Abstract:
Catfish is one of the nine potential leading commodities in Indonesia. The biggest demand of the fish reaches 250 tons a day in Java and this makes the fish breeding business very important. One of the catfish breeding areas is Babakan Seeng, Bogor District. There are two catfish verieties, i.e Dumbo, which is cultivated in Pokdakan Jumbo Lestari and Sangkuriang in Pokdakan kubang sejahtera. The breeding system of those varieties is different: stocking density of 10 000 juveniles per 300 m2 for Dumbo and 20 000 juveniles per 300 m2 for Sangkuriang, due to the bigger egg hatching expectation; i.e 90%. The research showed that the income of Dumbo catfish farm was Rp 7 544 531, while that of the sangkuriang catfish farm was only Rp 3 857 981. R/C ratio of Dumbo catfish farm was 1.49 and R/C ratio of Sangkuriang catfish farm was 1.16. Chi-squares analysis also proves that the difference the way aquaculture can effect farm income. This proves that Dumbo catfish farm is more profitable compared to Sangkuriang farm. Sangkuriang catfish hatchery can be more profitable when the stocking density is lessened.

Out of screened 280 lines,one line KMH-152 was found resistant ,77 lines were identified as moderately resistant,one line Kaveri -50 (check) was severely affected by Charcoal rot with 8.0disease rating and rated as susceptible during kharif, 2022.

Patients with transthyretin cardiac amyloidosis (TTR CA) suffer from impaired exercise capacity, have a poor quality of life (QoL), and approved treatments are lacking. Stimulators of the soluble guanylate cyclase are promising new pharmaceuticals in the treatment armamentarium of heart failure patients. The aim of the present study was to report on the safety and efficacy of riociguat administration in patients with TTR CA. TTR CA patients received riociguat for 4–6 months within the frames of a national named patient use (NPU) program. Parameters of interest included changes in submaximal exercise capacity, invasive hemodynamic parameters, and QoL. Between March 2012 and June 2017, 86 CA patients were screened for the NPU program, of whom 13 TTR CA patients were eligible for participation. In our study cohort, riociguat had an acceptable tolerability profile. At follow-up, we could detect slight improvements in median 6-min walk distance (396 m [interquartile range (IQR) = 340–518] vs. 400 m [IQR = 350–570], P = 0.045), New York Heart Association class ≥ III (n = 7 [53.9%] vs. n = 0 [0.0%], P = 0.031), cardiac output (4.3 L/min [IQR = 3.9–5.1] vs. 4.5 L/min [IQR = 4.2–5.1], P = 0.022), diastolic pressure gradient (1.0 mmHg [IQR = −1.5–3.0) vs. −1.0 mmHg [IQR = −3.0–1.0], P = 0.049), and QoL (50.0% [IQR = 40.0–58.0] vs. 60.0% [IQR = 50.0–75.0], P = 0.021). Pulmonary arterial pressures were not altered. The present case series of TTR CA patients indicates that riociguat administration was safe and associated with minor clinical as well as hemodynamic improvements.

Abstract:
Parameters of external respiration are considered for white whales Delphinapterus leucas living in captivity on the observations in 2009-2012. Low respiration rate (on average 2.2 ± 0.3 breath per minute) and short apnea phase (no more than 1.7 minutes) were observed in the first days after the catch, but both parameters returned to normal values (3.0 ± 0.2 breath per minute, apnea up to 5.1 minutes) in the next 10-34 days. The respiration rate in the morning is higher in comparison with the evening. The respiration rate dependence on the whales motor activity, their age, and water temperature is discussed, as well.

Background: The aim of my study is to evaluate post-operative pain reliefon patients who had hemorrhoidectomy. Materials and Methods: 300 patients who hadhemorrhoidectomy were divided equally in to three groups, according to anesthesia type,group 1 (local anesthesia and sedation), while spinal anesthesia was group 2 and generalanesthesia was considered to be group 3. Pain relief, post-operative complications, hospitalstaying time were measured and compared between the three groups. Period: Study wasperformed between Jan 2012 to Dec 2014. Results: The study showed that patients who hadlocal anesthesia infiltration and sedation a significant decrease of post-operative total painscores at 6/12/18/24 hours of more than 50%,200/240/300/320 out of 1000 points in group IIas compared to 420/500/540/580,700/680/660/660 in 3rd groups respectively. The total postoperativeanalgesia doses in the 3 groups were 120:140:180 respectively, total hospital stayingtime were 130:210:260 days, headache in the ratio of 0:8:1, urine retention in 0:6:1 patients,nausea and vomiting in 0:1:5 patients were reduced by 30 %,. P-value < 0.05. On the otherhand, spinal anesthesia which is group 2 showed slight decline of patients numbers who hadrespiratory symptoms, hypotension and urticarial.Conclusion: Post-operative pain, analgesia,total cost, hospital staying time, nausea and vomiting have been significantly reduced by localanesthesia infiltration compared to non-infiltrated groups while spinal anesthesia had a higherrate in post-operative urine retention, headache and hypotension compared to local anesthesiawith sedation and general anesthesia. Respiratory symptoms, urticaria, were slightly reduced inlocal anesthesia with sedation compared to spinal and general anesthesia.

Abstract: The objective of this work was to evaluate the productivity and quality of papaya fruits (Carica papaya) of the Solo (12 genotypes) and Formosa (9 genotypes) heterotic groups over two harvest seasons. The experiment was conducted under field conditions in the municipality of Pinheiros, in the state of Espírito Santo, Brazil, in a randomized complete block design. Ten morpho-agronomic and physicochemical variables of the fruits were measured in winter (210-300 days after planting) and summer (450-540 days after planting). The data were subjected to the combined analysis of variance, to the breakdown of the genotype x environment interactions, and to the grouping of means by the Scott-Knott test. Although harvest season affected the evaluated characteristics, it did not alter the classification of the genotype. In the Solo group, the H 36-45 and UC 15 hybrids stand out due to their greater productivity, fruit weight, and soluble solids contents. In the Formosa group, the Rubi Incaper 511 cultivar shows greater productivity, higher number of commercial fruits, and lower number of deformed fruits over both evaluated harvest seasons.

Context College of American Pathologists (CAP) surveys are used to establish national benchmarks for laboratory parameters. Objective To evaluate changes in laboratory human papillomavirus (HPV) testing patterns in laboratories incorporating HPV testing with Papanicolaou tests in 2012. Design Data were analyzed from the CAP HPV Supplemental Questionnaire distributed to 1771 laboratories participating in either CAP HPV or CAP Papanicolaou proficiency testing in 2013. Results A total of 1022 laboratories (58%) responded. There were more high-risk (HR) HPV tests performed per institution as compared to previous surveys. There were more HPV tests performed within an institution as compared to previous surveys. Hybrid Capture 2 (HC2) remains the most common method (42.4%, 239 of 564); Cervista and cobas methods are used in 37.2% (210 of 564) and 14.9% (84 of 564) of laboratories, respectively. Human papillomavirus testing is offered as a reflex test after a Papanicolaou test result of atypical squamous cells of undetermined significance (ASC-US) in 89.6% of laboratories (476 of 531); as a cotest for women aged 30 years and older in 60.3% (404 of 531); as reflex testing after atypical squamous cells, cannot exclude high-grade squamous intraepithelial lesion (ASC-H) in 42.7% (320 of 531); and as reflex testing after atypical glandular cells (AGC) in 27.3% (145 of 531). The HPV-positive rates for ASC-US and ASC-H were similar in 2012 and 2006. Cervista (49.2%, 88 of 179) and Roche cobas (27.4%, 49 of 179) are the most common methods used for genotyping. Most laboratories use the CAP Human Papillomavirus for Cytology Program for proficiency testing. Conclusions There was an increase in annual volume of HR-HPV testing with a shift toward in-house HR-HPV testing. Genotyping volumes also increased. HC2 and Cervista are most commonly used, with an increasing volume of Roche cobas testing. The most common indication for HPV testing among all laboratories was ASC-US reflex testing, but an increase in HPV cotesting was observed. The data provide an update into persisting and newer trends in HPV testing practices.

Blainvillea Cassini (1823: 493) (Asteraceae-Heliantheae), as recently circumscribed by Orchard (2012, 2015), is a pantropical genus comprising four species. Blainvillea calcicola Orchard (2012: 658) and B. cunninghamii (Candolle 1836: 540) Orchard (2012: 660) are endemic to Australia. Blainvillea gayana Cassini (1827: 90) is native to Africa and adventive in South America and Australia. Blainvillea acmella (Linnaeus 1753: 901) Philipson (in Koster & Philipson 1950: 350), the type species of the genus, is native to India and Sri Lanka, and adventive in northern Africa, Galápagos Islands, and Australia.

Objective: To determine the prevalence of benign vocal cord lesions among Filipino patients in a tertiary institution and identify the demographic characteristics and possible risk factors found among these patients. Methods Study Design: Retrospective case series Setting: Private tertiary hospital Subjects: Records of 2,375 patients who underwent laryngeal video endoscopy and stroboscopy from 2012-2014 were reviewed. Results: There were 632 records of patients with benign vocal fold lesions, of which nodules were most common (211, 33.38%), followed by Reinke’s edema (165, 26.10%), cysts (122, 19.30%), and polyps (74, 11.70%), with hoarseness as the most common symptom (542, 85.76%). More than half (336, 53.16%) were aged 21-40 years, and almost two-thirds (469, 74.21%) were female. The most common associated factors were caffeine intake (445, 70.41%) and inadequate water intake (370, 58.54%), followed by alcohol (253, 40.03%). Smoking was only present in 146 (23.19%). Conclusion: Baseline evidence on the prevalence of benign vocal fold lesions in this institution as well as baseline data on the common characteristics and associated factors seen in the sample population may assist us in current practices and guide future studies directed toward treatment and prevention. Abstract:
This article presents and analyzes the content of the monograph by L. B. Khavzhokova «Evolution of the system of Adyghe versification», published in Nalchik in 2022. Reviewers consistently characterize all five chapters of the book, which discuss issues related to the genesis of Adyghe verse (from folklore to literary), its rhythm (stress, intonation, inversion, graphics), metric (iamb, trochee, dactyl, amphibrachium, anapest, logaed, free verse), rhyme and stanza. The fruitfulness of the author’s use of the comparative method in analyzing the poetic texts of Kabardian, Adyghe and Circassian poets with the identification of common and special features is noted. Processing of statistical data allowed the author to draw a conclusion about the harmonious synthesis of the models of European verse with the original Adyghe laws of versification. This author is recom-mended to use the analytical capabilities of modern digital technologies in further developments of problems of national versification.

A new Tb(III) complex, [Tb(L)3(2,2´-bipy)(DMF)]·(2,2´- bipy) (1), has been synthesized with 2-benzoylbenzoic acid (HL) and 2,2´-bipyridine (2,2´-bipy) as ligands. In 1, the Tb(III) ion is coordinated by nine atoms to give a distorted tri-capped trigonal-prismatic geometry. The magnetic and fluorescence properties of 1 are discussed. Compound 1 is paramagnetic in the temperature range of 300 - 24 K, and when the temperature decreases from 22 to 2 K, it exhibits weak ferromagnetism. The compound shows four intense fluorescence emission bands arising from the transitions of Tb3+: 5D4→7F6 (492 nm), 5D4→7F5 (546 nm), 5D4→7F4 (590 nm) and 5D4 →7F3 (622 nm) with an excitation wavelength of 315 nm.

This research attempted to find out the view of Islamic law in modern fiqh studies on plastic surgery and Appearance oriented views (Lookism) in South Korea. The use of juridical empirical method, or also known as sociological research, aimed to provide an overview of the implementation of plastic surgery in society based on applicable law by analyzing problems in society through secondary data.According to the data from the International Society of Aesthetic Plastic Surgery (ISAPS) in 2019, South Korea was in fifth position as a country that is estimated to carry out the most plastic surgery procedures in the world, 2,571 times (5.1%). This shows that this country has an Appearance-oriented view (Lookism) which is part of the Lookism culture. According to Bio Med Central 2017 data, there was a high ratio of recurrent discrimination rates (OR: 3.70; 95% CI: 2.19-6.27) and ‘incident’ (OR: 3.10; 95% CI: 2.99-4.83) from 2005 to 2013 against individuals in the age group of 15-24 years.

BRCA2 mutations (g BRCA2m) associate with poor prognosis from localized PC, but prognostic and predictive value for standard therapy in CRPC is unclear. Response (PSA50%/RECIST) and PFS (RECIST/PSA progression or start of a new therapy due to clinical progression) were collected for Abiraterone, Enzalutamide and Docetaxel. To account for potential differences between cohorts, a mixed effect model (Weibull distribution) with random intercept per cohort was pursued. Results: dDDRm status was available for n = 390 (60 gDDRm+, including 37 g BRCA2m, and 330 gDDRm-). Overall, 74% and 69% received Docetaxel and Abiraterone/Enzalutamide respectively; 47% gDDRm+ and 34% gDDRm- received PARPi and/or platinum. Median overall survival from CRPC was 3.0 vs 3.2 years in gDDRm+ vs gDDRm- (p = 0.73; g BRCA2m = 3.0 years, p = 0.72). Age and Gleason score at diagnosis were associated with survival from castration-resistance in multivariate analysis. Median PFS on Docetaxel for gDDRm+ (6.8 months; 6.3 for g BRCA2m) and gDDRm- (5.1 months) were not significantly different (p = 0.2). Similarly, RR to Docetaxel was similar for the two groups (61% vs 54% in gDDRm+ vs gDDRm-; 63% g BRCA2m). Median PFS and RR on first Abiraterone/Enzalutamide were similar across groups (PFS: 8.3 months gDDRm+, 8.3 months for gDDRm-; p = 0.9; RR 46% vs 56% respectively) The Interaction of PARPi/platinum therapy among gDDRm+ patients resulted in an aHR for OS from CRPC of 0.59 (95%CI 0.28-1.25; p = 0.17). Conclusions: In this retrospective analysis, CRPC patients with gDDRm still benefited from standard therapies similarly to non-mutation carriers; interpretation of survival data should consider the high proportion treated with PARPi/platinum.

Aim: to investigate relationship between arterial hypertension (AH) and risk factors / subclinical damage of target organs in patients with type 2 diabetes mellitus (DM2). Methods. We included into this clinical epidemiological study 528 patients with DM2 (30.5 % men, 69.5 % women; mean age 54.1±0.3 years; 80.3 % with AH, 19.7 % without AH), who answered questions of the ARIC study questionnaire related to risk factors. Also, we studied features of target organ damage and laboratory indicators. Results. In comparison with normotensives patients with AH more frequently had ischemic heart disease (12.7±1.6 % vs. 5.8±2.3 %, p<0.05), chronic heart failure (CHF) (30.9±2.2 % vs. 9.6±2.9 %, p<0.001), atherosclerosis of vessels of lower extremities (69.8±2.2 % vs. 53.8±4.9 %, p<0.01) and cerebral vessels (50.9±2.4 % vs. 28.8±4.4 %, p<0.001), history of stroke (5.0±1.1 % vs. 0 %, p<0.05), hypertonic angiopathy (14.5±1.8 % vs. 6.5±2.5 %, p<0.05), low level of high density lipoprotein (87.3±2.2 % vs. 74.5±6.4 %, p<0.05), electro- and echocardiographic signs of left ventricular hypertrophy (75.6±2.1 % vs. 45.4±5.1 %, p<0.001; 61.1±2.6 % vs. 24.4±4.7 %, p<0.001, respectively), lowering of left ventricular ejection fraction (12.5±1.7 % vs. 7.8±2.8 %, p<0.001), diastolic disfunction of the left ventricle (52.6±2.7 % vs. 23.2±4.7 %, p<0.001), atherosclerosis of the aorta (38.0±2.6 % vs. 20.7±4.5 %, p<0.01), lowering of the ankle-brachial index (left – 29.8±2.3 % vs. 14.9±3.5 %, p<0.01; right – 31.5±2.3 % vs. 9.9±3.0 %, p<0.001, respectively), increased intima-media thickness of the right carotid artery (84.6±5.0 % vs. 60.0±11.0 %, p<0.05). Conclusion. In patients with type 2 diabetes and AH, in order to develop strategy of macro- and microvascular complications prevention, it is necessary to conduct early screening of risk factors and subclinical damage of target organs.

Abstract. A total of 1,33,507 eggs obtained from 7864 pullets of two selected White Leghorn strains over four generations representing 304 sires and 1669 dams were incubated. The unhatched eggs were broken-open for examination of embryonic development and classified as early-dead (0-11d), late-dead (12-22d) and pips. The sire component heritabilities (%) pooled over years in the two strains, respectively were 10.5±3.4 and 11.0±3.2 for total incubation mortality; 2.2 ± 2.2 and 5.1 ± 2.4 for early-dead; 6.8 ± 3.0 and 8.0 ± 3.0 for late-dead and 8.7 ± 3.1 and 4.8 ± 2.4 for pips. Total embryonic mortality as well as its three components had high genetic correlations amongst each other. However, the genetic correlations of pipping mortality with early and late mortality were weaker as compared with those of early and late mortality.

Objetivou-se, com este estudo, analisar o efeito da adubação nitrogenada de cobertura na qualidade do caldo para produção de etanol de quatro cultivares de sorgo sacarino. O experimento foi conduzido em Campos dos Goytacazes- RJ, de janeiro a maio de 2014. Foram estudadas quatro cultivares de sorgo sacarino: BRS 506, BRS 508, BRS 509 e BRS 511 e cinco de doses de nitrogênio (0, 80, 160, 240 e 320 Kg ha-1), aplicadas aos 25 e 45 dias após semeadura. Os resultados mostraram que valor Brix, açúcares redutores totais, açúcares totais recuperáveis, volume do caldo e rendimento de etanol foram afetados pela adubação nitrogenada. Para todas as características, o valor máximo foi obtido pela dose de 240 kg de N ha-1. Entre as cultivares, a BRS 506 mostrou-se superior em produção de caldo e rendimento de etanol. Enquanto a BRS 511 destacou-se nos atributos ART e ATR, e a cultivar BRS 508 sobressaiu-se no valor Brix. A cultivar BRS 506 apresentou melhor resultado combinado, com maior volume de caldo e rendimento de etanol com as doses de 204 e 212 kg N ha-1, respectivamente. The results showed that the value Brix, total reducing sugars, total recoverable sugars, broth volume and ethanol yield were affected by nitrogen fertilization. To investigate the correlations between different thicknesses and heat treatments, this study used a sputtering method to create CoFeY films. The results of X-ray diffraction (XRD) revealed the appearance of oxide peaks at 2θ = 47.7°, 54.5°, and 56.3° in agreement with YFeO3 (212), Co2O3 (422), and Co2O3 (511), respectively. The findings also demonstrated a relationship between the low-frequency alternative-current magnetic susceptibility (χac) values and the thickness of the CoFeY thin films. At a thickness of 50 nm and an annealing temperature of 300 °C, the ideal value of ac was 0.159. The presence of Y and the thickness impact were both evident in the χac value, which improved spin-exchange coupling as well as grain refining. With increasing thickness, the resistance decreased. At 300 °C and 40 nm in thickness, this film has a maximum surface energy of 31.2 mJ/mm2. The hardness of the 50-nm films reached a maximum of 16.67 GPa when annealed at 100 °C. Due to the high χac, strong adhesion, good nanomechanical properties, and low resistivity, the optimal conditions were determined to be 50 nm with annealing at 300 °C.

AbstractThe Tern Oilfield is situated 150 km northeast of the Shetland Islands in Block 210/25a in the UK sector of the northern North Sea. The discovery well 210/25-1 was drilled in 1975 in a water depth of about 541 ft. The trap is defined at around 8000 ft TVSS by a tilted horst-structure. The hydrocarbons are contained in reservoirs belonging to the Middle Jurassic Brent Group sands deposited by a wave-dominated delta system in the East Shetland Basin. Complex faulting of the structure is responsible for the division of the field into two areas with different original oil-water contacts: the Main Area of the field with an oil-water contact at 8260 ft TVSS, and the Northern Area with a possible oil-water contact at 8064 ft TVSS. Reservoir quality is good with average porosities ranging from 20-24% and an average permeability of 350 md. The expected STOIIP and ultimate recovery of oil are 452 and 175 MMBBL, respectively which represents a recovery factor of 39%.The initial stage of the development plan calls for ten wells, five oil producers and five water injectors, to be drilled from a single platform, Tern Alpha. Development drilling started in February 1989 and first oil was produced on 2 June 1989. The oil is evacuated via the North Cormorant and Cormorant Alpha platforms into the Brent System pipeline for export to the Sullom Voe terminal.To date, two producers have b een drilled and total cumulative production is 6.4 MMBBL (1 January 1990). Ultimate recovery is estimated to be some 175 MMBBL.

In this study molecular characterization of β-tubulin isotype 3 gene in Fasciola hepatica isolates from cattle and sheep in Turkey was carried out. For this purpose a total of 80 adult Fasciola hepatica isolates were collected from 20 sheep and 20 cattle in Kayseri and Erzurum provinces. PCR-RFLP was performed on β-tubulin isotype 3 gene and MboII revealed two fragments of approximately 350 bp and 390 bp, whereas HphI enzyme yielded 210, 340 and 540 bp bands, HindII yielded 380 and 450 bp bands in all samples. A total of 80 isolates were tested by SSCP and all of them presented the same band profiles. Six samples (4 sheep and 2 cattle) were randomly selected and DNA sequence of a 935 bp coding fragment of β-tubulin isotype 3 was performed. Sheep samples were more polymorphic than the cattle. This β-tubulin isotype 3 gene polymorphism of F.hepatica isolates from sheep and cattle of two distinct geographical areas of Turkey have been investigated for the first time.

A cDNA probe encoding the entire structural region of the 62-kDa rat intestinal alkaline phosphatase from amino acid residues 1 to 531 detected multiple mRNA species (3.0, 2.7, and 2.2 kb) in rat intestinal RNA. The 3.0-kb species was most evident in duodenum but could be easily detected in jejunum using a 48-mer oligonucleotide encoding amino acid residues 492-508. This 48-mer oligonucleotide bound preferentially to the 3.0-kb mRNA, suggesting that the 2.7-kb mRNA differed in this region. To determine whether each of the mRNAs encoding rat intestinal alkaline phosphatase responded coordinately to physiological stimuli, the full-length cDNA and the 48-mer oligonucleotide were used as probes for the 2.7- and 2.2-kb and the 3.0-kb mRNAs, respectively. Intestinal mRNA concentration was measured by Northern blot analysis in acute (single feed, 17 kcal) and chronic (3 wk, 30% fat diet) fat feeding and in rachitic rats after 1,25-dihydroxyvitamin D3 therapy. There was a large increase (8- to 25-fold) in the 3.0-kb mRNA 7 h after acute fat feeding, with a much smaller increase (1.4- to 5.0-fold) in the 2.7- and 2.2-kb species. The peak in 3.0-kb mRNA accumulation correlated in time with the maximal activity of serum phosphatase activity after acute fat feeding (4- to 5-fold increase). The paper presents the results of the assessment of the efficiency of Volnik Smart, WS herbicide (glyphosate, 545 g/l), Frandesa Co., Ltd applied in the fields for different crops (spring cereals, potato, vegetables, industrial, oil and medicinal crops and others). Volnik Smart, WS herbicide applied in a dose of 1,3–1,8 l/ha was effective against annual cereal and dicotyledonous weeds; when applied in a dose of 2,2–2,6 l/ha it destroyed effectively perennial cereal and dicotyledonous weeds. When the herbicide was applied in a dose of 1,3–1,8 l/ha the number of weeds decreased from 85,6 to 95,7 % and their vegetative mass – from 77,8 to 95,8 % in a month after the treatment. When it was applied in a dose of 2,2–2,6 l/ha the number of weeds decreased from 94,0 to 99,3 % and their vegetative mass – from 88 to 100 %.

Aim. To develop an algorithm for diagnosis and integrated treatment of the poor people of the Republic of Bashkortostan, taking into account the possibility of using state support programs. Methods. The study sample included 296 low-income patients (women over 58 years, men over 63 years) receiving social pension supplements. As a comparison group, 300 patients of retirement age, who had paid for orthodontic treatment by themselves, were recruted. Results. Out of 296 patients, 211 (71.28%) had previous orthopedic treatment, the remaining 85 (28.72%) had never received orthopedic treatment. All poor patients received treatment according to the full comprehensive program of preferential dentures. Orthopedic treatment using the non-removable prosthesis was performed in 39 (13.17%) patients, removable dentures - in 27 (9.12%), combined prosthetics - in 230 (77.70%). In 300 patients who paid for orthopedic treatment from their own funds, 133 (44.33%) patients got removable prosthesis, 70 (23.33%) patients - non-removable dentures, combined prosthetics was used in the remaining 97 (32.33%). An algorithm of therapeutic and preventive measures for low-income citizens in order to improve oral health and quality of life, as well as to optimize the use of financial resources, was developed. It consists of five stages: I - examination of the anamnesis vitae and the history of the disease; II - Diagnosis and treatment of comorbidities; III - full mouth debridement; IV - selection and rational prosthetics, medical treatment; V - follow-up examination in 1 and 6 months. An integral indicator of quality of life after the conducted complex treatment has improved by 15.11 points (27.0%). Some criteria for the quality of life has improved by more than 2 points (3.51%): «Physical discomfort» - by 2.133, «physical disorders» - by 2.916, «psychological disorders» - by 2.855 points. Conclusion. The program of preferential dentures allows to expand the range of the treatment options with the use of new technologies; implementation of this program is not a direct source of health improvement, but serves as an auxiliary arm of social support and helps to minimize the severe health consequences for the examined population.

A water-soluble porphyrin, 5,10,15,20-tetrakis[3,4-bis(carboxymethyleneoxy)phenyl] porphyrin (T3,4BCPP), was successfully labeled with 99 m Tc and biodistribution studies were performed in Wistar rats. Scintiimaging and in vivo distribution studies were also carried out in C 6-gliomas and mammary tumor-bearing animals using a gamma camera. Tumor-to-muscle (T/M) ratios were calculated and compared with those obtained with the known tumor-seeking radiopharmaceuticals 99 m Tc (V)- DMSA (DMSA = dimercaptosuccinic acid), 99 m Tc -citrate and 201 TlCl . In the case of C 6-gliomas, the ratios were 4.2, 2.2, 4.00 and 3.0; while in the case of C 3 H / J mammary tumor, the ratios were 9.4, 8.8, 8.1 and 8.5 for T3,4BCPP, 99 m Tc ( V )- DMSA , 99 m Tc -citrate and 201 TlCl , respectively. Similar studies were carried out in N-methyl-N-nitrosourea ( NMU )-induced mammary tumor animals and the T/M ratios obtained were 5.9, 2.0, 5.3 and 3.3 for T3,4BCPP, 99 m Tc ( V )- DMSA , 99 m Tc -citrate and 201 TlCl , respectively. The radiolabeled photosensitizer could perhaps be used to detect cancer non-invasively and could even prove useful in monitoring the progression/regression of tumors before, during, and after chemotherapy, radiation therapy or photodynamic therapy (PDT).

The excitation wavelength of conventional Tb3+-activated phosphor is near 270 nm. This study describes novel green-emitting Tb3+-activated amorphous calcium silicate by ultraviolet excitation at 378 nm. The Tb3+-activated amorphous calcium silicate was prepared by heating a sample of Tb3+-activated calcium silicate hydrate (CSH) at 900°C for 30 minutes. The emission wavelength of the resulting phosphor was 544 nm. The optimum excitation wavelength within the range 300–400 nm was 378 nm. The Tb3+-activated amorphous calcium silicate emitted green by ultraviolet irradiation. The optimum initial Tb/Ca atomic ratio of this phosphor was about 0.5. A mechanism for the action of the phosphor is proposed, in which Tb3+ions existing in the layer of the CSH lead to loss of water molecules and OH groups.

AbstractSARS-CoV2-induced direct cytopathic effects against type II pneumocytes are suspected to play a role in mediating and perpetuating lung damage. The aim of this study was to evaluate serum KL-6 behavior in COVID-19 patients to investigate its potential role in predicting clinical course. Sixty patients (median age IQR, 65 (52–69), 43 males), hospitalized for COVID-19 at Siena COVID Unit University Hospital, were prospectively enrolled. Twenty-six patients were selected (median age IQR, 63 (55–71), 16 males); all of them underwent follow-up evaluations, including clinical, radiological, functional, and serum KL-6 assessments, after 6 (t1) and 9 (t2) months from hospital discharge. At t0, KL-6 concentrations were significantly higher than those at t1 (760 (311–1218) vs. 309 (210–408) p = 0.0208) and t2 (760 (311–1218) vs 324 (279–458), p = 0.0365). At t0, KL-6 concentrations were increased in patients with fibrotic lung alterations than in non-fibrotic group (755 (370–1023) vs. 305 (225–608), p = 0.0225). Area under the receiver operating curve (AUROC) analysis showed that basal KL-6 levels showed good accuracy in discriminating patients with fibrotic sequelae radiologically documented (AUC 85%, p = 0.0404). KL-6 concentrations in patients with fibrotic involvement were significantly reduced at t1 (755 (370–1023) vs. 290 (197–521), p = 0.0366) and t2 (755 (370–1023) vs. 318 (173–435), p = 0.0490). Serum concentrations of KL-6 in hospitalized COVID-19 patients may contribute to identify severe patients requiring mechanical ventilation and to predict those who will develop pulmonary fibrotic sequelae in the follow-up.

Abstract:
A study of batch fermentation of green algae (Spirogyra majuscula Kuetz) from Pengging Lake, Boyolali, Central Java for bioethanol source using immobilization technique of Ca-alginate for Saccaromyces cerevisiae entrapment has been done. The scope of the study emphasized on the best condition for the processes of hydrolysis and fermentation. Concentration of sulfuric acid and hydrolysis time were varied with 0.1, 0.2, 0.3, 0.4 and 0.5 M for 30, 90, 150, 210, 270, 330, 360, 390, 420, and 450 min to obtain the maximum glucose content of UV analysis. Na-alginate : yeast ratio and fermentation time were varied with 1:5, 2:4, 3:3, 4:2 and 5:1 (w/w) for 1, 2, 3, 4, 5, 6 and 7 days. Distillation at 70-80 °C was deployed to purify the fermentation product. The ethanol content in the product was analyzed using gas chromatography-flame ionization detector (GC-FID). The result of study showed that maximum glucose content was obtained 2.1% from 0.2 M sulfuric acid for 6 h of hydrolysis. Maximum ethanol content was obtained 54.1% from 2:4 ratio of Na-alginate : yeast (w/w) for 4 days of fermentation. The study also concludes that immobilization technique of Ca-alginate increase alcohol content compared to without immobilization of green-algae fermentation.

ABSTRACT Objective: To evaluate the direct costs of transforaminal lumbar interbody fusion (TLIF) and minimally invasive surgery (MIS) or open technique (OPEN). Methods: The present study is descriptive and retrospective. Sixteen patients with degenerative spinal pathology operated on with the TLIF MIS technique and TLIF OPEN were included over a 13-month period. Days of hospital stay, blood loss, surgical time, medical care and costs were compared. Results: The mean number of days of hospital stay was 6.7 ± 4.3 days with TLIF MIS and 11.1 ± 6.5 days with TLIF OPEN. The blood loss was 307 ± 81.6 ml (range 200400 ml) with TLIF MIS and 803 ± 701.3 ml (range 200-1800 ml) with TLIF OPEN. The surgical time was 320 ± 92.6 minutes (range 210-500 minutes) in TLIF MIS and 372 ± 95.2 minutes (280-540 minutes) in TLIF OPEN. Conclusions: The difference in surgical costs and time between the two procedures was not statistically significant. There was less bleeding during the TLIF-MIS surgery, as well as a correlation between shorter days of hospital stay proportional to bleeding and surgical time, which translates into a reduction in the cost of these items. Abstract. The disappearance rate of [D-Ser(t-bu)6,des-Gly10]GnRH ethylamide (Buserelin®, HOE 766) was studied in plasma and urine after intranasal (300 μg) or sc (10 μg/kg) administration. A radioimmunoassay for HOE 766 was developed using 125I[D-Trp6,Des-Gly10]GnRH ethylamide as tracer and an antiserum raised against HOE 766. Cross-reaction with native GnRH was only 1.7%. Sensitivity was 1 pg/tube. In 6 male adolescents, the mean plasma HOE 766 concentration (± sem) was 0.46 ± 0.08, 0.50 ± 0.10, 0.28 ± 0.04, 0.24 ± 0.04, 0.13 ± 0.03, and 0.08 ± 0.02 μg/l 30, 60, 90, 120 and 180 min after the intranasal administration, respectively. Concomitant urinary excretion of HOE 766-like material was 9.43 ± 1.96 μg/4 h. There was a good correlation between integrated plasma levels and urinary excretion (r = 0.92). In the same 6 volunteers, the plasma HOE 766 levels were 21.2 ± 3.0, 25.9 ± 0.8, 21.2 ± 0.9, 17.1 ± 0.7, 12.8 ± 1.1, 8.9 ± 0.4, and 5.9 ± 0.8 μg/l 20, 40, 60, 90, 120, 180 and 240 min after sc injection, respectively. The mean urinary excretion was 543 ± 61 μg/4 h. In two girls with precocious puberty treated during 12 to 15 months with intranasal administration of HOE 766, urinary excretion of HOE 766-like material was shown to correlate well with the degree of inhibition of plasma 17β-E2and of plasma LH and FSH responses to a GnRH challenge. Thus, monitoring of HOE 766 in urine appears to be helpful for evaluating of intranasal therapy with a GnRH analog in precocious puberty.

Background: Single photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI) provides excellent diagnostic and prognostic information. A primary limitation of SPECT is radiation exposure. New cadmium zinc telluride (CZT) technology has allowed for reduced radiation exposure. Methods: A cohort of 414 consecutive patients undergoing very low-dose SPECT MPI utilizing CZT technology was analyzed. Patients received rest/stress dosing strategies of 111/333, 148/444, 222/666, 296/888, 370/1110 Megabecquerels (MBq) (3/9, 4/12, 6/18, 8/24, 10/30 millicuries) depending on their weight and body mass index. Image quality was visually assessed by the reading cardiologist (scale: 1 = poor; 2 = fair; 3 = good; 4 = excellent). Results: 58% of patients received 111/333MBq, 22% received 148/444MBq, 13% received 222/666MBq, 6% received 296/888MBq, and 1% received 370/1110MBq. The estimated radiation dose was 3.6, 4.8, 7.2, 9.6, and 12 millisieverts respectively. The average image quality for the cohort was 3.3 ± 0.7. The rest/stress counts (103) for 111/333 and 148/444 groups were 532 ± 212/1718 ± 525 and 545 ± 177/1748 ± 399, while the rest/stress counts (103) for the 222/666 and 296/888 groups were 673 ± 178/2235 ± 923 and 764 ± 249/2588 ± 788 respectively. The 111/333 and 148/444 groups had significantly lower counts than the 222/666 and 296/888 groups, but only the 296/888 group had a significant difference in image quality when compared to the 111/333 group, with the 111/333 group having better image quality. Conclusion: CZT technology permits rapid, very low-dose MPI studies to be performed, thus, reducing the radiation exposure for patients, while still providing high quality, diagnostic perfusion results. The majority of our patients were imaged at less than 4mSv.

Background For the reconstruction of acromioclavicular (AC) joint separation, several operative procedures have been described; however, the anatomic reconstruction of both coracoclavicular ligaments has rarely been reported. Purpose The aim of this biomechanical study is to describe a new procedure for anatomic reconstruction of the AC joint. Study Design Controlled laboratory study. Materials and Methods Forty fresh-frozen cadaveric shoulders were tested. Cyclic loading and a load-to-failure protocol was performed in vertical (native, n = 10; reconstructed, n = 10) and anterior directions (native, n = 10; reconstructed, n = 10) on 20 AC joints and repeated after anatomic reconstruction. Reconstruction of conoid and trapezoid ligaments was achieved by 2 TightRope devices (Arthrex, Naples, Florida). Dynamic, cyclic, and static loading until failure in vertical (n = 5) and horizontal (n = 5) directions were tested in native as well as reconstructed joints in a standardized setting. Results The native coracoclavicular ligaments in static load for vertical force measured 598 N (range, 409–687), elongation 10 mm (range, 6–14), and stiffness 99 N/mm (range, 67–130); static load for anterior force was 338 N (range, 186–561), elongation 4 mm (range, 3–7), and stiffness 140 N/mm (range, 70–210). The mean maximum static load until failure in reconstruction for vertical force was 982 N (range, 584–1330) ( P = .001), elongation 4 mm (range, 3–6) ( P < .001), and stiffness 80 N/mm (range, 66.6–105) ( P = .091); and for anterior static force 627 N (range, 364–973) ( P < .001), elongation 6.5 mm (range, 4–10) ( P = .023), and stiffness 78 N/mm (range, 46–120) ( P = .009). During dynamic testing of the native coracoclavicular ligaments, the mean amount of repetitions (100 repetitions per stage, stage 0–100 N, 100–200 N, 200–300 N, etc, and a frequency of 1.5 Hz) in native vertical direction was 593 repetitions (range, 426–683) and an average of 552 N (range, 452–683) load until failure. In vertical reconstructed testing, there were 742 repetitions (range, 488–893) ( P = .222; with a load until failure of 768 N (range, 486–900) ( P = .095). In the anterior direction load, the native ligament failed after an average of 365 repetitions (range, 330–475) and an average load of 360 N (range, 307–411), while reconstructed joints ended in 549 repetitions (range, 498–566) (P = .008J with a load until failure of 547 N (range, 490–585) ( P = .008). In all testing procedures, a preload of 5 N was performed. Conclusion The anatomic reconstruction of the AC joint using TightRope is a stable and functional anatomic reconstruction procedure. The reconstruction technique led to favorable in vitro results with equal or even higher forces than native ligaments. Clinical Relevance Through anatomic repair, stable function of the AC joint can be achieved in an anatomic manner.

ImportanceLimited knowledge about interactions among health disorders impedes optimal patient care. Because comorbidities are common among patients 50 years and older with fractures, these fractures provide a useful setting for studying interactions among disorders.ObjectiveTo define multimorbidity clusters at the time of fracture and quantify the interaction between multimorbidity and fracture in association with postfracture excess mortality.Design, Setting, and ParticipantsThis nationwide cohort study included 307 870 adults in Denmark born on or before January 1, 1951, who had an incident low-trauma fracture between January 1, 2001, and December 31, 2014, and were followed up through December 31, 2016. Data were analyzed from February 1 to March 31, 2022.Main Outcomes and MeasuresFracture and 32 predefined chronic diseases recorded within 5 years before the index fracture were identified from the Danish National Hospital Discharge Register. Death was ascertained from the Danish Register on Causes of Death. Latent class analysis was conducted to identify multimorbidity clusters. Relative survival analysis was used to quantify excess mortality associated with the combination of multimorbidity and fractures at specific sites.ResultsAmong the 307 870 participants identified with incident fractures, 95 372 were men (31.0%; mean [SD] age at fracture, 72.3 [11.2] years) and 212 498 were women (69.0%; mean [SD] age at fracture, 74.9 [11.2] years). During a median of 6.5 (IQR, 3.0-11.0) years of follow-up, 41 017 men (43.0%) and 81 727 women (38.5%) died. Almost half of patients with fractures (42.9%) had at least 2 comorbidities. Comorbidities at fracture were categorized as low-multimorbidity (60.5% in men and 66.5% in women), cardiovascular (23.7% in men and 23.5% in women), diabetic (5.6% in men and 5.0% in women), malignant (5.1% in men and 5.0% in women), and mixed hepatic and/or inflammatory (5.1% in men only) clusters. These clusters distinguished individuals with advanced, complex, or late-stage disease from those with earlier-stage disease. Multimorbidity and proximal or lower leg fractures were associated with increased mortality risk, with the highest excess mortality found in patients with hip fracture in the malignant cluster (1-year excess mortality: 40.8% [95% CI: 38.1%-43.6%]). The combination of multimorbidity and fracture compounded the association with mortality, conferring much greater risk than either alone.Conclusions and RelevanceConcomitant illnesses were common and clustered into distinct multimorbidity clusters that were associated with excess postfracture mortality. The compound contribution of multimorbidity to postfracture excess mortality highlights the need for more comprehensive approaches in these high-risk patients. The analytical approach applied to fracture could also be used to examine other sentinel health events.

AbstractThis study examines the effect of irradiance level produced by solid-state light-emitting diodes (LEDs) on the growth, nutritional quality and antioxidant properties of Brassicaceae family microgreens. Kohlrabi (Brassica oleracea var. gongylodes, ‘Delicacy Purple’) mustard (Brassica juncea L., ‘Red Lion’), red pak choi (Brassica rapa var. chinensis, ‘Rubi F1’) and tatsoi (Brassica rapa var. rosularis) were grown using peat substrate in controlled-environment chambers until harvest time (10 days, 21/17°C, 16 h). A system of five lighting modules with 455, 638, 665 and 731 nm LEDs at a total photosynthetic photon flux densities (PPFD) of 545, 440, 330, 220 and 110 µmol m−2s−1 respectively were used. Insufficient levels of photosynthetically active photon flux (110 µmol m−2 s−1) suppressed normal growth and diminished the nutritional value of the Brassica microgreens studied. In general, the most suitable conditions for growth and nutritional quality of the microgreens was 330–440 µmol m−2 s−1 irradiation, which resulted in a larger leaf surface area, lower content of nitrates and higher total anthocyanins, total phenols and 2,2-diphenyl-1-picrylhydrazyl (DPPH) free-radical scavenging capacity. High light levels (545 µmol m−2 s−1), which was expected to induce mild photostress, had no significant positive impact for most of investigated parameters.

Abstract:
This research intended to study the effect of washing period of skipjack (Katsuwonus pelamis L) meat with cold water to the chemical and physical properties of surimi. The functional properties of surimi was tested either by physical or chemical analysis. In this study we measured folding test, texture and fat content. A complete randomized design were designated for this study with washing period as a treatment ( 0, 1, 2, 3, or 4 min). The average value of folding test were 2.2, 2.6, 3.0, 3.8 and 4.6, respectively. There is an increment of folding test value as the washing period increased. The average value of the texture analysis were 3.0, 3.8, 4.4, 5.1 and 5.9. Likewise, the longer the washing time, the firmer the texture. However, as the washing period increased, the fat content of surimi tend to decrease.

Araştırmaya, aktif olarak çeşitli branşlarda spor yapan 330 ergen sporcu (kadın n = 181; yaş = 14,8 ± 2,2; sporcu yaşı = 4,5 ± 6,0 ve erkek n = 149; yaş=14,2 ± 2,3; sporcu yaşı = 4,5 ± 2,6) ve 200 yetişkin sporcu (kadın n = 71; yaş = 21,9 ± 3,0; sporcu yaşı = 8,5 ± 4,6 ve erkek n = 129; yaş = 24,1 ± 5,1; sporcu yaşı = 9,0 ± 6,8) olan toplamda 530 ergen ve yetişkin sporcu gönüllü olarak katılmıştır. Egzersiz Keyfi ölçme aracı sekiz (8) madde ve tek faktörlü bir yapıdan oluşan egzersiz sırasında katılımcıların keyif düzeyini ölçmek için kullanılan bir bipolar ölçme aracıdır. Araştırmanın sonuçlarına göre yapılan doğrulayıcı faktör analizi sonucuna göre egzersiz keyfi ergen formunda tespit edilen uyum indeksleri (χ2/sd=4.106; p&lt;.01, SRMR=.045, RMESA=.097, GFI=.95, IFI=.96, CFI=.96 ve NFI=.95) ve egzersiz keyfi yetişkin formunda tespit edilen uyum indeks değerlerinin de (χ2/sd=2.431; p&lt;.01, SRMR=.048, RMESA=.085, GFI=.94, IFI=.95, CFI=.94 ve NFI=.91) olarak bulunmuştur. Araştırmanın sonuçlarına göre, egzersiz keyfi ölçme aracı yetişkin ve ergen sporcular için egzersiz sırasında, egzersizden alınan keyif duygusunun ölçülebilmesi için egzersiz ve spor alanında geçerli ve güvenilir bir ölçme aracı olarak araştırmacılar ve spor bilimciler tarafından kullanılabileceği söylenebilir.

Os transportadores do tipo helicoidais que equipam a maioria dos reboques graneleiros causam danos às sementes durante o processo de descarga, diferenciando-se entre implementos a partir de seus requisitos e parâmetros de projeto. Neste sentido, o presente estudo teve por objetivo avaliar os danos causados em sementes de soja durante o transbordo utilizando reboques graneleiros com diferentes capacidades de descarga. O experimento foi conduzido em delineamento estatístico blocos ao acaso em arranjo bifatorial (2 x 6 x 4) constituído pelos fatores reboques graneleiros e velocidades angulares de acionamento fornecidas pela da tomada de potência do trator (TDP). Os níveis de cada fator foram: dois reboques graneleiros (A e B); seis velocidades angulares da TDP (0, 300, 405, 432, 540 e 648 rpm). Os resultados demonstram que para cada reboque graneleiro, tomado isoladamente, não ocorreram diferenças estatística significativa para as variáveis sementes quebradas, amassadas e inteiras nas diferentes velocidades angulares na TDP utilizadas. No entanto, para o poder germinativo na velocidade angular padrão da TDP (540 rpm) não ocorreu variação significativa, sendo que no reboque B ocorrem os menores valores de plantas normais em relação à testemunha, podendo estar associado a maior taxa de descarga do reboque B, nas condições em que o experimento foi realizado.

Abstract Splenomegaly is an important measure of cancer progression and immune disorders. Magnetic resonance imaging (MRI) and computed tomography (CT) are the standard in the field but are not optimal for preclinical studies due to their high cost, low throughput and reliance on contrast agents for soft tissue. Here, we report the use of a novel 3D automated ultrasound scanner (Vega, Revvity Inc.) for the rapid assessment of spleen volume in two murine models (cancer and lupus). Cancer cohort: Tumor-bearing (4T1 breast cancer, right flank) mice were imaged alongside control mice at five timepoints over 25 days. Spleen volume was found to increase 18-fold (30±3 mm3 to 545±105 mm3) after 25 days of tumor growth (938±77 mm3). Lupus cohort: MRL/lpr mice were allowed to age until they developed lymphoproliferation (12-14 weeks) and then imaged biweekly until 20 weeks of age. A 300-fold increase in calculated spleen volume by 18 weeks of age in MRL/ lpr mice was observed. Kidney volume doubled and a statistical increase (p&lt;0.05) in vascular density was observed (using acoustic angiography), indicating systemic disease progression. Average acquisition times for both cohorts were 30 seconds per mouse, showing excellent applicability of spleen volume as a measurement of disease progression compared to more laborious techniques (e.g. MRI). Future work in this area will explore the application of machine learning and AI-assisted auto segmentation tools for higher throughput data analysis.

Data on the efficacy of Janus kinase inhibitors in patients with PsA with low joint counts are limited.Objectives:To evaluate the efficacy of upadacitinib (UPA) in subgroups of patients with PsA with a low (baseline swollen joint count [SJC] <5) or high (SJC ≥5) SJC (LSJ or HSJ).Methods:Data were pooled across the SELECT-PsA 11 (non-biologic disease-modifying antirheumatic drug [non-bDMARD] inadequate response [IR] or intolerance) and SELECT-PsA 22 (bDMARD IR or intolerance) trials, which both enrolled patients with ≥3 involved joints (SJC ≥3 and tender joint count [TJC] ≥3). Subgroup analysis was performed for patients with LSJ or HSJ treated with UPA 15 mg once daily (QD) or placebo (PBO). Efficacy endpoints included minimal disease activity (MDA), very low disease activity (VLDA), Psoriatic Arthritis Disease Activity Score (PASDAS) low disease activity (LDA; ≤3.2), PASDAS remission (≤1.9), and 20/50/70% improvement in American College of Rheumatology (ACR) criteria (ACR20/50/70), all at Week 24, and Psoriasis Area Severity Index (PASI) 75 and static Investigator Global Assessment of Psoriasis (sIGA) 0/1 at Week 16.Results:At baseline, patients with HSJ (n=1060) had similar demographic characteristics but tended to have higher overall disease activity than patients with LSJ across multiple disease domains (n=215; Table 1). UPA efficacy appeared comparable in patients with LSJ and HSJ, with similar proportions of patients achieving composite (MDA, VLDA, PASDAS LDA, and PASDAS remission) measures at Week 24, and skin endpoints (PASI 75 and sIGA 0/1) at Week 16 (Figure 1). INTRODUCTION:Since the creation of the National Committee for Health Technology Incorporation in the Brazilian Health System (CONITEC), a new phase started in the public Brazilian Health System (SUS): a continuous updating of the system based on Health Technology Assessment (HTA). CONITEC was created by federal law in 2012 and is responsible for advising the Ministry of Health regarding the incorporation or disinvestment of health technologies. The whole process involves a strong interaction with society, including the composition of the committee, which has the participation of the National Health Council. The objective of this study was to describe the results of CONITEC in five years of operation.METHODS:This is a retrospective descriptive study, based on information from the database (period 2012–2016) and CONITEC's website.RESULTS:Since 2012, CONITEC assessed 541 technologies, including drugs (360), health products (71) and procedures (110); 303 assessment requests came from SUS agencies and institutions and the other 238 requests from pharmaceutical companies, medical societies, patient associations and the judiciary bodies. In this period, there were 190 public consultations, during which more than 24,000 feedback from society were received. The average time for evaluation was 146 days. The committee recommended the incorporation of 186 technologies into SUS, the disinvestment of 43 and was unfavorable to the incorporation of 88, generating a budgetary impact of approximately BRL2.5 billion (USD764 million).CONCLUSIONS:From 2012–2016, CONITEC tripled the average annual incorporation of new technologies compared to the period 2006–2011. In this process, it was necessary to assess efficacy, safety and cost-effectiveness of technologies, generating positive results for the expansion of access, health gains for patients and sustainability for the system. It should be considered that the use of evidence for decision making strengthens transparency in public management and the development of active processes of information, communication and social participation.

Abstract. The transportable ground based microwave radiometer MIAWARA-C monitored the upper stratospheric and lower mesospheric (USLM) water vapor distribution over Sodankylä, Finland (67.4° N, 26.6° E) from January to June 2010. At the end of January, approximately 2 weeks after MIAWARA-C's start of operation in Finland, a stratospheric sudden warming (SSW) disturbed the circulation of the middle atmosphere. Shortly after the onset of the SSW water vapor rapidly increased at pressures between 1 and 0.01 hPa. Backward trajectory calculations show that this strong increase is due to the breakdown of the polar vortex and meridional advection of subtropical air to the Arctic USLM region. In addition, mesospheric upwelling in the course of the SSW led to an increase in observed water vapor between 0.1 and 0.03 hPa. After the SSW MIAWARA-C observed a decrease in mesospheric water vapor volume mixing ratio (VMR) due to the subsidence of H2O poor air masses in the polar region. Backward trajectory analysis and the zonal mean water vapor distribution from the Microwave Limb Sounder on the Aura satellite (Aura/MLS) indicate the occurrence of two regimes of circulation from 50° N to the North Pole: (1) regime of enhanced meridional mixing throughout February and (2) regime of an eastward circulation in the USLM region reestablished between early March and the equinox. The polar descent rate determined from MIAWARA-C's 5.2 parts per million volume (ppmv) isopleth is 350 &amp;pm; 40 m d−1 in the pressure range 0.6 to 0.06 hPa between early February and early March. For the same time interval the descent rate in the same pressure range was determined using Transformed Eulerian Mean (TEM) wind fields simulated by means of the Whole Atmosphere Community Climate Model with Specified Dynamics (SD-WACCM). The average value of the SD-WACCM TEM vertical wind is 325 m d−1 while the along trajectory vertical displacement is 335 m d−1. The similar descent rates found indicate good agreement between the model and MIAWARA-C's measurements.

Abstract Aims While the CLOSE protocol proposes a maximally tolerable interlesion distance (ILD) of 6 mm for ablation index ablation index-guided atrial fibrillation (AF) ablation, a target ILD has never been defined. This randomized study sought to establish a target ILD for ablation index-guided AF ablation. Methods and results Consecutive patients scheduled for first-time pulmonary vein (PV) isolation (PVI) were randomly assigned to ablation protocols with a target ILD of 5.0–6.0 mm or 3.0–4.0 mm, with the primary endpoint of first-pass PVI. In compliance with the CLOSE protocol, the maximum tolerated ILD was 6.0 mm in both study protocols. A target ablation index of ≥550 (anterior) or ≥400 (posterior) was defined for the ‘5–6 mm’ protocol and ≥500 (anterior) or ≥350 (posterior) for the ‘3–4 mm’ protocol. The study was terminated early for superiority of the ‘3–4 mm’ protocol. Forty-two consecutive patients were randomized and 84 ipsilateral PV pairs encircled according to the study protocol. First-pass PVI was accomplished in 35.0% of the ‘5–6 mm’ group and 90.9% of the ‘3–4 mm’ group (P &lt; 0.0001). Median ILD was 5.2 mm in the ‘5–6 mm’ group and 3.6 mm in the ‘3–4 mm’ group (P &lt; 0.0001). In line with the distinct ablation index targets, median ablation index was lower in the ‘3–4 mm’ group (416 vs. 452, P &lt; 0.0001). While mean procedure time was shorter in the ‘3–4 mm’ group (149 ± 27 vs. 167 ± 33min, P = 0.004), fluoroscopy times did not differ significantly (4.7 ± 2.2 vs. 5.1 ± 1.8 min, P = 0.565). Conclusion In ablation index-guided AF ablation, an ILD of 3.0–4.0 mm should be targeted rather than 5.0–6.0 mm. Moreover, the lower target ILD may allow for less extensive ablation at each given point.
