The main-chain structure and chlorine distribution of chlorosulfonated polyethylene (CSM), which was prepared via gas-solid phase method, were studied in this article. All the structure types of CSM 3550 and CSM 3570 were completely characterized by Nuclear Magnetic Resonance (NMR). According to the1H NMR and13C NMR spectra, the chlorine content (Cl wt %) and sulfur content (S wt %) of CSM 3550 and CSM 3570 could be completely calculated and this value was much close to the theoretical values, which were provided by the manufacturer. The comparison shows that the chlorine distribution of CSMs via gas-solid phase method is more uniform than that of CSMs via solution method.

"Barriers to and Facilitators of the Use of Digital Tools in Primary Care to Deliver Physical Activity Advice: Semistructured Interviews and Thematic Analysis." JMIR Human Factors 9, no. 3 (August 30, 2022): e35070. http://dx.doi.org/10.2196/35070.

Full text
Abstract:
Background Physical inactivity is a leading risk factor for many health conditions, including cardiovascular disease, diabetes, and cancer; therefore, increasing physical activity (PA) is a public health priority. Health care professionals (HCPs) in primary care are pivotal in addressing physical inactivity; however, few HCPs provide PA advice to patients. There can be obstacles to delivering PA advice, including lack of time, confidence, or knowledge. Digital technology has the potential to overcome obstacles and facilitate delivering PA advice. However, it is unknown if and how digital tools are used to deliver PA advice in primary care consultations and what factors influence their use. Objective We aimed to understand the use of digital tools to support primary care consultations and to identify the barriers to and facilitators of using these systems. Methods Overall, 25 semistructured interviews were conducted with primary care HCPs. Professionals were sampled based on profession (general practitioners, practice nurses, and health care assistants), prevalence of long-term conditions within their practice area, and rural-urban classification. The data were analyzed thematically to identify the influences on the use of digital tools. Themes were categorized using the COM-B (capability, opportunity, and motivation—behavior) model and the Theoretical Domains Framework to identify the barriers to and facilitators of using digital tools to support the delivery of PA advice in primary care consultations. Results The identified themes fell within 8 domains of the Theoretical Domains Framework. The most prominent influence (barrier or facilitator) within psychological capability was having the skills to use digital tools. Training in the use of digital tools was also mentioned several times. The most notable influences within physical opportunity were limited digital tools to prompt/support the provision of PA advice, time constraints, efficiency of digital tools, simplicity and ease of use of digital tools, and integration with existing systems. Other physical opportunity influences included lack of access to digital tools and technical support in the use of digital tools. Within social opportunity, a notable barrier was that digital tools reduce interpersonal communication with patients. Patient preference was also identified. Several important influences were within reflective motivation, including confidence to use digital tools, beliefs about the usefulness of digital tools, the belief that digital tools “are the way forward,” beliefs related to data privacy and security concerns, and perceptions about patient capabilities. About automatic motivation, influences included familiarity and availability regarding digital tools and the fact that digital tools prompt behavior. Conclusions A variety of influences were identified on the use of digital tools to support primary care consultations. These findings provide a foundation for designing a digital tool addressing barriers and leverages the facilitators to support PA advice provision within primary care to elicit patient behavior change and increase PA.

Abstract:
The polycystic ovarian syndrome is an endocrine disorder with a high prevalence affecting reproductive-aged women and adolescent girls. The most common symptoms are infertility, numerous ovarian cysts, hyperandrogenism, menstrual cycle abnormalities. The disease has a great impact on patients, impairing their quality of life. Bearing that in mind, innovative therapeutic approaches are needed. We aim to describe current management and treatment concepts adopted in PCOS therapy. The analysis of previously published studies was conducted by using the PubMed, Google Scholar, and Scopus databases.Mentioned studies show that lifestyle changes may benefit PCOS therapy in multiple ways. Dietary and exercise interventions have a notable influence on women’s body composition as well as biochemical parameters. The reduction of body weight or BMI is a therapeutic achievement likewise patients’ contribution to managing PCOS. Pharmacotherapy provides plentiful treatment choices to adopt. Studies show that hormonal contraceptive has a significant impact on both hyperandrogenism and menstrual abnormalities. Metformin is proven to reduce BMI, improve LDL levels, and lower the risk of OHSS in women undergoing IVF/ICSI-ET. A meta-analysis proved metformin’s monotherapy superiority over clomifene citrate or their combination in treating infertility. However, both substances are not recommended as the first-line choice. Regrettably, many studies mention short of evidence, lack of larger sample sizes, or insufficient duration preventing further research. The conclusion we draw is that approaches of managing and treating PCOS are constantly advancing and updating. However, there is a necessity for future studies to expand their research by gathering more data and considering diverse cases.

Full text
Abstract:
OBJECTIVES/SPECIFIC AIMS: Since 1971, Naloxone has been the only FDA approved opioid antagonist indicated for use after opioid overdose. New formulations of Naloxone have been introduced into the market, including an injectable, auto-injector, and nasal spray. However, Naloxone is short-acting and as such often requires multiple doses and may induce severe withdrawal symptoms. This study examines the regulatory framework to understand the evolution of products indicated to treat opioid overdose and the landscape of therapies in development. Furthermore, this study examines how the Food and Drug Administration (FDA) and other government agencies have approached the opioid crisis. METHODS/STUDY POPULATION: A PubMed search of “naloxone AND opioid overdose” with the filter “humans” was conducted to understand Naloxone’s regulatory framework. The term “naloxone” was searched on the Drugs@FDA: Approved Drug Products database. Additionally, “nalmefene” was searched on ClinicalTrials.gov. To examine the opioid antagonist market landscape, a PubMed search of “opioid antagonist AND opioid overdose” with the filters “humans” and “clinical trial,” and a ClinicalTrials.gov search of “opioid antagonist and opioid overdose,” were conducted. Government agency reports were reviewed and cataloged. RESULTS/ANTICIPATED RESULTS: Preliminary findings suggest a lack of innovation in the development of novel opioid antagonists. Most literature review findings focused on already-marketed Naloxone products, including the original injectable approved in 1971, the 2014 Evzio Auto-Injector, and the 2015 Narcan Nasal Spray (Figure 1). For example, there were 14 results yielded from the FDA approvals database, but none of these results represented a new opioid antagonist molecule. A longer-acting opioid antagonist, Nalmefene injectable, was approved in 1995 but has since been removed from the market due to low sales. Our initial ClinicalTrials.gov search using condition “opioid overdose” and other terms “opioid antagonist”,revealed no new studies being conducted on alternative opioid antagonist treatments for opioid overdose. Findings only focused on the distribution, co-dispensing, intervention, pharmacokinetics/pharmacodynamics (PK/PD) of Naloxone (Figure 2). However, a Google search yielded one new trial with an opioid antagonist by Opiant Pharmaceuticals, almost fifty years after FDA’s approval of Naloxone. A ClinicalTrials.gov search was then performed using the search term “nalmefene” to find whether Opiant Pharmaceuticals’ trial was in the ClinicalTrials.gov database. However, the Opiant trial is phase I, and as such does not require reporting on ClinicalTrials.gov. In 2017, the National Institutes of Health (NIH) launched an initiative for longer-acting opioid antagonist formulations. In 2018, Opiant Pharmaceuticals announced positive phase I results for intranasal Nalmefene. The potential return of Nalmefene in intranasal form may play a significant role in reducing overdoses, especially in cases where a longer-acting opioid antagonist is necessary. Opiant Pharmaceuticals’ trial commenced after the NIH announced their initiative; furthermore, the NIH’s National Institute on Drug Abuse granted the company $7.4 million to further the investigation of this drug. We will continue to research drugs that have previously been studied for the indication of treating opioid overdose in the United States and abroad and catalog them. DISCUSSION/SIGNIFICANCE OF IMPACT: The abuse and misuse of opioids in the United States has caused an epidemic accounting for over 115 opioid-overdose deaths each day, devastating our nation, both socially and economically. The United States spends $78.5 billion annually to combat the misuse of these drugs. Due to the severity of the opioid crisis, efforts to better understand approved therapies and investigational products in development to treat opioid overdose will be of significance moving forward. This research can inform agencies who are developing strategies to reduce opioid overdoses and pharmaceutical product developers about the current opioid antagonist landscape.

Abstract:
Progressivity of type 2 Diabetes Mellitus (DM) is associated with a condition of chronic inflammation. The Neutrophil-Lymphocyte Ratio (NLR) has become a potential new marker of inflammation to detect chronic inflammation. This research aimed to determine NLR differences between controlled type 2 DM and uncontrolled type 2 DM groups. This research conducted an observational with a cross-sectional approach to 56 patients with type 2 diabetes. The identity, anthropometric measurements, and laboratory data of routine blood exam and HbA1c were carried out on each research subject, and then the NLR calculations were performed. The subjects were 20 (35.70%) controlled type 2 DM patients, 36 (64.30%) uncontrolled type 2 DM patients, consisting of 36 (64.30%) male and 20 (35.70%) females. The NLR value was statistically significantly higher in uncontrolled type 2 DM patients than controlled type 2 DM patients, which was 1.90±0.84 compared to 1.52±0.50 (p=0.035). There was a significant difference in the NLR value between the uncontrolled type 2 DM group and the controlled type 2 DM group.

Abstract:
This work reflects the extraction process of oil and pectin from grape peels using a response surface method in which a central composite rotatable design of 25 and 35 was used for the two extractions. Output temperatures (80–100°C) and heating times (5–9 hours) were used for oil extraction, while (80 -100oC) and heating times (20 – 60 minutes) and a pH of extract (1.0 – 3.0) were selected for pectin removal. Oil yield ranged from 7.90 - 15.30%, while pectin yield ranged from 19.90 – 35.70%. A maximum oil yield of 15.30% was obtained at a temperature of 90oC at a heating time of 9.0 hours, while a maximum yield of pectin of 35.70% was obtained at a pH of 2.5, 95oC temperature and 50 minutes heating time. The optimum value for oil production was 15.63% at an average temperature of 99.64oC and heating time of 8.99 hours, while the average value of pectin yield was 38.01% at an output temperature of 94.00oC, the period release time of 58.00 minutes with a pH of 2.00. The deviation between the experimental and predicted values was low and not significant. All processing conditions have important impacts on the extraction of oil and pectin from grape peels.

Abstract:
Web portals have been used as information products to deliver personalized, feature-rich, and flexible information needs to Internet users. However, all portals are not equal. Most of them have relatively a small number of visitors, while a few capture the majority of surfers. This study seeks to uncover the factors that contribute the perceived quality of a general portal. Based on 21 factors derived from an extensive literature review on Information Product Quality (IPQ), web usage, and media use, an experimental study was conducted to identify the factors that are perceived by web portal users as most relevant. The literature categorizes quality factors of an information product in three dimensions: information, physical, and service. This experiment suggests a different clustering of factors: Content relevancy, Communication interactiveness, Information currency, and Instant gratification. The findings in this study will help developers find a more customer-oriented approach to developing high-traffic portals.

Abstract:
Lactic acid bacteria (LAB) in the natural environment meet multiple stressors such as pH and temperature variations, increased nutrition and metabolite concentrations, harmful chemicals, acidic/oxidative conditions, osmotic pressure, and starvation. However, LAB strains are not subjected to high hydrostatic pressure (HHP) which currently is the most common non-thermal decontamination technology in the food industry. In this context, the LAB response to HHP is more difficult to identify compared to other stress-induced responses, and dnaK, ctsR, and hrcA can serve as essential regulators in this reaction. In the present study, the expression level of dnaK, ctsR, and hrcA mRNAs in 15 LAB strains after the HHP (300 MPa/5′) exposure was evaluated. As a result, the HHP-treatment affected the up-regulation of dnaK, ctsR, and hrcA in L. backii KKP 3565, L. backii KKP 3566, L. rhamnosus KKP 3570, L. brevis KKP 3575 strains, whereas, in L. plantarum KKP 3569, L. rhamnosus KKP 3571, L. brevis KKP 3573 all genes were lower expressed. The relative expression level of the dnaK, ctsR, and hrcA either before or after the pressure treatment for L. brevis DSM 6235, L. rhamnosus KKP 3572, L. brevis KKP 3574, L. brevis KKP 3576, L. rossiae KKP 3577, L. curvatus KKP 3578 strains were undetectable. Significant differences in the expression levels were observed, between the control and the HHP treatment strains for dnaK in L. backii KKP 3565, L. backii KKP 3566, L. plantarum KKP 3569, L. rhamnosus KKP 3570, L. rhamnosus KKP 3571, ctsR in, L. backii KKP 3565, L. rhamnosus KKP 3570, L. rhamnosus KKP 3571, and hrcA in L. plantarum KKP 3569, L. rhamnosus KKP 3571. Overall, the studied genes, dnaK, ctsR, and hrcA can be useful markers to indicate the LAB cellular response to HHP. These molecular parameters can help to optimize the desirable LAB growing conditions in industrial processes and to understand the complexity of the stress-related mechanism.

Abstract:

Abstract:
This study reports a combination of two indices, air pollution tolerance index (APTI) and anticipated performance index (API) as viable tools for selecting suitable plants for pollution abatement program. Leaf samples of 6 plant species; Mangifera indica, Araucaria heterophylla, Elaeis guineensis, Syzygium malaccense, Acacia auriculiformis, and Chrysophyllum albidium were collected from an industrial and academic areas at Ado-Odo, Ota, Nigeria; during the dry season of January to March 2018. Biochemical parameters; leaf-pH, relative leaf water content, total chlorophyll content, and ascorbic acid content were analyzed to compute the APTI values. Combined APTI, botanical and socioeconomic indices were graded to evaluate the API of the different plant species. The APTI for the species ranged between 4.79 and 10.7, ideal for sensitive species category (APTI < 11), and the plants are classified as bio indicators of air pollution. The API indicates Mangifera indica and Syzygium malaccense (API = 4) as good performers while Chrysophyllum albidum is a moderate performer (API = 3). The three tree species were identified as suitable green belt plants and thus valuable additions to the green belt development plant list in tropical Africa.

Abstract:
Since the rise of the new “ethics culture” in the USA and Canada, there has been a noticeable decline in field research on new, controversial religions and social movements. This study examines some of the new administrative obstacles to research, as experienced by twelve researchers in the course of negotiations with their ethics boards (“REBs” in Canada, “IRBs” in the U.S.) for ethics approval regarding projects involving “human subjects”. The twelve informants’ critiques of their ethics committees, conveyed in interviews, fall into eight categories: (1) unnecessary delays; (2) poor communication skills; (3) excessive concern for potential risk; (4) impeding spontaneity and flexibility in field research; (5) secrecy, immunity and lack of accountability; (6) the hierarchical relationship; (7) REBs exceeding their mandate; (8) disregard for the well-being of human subjects. On the basis of these interviews (and previous studies), the strategic responses of North American researchers to obstacles posed by ethics committees might be analyzed as corresponding to four types: capitulation, adjustment, resistance and reform. While capitulation appears to be a common response among graduate students, resistance appears to be widely practised among experienced researchers, who cooperate deceptively through “benign fabrication” or “gamesmanship”. This study explores the implications of the rise of this rapidly evolving “moral bureaucracy”, criticized by scholars for inhibiting field research through the delaying or halting of research projects, distorting methodologies, and discouraging initiative and originality. Finally, it is argued that the ethical concern for potential harm to human subjects must be balanced with the right of minority groups to be heard; to tell “their side of the story”.
