Abstract:
Study objective To identify sex differences associated with caudal epidurals, the most commonly used technique of pediatric regional anesthesia, based on individually validated data of ultrasound-guided blocks performed between 04/2014 and 12/2020. Methods Prospectively collected and individually validated data of a cohort of children aged between 0–15 years was analyzed in a retrospective observational study. We included pediatric surgeries involving a primary plan of caudal epidural anesthesia under sedation (without airway instrumentation) and a contingency plan of general anesthesia. Sex-specific rates were analyzed for overall failure of the primary anesthesia plan, for residual pain, for block-related technical complications and for critical respiratory events. We used Fisher´s exact tests and multivariable logistic regressions were used to evaluate sex-specific associations. Results Data from 487 girls and 2060 boys ≤15 years old (ASA status 1 to 4) were analyzed. The primary-anesthesia-plan failure rate was 5.5% (95%CI 3.8%-7.8%) (N = 27/487) among girls and 4.7% (95%CI 3.9%-5.7%) (N = 97/2060) among boys (p = 0.41). Residual pain was the main cause of failure, with rates of 4.5% (95%CI 2.9–6.6%) (N = 22/487) among girls and 3.0% (95%CI 2.3–3.8%) (N = 61/2060) among boys (p = 0.089). Block-related technical complications were seen at rates of 0.8% (95%CI 0.3%-1.9%) (N = 4/487) among girls vs 2.5% (95%CI 0.5–2.7%) (N = 51/2060) among boys and, hence, significantly more often among male patients (p = 0.023). Male sex was significantly associated with higher odds (adjusted OR: 3.18; 95% CI: 1.12–9; p = 0.029) for such technical complications regardless of age, ASA status, gestational week at birth or puncture attempts. Critical respiratory events occurred at a 1.7% (95%CI 1.2%-2.3%) rate (N = 35/2060) twice as high among boys as 0.8% (95%CI 0.3%-1.9%) (N = 4/487) among girls (p = 0.21). Conclusions While the the primary-anesthesia-plan failure rate was equal for girls and boys, technical complications and respiratory events are more likely to occur in boys.

Abstract Background In the United States, every thirty nine seconds an individual dies from complications from Cardiovascular Diseases. Persistent thrombus formation at the genesis of these diseases, such as stroke and other coagulation disorders, has no full model to date. Intrinsically blood clots are produced due to excessive/unnecessary thrombin formation, which leads to the conversion of fibrinogen to fibrin. As a result the regulation of thrombin formation is critical in controlling clot generation. Upon vasculature damage, the proteolytic conversion of prothrombin (Pro) to thrombin compatible to rates of survival is catalyzed by the prothrombinase complex composed of the enzyme, factor Xa (fXa), the cofactor, factor Va (fVa), assembled on a phospholipid membrane in the presence of calcium ions. Although fXa is capable of activating Pro through the initial cleavage at Arg271 followed by the cleavage at Arg320 (pre2 pathway), it would take approximately six months to form a clot. However, the incorporation of fVa into prothrombinase results in a five-fold increase in the catalytic efficiency of fXa for thrombin generation and the order of cleavages reversed (meizo pathway). Thus the timely arrest of unwarranted bleeding is due to the assembly of prothrombinase at the site of injury. Inevitably the presence and absence of fVa dictates the pathway of Pro activation and previous studies have suggested that fXa interacts with Pro within amino acid region 473-487 in a fVa-dependent manner. Aim To evaluate the role amino acid region 473-487 of Pro has in coagulation. Methods A recombinant Pro molecule with the region 473-487 was deleted (rProΔ473-487) using site-directed mutagenesis. Methotrexate was used for selection to stably transfect BHK-21 cells with rProΔ473-487 and wild-type Pro (rProWT). The two recombinant molecules were purified according to a well-established protocol and, at the last step, Fast Performance Liquid Chromatography was used equipped with a strong anionic Mono-Q 5/50 column. Properly carboxylated rProΔ473-487 and rProWT was isolated and removed from the column by utilizing a calcium gradient. Subsequently Pro deficient plasma was used to assess the molecules clotting activities on a Diagnostica Stago STart® 4 Hemostasis Analyzer. Gel electrophoresis was used to evaluate both recombinant molecules and their ability to generate active thrombin by either the multifaceted prothrombinase or fXa alone. Further studies were then performed using generated recombinant thrombin from the recombinant Pro molecules to investigate in their ability to activate procofactors V (fV) & VIII (fVIII). Results The investigation into the Activated Partial Thromboplastin Time [APTT] revealed clotting activity for human Pro and rProWT to be comparable, whereas rProΔ473-487 was substantially limited in the process of forming a fibrin clot. Next gel electrophoresis and scanning densitometry indicated the consumption of rProΔ473-487 by prothrombinase and subsequent thrombin formation was decreased 24-fold when compared to rProWT. In contrast membrane-bound fXa alone, in the absence of fVa, exhibited a 6-fold increase in the rate of initial cleavage Arg271 and subsequent activation of rProΔ473-487. Both recombinant Pro molecules demonstrated a similar cleavage pattern of activation equivalent with human Pro suggesting no structural alterations took place in rProΔ473-487following the mutation. Furthermore, generated human thrombin and recombinant wild-type thrombin were found to activate fV and fVIII within five minutes while the recombinant mutant thrombin was impeded in the activation process out to three hours. Conclusion Overall the data demonstrate that amino acid sequence 473-487 of Pro plays a preeminent role in 1) timely activation of Pro at initial cleavage Arg320 by prothrombinase, and 2) suitable macromolecular procofactor activation. Thus there is incisive rationale why no major mutations have been identified in this dynamic region which would be problematic for inherent physiological hemostasis. The spectrophotometric properties of the green fluorescent protein (GFP) result from the post-translationally cyclized chromophore composed of three amino acids including a tyrosine at the center of the β-barrel protein. Altering the amino acids in the chromophore or the nearby region has resulted in numerous GFP variants with differing photophysical properties. To further examine the effect of small atomic changes in the chromophore on the structure and photophysical properties of GFP, the hydroxyl group of the chromophore tyrosine was replaced with a nitro or a cyano group. The structures and spectrophotometric properties of these superfolder GFP (sfGFP) variants with the unnatural amino acids (UAAs) 4-nitro-L-phenylalanine or 4-cyano-L-phenylalanine were explored. Notably, the characteristic 487 nm absorbance band of wild-type (wt) sfGFP is absent in both unnatural amino-acid-containing protein constructs (Tyr66pNO2Phe-sfGFP and Tyr66pCNPhe-sfGFP). Consequently, neither Tyr66pNO2Phe-sfGFP nor Tyr66pCNPhe-sfGFP exhibited the characteristic emission of wt sfGFP centered at 511 nm when excited at 487 nm. Tyr66pNO2Phe-sfGFP appeared orange due to an absorbance band centered at 406 nm that was not present in wt sfGFP, while Tyr66pCNPhe-sfGFP appeared colorless with an absorbance band centered at 365 nm. Mass spectrometry and X-ray crystallography confirmed the presence of a fully formed chromophore and no significant structural changes in either of these UAA-containing protein constructs, signaling that the change in the observed photophysical properties of the proteins is the result of the presence of the UAA in the chromophore.

Background The digital health divide for socioeconomic disadvantage describes a pattern in which patients considered socioeconomically disadvantaged, who are already marginalized through reduced access to face-to-face health care, are additionally hindered through less access to patient-initiated digital health. A comprehensive understanding of how patients with socioeconomic disadvantage access and experience digital health is essential for improving the digital health divide. Primary care patients, especially those with chronic disease, have experience of the stages of initial help seeking and self-management of their health, which renders them a key demographic for research on patient-initiated digital health access. Objective This study aims to provide comprehensive primary mixed methods data on the patient experience of barriers to digital health access, with a focus on the digital health divide. Methods We applied an exploratory mixed methods design to ensure that our survey was primarily shaped by the experiences of our interviewees. First, we qualitatively explored the experience of digital health for 19 patients with socioeconomic disadvantage and chronic disease and second, we quantitatively measured some of these findings by designing and administering a survey to 487 Australian general practice patients from 24 general practices. Results In our qualitative first phase, the key barriers found to accessing digital health included (1) strong patient preference for human-based health services; (2) low trust in digital health services; (3) high financial costs of necessary tools, maintenance, and repairs; (4) poor publicly available internet access options; (5) reduced capacity to engage due to increased life pressures; and (6) low self-efficacy and confidence in using digital health. In our quantitative second phase, 31% (151/487) of the survey participants were found to have never used a form of digital health, while 10.7% (52/487) were low- to medium-frequency users and 48.5% (236/487) were high-frequency users. High-frequency users were more likely to be interested in digital health and had higher self-efficacy. Low-frequency users were more likely to report difficulty affording the financial costs needed for digital access. Conclusions While general digital interest, financial cost, and digital health literacy and empowerment are clear factors in digital health access in a broad primary care population, the digital health divide is also facilitated in part by a stepped series of complex and cumulative barriers. Genuinely improving digital health access for 1 cohort or even 1 person requires a series of multiple different interventions tailored to specific sequential barriers. Within primary care, patient-centered care that continues to recognize the complex individual needs of, and barriers facing, each patient should be part of addressing the digital health divide.

Abstract:
Fourteen, 3-month-old calves were intracerebrally inoculated with the agent of chronic wasting disease (CWD) from white-tailed deer (CWDwtd) to compare the clinical signs and neuropathologic findings with those of certain other transmissible spongiform encephalopathies (TSE, prion diseases) that have been shown to be experimentally transmissible to cattle (sheep scrapie, CWD of mule deer [CWDmd], bovine spongiform encephalopathy [BSE], and transmissible mink encephalopathy). Two uninoculated calves served as controls. Within 26 months postinoculation (MPI), 12 inoculated calves had lost considerable weight and eventually became recumbent. Of the 12 inoculated calves, 11 (92%) developed clinical signs. Although spongiform encephalopathy (SE) was not observed, abnormal prion protein (PrPd) was detected by immunohistochemistry (IHC) and Western blot (WB) in central nervous system tissues. The absence of SE with presence of PrPd has also been observed when other TSE agents (scrapie and CWDmd) were similarly inoculated into cattle. The IHC and WB findings suggest that the diagnostic techniques currently used to confirm BSE would detect CWDwtd in cattle, should it occur naturally. Also, the absence of SE and a distinctive IHC pattern of CWDwtd and CWDmd in cattle suggests that it should be possible to distinguish these conditions from other TSEs that have been experimentally transmitted to cattle.

BackgroundCollagen and C1q in the extracellular matrix (ECM), are the predominant ligands for LAIR-1, an inhibitory receptor expressed on the cell surface of several immune cell subsets that inhibits immune activation and migration. LAIR-2, a soluble homolog of LAIR-1, competes for binding to collagen and C1q and serves as a natural decoy to promote immune function under normal conditions. Dysregulation of the ECM and increased expression of collagen and C1q within the tumor microenvironment (TME) plays a critical role in promoting tumor progression. NC410 was engineered to overcome this highly immunosuppressive environment by blocking LAIR-1 function, reversing immune suppression, and inducing ECM remodeling to promote immune cell infiltration within the TME.MethodsThis is a first in human, phase 1/2, open-label, single-armed dose-escalation study to determine the safety, tolerability, dose-limiting toxicity (DLT), maximum tolerated dose (MTD), recommended phase 2 dose, preliminary efficacy and to explore pharmacodynamic biomarkers of NC410 (figure 1). Key eligibility criteria include subjects with advanced or metastatic solid tumors with measurable disease based on RECIST v1.1.ResultsAs of 07/22/2021, a total of 16 patients have been enrolled, treated, and completed the DLT period. NC410 (up to 60 mg), was well tolerated, with no safety concerns, infusion-related toxicities, or DLT reported. No anti-drug antibody (ADA) was detected post-NC410 up to 60 mg treatment. As expected, the C1Q level decreased immediately after the NC410 infusion and was replenished after two hours. Evaluation from samples to date available up to cycle 5 suggests that there was no reduction in the baseline C1Q level with subsequent dosing (figure 2). LAIR-2 levels continued to increase in a dose-dependent fashion post-NC410 dosing and marginal increase pre-dose (figure 3). Interestingly, we observed an increase in soluble LAIR-1 over time in a similar pattern to LAIR-2 (figure 4). Furthermore, immunophenotyping of patient whole blood suggests a trend towards an increase in CD4+ and CD8+ T cells including LAIR-1+ T cells in cohort 4, although overall expression levels of LAIR-1 did not appear to increase (ongoing analysis). Hemiballismus-hemichorea is an uncommon disorder. In the past, patients developing hemiballismus and hemichorea were considered to have poor prognosis with high morbidity and even mortality at times. While majority of patients of hemiballismus go into spontaneous remission, some severe cases require prolonged treatment. We here describe two such cases of which one presented to us with post stroke hemiballismus, unresponsive to monotherapy and the other was an HIV positive patient with unresponsive hemichorea secondary to CNS toxoplasmosis. Both these patients responded well when combination therapy was instituted and had normal recovery. A natural mineral, palygorskite clay, was used to synthesize pure zeolite X via a two-stage method. The synthetic parameters, such as calcination, crystallization time, and SiO2/Al2O3molar ratio, were investigated. The results indicated that calcination enhanced the extraction of silica from palygorskite clay and was beneficial for the formation of zeolite X. Micro-sized pure zeolite X with the particle size of 1-4 μm was obtained in the gel with the SiO2/Al2O3molar ratio range of 1.93 to 3 after hydrothermal treatment at 80 °C for 5 h. The synthetic zeolite X showed high specific surface area (487 m2/g) and low Si/Al ratio (1.17).

Isolation and identification of one chemical compound from the seeds of soursop, Annona muricata (Annonaceae) based on bioactivity-guided fractionation by the Brine Shrimp Lethality Assay (BSLA) have been done. The powder of soursop seed was extracted successively with n-hexane, chloroform, and methanol then the methanol extract was partitioned with ethylacetate water. Isolation and purification of ethylacetate extracts were carried out by column chromatography [(SiO2; i. n-hexane-chloroform = 15:1 ~ 1:1 gave one pure isolatedcompound. Based on Ultra Violet (UV), Infra-Red (IR), Nuclear Magnetic Resonance (NMR), and mass spectra (LC-MS/MS), the isolated Fr-4 was an acetogenin compound, sabadelin which had a weak cytotoxicity LC50 of 487 ppm.

Досліджено лісівничо-таксаційні показники та санітарний стан дубових деревостанів різного віку за участю у їх складі липи дрібнолистої. Встановлено, що тут формуються складні деревостани за участю дуба звичайного, ясена звичайного, граба звичайного, липи дрібнолистої, в’яза голого, кленів гостролистого і явора та інших деревних видів. Частка липи у їх складі становить від поодиноких дерев до 6-8 одиниць. Серед них переважають деревостани, в яких частка липи становить 2-4 одиниці. Дуб звичайний росте за ІІ-Іb бонітетом та досягає максимальної продуктивності (Іа-Іb бонітет) у деревостанах, де частка липи у складі змінюється в межах 2-8 одиниць і відстає в рості від дуба за висотою на 10,0-46,1%. Найгіршим ростом дуб звичайний характеризується в деревостанах, де відмінності за висотою між ним і липою становлять до 7%. Повнота деревостанів і запас стовбурової деревини відзначаються високою варіабельністю навіть у межах вікових груп. У середньовікових деревостанах абсолютна повнота становить 21,4-34,0 м2/га, а запас деревини – 172-374 м3/га. У пристигаючих деревостанах ці показники, відповідно, становлять 24,6-34,9 м2/га і 263-409 м3/га, а в стиглих – 26,6-40,3 м2/га і 330-487 м3/га. Найбільшого запасу (409-487 м3/га) досягають дубові деревостани, де частка липи у їх складі становить 3-6 одиниці. Водяні пагони у дерев дуба не виявлено на 20% дослідних ділянок. В інших деревостанах різної повноти і продуктивності кількість дерев дуба з водяними пагонами становить 5,3-53,3%. У деревостанах переважають дерева І і ІІ категорій санітарного стану. Сухі дерева трапляються на 70% дослідних ділянок в кількості 1,2-7,2%. Індекс санітарного стану дуба змінюється в межах 1,36-2,65.

In this study, we investigated the impact of high-energy milling on the structural and dielectric properties of Pb[Zr(1-x)Tix ]O3 [PZT] ceramics synthesized using the solid-state reaction process. The sample was milled for 2, 4 & 6 hours using a high-energy ball milling machine. The unit cell structure for all of the samples was observed to be monoclinic, according to x-ray diffraction measurements (space group: C1m1). A significant reduction in crystallite size was observed, from 132 nm to 46 nm after 6 hours of milling. The dielectric study indicated a classical ferroelectric type behaviour for the un-milled sample and diffused phase transition for all milled samples. However dielectric constant dropped from 940 to 487 after 6 hours of milling.

1 A ten-year (1980-1989) retrospective study of poisoning admissions due to food poisoning at six referral hospitals in urban Zimbabwe revealed 487 cases. This constituted 8.1 % of all poisoning admission cases (6018). 2 The majority of patients were among the 2.1-5 (20%), 5.1-10 (16%) and 21-30 (15%) year age groups in that descending order. 3 The agents implicated in food poisoning were mushrooms (47%), food-borne and other food-related toxins (37%), Elephants Ear (10%), alcohol (7%) and cassava (1 %). 4 The majority of poisoning cases were accidental, accounting for 99% of the total. 5 The treatment regimens mostly used included agents for correcting fluid and electrolyte imbalance (22.1 %), antibiotics (16.7%) and supportive measures (15.5%). 6 Mortality was recorded at 2.5% and the main agents associated with fatality were food-related toxins (5 cases), mushrooms (4 cases) and alcohol (4 cases). 7 The prevention and treatment of poisoning caused by food poisoning is mandatory in the effort to reduce poisoning caused by these agents.

Abstract:
Aim: to evaluate the clinical efficacy and safety of the hyaluronic acid (HA) with scapulohumeral periarthritis.Patients and Methods: the study involved 42 patients (mean age 55.8±3.03 years) with scapulohumeral periarthritis, who were divided into 2 groups. Group 1 received HA once a week around periarticular tissue of the shoulder joint. Course duration — 3 injections. Group 2 included patients (n=23) who were treated with glucocorticosteroids (GCS) once for the entire treatment course. The drug was re-admin-istered only if there was no clinical effect. Before and after the therapy, the severity of pain in active and passive movements was assessed using a 100-mm long visual-analog scale, as well as Health Assessment Questionnaire disability index in diseases of the shoulder girdle and Oxford Shoulder Score.Results: during therapy, there was a significantly more pronounced reduction in the shoulder joint pain, as well as a decrease in disability index due to shoulder joint disorder in group 1 versus group 2. In group 1, the rehabilitation potential exceeded that of group 2, in 18 (95%) patients of group 1 there was a positive response to the therapy, and only group 1 had pain in the shoulder joint, while in the same period of follow-up in group 2, 8 (35%) patients did not reach the values of group 1. No systemic adverse events or changes in the hemogram were registered in patients from both groups.Conclusion: the use of HA enhances the rehabilitation potential in patients with scapulohumeral periarthritis. ObjectiveThis study aimed to investigate the non-prescription use of antibiotics for cough among children under 5 years in China.DesignA community-based cross-sectional survey.SettingA face-to-face interview based on a standard questionnaire in the community from October to December 2019.ParticipantsA total of 3102 children under 5 years of age were enrolled with probability proportionate to size sampling method. The children’s caregivers provided the responses as their agents.Outcome measuresCough in the past month, non-prescription use of antibiotics after cough.Results1211 of 3102 children were reported to have a cough in the past month. Of these, 40.2% (487/1211) were medicated with antibiotics, and 18.7% (91/487) of these were not prescribed. Cephalosporins were the most frequently used antibiotic (52.8%), and community pharmacies were the main source (53.7%). Children who coughed for 1–2 weeks (OR 1.73, 95% CI 1.03 to 2.90) or 3–4 weeks (OR 2.39, 95% CI 1.08 to 4.97), with runny nose (OR 1.86, 95% CI 1.13 to 3.19) or those whose family annual income between ¥50 000 and ¥100 000 (OR 4.44, 95% CI 1.52 to 18.95) had a higher risk of non-prescription use of antibiotics than those coughing for <1 week, without runny nose or with family annual income <¥50 000.ConclusionsOur findings indicated that a high proportion of infants and young children had been treated with antibiotics for cough, and nearly one in five of them were used without prescription. More public health campaigns and further education on the appropriate use of antibiotics are needed to ensure the rational treatment of cough in children.

Our results show that the G6PD activities of these three mutants were greatly reduced. No significant alteration in G6PD kinetics was observed for both 487 and 493 mutations. However, a drastic reduction in the Km for G6P (4-fold decrease) and tremendous increases in utilization rates of 2-deoxy G6P (32-fold increase) and deamino NADP (6-fold increase) were associated with the 592 mutation. This results suggests that arginine 198 in human G6PD, possibly located within the putative G6P binding domain, may play an important role in binding the substrate G6P. In addition, we and others have recently identified that at least nine different types of mutations are responsible for G6PD deficiency in Chinese. However, a drastic reduction in the Km for G6P (4-fold decrease) and tremendous increases in utilization rates of 2-deoxy G6P (32-fold increase) and deamino NADP (6-fold increase) were associated with the 592 mutation. This results suggests that arginine 198 in human G6PD, possibly located within the putative G6P binding domain, may play an important role in binding the substrate G6P. In addition, we and others have recently identified that at least nine different types of mutations are responsible for G6PD deficiency in Chinese. In this report, we also present the occurrence rate of each mutation present in the population of Taiwan.
