Abstract:
Study objective To identify sex differences associated with caudal epidurals, the most commonly used technique of pediatric regional anesthesia, based on individually validated data of ultrasound-guided blocks performed between 04/2014 and 12/2020. Methods Prospectively collected and individually validated data of a cohort of children aged between 0–15 years was analyzed in a retrospective observational study. We included pediatric surgeries involving a primary plan of caudal epidural anesthesia under sedation (without airway instrumentation) and a contingency plan of general anesthesia. Sex-specific rates were analyzed for overall failure of the primary anesthesia plan, for residual pain, for block-related technical complications and for critical respiratory events. We used Fisher´s exact tests and multivariable logistic regressions were used to evaluate sex-specific associations. Results Data from 487 girls and 2060 boys ≤15 years old (ASA status 1 to 4) were analyzed. The primary-anesthesia-plan failure rate was 5.5% (95%CI 3.8%-7.8%) (N = 27/487) among girls and 4.7% (95%CI 3.9%-5.7%) (N = 97/2060) among boys (p = 0.41). Residual pain was the main cause of failure, with rates of 4.5% (95%CI 2.9–6.6%) (N = 22/487) among girls and 3.0% (95%CI 2.3–3.8%) (N = 61/2060) among boys (p = 0.089). Block-related technical complications were seen at rates of 0.8% (95%CI 0.3%-1.9%) (N = 4/487) among girls vs 2.5% (95%CI 0.5–2.7%) (N = 51/2060) among boys and, hence, significantly more often among male patients (p = 0.023). Male sex was significantly associated with higher odds (adjusted OR: 3.18; 95% CI: 1.12–9; p = 0.029) for such technical complications regardless of age, ASA status, gestational week at birth or puncture attempts. Critical respiratory events occurred at a 1.7% (95%CI 1.2%-2.3%) rate (N = 35/2060) twice as high among boys as 0.8% (95%CI 0.3%-1.9%) (N = 4/487) among girls (p = 0.21). Conclusions While the the primary-anesthesia-plan failure rate was equal for girls and boys, technical complications and respiratory events are more likely to occur in boys.

Abstract Background In the United States, every thirty nine seconds an individual dies from complications from Cardiovascular Diseases. Persistent thrombus formation at the genesis of these diseases, such as stroke and other coagulation disorders, has no full model to date. Intrinsically blood clots are produced due to excessive/unnecessary thrombin formation, which leads to the conversion of fibrinogen to fibrin. As a result the regulation of thrombin formation is critical in controlling clot generation. Upon vasculature damage, the proteolytic conversion of prothrombin (Pro) to thrombin compatible to rates of survival is catalyzed by the prothrombinase complex composed of the enzyme, factor Xa (fXa), the cofactor, factor Va (fVa), assembled on a phospholipid membrane in the presence of calcium ions. Although fXa is capable of activating Pro through the initial cleavage at Arg271 followed by the cleavage at Arg320 (pre2 pathway), it would take approximately six months to form a clot. However, the incorporation of fVa into prothrombinase results in a five-fold increase in the catalytic efficiency of fXa for thrombin generation and the order of cleavages reversed (meizo pathway). Thus the timely arrest of unwarranted bleeding is due to the assembly of prothrombinase at the site of injury. Inevitably the presence and absence of fVa dictates the pathway of Pro activation and previous studies have suggested that fXa interacts with Pro within amino acid region 473-487 in a fVa-dependent manner. Aim To evaluate the role amino acid region 473-487 of Pro has in coagulation. Methods A recombinant Pro molecule with the region 473-487 was deleted (rProΔ473-487) using site-directed mutagenesis. Methotrexate was used for selection to stably transfect BHK-21 cells with rProΔ473-487 and wild-type Pro (rProWT). The two recombinant molecules were purified according to a well-established protocol and, at the last step, Fast Performance Liquid Chromatography was used equipped with a strong anionic Mono-Q 5/50 column. Properly carboxylated rProΔ473-487 and rProWT was isolated and removed from the column by utilizing a calcium gradient. Subsequently Pro deficient plasma was used to assess the molecules clotting activities on a Diagnostica Stago STart® 4 Hemostasis Analyzer. Gel electrophoresis was used to evaluate both recombinant molecules and their ability to generate active thrombin by either the multifaceted prothrombinase or fXa alone. Further studies were then performed using generated recombinant thrombin from the recombinant Pro molecules to investigate in their ability to activate procofactors V (fV) & VIII (fVIII). Results The investigation into the Activated Partial Thromboplastin Time [APTT] revealed clotting activity for human Pro and rProWT to be comparable, whereas rProΔ473-487 was substantially limited in the process of forming a fibrin clot. Next gel electrophoresis and scanning densitometry indicated the consumption of rProΔ473-487 by prothrombinase and subsequent thrombin formation was decreased 24-fold when compared to rProWT. In contrast membrane-bound fXa alone, in the absence of fVa, exhibited a 6-fold increase in the rate of initial cleavage Arg271 and subsequent activation of rProΔ473-487. Both recombinant Pro molecules demonstrated a similar cleavage pattern of activation equivalent with human Pro suggesting no structural alterations took place in rProΔ473-487following the mutation. Furthermore, generated human thrombin and recombinant wild-type thrombin were found to activate fV and fVIII within five minutes while the recombinant mutant thrombin was impeded in the activation process out to three hours. Conclusion Overall the data demonstrate that amino acid sequence 473-487 of Pro plays a preeminent role in 1) timely activation of Pro at initial cleavage Arg320 by prothrombinase, and 2) suitable macromolecular procofactor activation. Thus there is incisive rationale why no major mutations have been identified in this dynamic region which would be problematic for inherent physiological hemostasis. AbstractIn Southern Europe there’s a specific issue regarding young people – the focus of our overheated identities. Not only do youth unemployment rates in the Mediterranean remain among the highest in Europe, but these countries also perform poorly in at least two of the three dimensions of job quality, which are: the quality of earnings, or the extent to which workers’ earnings from their job contribute to their well-being; labor market security; and the quality of the working environment (OECD, Society at a glance 2019, 2019). Like Spain and Greece, Italy is always at the top of the Eurostat tables for important problems (see NEET rates: with the pandemic, the number of young people aged 15–29 who are not in education, employment or training in Italy increased from 22.1 percent in 2019 to 23.3 percent in 2020 and 24 percent in 2021 There are more than two million NEETs in Italy - boys and girls who are neither studying nor working and more than 97,000 young people have left their work or study paths in the last year. From the dispersion relations of the magnetostatic waves (MSW) in Yttrium Iron Garnet - Gadolinium Gallium Garnet (YIG-GGG) thin film[1], it is found that while the theoretical bandwidth of the magnetostatic surface waves (MSSW) decreases considerably as the carrier frequency is increased, the theoretical bandwidth of the magnetostatic forward volume waves (MSFVW) increases substantially with the carrier frequency. We have noted previously the decrease of magnetooptic (MO) bandwidth from 487 MHz to 250 MHz in noncollinear coplanar guided-wave interaction involving the MSSW[2] as the carrier frequency was increased from 3.0 GHz to 7.0 GHz. However, in view of the dispersion characteristics of the MSFVW as mentioned above, the MO bandwidth associated with noncollinear coplanar interaction involving the MSFVW can potentially be significantly larger than that involving the MSSW as the carrier frequency goes beyond 2.0 GHz. Since it is of great interest to accomplish wideband MO interaction at high carrier frequency, the MSFVW was also used in our earlier work. For example, a MO bandwidth of 550 MHz (defined at the two -3dB points) and a TM0 to TE0 mode-conversion efficiency of 0.5% were obtained in our earlier experiment with the MSFVW centered at 3.0 GHz using a homogeneous (uniform) DC magnetic, field of 2250 Oe[3]. Recently, a technique for enhancement of MO bandwidth that employs inhomogeneous DC magnetic field[4] was reported[5]. In that particular work, an increase of MO bandwidth from 30 to 350 MHz (defined between the null points or approximately 125 MHz based on the conventional definition of -3dB points) centered at the carrier frequency of 1.45 GHz was obtained at the optical wavelength of 1.15µm by switching from a homogeneous to an inhomogeneous DC magnetic field. Specifically, an inhomogeneous field that varied from 2150 to 2350 Oe across an 8.0 mm aperture of the MSFVW transducer was used. Due to the concomitant "channelization effect" with the resulting MSFVW the measured TM0 to TE0 mode-conversion efficiency was found to decrease from 1.0 to 0.03% by switching from the homogeneous to the inhomogeneous field.

Abstract:
The majority of optimization methods lose their applicability when solving Highly Dimensional Functions. The required calculation effort usually becomes enormous as dimensions increase, regardless of the elegance of the method. Most methods concern themselves with finding a single optimum which satisfies the required Accuracy, but which provide no quantitative measure (i.e. Probability of correctness) indicating whether the true optimum is found). Furthermore, there is usually no exact measure of the Calculation Effonprior to starting the procedure. There is always an unavoidable coupling (i.e. relation) between the Accuracy, Probability and Calculation Effort of an optimization method but the exact form of this relation is dependant on the procedures followed to reach optimum. Ideally, an optimization method should facilitate the statement of required accuracy, required probability and the required calculation effort separately and the method should take care of the rest (i.e. total decoupling of the three requirements). Although this ideal case is generally not possible, it is possible to move towards it by finding procedures that reduce the strength of this unwanted coupling. This report derives simple analytical relations between the required accuracy, probability and calculation effort of a general multidimensional adaptive grid Non-Gradient Guided (NGG) search method where the search points are generated either decisively or randomly. It is then shown that any adaptive method based on reducing the total solution space is heavily penalized. Further, it is analytically illustrated that if the adaptive grid is randomly generated, it is far less successful than the non random adaptive grid, because the amount of grid adaptation is less decisive at every step., due to the randomness. As with many optimization techniques, the Dimensionality Problem limits the application of this method to cases where the function evaluation is real time (∼ milliseconds) and dimensions are lower than say 25, which occurs in Conceptual/Preliminary Design systems such as CAGEDR [Shahroudi, K.E. (Ref. 4)].
