Abstract:
Reversed-phase liquid chromatography hyphenated with inductively coupled plasma mass spectrometry (LC-ICP-MS) was used for mercury speciation analysis in food samples. A short chromatographic column (Purospher<sup>&reg;</sup> RP-8e, 75 &acute; 4&nbsp;mm, 3 &micro;m) and a mobile phase containing 0.02 mol/l CH<sub>3</sub>COONH<sub>4</sub> + 0.2% (v/v) 2-mercaptoethanol (2-ME) + 1% (v/v) CH<sub>3</sub>OH were applied. A repeated extraction of samples with hydrochloric acid/2-ME solution (1 mol/l HCl + 0.2% (v/v) 2-ME) was applied as the isolation step. The results were satisfactory for most food matrices (fish, shellfish, plant materials). Conversely, to analyse high-protein animal matrices, which contain mostly the inorganic form of mercury, a procedure including partial hydrolysis using hydrochloric acid should be used. For methylmercury and inorganic divalent mercury, the LOQ values of 0.3 and 2 ng/g, respectively, can be achieved if precautionary measures against contamination are fulfilled. The method was applied for the determination of methylmercury and inorganic divalent mercury in fish, vegetables, herbs and cereal products.

ImportanceIt is estimated that, from 2023 to 2025, lung cancer (LC) will be the second most frequent cancer in Brazil, but the country does not have an LC screening (LCS) policy.ObjectiveTo compare the number of individuals eligible for screening, 5-year preventable LC deaths, and years of life gained (YLG) if LC death is averted by LCS, considering 3 eligibility strategies by sociodemographic characteristics.Design, Setting, and ParticipantsThis comparative effectiveness research study assessed 3 LCS criteria by applying a modified version of the LC-Death Risk Assessment Tool (LCDRAT) and the LC-Risk Assessment Tool (LCRAT). Data are from the 2019 Brazilian National Household Survey. Participants included ever-smokers aged 50 to 80 years. Data analysis was performed from February to May 2023.ExposuresExposures included ever-smokers aged 50 to 80 years, US Preventive Services Task Force (USPSTF) 2013 guidelines (ever-smokers aged 55 to 80 years with ≥30 pack-years and &amp;lt;15 years since cessation), and USPSTF 2021 guidelines (ever-smokers aged 50 to 80 years with 20 pack-years and &amp;lt;15 years since cessation).Main Outcomes and MeasuresThe primary outcomes were the numbers of individuals eligible for LCS, the 5-year preventable deaths attributable to LC, and the number of YLGs if death due to LC was averted by LCS.ResultsIn Brazil, the eligible population for LCS was 27 280 920 ever-smokers aged 50 to 80 years (13 387 552 female [49.1%]; 13 249 531 [48.6%] aged 50-60 years; 394 994 Asian or Indigenous [1.4%]; 3 111 676 Black [11.4%]; 10 942 640 Pardo [40.1%]; 12 830 904 White [47.0%]; 12 428 536 [45.6%] with an incomplete middle school education; and 12 860 132 [47.1%] living in the Southeast region); 5 144 322 individuals met the USPSTF 2013 criteria for LCS (2 090 636 female [40.6%]; 2 290 219 [44.5%] aged 61-70 years; 66 430 Asian or Indigenous [1.3%]; 491 527 Black [9.6%]; 2 073 836 Pardo [40.3%]; 2 512 529 [48.8%] White; 2 436 221 [47.4%] with an incomplete middle school education; and 2 577 300 [50.1%] living in the Southeast region), and 8 380 279 individuals met the USPSTF 2021 LCS criteria (3 507 760 female [41.9%]; 4 352 740 [51.9%] aged 50-60 years; 119 925 Asian or Indigenous [1.4%]; 839 171 Black [10.0%]; 3 330 497 Pardo [39.7%]; 4 090 687 [48.8%] White; 4 022 784 [48.0%] with an incomplete middle school education; and 4 162 070 [49.7%] living in the Southeast region). The number needed to screen to prevent 1 death was 177 individuals according to the USPSTF 2013 criteria and 242 individuals according to the USPSTF 2021 criteria. The YLG was 23 for all ever-smokers, 19 for the USPSTF 2013 criteria, and 21 for the USPSTF 2021 criteria. Being Black, having less than a high school education, and living in the North and Northeast regions were associated with increased 5-year risk of LC death.Conclusions and RelevanceIn this comparative effectiveness study, USPSTF 2021 criteria were better than USPSTF 2013 in reducing disparities in LC death rates. Nonetheless, the risk of LC death remained unequal, and these results underscore the importance of identifying an appropriate approach for high-risk populations for LCS, considering the local epidemiological context.

Abstract:
Mammalian embryos are thought to cleave equally during early development. Therefore, the contents including organelles such as mitochondria (mt) in oocytes are supposed to be delivered symmetrically into each blastomere of the resulting embryo. However, a recent report showed higher oxygen consumption, ATP production, and mt distribution in trophectoderms from mouse blastocysts than that in inner cell masses (Houghton 2006 Differentiation 74, 11–18). We hypothesized that this phenomenon could be preceded by the asymmetrical mt distribution into blastomeres at earlier stages. Oocytes, 2-cell embryos, and 4-cell embryos from BDF1 were measured to determine volumes, ATP contents, and mtDNA copy numbers in the whole egg and individual blastomeres that had been separated in Ca/Mg-free KSOM with added 0.02% EDTA. The volumes of blastomeres were calculated by the sphere volume formula. The ATP contents were determined using a luminometer and commercial kits (BacTiter-Glo, Promega, Madison, WI, USA). The mtDNA copy numbers were quantified absolutely using real-time PCR. The data was analyzed by 1-way ANOVA followed by Fisher's least significant difference test. We found about a 10% difference in volume between the largest and the smallest blastomeres from a 4-cell embryo. At the 4-cell stage, the largest blastomeres were significantly larger than the middle-sized and smallest blastomeres, and the next largest blastomeres were significantly larger than the smallest blastomeres (largest: 41 093 µm3, next largest: 39 671 µm3, middle-sized: 37 725 µm3, smallest: 36 518 µm3; n = 25). The procedure for separation of blastomeres did not significantly reduce theATP contents of embryos. We observed the same pattern of significant differences in theATP contents among the 4 blastomeres at the 4-cell stage (largest: 0.141 pmol, next largest: 0.124 pmol, middle-sized: 0.112 pmol, smallest: 0.098 pmol, n = 27). The largest blastomeres from 4-cell embryos contained about 1.4-fold higherATP than the smallest blastomeres. There was no significant difference in the mtDNA copy numbers in oocytes (n = 7), whole 2-cell embryos (n = 6), whole 4-cell embryos (n = 13), the sum of 2 blastomeres from the same 2-cell embryo (n = 6), and the sum of 4 blastomeres from the same 4-cell embryo (n = 6; 245 071 � 22 696, 267 567 � 30 989, 262 931 � 12 952, 239 717 � 16 813, 247 012 � 16 166, respectively; � SEM). However, the larger blastomere of 2-cell embryo had a higher mtDNA copy number than the smaller blastomeres (largest: 137 100 � 11 493, smallest: 102 617 � 6205; P < 0.05). The largest blastomeres of 4-cell embryo had about 1.4 times (P < 0.05) higher mtDNA copy numbers than the smallest blastomeres (largest: 72 072 � 4112, next largest: 66 198 � 5767, middle-sized: 57 596 � 3922, smallest: 51 146 � 3081). We found differences in volumes, ATP contents, and mtDNA copy numbers among blastomeres from the same embryos at the 4-cell stage. These qualitative differences could be related to embryonic metabolism in mouse early development.

Abstract:
4002 Background: Cetuximab prolongs survival in pretreated advanced colorectal cancer (CRC) patients (pts). For these pts, the effect of palliative treatment on quality of life (QOL) is also an important outcome. Methods: CO.17 was a randomized controlled trial (n=572) of cetuximab combined with best supportive care (BSC) vs BSC alone in pts with pretreated epidermal growth factor receptor-positive advanced CRC. Primary endpoint was survival. QOL was a secondary endpoint using the EORTC QLQ-C30 at baseline, 4, 8, 16, and 24 weeks (wks). Primary endpoint for QOL compared physical function (PF) and global health status (Global) mean changes from baseline to 8 and 16 wks using Wilcoxon test. Negative change scores denote worsening QOL. The proportion of pts in either arm with at least a 10 unit deterioration at 8 and 16 wks from baseline was compared using Fischer’s exact test. Kaplan-Meier estimates and logrank tests on median time to definitive QOL deterioration, defined as at least a 10 unit deterioration in PF or Global, were evaluated. Exploratory analyses of all other scale and symptom change scores at 8 and 16 wks were also performed. Results: Compliance rates for QOL questionnaires was high at baseline, 94% each arm, but did drop off over time: 73% vs 60% at week 8 and 60% vs 39% at week 16 for cetuximab vs BSC, respectively. PF change scores at 8 wks were -3.9 for cetuximab vs -8.6 for BSC (p=.046); at 16 wks -5.9 vs -12.5 (p=.027). Global change scores were -0.5 vs -7.1 (p=.008) and -3.6 vs -15.2 (p=.008) at 8 and 16 wks respectively. The proportion of pts with deteriorating PF was 25% for cetuximab vs 35% for BSC at 8 wks (p=.051) and 30 vs 43% at 16 wks (p=.069). Deteriorating Global was seen in 23 vs 38% at 8 wks (p=.004) and 31 vs 49% at 16 wks (p=.0011). Time to PF deterioration was 5.4 months [95% CI 3.8–5.7] for cetuximab vs 3.7 months [2.0–3.9] for BSC (p=.022). Time to Global deterioration was 5.4 [3.9–5.7] vs 3.7 months [2.1–3.9] (p=.062). Exploratory analyses of all other scale and symptom change scores at 8 and 16 wks showed all differences of p<.05 also favored cetuximab. Conclusions: Cetuximab provides palliation in pretreated pts with advanced CRC, delaying deterioration in QOL as well as improving survival. [Table: see text]

Abstract:
The effects of tetrakis(1-methylpyridinium-4-yl)porphyrinatomercury(II) [ Hg ( TMPyP )]4+ (6) along with [ Pb ( TMPyP )]4+ (7), [ Cd ( TMPyP )]4+ (8), and [ H 2( TMPyP )]4+ (1) (free base porphyrin) on DNA cleavage by Haemophilus aegyptius (HaeIII) have been studied using gel electrophoresis, UV-vis absorption spectroscopy, and circular dichroism (CD) spectroscopy. The gel electrophoresis results indicate that in the absence of 6, HaeIII restriction enzyme could not cleave plasmid DNA at a low concentration of 0.2 units μl-1 after 1 h incubation at 37 °C. However, DNA cleavage occurred in the presence of 6 at the concentrations of 1.0 × 10-6 and 1.0 × 10-7 M and 0.2 units μl-1 of HaeIII under the same conditions. In addition, acceleration of DNA cleavage occurred at a higher concentration of HaeIII (0.4 units μl-1) in the presence of a wider concentration range of 6 from 1.0 × 10-5 to 1.0 × 10-7 M . 8, 7, and 1 could not enhance DNA cleavage in the presence of HaeIII (0.2 units μl-1). However, when the concentration of HaeIII was increased to 0.4 units μl-1, only 8 inhibited DNA cleavage by HaeIII at higher concentrations (1.0 × 10-5-10-6 M ) whereas 6, 7, and 1 did not. The UV-vis and CD spectroscopic results indicate that these porphyrins interact differently with DNA based on their binding modes. 6 enhanced DNA cleavage due to the synergistic effect of the Hg 2+ ions and the free base porphyrin 1 that dissociated from the mercury(II) porphyrin complex upon binding to DNA which resulted in the enhanced transformation of the DNA conformation from the original B-form to a Z-like conformation, while 8 inhibited HaeIII activity at higher concentrations and 7 and 1 neither enhanced nor inhibited DNA cleavage. A mechanism for this phenomenon is suggested.

Abstract:
Introduction: Little is known about prevailing ribotypes of Clostridium difficile infection in Romania where CDI is not a mandatory notifiable disease. Methodology: We studied 64 non-duplicate C. difficile isolates from patients hospitalised at the National Institute of Infectious Diseases, Bucharest, Romania between March 2011 and March 2012. Results: Sixty-three of the 64 C. difficile isolates produced toxins A and B whereas 44 (69%) isolates produced a binary toxin. Ribotype 027 accounted for 43 (68%) of the 63 toxigenic strains. The remaining 20 isolates belonged to ribotypes 018 (n = 9), 012 (n = 3), and, with one isolate each, 014, 031, 081, 416, 433, 500, 507 and PR03035 (new ribotype). Information on hospital mortality was available for 62 of the 64 patients; among these 62 cases, 4 (6.4%) ended fatal. Recurrence was documented for 11 (18.3%) of the 60 patients for whom this information was available. Multilocus variable-number tandem repeat analysis of the 43 isolates of ribotype 027 yielded a unique cluster for the Romanian isolates when compared to Austrian or Italian isolates. Conclusion: Our findings sustain the hypothesis of a recent emerged outbreak of C. difficile PCR ribotype 027 infections in the area of Bucharest.

Abstract:
The aim of this study was to characterize C. difficile isolates from the farm, abattoir, and retail outlets in Ireland in terms of ribotype and antibiotic resistance (vancomycin, erythromycin, metronidazole, moxifloxacin, clindamycin, and rifampicin) using PCR and E-test methods, respectively. The most common ribotype in all stages of the food chain (including retail foods) was 078 and a variant (RT078/4). Less commonly reported (014/0, 002/1, 049, and 205) and novel (RT530, 547, and 683) ribotypes were also detected, but at lower frequencies. Approximately 72% (26/36 tested) of the isolates tested were resistant to at least one antibiotic, with the majority of these (65%; 17/26) displaying a multi-drug (three to five antibiotics) resistant phenotype. It was concluded that ribotype 078, a hypervirulent strain commonly associated with C. difficile infection (CDI) in Ireland, was the most frequent ribotype along the food chain, resistance to clinically important antibiotics was common in C. difficile food chain isolates, and there was no relationship between ribotype and antibiotic resistance profile.

The results of the qualitative composition analysis of the dried biomass extracts of in vitro callus, cell suspension, and root cultures show that the main biologically active substances (BAS) in the medicinal plant, Rhodiola rosea, are 6-C-(1-(4-hydroxyphenyl)ethyl)aromadendrin (25 mg, yield 0.21%), 2-(3,7-dihydroxy-2-(2-hydroxypropan-2-yl)-2,3-dihydrobenzofuran-5-yl)-6,7-dihydroxychroman-4-one (23 mg, yield 0.2%), 2-(3,4-dimethoxyphenyl)-5,7-dimethoxychroman-4-one (175 mg, yield 1.5%), 5,7-dihydroxy-2-(4-hydroxy-3-(2-(4-hydroxyphenyl)-4-oxo-4H-chromen-6-yl)phenyl)-4H-chromen-4-one (45 mg, yield 0.5%), 5,6,7,8-tetrahydroxy-4-methoxyflavone (0.35 mg, 0.5%). Abstract:
Abstract Background Clostridioides difficile is an organism acquired not only in healthcare settings but also in community settings. For the past several years our hospital has performed screening tests to detect asymptomatic carriage of C. difficile. We now aim to better understand the ribotypes and degree of diversity among these C. difficile strains obtained in a systematic screening. Methods This study was performed at a 600 bed teaching affiliated hospital in Milwaukee, WI, where surveillance testing is performed in selected units upon admission and weekly thereafter using nucleic acid amplification test (NAAT; Xpert® C. difficile; Cepheid, Sunnyvale, CA). Screening tests are obtained regardless of symptoms. NAAT positive samples underwent anaerobic cultures in C. difficile selective broth (CCMB-TAL) for 24–48 hr and then to Brucella blood agar plates (BA) for 48–72 hr to confirm C. difficile presence. PCR-ribotyping was performed as previously described by Stubbs et al. with minor modifications. The results were compared with a database containing >3,000 clinical isolates including C. difficile reference strains from the Cardiff ribotype collection. Results A total of 104 strains belonging to 93 unique patients were processed. Patients had a mean age of 60 years (range: 18 - 89) and 49% were females. Most patients were hospitalized in the hematology oncology units (55.7%) or in the solid-organ transplant step down unit (9.6%). A total of 25 different ribotypes were identified. The most common ribotype was 014/020 (23; 22%), followed by ribotype 56 (13; 12.5%), 106 (12; 11.5%), 027 (11; 10.5%), 002 (6; 5.7%), and 078/126 (5; 4.81). Regarding the timing of admission, 70 strains were obtained within 4 days from hospital admission and the remaining 34 were obtained afterwards. Both groups had ribotype 014/020 as the most frequently detected ribotype but 027 were the second most commonly detected at the time of admission (table). A total of 7 patients had more than one stool sample processed and 5 of them had discordant ribotypes at different time points. Conclusion Systematic screening tests for C. difficile carriage in a single center showed a large heterogeneity of ribotypes with the majority not being 027. Abstract Introduction In Colombia Breast cancer (BC), is the most frequent and has the highest mortality rate among all types of cancer. There are few studies of the genomic profile in unselected affected population by BC in Colombia. Some of these studies have only tested the presence of variants reported as founders named “Colombian Profile”.We conducted a large-scale genomic analysis using Whole Exome Sequencing (WES) to evaluate germline mutations in unselected BC patients. Methods This trial included 299 unselected BC female patients aged over 18 years old, without personal and family history of germline BC risk mutations.The protocol was approved by the IRC and EC of Fundación Cardioinfantil (FCI). All patients signed informed consent before recruitment.Genomic DNA was extracted from peripheral blood samples and was used to WES (Novogene Inc. Beijing, China). The variants were filtered using VarSeq v2.1.1 software, following the criteria: missense, non-sense, frameshift, and intronic variants, we additionally considered a MAF ≤0.01 for ATM, CHEK2, and PALB2 genes.Clinical significance of each variant was annotated according to the ACMG/AMP and ENIGMA guidelines.MLPA was assessed using the commercial kit SALSA MLPA Probemix P002-D1 for BRCA1 and P090-C1 for BRCA2 (MRC-Holland, Amsterdam).This study was financially supported by an unrestricted grant from Pfizer. Results This abstract is the first report from 299 patients. To determine the presence of germline variants in the patients a WES was performed. Here we describe the pathogenic and probably pathogenic mutations in BRCA1, BRCA2, ATM, CHEK2, PALB2. We found BRCA1/2 alterations were found in 3.7% of the patients (11 patients, IC 95% 1.7-5.6%), 5 patients in BRCA1 and 6 patients in BRCA2 (1.7% IC 95% 0.7-4%, and 2% IC 95% 0.9-4.4% respectively). We found 29 patients had mutations unrelated to BRCA1/2 (9.5% IC 95% 5.8-11.7%). The most frequently affected gene was ATM (17 patients, 5.7% IC95% 3.6-9%). Discussion and conclusion We found that 12.2% of the population of the study were carriers of a pathogenic/likely pathogenic variant in the evaluated genes, and interestingly 9.5% of them corresponded to non-BRCA1/2 genes. ATM variants have a prevalence of 5.7% in the whole population and represent 42% of all the variants. Other mutations in genes like BRCA2, ATM and CHEK2 were exclusive in non-TNBC. Meanwhile, BRCA1 and PALB2 mutations had higher frequencies in TNBC. We identified five novel mutations.We demonstrate that LGRs are not an important molecular cause in non-hereditary cases of BC.27% of the carriers of mutations in BRCA1/2 did not fulfilled NCCN criteria and 82% of the mutations are not described in “Colombian Profile”. These findings demonstrate the particular genetic profile in an unselected population with breast cancer, and this highlights the importance of WES as a molecular diagnostic tool. We think that universal germline testing in cancer should be considered. Polycrystalline Ca3-xMgxCo4O9(x=0-0.3)ceramics were prepared by the sol–gel method combined with the ordinary pressing sintering and the thermal conductivities were measured from room temperature to 673 K. The influence of Mg2+ substitution for Ca2+ on the thermal conductivities of Ca3Co4O9 ceramics was investigated systematically. The influence of Mg doping on the thermal conductivities is mainly embodied in the lattice thermal conductivities, which shows a significant decrease with the increase of the dopant content for the samples with x ≤ 0.2, while the carrier thermal conductivity had no obvious change with Mg doping increasing. These results indicated that the thermal conductivities of the material could be reduced remarkably with the substitution of Mg from 1.427 W/m·K to 0.731 W/m·K at 573 K with x = 0.2.

Syntheses and room-temperature single crystal X-ray structure determinations are recorded for adducts [M(EPh3)4] (ClO4), M = Cu, E = As (1), Sb (2); M = Ag, E = As (3), Sb (4), enabling comparison with analogous, previously studied MP4 and MN4 environments. The four complexes so characterized all crystallize in the familiar rhombohedral R-3 array (a ≈ 14·4, c ≈ 52 Å, hexagonal setting) with the metal and chlorine atoms located on crystallographic threefold axes. In these arrays Cu–E (axial, off axis) are 2·493(2), 2·533(1) for (1), and 2·572(1), 2·577(1) Å for (2), the disparity in the two independent M-E distances being diminished relative to those of previously studied phosphorus analogues, perhaps in consequence of increasing E size. In (3) and (4), some cation disorder is evident in respect of the ligand pnictide atom (E); for the major components Ag-E are 2·652(3), 2·680(2) in (3); 2·732(2), 2·7295(8) Å in (4). Problems associated in defining the metal atom environment geometries in the latter are, nevertheless, more satisfactorily overcome in two further determinations of nitrate salts: [Ag(SbPh3)4] (NO3) (5) is of the rhombohedral family with no cation core disorder, Ag–E (axial, off axis) being 2·720(4), 2·725(2) Å, while the structural characterization of a 1 : 4 triphenylarsine adduct of silver nitrate is recorded as its tetraethanol solvate, namely [Ag(AsPh3)4] (NO3).~ 4C2H5OH(6), monoclinic, C2/c, a 18·373(4), b 20·786(5), c 21·070(8) Å, β 108·18(3)°, Z = 4 f.u. The silver atom of the [Ag(AsPh3)4]+cation unusually lies on a crystallographic 2 (rather than a 3 ) axis of the incipiently23 (T) array, with Ag-As 2·649(2), 2·650(2) Å and As-Ag-As ranging from 108·22(4) to 111·41(6)°. The structure determination of a chloroform solvate of (2), denoted (2a), of low precision, is noted, together with the structure of an interesting artefact, isomorphous with the rhombohedral oxoanion array, modelled as [Ag(PPh3)4]2 (SiF6).py (7). Bands in the far-infrared spectra of [M(AsPh3)4] (ClO4) are assigned to v(MAs) vibrations at 119 (M = Cu) and 103 cm¯1 (M = Ag); this produces a more consistent picture of the variation in the wavenumbers of the v(ME) modes in [M(EPh3)4]+ compounds than was available previously.

Abstract Introduction Youth is categorized as combination of adolescence and young adulthood. Young people are prone to injuries, because this period involves essential changes in many aspects of life within a complicated physiological and mental developmental age. Our aim was to investigate the features of burn injuries in youth. Methods Subjects were 622 adolescents (10-18yrs, n=313) and young adults (19-24yrs, n=309) who were treated at our burn-center from 2010 to 2021. Data collected for each case were age, sex, social-security status, occupation, marital status, scene of injury, burn extent, burn cause, history of injury, affected major body sites, and need for inpatient-care (median±SD,mean±SD)(p&lt;.05). Results Median age was 18yrs±4,76. Male to female ratio was 0,66:1 with female predominance especially among young adults (0,48:1)(p˂.05). Most subjects were in civil social-security-system (n=612, 98,4%); most were students (n=475, 76,4%); a total of 103 subjects were in labour-force(16,5%); 44 were unemployed(7,1%). Most young people were single (n=600, 96,4%); 4 subjects in adolescent group and 18 subjects in young adult group were married(0,64% and %2,9 respectively); 15 of married cases were female(68,2%)(p&lt; .05). Injuries occurred mostly at home (n=411, 66,1%). Mean total body surface area (TBSA) burned was 3,21 % ± 7,5(min:0,2 max:75). The most commonly affected body site was the hand (n=165, 26,5%). Leading burn cause was scalds (n=433, 69,6%). Female subjects mostly suffered from scalds with mean TBSA burned of 2,0%±2,21 (min:0,2 max:18)(n=297, 79,2%). However, vast majority of flame burn victims were male (n=41, 82,0%) (mean TBSA burned: 12,8%±21,35, min:0,2 max: 75) and almost all severe electrical injuries happened to male subjects (n=13, 86,7%)(mean TBSA burned:14,2±14,81, min:0,2 max:40)(p˂.05). All cases were preventable accidents; the unique instance for ‘substance-abuse related burns’ was butane-lighter-liquid burns in 9 cases(1,4%). Inpatient-care was needed for 52(8,2%) victims. Mean TBSA burned for inpatients was 13,5%±19,6(min:1, max:75). All subjects survived. Conclusions Our results suggest that young female subjects are prone to burn injuries, but severe injuries happen to male and there are many other aspects that should be considered. Combined evaluation of adolescents and young adults may provide purposive data for burn repositories.

Abstract This study evaluated the efficacy and safety of single-agent bortezomib in indolent B-cell lymphoma that had relapsed from or was refractory to rituximab. Sixty patients enrolled: 59 were treated with bortezomib 1.3 mg/m2 on days 1, 4, 8, and 11 for up to eight 21-day cycles; responders could receive 4 additional cycles; maintenance was optional. Fifty-three evaluable patients completed more than 2 cycles. The median age was 70 years, 53% female, Ann Arbor stage III-IIIE (28%) and IV (65%); 43 patients (72%) had more than 2 prior regimens; and 6 patients went on to maintenance. Overall responses are as follows: 1 complete response (1.9%), 3 unconfirmed complete response (5.7%), 3 partial response (5.7%), 34 stable disease (64.2%), and 12 progressive disease (22.6%). Median time to response = 2.2 months (range, 1.2-5.3 months); duration of response = 7.9 months (2.8-21.3 months); 1-year survival was 73% and 2-year survival was 58%; median survival = 27.7 months (range, 1.4-30.9 months); median progression-free survival = 5.1 months (range, 0.2-27.7 months), median time to progression = 5.1 months (range, 0.2-27.7 months), and median event-free survival = 1.8 months (range, 0.2-27.7 months). Treatment-related grade 3 or 4 adverse events included: thrombocytopenia (20%), fatigue (10%), neutropenia (8.5%), and neuropathy and diarrhea (6.8% each). This study demonstrates that bortezomib has modest activity against marginal zone and follicular lymphoma; it has the potential for combination with other agents in low-grade lymphomas. Maintenance therapy should be explored further.

Hasil evaluasi sediaan untuk 4 formula yaitu uji organoleptis semua sediaan tidak mengalami perubahan selama penyimpanan 6 minggu, sediaan yang homogen, PH sama dengan rentang pH kulit (4,5-5,7), termasuk kedalam tipe krim M/A, mudah tercuci oleh air, memiliki ukuran partikel yang dapat memenuhi syarat ukuran partikel 0,2-50 µm, Stabil secara fisik selama penyimpanan pada suhu yang berbeda dan memiliki nilai viskositas yang memenuhi syarat SNI yaitu 2000- 5000 cP. Uji aktivitas antioksidanekstrak daun katuk 50,62 µg/mL, dan masing masing formula krim body scrub yaitu F0 102, 43 µg/mL, F1 88,88 µg/mL, 82,53 µg/mL dan 80,50 µg/mL. Kesimpulan pada penelitiaan ini adalah ekstrak daun katuk (Sauropus androgynus (L.) dapat dijadikan sediaan krim dengan aktivitas antioksidan F1, F2, F3 dengan kriteria antioksidan kuat. Abstract Katuk leaves (Sauropus androgynus (L.) Merr.) are known to contain flavanoids which have the potential to be a source of antioxidants, which are quite strong. The purpose of this study was to formulate katuk leaf extract in the form of body scrub cream using 4 formulas, namely F0 (not using extract), F1 (0.08%), F2 (0.17%) and F3 (0.24%) and determine the antioxidant activity of the body scrub cream of katuk leaf extract (Sauropus androgynus (L.) usingthe DPPH method. The evaluation results for the 4 formulas, namely the organoleptic test for all preparations did not change during 6 weeks of storage, the preparations were homogeneous, the pH was the same as the range Skin pH (4.5-5.7), belongs to the type of cream M/A, easily washed off by water, has a particle size that can meet the requirements of particle size 0.2-50 µm, Physically stable during storage at different temperatures and has a viscosityvalue that meets SNI requirements, namely 2000-5000 cP.The antioxidant activity test of katuk leaf extract is 50.62 µg/mL, and each body scrub cream formula is F0 102, 43 µg/mL, F1 88.88 µg/mL , 82.53 µg/mL and 80.50 µg/mL.The conclusion of this research is katuk leaf extract (Sauropus androgynus (L.) can be used as cream preparations with antioxidant activity F1, F2, F3 with strong antioxidant criteria.

Abstract:
Background:Patients with psoriatic arthritis (PsA) require long-term treatment, which may lead to adverse events (AEs). Ixekizumab, an interleukin-17A antagonist, is approved for the treatment of adults with active PsA.Objectives:We report a summary of safety outcomes for patients enrolled in 4 PsA studies with up to 3 years of exposure to ixekizumab.Methods:This integrated safety analysis included all patients with PsA who received at least 1 dose of ixekizumab (80 mg every 2 or 4 weeks) in 4 clinical trials (NCT01695239, NCT02349295, NCT02584855, NCT03151551). Safety outcomes included treatment-emergent adverse events (TEAEs), serious AEs (SAEs), discontinuations due to AEs, deaths, and AEs of special interest.Results:A total of 1401 patients were included in this safety analysis (51.5% female; mean age 49 years), with 2247.6 patient-years of exposure (Table 1). In all, 1131 patients (80.7%) reported ≥1 TEAE (exposure-adjusted incidence rate per 100 patient-years [IR] 50.3, 95% CI 47.5–53.3), mostly mild (32.9%) or moderate (39.7%) in severity. The most common TEAEs were nasopharyngitis (n=202, IR 9.0), upper respiratory infections (n=186, IR 8.3), and injection site reaction (n=156, IR 6.9). SAEs were reported by 134 patients (IR 6.0, 95% CI 5.0–7.1). 115 (8.2%) patients discontinued due to AEs (IR 5.1, 95% CI 4.3–6.1). Six deaths were reported (IR 0.3, 95% CI 0.1–0.6). Allergic reactions/hypersensitivity were reported in 102 patients (IR 4.5, 95% CI 3.7–5.5). Three cases were adjudicated as de novo inflammatory bowel disease (IR 0.13, 95% CI 0.04–0.41); 1 was ulcerative colitis (IR 0.04, 95% CI 0.01–0.32), 2 were Crohn’s disease (IR 0.09, 95% CI 0.02–0.36). Major adverse cardiac events occurred in 12 patients (IR 0.5) and malignancies in 15 (IR 0.7), 9 of which were non-melanoma skin cancer. Opportunistic infections occurred in 40 (2.9%) patients (IR 1.8, 95% CI 1.3–2.4). Candidiasis occurred in 24 patients (oral: IR 0.7, 95% CI 0.4–1.2; oral fungal infection: IR 0.3, 95% CI 0.1–0.6; esophageal infection: IR 0.1, 95% CI 0.0–0.4). No active or reactive cases of tuberculosis were reported. Other opportunistic infections included hepatitis B (IR 0.0, 95% CI 0.0–0.3), herpes simplex (IR 1.8, 95% CI 1.3–2.5), and herpes zoster (IR 0.7, 95% CI 0.4–1.2).Conclusion:The safety profile of ixekizumab across 4 clinical trials and up to 3 years of continuous treatment in patients with active PsA was consistent with the known safety profile reported in previous studies for psoriasis and PsA. Abstract:
In the present study, surface-enhanced resonance Raman scattering (SERRS) spectra of α,β,γ,δ-tetra-(4-trimethyl ammonium phenyl) porphyrin [T(4-TAP)P] were obtained. With increasing pH, the relative intensities of the bands at 890 and 1244 cm−1 decreased. These bands were attributed to γ(C-H) and δ(Cm-phenyl), respectively. The bands at 420 and 576 cm−1, which were assigned to γ(phenyl-ring), were enhanced. The molecular symmetry of T(4-TAP)P is discussed in terms of group theory. The bands at 1554 and 1496 cm−1 could be attributed to the vibrational modes of the porphyrin macrocycle; the bands at 1460, 1362, and 1330 cm−1 were assigned to ν(C-C) + δ(C-H), ν(C-N) + δ(C-H), and ν(C-N), respectively. All these bands change in band intensities, positions and widths, with the potential changing from +0.2 V to −0.2 V. It was concluded that the adsorbed porphyrins underwent partial incorporation with Ag from the electrode, and the adsorbate assumed a flat orientation on the silver electrode as well as the Ag colloid surface.

Abstract:
Background. Mirizzi syndrome (MS) is a relatively rare complication of cholelithiasis, which occurs in a wide range from 0.2 to 5% according to different authors. Today, the surgical treatment of MS remains a challenge.Aim of study. To determine the possibilities of laparoscopic interventions in the surgical correction of various types of MS.Material AND methods. The work is based on a prospective analysis of cases of laparoscopic treatment of 19 patients with MS who were treated in the emergency surgery departments of the Republican Scientific Center for Emergency Medical Care in 2017–2019. This is 22.9% of all admitted patients (83) with MS during this period. Type 1 MS was diagnosed in 3 patients (15.7%), type 2 was diagnosed in 16 patients (84.2%).Results. In all cases of type 1 MS, laparoscopic cholecystectomy was performed. A new method for the correction of type 2 MS by forming a sleeve from the gallbladder wall was suggested, which was performed in 10 patients with good results. In the postoperative period, no nonspecific complications were observed in patients undergoing laparoscopic interventions. One patient had residual choledocholithiasis, which was managed by day 5 after the surgery with endoscopic retrograde cholangiopancreatography and endoscopic papillosphincterotomy and the removal of the calculus from the common bile duct. The average duration of stay of patients in a hospital bed was 10.8 days. Fatal outcome was observed in 1 case (5.3%).Conclusion. 1. The inclusion of endoscopic retrograde cholangiopancreatography, percutaneous transhepatic cholangiography and magnetic resonance imaging cholangiography in the diagnostic scheme improves the accuracy and quality of recognition of Mirizzi syndrome and allows the indications for the use of video laparoscopy to be evaluated. 2. Indication for laparocopic treatment of type 2 Mirizzi syndrome is the presence of a single calculus in the supraduodenal part of the common bile duct, which makes it possible to reduce the number of conversion to laparotomy. 3. In cases of type 1 Mirizzi syndrome, the operation of choice is laparoscopic cholecystectomy. 4. The operation of choice in patients with type 2 Mirizzi syndrome is laparoscopic subtotal cholecystectomy, fistula plasty with a gallbladder flap on the Kehr’s T-tube drain with the formation of a “cystic duct”-type sleeve.

The equilibrium reactions between aluminium(III) and an epicatechin polymer or tannin (T) have been studied by potentiometric titration at 25�C, I = 0.001 M. The tannin, which has on average ten 1,2-dihydroxybenzene units per molecule, encapsulates the aluminium ion to form a bis(l,2-diphenolato) complex at pH 5-7. Stability constants are reported for Al(TH-2) (logK11-2, -3.95 � 0.05), Al(TH-3) (logK11-3, -4.5 � 0.1) and Al(TH-4) (logKl1-4, -5.7 � 0.2). A small chelate effect arising from coordination of remote pairs of 1,2-diphenolato groups was observed.

Abstract:
Adam Mickiewicz’s poetry attracted many composers, but it left the most noticeable trace in the work of Stanisław Moniuszko, who turned to lyrical poems, ballads, sonnets, as well as excerpts from the poems Konrad Wallenrod and Dziady. Fragments of them were also used by the composer in romances included in various issues of his Home Songbooks (Śpiewnik domowy), and as a result, the texts of Mickiewicz accompanied Moniuszko from the very first works to those published posthumously. The composer’s works based on Mickiewicz’s poems, like the original source itself, represent completely different forms and genres, but they are all in the halo of Polish romantic creativity and one aesthetic and patriotic concept. The purpose of the work is to consider various ways of implementing the ballad genre and the genre of musical portrait in Stanisław Moniuszko’s chamber-vocal works based on Adam Mickiewicz’s poems. Among Moniuszko’s numerous appeals to Mickiewicz’s poetry (more than 20 works), the sixth issue in the series of Home Songbooks, written entirely on the poet’s texts, stands out. A special place in it is occupied by the ballad Fish (Rybka, 1859) based on the text of the poet’s youth work of the same name, representing the composer’s chamber vocal style in the most perfect form and written with a truly symphonic scope. The dynamic musical narrative follows all the twists and turns of the plot, its dramaturgy is based on the comparison of contrasting images, direct speech, dialogues of the characters, and replicas from the author, embodying the specifics of the poetic original. The influence of ideological and aesthetic views associated primarily with folklore and patriotic themes, the poetics and topos of Mickiewicz manifested itself in many ways in Moniuszko’s chamber-vocal works, which allows us to draw conclusions about the synthesis of meanings and symbols characteristic of romantic art.

517 Background: Neoadjuvant endocrine therapy has shown efficacy in hormone-responsive postmenopausal breast cancer patients. We aimed to assess the efficacy and safety of cytotoxic chemotherapy versus endocrine therapy for hormone-responsive lymph node-positive premenopausal breast cancer patients in a neoadjuvant setting. (NCT01622361). Methods: In this phase 3, randomized, double blind, parallel group, multicenter study, we enrolled premenopausal women with estrogen receptor (ER)-positive, HER2-negative, and lymph node-positive premenopausal breast cancer patients. Patients were randomized to either 24 weeks of neoadjuvant chemotherapy with adriamycin plus cyclophosphamide (AC) followed by taxane (T) or neoadjuvant endocrine therapy with zoladex and tamoxifen. Results: 187 patients were randomly assigned to chemotherapy (n=95) or endocrine therapy (n=92), and 174 patients completed the 24 week neoadjuvant treatment period (n=87, both). More patients in the chemotherapy group had a complete or partial response than did those in endocrine therapy arm on both caliper (chemotherapy 83.9% vs endocrine therapy 71.3%, OR 0.476 95% CI 0.228 to 0.994) and MRI (chemotherapy 83.7% vs endocrine therapy 52.9%, OR 0.219, 95% CI 0.107 to 0.447). Three patient on chemotherapy group (3.4%) and 1 patients (1.15%) on endocrine treatment group showed complete pathologic response. In the patients who had breast cancer with low Ki 67 expression (<20%) on initial biopsy, clinical response on caliper were shown similar on both treatment group (HR 0.958, 95%CI 0.296 to 3.101). Five patients who had no tumor on the breast or lymph node after 24 week neoadjuvant endocrine therapy had higher ER score (all allred score >6), all low grade (1or 2) low Ki 67(≤20%) expression (4/5 patients) on initial biopsy specimen. Conclusions: In premenopausal breast cancer patients, 24 weeks neoadjuvant chemotherapy showed better clinical response than endocrine therapy. Low Ki 67 expression could be a parameter of the endocrine treatment. Clinical trial information: NCT01622361.

Three experiments investigated effects of jejunal lipid infusions given on 4 or 21 consecutive days in adult, male Sprague-Dawley rats. In experiment 1, 7-h infusions of linoleic or oleic acid (0.2 ml/h for 7 h; total load = 11.5 kcal) on 4 consecutive days reduced total intake (ad libitum consumption of the liquid diet Boost, Mead Johnson, plus load) by ∼15% and decreased weight gain compared with 4-day tests with saline administration. In experiment 2, linoleic acid at 0.1 ml/h for 7 h (5.7 kcal) was ineffective, whereas the same load delivered in 3.5 h produced effects similar in magnitude to those in the first experiment. In experiment 3, jejunal infusions of linoleic acid (0.2 ml/h for 7 h) on 21 consecutive days reduced mean total intake by 16%, body weight by 10%, and carcass fat by 48% compared with controls receiving saline. The net decrease in caloric intake may reflect the combined activation of pre- and postabsorptive mechanisms, and it suggests a possible treatment for obesity.

ABSTRACT With the Australian Square Kilometre Array Pathfinder (ASKAP) we monitored the black hole candidate X-ray binary MAXI J1535–571 over seven epochs from 2017 September 21 to October 2. Using ASKAP observations, we studied the H i absorption spectrum from gas clouds along the line of sight and thereby constrained the distance to the source. The maximum negative radial velocities measured from the H i absorption spectra for MAXI J1535–571 and an extragalactic source in the same field of view are −69 ± 4 and −89 ± 4 km s−1, respectively. This rules out the far kinematic distance ($9.3^{+0.5}_{-0.6}$ kpc), giving a most likely distance of $4.1^{+0.6}_{-0.5}$ kpc, with a strong upper limit of the tangent point at $6.7^{+0.1}_{-0.2}$ kpc. At our preferred distance, the peak unabsorbed luminosity of MAXI J1535–571 was >78 per cent of the Eddington luminosity, and shows that the soft-to-hard spectral state transition occurred at the very low luminosity of (1.2–3.4) × 10−5 times the Eddington luminosity. Finally, this study highlights the capabilities of new wide-field radio telescopes to probe Galactic transient outbursts, by allowing us to observe both a target source and a background comparison source in a single telescope pointing.

Abstract:
El objetivo de esta investigación fue evaluar la mejor fuente de material vegetal para el aislamiento de protoplastos, suspensiones celulares o foliolos en tres genotipos de tomate con diferente grado de resistencia a Phytophthora infestans: Solanum habrochaites (503) resistente, S. lycopersicum (507) susceptible y el híbrido parcialmente resistente (508). Luego de lograr el establecimiento in vitro a partir de semilla certificada en los genotipos 503 y 507 y de estacas del genotipo 508, las hojas de las vitroplantas desarrolladas y multiplicadas se usaron para la inducción de callo. Se evaluaron cuatro combinaciones de reguladores de crecimiento y dos condiciones lumínicas por genotipo, encontrándose los porcentajes más altos de formación de callo en oscuridad. Las combinaciones de reguladores con mayor efecto sobre el crecimiento y friabilidad de los callos fueron BAP (1 ppm) + AIA (4 ppm) para el genotipo 503 y BAP (4 ppm) + AIA (1 ppm) para los genotipos 507 y 508. Estas combinaciones de reguladores fueron utilizadas para el establecimiento de las suspensiones celulares y el trazado de la curva de crecimiento. El aislamiento de los protoplastos a partir de suspensiones celulares se realizó en la fase exponencial, evaluando diferentes mezclas enzimáticas y tiempos de incubación; sin embargo, no se logró respuesta para ningún genotipo. A partir del mesófilo, se obtuvo 1,2 x 105 protoplastos/ml con alta viabilidad para el genotipo 503 (celulasa 0,6% y pectinasa 0,1%, 8 h de incubación); 1,9 x 105 protoplastos/ml para el genotipo 507 (celulasa 4% y pectinasa 0,4%, 8 h de incubación); y para el genotipo 508, 1,1 x 105 protoplastos/ml (celulasa 2% y pectinasa 0,2%, 7 h de incubación).

Abstract:
A new aromatic diamine, 10,10-bis[4-(4-amino-3-trifluoromethylphenoxy)phenyl]-9( 10H)-anthrone (BATFPA), has been synthesized from anthrone via three-step procedure. Low-temperature polycondensation of BATFPA with various aromatic dicarboxylic acid chlorides produced novel fluorinated polyamides with moderate to high inherent viscosities of 0.75–1.03 dL/g while the weight-average molecular weight and number-average molecular weight were in the range of 66,000–74,000 and 37,000–41,000, respectively. Nearly, all the polymers were readily soluble in polar solvents, such as N,N-dimethyl acetamide (DMAc), N-methyl-2-pyrrolidinone, pyridine, and tetrahydrofuran, and afforded transparent, light-colored, and flexible films upon casting from DMAc solvent. These polymers had glass transition temperatures of 249–309°C, 10% weight loss temperatures of 486–517°C, and char yields of 53–61% at 800°C under nitrogen. All polymers were amorphous and their films exhibited tensile strengths of 64–86 MPa, tensile moduli of 1.8–2.2 GPa, and elongation at break of 10–18%. These polymers had dielectric constants ranging from 3.25 to 3.76 (100 Hz), low moisture uptake in the range of 0.72–1.18%, and high transparency with an ultraviolet–visible absorption cutoff wavelength in the 360–378 nm range.

5077 Background: The standard chemotherapy for epithelial ovarian cancer (EOC) is carboplatin and paclitaxel every 3 weeks together with debulking surgery. This phase II trial was designed to determine the safety and efficacy of weekly carboplatin and paclitaxel treatment in patients with EOC. Methods: Between October 2003 to August 2005, 37 patients with stage Ic-IV epithelial ovarian, tubal or primary peritoneal carcinoma were enrolled into the study. Carboplatin at AUC=2 and paclitaxel at 80 mg/m2 were administered on days 1,8,15 of a 28-day cycle. Cytoreductive surgery was performed as primary treatment or after 3 cycles of neoadjuvant chemotherapy with additional chemotherapy after the surgery. Results: Median age of the patients was 67 (range 49–82). A mean of 6 chemotherapy cycles were administered (range 3–8). Median time of follow-up (from the beginning of chemotherapy until the last follow-up visit) was 15.57 months (range 0.2–26months). Thirty-three patients were evaluable for response. Complete response (CR) was observed in 26 patients (78.8%) and partial response (PR) in 7 (21.8%). By the time of data collection 13 out of 33 women (39.4%) experienced recurrent or persistent disease and one patient (3%) died from progressive disease during 2nd line chemotherapy. Since 20 out of 33 patients are still free of disease and all but one are still alive, it is too early to evaluate time to progression (TTP) and overall survival (OS). The median time to disease recurrence or progression after completion of primary chemotherapy was 7.5+ months (0.2–18.2+). As for toxicity; grade 3 and 4 neutropenia were seen in 5 (13.5%) and one patient (2.7%) respectively. There was no neutropenic fever. Other grade 3 and 4 hematologic toxicities were not observed. Six (16.2%) and 5 (13.5%) patients needed G-CSF and Epoetin support respectively. The main non-hematologic toxicities were alopecia (grade 1) and fatigue (grade 3 in two patients). Only two patients (5.4%) experienced grade 3 neuropathy. Conclusion: Weekly treatment with carboplatin and paclitaxel is feasible and well tolerated. The low toxicity rate especially regarding neuropathy warrants further investigation of this regimen. No significant financial relationships to disclose.

Abstract Background Chronic myelomonocytic leukemia (CMML) is a clonal hematologic disorder that was classified by the World Health Organization (WHO) as a myelodysplastic/ myeloproliferative overlap disease. Cytogenetic abnormalities have a significant prognostic role in many hematologic neoplasms, but their prognostic value in CMML has been debatable. Recently, monosomal karyotype (MK) has been reported to be a marker of poor prognosis in patients with myelodysplastic syndromes (MDS) and primary myelofibrosis, but its value in CMML is unknown. Aim To study MK effect on clinical outcome for patients diagnosed with CMML Method A retrospective study of all cases diagnosed with CMML at Mayo Clinic Rochester between 1994 to 2011 was performed. Only pts with complete cytogenetic analysis at presentation to our institution were included. MK was defined as the presence of ≥ 2 autosomal monosomies or one autosomal monosomy with at least one structural abnormality (Breems et al, JCO 2008). CK was defined as the presence of at least 3 chromosomal abnormalities. Appropriate IRB approval was obtained in accordance with Helsinki declaration. Comparison between groups’ medians was done using Wilcoxon test, while survival estimates were calculated using Kaplan-Meier curves using JMP V9. Results A total of 262 pts diagnosed with CMML had available cytogenetic data at diagnosis. Median age was 72 years, 176 (67%) were male. Median hemoglobin 10.5 g/dL, white blood cells (wbc) 12 x109/L, platelet 89 x109/L, peripheral blood (PB) blast 0, and bone marrow (BM) blast 4%. CMML2 was seen in 9% while 47% were proliferative (wbc >13). Leukemic transformation was documented in 34 pts (13%). Median overall survival was 513 days. Cytogenetic (CG) analysis was diploid in 167 pts (64%). Trisomy 8 was the most frequent cytogenetic abnormality at 8% (22), followed by complex karyotype (CK) 5% (14), then -7 at 4% (10) and MK 3% (7, six of which were also CK). Comparing pts with diploid CG to other categories indicates: to abnormal CG pts had lower wbc (0.001), PB blasts (p<0.0001), and BM blasts (p=0.0001); to CK pts had lower PB blasts (p=0.003) and higher platelets (p=0.03); to -7 pts had lower wbc (p=0.005), PB blast (p=0.0004), BM blasts (p=0.03) and higher platelets (p=0.03); to +8 pts had lower PB blast (p=0.02) and BM blasts (p= 0.01); no difference was noted when compared to MK. Median OS was statistically significantly worse in MK+ vs MK- (24 vs 527 days, p= 0.002), but not in other comparisons: -7 vs others (250 vs 527 days, p=0.2), CK+ vs CK- (256 vs 527 days, p=0.05), diploid vs others (570 vs 365 days, p=0.1), +8 vs others (312 vs 527 days, p=0.1). Pts with MK+ only or MK+CK+ did worse than CK+ only or other groups (4, 63, 304, 527 days, respectively, p<0.0001). On a multivariate analysis, MK+ (in addition to platelet, BM blast, hemoglobin, and wbc) did have an impact on OS (p=0.0004), while CK+, -7, +8, diploid CG, age, PB blast did not. Conclusion Cytogenetic abnormalities were not frequent findings in pts diagnosed with CMML (36%), but did affect wbc, PB and BM blasts. The most common cytogenetic abnormality was +8 while MK was present at 3% (less than published data in MDS). Only MK predicted statistically significant shorter mOS between all other cytogenetic categories on both univariate and multivariate analysis. This finding needs to be validated by larger cohorts of pts due to its rare occurrence in CMML. Disclosures: No relevant conflicts of interest to declare.

Abstract. We describe in this paper the new version of LANL*. Just like the previous version, this new version V2.0 of LANL* is an artificial neural network (ANN) for calculating the magnetic drift invariant, L*, that is used for modeling radiation belt dynamics and for other space weather applications. We have implemented the following enhancements in the new version: (1) we have removed the limitation to geosynchronous orbit and the model can now be used for any type of orbit. (2) The new version is based on the improved magnetic field model by Tsyganenko and Sitnov (2005) (TS05) instead of the older model by Tsyganenko et al. (2003). We have validated the model and compared our results to L* calculations with the TS05 model based on ephemerides for CRRES, Polar, GPS, a LANL geosynchronous satellite, and a virtual RBSP type orbit. We find that the neural network performs very well for all these orbits with an error typically Δ L* < 0.2 which corresponds to an error of 3% at geosynchronous orbit. This new LANL-V2.0 artificial neural network is orders of magnitudes faster than traditional numerical field line integration techniques with the TS05 model. It has applications to real-time radiation belt forecasting, analysis of data sets involving decades of satellite of observations, and other problems in space weather.

Hot dip galvanizing is being widely used to the numerous constructional steels such as a guard rail of high way, various types of structural steel for manufacturing ship and for some other fields etc.. Recently, the cost of zinc is getting higher and higher, thus, a proper manufacturing process should be developed possibly to reduce the cost of production. one of hot dip galvanizing process, that is, flux solution immersion process is very important to control more effective coating. However, flux solution is deteriorated with increasing its using time because dissolved iron from structural steel due to continuous flux treatment was increasingly increased, which is resulted in coating badness and increment of dross. In this study, the effect of additives to elimination of iron dissolved in flux solution was investigated with four types of inner solution, that is, solution of inner barrel was filled with seawater, NaOH, thiourea, including parameters of amounts of NH4Cl(5,10,15 and 20g) and electrolysis experiment was carried out with applied current density at 20mA/cm2. For the best elimination effect of iron from flux solution, it is considered that inner barrel with separator should be filled with inner solution different from original flux solution which is involved in iron ion, and the inner solution should be composed with seawater added with additives such as NH4Cl(10g), NaOH(0.2 g) and thiourea(0.5g) when the applied current density by electrolysis method is 20mA/cm2.

Gereç ve Yöntemler: Hastanemizin 1 Ocak 2015-15 Haziran 2019 tarihleri arasında genel cerrahi(GC), Dahiliye (IM), Kadın Hastalıkları ve Doğum (OG) polikliniklerine başvuran hastalar retrospektif olarak incelendi. Sürekli değişkenler ortalama ve standart sapma olarak rapor edilirken, kategorik nominal değişkenler toplam popülasyonun yüzdesi olarak ifade edildi. Bulgular: 1 Ocak 2015-15 Haziran 2019 tarihleri arasında meme muayenesi için gerekli olan toplam mamografi sayısı 7998'dir. Bunların 1769'u Gc, 456'sı IM ve 5773'ü OG poliklinikleridir. Yaş ortalaması Gc'de 48±2,3, IM'de 48±6,7, OG polikliniklerinde 47±3,9 idi (p&gt;0,05). Malign meme vaka sayılarının dağılımı sırasıyla GC: 43, OG: 21, IM: 5 şeklindedir. Toplamda 69 meme kanseri teşhisi konulmuştur. Klinik hakimiyet açısından genel cerrahi kliniği meme kanseri koymada en etkili klinik olarak ortaya çıkmıştır [AOR: 0,34 (0,2-0,54) (P &lt;0,001)]. Mamografi BIRADS 4 ve 5 olan hastalarda malignite riski BIRADS 0-1-2-3 olanlara göre daha yüksekti [AOR: 0,81 (0,72-0,9) (P &lt;0,001)]. Sonuç: Özellikle maligniteyi ilgilendiren meme hastalıklarının tanısında en önemli mihenk taşının klinikler arası işbirliği ile yapılan fizik muayene, anamnez ve görüntüleme teknikleri olduğuna inanıyoruz.

Abstract:
We examined the role of melanocortin-4 receptors (MC4R) in proopiomelanocortin (Pomc) neurons in regulating metabolic and cardiovascular functions. Using Cre-loxP technology, we selectively rescued MC4R in Pomc neurons of mice with whole body MC4R deficiency (MC4R-Pomc-Cre mice). Body weight, food intake, and whole body oxygen consumption (V̇o2) were determined daily, and blood pressure (BP), heart rate (HR), and body temperature were measured 24 h/day by telemetry. An intracerebroventricular cannula was placed in the right lateral ventricle for intracerebroventricular infusions. Littermate MC4R-deficient (LoxTB-MC4R) mice were used as controls. After control measurements, the MC4R antagonist (SHU-9119; 1 nmol/h) was infused intracerebroventricularly for 7 days. Compared with LoxTB-MC4R mice, MC4R-Pomc-Cre mice were less obese (47 ± 2 vs. 52 ± 2 g) and had increased energy expenditure (2,174 ± 98 vs. 1,990 ± 68 ml·kg−1·min−1), but food intake (4.4 ± 0.2 vs. 4.3 ± 0.3 g/day), BP (112 ± 1 vs. 109 ± 3 mmHg), and HR [557 ± 9 vs. 551 ± 14 beats per minute (bpm)] were similar between groups. Chronic SHU-9119 infusion increased food intake (4.2 ± 0.2 to 6.1 ± 0.5 g/day) and body weight (47 ± 2 to 52 ± 2 g) in MC4R-Pomc-Cre mice, while no changes were observed in LoxTB-MC4R mice. Chronic SHU-9119 infusion also increased BP and HR by 5 ± 1 mmHg and 60 ± 8 bpm in MC4R-Pomc-Cre mice without altering BP or HR in LoxTB-MC4R mice. These results indicate that MC4Rs in Pomc neurons are important for regulation of energy balance. In contrast, while activation of MC4R in Pomc neurons facilitates the BP response to acute stress, our data do not support a major role of MC4R in Pomc neurons in regulating baseline BP and HR.

Abstract Context Pheochromocytoma and sympathetic paraganglioma (PPGL) are rare catecholamine-secreting tumors but recent studies suggest increasing incidence. Traditionally, PPGL are described to present with paroxysmal symptoms and hypertension, but existing data on clinical presentation of PPGL come from referral centers. Objective We aimed to describe time trends in clinical presentation and incidence of PPGL in a population-based study. Methods We conducted a nationwide retrospective cohort study of a previously validated cohort of 567 patients diagnosed with PPGL in Denmark 1977-2015. We collected clinical data from medical records of a geographic subcohort of 192 patients. We calculated age-standardized incidence rates (SIRs) and prevalence for the nationwide cohort and descriptive statistics on presentation for the subset with clinical data. Results SIRs increased from 1.4 (95% CI 0.2-2.5) per million person-years in 1977 to 6.6 (95% CI 4.4-8.7) per million person-years in 2015, corresponding to a 4.8-fold increase. The increase was mainly due to incidentally found tumors that were less than 4 cm and diagnosed in patients older than 50 years with no or limited paroxysmal symptoms of catecholamine excess. On December 31, 2015, prevalence of PPGL was 64.4 (CI 95% 57.7-71.2) per million inhabitants. Of 192 patients with clinical data, 171 (89.1%) had unilateral pheochromocytoma, while unilateral paraganglioma (n = 13, 6.8%) and multifocal PPGL (n = 8, 4.2%) were rare. Conclusion Incidence of PPGL has increased 4.8-fold from 1977 to 2015 due to a “new” group of older patients presenting with smaller incidentally found PPGL tumors and few or no paroxysmal symptoms.

Abstract:
Object No clear treatment guidelines for pediatric craniopharyngiomas exist. The authors developed a decision analytical model to evaluate outcomes of 4 surgical approaches for craniopharyngiomas in children, including attempted gross-total resection (GTR), planned subtotal removal plus radiotherapy, biopsy plus radiotherapy, and endoscopic resections of all kinds. Methods Pooled data, including the authors' own experience, were used to create evidence tables, from which incidence, relative risks, and summary outcomes in quality-adjusted life years (QALYs) were calculated for the 4 management strategies. Results Quality-adjusted life years at the 5-year follow-up were 2.3 ± 0.1 for attempted GTR, 2.9 ± 0.2 for planned subtotal removal plus radiotherapy, 3.9 ± 0.2 for biopsy plus radiotherapy, and 3.7 ± 0.2 for endoscopic resection (F = 17,150, p < 0.001). Similarly, QALYs at 10-year follow-up were 4.5 ± 0.2 for attempted GTR, 5.7 ± 0.5 for planned subtotal removal plus radiotherapy, and 7.8 ± 0.5 for biopsy plus radiotherapy (F = 6,173, p < 0.001). On post hoc pairwise comparisons, the differences between all pairs compared were also highly significant (p < 0.001). Since follow-up data at 10 years are lacking for endoscopic cases, this category was excluded from 10-year comparisons. Conclusions Biopsy with subsequent radiotherapy is the preferred approach with respect to improved overall quality of life. While endoscopic approaches also show promise in preserving quality of life at five-year follow-up, there are not sufficient data to draw conclusions about this comparison at 10 years.
