/187) were MDR and 93% (173/187) were Nar. The clinical cure rate was 64% (40/63) with ofloxacin, 76% (47/62) with ofloxacin-azithromycin, and 82% (51/62) with azithromycin (P = 0.053). The mean (95% confidence interval [CI]) fever clearance time for patients treated with azithromycin (5.8 days [5.1 to 6.5 days]) was shorter than that for patients treated with ofloxacin-azithromycin (7.1 days [6.2 to 8.1 days]) and ofloxacin (8.2 days [7.2 to 9.2 days]) (P < 0.001). Positive fecal carriage immediately posttreatment was detected in 19.4% (12/62) of patients treated with ofloxacin, 6.5% (4/62) of those treated with the combination, and 1.6% (1/62) of those treated with azithromycin (P = 0.006). Both antibiotics were well tolerated. Uncomplicated typhoid fever due to isolates of MDR S. enterica serovar Typhi with reduced susceptibility to fluoroquinolones (Nar) can be successfully treated with a 7-day course of azithromycin.

: Introduction: The lesbian, gay, bisexual, and transgender (LGBT+) population experience health and social inequalities, including discrimination within healthcare services. There is a growing international awareness of the importance of providing healthcare professionals and students with dedicated training on LGBT+ health. Methods: We introduced a compulsory teaching programme in a large London-based medical school, including a visit from a transgender patient. Feedback was collected across four years, before (n = 433) and after (n = 541) the session. Student confidence in using appropriate terminology and performing a clinical assessment on LGBT+ people was assessed with five-point Likert scales. Fisher exact tests were used to compare the proportion responding “agree” or “strongly agree”. Results: Of the students, 95% (CI 93–97%) found the teaching useful with 97% (96–99%) finding the visitor’s input helpful. Confidence using appropriate terminology to describe sexual orientation increased from 62% (58–67%) to 93% (91–95%) (Fisher p < 0.001) and gender identity from 41% (36–46%) to 91% (88–93%) (p < 0.001). Confidence in the clinical assessment of a lesbian, gay or bisexual patient increased from 75% (71–79%) to 93% (90–95%) (p < 0.001), and of a transgender patient from 35% (31–40%) to 84% (80–87%) (p < 0.001). Discussion: This teaching programme, written and delivered in collaboration with the LGBT+ community, increases students’ confidence in using appropriate language related to sexual orientation and gender identity, and in the clinical assessment of LGBT+ patients.

Regulation of intracellular Ca2+ concentration ([Ca2+]i) in airway smooth muscle (ASM) during agonist stimulation involves sarcoplasmic reticulum (SR) Ca2+ release and reuptake. The sarco(endo)plasmic reticulum Ca2+-ATPase (SERCA) is key to replenishment of SR Ca2+ stores. We examined regulation of SERCA in porcine ASM: our hypothesis was that the regulatory protein phospholamban (PLN) and the calmodulin (CaM)-CaM kinase (CaMKII) pathway (both of which are known to regulate SERCA in cardiac muscle) play a role. In porcine ASM microsomes, we examined the expression and extent of PLN phosphorylation after pharmacological inhibition of CaM (with W-7) vs. CaMKII (with KN-62/KN-93) and found that PLN is phosphorylated by CaMKII. In parallel experiments using enzymatically dissociated single ASM cells loaded with the Ca2+ indicator fluo 3 and imaged using fluorescence microscopy, we measured the effects of PLN small interfering RNA, W-7, and KN-62 on [Ca2+]i responses to ACh and direct SR stimulation. PLN small interfering RNA slowed the rate of fall of [Ca2+]i transients to 1 μM ACh, as did W-7 and KN-62. The two inhibitors additionally slowed reuptake in the absence of PLN. In other cells, preexposure to W-7 or KN-62 did not prevent initiation of ACh-induced [Ca2+]i oscillations (which were previously shown to result from repetitive SR Ca2+ release/reuptake). However, when ACh-induced [Ca2+]i oscillations reached steady state, subsequent exposure to W7 or KN-62 decreased oscillation frequency and amplitude and slowed the fall time of [Ca2+]i transients, suggesting SERCA inhibition. Exposure to W-7 completely abolished ongoing ACh-induced [Ca2+]i oscillations in some cells. Preexposure to W-7 or KN-62 did not affect caffeine-induced SR Ca2+ release, indicating that ryanodine receptor channels were not directly inhibited. These data indicate that, in porcine ASM, the CaM-CaMKII pathway regulates SR Ca2+ reuptake, potentially through altered PLN phosphorylation.

Conventionally, curing is performed by moisturizing concrete periodically when the fresh concrete is still undergoing chemical reaction processes. This study is focused on early age concrete strength treated with special curing of early heating treatment and steaming. The addition of chemical accelerator and normal curing are also examined. Tests were conducted on normal and high strength concrete specimens. Treatments of the test specimens with steaming and early heating were carried out for 3 and 8 hours, respectively, at a temperature of about 70° C. Test results showed that concrete compressive strengths gained in 3-days compared to 28-days under normal curing, chemical accelerator, steam curing, and elevated temperature curing were 39%, 50%, 64% and 59%, respectively. While in 7-days were 66%, 75%, 80% and 83%, respectively. And in 14- days were 87%, 91%, 93% and 93%, respectively. For high-strength concrete, the compressive strengths gained in 3-days were 37%, 62%, 68% and 71%, respectively. The strengths in 7-days were 65%, 77%, 83% and 82%, respectively. And in 14-days were 85%, 89%, 90% and 93%, respectively. Test results indicated that the additions of chemical accelerator, steam and elevated temperature curing in order to obtain the high early age concrete strengths are highly recommended.

Introduction: Continuing Medical Education (CME) significantly improves the competency of healthcare workers in Methadone Maintenance Treatment (MMT) clinics. However, CME courses are very costly, and a few participants fully attended a course. Online training is an alternative approach to efficiently improve training outcomes. The study assessed needs and possibility of online training courses of MMT clinics in southern Vietnam. Methods: A google form was designed to collect characteristics, man-powers, facilities, online activities and training needs of MMT clinics. E-mails were sent to all MMT clinics in southern Vietnam to ask for their participants. A representative of MMT clinics who satisfied the inclusion invited to complete the form. Result: 93 MMT clinics completed the survey. The response rate was 62% (93/150). One MMT clinic had 3 doctors/assistant doctors, 3 pharmacists/drug dispensers, 2 consultants and 3 other professionals on average. The number of clients visiting the clinic in the last month was 150. About 94% (93/95) of MMT clinics provide other additional services. On average, 385 clients came to MMT for other services. All clinics had adequate devices for online and blended training. Conclusion: MMT clinics had high training needs and were willing to attend online and blended training courses. Online and blended training were possible in MMT clinics.

AbstractVerrucous carcinoma is a slow growing, well demarcated, exophytic variant of squamosus cell carcinoma with a characteristic verrucous presentation. It is a rare tumour and in 75% of cases it is localised in the oral cavity, and sporadically within the tongue. HPV infection is identified in 40% of patients. Good prognosis is characteristic for this tumour, since the 5-year survival is 93%. The authors describe a case of verrucous carcinoma localised in the tongue of a 62-year old patient. The clinical course, diagnostics and proposed treatment was described and discussed with the existing literature data.

The purpose of this study was to review retrospectively the primary total hip arthroplasties operated upon with the cementless Wagner self-locking stem in patients with Type C femoral bone. Twenty-five total hip arthroplasties were performed in 23 patients aged ≥60 years using a cementless Wagner self-locking femoral component between 2006 and 2011. According to Dorr's criteria, all 25 femora were classified as type C bone. Mean follow-up period was 62 months (range, 47–85 months). Average Harris Hip score preoperatively was 46 ± 9 (range, 39–62) and at the last follow-up was 90 ± 9 (range, 83–98). The mean proximal stem-to-canal fill percentage was 95% ± 3.5% (anteroposterior view) and 90% ± 2.8% (lateral view) and the mean distal canal fill was 93% ± 4.5% (anteroposterior) and 92% ± 3.9% (lateral). Radiographic evaluation demonstrated good osteointegration of the implants in follow-up. Based on the long-straight cylindrical tapered stem design, the cementless Wagner SL stem can achieve reliable stability by close apposition of stem and wide stovepipe femoral canal from metaphysis to diaphysis in Type C bone.

Abstract:
Background: Few studies evaluated over 5-year outcomes of dialysis patients. This study examined 10-year all-cause mortality and death-censored technique failure in a cohort of incident peritoneal dialysis (PD) individuals. Methods: Five hundred and thirty-three incident PD individuals from 2006 to 2008 were prospectively followed up for more than 10 years until 2018. Clinical characteristics at PD initiation were collected. The primary outcome was all-cause mortality, and the secondary outcome was death-censored technique failure. Cox hazards models were fit using clinical characteristics at PD initiation. Results: The mean age starting PD for these participants was 48 ± 16 years; 130 (24%) patients had diabetic nephropathy. During follow-up, 208 patients died, and 84 patients experienced technique failures. The 1, 3, 5, and 10 years’ survival rates for incident PD patients were 93%, 81%, 64%, and 36%, respectively, and the technical survival rates were 98%, 93%, 85%, and 62%, respectively. Mortality risk was much higher after 3 years on PD. The peritonitis rate was 0.19 episodes per patient-year, and 7 (1.3%) patients had encapsulating peritoneal sclerosis (EPS) giving an incidence rate of 3.1 of 1000 patient-years. The main causes of death were cardiovascular events (97 of 208, 47%), and technique failure was mainly due to peritonitis (41 of 84, 49%). Older age, higher Charlson comorbidity index, and lower level of education were strongly associated with mortality, and diabetic nephropathy was an independent risk factor for technique failure. Conclusions: The 10-year’s survival and technique survival rates of incident PD patients were 36% and 62%. Long-term PD can be continued successfully with improved outcomes and low risk for EPS.

Abstract:
Purpose To improve prognosis in children with acute myeloid leukemia (AML) by randomized comparisons of (1) two short consolidation cycles versus the Berlin-Frankfurt-Muenster (BFM) -type biphasic 6-week consolidation and (2) the prophylactic administration of granulocyte colony-stimulating factor (G-CSF) versus no G-CSF. Further, therapy for standard risk patients was intensified by addition of a second induction, HAM (high-dose cytarabine and mitoxantrone). Patients and Methods Four hundred seventy-three patients younger than 18 years with de novo AML were enrolled in trial AML-BFM 98. Patients received five courses of intensive chemotherapy, cranial irradiation, and 1-year maintenance therapy. Results Four hundred eighteen patients (88%) achieved remission. Compared with trial AML-BFM 93, early deaths decreased from 7.4 to 3.2% (P = .005), and 5-year overall survival increased from 58% to 62% (log-rank P = .03). Both types of consolidation therapy led to similar outcome (event-free survival, 51% v 50%), but in the two-cycle arm, treatment duration was shorter (median duration, 15 days), and treatment related mortality was lower (five v nine patients). G-CSF shortened neutropenia, but did not reduce the rate of severe infections. Intensification of induction therapy did not improve prognosis of standard-risk patients (event-free survival, 62% v 67%). Conclusion Overall results were improved by neither the administration of G-CSF nor by cycle therapy; however, the latter was easier to perform. Compared with study AML-BFM 93, therapy intensification with HAM in standard-risk patients did not result in improved prognosis. Future treatment designs have to balance intensification of treatment with higher toxicity, improve supportive care, and to consider alternative treatment strategies.

Background Anesthetic preconditioning is mediated by beta-adrenergic signaling. This study was designed to elucidate the role of beta-adrenergic signaling in desflurane-induced postconditioning. Methods Pentobarbital-anesthetized New Zealand White rabbits were subjected to 30 min of coronary artery occlusion followed by 3 h of reperfusion and were randomly assigned to receive vehicle (control), 1.0 minimum alveolar concentration of desflurane, esmolol (30 mg x kg(-1) x h(-1)) for the initial 30 min of reperfusion or throughout reperfusion, the beta2-adrenergic receptor blocker ICI 118,551 (0.2 mg/kg), the protein kinase A inhibitor H-89 (250 microg/kg), or the calcium/calmodulin-dependent protein kinase II inhibitor KN-93 (300 microg/kg) in the presence or absence of desflurane. Protein expression of protein kinase B, calcium/calmodulin-dependent protein kinase II, and phospholamban was measured by Western immunoblotting. Myocardial infarct size was assessed by triphenyltetrazolium staining. Results Infarct size was 57 +/- 5% in control. Desflurane postconditioning reduced infarct size to 36 +/- 5%. Esmolol given during the initial 30 min of reperfusion had no effect on infarct size (54 +/- 4%) but blocked desflurane-induced postconditioning (58 +/- 5%), whereas esmolol administered throughout reperfusion reduced infarct size in the absence or presence of desflurane to 42 +/- 6% and 41 +/- 7%, respectively. ICI 118,551 and KN-93 did not affect infarct size (62 +/- 4% and 62 +/- 6%, respectively) but abolished desflurane-induced postconditioning (57 +/- 5% and 64 +/- 3%, respectively). H-89 decreased infarct size in the absence (36 +/- 5%) or presence (33 +/- 5%) of desflurane. Conclusions Desflurane-induced postconditioning is mediated by beta-adrenergic signaling. However, beta-adrenergic signaling displays a differential role in cardioprotection during reperfusion.

Background: Non-small cell lung cancer (NSCLC) commonly presents at advanced stage. We previously reported systemic treatment uptake in stage IV NSCLC climbing from 55% (2009–2012) to 62% (2015–2017). Since then, first-line immunotherapy and 2nd/3rd generation tyrosine kinase inhibitors (TKIs) have emerged as standards of care. We explored whether treatment rates continued to rise and studied outcomes. Methods: We reviewed all cases of de novo stage IIIB/IIIC/IV NSCLC seen in out-patient medical oncology consultation at our institution between 2009–2012 (cohort A), 2015–2017 (cohort B), and June–December 2018 (cohort C). We compared rates of systemic treatment, molecular testing, targeted therapy, and immune checkpoint inhibitor (ICI) use. We compared survival in the overall, treated/untreated, younger and elderly population in cohort A vs. cohort B + C (=cohort D). Results: Cohorts A, B, and C included 528, 463, and 93 patients, respectively. Overall, 66% received any systemic therapy in cohort C, compared to 62% in cohort B and 55% in cohort A. Across three time periods, first-line chemotherapy rates fell (93, 76, 46%) while rates of first-line targeted therapy (5, 16, 15%) and ICI (0, 2, 36%) rose. Among molecular subtypes, first-line targeted treatment in EGFR-positive patients (63, 94, 100%) and anaplastic lymphoma kinase (ALK)-positive patients (0, 91, 100%) rose. Survival improved in all subgroups in cohort D vs. cohort A, except for patients ≥ 70 years and the untreated population. Conclusions: Systemic treatment rose across three time periods, reflecting the introduction of rapid diagnostic pathways, reflex molecular testing, ICI, and targeted therapies. Survival outcomes of advanced NSCLC patients have significantly improved.

AbstractObjective:To evaluate the food and nutrient intake of members of a birth cohort study when young children in 1950 and investigate differences from present-day children's diets.Design:One-day recall diet records from the MRC National Survey of Health and Development (NSHD) (1946 Birth Cohort) at age 4 years were analysed for energy and selected nutrients and compared to the published results for 4-year-olds in the 1992/93 National Diet and Nutrition Survey (NDNS).Setting:England, Scotland and Wales in 1950 and 1992/93.Subjects:4599 children in 1950 and 493 children in 1992/93.Results:Mean (SD) daily intakes in 1950 were energy 1445 (343) kcal, or 6.1 (1.4) MJ, protein 46 (11)g, fat 64 (20)g, starch 117 (33)g, sugar 62 (24)g, unavailable carbohydrate 13 (4)g, calcium 736 (230)mg, iron 7.7 (2.1)mg, retinol 738 (1273) μg, carotene 1049 (1130) μg and vitamin C 40 (26) mg. Compared to 1992/93, the 1950 diet contained substantially more bread and vegetables and less sugar and soft drinks, giving it a higher starch and fibre content and making it more in line with current recommendations on healthy eating. However, fat provided 40% of energy in 1950, compared to 35% in 1992/93. In 1950, red meat was an important source of iron, but by 1992 most iron came from fortified breakfast cereals. Vitamin C came mainly from vegetables in 1950, but from soft drinks in 1992.Conclusions:The relative austerity of post-war food supplies resulted in food and nutrient intakes in 1950 which in many respects may well have been beneficial to the health of young children, despite fat intake being higher than present-day recommendations.
