Abstract:
Reversed-phase liquid chromatography hyphenated with inductively coupled plasma mass spectrometry (LC-ICP-MS) was used for mercury speciation analysis in food samples. A short chromatographic column (Purospher<sup>&reg;</sup> RP-8e, 75 &acute; 4&nbsp;mm, 3 &micro;m) and a mobile phase containing 0.02 mol/l CH<sub>3</sub>COONH<sub>4</sub> + 0.2% (v/v) 2-mercaptoethanol (2-ME) + 1% (v/v) CH<sub>3</sub>OH were applied. A repeated extraction of samples with hydrochloric acid/2-ME solution (1 mol/l HCl + 0.2% (v/v) 2-ME) was applied as the isolation step. The results were satisfactory for most food matrices (fish, shellfish, plant materials). Conversely, to analyse high-protein animal matrices, which contain mostly the inorganic form of mercury, a procedure including partial hydrolysis using hydrochloric acid should be used. For methylmercury and inorganic divalent mercury, the LOQ values of 0.3 and 2 ng/g, respectively, can be achieved if precautionary measures against contamination are fulfilled. The method was applied for the determination of methylmercury and inorganic divalent mercury in fish, vegetables, herbs and cereal products.

ImportanceIt is estimated that, from 2023 to 2025, lung cancer (LC) will be the second most frequent cancer in Brazil, but the country does not have an LC screening (LCS) policy.ObjectiveTo compare the number of individuals eligible for screening, 5-year preventable LC deaths, and years of life gained (YLG) if LC death is averted by LCS, considering 3 eligibility strategies by sociodemographic characteristics.Design, Setting, and ParticipantsThis comparative effectiveness research study assessed 3 LCS criteria by applying a modified version of the LC-Death Risk Assessment Tool (LCDRAT) and the LC-Risk Assessment Tool (LCRAT). Data are from the 2019 Brazilian National Household Survey. Participants included ever-smokers aged 50 to 80 years. Data analysis was performed from February to May 2023.ExposuresExposures included ever-smokers aged 50 to 80 years, US Preventive Services Task Force (USPSTF) 2013 guidelines (ever-smokers aged 55 to 80 years with ≥30 pack-years and &amp;lt;15 years since cessation), and USPSTF 2021 guidelines (ever-smokers aged 50 to 80 years with 20 pack-years and &amp;lt;15 years since cessation).Main Outcomes and MeasuresThe primary outcomes were the numbers of individuals eligible for LCS, the 5-year preventable deaths attributable to LC, and the number of YLGs if death due to LC was averted by LCS.ResultsIn Brazil, the eligible population for LCS was 27 280 920 ever-smokers aged 50 to 80 years (13 387 552 female [49.1%]; 13 249 531 [48.6%] aged 50-60 years; 394 994 Asian or Indigenous [1.4%]; 3 111 676 Black [11.4%]; 10 942 640 Pardo [40.1%]; 12 830 904 White [47.0%]; 12 428 536 [45.6%] with an incomplete middle school education; and 12 860 132 [47.1%] living in the Southeast region); 5 144 322 individuals met the USPSTF 2013 criteria for LCS (2 090 636 female [40.6%]; 2 290 219 [44.5%] aged 61-70 years; 66 430 Asian or Indigenous [1.3%]; 491 527 Black [9.6%]; 2 073 836 Pardo [40.3%]; 2 512 529 [48.8%] White; 2 436 221 [47.4%] with an incomplete middle school education; and 2 577 300 [50.1%] living in the Southeast region), and 8 380 279 individuals met the USPSTF 2021 LCS criteria (3 507 760 female [41.9%]; 4 352 740 [51.9%] aged 50-60 years; 119 925 Asian or Indigenous [1.4%]; 839 171 Black [10.0%]; 3 330 497 Pardo [39.7%]; 4 090 687 [48.8%] White; 4 022 784 [48.0%] with an incomplete middle school education; and 4 162 070 [49.7%] living in the Southeast region). The number needed to screen to prevent 1 death was 177 individuals according to the USPSTF 2013 criteria and 242 individuals according to the USPSTF 2021 criteria. The YLG was 23 for all ever-smokers, 19 for the USPSTF 2013 criteria, and 21 for the USPSTF 2021 criteria. Being Black, having less than a high school education, and living in the North and Northeast regions were associated with increased 5-year risk of LC death.Conclusions and RelevanceIn this comparative effectiveness study, USPSTF 2021 criteria were better than USPSTF 2013 in reducing disparities in LC death rates. Nonetheless, the risk of LC death remained unequal, and these results underscore the importance of identifying an appropriate approach for high-risk populations for LCS, considering the local epidemiological context.

Abstract:
Mammalian embryos are thought to cleave equally during early development. Therefore, the contents including organelles such as mitochondria (mt) in oocytes are supposed to be delivered symmetrically into each blastomere of the resulting embryo. However, a recent report showed higher oxygen consumption, ATP production, and mt distribution in trophectoderms from mouse blastocysts than that in inner cell masses (Houghton 2006 Differentiation 74, 11–18). We hypothesized that this phenomenon could be preceded by the asymmetrical mt distribution into blastomeres at earlier stages. Oocytes, 2-cell embryos, and 4-cell embryos from BDF1 were measured to determine volumes, ATP contents, and mtDNA copy numbers in the whole egg and individual blastomeres that had been separated in Ca/Mg-free KSOM with added 0.02% EDTA. The volumes of blastomeres were calculated by the sphere volume formula. The ATP contents were determined using a luminometer and commercial kits (BacTiter-Glo, Promega, Madison, WI, USA). The mtDNA copy numbers were quantified absolutely using real-time PCR. The data was analyzed by 1-way ANOVA followed by Fisher's least significant difference test. We found about a 10% difference in volume between the largest and the smallest blastomeres from a 4-cell embryo. At the 4-cell stage, the largest blastomeres were significantly larger than the middle-sized and smallest blastomeres, and the next largest blastomeres were significantly larger than the smallest blastomeres (largest: 41 093 µm3, next largest: 39 671 µm3, middle-sized: 37 725 µm3, smallest: 36 518 µm3; n = 25). The procedure for separation of blastomeres did not significantly reduce theATP contents of embryos. We observed the same pattern of significant differences in theATP contents among the 4 blastomeres at the 4-cell stage (largest: 0.141 pmol, next largest: 0.124 pmol, middle-sized: 0.112 pmol, smallest: 0.098 pmol, n = 27). The largest blastomeres from 4-cell embryos contained about 1.4-fold higherATP than the smallest blastomeres. There was no significant difference in the mtDNA copy numbers in oocytes (n = 7), whole 2-cell embryos (n = 6), whole 4-cell embryos (n = 13), the sum of 2 blastomeres from the same 2-cell embryo (n = 6), and the sum of 4 blastomeres from the same 4-cell embryo (n = 6; 245 071 � 22 696, 267 567 � 30 989, 262 931 � 12 952, 239 717 � 16 813, 247 012 � 16 166, respectively; � SEM). However, the larger blastomere of 2-cell embryo had a higher mtDNA copy number than the smaller blastomeres (largest: 137 100 � 11 493, smallest: 102 617 � 6205; P < 0.05). The largest blastomeres of 4-cell embryo had about 1.4 times (P < 0.05) higher mtDNA copy numbers than the smallest blastomeres (largest: 72 072 � 4112, next largest: 66 198 � 5767, middle-sized: 57 596 � 3922, smallest: 51 146 � 3081). We found differences in volumes, ATP contents, and mtDNA copy numbers among blastomeres from the same embryos at the 4-cell stage. These qualitative differences could be related to embryonic metabolism in mouse early development.

Abstract:
4002 Background: Cetuximab prolongs survival in pretreated advanced colorectal cancer (CRC) patients (pts). For these pts, the effect of palliative treatment on quality of life (QOL) is also an important outcome. Methods: CO.17 was a randomized controlled trial (n=572) of cetuximab combined with best supportive care (BSC) vs BSC alone in pts with pretreated epidermal growth factor receptor-positive advanced CRC. Primary endpoint was survival. QOL was a secondary endpoint using the EORTC QLQ-C30 at baseline, 4, 8, 16, and 24 weeks (wks). Primary endpoint for QOL compared physical function (PF) and global health status (Global) mean changes from baseline to 8 and 16 wks using Wilcoxon test. Negative change scores denote worsening QOL. The proportion of pts in either arm with at least a 10 unit deterioration at 8 and 16 wks from baseline was compared using Fischer’s exact test. Kaplan-Meier estimates and logrank tests on median time to definitive QOL deterioration, defined as at least a 10 unit deterioration in PF or Global, were evaluated. Exploratory analyses of all other scale and symptom change scores at 8 and 16 wks were also performed. Results: Compliance rates for QOL questionnaires was high at baseline, 94% each arm, but did drop off over time: 73% vs 60% at week 8 and 60% vs 39% at week 16 for cetuximab vs BSC, respectively. PF change scores at 8 wks were -3.9 for cetuximab vs -8.6 for BSC (p=.046); at 16 wks -5.9 vs -12.5 (p=.027). Global change scores were -0.5 vs -7.1 (p=.008) and -3.6 vs -15.2 (p=.008) at 8 and 16 wks respectively. The proportion of pts with deteriorating PF was 25% for cetuximab vs 35% for BSC at 8 wks (p=.051) and 30 vs 43% at 16 wks (p=.069). Deteriorating Global was seen in 23 vs 38% at 8 wks (p=.004) and 31 vs 49% at 16 wks (p=.0011). Time to PF deterioration was 5.4 months [95% CI 3.8–5.7] for cetuximab vs 3.7 months [2.0–3.9] for BSC (p=.022). Time to Global deterioration was 5.4 [3.9–5.7] vs 3.7 months [2.1–3.9] (p=.062). Exploratory analyses of all other scale and symptom change scores at 8 and 16 wks showed all differences of p<.05 also favored cetuximab. Conclusions: Cetuximab provides palliation in pretreated pts with advanced CRC, delaying deterioration in QOL as well as improving survival. [Table: see text]

Abstract:
The effects of tetrakis(1-methylpyridinium-4-yl)porphyrinatomercury(II) [ Hg ( TMPyP )]4+ (6) along with [ Pb ( TMPyP )]4+ (7), [ Cd ( TMPyP )]4+ (8), and [ H 2( TMPyP )]4+ (1) (free base porphyrin) on DNA cleavage by Haemophilus aegyptius (HaeIII) have been studied using gel electrophoresis, UV-vis absorption spectroscopy, and circular dichroism (CD) spectroscopy. The gel electrophoresis results indicate that in the absence of 6, HaeIII restriction enzyme could not cleave plasmid DNA at a low concentration of 0.2 units μl-1 after 1 h incubation at 37 °C. However, DNA cleavage occurred in the presence of 6 at the concentrations of 1.0 × 10-6 and 1.0 × 10-7 M and 0.2 units μl-1 of HaeIII under the same conditions. In addition, acceleration of DNA cleavage occurred at a higher concentration of HaeIII (0.4 units μl-1) in the presence of a wider concentration range of 6 from 1.0 × 10-5 to 1.0 × 10-7 M . 8, 7, and 1 could not enhance DNA cleavage in the presence of HaeIII (0.2 units μl-1). However, when the concentration of HaeIII was increased to 0.4 units μl-1, only 8 inhibited DNA cleavage by HaeIII at higher concentrations (1.0 × 10-5-10-6 M ) whereas 6, 7, and 1 did not. The UV-vis and CD spectroscopic results indicate that these porphyrins interact differently with DNA based on their binding modes. 6 enhanced DNA cleavage due to the synergistic effect of the Hg 2+ ions and the free base porphyrin 1 that dissociated from the mercury(II) porphyrin complex upon binding to DNA which resulted in the enhanced transformation of the DNA conformation from the original B-form to a Z-like conformation, while 8 inhibited HaeIII activity at higher concentrations and 7 and 1 neither enhanced nor inhibited DNA cleavage. A mechanism for this phenomenon is suggested.

Abstract:
Introduction: Little is known about prevailing ribotypes of Clostridium difficile infection in Romania where CDI is not a mandatory notifiable disease. Methodology: We studied 64 non-duplicate C. difficile isolates from patients hospitalised at the National Institute of Infectious Diseases, Bucharest, Romania between March 2011 and March 2012. Results: Sixty-three of the 64 C. difficile isolates produced toxins A and B whereas 44 (69%) isolates produced a binary toxin. Ribotype 027 accounted for 43 (68%) of the 63 toxigenic strains. The remaining 20 isolates belonged to ribotypes 018 (n = 9), 012 (n = 3), and, with one isolate each, 014, 031, 081, 416, 433, 500, 507 and PR03035 (new ribotype). Information on hospital mortality was available for 62 of the 64 patients; among these 62 cases, 4 (6.4%) ended fatal. Recurrence was documented for 11 (18.3%) of the 60 patients for whom this information was available. Multilocus variable-number tandem repeat analysis of the 43 isolates of ribotype 027 yielded a unique cluster for the Romanian isolates when compared to Austrian or Italian isolates. Conclusion: Our findings sustain the hypothesis of a recent emerged outbreak of C. difficile PCR ribotype 027 infections in the area of Bucharest.

Abstract:
The aim of this study was to characterize C. difficile isolates from the farm, abattoir, and retail outlets in Ireland in terms of ribotype and antibiotic resistance (vancomycin, erythromycin, metronidazole, moxifloxacin, clindamycin, and rifampicin) using PCR and E-test methods, respectively. The most common ribotype in all stages of the food chain (including retail foods) was 078 and a variant (RT078/4). Less commonly reported (014/0, 002/1, 049, and 205) and novel (RT530, 547, and 683) ribotypes were also detected, but at lower frequencies. Approximately 72% (26/36 tested) of the isolates tested were resistant to at least one antibiotic, with the majority of these (65%; 17/26) displaying a multi-drug (three to five antibiotics) resistant phenotype. It was concluded that ribotype 078, a hypervirulent strain commonly associated with C. difficile infection (CDI) in Ireland, was the most frequent ribotype along the food chain, resistance to clinically important antibiotics was common in C. difficile food chain isolates, and there was no relationship between ribotype and antibiotic resistance profile.

Abstract:
Lo studio delle anfore della cuspide nord-orientale della Sicilia, condotto su base bibliografica per alcuni centri e attraverso l’analisi diretta dei materiali provenienti dai siti campione di Patti Marina e dalle isole Eolie, ha rivelato l’importante ruolo che l’isola in generale e il comprensorio analizzato nello specifico, hanno ricoperto all’interno del commercio dei prodotti sin dalla tardo età repubblicana e per tutta imperiale e tardoantica, ridimensionandosi gradualmente, senza mai tuttavia scomparire del tutto, in età bizantina. La presenza alla fine del I sec. a.C. delle numerose Dressel 2-4 di provenienza campana e, in quantità seppur più modeste, di prodotti dell’area adriatica definisce i primi rapporti commerciali del comprensorio con la penisola italica, destinati ad intensificarsi durante i secoli successivi. Contestuale è l’apertura verso i commerci internazionali, riguardanti in una prima fase l’area del Mediterraneo occidentale ed in particolare la Gallia e la Spagna, i cui prodotti tuttavia, destinati principalmente ai mercati romani, toccarono solo di passaggio le coste settentrionali dell’isola. Commerci con il Mediterraneo orientale, e principalmente con l’isola di Creta, sono attestati in misura nettamente maggiore, soprattutto nell’arcipelago eoliano, tappa di passaggio obbligata nelle rotte verso occidente. Dal quadro esaminato emerge chiaramente la forte attività produttiva dei centri dell’isola già dalla tardo età repubblicana: alle Dressel 2-4 prodotte nei diversi centri del comprensorio (Caronia, Naxos), fecero seguito le produzioni nassie di anfore ritenute di imitazione gallica e i tipi Ostia II 523/Sant’Alessio e Spinella, adibiti al trasporto del vino tauromenitano verso i mercati romani, a cui si affiancarono le produzioni di MR 1A e B della più meridionale Acium. All’estrema vivacità produttiva dell’area ionica dovette fare seguito una analoga attività nell’area tirrenica, dove alcune delle successive produzioni sembrano essere presenti già alla metà del III sec. d.C. Lo stesso periodo è segnato dalla comparsa sui mercati dei prodotti africani, ben attestati nel comprensorio, tuttavia in quantità alquanto esigue se confrontate, a partire dalla metà del IV sec. d.C. con i prodotti di produzione locale, in particolare la grande famiglia tipologica delle anfore a fondo piatto e umbonato, caratterizzata da una estrema varietà morfologica dell’orlo che riflette i numerosi centri produttivi della costa: Termini Imerese, Caronia, Furnari/Tonnarella e Capo-d’Orlando Bagnoli, a cui è possibile aggiungere, grazie alle analisi condotte sui materiali, anche Patti, dove le numerose varianti identificate, alcune delle quali confrontabili solo con gli esemplari romani, lasciano chiaramente intendere il ruolo che questo centro dovette ricoprire nelle attività produttive di età romana e oltre. Nello stesso ambito produttivo rientrano le Keay LII, alla cui produzione, ampiamente attestata sul litorale ionico (Naxos e Acium), si unisce quelle dei centri della Calabria meridionale e le anfore Crypta Balbi, tipi entrambi largamente attestati e dominanti nelle stratigrafie romane di fine IV-VI sec. d.C. Tenendo conto di un distretto così produttivamente sviluppato, che probabilmente doveva riuscire da solo a soddisfare il fabbisogno locale, perlomeno di vino, tanto da poter esportare i prodotti in eccedenza verso i mercati romani, non stupisce la quantità circoscritta delle importazioni africane a cui si affiancano, a partire dal IV sec. d. C. anche quelle orientali costituite dalle Late Roman. Abstract:
This work is part of a research and development project (R&D - 06631-0009/2019), which aims to develop and validate technological strategies integrating physicochemical systems for the repulsion of the ichthyofauna from the suction pipe during the shutdown of the generating units of the Jirau Hydroelectric Power Plant, located on the Madeira River, state of Rondônia, Brazil. One of the strategies is the acoustic barrier, which is being developed to meet the particularities of the Amazonian fish species in the study area. The barrier will prevent and/or minimize fish entering the suction pipes during turbine stops and restarts. In the first stage, acoustic parameters were identified: auditory threshold and hearing frequency range of the most common fish caught at the plant. In the second stage, computer simulations were carried out that defined the configuration and power of the sound source required for the barrier. In the third stage, the characterization of aerial and underwater sounds in the vicinity of the plant was carried out. In this stage, in situ tests were carried out using the simulated arrangement with the sound source of the Lubell LL916 and the C75 hydrophone of Cetacean Research, testing the efficiency of 4 different noise configurations: pink, thunder, shooting and with well-defined tonal components. The tests were carried out at the spillway of the Jirau Hydroelectric Power Plant, where the movement of fish was monitored before and during the activation of the sound source. The results indicated that all the different types of sounds have the potential to increase the movement of the fish, but with different performances. Sound generated with tonal components increased movement by 57 percent, thunder noise by 43 percent, gunshot noise by 37 percent, and pink noise by 29 percent. These results are promising and will be applied in tests on suction tubes in the next stage of the study.

Abstract:
To speed-up monitoring of the hygienic status of meat, fast and non-invasive measuring techniques are required. The feasibility of a hand-held fluorescence device (freshdetect) was investigated to quantify the bioburden of minced pork meat. In total, 144 minced pork samples from 18 batches were stored at 2°C for up to 8 days. Fluorescence spectra, L*a*b* colour and total viable mesophilic plate counts (TVC) were measured in parallel during this period. Correlation of the fluorescence spectra with log(TVC/g) using partial least squares regression yielded cross-validated predictions with R²cv = 0.72 and a prediction error of RMSECV = 0.97 log(TVC/g). Cross-validated limits of detection and quantification were determined as 1.03 and 3.38 log(TVC/g), respectively. This would allow for a quantification well below the alarm threshold of 5.7-6.7 log(TVC/g). The spectra correlated also well with storage time (R²cv = 0.81), but less strong with L*a*b* colour.

Abstract:
Drum1 measured threshold-versus-adapting-intensity (TVI) functions for chromatic increments on achromatic backgrounds. The TVI slopes became greater than unity at background levels above 4 log trolands (log td) for stimulus wavelengths across the visible spectrum. Because only one background level above 4 log td was tested, however, the detailed shapes of the curves and the full extent of the steepening could not be resolved. We now report more detailed TVI measurements at four test wavelengths (430, 490, 575, and 660 nm) and background illuminances at 0.2 log-unit intervals from 4.0 log td to 5.6 log td. Foveal, 2° diameter test stimuli were presented for 1 s on a concentric, perceptually white (5128 K color temperature), 7° diameter, steady background. Thresholds were obtained by the method of adjustment, after which the test stimulus illuminances were increased 0.6 log-unit and the subject estimated the percentages of red, yellow, green, blue, and white. Average TVI slopes for two subjects were 2.75 for 430 nm, 1.55 for 490 nm, 1.15 for 575 nm, and 1.4 for 660 nm, which is in qualitative agreement with published estimates of the ratio of chromatic to achromatic sensitivity. Also, the percentage of white apparent in the suprathreshold increments generally increased with increasing background illuminance despite substantial increases in excitation purity. These findings imply that steady, intense achromatic backgrounds can produce response saturation in color-oppo-nent mechanisms.

Abstract:
Our research has shown that the greatest influence on the height of Tulipa L. plants of the ‘Anna Krasavitsa’ (‘AK’) and ‘Blushing Lady’ (‘BL’) varieties had Aurill and Complex of Microbial Preparations (CMP). The tulip plant height increased by 3-8 % compared to control. The height of the flower glass of ‘AK’ and ‘Holland Chic’ (‘HC’) varieties also increased by 6-7 % compared to control when using microbial preparations (MP). The width of the glass also increased by 4-9 % compared to control. The length of the lower leaf increased by 3-7 % compared to control under the action of Aurill and CMP, respectively. The width of the lower leaf of ‘HC’ increased by 3 % under the action of Aurill compared to the control variant. Depending on the variety and the preparation, the number of ammonifying bacteria increased by 57 % relative to control. The number of amylolytic and phosphate mobilizing bacteria increased on average by 76 % and 63 % under the influence of CMP. The number of oligonitrophilic and oligotrophic bacteria, when using CMP, significantly exceeded the control values: on average, by 60 and 74 %, respectively. The number of micromycetes under the influence of Aurill decreased by 49 % compared to control. The number of cellulolytic bacteria increased by 78 % compared to control mainly because of the introduction of CMP into the tulips rhizosphere. Thus, we found that the bacterization of the rhizosphere of tulips with Aurill and CMP positively affected the morphological characteristics of this flower. Furthermore, these preparations contributed to an increase in the number of agronomically valuable bacteria in the rhizosphere of plants.

Abstract A cracking damage was found in 347SS flange neck operated at about 520 deg. C after long term operation. In previous report presented in PVP2019-9304, it was well explained by creep damage from FEM analysis using isochronous curve based on API 579/ASME FFS-1 MPC Omega method. Following our previous paper, specimens were obtained from the replaced flange operated for 4 years. Using those specimens, destructive testing and remaining life assessment were performed to validate the analysis. As a result, specimens from the flange neck showed relatively short rupture life, compared to those from other locations. MPC Omega method along with adjustment factors derived from testing resulted in remaining life of 3.1 to 6.8 × 105 hrs. at average operating condition. In consideration with temperature variation during operation, remaining life is estimated to be 2.6 × 10-5 hrs., indicating 10 % life consumption at the time of taking sample. Life consumption estimated by FEM analysis is rather conservative but is comparable within factor of 2 when average strength is applied for the evaluation. Since the estimated stress conditions and test conditions are close to 0.2% proof stress of the material at this temperature range, significant primary creep strain was observed in the tests, that can cause some conservativeness in remaining life by the FEM analysis. While above, Larson Miller Parameter method also adjusted with rupture test data agrees with MPC Omega method on the assumption of uniaxial tension. However, it gives less-conservative life under the multiaxial stress conditions. Effect of multiaxial stress on creep life is more significantly observed in MPC Omega method, especially when stress ratio is higher. In this part I of two papers, the detail of creep test will be presented and discussed. In part II, current creep temperature limit shown in API579-1/ASME FFS-1 Part 4 Table 4.1 will be discussed and suggest an upgrade of creep temperature criteria based on the multi-axial creep model we learned through studies.

Objetivos Reportar algunos indicadores de proceso resultado e impacto en muestra de pacientes incorporados al Programa de Hospital Día en Adicciones del Servicio de Psiquiatria del Hospital Goyoneche entre 2017- 2019 Material y Métodos Se realiza estudio descriptivo retrospectivo de 47 pacientes con TUS y conductas adictivas evaluados clínicamente con criterios CIE10 consignando diagnostico de patología dual los cuales fueron seleccionados del total de pacientes que ingresaron a consulta ambulatoria y derivados a esta modalidad de tratamiento en las instalaciones del Hospital Goyeneche en virtud de su adherencia y motivación inicial trabajada seguidamente, ellos se incorporaron a una matriz de indicadores utilizando el programa exel. Los datos se obtienen de registros realizados en consultas individuales ambulatorias y las observaciones consignadas en registros psicoterapéuticos de las intervenciones de Hospital Día terapias de grupo y cambio así como los test psicológicos aplicados a cada paciente. Se realiza seguimiento telefónico. Resultados 43 pacientes 90.8% tienen un nivel de instrucción secundario completo y superior, 20 pacientes 38% mantienen estudios y trabajan, todos tienen tiempos de enfermedad superiores a 10 años, trastorno por uso de alcohol fue la dependencia mas común en 42 pacientes 89%, trastorno por uso de sustancias en 3 pacientes 6%, en 2 casos ludopatía 4%. Se identificó patología dual en 20 casos 43% de la muestra. Se han mantenido en abstinencia total 6 pacientes 13% de la muestra, con 1-3 recaídas 07 pacientes 15% de la muestra, 27 pacientes abandonaron el programa 57%. Se mantienen conectados al programa 13 pacientes con tiempos de abstinencia de 6 meses a mas de un año. Conclusiones: Se Obtiene una adherencia del 27% asociada con abstiencias superiores a los 6 meses en 13 de los 47 casos ingresados al programa de Hospital de Dia para adicciones del Servicio de Psiquiatría del Hospital Goyoneche Arequipa Peru.

The April 2016 surveys of expectations conducted by Central Banks with inflation targeting regimes indicate that simple average growth expectations for 2016 for the eight countries covered by REVELA have decreased by 0.1% from 1.8% to 1.7% compared to previous month. Similarly, the GDP weighted average growth expectations have decreased by 0.1% from -0.6% to -0.7%. Meanwhile, inflation expectations for 2016 have remained stable at 5.3% on a simple average basis, and have fallen by 0.2% on a GDP weighted average basis from 5.7% to 5.5% compared to the previous month. There were differences across countries both for growth and inflation expectations. Growth expectations increased in Guatemala and Peru by 0.1%, while they decreased in Uruguay by 0.7%, in Brazil by 0.2% and in Colombia by 0.1% - and they remained stable in Paraguay at 3.0%, in Mexico at 2. 4% and in Chile at 1.7%. Inflation expectations increased in Colombia by 0.3%, Uruguay by 0.2% and Guatemala by 0.1%, and they decreased in Brazil by 0.4% and in Chile, Mexico and Paraguay by 0.1% in all three cases, and remained constant in Peru at 3. 5%. Expected growth for the region is as low as -3.8% for Brazil and as high as 3.7% in Guatemala, while expected inflation ranges from 3.2% in Mexico to 10.1% in Uruguay.

Abstract:
Objectives. Diagnostic errors are a known patient safety concern across all clinical settings, including the emergency department (ED). We conducted a systematic review to determine the most frequent diseases and clinical presentations associated with diagnostic errors (and resulting harms) in the ED, measure error and harm frequency, as well as assess causal factors. Methods. We searched PubMed®, Cumulative Index to Nursing and Allied Health Literature (CINAHL®), and Embase® from January 2000 through September 2021. We included research studies and targeted grey literature reporting diagnostic errors or misdiagnosis-related harms in EDs in the United States or other developed countries with ED care deemed comparable by a technical expert panel. We applied standard definitions for diagnostic errors, misdiagnosis-related harms (adverse events), and serious harms (permanent disability or death). Preventability was determined by original study authors or differences in harms across groups. Two reviewers independently screened search results for eligibility; serially extracted data regarding common diseases, error/harm rates, and causes/risk factors; and independently assessed risk of bias of included studies. We synthesized results for each question and extrapolated U.S. estimates. We present 95 percent confidence intervals (CIs) or plausible range (PR) bounds, as appropriate. Results. We identified 19,127 citations and included 279 studies. The top 15 clinical conditions associated with serious misdiagnosis-related harms (accounting for 68% [95% CI 66 to 71] of serious harms) were (1) stroke, (2) myocardial infarction, (3) aortic aneurysm and dissection, (4) spinal cord compression and injury, (5) venous thromboembolism, (6/7 – tie) meningitis and encephalitis, (6/7 – tie) sepsis, (8) lung cancer, (9) traumatic brain injury and traumatic intracranial hemorrhage, (10) arterial thromboembolism, (11) spinal and intracranial abscess, (12) cardiac arrhythmia, (13) pneumonia, (14) gastrointestinal perforation and rupture, and (15) intestinal obstruction. Average disease-specific error rates ranged from 1.5 percent (myocardial infarction) to 56 percent (spinal abscess), with additional variation by clinical presentation (e.g., missed stroke average 17%, but 4% for weakness and 40% for dizziness/vertigo). There was also wide, superimposed variation by hospital (e.g., missed myocardial infarction 0% to 29% across hospitals within a single study). An estimated 5.7 percent (95% CI 4.4 to 7.1) of all ED visits had at least one diagnostic error. Estimated preventable adverse event rates were as follows: any harm severity (2.0%, 95% CI 1.0 to 3.6), any serious harms (0.3%, PR 0.1 to 0.7), and deaths (0.2%, PR 0.1 to 0.4). While most disease-specific error rates derived from mainly U.S.-based studies, overall error and harm rates were derived from three prospective studies conducted outside the United States (in Canada, Spain, and Switzerland, with combined n=1,758). If overall rates are generalizable to all U.S. ED visits (130 million, 95% CI 116 to 144), this would translate to 7.4 million (PR 5.1 to 10.2) ED diagnostic errors annually; 2.6 million (PR 1.1 to 5.2) diagnostic adverse events with preventable harms; and 371,000 (PR 142,000 to 909,000) serious misdiagnosis-related harms, including more than 100,000 permanent, high-severity disabilities and 250,000 deaths. Although errors were often multifactorial, 89 percent (95% CI 88 to 90) of diagnostic error malpractice claims involved failures of clinical decision-making or judgment, regardless of the underlying disease present. Key process failures were errors in diagnostic assessment, test ordering, and test interpretation. Most often these were attributed to inadequate knowledge, skills, or reasoning, particularly in “atypical” or otherwise subtle case presentations. Limitations included use of malpractice claims and incident reports for distribution of diseases leading to serious harms, reliance on a small number of non-U.S. studies for overall (disease-agnostic) diagnostic error and harm rates, and methodologic variability across studies in measuring disease-specific rates, determining preventability, and assessing causal factors. Conclusions. Although estimated ED error rates are low (and comparable to those found in other clinical settings), the number of patients potentially impacted is large. Not all diagnostic errors or harms are preventable, but wide variability in diagnostic error rates across diseases, symptoms, and hospitals suggests improvement is possible. With 130 million U.S. ED visits, estimated rates for diagnostic error (5.7%), misdiagnosis-related harms (2.0%), and serious misdiagnosis-related harms (0.3%) could translate to more than 7 million errors, 2.5 million harms, and 350,000 patients suffering potentially preventable permanent disability or death. Over two-thirds of serious harms are attributable to just 15 diseases and linked to cognitive errors, particularly in cases with “atypical” manifestations. Scalable solutions to enhance bedside diagnostic processes are needed, and these should target the most commonly misdiagnosed clinical presentations of key diseases causing serious harms. New studies should confirm overall rates are representative of current U.S.-based ED practice and focus on identified evidence gaps (errors among common diseases with lower-severity harms, pediatric ED errors and harms, dynamic systems factors such as overcrowding, and false positives). Policy changes to consider based on this review include: (1) standardizing measurement and research results reporting to maximize comparability of measures of diagnostic error and misdiagnosis-related harms; (2) creating a National Diagnostic Performance Dashboard to track performance; and (3) using multiple policy levers (e.g., research funding, public accountability, payment reforms) to facilitate the rapid development and deployment of solutions to address this critically important patient safety concern.
