Abstract:
There are few handbooks devoted to clinical neuropsychology or behavioral neurology written in French. Most recently, there have been two: one by Habib and the other edited by Seron and Jeannerod. This revised and improved edition of the book edited by M.I. Botez completes this short list. The Botez book is impressive in terms of both its form and its content. The first edition, published in 1987, had 413 pages, 36 collaborators, and 37 chapters. The new edition has 682 pages, 55 collaborators, and 37 chapters. Fourteen of the chapters are quasi-exact reproductions of those in the first edition, while the others have been modified or added.

Abstract:
Background Osteoarthritis (OA) is the most common cause of disability in people older than 65 years. Readability of online OA information has never been assessed. A 2003 study found the quality of online OA information to be poor. Objective The aim of this study was to review the readability and quality of current online information regarding OA. Methods The term osteoarthritis was searched across the three most popular English language search engines. The first 25 pages from each search engine were analyzed. Duplicate pages, websites featuring paid advertisements, inaccessible pages (behind a pay wall, not available for geographical reasons), and nontext pages were excluded. Readability was measured using Flesch Reading Ease Score, Flesch-Kincaid Grade Level, and Gunning-Fog Index. Website quality was scored using the Journal of the American Medical Association (JAMA) benchmark criteria and the DISCERN criteria. Presence or absence of the Health On the Net Foundation Code of Conduct (HONcode) certification, age of content, content producer, and author characteristics were noted. Results A total of 37 unique websites were found suitable for analysis. Readability varied by assessment tool from 8th to 12th grade level. This compares with the recommended 7th to 8th grade level. Of the 37, 1 (2.7%) website met all 4 JAMA criteria. Mean DISCERN quality of information for OA websites was “fair,” compared with the “poor” grading of a 2003 study. HONcode-endorsed websites (43%, 16/37) were of a statistically significant higher quality. Conclusions Readability of online health information for OA was either equal to or more difficult than the recommended level.

Abstract:
Abstract Background Medical officers (trainees) in their first to third postgraduate years (PGY-1–3s) work in complex, busy environments, performing tasks that require concentration and application of learned skills. There are frequently competing demands, and being paged is among the most common. Objective We quantified and described the effect of interruptions that paging created on the clinical workflow of PGY-1–3s during ward duties. Methods This prospective study was conducted at 2 teaching hospitals in Sydney, Australia. Medical students were recruited as observers to log interruptions of PGY-1–3s' workflow arising from pages from other members of the hospital team. Results Forty-two pairs consisting of a PGY-1–3 trainee and an observer were recruited, with 24 proceeding to data collection. Nursing was the most frequent source of pages (47%); other medical staff accounted for 16% of pages, allied health for 12%, and others for 24% (with pharmacy the most common). Pages commonly involved direct patient care (46%), followed by medication issues (21%). Tasks interrupted by pages encompassed direct patient care (37%), indirect patient care (15%), and documentation (12%). Only 27% of pages were assessed as appropriate and urgent, while 58% were considered appropriate but not urgent, and 16% were not appropriate. Only 38% of pages were judged to be clinically more important than the task they interrupted. Conclusions Pages frequently interrupted direct patient care activities for PGY-1–3 trainees, and a significant proportion of pages were identified as either not requiring immediate attention or not appropriate, resulting in potentially avoidable interruptions to clinical workflow. Alternate means of alerting trainees to nonurgent tasks may reduce interruptions and facilitate patient care.

Abstract:
45 Background: To page specific services within the Division of Hematology and Oncology (Heme/Onc) at Northwestern Memorial Hospital previously required hospital staff to enter the specific service name into the web-based paging system. Turnover of faculty and staff, hospital growth, and use of locums staffing contributed to an increasing number of erroneous pages (pages intended for a different service). In this quality improvement initiative, we restructured the paging system so that all services within the Division of Heme/Onc populated when “hem” was entered into the web-based system. We aimed to decrease the number of erroneous pages received by heme/onc fellows by 20% with a subsequent improvement in perceived workload burden and job satisfaction. Methods: Pre- and post-intervention surveys were administered to all heme/onc fellows to assess impact on workload burden and job satisfaction. Fellows were asked to log all erroneous pages for a five-month period pre- and post-intervention. Results: Pre-intervention, fellows logged a total of 48 erroneous pages with 10 pages (21%) occurring between the hours of 10 PM and 6 AM. Post-intervention, a total of 37 erroneous pages were logged with only 3 pages (8%) occurring overnight. Of the 12 (92.3%) fellows that responded to the pre-intervention survey, one (8.3%) reported that the number of incorrect pages significantly increased workload burden, 9 (75%) increased workload burden, and 2 (16.7%) neither increased nor decreased workload burden. Five responders (41.7%) reported the number of incorrect pages significantly decreased job satisfaction, 5 (41.7%) decreased job satisfaction, and 2 (16.7%) neither increased nor decreased job satisfaction. 100% of fellows responded to the post-intervention survey. One (7.7%) reported that the number of incorrect pages significantly increased workload burden, 8 (61.5%) increased workload burden, and 4 (30.8%) neither increased nor decreased workload burden. Two responders (16.4%) reported the number of incorrect pages significantly decreased job satisfaction, 6 (46.2%) decreased job satisfaction, and 5 (38.4%) neither increased nor decreased job satisfaction. Conclusions: By changing the departmental nomenclature to a structured format, we decreased the number of erroneous pages received over a 5-month period by 23% with a 70% reduction in overnight pages resulting in an improvement in perceived workload burden and job satisfaction among Hematology/Oncology fellows.

Abstract:
Abstract Background: Amyotrophic Lateral Sclerosis (ALS) is a progressive neurodegenerative disorder that causes muscle weakness, disability, and death, with survival of 3-5 years and affecting all populations including Hispanics. ALS patients are mostly cared for by family caregivers (FCG) who experience burden, psychological distress and impaired quality-of-life. Thus, FCG may not leave their homes to access resources. Social media might be a way to accessing support, but little is known about quality/quantity of AL- FCG resources. Research Objectives: To identify and categorize types of bilingual resources available to ALS-FCG in Facebook. Methods: We used Facebook both groups (public and closed groups) and pages and collected numbers-of-likes, organization type and resource provided. Bilingual search terms (Spanish) included ALS/ALS-caregiver. Results: 24 pages/31 groups in English vs. 37 pages/29 groups in Spanish. English resources had more likes/members (3510/790 vs.410/95; p<0.001) and were more active (60+ posts in recent month). Community resources and provision of social support were similarly shared through groups in both languages; advocacy was more prominent among Spanish groups (69%vs.39%, p<0.001). English pages significantly provided more social support (96% vs.65%, p<0.001), more caregiver skills (67%vs.49%, p<0.001), inspirational roles (88% vs.59%, p<0.001). Spanish pages and groups were better in providing news/research to FCG (59%vs.16%, p<0.001). Implications for research, policy or practice: Facebook provides numerous useful bilingual resources to ALS-FCG. FCG may receive social support, inspiration, training, news/research and advocacy. Further interventions need to actively improve the role of social-media in supporting bilingual ALS FCG and use it as a platform of dissemination.

Abstract:
Introduction: Barriers to implementing effective Knowledge Translation (KT) in Emergency Departments include lack of awareness, lack of time and limited access to resources. In our teaching hospital emergency department (ED), we implemented a new department website (www.sjrhem.ca) to provide improved access to our KT resources. Having published the website, we wanted to know if the addition of conveniently situated pre-configured iPads would increase access to the website from within the department. Methods: The website was developed and first published in April 2014. Two iPads (Apple Inc.) were preconfigured with icons linking directly to targeted pages on our website, including the physician schedule, academic calendar. The iPads were securely located at physician charting desks in October 2014. We used Google analytics to record of webpage visits for 25 weeks before and after the installation of the iPads. Comparisons of mean weekly visits were made using the Student T test (GraphPad Prism). Results: The mean weekly page views for the website increased after the installation of the iPads from a baseline of 103 (95%CI 83.9-121) to 198 (181-215); an increase of 95% (71-120; p<0001). Limiting analysis to devices utilising our hospital IP address we saw a 403% increase in mean weekly page views from 6.4 (4.35-8.45) to 32.2 (26.7-37.8; p<0001). There was a clear step increase in website access from the date of iPad installation. Comparing the increases in average weekly views for those pages with direct link iPad icons (Clinical 11.4 before, 16.6 after, 46%, Schedule 30, 39, 30%, Calendar 10.7, 46.3, 330%, Home 84.1, 115.4, 37%) to the top accessed pages without iPad icons (Research 4.3, 6.2, 44%, Ultrasound 4.9, 9.8, 100%) did not, other than for the calendar page, demonstrate an observable difference. However, when analysed by views originating the hospital IP address, the pages with iPad icons were responsible for 88% of landing pages. Over the last two years the department iPads were responsible for 17% of our page views, with 6 of the department guideline pages featuring in the top 20 pages viewed. Conclusion: Provision of preconfigured iPad devices within the clinical environment of a busy ED significantly increases access from within that environment to a department website.

Abstract:
Firms must strategically manage their responses to user comments to keep users engaged on their social media business pages. The question of whether, how, and when a firm should respond to user comments to achieve favorable outcomes is of great interest to researchers and practitioners. We focus on these questions and study the effects of initial user comments and firm responses on subsequent user engagement on social media business pages. In particular, we theorize and examine how two features of initial user comments (i.e., sentiment and controversialness) and two features of firm responses (i.e., uniqueness and timeliness) jointly affect the volume and sentiment of subsequent user comments. By analyzing data from the Facebook business pages of multiple U.S. retailers (10,312 firm posts from 37 firms and over 1 million user comments), we found that firms are more likely to respond to negative comments (than positive or neutral comments) but less likely to respond to controversial comments (which evoke diverse opinions and emotions). Further, we found that engaging with negative and controversial comments and promptly responding to comments are linked to an increase in the volume of subsequent user comments but also to a more negative sentiment in these comments. We also found that providing unique responses improves the volume and sentiment of subsequent user comments. Our findings offer theoretical and practical insights into firms’ response management on social media.

Full text
Abstract:
Text summarization remains a challenging task in the natural language processing field despite the plethora of applications in enterprises and daily life. One of the common use cases is the summarization of web pages which has the potential to provide an overview of web pages to devices with limited features. In fact, despite the increasing penetration rate of mobile devices in rural areas, the bulk of those devices offer limited features in addition to the fact that these areas are covered with limited connectivity such as the GSM network. Summarizing web pages into SMS becomes, therefore, an important task to provide information to limited devices. This work introduces WATS-SMS, a T5-based French Wikipedia Abstractive Text Summarizer for SMS. It is built through a transfer learning approach. The T5 English pre-trained model is used to generate a French text summarization model by retraining the model on 25,000 Wikipedia pages then compared with different approaches in the literature. The objective is twofold: (1) to check the assumption made in the literature that abstractive models provide better results compared to extractive ones; and (2) to evaluate the performance of our model compared to other existing abstractive models. A score based on ROUGE metrics gave us a value of 52% for articles with length up to 500 characters against 34.2% for transformer-ED and 12.7% for seq-2seq-attention; and a value of 77% for articles with larger size against 37% for transformers-DMCA. Moreover, an architecture including a software SMS-gateway has been developed to allow owners of mobile devices with limited features to send requests and to receive summaries through the GSM network.

Abstract:
This essay uses Beckett's 1936 reference to a specifically German 'associative complex' to examine its aesthetic and creative manifestations. Drawing on notebook material and correspondences from the 1930s, the essay reveals the importance of Beckett's extensive reading of German literature precipitating his move from a recondite mode of writing to one that would allow his writing to issue more directly from his own personal pressures. Beckett's project of self-writing is traced to its culmination in the German Diaries of 1936-37 and a new project entitled the "Journal of a Melancholic" formulated in its pages.

Full text
Abstract:
Background: Many computerized tomography (CT) scans ordered after-hours from the emergency department (ED) at our institution required a discussion between the emergency physician (EP) and radiology resident (RR), leading to workflow inefficiency. Aim Statement: The aim was to improve workflow efficiency and provider satisfaction, and reduce CT turnaround time, without significantly affecting CT utilization within six months. Measures & Design: We created a new workflow by creating an electronic list of ED CT requests that RRs monitor. RRs protocolled all requests and only called the ED physician for more details when required. The intervention was implemented in a stepwise fashion via plan-do-study-act cycles. An electronic survey measured qualitative outcomes, and quantitative outcomes were analyzed via statistical process control (SPC) charts and other statistical methods. Evaluation/Results: Survey response was high (76% EP, 79% RR). Most EPs and RRs felt more efficient (96.3%, 73.3%), RRs felt fewer disruptions (83.3%), and most EPs felt that scans were done faster (84.1%). We analyzed CT turnaround times and utilization using SPC charts and segmented regression analyses. Turnaround time trended to improvement (33 mins vs 29 mins on weekdays [WD], 37 mins vs 33 on weekends [WE]), but was not statistically significant. There was background rising CT utilization over time (+0.7 and + 1.9 CT/100 ED visits/year on WD and WE, respectively, p < 0.0005), but the intervention itself did not cause a significant change. The total number of pages to RR (a measure of workflow disruption) decreased significantly on the WDs (23 vs 19 pages, p = 0.0011), but not on WE (79 vs 75 pages, p = 0.1663). However when adjusting for number of scans ordered, there was a decrease in paging rates (0.73 vs 0.54 pages per scan ordered on WD [p < 0.00005], 3.24 vs 2.63 pages per scan ordered on WE [p = 0.0012]). Discussion/Impact: Our intervention led to improved work satisfaction and perceived efficiency experienced by both EPs and RRs. It did not statistically significantly affect imaging turnaround times or utilization rates. Our project shows that calling for preapproval of imaging studies does not seem to provide any benefit in our setting.

Abstract:
BackgroundChanges in the character of medical authorship.AimsTo compare the impact of industry-linked and non-industry linked articles.MethodWe compared articles on sertraline being coordinated by a medical writing agency with articles not coordinated in this way. We calculated numbers of Medline-listed articles per author, journal impactfactors, literature profiles and citation rates of both sets of articles.ResultsNon-agency-linked articles on sertraline had an average of 2.95 authors per article, a mean length of 3.4 pages, a mean Medline listing of 37 articles per author (95% CI 27–47) and a mean literature profile of 283 per article (95% CI 130–435). Agency-linked articles on sertraline had an average of 6.6 authors per article, a mean length of 10.7 pages, a mean Medline listing of 70 articles per author (95% CI 62–79) and a mean literature profile of 1839 per article (95% CI 1076-2602). The citation rate for agency articles was 20.2 (95% CI 13.4–27.0) and for non-agency articles it was 3.7 (95% CI 3.3–8.1).ConclusionsThe literature profiles and citation rates of industry-linked and non-industry-linked articles differ. The emerging style of authorship in industry-linked articles can deliver good-quality articles, but it raises concerns for the scientific base of therapeutics.

Abstract:
Recommendation systems are growing very rapidly. While surfing, users frequently miss the goal of their search and lost in information overload problem. To overcome this information overload problem, the authors have proposed a novel web page recommendation system to save surfing time of user. The users are analyzed when they surf through a particular web site. Authors have used relationship matrix and frequency matrix for effectively finding the connectivity among the web pages of similar users. These webpages are divided into various clusters using enhanced graph based partitioning concept. Authors classify active users more accurately to found clusters. Threshold values are used in both clustering and classification stages for more appropriate results. Experimental results show that authors get around 61% accuracy, 37% coverage and 46% F1 measure. It helps in improved surfing experience of users.

The modern emerging infection Ebola Virus Disease (EVD) is of global threat originates from Africa region. This is zoonotic and identified as human diseases or previously called Ebola hemorrhagic fever which is a highly fatal human illness where case fatality rate is found up to 90%. The virus transmission begins from wild animals to human and then spreads within population through human to human. Fruit bats are found as natural host of Ebola virus. There is no specific treatment or vaccine available in the market so far, intensive supportive care is needed for severely ill patients. This paper highlights background information, problem statement, viral characteristics, mode of transmission, signs and symptoms, prevention & vaccination. Abstract:
Use of contrast media for cardiovascular intervention is associated with risk of contrast induced nephropathy. Contrast induced nephropathy is associated with increased morbidity, prolonged hospitalization, potential need for dialysis and increased mortality rate. Although the consequences of contrast induced nephropathy are well known, the prospective identification of patient at risk for nephropathy has been inconsistent. The mechanism of contrast induced nephropathy is complex and not fully understood. Direct tubular toxicity and disturbances of renal hemodynamic with altered glomerular filtration and renal medullary ischemia are the most important path-physiological mechanism. Prevention of contrast induced nephropathy is address in numerous studies. The most attractive agents include hydration, N-acetylcysteine, and infusion of sodium bicarbonate. This review focuses on the definition, pathophysiology and prevention of contrast induced nephropathy. Abstract:
Abstract Purpose The aim of this study was to determine whether the coronavirus disease 2019 (COVID-19) pandemic influenced the presence of ophthalmology residency programs on social media platforms. Methods The presence of all accredited ophthalmology residency programs on Twitter and Instagram was assessed for January 2020 and August through March 2021 through an online search. The number of followers and posts per month during these periods was collected for each program. The presence of programs on social media was compared between the two time periods. Results The percentage of programs on Instagram rose from 18% in January of 2020 to 57% in March of 2021 (p < 0.001). On Twitter, the percentage of programs rose from 24 to 37% (p < 0.001). From January 2020 to March 2021, the mean number of Instagram followers increased from 265 to 649 (p < 0.0001), while the mean number of Twitter followers increased from 421 to 532 (p < 0.0001). The top ten ophthalmology residency programs in 2021 according to Doximity all had Instagram pages, while 57 (53%) of remaining programs had pages (p = 0.005). Meanwhile, 7 top ten programs (70%) were on Twitter compared with 36 (34%) of the remaining programs (p = 0.036). Conclusions The presence of ophthalmology residency programs on Instagram and Twitter increased significantly after the onset of the COVID-19 pandemic. Social media represents an evolving platform through which programs can connect with prospective residents.

Abstract:
A 500-pages collection of documents in English showing the sovereignty of Vietnam on Hoang Sa (Paracel islands) & Truong Sa (Spratly islands) was introduced at Harvard University on 16/6/2012 after having been sent to the U.S. National Geographic Society and two Senators John McCain and Jim Webb office and the The Center For Strategic & Internatinonal Studies in 2011. This document is being proofread and completed in English in order to bring to overseas libraries especially in the United States at the addresses where the documents used to be distributed by the U.S. Army in the Pacific in 1960s over the sovereignty of Vietnam on disputing territory. It has 11 pages of summary and the full text is nearly 500 pages, including Part I that consists of textual analysis in the sovereignty dispute over the Hoang Sa and Truong Sa by U.S. Army in the Pacific in 1960. The 37 quotations from geography books , Journey of the Western countries from the 19th century and before had clearly stated from 1816 Hoang Sa has belonged to Vietnam. Part II consists of three presentations at the conferences in Hanoi and Philadelphia, USA in 2010. Part III includes the full text of a history dissertation entitled “Establishment of Vietnam’s sovereignty on Hoang Sa and Truong Sa” and the annexes of the thesis updated by the author. There are no other countries like Vietnam where the official history, codified policy books, geography books of Vietnam especial texts of the state, the local commanding sheets, clearly state the establishment of Vietnam’s sovereignty on Hoang Sa and Truong Sa. The most valuable document is the codified policy book of Nguyen dynasty (the nineteenth century reports from high-ranked officers of the Government, and other agencies or the king announcement about the establishment of Vietnam's sovereignty over Hoang Sa archipelago under the Nguyen Dynasty as the current expedition, measuring, sketching Hoang Sa, plug milestone...
