Abstract:
The article contains information on the results of the analysis of the distribution of atmospheric precipitation and humidification in the Vinnytsia region. Vinnytsia region is located in the forest-steppe zone, but belongs to an area with unstable humidification with a high probability of dry years. Both in wet and dry years, the amount of precipitation increases from east to west towards the Carpathians. To show the patterns of precipitation distribution, published sources and reference books on climate with normative indicators for precipitation for 45 points of the Vinnitsa region were analyzed. The humidification coefficient was calculated in relation to annual precipitation to evaporability. Analysis of the geographical distribution of precipitation and humidification showed their dependence on relief, orohydrography and exposure of slopes. At altitudes less than 100 m (in the Dniester river valley), the amount of precipitation is 480-490 mm per year and the humidification coefficient is 0.60-0.62, and on windward slopes they increase to 517-520 mm and 0.62-0.64, respectively. On vistas of 100-150 m, the amount of precipitation increases to 504-520 mm and humidification to 0.63-0.65. For heights of 150-200 m, the average precipitation for the year is 500-510 mm and the humidification coefficient is 0.64. Most observation points are located at absolute altitudes from 200 to 300 m. At altitudes of 200-250 m, the amount of precipitation and humidification increases to 490-520-540-560 mm per year and 0.62-0.67-0.70-0.76. At altitudes of 250-275 m, precipitation amounts to 485-505-530-550 mm per year and humidification coefficients 0.63-0.66-0.73-0.75. The largest values ​ ​ of average annual precipitation and humidification coefficients are observed at altitudes of 275-300 and more than 300 m (510-540-550-560-570 mm and 0.65-0.72-0.76-0.77). The northern territory of the region is located in the zone of optimal humidification of typical forest-steppes. Most of the territory is located in the zone of insufficient humidification (less than 0.7-0.65). A coefficient of insufficient humidification typical of the steppe zone (0.65-0.60) is determined for 16 points in areas south of the latitude of the cities of Gaisin and. Mooded Kurilovtsy. In a warm season, there are 3-5 periods without rains lasting 10-11 days. In 25% of cases, the duration of idle periods increases to 11-20 days, in 5% - to 21-30 days and 1-3% cases to 30-50 days or more. Thus, the largest amount of precipitation (525-560 mm on average per year) falls in the north and northeast within the Dnieper Upland. In the South Bug Valley, annual rainfall decreases from northwest to southeast to 520-490 mm. On the Podolsk Upland and Vinnytsia Transnistria, the amount of precipitation varies from 500-550 mm per year on watersheds to 490-480 mm in river valleys. In most of the territory south of the latitude of Vinnitsa, humidification is insufficient, and south of the latitude of Gaisin, the humidification coefficient is the same as in the steppe zone (0.64-0.60). Abstract:
Triptans referred as 5-HT1-receptor agonists are groups of medicines, that are utilized to treat migraine and certain other headaches. İn this study, zolmitriptan and Sumatriptan as a selective serotonin receptor agonist for the 1B and 1D subtypes have been studied. This study was conducted to investigate the qualitative determination of triptan drugs (sumatriptan and zolmitriptan) from their pharmaceutical preparation by using oxidation coupling reaction. We studied the interaction of drugs by using five different type of coupling reactions in the presence of potassium permanganate as an oxidizing agent. The final product was analysed under Uv-VIs spectrophotometer, and it was found that, the resultant product of zol-shc, zol-azo, zol-oxi, zol-chro and zol-blrch give maximum absorbance at 510 nm, 540 nm, 490 nm, 520 nm, 505 nm of wavelength repectively, compare to zolmitriptan at 240 nm. Similar results were observed for suma-shc, suma-azo, suma-oxi, suma-chro and suma-blrch 525 nm, 540nm, 515 nm, 530nm, 520 nm and 480 nm of wavelength respectively compared to pure sumatriptan of 280 nm. These methods provides an effective and less expensive method for measurement of the triptans specially for developing countries

Abstract:
Исследовано влияние режимов термической обработки на микроструктуру, микротвердость и электропроводность алюминиевого металломатричного композита (ММК), армированного наночастицами Al2O3 в количестве 2,5 % (масс.). ММК изготовлен методом многостадийного литья с перемешиванием по разным режимам. Проведена термическая обработка ММК по режимам: закалка от 510, 530 или 550 °C с последующим искусственным старением при 140, 160, 180, 200 или 220 °C. Установлено, что композит в литом состоянии имеет микротвердость по Виккерсу 36 HV и электропроводность 45 % IACS. Показано, что термическая обработка повышает твердость и электропроводность ММК. После закалки от 530 °C и старения при 200 °C достигается наибольшее значение микротвердости ММК (45 HV), а после закалки от 530 °C и старения при 180 °C — наибольшая электропроводность (50 % IACS).

The effect of rainfall quantity and distribution and of N, P, K, Ca and Mg fertilisation on the yields of rye, potato, winter wheat and triticale were evaluated in the 42 years of a long-term mineral fertilisation experiment [soil (acidic, sandy, brown forest) × fertilisation (N, P, K, Ca, Mg) × rainfall (quantity, distribution) × crop (rye, potato, winter wheat, triticale)] set up in 1962 under fragile agro-ecological conditions in the Nyírlugos-Nyírség region of Eastern Hungary. The soil had the following agrochemical characteristics: pH (H2O) 5.9, pH (KCl) 4.7, hydrolytic acidity 8.4, hy1 0.3, humus 0.7%, total N 34 mg kg-1, ammonium lactate (AL)-soluble P2O5 43 mg kg-1, AL-K2O 60 mg kg-1 in the ploughed layer. From 1962 to 1980 the experiment consisted of 2×16×4×4=512 plots and from 1980 of 32×4=128 plots in split-split-plot and factorial random block designs. The gross plot size was 10×5=50 m2. The average fertiliser rates in kg ha-1 year-1 were nitrogen 45, phosphorus 24 (P2O5), potassium 40 (K2O), magnesium 7.5 (MgO) until 1980 and nitrogen 75, phosphorus 90 (P2O5), potassium 90 (K2O), magnesium 140 (MgCO3) after 1980. The main results and conclusions were as follows: The rainfall quantities averaged over many years and in the experimental years, and during the growing season, averaged over many years and in the experimental years, were 567, 497, 509, 452 mm for rye and 586, 509, 518 and 467 mm for winter wheat. Rainfall deviations from the many years' average -3% and -13% in the experimental years and during the growing season for potato and 2% and -3% for triticale. During the vegetation period the relationships between rainfall quantity, NPKCaMg nutrition and yield could be characterised primarily by quadratic correlations. Maximum yields of 4.0 t ha-1 for rye, 21.0 t ha-1 for potato, 3.4 t ha-1 for winter wheat and 5.0-6.0 t ha-1 for triticale were recorded when the natural rainfall amounted to 430-500, 280-330, 449-495 and 550-600 mm, respectively. At values above and below these figures there was a considerable reduction in the yield. The results showed that the crop yields were strongly influenced (quadratic correlation) by interactions between N, P, K, Ca and Mg fertilisation and rainfall quantity and distribution.

Abstract:
BackgroundPsoriasis is one of the most common extra-articular involvement in axial spondyloarthritis (axSpA) patients.ObjectivesTo examine the effect of the presence of psoriasis on the clinical and radiological features of patients with axSpA.MethodsIn this single-center study, 657 axSpA patients (mean age; 39.9±11.7, male gender n:411 [62,7%,]) were included in this analysis, according to the criteria of the Assessment of Spondyloarthritis International Society (ASAS). In addition to the initial demographic, clinical and radiological characteristics of the patients, radiological damage and progression were also evaluated. Demographic and disease-related characteristics were reviewed according to the presence of psoriasis.ResultsIn total 43 (6.5%) patients with axSpA had psoriasis. While patients with psoriasis had a higher mean age (44.8±12.4 vs 39.6±11.5; p:0.008), their gender distribution was similar (Table 1). According to the ASAS definition, positive family history (55.6% vs 24.5%; p<0.0001) and basal biological use (41.9% vs 25.9%; p:0.023) rates were also higher in psoriasis patients. While activity scores were found to be similar in patients with and without psoriasis; in the psoriasis group, patient-reported outcome measures (night and general low back pain) and function (BASFI) scores were found to be significantly higher. In addition to the presence of basal cervical (50% vs 31.6%; p:0.032), basal lumbar syndesmophytes (40% vs 18.8%; p<0.006), new syndesmophyte development in the lumbar spine (53.3% vs 10.1%); p<0.0001) rate was higher in axSpA patients with psoriasis. Although the baseline mSASS scores were higher in these patients, the difference did not reach statistical significance. The dissemination of pornography is regulated by the pornography law, and the offense of decency is regulated in the Criminal Code. Articles 281, 282, 283, 532, and 533 of the Criminal Code prohibit pornography and pornographic acts and provide punishment. The Law Number 19 of 2016 specifically regulates decency in the electronic field, amending the Law Number 11 of 2008 concerning Electronic Information and Transactions. Komnas Perempuan reported an increase in gender-based violence on the internet, specifically revenge porn, with cases increasing from 126 in 2019 to 510 in 2020. Law enforcement efforts against perpetrators of revenge pornographic content on social media include preventive and repressive measures.

Abstract:
The effects of mutation of key conserved active-site residues (Tyr-73, Phe-418, Trp-430, Arg-516, Asn-518, His-520 and His-563) of glucose oxidase from Penicillium amagasakiense on substrate binding were investigated. Kinetic studies on the oxidation of β-D-glucose combined with molecular modelling showed the side chain of Arg-516, which forms two hydrogen bonds with the 3-OH group of β-D-glucose, to be absolutely essential for the efficient binding of β-D-glucose. The R516K variant, whose side chain forms only one hydrogen bond with the 3-OH group of β-D-glucose, exhibits an 80-fold higher apparent Km (513 mM) but a Vmax only 70% lower (280 units/mg) than the wild type. The complete elimination of a hydrogen-bond interaction between residue 516 and the 3-OH group of β-D-glucose through the substitution R516Q effected a 120-fold increase in the apparent Km for glucose (to 733 mM) and a decrease in the Vmax to 1/30 (33 units/mg). None of the other substitutions, with the exception of variant F418A, affected the apparent Km more than 6-fold. In contrast, the removal of aromatic or bulky residues at positions 73, 418 or 430 resulted in decreases in the maximum rates of glucose oxidation to less than 1/90. Variants of the potentially catalytically active His-520 and His-563 were completely, or almost completely, inactive. Thus, of the residues forming the active site of glucose oxidase, Arg-516 is the most critical amino acid for the efficient binding of β-D-glucose by the enzyme, whereas aromatic residues at positions 73, 418 and 430 are important for the correct orientation and maximal velocity of glucose oxidation.

Abstract:
Objective: To determine the lowest dose of porcine luteinizing hormone (pLH) that induces ovulation in a predictable manner in mixed-parity sows 80 hours after treatment with equine chorionic gonadotropin (eCG) at weaning. Materials and methods: All sows were injected intramuscularly (IM) with 600 IU eCG at weaning. At 80 hours post weaning, sows were treated IM with pLH at doses of 0.625 mg (n = 5), 1.25 mg (n = 14), 2.5Â mg (n = 21), and 5.0 mg (n = 5). The 15 controls were untreated. Approximate time of ovulation was determined using transrectal realtime ultrasound 8 hours before and 16 hours post pLH injection, and thereafter at 8-hour intervals until ovulation was confirmed. Results: Administration of pLH was effective in inducing ovulation in weaned sows. The pLH-to-ovulation interval was shorter in sows given pLH at 1.25 mg (P < .05), 2.5 mg (P < .01), and 5.0 mg (P < .05) than in controls. Variance associated with the pLH-to-ovulation interval tended to be less in treated than in control sows (P < .10). More sows administered either 2.5 or 5.0 mg pLH ovulated by 40 hours post pLH administration than all other treatments (P < .01). Implications: Doses of 2.5 and 5.0 mg of pLH did not differ in their effect of inducing ovulation in an estrus-synchronization program initiated with 600 IU of eCG at weaning. Controlling the time of ovulation allows insemination to occur at the time of optimal fertilization.

510 Background: HER2-positive breast cancer (bc) is a very heterogenous disease. We hypothesized that molecular subtype may predict disease response to investigational agents in HER2+ bc. Here, we report the pathologic complete response (pCR) rate in the first six agents tested in HER2+ bc in the I-SPY 2 trial for the full HER2+ cohort, by molecular subtype, and by disease receptor status. Methods: Women with HER2+ tumors which were > 2.5 cm were eligible. The I-SPY2 platform trial tests novel agents given neoadjuvantly with a backbone of taxol (T) and trastuzumab (H) followed by doxorubicin and cyclophosphamide. Agents investigated in HER2+ bc were TH (control), MK2206, AMG386, pertuzumab (P), neratinib (N (given in place of H), and TDM1+P (given in place of TH). An investigational arm graduated if there was >85% chance of success compared to control in a 300-person phase 3 neoadjuvant trial. Further details of the I-SPY2 methods have been previously published. Molecular subtyping based on gene expression was utilized to categorize tumors into 5 response predictive subtypes (RPS) (HER2-/Immune-/DRD (DNA repair deficiency)-, HER2-/Immune+, HER2-/Immune-/DRD+, HER2+/Her2_or_Basal and HER2+/Luminal). Results: For the full HER2+ cohort (N=245) pCR rate was higher in all investigational arms than control (Table). By tumor receptor status, HER2+/HR- tumors (N=89) had a higher pCR rate than HER2+/HR+ tumors (N=156; 63% vs 37%, p = 0.0001). In HER2+/HR- tumors N, MK2206, P and TDM1/P graduated. In HER2+/HR+ tumors P and TDM1/P graduated. 76% (185/245) of I-SPY 2 HER2+ patients were classified as HER2+/Her2_or_Basal and 24% (60/245) were HER2+Luminal. pCR rate was significantly higher in the HER2+/Her2_or_Basal group than in the HER2+/Luminal group (57% vs 15%, p < 0.0001). All agents, except for MK2206, where numbers were small, showed greater efficacy in the HER2+/Her2_or_Basal group than in the HER2+/Luminal group. HER2+/Luminal appeared to be more sensitive to the AKT inhibitor MK2206 than to targeted HER2 agents, though numbers are small. Conclusions: pCR rates for patients with HER2+ bc treated with investigational agents, particularly dual HER2-blockade, were promising. Molecular response predictive subtype classification provides insight on how to better target therapy. The HER2+/Luminal group had low pCR rates with dual HER2-blockade but may have higher pCR rate with the addition of an AKT inhibitor and identifies a subgroup of HER2+ tumors in need of novel approaches. 510 Background: The overall survival (OS) of pts with mRCC has improved since the introduction of targeted therapies (TT). We sought to characterize the baseline characteristics, management, and outcomes of pts who survived > 4 yrs from date of diagnosis of metastatic disease (Met Dx). Methods: We retrospectively reviewed medical records of consecutive pts who were diagnosed with mRCC and evaluated at MDACC from 1/1/2001 to 12/31/2008. Descriptive statistics and Kaplan-Meier methods were used to estimate OS times. Results: For 729 mRCC pts identified, median OS was 2.3 years (95% CI: 2.1, 2.5); 219 (30.0%) pts have survived >4 yrs, and 167 (22.9%) pts have survived >5 yrs. For pts diagnosed in the cytokine era (2000 to 2004) median OS was 2.3 yrs (95%CI: 2-2.6) compared to median OS 2.4 yrs (95%CI: 2.0-3.0) for pts diagnosed in the targeted era (2005-2008). Of the 219 pts, 205 pts had adequate baseline information. Conclusions: Since 2000, approximately 30% of mRCC pts have survived >4 yrs and 23% have survived >5 yrs. Patients spent more time on treatment in the targeted compared to the cytokine era. Изучено влияние температуры и ультразвука на интенсивность извлечения антоцианового пигмента и пектиновых веществ. Показано, что спектры, описывающие интенсивность извлечения антоцианового пигмента черной смородины, имеют два максимума поглощения при длинах волн 510 и 280 нм. Установлено соответствие максимумов поглощения антоцианового пигмента длине волны 510 нм и пектиновых веществ, ассоциированных с полифенолами, длине волны 280 нм. Показано, что наиболее полно экстрагирование антоцианового пигмента и пектиновых веществ происходит при температуре 65 °С в течение 30 мин. Увеличение температуры экстрагирования до 90 °С приводит к снижению интенсивности извлечения биологически активных веществ. Использование ультразвука по-разному влияет на интенсивность извлечения антоцианового пигмента и пектина, выделенных из ягодного сырья. Использование ультразвука для экстрагирования антоцианового пигмента увеличивает интенсивность его выделения. При ультразвуковой экстракции в течение 30 мин содержание антоцианового пигмента в растворе увеличивается на 50 %. Максимальное извлечение антоцианового пигмента наблюдается при ультразвуковой экстракции в течение 60 мин. Степень извлечения антоцианового пигмента повышается в 2,5 раза. Напротив, использование ультразвука для выделения пектиновых веществ приводит к уменьшению их количества в растворе. Полученный методом концентрирования краситель, содержащий антоциановый пигмент и пектиновые вещества, имеет темно-красный цвет и ярко выраженный запах черной смородины, густую сиропообразную консистенцию, содержит в своем составе 4,83 мг/мл антоцианового пигмента, обладает полной растворимостью.

Maddox Dairy, located in Riverdale, CA, USA, is a Holstein herd that milks 3500 cows with a 305-day mature-equivalent milk production of 12 800 kg, and they have been producing high genetic animals by embryo transfer (ET) since the early 1980s. Invivo-derived embryos from Holstein donors were transferred fresh (grade 1 or 2) or frozen (grade 1), at morula (4), early blastocyst (5), or blastocyst (6) stage, to virgin heifers (VH, natural oestrus, 13-15 months old) or lactating cows (LC, Presynch-Ovsynch, 86 days in milk, first or second lactation) 6 to 9 days after oestrus. Pregnancy diagnosis was done by transrectal ultrasonography at 32-46 days in VH and by the IDEXX PAG test at 30 days in LC. June, July, August, September, and October were called critical months (first service AI conception rate drops below 44%) and compared with the other months. The data from 32 503 ETs between January 2008 and December 2018 are summarised on Table 1. Pregnancy rates (PR) are lower for LC recipients than for VH. Embryo transfers performed 7 or 8 days after oestrus had higher PR in both types of recipients and embryos, but Day 6 and 9 oestrus are also used with fair results. The season does not seem to affect PR. There is not enough difference in the combination of stage and days from oestrus for invivo-derived embryos. These numbers do not belong to a planned experiment. Several management changes during the years were made, which make it very difficult to apply statistical methods to analyse the data correctly. They are used as a tool to make decisions in an attempt to improve future results. Abstract:
Диференціація сортів і гібридних форм горіха грецького за морфологічним описом є недостатньою для генетичного контролю саджанців щодо їх відповідності сорту. Наразі існує потреба у створенні системи генетичної ідентифікації та ДНК-паспортизації сортів і гібридних форм J. regia. Наведено метод полімеразно-ланцюгової реакції для диференціації генотипів горіха грецького з використанням чотирьох ISSR-праймерів такого складу: (GA)8TC, (CA)8TG, (GA)8CG, (GA)8GT. Оптимізовано умови ПЛР та здійснено електрофоретичне розділення ампліконів у 6 % поліакриламідному гелі, фарбованого азотнокислим сріблом. За цими праймерами було проаналізовано 12 генотипів J. regia та отримано 50 ампліконів різної довжини, з яких 30 поліморфних та 20 мономорфних. Визначено значну частку поліморфності у праймера CR-252-80 %. Найменшу частку поліморфності – 65 %, виявлено у праймера CR-253. Визначено мономорфні амплікони із довжинами: 120 п.н., 125 п.н., 230 п.н., 275 п.н., 300 п.н., 375 п.н., 380 п.н., 510 п.н., 580 пар нуклеотидів і поліморфні амплікони із довжинами: 135-170 п.н., 190-225 п.н., 240-260 п.н., 320-360 п.н., 400-500 п.н., 520-560 п.н., 590-1010 п.н. Продемонстровано практичне застосування ISSR-аналізу для порівняння генотипів горіха грецького на відповідність заявлених сортів восьми саджанців, придбаних на українському ринку. Здійснено кластерний аналіз генотипів саджанців і визначено їх коефіцієнти розбіжності. З'ясовано, що саджанці заявленого сорту Вебу-6 № 1 та Вебу-6 № 2 – це різні гібридні форми насіннєвого походження, коефіцієнт розбіжності становить 0,46. Саджанці заявленого сорту Юкалівський крепиш № 1 та Юкалівський крепиш № 2 є генетично близькоспоріднені з найменшим коефіцієнтом розбіжності – 0,04, але мають насіннєве походження. Встановлено, що заявлені саджанці сорту Чандлер № 1 та Чандлер № 2, які походять від різних постачальників – це також різні генотипи з коефіцієнтом розбіжності 0,24. Запропоновано молекулярно-генетичне маркування генотипів горіха грецького з метою ДНК-паспортизації на підставі апробованих чотирьох ISSR-праймерів.

The study carried out the zoning of the city of Shelekhov, based on the assessment of acoustic pollution of the territory. Field observations were made and the number of vehicles moving along intracity roads and the federal highway R-258 “Baikal” was calculated. With the help of sound level meters, instrumental measurements were made in the daytime and at night, and maps of the sound pressure level of the road network were built. The calculation of statistical characteristics was carried out and the daily course of the distribution of the noise level on the main and secondary roads was constructed. The analysis of the obtained results and their comparison with the established sanitary standards for their excess was carried out. Evidence has been identified to support the implementation of strategies aimed at reducing acoustic pollution and, as a result, creating favorable conditions for the population to live.

e15713 Background: Pancreatic neuroendocrine tumors (PNET) represent a rare tumor histology among patients diagnosed with pancreatic cancer (less than 2% incidence). These tumors arise from the endocrine tissues of the pancreas and can present with a variable clinical course. We sought to examine the overall survival (OS) of patients based upon sex and anatomic location via the SEER database. Methods: Accessing the SEER-Medicare database, we identified 3,242 patients with both Pancreatic Cancer and Neuroendocrine Tumor (NET) histologic validation using ICD-O-3 Codes while excluding Large Cell, Small Cell, and Mixed Endocrine-Exocrine tumors. We then sub-stratified the data to male (N = 1816) and female (N = 1426) sex as well as pancreatic location: Head (Male = 831; Female = 657), Body (Male = 283; Female = 258), or Tail (Male = 701; Female = 510). We excluded PNETs of the pancreatic duct as there were only 2 cases (Male = 1; Female = 1). All comparison tests were run with SEER*STAT 8.3.2 via a Two-Sample t-Test Assuming Equal Variances. OS was analyzed using the Kaplan–Meier method. Results: Overall, both Male and Female patients with PNET have significantly improved 5-Year survival compared with those patients with Pancreatic Adenocarcinoma ((47.47% (n = 3242) vs 6.6% (n = 117,204), p < 0.05). There is no difference in 5-Year OS between Male (44.9%) and Female (50.5%) patients with PNET accounting for all locations. There is a trend towards significance in improved 5-Year OS in Female patients with Tail PNET versus Male patients with Tail PNET (55.8% (n = 510) vs 48.6% (n = 701), p < 0.06). Significantly, Female patients with Tail PNET have a significantly higher 5-Year OS compared to Female patients with Head PNET (55.8% (n = 510) vs 46.9% (n = 657), p < 0.05). There was a trend towards improved 5-Year OS in Female patients with Tail PNET versus those within the Body (55.8% (n = 510) vs 48.6% (n = 258), p < 0.01). Conclusions: Based upon this population study, Female patients with PNET localized to the Tail have a significantly higher 5-Year OS than Female patients with PNET localized to the Head of the Pancreas. These same patients trend towards improved survival when compared with Female patients with Body PNET as well as Male patients with Tail PNET.

Background: Patient satisfaction is the level to which the patients’ experience that their needs and expectations are being met by the service provided. The aim of the study was to measure and improve the quality of eye care services at Lumbini Eye Institute and Research center (LEIRC)/Shree Rana Ambika Shah Eye Hospital (SRASEH) both pediatric and adult groups. Methods: A cross sectional study with descriptive analysis was done from November 2018 to April 2019 at LEIRC, Bhairahawa Nepal. Five hundred seventeen (517) patients were enrolled by systematic random sampling, segregating 201- In patients, 205 – adult and elder patients and 111 pediatric patients. Interview was taken from the patients using the questionnaire which was already used in Arvind Eye Care System, India after validating in our context. Data was entered and analyzed using IBM SPSS -20 version. Results: Among 517 respondents Female (54%) respondents were more than male. Overall patient satisfaction was almost ninety-eight percent were satisfied. Similarly, more than eighty (83.94) percent felt the reasonable cost in service charge and almost sixty five percent respond the reasonable waiting time. Ninety percent patients expressed their interest to recommend their friends or relatives to the hospital. Conclusions: Majority of the patients were satisfied with the overall services of the LEIRC. However, there is need to reduce longer waiting time of the patients.

Background and Objective: Above 90% of childhood HIV infections result from mother-to child transmission (MTCT). This study examined the MTCT rates of HIV-exposed infants enrolled in the infant follow-up arm of the prevention of mother-to-child transmission (PMTCT) program in a teaching hospital in Southeast Nigeria. Methods: This was a 14-year review of outcomes of infants enrolled in the infant follow-up arm of the PMTCT program of Nnamdi Azikiwe University Teaching Hospital Nnewi, Nigeria. The majority of subjects were enrolled within 72 hours of birth and were followed up until 18 months of age according to the National Guidelines on HIV prevention and treatment. At enrollment, relevant data were collected prospectively, and each scheduled follow-up visit was recorded both electronically and in physical copy in the client’s folders. Data were analyzed using SPSS version 20. The major outcome variable was final MTCT status. Results: Out of 3,784 mother-infant dyads studied 3,049 (80.6%) received both maternal and infant Antiretroviral (ARV) prophylaxis while 447 (11.8%) received none. The MTCT rates were 1.4%, 9.3%, 24.1%, and 52.1% for both mother and infant, mother only, infant only, and none received ARV prophylaxis respectively. There was no gender-based difference in outcomes. The MTCT rate was significantly higher among mixed-fed infants (p<0.001) and among those who did not receive any form of ARVs (p<0.001). Among dyads who received no ARVs, breastfed infants significantly had a higher MTCT rate compared to never breastfed infants (57.9% vs. 34.8%; p<0.001). The MTCT rate was comparable among breastfed (2.5%) and never-breastfed (2.1%) dyads who had received ARVs. After logistic regression, maternal (p<0.001, OR: 7.00) and infant (p<0.001, OR: 4.00) ARV prophylaxis for PMTCT remained significantly associated with being HIV-negative. Conclusion and Global Health Implications: Appropriate use of ARVs and avoidance of mixed feeding in the first six months of life are vital to the success of PMTCT programs in developing countries. PMTCT promotes exclusive breastfeeding and reduces the burden of pediatric HIV infection, thereby enhancing child survival. Copyright © 2021 Ugochukwu et al. Abstract. Within the framework of the PAGES NAm2k project, 510 North American borehole temperature–depth profiles were analyzed to infer recent climate changes. To facilitate comparisons and to study the same time period, the profiles were truncated at 300 m. Ground surface temperature histories for the last 500 years were obtained for a model describing temperature changes at the surface for several climate-differentiated regions in North America. The evaluation of the model is done by inversion of temperature perturbations using singular value decomposition and its solutions are assessed using a Monte Carlo approach. The results within 95 % confidence interval suggest a warming between 1.0 and 2.5 K during the last two centuries. A regional analysis, composed of mean temperature changes over the last 500 years and geographical maps of ground surface temperatures, show that all regions experienced warming, but this warming is not spatially uniform and is more marked in northern regions.

Background. Inclisiran significantly reduced low-density lipoprotein cholesterol (LDL-C) in individuals with heterozygous familial hypercholesterolaemia, established atherosclerotic cardiovascular disease (ASCVD) or ASCVD risk equivalents (type 2 diabetes, familial hypercholesterolaemia or a 10-year risk of a cardiovascular event ≥20%) in the ORION phase III clinical trials. Infrequent dosing at days 1, 90, 270 and 450 resulted in a mean LDL-C reduction of ~50%. A total of 298 participants from South Africa (SA) were enrolled. Local data are needed to support the use of inclisiran in the SA population, potentially addressing an unmet need for additional LDL-C-lowering therapies. Objectives. To analyse the ORION phase III trial data to assess the efficacy and safety of inclisiran in SA participants. Methods. ORION-9, 10 and 11 were randomised, double-blind, phase III trials. Participants were receiving maximally tolerated statins with or without other lipid-lowering therapies (excluding protein convertase subtilisin/kexin type 9 (PCSK9) inhibitors). Participants were randomised 1:1 to inclisiran sodium 300 mg/284 mg (free acid) or placebo administered at days 1, 90, 270 and 450. The co-primary endpoints were the LDL-C percentage change from baseline to day 510 and the time-averaged percentage change in LDL-C from baseline after day 90 up to day 540. Key secondary endpoints included the absolute change in LDL-C from baseline to day 510, the time-averaged absolute change from baseline after day 90 up to day 540, and changes in other lipids and lipoproteins. Results. The mean age of the participants was 58.6 years (56% male). The mean LDL-C level at baseline was 3.6 mmol/L. At day 510, inclisiran reduced LDL-C levels by 54.2% compared with placebo (95% confidence interval (CI) –61.3 - –47.2; p<0.0001). The corresponding time-averaged reduction in LDL-C was 52.8% (95% CI –57.9 - –47.8; p<0.0001). Treatment-emergent adverse events at the injection site were more common with inclisiran compared with placebo (10.1% v. 0.7%); however, all were mild or moderate in nature and none were persistent. Conclusion. Inclisiran, given in addition to maximally tolerated standard lipid-lowering therapy, is effective and safe and results in robust reductions in LDL-C in SA patients at high cardiovascular risk.

Abstract. Inference of the lapse rate tropopause or the cold point from temperature profiles of finite vertical resolution entails an uncertainty of the tropopause altitude. For tropical radiosonde profiles the tropopause altitude inferred from coarse-grid profiles was found to be lower than that inferred from the original profiles. The mean (median) displacements of the lapse rate tropopause altitude when inferred from a temperature profile of 3 km vertical resolution and a Gaussian kernel are −130, −400, −730, and −590 m (−70, −230, −390, and −280 m) for Nairobi, Hilo, Munich, and Greifswald, respectively. In the case of a Michelson Interferometer for Passive Atmospheric Sounding (MIPAS) averaging kernel, the displacement of the lapse rate tropopause altitude is −640 m. The mean (median) displacement of the cold point tropopause inferred from a temperature profile of 3 km vertical resolution (Gaussian kernels) was found to be −510, −610, −530, and −390 m (−460, −510, −370, and −280 m) for the stations mentioned above. Unsurprisingly, the tropopause altitude displacement is larger for coarser resolutions. The effect of the tropopause displacement on the water vapor saturation mixing ratio is roughly proportional to the vertical resolution. In tropical latitudes the resulting error is about 1 to 2 ppmv per vertical resolution in kilometers. The spread of the tropopause displacements within each sample of profiles seems too large as to recommend a correction scheme for tropical temperature profiles, while for midlatitudinal temperature profiles of vertical resolutions of 1 to 5 km a lapse rate of −1.3 K km−1 reproduces tropopause altitudes determined from high-resolution temperature profiles with the nominal lapse rate criterion of −2 K km−1 fairly well.

Cu-TiB2 nanocomposite powders were in situ synthesized by combining high-energy ball milling of Cu-Ti-B elemental powder mixtures as precursors and subsequent self-propagating high temperature synthesis (SHS). Cu-40wt.% TiB2 was produced after SHS reaction and then diluted by copper to obtain desired homogeneous composites with 2.5, 5 and 10wt.%TiB2. Spark plasma sintering (SPS) was used to inhibit grain growth and thereby obtain fully Cu-TiB2 sintered bodies with nanocomposite structure. After SHS reaction, only Cu and TiB2 phases were detected in the SHS-product. Spheroidal TiB2 particles smaller than 250nm were formed in the copper matrix after SHS-reaction. Mechanical and electrical properties were investigated after SPS at 650°C for 30min under 50MPa. The electrical conductivity decreased from 75 to 54% IACS with increasing of TiB2 contents from 2.5 to 10wt.%. However, hardness increased from 56 to 97HRB. In addition, the tensile strength increased with increasing the TiB2 content.

The aim of this study is to analyze the influence of harmonic to power losses in transformator at Sedap Malam feeder by using the help of ETAP power station, so we can obtain how much the influence of harmonic in distribution system. By measuring THDv and THDI in DT0042, DT0048 and DT0319 transformator we got THDv value of 1.1% in DT0042, 1.56% in DT 48 and 1.23% in DT0319 from maximum margin of 5.0% in THDV according to IEEE 519–1992 standard. As for THDI we got the value of 9.36% in DT0042, 12.4% in DT0048 and 14.2% in DT0319 from maximum margin of 8.0% in THDI according to IEEE 519 – 1992 standard. On other side, the result by running ETAP power station we got THDI value of 1.59% in D00T42, 1.95% in DT0048 and 1.71% in DT0319. As for THDv we got running result of 12.22% in DT0042, 12.96% in DT0048 and 13.54% in DT0319. Based on the result of performed analisis of the influence of harmonic to power losses in transformator at Sedap Malam feeder, we obtained that the amount of power losses was 43,2 kW before having harmonic and 45,7 kW after having harmonic. The raise of 2,5 kW was caused by occurance of harmonic in DT0042, DT0048 and DT0319 transformator from total of 44 existed transformatorsPenelitian ini menganalisis pengaruh harmonisa terhadap rugi-rugi daya (losses) pada Transformator di peyulang sedap malam menggunakan bantuan ETAP power station, sehingga dapat diketahui hasil seberapa besar pengaruh harmonisa pada sistem distribusi. Hasil pengukuran THDv dan THDi pada Transformator DT0042, DT0048 dan DT0319 dari diperoleh hasil THDv sebesar 1,1% pada DT0042, 1,56% pada DT0048 dan 1,23% pada DT0319 dari batas maksimum THDV menurut standar IEEE 519–1992 adalah 5.0%. Untuk hasil pengukuran THDI didapatkan hasil pengukuran sebesar 9,36% pada DT0042. 12,4% pada DT0048 dan 14,2% pada DT0319 dari batas maksimum THDI menurut standar IEEE 519 – 1992 adalah 8.0 %. Abstract:
The erbium-vanadium co-doped borate glasses, embedded with silver nanoparticles (Ag NPs), were prepared to improve their optical properties for potential optical fiber and glass laser application. The borate glasses with composition (59.5–x) B2O3–20Na2O–20CaO–xV2O5–Er2O3–0.5AgCl (x = 0–2.5 mol%) were successfully prepared by conventional melt-quenching method. The structural properties of glass samples were investigated by XRD, TEM and by Fourier transform infrared (FTIR) spectroscopy while optical properties were carried out by UV–Vis spectroscopy by measuring optical absorption and the emission properties were investigated by photoluminescence spectroscopy. The XRD patterns confirmed the amorphous nature of the prepared glass samples whilst the FTIR confirmed the presence of VO4, VO5, BO3 and BO4 vibrations. UV–Vis–NIR absorption spectra reveal eight bands which were located at 450, 490, 519, 540, 660, 780, 980, and 1550 nm corresponding to transition of 4F5/2, 4F7/2, 2H11/2, 4S3/2, 4F9/2, 4I9/2, 4I11/2, and 4I13/2, respectively. The optical band gap (Eopt), Urbach energy and refractive index were observed to decrease, increase and increase, respectively, to the addition of vanadium. Under 800 nm excitation, three emission bands were observed at 516, 580 and 673 nm, which are represented by 2H11/2–4I15/2, 4S3/2–4I15/2 and 4F15/2–4I15/2, respectively. The excellent features of achieved results suggest that our findings may provide useful information toward the development of functional glasses.

Relevance. In 2018 over 151 000 cases of whooping cough were reported worldwide. In the Republic of Kazakhstan the incidence of whooping cough has always remained relevant. The incidence is recorded mainly among children under 14 years of age, whose share is 96–100% of the total incidence in certain years. It should also be noted that 82,9–96% of sick people are not vaccinated against whooping cough. Aims. Assessment of the duration of protection after vaccination against pertussis infection in Kazakhstan using antibodies against pertussis toxin (PT) and (IgA and IgG) as a specific marker of pertussis infection or vaccination in children and adolescents in the cities of the Republic of Kazakhstan: Aktobe, Karaganda, Taldykorgan, Shymkent for evaluation appropriate age for booster vaccination. Materials and methods. For the study included medical organizations in the cities of Aktobe, Karaganda, Taldykorgan, Shymkent. Participants aged 10–14 years who were hospitalized in children's hospitals and 15–18 years old who visited polyclinics were invited to participate in the study. Each city has 1 children's hospital (4 in total) and 2 polyclinics (8 in total). A total of 520 people of different age groups participated. Study period: February 2021 – October 2021. Inclusion criteria were such aspects as: age from 10 years to 18 years 11 months 29 days, informed consent obtained from parents or guardian(s) and patient consent, enrollment after visiting the clinic, the presence of documented evidence of immune status. An association between two qualitative variables is considered statistically significant if the p-value calculated using the Chi-squared test is less than 0.05. Results. Among 520 participants aged 10–14 years – 200 (38.5%) and 15–18 years – 320 (61.5%), including boys – 284 (54.6%), girls – 236 (45.4%). The largest number of study participants was aged 15, 16 and 17 years, 10 participants (1.9%) had periods of prolonged coughing in the past 6 months. The first, second and third pertussis vaccines were received by 100% of children. The first and second vaccine for all children was the Adsorbed Diphtheria-Tetanus-Pertussis Vaccine (DPT). The third vaccine for 519 children was also DPT, for one child – AbDPT, due to the transition to a cell-free vaccine from the second half of 2013. The additional vaccine scheduled at 18 months of age was given to 519 participants (99.8%), of which 517 children (99.6%) received DPT and two children (0.4%) DPT. Just over half of the children (276 or 53.1%) also received the whooping cough vaccine at age 6. In biological blood samples of 17 participants (3.3%). antibodies of the IgA class were detected in 245 samples (47.1%) – antibodies of the IgG class. Of the 17 participants with IgA antibodies, 15 also tested positive for IgG. In total antibodies of the IgA and IgG classes were detected in 247 participants which amounted to 47.5%. Conclusions. This study showed the duration of protection after pertussis vaccination in Kazakhstan among different age groups. In total, antibodies of the IgA and IgG classes were detected in 247 participants out of 520 which amounted to 47.5%. IgA antibodies were detected in biological blood samples in 17 participants (3.3%), IgG class antibodies in 245 samples (47.1%). The prevalence of IgA antibodies was statistically significantly higher among older children and among children who had respiratory symptoms in the 6 months prior to the study. The prevalence of IgG antibodies was statistically significantly higher among children vaccinated against whooping cough at the age of 6 years (47.1%) and at the age of 10–14 years (56.2%) than in the age group of 15–18 years (43.8%) and also significantly higher among those, who share a bedroom with other family members compared to children living in separate rooms. Thus an increase in whooping cough and a decrease in protective vaccine immunity among older age groups can be stated which raises concerns about transmission of the infection in the home to vulnerable children and the need to strengthen vaccination strategies.

Background and Aim: Channa striatus extract, a freshwater snakehead fish known as Haruan, is popular in Southeast Asia for consumption and as a traditional therapeutic remedy for wound healing. C. striatus is also used in osteoarthritic for its anti-inflammatory. The aim of this study was to determine the presence of antibacterial properties of C. striatus extract against oral bacteria and to investigate the cytotoxic activity against Vero cells. Materials and Methods: The authors prepared C. striatus extract in chloroform-methanol solvents. Next, the authors took subgingival microbiological samples from 16 cats that had periodontal disease. The authors determined the antibacterial properties of C. striatus extract against the isolated bacteria using the disk diffusion method and a broth microdilution-based resazurin microtiter assay. Finally, the authors used the Vero cell line to evaluate the cytotoxic activity, and they assessed the cell availability using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. Results: The results showed weak antibacterial activity of C. striatus extract against Pseudomonas spp. and Escherichia coli. In addition, the authors found that minimum inhibition concentration values ranged between 400 and 500 mg/mL, and minimum bactericidal concentration values ranged between 650 and 550 mg/mL. However, the cytotoxic results were promising, showing that C. striatus extract increased the cell viability and growth when it was at a higher concentration. The extract also promotes growth and cell proliferation. Conclusion: These findings suggest that C. striatus extract promoted cell proliferation in vitro and could be a plausible therapeutic wound healing alternative for periodontal disease in cats.

Four different solution treatment times at 540°C (1, 4, 16 and 64 hours) and six different artificial aging times at 160°C (0, 2.5, 5, 10, 20 and 40 hours) were used. Work hardening characteristics were investigated by Kocks-Mecking plots for each specimen. The effects of Si particle morphology (solution treatment) and matrix strength (aging) on Kocks-Mecking (Stage III) work hardening model parameters are discussed in the paper.

Background: NLR has historically been used as a predictor of morbidity and mortality in patients with COVID -19.Inflammatory markers serum ferritin, C-reactive protein (CRP) and serum LDH have been reported to be significantly associated with the high risks of the development of severe COVID19. Aims: To correlate with total lymphocyte count (TLC) and Neutrophil lymphocyte ratio (NLR) in assessing the clinical course of COVID 19 and to study the role of inflammatory markers in assessing the severity and clinical course of COVID 19. Methods: The study was done as a cross sectional study among 519 patients who admitted in Trichy SRM medical college hospital and research centre. All adults of both sexes diagnosed with COVID 19 were included in study by consecutive sampling and patients with known chronic inflammatory conditons, autoimmune disorders, malignancy were excluded from the study. The total lymphocyte count and neutrophil lymphocyte ratio were recorded and blood CRP, serum LDH, ferrtin, D dimer were done and correlated with the disease severity and clinical course of COVID. Results: Among 518 patients, 257(49.6%) patients were in the age group of 46 to 65 years and 309 (59.7%) patients were males and 209 (40.3%) were females. Neutrophil Lymphocte ratio was 3-2.5 in (30.9%) patients. Duration of stay in hospital was 11-14 days for 82 patients in that C reactive protein was positive for 55(67%) patients, serum ferritin range was 270-700 in 38(46.4%) patients, serum LDH range was 250-500 in 42(51.2%) patients and D-dimer was above 500 in 19(23.2%) patients. Conclusion: NLR and higher inflammatory markers correlates well with severity of disease and duration of hospitalization in COVID 19 patients.

Microenvironmental conditions in a shadehouse covered with shade fabric designed to exclude 70% of incoming light were monitored and compared to those in an adjacent field to quantify differences related to plant water use. Radiant flux density and photosynthetic, photon flux inside the shadehouse varied seasonally between about 18% to 28% of outside values. During the day, leaf and air temperatures around the crop canopy were generally lower and relative humidities higher inside the shadehouse than outside. Leaf-to-air vapor pressure gradients inside the shadehouse averaged about half those outside. Wind run inside was <10% of wind run outside. Differences between reference ET (ETo) values, calculated using Penman's equation), inside and outside the shadehouse were greatest during summer months. Outside evaporatory pan (Epan) water losses ranged from 205 mm in July to 95 mm in Nov. For the same months, Epan losses inside were about 80% lower. Monthly ETactual, as determined for Rumohra adiantiformis growing in lysimeters in the shadehouse, ranged from around 40% to 80% of inside Eo.

Because positivity of HER2 overexpression is reported in below 20%, identifying the predictive clinical factors for the HER2 overexpression before the pathologic analysis may be helpful and cost-effective. Methods: From JAN 2005 to DEC 2010, 517 gastric cancer patients performed with HER2 IHC and/or FISH tests at Yonsei University College of Medicine were enrolled. Results: Among the 517 patients, 61 patients (11.8%) were confirmed as HER2 overexpression. Forty three out of stage IV, 258 patients (16.7%) and 18 out of 259 (6.9%) localized gastric cancer patients had HER2 overexpression (p <0.001). HER2 overexpression was more common in male (14.2% vs. 7.5% in female, p-value 0.024), moderately differentiated (26.4% vs. 11.1%, 8.9%, 4.9% and 5.9% in well differentiated, poorly differentiated, signet ring cell, and mucinous, retrospectively, p <0.001). Patients with elevated CEA (> 5.0 ng/mL vs. ≤ 5.0 ng/mL: 24.3% vs. 8.4%, p <0.001), metastasis to distant lymph node (with vs. without: 28.6% vs. 14.8%, p-value 0.042), without carcinomatosis (without vs. with: 21.2% vs. 11.6%, p-value 0.039) and pulmonary metastasis (with vs. without: 47.4% vs. 14.2%, p <0.001) also frequently had HER2-positive disease. In addition, high risk patients with 4 or more of the 7 features (metastatic disease, male, moderate differentiated adenocarcinoma, elevated CEA level, metastasis to distant lymph node, pulmonary metastasis, and without carcinomatosis) had 32.9% of HER2 positivity. (p <0.001). Conclusions: Those clinicopathologic factors may be helpful to predict the high likelihood of HER2 positive cases prior to pathologic confirmation in gastric cancer. Further molecular study for the HER2 overexpression is needed to clarify and explain this clinical phenomenon.

The interfacial reaction between eutectic Sn-3.5Ag solder and ENIG substrate during reflow was investigated. During the reflow, the topmost Au layer dissolved into the molten solder, and the reaction layers of Ni3Sn4/Ni-Sn-P/Ni3P formed between Sn-Ag solder and Ni-P plating layer. After the reflow at 255 for 3 min, most of Ni3Sn4 intermetallic compound (IMC) spalled off the substrate. The formation of thick Ni3P and Ni-Sn-P layers was related to the direct reaction between solder and Ni-Sn-P layer by the IMC spalling.

510 Introduction: Letrozole is a selective non steroidal aromatase inhibitor (AI) used for the treatment of estrogen positive (ER+ve) post-menopausal breast cancer (BC) women. HIF-1α, as marker of hypoxia, is involved in the selection of more aggressive phenotype being associated with both chemo and endocrine resistance Whether HIF-1α expression could be modulated by AI in BC is the subject of this investigation. Patients and Methods: 114 elderly women with T2–4 N0–1 and ER+ve BC were randomly assigned to 6 months of primary letrozole (2.5 mg/daily) (L) plus/minus oral “metronomic” cyclophosphamide (50 mg/daily) (LC). Tumor response was assessed clinically with a calliper. On TMAs HIF-1α was evaluated in tumor specimens collected before and after treatment. The intensity of the staining for TMAs marker was scored 0 (no staining), 1 (weak/moderate staining), 2 (strong staining) in nuclear intensity for HIF- 1α expression. Results: 107 patients had HIF-1a assessed at baseline, 80 patients had HIF-1α assessed both at baseline and after treatment. At baseline HIF-1α expression inversely correlated with tumor grade (p<0.01) and N positivity (p<0.01) while it did not correlate with stage, p53, bcl2, Ki67, progesterone ERa expression. HIF-1α expression was significantly reduced after treatment (p<0.004, Wilcoxon Rank sum test), with no difference between the arms. Reduction in HIF-1α did not showed any relationship with disease response. Conclusions: Letrozole was able to down-regulate HIF-1α expression and this may be a mechanism of its efficacy. These results should be taken into account when administering new target therapy able to modulate directly HIF-1α or indirectly via HIF- 1α related pathways in association with letrozole. No significant financial relationships to disclose.

Enterocytozoon bieneusi, a zoonotic pathogen prevalent in both humans and animals, is the most frequently diagnosed microsporidian species in humans and presents significant public health risks. However, data on the prevalence and genotypes of E. bieneusi in farmed minks (Neovison vison) and raccoon dogs (Nyctereutes procyonoides) in China are limited. Therefore, 275 minks (89 from Hebei Province, 57 from Heilongjiang Province, 109 from Liaoning Province, 20 from Shandong Province) and 235 raccoon dogs (114 from Hebei Province, 27 from Heilongjiang Province, 61 from Liaoning Province, 33 from Jilin Province) were examined for the prevalence and genotypes of E. bieneusi through sequence analysis of the internal transcribed spacer (ITS) region of the rRNA gene. The overall prevalence of E. bieneusi was 18.6% (95/510), with 10.5% (29/275) in farmed minks and 28.1% (66/235) in raccoon dogs. Ten genotypes (CHN-F1, genotype D, Type IV, EbpC, NCF2, NCF5, NCF6, Peru8, Henan V, and MJ5) were identified in minks and raccoon dogs. This study is the first to detect the CHN-F1, NCF2, NCF6, Peru8, and Henan V genotypes in minks and the NCF5, NCF6, and MJ5 genotypes in raccoon dogs. Additionally, the D, Type IV, and Peru8 genotypes, previously identified in humans, were also found in minks and raccoon dogs, suggesting that these animals could be potential sources of human microsporidiosis. These findings expand the understanding of E. bieneusi’s host distribution in China and contribute to the prevention of zoonotic E. bieneusi infections among farmed animals.

Введение. Профилактика внезапной сердечной смерти (ВСС), основанная на популяционных факторах риска, имеет низкую степень персонализации и недостаточную предиктивную точность 51,3–66,7%.Цель. Разработка информационной технологии первичной профилактики ВСС на основе ЭКГ-маркеров электрической нестабильности миокарда (ЭНМ), ориентированной на поликлиническое звено, где сконцентрирован основной поток кардиологических пациентов.Материалы и методы. Выполнен анализ ЭКГ высокого разрешения 1014 пациентов с коронарными и некоронарогенными заболеваниями, средний возраст 49,9±5,9 года. Первичные конечные точки: желудочковая тахикардия, успешная сердечно-легочная реанимация, разряды ИКД или ВСС. Период наблюдения 5,0 (2,1; 5,9) года. Контролировали ЭхоКГ, ХМ-ЭКГ и ЭКГ-маркеры ЭНМ: фрагментация QRS, угол QRS-T, длительность и дисперсия QRS, альтернация Т-волны, длительность и дисперсия QT, Tpeak-Tend, турбулентность и замедление сердечного ритма. Использовалась компьютерная программа «Интекард 7.3».Результаты. Среди ЭКГ-маркеров ЭНМ в фазе деполяризации наибольшей прогностической мощностью обладают фрагментация QRS – относительный риск (ОР) 4,3, 95% ДИ (3,5–6,3), p<0,001 и пространственный угол QRS-T – ОР=2,2, 95% ДИ (1,5–2,5), p<0,001. В фазе реполяризации наиболее высокие значения ОР имеют альтернация Т-волны – ОР=4,1, 95% ДИ (2,2–6,2), p<0,001 и дисперсия интервала QT – ОР=2,7, 95% ДИ (1,3–5,4), p<0,001. Маркеры дисфункции вегетативной регуляции турбулентность и замедление сердечного ритма имели ОР, равные 1,7, 95% ДИ (1,0–5,5) и 1,4, 95% ДИ (1,1–5,9) соответственно, p<0,05. Разработан персонифицированный алгоритм первичной профилактики ВСС на основе оценки ФВ, ХМ-ЭКГ и ЭКГ-маркеров ЭНМ. Риск стратифицируется на низкий, средний, высокий и критический. В соответствии с уровнем риска пациентам рекомендуется имплантация ритм-поддерживающих систем, медикаментозная терапия или мониторинг. При клинической апробации алгоритма у 52% пациентов зафиксирован низкий риск, у 42% – средний и у 6% – высокий риск ВСС.Заключение. Предложенная стратегия управления кардиоваскулярными рисками направлена на снижение преждевременной смертности от сердечно-сосудистых заболеваний. Неинвазивность, персонификация и высокая пропускная способность создают предпосылки для широкого внедрения программы первичной профилактики ВСС. Introduction. Prevention of sudden cardiac death (SCD) based on population risk factors has a low degree of personalization and, as a result, an insufficient predictive accuracy of 51.3–66.7%.Purpose. Development of information technology for primary prevention of SCD based on ECG-markers of electrical myocardial instability (EMI), focused on the polyclinics, where the main stream of cardiological patients is concentrated.Materials and methods. High-resolution ECG analysis was performed in 1014 patients with coronary and non-coronary heart diseases, mean age 49.9±5.9 years. Primary endpoints: ventricular tachycardia, successful cardiopulmonary resuscitation, ICD shocks or SCD. Follow-up period was 5.0 (2.1; 5.9) years. Controlled echoCG, HM-ECG and ECG- markers of the EMI: QRS fragmentation, QRS-T angle, QRS duration and dispersion, T-wave alternans, QT duration and dispersion, Tpeak-Tend, turbulence and deceleration of heart rate. The "Intecard 7.3" computer program was used.Results. Among the ECG-markers of EMI in the depolarization phase, QRS fragmentation has the highest predictive power – relative risk (RR)=4.3 95% CI (3.5–6.3), p<0.001 and QRS–T angle – RR=2.2 95% CI (1.5–2.5), p<0.001. In the repolarization phase, the highest RR values have T wave alternans – RR=4.1 95% CI (2.2–6.2), p<0.001 and QT interval dispersion – RR=2.7 95% CI (1.3–5.4), p<0.001. Autonomic regulation dysfunction markers heart rhythm turbulence and deceleration rate had RRs of 1.7 95% CI (1.0–5.5) and 1.4 95% CI (1.1–5.9), respectively, p<0.05. The personalized algorithm for the primary prevention of SCD based on the assessment of EF, HM-ECG and ECG-markers of EMI has been developed. The risk is stratified into low, medium, high and critical. According to the level of risk, implantation of rhythm-support systems, drug therapy or monitoring is recommended. In clinical testing of the algorithm, 52% of patients had a low risk, 42% – an average risk, and 6% – a high risk of SCD.Conclusion. The proposed strategy for managing cardiovascular risks is aimed at reducing premature mortality from cardiovascular diseases. Non-invasiveness, personalization and high throughput create the prerequisites for the widespread introduction of the SCD primary prevention program.

Nakuru North sub-county is a peri-urban area which has both dry and wet seasons. Its residents rely mostly on untreated water sources for daily water needs due to unreliable water supply from the urban council. However, this water has not been evaluated on its quality despite residents solely depending on it. This study was aimed at determining the physico-chemical and bacteriological quality of water drawn from River Kandutura and water taps in Nakuru North sub-county. In addition, the study was aimed at carrying out sensitivity test of the isolates to antibiotics and determining effectiveness of solar disinfection in water treatment. A total of 510 water samples; river (255) and taps (255) were collected and analyzed between January and December 2013. Antimicrobial sensitivity test was carried out using Kirby Bauer disk diffusion test. Out of five hundred and ten (510) samples examined for microorganisms, 36.86 % (188/510) were positive for E. coli, Shigella and Salmonella. Water used by Nakuru North sub-county residents is highly contaminated thus posing public health risk. Solar disinfection experiment indicated a possibility of effective decontamination of water up on exposure to sun light for 3-5 h. E. coli showed the highest resistance (26.3 %) followed by Salmonella (17.4 %) while Shigella showed the least (17.1 %). However, there was no significant deference (p=0.98) in resistance among total coliforms, Total heterotrophic and Salmonella at 0.05 level of significant. There is a need to enforce laws and policies on proper waste disposal as part of water pollution control.

510 Background: Mutations in PIK3CA, the gene encoding the p110a subunit of PI3K, have been associated with antiestrogen resistance in ER+ BC. In general, antiestrogen-resistant cancers retain ER and responsiveness to estradiol. This suggests that treatment of ER+/PI3K mutant BC should include PI3K inhibitors plus antiestrogens. Methods: We conducted a phase Ib trial of letrozole (2.5 mg/d) with the pan-PI3K inhibitor BKM120 in post-menopausal patients (pts) with ER+/HER2 MBC. BKM120 (100 mg/d) was given continuously (Arm A) or intermittently (5 on/2 off days; Arm B). Upon toxicity, BKM120 was reduced to 80 and 60 mg/d. Treatment continued until unacceptable toxicity or disease progression. Disease was assessed every 2 months. FDG-PET was done at baseline and at 2 weeks in all pts in Arm A. Results: Fifty-one pts were accrued; 49 had progressed on an AI previously. Median age was 56 years (range 34-77); 95% had bone and 70% visceral metastases. Toxicities and outcomes are summarized in the table. The only DLT* in Arm A and B were transaminitis and depression, respectively; both at 100 mg. Over 50% of Arm A pts had >25% reduction in their peak SUV at the 2-week FDG-PET. Pts who did not exhibit a metabolic response by FDG-PET progressed rapidly on therapy. Three pts in Arm A had a PI3K mutation; one of them has had stable disease on treatment for >12 months. Conclusions: The combination of letrozole/ BKM120 is safe in pts with AI-refractory ER+/HER2 MBC. Arm B is ongoing and will be updated at the meeting. All tumor biopsies will be analyzed for PI3K pathway alterations. FDG-PET at 2 weeks appears to be a useful pharmacodynamic biomarker of PI3K pathway inhibition in most patients. Menurut Rencana Induk Perkeretaapian Nasional (RIPNAS) pada tahun 2030 diperkirakan terjadi peningkatan perpindahan manusia di Pulau Jawa sejumlah 858,5 juta orang/tahun. Oleh sebab itu, pemerintah berencana mengembangkan jaringan jalan rel di Pulau Jawa dari jalur tunggal (single track) menjadi jalur ganda (double track) agar dapat mengimbangi jumlah permintaan yang akan datang. Kota Surabaya dan Sidoarjo merupakan daerah yang saling terhubung oleh jaringan rel kereta api namun jalan kereta api yang ada pada lintas Stasiun Wonokromo – Stasiun Sidoarjo masih menggunakan jalur tunggal yang menyebabkan layanan kereta api terkadang mengalami keterlambatan keberangkatan dan kereta api harus bergantian jalur rel dengan kereta api lainnya. Berdasarkan urgensi tersebut maka perlu dilakukan perencanaan jalur ganda (double track) pada lintas Stasiun Wonokromo – Stasiun Sidoarjo (KM 7+881 – KM 25+510) agar dapat memenuhi peningkatan perpindahan masyarakat seperti prediksi RIPNAS. Hasil analisis dan perencanaan diperoleh penempatan trase jalur ganda dilakukan di sebelah timur trase eksisting dari Stasiun Wonokromo menuju Stasiun Sidoarjo. Konstruksi jalan rel termasuk kelas jalan rel III dengan dimensi struktur atas tipe rel rencana R54, sambungan tipe R54, penambat elastis ganda pandrol e-clip, dan bantalan beton prategang tipe N-67 PT. WIKA Beton. Dimensi struktur bawah diperoleh ukuran ballast atas setebal 30 cm dan sub-ballast setebal 40 cm. Lapisan subgrade direncanakan dengan timbunan setinggi 1 meter dari permukaan tanah asli dengan kemiringan 1:1,5 menggunakan tanah granular. Saluran drainase pada direncanakan menggunakan beton bertulang pracetak dengan bentuk persegi panjang berdimensi 1,4 m x 0,7 m dan terletak 285 cm dari as rel.

This work analyzed the spatial and temporal variations of the glaciers in the Ebi Lake basin during the period 1964 to 2019, based on the 1st and 2nd Chinese Glacier Inventories (CGI) and remote sensing data; this is believed to be the first long-term comprehensive remote sensing investigation on the glacier change in this area, and it also diagnosed the response of the glaciers to the warming climate by analyzing digital elevation modeling and meteorology. The results show that there are 988 glaciers in total in the basin, with a total area of 560 km2 and average area of 0.57 km2 for a single glacier. The area and number of the glaciers oriented north and northeast are 205 km2 (327 glaciers) and 180 km2 (265 glaciers), respectively. The glaciers are categorized into eight classes as per their area, which are less than 0.1, 0.1–0.5, 0.5–1.0, 1.0–2.0, 2.0–5.0, 5.0–10.0, 10.0–20.0, and greater than 20.0 km2, respectively. The smaller glaciers between 0.1 km2 and 10.0 km2 account for 509 km2 or 91% in total area, and, in particular, the glaciers smaller than 0.5 km2 account for 74% in the total number. The glacial area is concentrated at 3500–4000 m in altitude (512 km2 or 91.4% in total). The number of glaciers in the basin decreased by 10.5% or 116, and their area decreased by 263.29 km2 (−4.79 km2 a−1) or 32% (−0.58% a−1) from 1964 to 2019; the glaciers with an area between 2.0 km2 and 5.0 km2 decreased by the largest, −82.60 km2 or −40.67% in the total area at −1.50 km2 a−1 or −0.74% a−1), and the largest decrease in number (i.e., 126 glaciers) occurs between 0.1 km2 and 0.5 km2. The total ice storage in the basin decreased by 97.84–153.22 km3 from 1964 to 2019, equivalent to 88.06–137.90 km3 water (taking 0.9 g cm−3 as ice mass density). The temperature increase rate in the basin was +0.37 °C decade−1, while the precipitation was +13.61 mm decade−1 during the last fifty-five years. This analysis shows that the increase in precipitation in the basin was not sufficient to compensate the mass loss of glaciers caused by the warming during the same period. The increase in temperature was the dominant factor exceeding precipitation mass supply for ruling the retreat of the glaciers in the entire basin.

ImportanceCefepime/enmetazobactam is a novel β-lactam/β-lactamase inhibitor combination and a potential empirical therapy for resistant gram-negative infections.ObjectiveTo evaluate whether cefepime/enmetazobactam was noninferior to piperacillin/tazobactam for the primary outcome of treatment efficacy in patients with complicated urinary tract infections (UTIs) or acute pyelonephritis.Design, Setting, and ParticipantsA phase 3, randomized, double-blind, active-controlled, multicenter, noninferiority clinical trial conducted at 90 sites in Europe, North and Central America, South America, and South Africa. Recruitment occurred between September 24, 2018, and November 2, 2019. Final follow-up occurred November 26, 2019. Participants were adult patients aged 18 years or older with a clinical diagnosis of complicated UTI or acute pyelonephritis caused by gram-negative urinary pathogens.InterventionsEligible patients were randomized to receive either cefepime, 2 g/enmetazobactam, 0.5 g (n = 520), or piperacillin, 4 g/tazobactam, 0.5 g (n = 521), by 2-hour infusion every 8 hours for 7 days (up to 14 days in patients with a positive blood culture at baseline).Main Outcomes and MeasuresThe primary outcome was the proportion of patients in the primary analysis set (patients who received any amount of study drug with a baseline gram-negative pathogen not resistant to either treatment and ≥105 colony-forming units [CFU]/mL in urine culture or the same pathogen present in concurrent blood and urine cultures) who achieved overall treatment success (defined as clinical cure combined with microbiological eradication [&amp;lt;103 CFU/mL in urine] of infection). Two-sided 95% CIs were computed using the stratified Newcombe method. The prespecified noninferiority margin was −10%. If noninferiority was established, a superiority comparison was also prespecified.ResultsAmong 1041 patients randomized (mean age, 54.7 years; 573 women [55.0%]), 1034 (99.3%) received study drug and 995 (95.6%) completed the trial. Among the primary analysis set, the primary outcome occurred in 79.1% (273/345) of patients receiving cefepime/enmetazobactam compared with 58.9% (196/333) receiving piperacillin/tazobactam (between-group difference, 21.2% [95% CI, 14.3% to 27.9%]). Treatment-emergent adverse events occurred in 50.0% (258/516) of patients treated with cefepime/enmetazobactam and 44.0% (228/518) with piperacillin/tazobactam; most were mild to moderate in severity (89.9% vs 88.6%, respectively). A total of 1.7% (9/516) of participants who received cefepime/enmetazobactam and 0.8% (4/518) of those who received piperacillin/tazobactam did not complete the assigned therapy due to adverse events.Conclusions and RelevanceAmong patients with complicated UTI or acute pyelonephritis caused by gram-negative pathogens, cefepime/enmetazobactam, compared with piperacillin/tazobactam, met criteria for noninferiority as well as superiority with respect to the primary outcome of clinical cure and microbiological eradication. Abstract:
A Nyírlugoson 1962-ben beállított mutrágyázási tartamkísérletben homokos, savanyú, kovárványos barna erdotalajon vizsgáltuk a N-, P-, K-, Ca- és Mg-trágyázás és a csapadékmennyiségek hatását a rozs, burgonya, oszi búza és a triticale termésére. A talaj agrokémiai paraméterei pH(H2O): 5,8, pH(KCl): 4,6, hidrolitos aciditás: 8,1, hy1: 0,27, humusz-: 0,58%, összes-N-: 32,8, AL-P2O5-: 43, AL-K2O-tartalom: 52 mg·kg-1 értékekkel jellemezhetok. A kísérlet 1962-tol 1980-ig 2×16×4×4 = 512, 1980-tól 32×4 = 128 parcellát tartalmazott kétszeresen osztott és faktoriális véletlen blokk elrendezésben. A bruttó parcellák mérete 10×5 = 50 m² volt. 1980-ig a nitrogén 55, foszfor 24 (P2O5), kálium 52 (K2O), magnézium 10 (MgO); 1980-tól a nitrogén 75, foszfor 90 (P2O5), kálium 90 (K2O), kalcium 437,5 (CaCO3) és a magnézium 140 (MgCO3) kg·ha-1·év-1 átlagos adagokban került kijuttatásra. Fobb eredményeink az alábbiakban foglalhatók össze röviden: A rozs jó (N: 90 kg·ha-1 + NP, NK, NPK, NPKMg kombinációk) tápelem-ellátottságánál átlagos évben a termések meghaladták a 3,5 t·ha-1 mennyiséget. Aszályos évben 17, a csapadékboben 52%-kal csökkent a termés. A legkedvezobb (4 t·ha-1 körüli) termések a csapadék 400-500 mm-es mennyiségeinél voltak regisztrálhatók. A burgonya jó tápláltságánál (N: 150 kg·ha-1 + NP, NK, NPK, NPKMg kombinációk) a csapadékbo és aszályos évjáratok terméscsökkento hatása nem jelentkezett. A maximálishoz (21 t·ha-1) közeli termések a 280-330 mm közötti természetes csapadékoknál adódtak. A búzánál csapadékos évben az aszálykárt meghaladó mértékben csökkent a hozam. Az optimális csapadékok és a hozzárendelheto termések 449-495 mm és 1,7-3,4 t·ha-1 között változtak. A triticale-kísérletekben szárazságban és aszályban az egyoldalú N- és a hiányos NP- és NK-kezeléseknél 45 és 24% volt a terméskiesés, amelyet a NPK-, NPKCa-, NPKMg- és NPKCaMg-adagok még 22 és 22 %-kal fokoztak. Az 5,0-6,0 t·ha-1 körüli maximális termések az 550-600 mm közötti csapadéktartományban, 580 mm-nél mutatkoztak.

Abstract:
Melt-spun flakes and air atomised powder of a multi-component Al-Si-Fe-X alloy were consolidated by field assisted (FAST) or spark plasma sintering (SPS) in vacuum using steel dies and punches. Experiments were carried out at 350, 400, 450 and 500°C under applied loads ranging from 81 to 283 MPa. The resulting compacts were microstructurally and mechanically characterized. Ultimate strength values up to 1000 MPa and plastic strains up to 20% were observed during compression tests. The effect of the powder shape on the sintering behaviour is compared. The effect of process parameters such as temperature and applied load on the densification and mechanical properties is discussed. It was found that compacts sintered from melt-spun flakes resulted in a higher strength and ductility than compacts produced from air-atomized powder, sintered under identical conditions.
