Abstract:
Tyrimo tikslas. Išanalizuoti 1 metų duomenis apie Vilniaus miesto klinikinėje ligoninėje (VMKL) gimusių vėlyvų neišnešiotų naujagimių maitinimą, įvertinti gestacinio amžiaus įtaka natūralaus maitinimo dažniui. Metodika. Atlikta retrospektyvinė 2015 m. - 2016 m. Rezultatai. Iš 100 tyrime dalyvavusių vėlyvų neišnešiotų naujagimių iš stacionaro išimtinai žindomi išvyko 78 (78 proc.). Vėlyvi neišnešioti naujagimiai statistiškai rečiau išvyko natūraliai maitinami neišnešioti naujagimiai (p<0,05). Didžioji dalis vėlyvų neišnešiotų naujagimių (n=92, 92 proc.) toliau sėkmingai žindyti iki 3 mėn., 88 (88 proc.) iki 6 mėn., 51 (51 proc.) iki 12 mėn., 24 (24 proc.) iki ≥18 mėn. Išnešiotų ir vėlyvų neišnešiotų kūdikių iki 6 -18 mėnesių amžiaus natūralaus maitinimo ir sėkmingo žindymo rezultatai praktiškai vienodi. Išvados. Vėlyvi neišnešioti naujagimiai po gimimo rečiau natūraliai maitinami nei išnešioti naujagimiai (p<0,05). Tačiau ženklaus skirtumo tarp tolimesnio šių naujagimių sėkmingo žindymo dažnio nenustatėme.

Abstract:
Background: The aim was to investigate pharmacy students’ views on the role of the pharmacist in facilitating self-care with over-the-counter (OTC) medicines, particularly in light of new roles, and establish personal practice. Methods: Final year pharmacy students at Queen’s University Belfast were invited to participate. Data were collected via a pre-piloted questionnaire, distributed at a compulsory class (only non-identifiable data were requested). Descriptive statistics were performed, and non-parametric tests were employed for inferential statistical analysis (responses by gender). Results: The response rate was 87.6% (78/89); 34.6% (27/78) males and 65.4% (51/78) females. Over a third [34.6% (27/78)] reported using OTC medicines about once a month. All appreciated the importance of an evidence-based approach to optimize patient care. Most [(96.2% (75/78)] deemed OTC consultations should remain a fundamental responsibility of pharmacists and 69.2% (54/78) thought OTC consultations have the potential to be as complex as independent pharmacist prescribing. Females felt more confident recommending OTC emergency contraception than males (p = 0.002 for levonorgestrel and p = 0.011 for ulipristal acetate). Many [61.5% (48/78)] considered more medicines should not be deregulated from prescription-only status. Conclusions: Data from this single institution suggests that enabling self-medication is an important part of practice but there were confidence issues around deregulations.

Abstract:
Nowadays, digital libraries are becoming popular. These libraries provide many services for their users. The information retrieval service is an important service of the digital libraries. Personalizing this service in order to better satisfy users' requirements is an approach that attract much attention of the scientific community. Many present personalized information retrieval systems re-rank results of a search engine by taking into account the similarities between these results and the user profiles to return more relevant results. However, most of these systems only use content-based approaches for this purpose. In our work, we propose to use also citation-based methods like co-citation method and bibliographic coupling method to compute document-profile similarities. We study the performance of the co-citation method with different bibliographic databases. We also use many different combination functions to combine individual scores. The proposed approaches were validated by experiments with the test collection used in INEX 2005

Abstract:
This work proposes to develop a didactic approach using music in learning mathematics. These two subjects, though usually treated as fields of knowledge isolated, have similarities being possible to provide activities that relate. One of the main objectives is to include music in the school context as an interdisciplinary proposal, motivating and empowering learning mathematical concepts. Students do not learn exclusively with reason but also with recourse to intuition, the feeling, the emotion and feelings. Thus, interdisciplinarity between mathematics and music, enhances the diversity of strategies making the most dynamic and engaging teaching, leading students to feel motivated to learn. The presented activities involve these two areas, some performed in the classroom during the Supervised Teaching Practice, and the rest are possible proposals to implement a pre-school education class or in the 1st cycle of basic education. In this way, we intend to highlight the potential of music as an educational resource, applied in the context of classroom to motivate students disenchanted with mathematics.

Full text
Abstract:
This study evaluates the constitutionality of section 78(1B) of the Criminal Procedure Act 51 of 1977 (CPA), which places the burden of proving criminal capacity on the party who raises the issue, against section 9 of the Constitution of the Republic of South Africa, 1996 (CRSA). In a legal system such as ours, that has a high regard for equality, any form of unequal treatment must be scrutinised, assessed and, if found to be unjust, rooted out. Even more so where the unequal treatment affects a marginalised minority group such as the mentally disabled. This study weighs section 78(1B) against section 9(1) of the CRSA. It also weighs the section against section 9(3) of the CRSA. Attempts are made to justify possible infringements according to section 36 of the CRSA. An appropriate remedy is then ascertained. This study also provides the historical development of section 78(1B) of the CPA – both in the common law and statute. This study furthermore provides original guidelines and principles in assessing expert evidence where criminal capacity is placed in dispute due to a mental illness or defect of the accused. The main findings are that section 78(1B) infringes on both section 9(1) and section 9(3), that it cannot be justified in terms of section 36 of the CRSA and that the appropriate remedy is the striking out of the whole section from the CPA. Van, der Merwe Frederik Wilhelm. "Nie-patologiese ontoerekeningsvatbaarheid as verweer in die Suid-Afrikaanse strafreg." Diss., 1996. http://hdl.handle.net/10500/16265.

Full text
Abstract:
Text in Afrikaans
The defence of non-pathological criminal incapacity in the South African criminal law is discussed. It is a relatively new defence and should be distinguished from the defences such as youth and mental illness set out in section 78(1) of the Criminal Procedure Act 51 of 1977. This defence covers cases in which criminal capacity is excluded by factors such as intoxication and emotional stress. It is also known as a general defence of criminal incapacity. In a number of cases, inter alia in S v Arnold 1985(3) SA 256 (C); S v Campher 1987 (1) SA 940 (A) and S v Chretien 1981 (1) SA 1 097 (A) the conclusion is reached that the defence, non-pathological criminal incapacity, does have a right of existence in the South African criminal law. In order to successfully raise the defence on non-pathological criminal incapacity, the mere ipse dixit of the accused is insufficient. A proper foundation for the defence must be laid. The onus of proof is discussed.

Abstract:
At Leipzig Bach held a double position as cantor in the St. Thomas School and music director for the city as a whole. The first was primarily educational; the second included not only composition of cantatas for the two main churches but oversight of all music in the city. This chapter examines Bach’s official work as cantor and music director as well as his growing frustration and conflicts in those capacities (as evidenced in his “Entwurf”)—and the resulting shift toward greater participation in secular music making with the Collegium Musicum. After describing his routines for producing some 150 surviving church cantatas from this period, the chapter examines selected examples, including BWV 105, 78, 103, 56, 82, 51, and 140. Also discussed are secular works, including BWV 201 and the Coffee Cantata, and the motets.

Abstract:
Palakodety Radha Krishna of the Indian Institute of Chemical Technology observed (Synlett 2012, 2814) high stereocontrol in the addition of allyltrimethylsilane to the cyclic imine derived from 1. The product piperidine 2 was carried onto (+)-deoxoprosopinine 3. Glenn C. Micalizio of Scripps Florida condensed (J. Am. Chem. Soc. 2012, 134, 15237) the amine 4 with 5. The ensuing intramolecular dipolar cycloaddition led to 6, which was carried onto the Dendrobates alkaloid (–)-205B 7. Pei-Qiang Huang of Xiamen University showed (Org. Lett. 2012, 14, 4834) that the quaternary center of 9 could be established with high diastereoselectivity by activation of the lactam 8, then sequential addition of two different Grignard reagents. Subsequent stereoselective intramolecular aldol condensation led to FR901843 10. More recently, Professor Huang, with Hong-Kui Zhang, also of Xiamen University, published (J. Org. Chem. 2013, 78, 455) a full account of this work. In an elegant application of the power of phosphine-catalyzed intermolecular allene cycloaddition, Ohyun Kwon of UCLA added (Chem. Abstract:
Researchers have tried to assess the morphological and mechanical characteristics of bone using various methods with a limited success (Mow and Hayes, 1997, and Cowin, 2001). This may be due to the complexity for documentation and characterization of architecture by the anisotropic and nonhomogeneous characteristic of the bone. The bones of the shoulder joint complex are particularly deprived of information on their mechanical and morphological properties. Very little is known for example on the mechanics and morphology of the glenoid of the shoulder. The present work presents an attempt to identify a proper method for the measurement and characterization of the morphology and mechanics of bone, particularly in the glenoid. Our results show that trabecular lines are directed at 82.15° ± 7.98° relative to the glenoid articular surface, young’s modulus obtained from CT scan was 230 ± 8 MP, and the elastic tensors obtained from cadaver were 326 ± 78 MPa for E2222, 144 ± 22 MPa for E1111=E3333, 49 ± 7 MPa for E3311, 59 ± 9 MPa for E1122=E2233, 51 ± 8 MPa for E1313, and 70 ± 10 MPa for E1212=E2323. These results were consistent with Wolf’s Law and were in agreement with results reported in literatures (Anglin et al., 1999, Frich et al., 1998, and Mansat et al., 1998).

Abstract:
Medical marijuana (MMJ) is legal in the state of Florida for the treatment of specific qualifying medical conditions.1,2 As of July 2020, over 2,450 physicians are authorized to order MMJ, and 360,000 patients are registered in Florida’s MMJ program.3 With this rapid uptake come concerns regarding physicians’ knowledge about MMJ,4–7 and the lack of preparing physicians-in-training to manage MMJ.4,7,8 We conducted a state-wide survey of certified MMJ providers in Florida. The survey was developed by the Consortium for Medical Marijuana Clinical Outcomes research team. The aim of the survey was to inform physicians of the mission of the consortium, which is to support and disseminate research. The survey items were developed accordingly, and the survey was pilot tested with a small group of physicians. We identified all physicians licensed to certify patients for MMJ who care currently practicing in the State of Florida (n=1609), to investigate their information sources and training needs regarding MMJ. The survey was disseminated via mail and email, including a $40 incentive for survey completion. Preliminary responses from 51 (5%) providers (mean age 56, 74% male) are summarized here. The sample included providers from 22 Florida counties and represented a broad range of medical specialties. The majority (92%) practiced in both medical marijuana and traditional medical practice. To learn about MMJ, 98% used research articles, 90% used online sources, 86% learned from dispensary staff, 84% learned from discussions with other providers, 72% used books, 65% used conferences, 61% used magazines, and 35% had a personal experience with marijuana. The sources most cited as “very useful” were conferences (51%), research articles (50%), discussions with other providers (47%), and online sources (47%). Topics rated as a high priority for training included drug-MMJ interactions (80%), strategies to help patients reduce their use of opioids or other drugs (80%), information about the selection of doses and CBD: THC ratios (80%), evidence for managing specific medical conditions or symptoms (78%), information about the effect of different phytocannabinoids and terpenes (75%), advantages and disadvantages of specific modes of delivery (71%), general updates on research findings (71%), educational information about the endocannabinoid system (67%), the safety of medical marijuana use (55%), identification and management of cannabis use disorder (51%), and comparison of products available in different dispensaries (49%). The majority of providers either strongly agreed or agreed (77%) that they could provide better care if they knew which products their patients receive at dispensaries. Physicians use a blend of primary research, online sources, and exchanges with colleagues to learn about MMJ. Perceived needs for more pharmacological information and indication-specific detail for treatment regimen were high. Most physicians believe that details on dispensed MMJ would improve patient care.

Abstract:
The dose response (DR) of tissue plasminogen activator (t-PA) in patients (pts) with acutemyocardial infarction (AMI) and the efficacy of 4-6 hr maintenance infusions (M) of t-PA in preventing early reocclusion (R) have been reported in small numbers of pts. No studies have determined the efficacy of longer M in limiting short term R.In two related trials weexamined DR and long term M in a large numberof pts. We administered a uniform t-PA product [>98% double chain (DC)] to 223 pts 3.1 ±1.1 hrs after the onset of AMI.Pts received 0.25 - 0.95 megaunits (MU)/kg/90min and then underwent coronary angiography(CA):Responders were defined by TIMI grades 2 or 3. Results were In a logistic regression anaylsis the coefficient for dose was significant(p<.02). Two subsets of the above pts received M. Groups A (4l pts) and B (51 pts) had received 0.26 - 0.40 and 0.4l - 0.54 MU/kg/90 min respectively. 66% of A and 78%of B were responders, received an additional 90 minof t-PA at 1/3 the initial rate and then 1 of4 M for 9-21 hrs (mean total 17.2±4.5 hrs) at which time CA was repeated. Results for A + B were:For all pts fibrinogen decreases were mild. Bleeding was primarily related to vascular invasion and M length. We conclude that the DR for DC tPA can be accurately defined and long term M to limit .short term R are feasible. Future studies may define t-PA dosing regimens suited for specific clinical situations.

Abstract:
We have previously demonstrated binding of the CNBr-split N-terminal disulphide knot of the fibrinogen molecule (N-DSK) to blood platelets and to their immunoprecipitated fibrinogen receptor, the glycoprotein IIb-IIIa complex. To further investigate which part of the N-DSK molecule that is responsible for its binding to GP IIb-IIIa, this fragment was split into its separate Aα (1-51), Bβ (1-118) and γ- (1-78) chains and carboxymethylated. GP IIb-IIIa was immunoprecipitated by crossed immunoelectrophoresis (CIE) of Triton X-100 extracts of platelets against rabbit antibodies to whole platelet proteins. The CIE plates were incubated with 125-I-radiolabelled N-DSK chains, and investigated for binding by autoradiography. The γ-chain but not the Aβ or Bβchains demonstrated binding to the GP IIb-IIIa. These results demonstrate that the fibrinogen molecule contains a third sequence of amino acids which is capable of binding to the fibrinogen receptor of the blood platelets, in addition to the two previously reported sequences C-terminally in the Aα-and γ-chains. Fragment E derived from fibrinogen (E fg) does not interact with the fibrinogen receptor. Thus, the γ-chain interaction site can not be present in the E fg-γ-chain (1-53). However, fragment E derived from fibrin (E1) with the γ-chain of residues 1-62, does react. The residues of N-DSK and E1 which are involved in binding to GP IIb-IIIa therefore appear to be present in the γ-chain sequences 54-62.

Abstract:
The Croatian islands make the second-largest archipelago in the Mediterranean. There are 1244 islands (78 islands, 524 islets, and 642 rocks and rocks awash) that are situated in a range of 450 km along the eastern coast of the Adriatic Sea. The total area covers 3300 km2 (about 5.8 % of Croatian mainland territory) and the length of their coast is 4057 km. Twenty of these islands occupy an area larger than 20 km2 each, and the other 58 ones have a surface from 1 km2 to 20 km2. There are 47 islands that are permanently inhabited. Administratively, islands belong to seven coastal counties and 51 island towns/municipalities. Several small islands are in the jurisdiction of seven coastal cities. By the Constitution, islands are considered as a region of special protection and unique value. Islands specificities require development policy that takes account of demographic, economic, environmental, and other island issues. In the situation of the COVID-19 pandemic, island development policy becomes even more challenging and complex. This paper aims to analyse the attitudes and perceptions of the islanders about the living conditions on the island and the impact of public policies on island development. Identifying the problems from the local perspective helps us to evaluate the success of island development policy. Effective island policy increases the quality of life of the islanders while respecting island specifics. For this purpose, the results of a survey conducted on a sample of the inhabitants of Croatian islands conducted in 2020 are used. The surveys were conducted using the online tool LimeSurvey. The results indicate that 42 percent of the islanders think that living conditions on the islands are not improving. Most of the islanders think that government policy does not encourage island development. The paper also identifies factors that are responsible for differences in attitudes and perceptions of islanders.

Abstract:
Objective: Breast cancer (BC) in young women with positive hormone receptors (HR+) has a poor prognosis. There are possible clinical and biological explanations for these findings, being partially attributed to suboptimal adherence to endocrine therapy (ET). Fertility is one of the key factors for treatment discontinuation. This study aims to estimate the adherence of ET and the overall survival (OS) rate of BC/HR+ in young patients. Methods: We identified women from the public health system, diagnosed with stage I–III BC presenting at one single BC Center, between January 2006 and December 2015. Using the medical records of the hospital database, we constituted a cohort of 74 women aged ≤40 years. The discontinuation rate with associated factors and OS were summarized as percentages. Results: A total of 51 women were BC/HR+. The mean age at diagnosis was 35 years. The median follow-up was 89 months. Among them, 45% had BC recurrence (local and/or distance), and 21% died. A total of 15% of patients interrupted the ET. The reasons for interruption were pregnancy (three patients), menstrual disorders (two patients), and irregular adherence (three patients). Tamoxifen (TMX) was prescribed in 74% of cases. About 19% switched their treatment to aromatase inhibitors. The genetic risk assessment was recommended to 58% of patients, 13% performed genetic tests, and 2% of patients carried out the pathogenic mutation in the high-risk BC genes. Conclusion: This cohort showed 84% of ET intake should be improved. Pregnancy issues and irregular adherence were the main reasons for discontinuation. Considerations of 78% OS in 7 years are that TMX was the only ET for most of the cohort, and it is considered undertreatment according to the current recommendations, and the low rate of genetic tests performed leads unrecognized high risk of potential recurrence in women with hereditary BC. Research in medical records should be addressed as a limitation.

Abstract:
Isolated cases of lupus anticoagulants (LA) in association with procainamide have been reported. This study was done to estimate the frequency of LA in patients taking procainamide. Two groups of patients were evaluated: Group A: 110 hospitalized patients (84 males, 26 females, ages 51-78, mean age 74.3) and Group B: 80 ambulatory patients (54 males, 26 females, ages 36-89, mean 67.5 years). The latter group of patients was on this drug for a period of two-five years, while the former group hadbeen on drug at least two full days. All patients were screened with baseline laboratory data including activated partial thromboplastin time (APTT) and prothrombin time (PT) which were performed using Auto APTT® and Simplastin® on a Coagulamate X2® (General Diagnostics/Organon Teknika). Patients taking drugs known to alter the APTT and PT were excluded. All patients were followed with daily (hospital patients) or weekly (ambulatory patients) APTT and PT. Prolongation of the APTT of 5 sec or PT of 3 sec over baseline was considered as a positive LA screening test. Patients with a positive screening test were further evaluated with tissue thromboplastin inhibitor assay (TTI), platelet neutralization procedure (PNP, anti-nuclear antibodies (ANA) and blood serology (RPR). In Group A. 11 out of 110 (1096) developed prolonged APTT while on procainamide . Of these, 9 developed abnormal TTI and 2 had positive PNP. The ANA was positive (titers of 1:320-1:2560) in 10 patients with the only positive RPR test, being in the patient with the highest ANA titer. In Group B, 12 out of 80 (1596) developed prolonged APTT, 11 had positive TTI. The ANA titer was elevated in all positive cases, although one patient's titer was 1:30. The PNP was positive in 1/12. All blood serologies were negative in this group. The difference in incidence between Groups A and B may reflect longer exposure to drug in the latter group. This difference is not statistically significant. This study indicates that the incidence of procainamide-induced lupus antocoagulant is between 10-1596 when the APTT (or PT) is used as a screening test. The TTI and ANA seem to have equal sensitivity in this syndrome when taking this drug. The failure of the PNP to be sensitive to LA may be due to the minimal prolongation of the APTT arbitrarily chosen as representing a positive screening test.
