Abstract:
IntroductionNarcissistic personality disorder influences relationships and beliefs in different life spheres.ObjectiveWe aimed to measure narcissism among Bulgarian adults by establishing validity of two scales measuring narcissism, specifying some social and demographic factors and differentiating narcissistic features.MethodOne hundred and thirty-four Bulgarians (17–62 years old, M = 31; SD = 11) were examined with two scales measuring narcissism. Higher score on Narcissistic personality disorder scale from the Personality Diagnostic Questionnaire-4 correlated positively (r = .346; P < .001) with higher score on A. T. Beck and J. S. Beck's scale measuring Narcissistic beliefs from Personality Belief Questionnaire that indicated their validity.ResultsNo significant gender differences in narcissism were found, with female participants prevailing in the sample (n = 118). Family status (FPDQ−4(4,128) = 0.593; pPDQ-4 = 569; FPBQ(4,128) = 0.295; pPBQ = .881), occupational status – student, employed or unemployed (FPDQ−4(4,128) = 0.593; pPDQ−4 = .569; FPBQ(4,128) = 0.295; pPBQ = .881), secondary or university education (tPDQ−4(112) = 1.809; pPDQ−4 = .073; tPBQ(112) = 1.338; pPBQ = .184), and the length of work experience did not differentiate narcissism (RPDQ−4 = .168; tPDQ−4 = −1.853; pPDQ−4 = .066; RPBQ = .063; tPBQ = −0.681; pPBQ = .497). Advance in age was related to diminished levels of narcissism (RPDQ−4 = .21; R2PDQ−4 = .044; BPDQ−4 = −0.034; tPDQ−4 = −2.463; pPDQ−4 = .015). Higher narcissism was related to the belief of being a special person (70.8%) and to the perceptions of other people as being envy of one's personality–own achievements (60.9%).ConclusionsNarcissistic personality disorder is rare in the studied Bulgarian adult population – 4.5% of the subjects (n = 6) had the highest levels of narcissism on both scales that corresponded to the proportion of 6% prevalence of lifetime narcissism in USA, according to Stinson et al.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Abstract:
Patients with acute coronary syndrome (ACS) and left ventricular (LV) dysfunction undergoing percutaneous coronary intervention (PCI) need adequate antithrombotic protection. We aim to compare the clinical outcomes between ticagrelor and clopidogrel in these patients. In total, 336 patients with ACS and LV dysfunction who undergoing PCI were included in this retrospective observational study. Of these, 137 received clopidogrel and 199 received ticagrelor. There was a 6-month follow-up period during which clinical outcomes were monitored. The incidence of the composite endpoint (23.1% vs 13.9%, P = .041) and bleeding events (6.5% vs 1.5%, P = .027) in the ticagrelor group were significantly higher compared to the clopidogrel group. Multivariate logistic regression analysis revealed that age (P = .006), hypertension (P = .007), liver insufficiency (P = .022), previous MI (P = .014) and ticagrelor (P = .044) were independent risk factors that affect the efficacy outcome. Age (P = .027) and ticagrelor (P = .016) were the independent risk factors for the safety outcome. Furthermore, in Cox survival regression analysis model, the survival rate of the efficacy endpoint in the clopidogrel group was seemingly higher than in the ticagrelor group (HR = 1.68, 95% CI: 0.97–2.90, P = .065). The survival rate of the bleeding endpoint in the clopidogrel group was higher than in the ticagrelor group (HR = 2.00, 95% CI: 1.17–3.40, P = .011). Compared to clopidogrel, ticagrelor showed increased risk of efficacy outcome and major bleeding events during 6-month follow-up in patients with ACS and LV dysfunction undergoing PCI.

Background/Objective Motor coordination impairment is common in children with neurodevelopmental disorders such as autism spectrum disorder (ASD) and attention deficit/hyperactivity disorder (AD/HD). The purpose of this study was to investigate the relationship between motor coordination, cognitive ability, and academic achievement in Japanese children with neurodevelopmental disorders. Methods Thirty-four school-age (6–12 years old) children with neurodevelopmental disorders and 34 age-matched typically developing (TD) children were recruited in this study. Correlations between the scores of the Movement Assessment Battery for Children-2 (M-ABC2) and the Kaufman Assessment Battery for Children — Second Edition (K-ABCII) that assesses cognitive abilities, and academic achievement were analyzed. Results The children with neurodevelopmental disorders obtained a lower total score and all component scores on M-ABC2 compared to the TD children. In children with neurodevelopmental disorders, M-ABC2 Manual Dexterity score was significantly correlated with K-ABCII Simultaneous Processing (r = .345, p = .046), Knowledge (r = .422, p = .013), Reading (r = .342, p = .048), Writing (r = .414, p = .017), and Arithmetic (r = .443, p = .009) scores. In addition, M-ABC2 Balance score was significantly correlated with K-ABCII Learning (r = .341, p = .048), Writing (r = .493, p = .004), and Arithmetic (r = .386, p = .024) scores. Conclusion These findings stress that it is essential to accurately identify motor coordination impairments and the interventions that would consider motor coordination problems related to cognitive abilities and academic achievement in Japanese children with neurodevelopmental disorders.

Abstract:
Background:Behçet’s Disease (BD) is a systemic vasculitis of unknown etiology[1]. Monocytes closely related to inflammation potentially contribute to BD’s pathogenesis. They are classified into three subsets: classical monocytes (CM), intermediate monocytes (IM) and non-classical monocytes (NCM). Abnormalities of monocyte subsets have been reported in many infectious, inflammatory and autoimmune diseases[2-6], but their implication in BD remains elusive.Objectives:To investigate the distribution, phenotypes and functions of monocyte subsets in BD and explore their roles in BD pathogenesis.Methods:The frequencies and phenotypes of monocyte subsets in BD and healthy controls (HC) were determined by flow cytometry, and their correlation with clinical parameters was analyzed. Intracellular cytokines and phosphorylated signal proteins [phosphorylated p65(p-p65) and phosphorylated p38(p-p38)] were determined in LPS-activated monocyte subsets by flow cytometry. Monocyte subsets of BD and HC were sorted and co-cultured with naïve CD4 + T cells, and Th1 cell frequencies were measured on day 5.Results:A higher IM (9.0±3.6 % vs. 4.5±2.0%, p<0.01) and lower NCM (2.6±1.2% vs. 4.2±2.0%, p<0.01) population in BD patients were noted. BD IM were positively correlated with CRP (r=0.5456, p<0.05), ESR (r=0.4683, p=0.05), and the serum level of TNF-a (r=0.7372, p<0.001) and IL-6(r=0.5013, p<0.05). BD NCM were negatively correlated with CRP (r=0.4822, p<0.05) and the serum level of IgM (r=-0.7830, p<0.001). Moreover, BD IM decreased (12.3±3.8% vs. 5.7±3.6%, p<0.05), while BD NCM increased (2.6±1.3% vs. 3.5±1.5%, p<0.01) after BD patients achieved remission. CD11b and CD64 expression on CM, IM and NCM in BD were enhanced. BD CM promoted TNF-a (61.0±11.4% vs 48.3±9.9%, p<0.05) and IL-6 (7.2±5.4% vs1.9±1.7%, p<0.05) production and facilitated Th1 differentiation. BD IM promoted IL-6 production (6.2±3.8% vs 2.6±1.6%, p<0.05). Furthermore, we demonstrated a higher level of p-p65 (12.8±2.9% vs 3.3±1.1%, p<0.01) in BD CM and increased p-p65 (3.2±0.6% vs 0.01±0.01%, p<0.01) and p-p38 (1.1±0.6% vs 0.03±0.01%, p<0.01) in BD IM.Conclusion:To our knowledge, our study is the first study on monocyte subsets in BD. BACKGROUND: Many ophthalmologists instruct patients with glaucoma to avoid coffee, although data supporting this practice are insufficient. OBJECTIVE: To estimate the effect of drinking coffee on intraocular pressure (IOP). METHODS: In this crossover study, the effect of the consumption of regular (180 mg caffeine in 200 mL beverage) and decaffeinated coffee (3.6 mg caffeine in 200 mL beverage) was compared in patients with normotensive glaucoma (n = 6) or ocular hypertension (n = 22). IOP was monitored in both groups at 30, 60, and 90 minutes after coffee ingestion. RESULTS: In patients with normotensive glaucoma who drank regular coffee, the mean ± SD changes in IOP at 30, 60, and 90 minutes were 0.9 ± 0.5, 3.6 ± 1.1, and 2.3 ± 0.66 mm Hg, respectively; in those who drank decaffeinated coffee, they were 0.75 ± 0.36, 0.70 ± 0.4, and 0.4 ± 0.6 mm Hg, respectively. The corresponding values in patients with ocular hypertension were as follows: after regular coffee, 1.1 ± 0.7, 3.4 ± 1.0, and 3.0 ± 2.7 mm Hg; and after decaffeinated coffee, 0.6 ± 0.4, 0.9 ± 0.2, and 0.5 ± 0.5 mm Hg. The difference in the change in IOP from baseline after ingestion of regular versus decaffeinated coffee was statistically significant in each group at 60 and 90 minutes. Subjects who drank regular coffee demonstrated a greater elevation in IOP; this elevation may be clinically significant. CONCLUSIONS: Intake of caffeinated beverage (≥180 mg caffeine) may not be recommended for patients with normotensive glaucoma or ocular hypertension.

Целью являлась разработка технологии и рецептуры каш быстрого приготовления на основе процесса экструзии дробленого риса и высушенной барды топинамбура, полученной в рамках комплексной переработки корнеплода на спирт и пищевые ингредиенты и характеризующейся высоким содержанием в ней пищевых волокон – 70,6 % и белка – 15,8 %. С использованием метода обратной экструзии проведена инструментальная оценка текстуры каш, представленных на российском рынке. Показатели консистенции, адгезии и динамической вязкости сред варьируются в широких пределах. Разработаны рецептуры каш, включающие помимо экструдатов овсяные хлопья и вкусоароматические ингредиенты: сахар, соль, кардамон. Повышение доли высушенной барды топинамбура при экструдировании от 0 до 6 % позволяет увеличить содержание пищевых волокон в кашах до 6 г на 100 г смеси. Это позволяет характеризовать смеси как продукт с высоким содержанием пищевых волокон. В аспекте реологических характеристик отмечен рост динамической вязкости с 2,7 до 3,6 Па•с и силы адгезии с 0,7 до 0,9 Н. Более значимо влияющим на реологию готовых к употреблению каш фактором является содержание в их рецептуре овсяных хлопьев. Повышение их дозировки от 0 до 37,5 % увеличило показатель твердости с 0,9 до 4,3 Н, консистенции – с 12,1 до 44,0 мДж, силы адгезии – с 0,6 до 3,6 Н, работы на преодоление адгезии – с 8,2 до 30,6 мДж. При этом отмечены рост вязкости с 2,4 до 3,7 Па•с и снижение набухаемости с 6 до 5 мл/г смеси. Добавление в рецептуру овсяных хлопьев оказывает положительное влияние на органолептическую оценку продукта за счет увеличения значения дескриптора текстуры. Варьирование количеством высушенной барды топинамбура в экструдатах и овсяных хлопьев в рецептурах каш не оказывает значимого влияния на вкус и запах разработанных, готовых к употреблению продуктов. The goal of the work was to develop a technology and recipes for instant porridge based on the process of extrusion of broken rice and dried stillage obtained as part of the complex processing of Jerusalem artichoke into alcohol and food ingredients and characterized by a high content of dietary fiber – 70.6 % and protein – 15.8 %. Using the back extrusion methodology, an instrumental assessment of the texture of instant porridges presented on the Russian market was carried out. It has been shown that the firmness, consistency, adhesion and dynamic viscosity of media vary widely. Porridge recipes have been developed that include, in addition to extrudates, oat flakes and flavoring ingredients: sugar, salt, cardamom. Increasing the proportion of dried Jerusalem artichoke stillage during extrusion from 0 to 6 % makes it possible to increase the content of dietary fiber in porridges to 6 g per 100 g of mixture. This allows the mixture to be characterized as a product with a high dietary fiber content. In terms of rheological characteristics, an increase in dynamic viscosity from 2.7 to 3.6 Pa s and adhesion force from 0.7 to 0.9 N was noted. A more significant factor influencing the rheology of ready-to-eat porridges is the content of oatmeal in their recipe. Increasing their dosage from 0 to 37.5 % increased firmness from 0.9 to 4.3 N, consistency from 12.1 to 44.0 mJ, adhesion force from 0.6 to 3.6 N, work to overcome adhesion from 8.2 to 30.6 mJ. At the same time, an increase in viscosity from 2.4 to 3.7 Pa s and a decrease in swelling power from 6 to 5 ml/g of the mixture were noted. The addition of oat flakes to the recipe has a positive effect on the sensory evaluation of the product by increasing the value of the texture descriptor. Varying the amount of dried Jerusalem artichoke stillage in extrudates and oat flakes in porridge recipes does not have a significant effect on the taste and smell of the developed ready-to-eat products.

Abstract:
Very little experimental data have been published relating to the pullout capacity of prestressing strand lifting loops. To address this gap in knowledge, 13 pullout tests were conducted on strand lifting loops with 0.6 in. (15.24 mm) diameter, 270 ksi (1860 MPa) strand. Straight and bent orientations were tested for single loops at different embedment depths. Loops were embedded in 12 in. (304.8 mm) wide and 44 in. (1117.6 mm) deep concrete blocks and subjected to monotonic, static loading until failure. Marginal bond quality of the strand (18.2 kip [81 kN]), Mohs hardness (3.6), and concrete strength (3000 psi [20.7 MPa]) resulted in an average bond stress value of 400 psi (2758 kPa) at failure. Most tests exhibited pullout failure modes and adequate ductility. Three loops tested at 32 in. (812.8 mm) embedment with 6 in. (152.4 mm), 90-degree bends experienced brittle side-face blowout failures. These failures were due to inclination of the lifting, which led to a reduced edge distance. A safe uniform bond stress of 199 psi (1372 kPa) is recommended for 0.6 in. diameter strand.

Abstract:
Purpose — to study visualized biomarkers before and after navigation laser treatment in patients with diabetic macular edema (DMO) using optical coherence tomography-angiography (OCT-A).Methods. 85 patients (85 eyes) with focal DMO were under observation. The patients were divided into two groups, depending on the method of laser treatment used: group 1 consisted of 50 patients (50 eyes) who underwent navigation sub-threshold laser exposure in micro-pulse and continuous modes with individual selection of energy parameters according to the developed technology (patent RF for invention No. 2752544 of 27.01.2021), in group 2 (35 patients, 35 eyes) laser coagulation was performed according to the type of “modified grid”. The treatment was carried out on the NAVILAS 577S navigation laser system (“OD-OS”, Germany). All patients in both groups underwent high-resolution OCT-A according to the Angio Retina 3×3 mm and HD Angio Retina 6×6 mm protocol. The following biomarkers were evaluated by the OCT-A method: vascular density of the superficial vascular plexus (SCP), deep vascular complex (DCP); the number of microaneurysms and hyperreflective points; the area of the foveal avascular zone, as well as cysts and zones of disorganization of the inner layers of the retina (DRIL), at various times before and after (1, 3 and 6 months) treatment.Results. According to OCT-A, as a result of treatment in group 1, there was an increase in total vascular density from 38.45 ± 0.4 % to 44.85 ± 0.66 % in SCP and from 43.55 ± 0.38 % to 44.85 ± 0.66 % in DCP by 6 months of follow–up. In group 2, the total vascular density in SCP increased from 37.4 ± 0.36 % to 37.85 ± 0.34 %, in DCP from 43.35 ± 0.38 % to 44.05 ± 0.41 %. The number of microaneurysms decreased on average from 12 ± 0.42 to 9 ± 0.3 by 3 months and 7.5 ± 0.26 by 6 months in group 1 and, from 19 ± 0.7 to 15.5 ± 0.6 (3 months) and 13 ± 0.5 (6 months) — in group 2. The number of hyperreflective points decreased in terms of 3 and 6 months in both groups. The area of cysts progressively decreased, and the area of FAZ and DRILS was stable throughout all the follow-up period.Conclusion. OCT-A is a highly informative diagnostic method that allows noninvasively to identify visualized OCT-A biomarkers, as well as to evaluate the effectiveness of navigation laser exposure in patients with diabetic maculopathy with focal diabetic macular edema.

Abstract:
PURPOSE In the KEYNOTE-010 study, pembrolizumab improved overall survival (OS) versus docetaxel in previously treated, programmed death-ligand 1 (PD-L1)‒expressing advanced non‒small-cell lung cancer (NSCLC) in patients with a tumor proportion score (TPS) ≥ 50% and ≥ 1%. We report KEYNOTE-010 long-term outcomes, including after 35 cycles/2 years or second-course pembrolizumab. METHODS Of 1,033 patients randomly assigned (intention to treat), 690 received up to 35 cycles/2 years of pembrolizumab 2 mg/kg (n = 344) or 10 mg/kg (n = 346) every 3 weeks, and 343 received docetaxel 75 mg/m2 every 3 weeks. Eligible patients with disease progression after 35 cycles/2 years of pembrolizumab could receive second-course treatment (up to 17 cycles). Pembrolizumab doses were pooled because no between-dose difference was observed at primary analysis. RESULTS Pembrolizumab continued to improve OS over docetaxel in the PD-L1 TPS ≥ 50% and ≥ 1% groups (hazard ratio [HR], 0.53; 95% CI, 0.42 to 0.66; P < .00001; and HR, 0.69; 95% CI, 0.60 to 0.80; P < .00001, respectively) after a 42.6-month (range, 35.2-53.2 months) median follow-up. Estimated 36-month OS rates were 34.5% versus 12.7% and 22.9% versus 11.0%, respectively. Grade 3-5 treatment-related adverse events occurred in 16% versus 37% of patients, respectively. Seventy-nine of 690 patients completed 35 cycles/2 years of pembrolizumab; 12-month OS and progression-free survival rates after completing treatment were 98.7% (95% CI, 91.1% to 99.8%) and 72.5% (95% CI, 59.9% to 81.8%), respectively. Seventy-five patients (95%) had objective response (RECIST v1.1, blinded independent central review) and 48 (64%) had ongoing response. Grade 3-5 treatment-related adverse events occurred in 17.7% of patients. Fourteen patients received second-course pembrolizumab: 5 completed 17 cycles, 6 (43%) had partial response, and 5 (36%) had stable disease. CONCLUSION Pembrolizumab provided long-term OS benefit over docetaxel, with manageable safety, durable responses among patients receiving 2 years of treatment, and disease control with second-course treatment, further supporting pembrolizumab for previously treated, PD-L1‒expressing advanced NSCLC.

Abstract:
To shed more light on the peculiarities of ground state formation in RB6 Hall effect in NdB6 and LaB6 has been investigated on the single crystals at temperatures 2K<T<300K in magnetic fields H < 80kOe. The Hall coefficient RH and resistivity ρ have been measured by the sample rotation technique in fixed magnetic field to be perpendicular to the axis of rotation (<110> for LaB6 and <100> for NdB6). A very similar temperature behavior of RH(T) with the step–like anomaly near T ~ 25K has been established for both magnetic NdB6 and diamagnetic LaB6 at intermediate temperatures T>10K. The temperature independent behaviour of Hall coefficient (RH(LaB6) ~ –3.6•10-4 cm3/C, RH(NdB6) ~ –3.7•10-4 cm3/C) observed in the interval 10K<T<20K allowed to estimate the reduced charge carriers’ concentration n/nR ≈ 1.21±0.03 (nR– the concentration of La (Nd) ions). The Hall mobility μH = RH(T)/ρ(T) estimated for LaB6 and NdB6 was shown to be well fitted by the power law μH ~ T-α where α(LaB6) ≈ 1.5, α(NdB6) ≈ 0.6 at the intermediate temperatures 50K<T<300K and α(LaB6) ≈ 3 in the range 20K<T<50K. The drastic decrease of both the Hall mobility μH and the exponent α is discussed in terms of the magnetic scattering enhancement of conduction electrons on the localized 4ƒ–states of Nd3+ ions.

Objective. While 6- to 12-month courses of isoniazid for tuberculosis prevention are considered safe in pregnant women, the effects of longer-term isoniazid prophylaxis or isoniazid in combination with antiretroviral therapy (ART) are not established in human-immunodeficiency-virus-(HIV-) infected women who experience pregnancy during the course of therapy.Design.Nested study of pregnancy outcomes among HIV-infected women participating in a placebo-controlled, TB-prevention trial using 36 months daily isoniazid. Pregnancy outcomes were collected by interview and record review.Results. Among 196 pregnant women, 103 (52.6%) were exposed to isoniazid during pregnancy; all were exposed to antiretroviral drugs. Prior to pregnancy they had received a median of 341 days (range 1–1095) of isoniazid. We observed no isoniazid-associated hepatitis or other severe isoniazid-associated adverse events in the 103 women. Pregnancy outcomes were 132 term live births, 42 premature births, 11 stillbirths, 8 low birth weight, 6 spontaneous abortions, 4 neonatal deaths, and 1 congenital abnormality. In a multivariable model, neither isoniazid nor ART exposure during pregnancy was significantly associated with adverse pregnancy outcome (adjusted odds ratios 0.6, 95% CI: 0.3–1.1 and 1.8, 95% CI 0.9–3.6, resp.).Conclusions. Long-term isoniazid prophylaxis was not associated with adverse pregnancy outcomes, such as preterm delivery, even in the context of ART exposure.

Two simple and sensitive spectrophotometric methods for the determination of zirconium and yttrium in YSZ specimens have been suggested in this study. The calibration graph for zirconium and yttrium was linear in the range of 0.08-0.4 and 0.1-0.6 μg/mL, respectively. The proposed analytical procedure was applied for the determination of zirconium and yttrium in the YSZ samples synthesized by aqueous sol-gel method. The relative standard deviation (RSD) for zirconium was in the range from 0.6 to 4.5%, for yttrium it was from 0.6 to 3.6%. The achieved results demonstrated that the suggested procedure could be successfully applied for the analysis of YSZ powders with satisfactory accuracy.

The ligand PTP (3,6-bis(2-pyridylthio)pyridazine) generates both hydroxo- and halogeno-bridged, square-pyramidal, binuclear copper(II) complexes ([Cu2(PTP)X4] (X = Cl (I), Br (II)); [Cu2(PTP)(OH)X3] (X = Cl (IV), Br (V)), which are antiferromagnetically coupled. The hydroxo-bridged derivatives are more strongly coupled than those with just halogen bridges and within each pair of complexes substantially increased spin exchange is observed for the bromo-derivatives (−2J = 130.5 ± 0.4 (I), 243.5 ± 0.3 (II), 294 ± 3 (IV), 326 ± 6 (V) cm−1). A related halogen-bridged derivative [Cu2(PAP46Me)Cl4] (III) (PAP46Me = 1,4-di(4,6-dimethyl-2-pyridylamino)phthalazine) exhibits much weaker spin exchange associated with the diminished superexchange capacity of the phthalazine bridge compared with pyridazine (−2J = 55 ± 0.1 cm−1). An X-ray study on [Cu2(PTP)Br4] reveals an asymmetric tribridged structure involving a pyridazine and two bromo-bridges with a copper–copper separation of 3.318(3) Å, one relatively short terminal copper–bromine bond (2.375(3), 2.398(3) Å), and a short and long copper–bromine bridging contact (2.380(3), 2.801(3); 2.414(3), 2.766(3) Å) per copper. Objective: аssess plasma levels of IL-6 and D-dimer in HIV-positive patients in groups formed on the base of opiate use activity. Materials and methods. Crosssectional study of 346 HIV-positive ART-naive individuals. Participants were categorized into 3 groups according to their history of opioid use: 1) current opioid use — past 30 day opioid use; 2) «ever» opioid use — no use in past 30 days; 3) «never» opioid use. Results. Рarticipants with current opioid use had higher log-transformed IL-6 (0,4 [ s 1,0]; p<0,0001) and D-dimer (-0,6 [ s 0,9]; p<0,0001) levels than participants who reported «ever» (-0,1 [ s 1,0] and -0,9 [ s 0,8]), or «never» (-0,4 [ s 0,7] and 1,3 [ s 0,7]) opioid use. Conclusion. Оpioid use in HIV-positive participants is associated with higher inflammatory response.

Purpose Intracranial efficacy of first-line crizotinib versus chemotherapy was compared prospectively in the phase III PROFILE 1014 study in ALK-positive non–small-cell lung cancer. Patients and Methods Patients were randomly assigned to receive crizotinib (250 mg twice daily; n = 172) or chemotherapy (pemetrexed 500 mg/m2 plus cisplatin 75 mg/m2 or carboplatin at area under the curve 5 to 6, every 3 weeks for ≤ six cycles; n = 171). Patients with stable treated brain metastases (tBM) were eligible. Intracranial efficacy was assessed at baseline and every 6 or 12 weeks in patients with or without known brain metastases (BM), respectively; intracranial time to tumor progression (IC-TTP; per protocol) and intracranial disease control rate (IC-DCR; post hoc) were measured. The intent-to-treat population was also assessed. Results Of 343 patients in the intent-to-treat population, 23% had tBM at baseline. A nonsignificant IC-TTP improvement was observed with crizotinib in the intent-to-treat population (hazard ratio [HR], 0.60; P = .069), patients with tBM (HR, 0.45; P = .063), and patients without BM (HR, 0.69; P = .323). Among patients with tBM, IC-DCR was significantly higher with crizotinib versus chemotherapy at 12 weeks (85% v 45%, respectively; P < .001) and 24 weeks (56% v 25%, respectively; P = .006). Progression-free survival was significantly longer with crizotinib versus chemotherapy in both subgroups (tBM present: HR, 0.40; P < .001; median, 9.0 v 4.0 months, respectively; BM absent: HR, 0.51; P < .001; median, 11.1 v 7.2 months, respectively) and in the intent-to-treat population (HR, 0.45; P < .001; median, 10.9 v 7.0 months, respectively). Conclusion Compared with chemotherapy, crizotinib demonstrated a significantly higher IC-DCR in patients with tBM. Improvements in IC-TTP were not statistically significant in patients with or without tBM, although sensitivity to detect treatment differences in or between the two subgroups was low.

Preemergence and postemergence application of metribuzin [4-amino-6-tert-butyl-3-(methylthio)-as-triazine-5(4H)-one] at 0.6 and 1.1 kg ai/ha controlled downy brome (Bromus tectorumL. ♯ BROTE) in winter wheat (Triticum aestivumL. ‘McDermid’) but caused considerable injury without the use of activated carbon over the seeded row. Activated carbon applied in 5-cm bands over the seeded row at 84, 167, and 336 kg/ha protected winter wheat at Pendleton on a silt loam soil. On a sandy loam soil, only a 336 kg/ha rate provided protection from metribuzin. Metribuzin toxicity to winter wheat was more difficult to neutralize when applied preemergence. Downy brome control was not reduced by carbon applied over the wheat row. The best treatment in this study was carbon at 336 kg/ha applied preemergence over the row followed by metribuzin at 0.6 or 1.1 kg/ha postemergence. A 10-week delay between preemergence carbon banding and postemergence metribuzin protected winter wheat from chemical injury.

Relevance. The rotavirus infection (RI) is one of the leading causative agent in the structure of acute enteric infections, established etiology. Long term epidemiological monitoring RI was shown that Podolsk-city in Moscow region was unfavorable place concerning prevalence of RI. Aims. Studying epidemiological efficiency and collective immunization safety of infancy children in Podolsk-city, Moscow region. Materials and methods. Registration rates of this morbidity and coverage of RI prophylactic immunization were analyzed on the basis of formal registration forms, epidemiological detection cards, out-patient cards for observation over a patients, medical reports, case register in policlinics and hospitals, vaccination documents (f. № 5, f. № 6, f. 025/a, f. 058/a, f. 060/a, f. 063/a, f. 064/a). RI data have been provided over a period of time from 2008 to 2018. Data coverage of vaccination have been given from 2015 to 2018. Used method is statistic observation. Results. Vaccination program concerning RI has been implemented in Podolsk-city from 2008 till 2018. Babies at breast in their first being were vaccinated. Live pentavalent was used for immunization. The coverage target contingent of vaccination reached 68%. This preparation was withstood by children well, unwanted effects and intestines invaginations during the vaccination were not registered. During vaccination, the number children gone to hospital with acute intestinal infections was decreasing annually. The number of children to hospital admissions at the age of one year decreased from 15.8 to 12.5%, at the age of 2–3 decreased from 43.8 to 36.3%, with small increase in specific weight of patients of an age group of 7–14 years from 7.70 to 11.2%, but at the reduced total number of inpatients of this age category. Reduction of RI morbidity marked as in target age group as in another groups which were not vaccinated. Thus, formation of population benefits was confirmed. Conclusion. Clinicoepidemiologic research paper was studied in 2014–2018 allowed to make a conclusion that the cohort immunization of infancy children with the rotavirus infection, contributes to decreasing rate morbidity in certain territory Moscow region which had surpassing factors of this infection in comparison with other cities. The number of covering collective immunization was not lower 60% of infancy children and assisted falling enteric infections in their 0–6 years, made a big impression on reducing of acute intestinal diseases among all age groups during immunization implementation company within three years.

Pr6O11-doped bismuth titanate and random oriented BixPryTi3O12 ( y = 0.3, 0.6, 0.9, 1.2) thin films were fabricated on Pt/Ti/SiO2/Si substrates by rf magnetron sputtering technique. These samples had polycrystalline Bi-layered perovskite structure without preferred orientation, and consisted of well developed rod-like grains with random orientation. Pr doping into BIT caused a shift of the Curie temperature (TC) of the BIT from 675°C to 578 , 517, 398 , and 315oC for the films with y = 0.3, 0.6, 0.9, and 1.2, respectively. The experimental results indicated that Pr doping into BIT result in a remarkable improvement in dielectric properties. Raman analysis shows that Pr3+ and Pr4+ ions substitution only appears in A-sit.

BackgroundRheumatoid arthritis (RA) is the most common autoimmune disease. Clinical remission is the main therapeutic target for patients with RA, with low disease activity (LDA) as a best possible alternative[1].ObjectivesEvaluate clinical response of patients with RA treated with biologic (bDMARDs) and the associated factors for remission and low activity disease at the first year of treatment.MethodsBIOBADAMEX is a Mexican ongoing cohort of patients using bDMARDs since 2016. In this analysis we included all patients with diagnosis of RA with at least two assessments, baseline and at one year of follow up. Clinical response was measured by DAS-28 score and factors associated with having a remission or low disease activity vs moderate/high activity were estimated using univariable and multivariable logistic regression analysis adjusting by potential confounders, such as age, gender, disease duration.ResultsA total of 336 RA patients were included in this analysis. At baseline 68 (20%) and after one year of follow 171 (50.1%) were in remission or low activity disease. Mean baseline disease duration was 8.8 years. Table 1 shows baseline characteristics by groups. Baseline steroid treatment and previous biologic treatment were the only factors associated with less probability of being in remission/low disease activity at one year in our cohort. In multivariable analysis adjusted by age, gender, disease duration, these two factors were no longer associated with remission/low disease activity (p=0.055 and p=0.14, respectively).ConclusionIn our registry baseline steroid treatment and previous biologic treatment were the only factors associated with less probability of being in remission/low disease activity at one year on biologic treatment, however these factors did not remain significant associated after adjusted analysis. In 1992 and 1993, 12 postemergence herbicide treatments were applied to field-grown buffalograss [Buchloe dactyloides (Nutt.) Engelm.] seedlings having 1 to 3 leaves and 2 to 4 tillers, respectively. The only herbicide treatments that did not cause plant injury at 1 or 2 weeks after treatment (WAT) or reduce turf coverage 4 or 6 WAT compared to nontreated plots (in 1992 or 1993) were (in kg·ha–1) 0.6 dithiopyr, 0.8 quinclorac, 2.2 MSMA, and 0.8 clorpyralid. Evaluated only in 1993, metsulfuron methyl (0.04 kg·ha–1) also caused no plant injury or reduction in coverage. Fenoxaprop-ethyl (0.2 kg·ha–1) caused severe plant injury and reduced coverage by >95% at 6 WAT. Dicamba reduced coverage by 11% at 6 WAT in 1992 but not 1993. The chemicals (in kg·ha–1) triclopyr (0.6), 2,4-D (0.8), triclopyr (1.1) + 2,4-D (2.8), 2,4-D (3.1) + triclopyr (0.3) + clorpyralid (0.2), and 2,4-D (2.0) + mecoprop (1.1) + dicamba (0.2) caused plant injury at 1 or 2 WAT in 1992 or 1993, but coverage was similar to that of nontreated turf by 6 WAT. IntroductionThis study aims to (i)evaluate the diagnostic sensitivity, specificity and predictive values of seizure-related heart rate (HR) in differentiating epileptic seizures(ES) from psychogenic non-epileptic seizures(PNES), (ii)define the most useful point of HR measurement: pre-ictal, ictal-onset, maximal-ictal or post-ictal, and (iii)define the HR cut-off points to differentiate ES from PNES.MethodsAll video EEG(VEEG) at Monash Health from May 2009 to November 2015 were retrospectively reviewed. Baseline(during wakefulness), one-minute pre-ictal, ictal-onset, maximal-ictal and one-minute post-ictal HR were measured for each ES and PNES event. Events less than ten seconds or with uninterpretable ECG due to artefacts were excluded. ROC curve analysis was performed to study the diagnostic accuracy reflected by area under the curve(AUC). The AUC was interpreted as follows; ≤0.5, differentiation of PNES from ES no better than chance; 0.80–0.89, good differentiation; and 0.9–1, excellent differentiation.ResultsVEEG of 341 ES and 265 PNES from 130 patients were analysed. The AUC for pre-ictal, ictal-onset, maximal-ictal and post-ictal HR were found to have poor differentiation between ES and PNES. Comparing PNES and bilateral tonic-clonic ES, AUC for absolute maximal-ictal HR was 0.84(CI 0.73–0.95) and for absolute post-ictal HR was 0.90(CI 0.81–1.00). Using Youden’s index, to diagnose tonic-clonic ES, the optimal cut-off point for absolute maximal-ictal HR was 114bpm (sensitivity 84%;specificity 82%;PPV 26.7%,NPV 98.5%) and for absolute post-ictal HR was 90bpm(sensitivity 91%;specificity 82%;PPV 30.3%;NPV 99.1%).ConclusionsThese findings suggest that seizure-related HR increase is useful in differentiating bilateral tonic-clonic ES from PNES. Based on the AUC, the best diagnostic measureme.

An experiment was conducted during three consecutive years of Rabi (2011-12 to 2013-14) at Agricultural Research Station-Ummedganj, Agriculture University, Kota (Rajasthan) on wheat based intercropping system. The experiment consisted of ten treatment combinations viz., two irrigation regimes (IW/CPE ratio 0.4 and 0.6) and five intercropping system (wheat + gram (6:4), wheat + mustard (6:4), wheat + fenugreek (6:4), wheat + field pea (6:4) and sole wheat) were under taken in split plot design with four replications.It is evident from pooled data the maximum wheat equivalent yield (53.68 q/ha) was observed with irrigation regime at IW/CPE ratio 0.6 over application of IW/CPE ratio 0.4 (45.04 q/ha). Among intercropping, wheat + gram (6:4) intercropping system gave significantly higher wheat equivalent yield (58.50 q/ha) overwheat + mustard (6:4)(50.91 q/ha), wheat + fenugreek (6:4)(46.28 q/ha) and wheat + field pea (6:4)(46.08 q/ha) intercropping system as well as sole wheat (45.04 q/ha), respectively. Significantly higher water use efficiency (23.49kg/ha-cm) was recorded under wheat + gram (6:4) intercropping system over wheat + mustard (6:4), wheat + fenugreek (6:4) and wheat + field pea (6:4) intercropping system as well as sole wheat.The maximum net return (Rs.55810/- ha-1) and B:C ratio (3.6) was observed with irrigation regime at IW/CPE ratio 0.6 as compared to IW/CPE ratio 0.4. Among intercropping, wheat + gram (6:4) intercropping system gave significantly higher net return (Rs.62426/- ha-1) and B:C ratio (4.0) over wheat + mustard (6:4), wheat + fenugreek (6:4) and wheat + field pea (6:4) intercropping system as well as sole wheat.

Background. In inflammatory bowel disease (IBD) patients there are reports of the occurrence of hepatobiliary manifestations, so the aim of this study was to evaluate the hepatobiliary manifestations in patients with Crohn’s disease (CD) and ulcerative colitis (UC) from an IBD reference center. Methods. Cross-sectional study in an IBD reference center, with interviews and review of medical charts, between July 2015 and August 2016. A questionnaire addressing epidemiological and clinical characteristics was used. Results. We interviewed 306 patients, and the majority had UC (53.9%) and were female (61.8%). Hepatobiliary manifestations were observed in 60 (19.6%) patients with IBD. In the greater part of the patients (56.7%) hepatobiliary disorders were detected after the diagnosis of IBD. In UC (18.2%) patients, the hepatobiliary disorders identified were 11 (6.7%) non-alcoholic fatty liver disease, 9 (5.5%) cholelithiasis, 6 (3.6%) primary sclerosing cholangitis (PSC), 3 (1.8%) hepatotoxicity associated with azathioprine, 1 (0.6%) hepatitis B, and 1 (0.6%) hepatic fibrosis. In CD (21.3%) patients, 11 (7.8%) had cholelithiasis, 11 (7.8%) non-alcoholic fatty liver disease, 4 (2.8%) PSC, 3 (2.1%) hepatotoxicity, 1 (0.7%) hepatitis B, (0.7%) hepatitis C, 1 (0.7%) alcoholic liver disease, and 1 (0.7%) autoimmune hepatitis (AIH). There was one case of PSC/AIH overlap syndrome. Conclusion. The frequency of hepatobiliary disorders was similar in both forms of IBD in patients evaluated. The most common nonspecific hepatobiliary manifestations in IBD patients were non-alcoholic liver disease and cholelithiasis. The most common specific hepatobiliary disorder was PSC in patients with extensive UC or ileocolonic CD involvement; this was seen more frequently in male patients.

Catalase from Bacillus sp. N2a (BNC) isolated from Antarctic seawater was purified to homogeneity. BNC has a molecular mass of about 230 kDa and is composed of four identical subunits of 56 kDa. The catalase showed optimal activity at 25 °C and at a pH range of 6–11. The enzyme could be inhibited by azide, hydroxylamine, and mercaptoethanol. These characteristics suggested that BNC is a small-subunit monofunctional catalase. The activation energy of BNC was 13 kJ/mol and the apparent kcat/Km values were 3.6 × 106 and 4 × 106 L·mol–1·s–1 at 4 and 25 °C, respectively. High catalytic efficiency of BNC at low temperatures enables this bacterium to scavenge H2O2 efficiently. BNC exhibited activation energy, catalytic efficiency, and thermostability comparable with some mesophilic homologues. Such similarity of enzymatic characteristics to mesophilic homologues, although uncommon among the cold-adapted enzymes in general, has also been observed in other psychrophilic small-subunit monofunctional catalases.

When the content is 3.5% in the alloy, the hardness of brass increases to 78.7HRB, up 6% compare with Cu-22.7Zn brass. With 1.0% Ni, the strength of the alloy raises to 76.5 HRB, increased by 3.6%. Cu-22.7Zn-3.4Al as-cast alloy, its strength of alloy increases 12.5% with 0.6% Co, and enhances 9.8% with 0.03% Y.

Type 316 stainless steel is a material that is often used in the medical world, especially as a material for making bone implant plates. However, the use of stainless steel as an implant material, stainless steel must be treated to increase the hardness and adjust the level of roughness on the surface. In this study, shot peening was carried out on the 316 stainless steel surface. The shooting parameters used were 2, 4, 6 minutes for the shooting duration, and 0o, 15o, 30o, 45o for the shooting angle, as well as the diameter of the steel shot used, i.e. 0.6 mm and 1 mm. The results of this study indicated that there was a change in the characteristic properties of 316 stainless steel, namely the hardness and surface roughness.

The marine sediment samples collected from the northeastern Brazilian continental shelf, at water depths between 20 and 80 m, consisted mainly of sands with an almost total absence of gravel and granules. Medium, coarse and very coarse sand grains are mostly composed of halimeda, lithothamnium, rodoliths and bioclastic sands with a carbonate content varying between 77 and 96 %. The chemistry in general shows a decreasing content of Ca (86.1 %) >Si (6 %) > Cl (3.6 %) > Sr (0.8 %) > K (0.66 %) > S (0.62 %) > Al (0.6 %) > Na (0.55%) > Mg (0.43 %) > Fe (0.4 %) > P (0.2 %) > Br (0.04 %) in the samples. There was no correlation between CaCO3 and chemical contents and grain size with depth and bio-components. With the exception of Sr of marine origin, all other elements (P, S, Br, Cl, Fe) are of continental origin. The lithothamnium of some offshore samples shows higher CaCO3 content, while Mg and Na are present only in halimedas. Bioclastic sands contain no Br, and silt and clay fractions are rare and characterize samples closer to the coast. These marine bioclastic granulates are of very pure biogenic calcium carbonates and are thus highly to be recommended for economic purposes.

Abstract Introduction Dental technicians are medical technology professionals responsible for the preparation of crown restorations and prosthetics, and the possibility that they may experience respiratory dysfunction due to the inhalation of metal fumes during dental laboratory work has been raised. In this study, we surveyed dental technicians for subjective symptoms and measured serum levels of inflammatory cytokines. Methods Forty subjects were investigated. The survey items included subjective symptoms and smoking history using the Japanese version of the ATS-DLD Standard Questionnaire for Respiratory Symptoms. Serum inflammatory cytokines such as TNF-α, IL-6 and 8-OHdG were also measured. The study was approved by the Ethics Committee of Showa University (approval number 2994). Results The number of years of employment (mean, range) was 20.4 (0.5-50) years. The most frequently reported respiratory symptoms was nasal congestion (35.0%). Nine patients (22.5%) had no subjective symptoms. Inflammation markers TNF-α (reference value 0.6-2.8 pg/ml) and IL-6 (reference value 8.0 pg/ml) were below the reference values in all subjects. None of the subjects showed elevated levels of 8-OHdG. However, a significant positive correlation was observed between the number of years of employment and TNF-α levels. Discussion and Conclusion About 80% of the dental technicians had some subjective symptoms of respiratory dysfunction, but most of the inflammatory markers and 8-OHdG were below standard values. TNF-α levels tended to be higher with longer working years, suggesting a higher risk of inflammation with longer working years.

The upper airway muscles play an important role in maintaining upper airway collapsibility, and the incidence of sleep-disordered breathing increases with age. We hypothesize that the increase in airway collapsibility with increasing age can be linked to changes in upper airway muscle mechanics and structure. Eight young (Y: 6 mo) and eight old (O: 30 mo) Fischer 344 rats were anesthetized and mechanically ventilated, and the pharyngeal pressure associated with flow limitation (Pcrit) was measured 1) with the hypoglossal (cnXII) nerve intact, 2) following bilateral cnXII denervation, and 3) during cnXII stimulation. With the cnXII intact, the upper airways of older rats were more collapsible compared with their younger counterparts [Pcrit = −7.1 ± 0.6 (SE) vs. −9.5 ± 0.7 cmH2O, respectively; P = 0.033]. CnXII denervation resulted in an increase in Pcrit such that Pcrit became similar in both groups (O: −4.2 ± 0.5 cmH2O; Y: −5.4 ± 0.5 cmH2O). In all rats, cnXII stimulation decreased Pcrit (less collapsible) in both groups (O: −11.3 ± 1.0 cmH2O; Y: −10.2 ± 1.0 cmH2O). The myosin heavy chain composition of the genioglossus muscle demonstrated a decrease in the percentage of the IIb isoform (38.3 ± 2.5 vs. 21.7 ± 1.7%; P < 0.001); in contrast, the sternohyoid muscle demonstrated an increase in the percentage of the IIb isoform (72.2 ± 2.5 vs. 58.4 ± 2.3%; P = 0.001) with age. We conclude that the upper airway becomes more collapsible with age and that the increase in upper airway collapsibility with age is likely related to altered neural control rather than to primary alterations in upper airway muscle structure and function.

The use of animal wastes for crop production and to reduce the menace of these wastes in man’s environment must also be considered. Hence, a field experiment was carried out at Lagos State University of Education, Michael Otedola Campus, Teaching and Research Farm, Epe Lagos using pig and poultry wastes; prepared into a slurry and applied at different rates for maize production under field conditions. The experimental design was randomized complete block design (RCBD) with six treatments; which are: NFE-Control, Pig dung (0.6, 1.2 and 1.8 kg/ 3.6 m2) and Poultry dung (0.6, 1.2 and 1.8 kg/ 3.6 m2) and replicated three times. From this study, animal waste slurry for maize production increased both the vegetative growth and maize grain yield. Maize Plant Height (PH, cm), at 4 weeks after sowing ranged from 19.8±8.8 – 33.1±8.8 under NFE and Poultry dung (0.6kg) applications, and 50.8±18.9 – 93.4±18.9 under NFE and Pig manure (1.8kg) application at 6 weeks after sowing. Both Poultry and Pig dung slurry at 0.6kg - 1.8kg/3.6 m2 on nutrient-depleted soil gave grains yield ranging from 1.1 to 2.0 t/ha. As a result, the application of these dungs increased the growth and yield of maize and is thus recommended for maize production.

Vascular remodeling and changes in vascular responsiveness occur in the rat cerebrum with old age. This includes reductions in cerebral arteriolar numerical density, cross-sectional area, distensibility, the relative proportion of distensible elements in the cerebral arteriolar wall, and reduced endothelium-dependent relaxation. The purpose of this study was to test the hypothesis that old age results in an increase in vascular resistance and, correspondingly, a decrease in blood flow to ocular, regional cerebral, and spinal tissue in the rat. Blood flow was measured in the eye, olfactory bulb, left and right cerebrum, pituitary gland, midbrain, pons, cerebellum, medulla, and spinal cord of juvenile (2-mo-old, n = 6), adult (6-mo-old, n = 7), and aged (24-mo-old, n = 7) male Fischer-344 rats. Arterial pressure and blood flow were used to calculate vascular resistance. Vascular resistance in the eye of aged rats (6.03 ± 1.08 mmHg ⋅ ml−1 ⋅ min ⋅ 100 g) was higher than that in juvenile (3.83 ± 0.38 mmHg ⋅ ml−1 ⋅ min ⋅ 100 g) and adult rats (3.12 ± 0.24 mmHg ⋅ ml−1 ⋅ min ⋅ 100 g). Similarly, resistance in the pons of older rats (2.24 ± 0.55 mmHg ⋅ ml−1 ⋅ min ⋅ 100 g) was greater than in juvenile (0.66 ± 0.06 mmHg ⋅ml−1 ⋅ min ⋅ 100 g) and adult rats (0.80 ± 0.11 mmHg ⋅ ml−1 ⋅ min ⋅ 100 g). In contrast, vascular resistance in the pituitary gland was lower in the aged rats (juvenile, 3.09 ± 0.22; adult, 2.79 ± 0.42; aged, 1.73 ± 0.32 mmHg ⋅ ml−1 ⋅ min ⋅ 100 g, respectively). Vascular resistance was not different in other cerebral tissues or in the spinal cord in the aged rats. These data suggest that regional cerebral and spinal blood flow and vascular resistance remain largely unchanged in conscious aged rats at rest but that elevations in ocular vascular resistance and, correspondingly, decreases in ocular perfusion with advanced age could have serious adverse effects on visual function.

Abstract:
The aim of the present study was to investigate changes in intramuscular triglyceride (IMTG) content and perilipin 2 expression in skeletal muscle tissue following 6 mo of endurance-type exercise training in type 2 diabetes patients. Ten obese male type 2 diabetes patients (age 62 ± 1 yr, body mass index BMI 31 ± 1 kg/m2) completed three exercise sessions/week consisting of 40 min of continuous endurance-type exercise at 75% V̇o2 peak for a period of 6 mo. Muscle biopsies collected at baseline and after 2 and 6 mo of intervention were analyzed for IMTG content and perilipin 2 expression using fiber type-specific immunofluorescence microscopy. Endurance-type exercise training reduced trunk body fat by 6 ± 2% and increased whole body oxygen uptake capacity by 13 ± 7% ( P < 0.05). IMTG content increased twofold in response to the 6 mo of exercise training in both type I and type II muscle fibers ( P < 0.05). A threefold increase in perilipin 2 expression was observed from baseline to 2 and 6 mo of intervention in the type I muscle fibers only (1.1 ± 0.3, 3.4 ± 0.6, and 3.6 ± 0.6% of fibers stained, respectively, P < 0.05). Exercise training induced a 1.6-fold increase in mitochondrial content after 6 mo of training in both type I and type II muscle fibers ( P < 0.05). In conclusion, this is the first study to report that prolonged endurance-type exercise training increases the expression of perilipin 2 alongside increases in IMTG content in a type I muscle fiber-type specific manner in type 2 diabetes patients.

Abstract:
Sucrose overfeeding increases intrahepatocellular (IHCL) and intramyocellular (IMCL) lipid concentrations in healthy subjects. We hypothesized that these effects would be modulated by diet protein/fat content. Twelve healthy men and women were studied on two occasions in a randomized, cross-over trial. On each occasion, they received a 3-day 12% protein weight maintenance diet (WM) followed by a 6-day hypercaloric high sucrose diet (150% energy requirements). On one occasion the hypercaloric diet contained 5% protein and 25% fat (low protein-high fat, LP-HF), on the other occasion it contained 20% protein and 10% fat (high protein-low fat, HP-LF). IHCL and IMCL concentrations (magnetic resonance spectroscopy) and energy expenditure (indirect calorimetry) were measured after WM, and again after HP-LF/LP-HF. IHCL increased from 25.0 ± 3.6 after WM to 147.1 ± 26.9 mmol/kg wet weight (ww) after LP-HF and from 30.3 ± 7.7 to 57.8 ± 14.8 after HP-LF (two-way ANOVA with interaction: p < 0.001 overfeeding x protein/fat content). IMCL increased from 7.1 ± 0.6 to 8.8 ± 0.7 mmol/kg ww after LP-HF and from 6.2 ± 0.6 to 6.9 ± 0.6 after HP-LF, (p < 0.002). These results indicate that liver and muscle fat deposition is enhanced when sucrose overfeeding is associated with a low protein, high fat diet compared to a high protein, low fat diet.

Bau langu yang dihasilkan oleh sari kacang kedelai membuat sebagian orang tidak menyukai sari kacang kedelai. Salah satu cara untuk mengurangi bau langu adalah dengan cara fermentasi. Yoghurt sari kacang kedelai dengan penambahan pasta hidrolisat ubi jalar ungu dapat menambah daya prebiotik yang mendukung pertumbuhan bakteri asam laktat. Tujuan penelitian yaitu mengetahui profil yogurt sari kacang kedelai dengan penambahan pasta ubi jalar ungu yang dihidrolisis secara enzimatis. Metode penelitian menggunakan Rancangan Acak Lengkap (RAL) satu faktor dengan 6 taraf perlakuan dengan penambahan pasta ubi jalar ungu (0%; 2%; 4%; 6%; 8%; dan 10%). Faktor yang digunakan merupakan penambahan pasta hidrolisat ubi jalar ungu Ayamurasaki. Analisis karakteristik produk yogurt meliputi uji pH, total asam laktat, total bakteri asam laktat, kadar air, kadar abu, kadar protein dan kadar lemak serta uji organoleptik meliputi uji hedonik dan mutu sensori. Analisis data menggunakan Anova dengan uji lanjut Duncan dengan selang kepercayaan 95%. Hasil penelitian menunjukkan bahwa yogurt dengan penambahan 6% pasta ubi jalar mempuyai karakter kimia untuk pH 4,03 – 4,33; total asam laktat 0,66 – 0,85%; total bakteri asam laktat 3,57×108 – 3,22 ×109 koloni/ mL; kadar air 86,18 – 86,74%; kadar abu 0,54 – 0,66%; kadar protein 3,46-3,59%; kadar lemak 2,11-2,18%.

Abstract Abstract 2445 Constitutive activation of signal transducer and activator of transcription 3 (STAT3) correlates with drug resistance and a poor prognosis in many cancers. STAT3 signaling is mediated by phosphorylation at tyrosine-705 (p STAT3Y705), dimerization, and nuclear transactivation. In chronic myeloid leukemia (CML), pSTAT3Y705 is demonstrable under two distinct resistance scenarios: (1) extrinsic resistance, in which BCR-ABL1 kinase-independent survival signals originating from the bone marrow (BM) microenvironment activate pSTAT3Y705 in a JAK2- or TYK2-dependent manner, and (2) intrinsic resistance, in which BCR-ABL1 kinase-independent signals activate pSTAT3Y705 in response to kinase inhibition. Based on these observations, we identified TKI-resistant CML as an excellent model for developing and optimizing pharmacologic STAT3 inhibitors. Using K562 and AR230 CML cells that are resistant to 1 μM imatinib (K562R and AR230R; intrinsic resistance) and primary CML CD34+ progenitor cells exposed to BM stromal-derived conditioned medium (CM; extrinsic resistance), we examined the effects of direct pharmacologic inhibition of STAT3 in TKI-resistant CML. Here, we report the design and validation of next-generation STAT3 inhibitors identified through computational modeling and screening in AR230R CML cells expressing high levels of pSTAT3Y705. We initially examined the effects of an established STAT3 inhibitor, S3I-201.1066 (SF1–066). K562R or AR230R cells were treated with 1 μM imatinib and/or 10 μM SF1–066, followed by culture in methylcellulose medium and scoring after 14–16 days. Combination treatment reduced the clonogenicity of K562R and AR230R cells to 31.4% (p<0.02) and 27.5% (p<0.004) of controls, respectively. In contrast, SF1–066 did not synergize with imatinib in parental K562 and AR230 cells that lack constitutive pSTAT3Y705 (p>0.05). Next, CD34+ cells from newly diagnosed CML patients (n=4) were cultured for 96 hours in the presence of CM and treated with 2.5 μM imatinib, 10 μM SF1–066 or both. Equal numbers of cells were then plated in colony forming assays. Imatinib combined with SF1–066 reduced colony formation to 54.8% (p<0.002) of controls treated with imatinib. Given that dual treatment reduces but does not completely suppress colony formation, we reasoned that some STAT3 signaling must persist in the presence of SF1–066. Thus, we developed next-generation STAT3 inhibitors with increased activity compared to SF1–066. Computational modeling informed the design and synthesis of a second-generation, SF1–066-based library. We evaluated compounds with shared molecular functionalities using AR230R cells expressing a luciferase reporter containing sequential STAT3 sis-inducible elements (AR230R-SIE). At 10 μM, we found two inhibitors, BP2–047 and BP3–163, that reduced luminescence by 69.2% (p<3.6×10−6) and 59.7% (p<8.6×10−6), respectively, compared to controls (n=3). However, the compounds also reduced luminescence in AR230R cells expressing a scrambled luciferase reporter (AR230R-NEG). This screen provided information on the structure-activity relationships of the compounds and was used to generate 64 more candidate STAT3 inhibitors. To account for the reduced luminescence in AR230R-NEG cells and analyze for more potent inhibitors, we performed the luciferase assay with candidate inhibitors at 5 μM. We identified BP5–087 and BP5–088, each with increased potency and STAT3 selectivity compared to SF1–066. These compounds decreased luminescence in AR230R-SIE cells by 36.1% (p<2.6×10−7) and 25.5% (p<6.1×10−10), respectively (n=3), with minimal effects on AR230R-NEG cells (p>0.05). Confirmation of STAT3 binding was obtained using fluorescence polarization assays, in which the EC50 values of BP5–087 and BP5–088 measured 8.5 μM and 4.6 μM, respectively. Studies with mouse and human liver microsomes also revealed that BP5–087 and BP5–088 exhibit enhanced metabolic stability compared to SF1–066. Interestingly, treatment with BP5–087 or BP5–088 (both at 1 and 5 μM) in CML CD34+ progenitors grown in CM showed increased cytoplasmic accumulation of pSTAT3Y705 compared to controls. Thus, we identified BP5–087 and BP5–088 as two of the most potent small-molecule binders of STAT3 reported. These compounds are promising frontrunners toward new therapies for TKI-resistant CML and other diseases in which STAT3 activation drives malignant phenotypes. Penelitian ini dilakukan di TK ABA 06 Medan. Penelitian ini bertujuan untuk mengetahui pengaruh penggunaan media flashcard terhadap kemampuan hahasa anak. Populasi dalam penelitian ini adalah seluruh siswa kelompok B di TK ABA 06 Medan Tahun Ajaran 2021/2022 yang terdiri dari dua kelas yaitu kelas mawar sebagai kelas eksperimen dan kelas teratai sebagai kelas kontrol. Adapun jenis penelitian yang digunakan dalam penelitian ini ialah menggunakan metode penelitian kuantitatif (pengujian) melalui bentuk semu, dalam gambaran Postets OnlyControl Design. Penelitian ini dilakukan dengan menggunakan dua kelas yang memiliki karakteristik yang sama, yaitu kelas B1 (Mawar) dengan menggunakan media flashcrad dan kelas B2 (Teratai) dengan menggunakan media kartu huruf. Penentuan sampel kelas dilakukan secara acak(random) denagn jumlah sampel tiap kelas sebanyak 13 anak. Hasil penelitian ini menunjukkan bahwa kemampuan bahasa pada anak usia 5-6 tahun meningkat setelah menggunakan media flashcard. Kemampuan bahasa pada kelas eksperimen lebih meningkat daripada kemampuan bahasa di kelas kontrol. Perihal ini dapat diketahui dengan rata-rata kelompok eksperimen sebesar 3,46 sedangkan rata-rata kelompok kontrol sebesar 2,86 dengan selisih 0,6. Hasil pengujian menunjukkan bahwa penggunaan media flashcrad memberikan pengaruh yang signifikan terhadap kemampuan bahasa anak dengan > yaitu 6,5 > 4. Dengan demikian dapat disimpulkan bahwa ada pengaruh yang signifikan penggunaan media flashcard terhadap kemampuan bahasa anak usia 5-6 tahun di TK ABA 06 Medan Tahun Ajaran 2021/2022.

ABSTRACTThree randomized-block experiments involving 87 British Friesian, 18 Simmental × Friesian and 18 Hereford × Friesian, artificially reared calves (mean initial live weight 47 kg and age 9 days), were carried out to examine the effects of the quantity of milk substitute consumed on the lifetime performance of beef cattle. The three treatments were (1) 400 g milk powder per head daily, weaned at 42 days of age (2) ad libitum intake of milk powder, weaned at 42 days and (3) ad libitum intake of milk powder, weaned at 60 days. All calves were individually penned and fed until after weaning; those on treatment 1 were bucket fed twice daily, while those on treatments 2 and 3 sucked the milk from 25·1 containers through artificial teats. They were offered a pelleted, barley/soya-bean meal concentrate (197 g crude protein per kg dry matter (DM)) ad libitum from purchase until intake reached a maximum of 2·7 kg per head daily, and well preserved grass silage ad libitum from 6 weeks of age. Three commercial, skimmed milk-based, acidified milk substitutes (242 and 187 g crude protein and oil per kg respectively) were used. Intakes of milk substitute, concentrate DM and silage DM per calf to 12 weeks of age for treatments 1 to 3 respectively were as follows: 13·0, 39·7 and 54·8 (s.e. 1·67) kg; 88, 73 and 58 (s.e. 1·5) kg and 9·4, 10·6 and 11·2 (s.e. 0·23) kg. Live-weight gains to 12 weeks of age and carcass weights adjusted to 19 months of age were 0·71, 0·82 and 0·84 (s.e. 0·026) kg/day and 335, 340 and 341 (s.e. 3·8) kg. It is concluded that increasing the input of milk substitute above 400 g per head daily until 6 weeks of age produced only a small and uneconomic increase in the lifetime performance of beef cattle.

BackgroundSafety is a driving factor when selecting treatment, but the risks should be balanced against the treatment benefits, relative to alternative treatment options. In JAKis, recent evidence of increased safety risks in older patients with cardiovascular (CV) risk factors, have led to concerns. However, an increased risk among certain patients might still be offset by an increasing benefit in the same patient segment, and vice versa. A full understanding of the JAKi benefit-risk profile thus requires a better understanding of the relative (JAKi vs other treatments) benefits in those patients where their relative safety profile may be worse.ObjectivesTo assess the observed and adjusted relative effectiveness (remission and response) of JAKis (and non-TNFi bDMARDs) when compared to TNFi, as used in clinical practice, and to assess whether the predicted effectiveness of either drug class is modified by age and the presence of CV risk factors.MethodsRA patients initiating treatment with a JAKi, non-TNFi, or TNFi 2016-2021 were followed using the Swedish Rheumatology Quality Register. Good EULAR response and remission (CDAI<=2.8) at 6 months were assessed at return visits (within 90 to 270 days after treatment initiation). Non-responder imputation was implemented where treatment ended prior to 210 days after initiation.Crude probabilities of outcomes were presented by age and having ≥1 CV risk factor (Y/N). Linear regression estimated the risk difference (versus TNFi) in age and CV risk groups, adjusted for sex, baseline DAS28 (response) and CDAI (remission), and line of therapy. Probability of outcome was predicted from logistic regression models, fitted to allow effect modification of treatment cohort by age and CV risk score (ERS-RA score), adjusted for sex, baseline DAS28 (response) and CDAI (remission), and line of therapy.Results10,309 treatment episodes contributed data on response at 6 months, and 12,016 episodes with data on remission. Overall, 21% achieved good EULAR response and 11% reached CDAI remission (Table 1).Crude observed risks overall indicated superior response and remission with TNFi versus JAKi (Table 1). Adjusted risk differences suggested superior outcomes in JAKi versus TNFi, overall, and in those with no CV risk for CDAI remission. Patterns of predicted response and remission varied across age and CV ERS-RA score (Figure 1), with JAKis having superior effectiveness overall once adjusting for line of therapy.ConclusionAge and CV risk may modify the absolute and relative effectiveness of JAKis vs. TNFi. The stability constants for the complexes formed between iron species existing in ambient soil environment and fulvic acids (FA) extracted from organic wastes like sewage sludge, farm yard manure (FYM), poultry manure, and press mud were determined in aqueous media of pH 5·0 and 8·5 by discontinuous spectrocolorimetric titration method. The values of stability constant (log K) of Fe–FA complexes estimated at pH 5·0 were 6·026, 6·212, 6·270, and 6·342 for FYM, sludge, poultry manure, and press mud, respectively. The respective values at pH 8·5 were 6·145, 6·276, 6·350, and 6·940. The order of the values of log K for different preparations of fulvic acids was press mud > poultry manure > sludge > FYM. The functional group contents, their pH of neutralisation, and electrostatic properties of the FA such as pKINT, pKm, and 0·868 nW, were found to be the determining factors for maximum binding ability of FA for metal cations and the stability constant of Fe–FA for different FA preparations. The basic assumptions and the limitations of the discontinuous spectrocolorimetric estimation of stability constants for Fe–FA are discussed.

Ten secondary metabolites, including a new grifolin analog, grifolin B (1); a new homovalencic acid derivative, 12-hydroxyhomovalencic acid (7); and a compound isolated from a natural source for the first time (9), along with seven known compounds, grifolin (2), averantin (3), 7-chloroaverantin (4), 1′-O-methylaverantin (5), 7-hydroxy-2-(2-hydroxypropyl)-5-pentylchromone (6), homovalencic acid (8), and bekeleylactone E (10), were isolated from two fungal strains. The structures of 1–10 were identified by detailed analysis and comparison of their spectroscopic data with literature values. Compounds 9 and 10 showed moderate cytotoxic activity against a panel of cancer cell lines (PC-3, HCT-15, MDA-MB-231, ACHN, NCI-H23, NUGC-3), with the GI50 values ranging from 1.1 µM to 3.6 µM, whereas 1 displayed a weak 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging activity without cytotoxicity against all tested cell lines.
