Abstract:
Objective: To assess the relationship between vitamin D deficiency (VD) and risk factors for metabolic syndrome (MS) in postmenopausal women with arterial hypertension (AH). Materials and methods of research. The study included 46 women aged 45–65 years with amenorrhea > 12 months, having grade 1 hypertension and metabolic syndrome. Anthropometric, laboratory and instrumental data were studied. Biochemical parameters included total cholesterol (OH), HDL, triglycerides, glucose, insulin, and 25-hydroxyvitamin-D [25 (OH)D]. Daily blood pressure monitoring was performed according to the standard protocol. Levels of 25(OH)D in blood serum was classified as-adequate (≥ 30 ng/ml) and insufficient (20–29 ng/ml). Results: Level 25 (OH)D in blood serum was sufficient in 21 women (45.6 %), insufficient – in 25 (54.4 %). Women with insufficient levels of 25 (OH)D had higher levels of cholesterol, triglycerides, insulin, and HOMA-IR. Metabolic syndrome was detected in 64 % (16/25) of women with hypovitaminosis D and in 43 % (9/21) of women with sufficient VD (p < 0.01). The correlation analysis established the relationship of the low level of 25 (OH)D (< 30 ng / ml) with MS (r = 0.68), high triglycerides (r = 0.74) and low HDL (r = 0.71). The average concentration of 25 (OH) D decreased with an increase in the number of MS components (p = 0.016). Conclusions: VD deficiency in postmenopausal women was associated with a higher prevalence of MS. Women with HDL deficiency had a higher risk of MS, hypertriglyceridemia, and low HDL levels compared to those with adequate levels.

Abstract:
Con el propósito de efectuar diagnóstico precoz y tratamiento del cáncer de cuello uterino en el Departamento de Caldas, Colombia, S.A., se creó un grupo interdisciplinario o institucional descentralizado, que desde junio de 1989 se desplaza periódicamente y en forma continua a los Hospitales Regionales del Departamento de Caldas, para hacer actividades de prevención, educación a población de riesgo, adolescentes y detección del cáncer del cuello uterino. El ginecólogo practica colposcopia a todas las mujeres con citología anormal compatible con NIC o infección por VPH. Entre junio de 1989 y junio de 1992, se realizaron 559 colposcopias de primera vez; se diagnosticaron 152 casos de NIC y 21 casos de cáncer avanzado. La edad promedio para Ca. microinvasivo e invasivo es de 47.5 años; de 33.9 años para lesiones preinvasoras. El 79.5% de mujeres tuvo relaciones sexuales antes de los 20 años y en la colposcopia se encontraron lesiones en el 60% de los casos, siendo el punteado y la zona acetoblanca los cambios más frecuentes. En 41 colposcopia se observaron vasos atípicos o lesiones clínicamente invasoras. De 404 biopsias tomadas se detectaron 21 carcinomas de tipo escamocelular, 4 microinvasores y 17 invasores. El 88 % de las lesiones fueron intraepiteliales. La sensibilidad de la colposcopia frente a la biopsia fue de 94 % y la especificidad del 46 %. El 97 % de los pacientes con PAP compatible con NIC-CA, presentaron algún tipo de lesión col poscópica. El promedio de edad de los pacientes con NIC fue 13.8 años menor que los pacientes con carcinoma invasor. El 100% de la población estudiada es de clase económica baja y 42% de zona rural. Se efectuaron tratamientos en el Hospital Regional consistentes en Crioterapia conización, histcrectomía simple o ampliada; según el caso o radioterapia a nivel central. El programa ha posibilitado detectar un gran número de lesiones pre-invasoras, cuyo tratamiento oportuno ha permitido mejorar las condiciones de salud de la población y racionalizar los recursos, demostrándose los beneficios de una atención regionalizada con colposcopia. Además ha permitido determinar características colpo-citohistológicas y epidemiológicas.

Abstract:
Research subject. The article considers the Famennian‑Tournaisian sequence of the South-Eastern part of the Tchernyshev Uplift (North Cis-Uralian). The sequence corresponds to the Izyayu Formation.Materials and methods. The article is focused on the clarification of the stratigraphy, composition and depositional environment of this formation. Research data about the Izyayu Formation in the type area (Izyayu River) were used. The stratigraphic framework of this study included data on conodonts, foraminifers and carbonate carbon isotopic shifts.Results. In the type area under study, the Izyayu Formation corresponds to the interval from the upper part of the Famennian (upper part of the Palmatolepis marginifera utahensis conodont Zone) up to the middle part of the Tournaisian (Lower crenulata conodont Zone). The formation is 100– 120 m thick. The Izyayu Formation grades to the upper part of the deep-water Syvyu Formation in the east, and to the shallow-water Kamenka Formation and the lower part of the Ydzhid Formation. The areal of the Izyayu Formation comprises the South-Eastern part of the Tchernyshev Uplift. The Formation was deposited in the environment of a gentle prograding slope of a carbonate platform.Conclusions. The Izyayu Formation in the type area is composed of thin clayey-carbonate graded cycles. It covers the stratigraphic interval from the Zelenets Regional Stage through the Tcherepet Regional Stage. This formation is easily distinguishable by lithological features in outcrops and borehole cores.

Abstract:
Background: Elevated fibroblast growth factor 21 (FGF21) levels have been suggested, from cross-sectional studies, as an indicator of subclinical diabetic nephropathy. We investigated whether serum FGF21 was predictive of the development of diabetic nephropathy. Method: Baseline serum FGF21 levels were measured in 1136 Chinese type 2 diabetic subjects recruited from the Hong Kong West Diabetes Registry. The role of serum FGF21 in predicting decline in estimated glomerular filtration rate (eGFR) over a median follow-up of 4 years was analyzed using Cox regression analysis. Results: At baseline, serum FGF21 levels increased progressively with eGFR category (P for trend &lt;.001). Among 1071 subjects with baseline eGFR ≥ 30 mL/min/1.73 m2, serum FGF21 levels were significantly higher in those with eGFR decline during follow-up (n = 171) than those without decline (n = 900) (P &lt; .001). In multivariable Cox regression analysis, baseline serum FGF21 was independently associated with eGFR decline (hazard ratio, 1.21; 95% confidence interval [CI], 1.01–1.43; P = .036), even after adjustment for baseline eGFR. In a subgroup of 559 subjects with baseline eGFR ≥60 mL/min/1.73 m2 and normoalbuminuria, serum FGF21 level remained an independent predictor of eGFR decline (hazard ratio, 1.36; 95% CI, 1.06–1.76; P = .016). Integrated discrimination improvement (IDI) suggested that the inclusion of baseline serum FGF21 significantly improved the prediction of eGFR decline (IDI, 1%; 95% CI, 0.1–3.0; P = .013) in this subgroup, but not in the initial cohort involving all subjects. Conclusions: Elevated serum FGF21 levels may be a useful biomarker for predicting kidney disease progression, especially in the early stages of diabetic nephropathy.

BackgroundReality television shows are popular with children and young adults; inclusion of tobacco imagery in these programmes is likely to cause smoking in these groups. Series 3 of the UK reality show Love Island, broadcast in 2017, attracted widespread media criticism for high levels of smoking depicted. We have quantified this tobacco content and estimated the UK population exposure to generic and branded tobacco imagery generated by the show.MethodsWe used 1-min interval coding to quantify actual or implied tobacco use, tobacco paraphernalia or branding, in alternate episodes of series 3 of Love Island, and Census data and viewing figures from Kantar Media to estimate gross and per capita tobacco impressions.ResultsWe coded 21 episodes comprising 1001 min of content. Tobacco imagery occurred in 204 (20%) intervals; the frequency of appearances fell significantly after media criticism. An identifiable cigarette brand, Lucky Strike Double Click, appeared in 16 intervals. The 21 episodes delivered an estimated 559 million gross tobacco impressions to the UK population, predominantly to women, including 47 million to children aged <16 and 44 million gross impressions of Lucky Strike branding, including 4 million to children <16.ConclusionDespite advertising legislation and broadcasting regulations intended to protect children from smoking imagery in UK television, series 3 of Love Island delivered millions of general and branded tobacco impressions both to children and adults in the UK. More stringent controls on tobacco content in television programmes are urgently needed.

Iron-oxidizing autotrophs and acidophilic heterotrophs were quantified at an incubation temperature of 18 °C in several samples obtained from the bioleaching areas of two uranium mines in Ontario, Canada. All samples were mine-water samples with temperatures in the range 13–18 °C. Iron-oxidizing autotrophs ranged from 2683 ± 377 to 245 000 ± 20 205 colony-forming units∙mL−1 and were always numerically superior to acidophilic heterotrophs, which ranged from 40 ± 8 to 9650 ± 161 colony-forming units∙mL−1. For each sample, approximately 20 isolates of each nutritional group were examined for the ability to grow at temperatures of 4, 18, 21, and 37 °C, respectively; overall, 559 isolates of iron-oxidizing bacteria (predominantly Thiobacillus ferrooxidans) and 252 acidophilic heterotrophic isolates were examined and categorized as 'broader temperature range psychrotrophs,' 'narrower temperature range psychrotrophs,' 'intermediates,' or mesophiles. Although psychrotrophic representatives of both groups were abundant, no psychrophiles were recovered from any of the samples. For the iron oxidizers, the temperature growth profiles of the isolates were similar from sample to sample. For the acidophilic heterotrophs, the temperature growth profiles varied considerably among samples.Key words: psychrotrophs; Thiobacillus ferrooxidans; uranium mines.

Abstract:
Purpose Surgical resection of gastric cancer has produced suboptimal survival despite multiple randomized trials that used postoperative chemotherapy or more aggressive surgical procedures. We performed a randomized phase III trial of postoperative radiochemotherapy in those at moderate risk of locoregional failure (LRF) following surgery. We originally reported results with 4-year median follow-up. This update, with a more than 10-year median follow-up, presents data on failure patterns and second malignancies and explores selected subset analyses. Patients and Methods In all, 559 patients with primaries ≥ T3 and/or node-positive gastric cancer were randomly assigned to observation versus radiochemotherapy after R0 resection. Fluorouracil and leucovorin were administered before, during, and after radiotherapy. Radiotherapy was given to all LRF sites to a dose of 45 Gy. Results Overall survival (OS) and relapse-free survival (RFS) data demonstrate continued strong benefit from postoperative radiochemotherapy. The hazard ratio (HR) for OS is 1.32 (95% CI, 1.10 to 1.60; P = .0046). The HR for RFS is 1.51 (95% CI, 1.25 to 1.83; P < .001). Adjuvant radiochemotherapy produced substantial reduction in both overall relapse and locoregional relapse. Second malignancies were observed in 21 patients with radiotherapy versus eight with observation (P = .21). Subset analyses show robust treatment benefit in most subsets, with the exception of patients with diffuse histology who exhibited minimal nonsignificant treatment effect. Conclusion Intergroup 0116 (INT-0116) demonstrates strong persistent benefit from adjuvant radiochemotherapy. Toxicities, including second malignancies, appear acceptable, given the magnitude of RFS and OS improvement. LRF reduction may account for the majority of overall relapse reduction. Adjuvant radiochemotherapy remains a rational standard therapy for curatively resected gastric cancer with primaries T3 or greater and/or positive nodes.

7172 Background: Erlotinib (E) was associated with superior survival in a phase III trial of previously treated advanced NSCLC patients (pts). Celecoxib (C) has been shown to potentiate the apoptotic and growth inhibitory effects of E in pre-clinical models. Methods: This was a phase II trial of E plus C in advanced NSCLC pts that failed one prior chemotherapy regimen. Primary endpoint: efficacy; secondary endpoint: toxicity. Pts received C (400mg b.i.d.) and E (150mg daily) until disease progression. Planned accrual: 40 pts. Results: 26 pts with stage IIIB/IV NSCLC were enrolled. Patient (pt) characteristics: male 65%; median age 66; ECOG performance status 0/1- 96%. Eighteen pts had tissue available for FISH and EGFR mutation analysis: 50% had chromosome 7 polysomy (> 4 copies per cell); none had EGFR gains (>2 EGFR/chromosome 7). Two pts had an EGFR gene mutation (1 exon 19, 1 exon 21). Response results: partial response- 2 pts (1 with exon 19 mutation), stable disease- 8 pts, and progressive disease- 16 pts (1 with exon 21 mutation). Median progression free survival (PFS) and overall survival (OS): 1.9 and 10.2 months, respectively. Grade 3/4 upper gastrointestinal bleeding (GIB) occurred in 4 pts prompting study closure. One pt was on therapeutic dalteparin and two pts receiving warfarin developed marked INR prolongation (INR >10). The fourth pt had a history of peptic ulcer disease. Platelet counts at time of GIB: 142 - 559. Three pts had endoscopy and gastric or duodenal ulcers were found in all three cases. No pts were taking anti-acid medication at the time of GIB. No other pts were on therapeutic anticoagulation. Three pts without upper GIB were taking low-dose aspirin. Other toxicities: 85% grade 1/2 rash; 65% grade 1/2 diarrhea, 30% grade 1/2 nausea, 30% grade 1/2 fatigue (one grade 3 fatigue); one grade 3 pneumonitis, one grade 3 esophageal stricture. Conclusions: These observations suggest that C plus E may be associated with increased incidence of gastrointestinal ulceration and GIB and that the regimen should not be given to pts with a previous history of peptic ulcer disease or to pts requiring therapeutic anticoagulation. Based on response rate, PFS, and OS in this group of pts, it appears that results with E and C are similar to those reported for E alone. [Table: see text]

Uvod: Domovi za starije i nemoćne osobe pružaju skrb starijim i nemoćnim osobama izvan vlastite obitelji, a u sklopu stalnog smještaja osiguravaju cjelovitu skrb. Tjelesna aktivnost ima zadatak održati ili poboljšati funkcijske sposobnosti do kojih neminovno dolazi starenjem i učestalijom pojavom bolesti, također prevenira, liječi i rehabilitira određene akutne i kronične bolesti. Materijali i metode: Cilj rada bio je prikazati stanje zdravlja starijih osoba s obzirom na osnovne varijable poput dobi, spola, rizičnog ponašanja, vrste oboljenja, s posebnim osvrtom na tjelesnu aktivnost. Istraživanje je provedeno u ustanovama domova za starije i nemoćne na području Federacije Bosne i Hercegovine na području Srednjobosanske, Sarajevske, Hercegovačko-neretvanske i Zapadnohercegovačke županije. Rezultati: Istraživanjem je utvrđeno kako je veći broj osoba ženskoga spola (60%) smješten u domovima za starije i nemoćne uz najčešću životnu dob od 75 do 79 godina života (21%). Najčešće kronične bolesti koje su zastupljene kod ovih osoba su hipertenzija (22%), dijabetes (21%) i osteoporoza (4%). Neznatno je veći udio ispitanika koji se bave tjelesnim vježbanjem (51%), a najveći broj ispitanika ne zna koje su prednosti bavljenja tjelesnim aktivnostima (55%). Zaključak: Možemo zaključiti kako svaka aktivnost, prilagođena pojedinom štićeniku, može osobu učiniti pokretljivijom i u konačnici zdravijom i manje ovisnom o pomoći drugih.

MIL-L-23699 equivalent synthetic oil at 50° C were fed at a ratio of 0.8 l/min and circumferential velocity of 8.8 m/sec. Also the running performance of the gears, variation of tooth profile errors, affairs of damage, etc. were examined. Types of tooth surface fatigue mere scoring, spalling, or micropitting. A durability limit of Hertz-stress was assumed to be around 2GPa at the load cycle of 108.

Questionnaires were sent to 1,218 hospitals with more than 200 beds, in order to know the incidence of hemorrhagic disease due to vitamin K deficiency in infancy beyond 2 weeks after birth, during 4 and a half years, i.e. from January 1981 to June 1985. Out of the 534 cases reported, 407 had no obvious reasons for vitamin K deficiency: "idiopathic vitamin K deficiency in infancy". Other 68 cases had bleedingepisodes due to vitamin K deficiency associated with hepatobiliary lesions (e.g.congenital bile duct atresia), chronic diarrhea, long term antibiotic therapy and so on: "secondary vitamin K deficiencyin infancy". The third group consisting of 59 cases was so called "near miss" type, in which hemorrhagic tendency was discovered at the time of mass screening tests for vitamin K deficiency or by chance withoutany clinical hemorrhage. In the idiopatic group, 345 cases (84.8%) developed their bleeding episodes between 21 and 59 days of age, and 368 cases (90.4%) were wholly breast-fed. Intracranial hemorrhage was seen in 338 cases (83.0%) of this group. In most cases of this series (97.3%),no vitamin K was supplemented after birth. Administration of vitamin K is an urgent routine procedure during the first one or two months of life for all newbornbabies, although the incidence of the idiopathic vitamin K deficiency in infancyhas not decreased significantly comparedto the results of the first nation-wide survey (Jan. 1978 - Dec. 1980). Whether failure to timely recanalize coronary artery with intravenous streptokinase (SK) is related to inadequate fibrinolysis remains controversial. Since coagulation might also interfere with the course of clot lysis, this relation was investigated in 27 patients (pts) pretreated with heparin (10.000 IU bolus). 500.000 to 1.500.000 IU SK were infused in 45 min, 160 ± 60 min after the onset of infarct.Angiographic control, performed 219 ± 199 min after SK, showed persistent coronary occlusion in 5 pts (0CC) and patency in 22 (PAT), unrelated to SK dosage. One to 3 hours post SK, fibrinogen (Fg) (Clauss Method) was undetectable in all 22 PAT and 4/5 0CC. However, Fg (and clottable breakdown products) assessed by Gram's method* was detected in all 5 OCC and 3/22 PAT (164 ± 89 VS 13 ± 38 mg %, p < 0.005). Prolongation of thrombin time (neutralized with protamine) was limited in OCC, compared to PAT (59 ± 6 VS 96 ± 11 sec, p < 0.005), supporting the hypothesis of the presence of clottable material in OCC. These results were accompanied by slight differences of ELT (< 20 min in 2/5 OCC VS 21/22 PAT) and non significant differences in plasminogen (1.78 ± 1 VS 2.18 ± 1.1 mg/dl ; NS), α2 antiplasmin (1.7 ± 1.7 VS 3.9 ± 6.9 % ; NS), plasmin (12 ± 4 VS 16 ± 11 mm2 ; NS) and plasminogen activators (95 ± 41 VS 105 ± 23 mm2 ; NS).Conclusion : at similar systemic lytic activity induced by SK, failure of arterial recanalization was associated wih an apparent incomplete fibrinogenolysis (and fibrinolysis) in 20 % of the patients.*Gram H.C. - A new method of determination of the fibrin percent in blood and plasma - J Biol Chem 49 : 279 - 295 (1921)mean ± SD

Furosemide (FUR) reduces the ∝2-adrenoceptor- (∝2-R) mediated pressor effect of norepinephrine. Since it has been shown that FUR reduces platelet aggregation ( AGG) we studied the effect of FUR on α2-R as well as that on the ∝2-R-medi ated epinephrine ( EPI)-induced AGG a) ex vivo and b) in vitro. For comparison the effect of FUR on ADP-induced AGG was also studied. Methods: a) 8 normotensive men received FUR (30 mg b. i.d. ) for 3 weeks. EPI- (1 μmol/1) and ADP- (1 μmol/1) induced platelet AGG were measured before as well as after 3 weeks of FUR application with a semiautomatic device ( APACT) . Platelet ∝2-R-density was measured by 3H-yohimbine-binding and the fraction of high-affinity binding-sites was measured by competi tion of 3H-yohimbine with EPI. b) Platelet-rich plasma was incubated with FUR (1 mmol/1) for 10 min at 37 °C and AGG was measured (n=7) as described. Results: a) FUR decreased ∝2-R-densi ty from 293± 28 to 251±; 21 fmol/mg protein (p< 0.01) and high affinity binding sites from 64± 3 to 55± 5 % (p< 0.01). FUR inhibited ADP-induced AGG ex vivo while EPI-induced AGG did not change (table). b) In vitro FUR inhibited total ADP-induced AGG from 7 4 ± 4 to 45± 8 % (p< 0. 01) while again EPI-induced AGG did not change.Conclusions: The results suggest separate effects of FUR on α2 R-density and on platelet AGG. Although FUR decreased < r2-R, EPI-induced platelet AGG remained unchanged. Since FUR inhibited ADP-induced AGG ex vivo as well as in vitro FUR may interfere directly with the mechanism of ADP-induced platelet AGG.

Abstract:
The clinical results of thromboembolism (TE) in Patients with prosthetic valVe endocarditis (PVE) and anticoagulant therapy were studied. 22 PVE patients (ll males and females each from 4 to 59 years old, average 32.7) were selected from 1939 patients who had undergone valve replacement at this hospital from 1964 and 1985. The complication frequency of TE and its clinical results, anticoagulant therapy and coagulation tests were investigated. Diagnostic criterion was determined in either of the following two: l) those patients who experienced valve replacement, with at least gradual pyrogenic symptons and inflammation factors such as a large increase in white blood cells, the progress of ESR and positive CRP, also with the same bacterium found more than twice in blood culture, also with the same bacterium found more than twice in blood culture, or 2) those patients who experienced valve replacement, with bacterial verruca found at re-valve replacement or at pathological anatomy. PVE onset took 2 days to 6.5 years (average 407 days) to appear after valve replacement. 8 out of the 22 PVE patients (36.3%) showed complications at TE onset, and 5 out of the 8 patients repeated. Embolism was found in 6 cases of brain, 4 cases of kidney, 2 cases of lung, 2 cases of limbs and 1 case of spleen, and 8 patients all died. On the other hand, 5 complications (22.7%) at bleeding were found in 3 cases of brain, 1 case of duodenum and 1 case of site of replaced aortic valve, and 4 patients died. Anticoagulant therapy was given to 21 out of the 22 PVE patients, and thrombotest (TT) values at TE onset were all less than 30%. Warfarin was administered as anticoagulant. 2 patients were administered with aspirin, but one was given with 250mg aspirin per day together with warfarin, and the other with 330mg aspirin per day alone. TT values at the onset of bleeding were from 10 to 56%. Anticoagulant therapy had been performed to the PVE patients since PVE onset did not yet appear, but complications coagulability by TT values, and all the patients died. In addition to this, complications at bleeding were found many and most of patients died even when the TT values were not so low. Therefore, we believe that the anticoagulant therapy that had been performed after PVE onset still needs further studies.

Abstract:
A 59 year-old white male with a life long history of severe bleeding following trauma or surgical procedures was documented to have a combined factor V and factor Vlll-C deficiency. His baseline factor V ranged between 16% - 30% and factor VIII-C was between 20% - 30%. His APTT ranged between 50 - 56 seconds (21-31 N) with a prothrombin time activity between 33% - 40% of normal ( ≥ 70% N). Factor II, Vll/X, X, IX, XI, XII, template bleeding time and platelet function studies were normal. There was no severe factor XIII deficiency. Since the response of these patients to DDAVP is unknown and the patient was admitted with a large hematoma in the subolecranon bursa, DDAVP was infused in a dose of 0.5 μ g/kg body weight over 15 minutes. The baseline factor VIIl-C of 20% rose to 55% at 5 minutes after termination of DDAVP and to 62% and 66% at 3 and 6 hours respectively. The factor VIII-C level dropped to 45% and 36% at 12 and 24 hours respectively after a single dose of DDAVP. As measured by a sensitive 125I-fibrin assay, this dose of DDAVP caused a net rise in plasminogen activator activity of 2.43 CTA U/ml.The patient had worn an inguinal truss for approximately 20 years for two large oblique inguinal hernias, one of which entended to the level of the mid-thigh. Pre and peri-operative management of the right inguinal herniorrhaphy consisted of DDAVP in a dose of 0.5 μg/kg Q 12 hourly for two doses, FFP at 9 ml/kg Q 12 hourly for three doses, and Amicar for 48 hours starting post-operatively. This regimen maintained the factor VIIl-C at ∼ 50% with factor V between 44% - 50% for a period of three days. On the 4th postoperative day a left inguinal herniorrhaphy was accomplished with DDAVP and FFP (dosage similar to previous) administered Q 12 hourly for three doses, then once in the next 24 hours, and Amicar for three days. A 4 cm wound hematoma noted on the first day of the second surgical procedure was evacuated, and was due to the presence of a bleeder since Vlll-C and V levels were higher than the values indicated on the first procedure. No red cell transfusions were given; fluids were restricted to 600 ml per day for 24 hours after the last dose of DDAVP. Successful bilateral inguinal herniorrhaphies without significant hemorrhages was achieved with exposure to a minimal volume of blood products.

Previous research has shown that a Homogeneous Charge Compression Ignition (HCCI) engine with efficient heat recovery can operate on a 35 to 65% volumetric mixture of ethanol-in-water while achieving high brake thermal efficiency (∼39%) and very low NOx emissions [4]. The major advantage of utilizing hydrated ethanol as a fuel is that the net energy gain improves from 21 to 55% of the heating value of ethanol and its co-products, since significant energy must be expended to remove water during production. This is required because wet ethanol is not suitable for conventional combustion engines. For example, spark ignition engines demand the use of pure ethanol because the dilution caused by water reduces the flame speed, resulting in misfire and problems due to condensation. The present study uses numerical simulations to explore the use of wet ethanol for Reactivity Controlled Compression Ignition (RCCI) operation in a heavy duty diesel engine. RCCI uses in-cylinder blending of a low reactivity fuel with a high reactivity fuel and has demonstrated significant fuel efficiency and emissions benefits using a variety of fuels, including gasoline and diesel. Combustion timing is controlled by the local blended fuel reactivity (i.e. octane number), and the combustion duration can be controlled by establishing optimized gradients in fuel reactivity in the combustion chamber. In the present study, the low reactivity fuel was hydrated ethanol while the higher reactivity fuel was diesel. First, the effect of water on ethanol/water/diesel HCCI was investigated using GT-Power and single-zone CHEMKIN simulations. The results showed that the main impact of the water in the ethanol is to reduce the IVC temperature due to vaporization cooling. Next, multidimensional engine modeling was performed using the KIVA code at engine loads from 5 to 17 bar IMEP at 1300 rev/min with various grades of hydrated ethanol and a fixed diesel fraction of the total fuel. The results show that hydrated ethanol can be used in a RCCI engine with gross indicated thermal efficiencies up to 55% and very low emissions. A 70/30 ethanol/water mixture (by mass) was found to yield the best results across the entire load range without the need for EGR.

Abstract:
Objetivo: Evaluar la eficacia y tolerabilidad del tratamiento con Vortioxetina en el trastorno depresivo mayor en comorbilidad con trastorno por consumo de sustancias. Metodología: 18 pacientes, de entre 21-55 años, diagnosticados de trastorno depresivo mayor y trastorno por consumo de sustancias (6 por consumo de alcohol, 6 por consumo de cannabis y 6 por consumo de cocaína) (criterios diagnósticos DSM-5) que inician tratamiento con Vortioxetina. Evaluación basal, a los 1, 3 y 6 meses de las siguientes escales y variables: a)Inventario de depresión de Beck, b) Cambio del consumo de sustancias durante el tiempo de seguimiento, evaluado a través del Timeline Follow-Back (TLFB) y c) Auto-reporte de efectos secundarios. Resultados: - Escala de Depresión de Beck: reducción estadísticamente significativa en la puntuación media, 21,67 (±3,36) en la visita basal, 5,21 (±4,76) a los 6 meses (Δ=16,47±5,23; p<0,01). Siendo independiente de la sustancia de consumo. - Reducción en el consumo de sustancias al final de estudio: A) Alcohol: Media basal de 100,53 g/día (± 59,28), a 25,3 g/día (± 21,46) (Δ=75,23 ±37,97; p<0,01), B) Cannabis, media balsa de 4, 99 g/día (± 2,22) a 2,4 g/día (± 1,47) (Δ=3,52 ± 1,51; p<0,05) y C) Cocaína, de 2,55 g/día de media (± 3,56) en la visita basal, a una media de 1,89/día (± 3,22) en la visita final. (Δ=0,66 ± 2,89). La reducción estadísticamente significativa para alcohol y cannabis. Tolerabilidad: La tasa de continuidad del tratamiento fue de un 66,66% y el efecto adversos mas frecuente fueron las náuseas 44,4% Conclusión: En esta serie de casos, vortioxetina resulta eficaz y bien tolerado como parte del abordaje integral del paciente con trastorno depresivo mayor y trastorno por consumo de sustancias.
