Abstract:
Whether left ventricular (LV) contractility changes during ventilation with positive end-expiratory pressure (PEEP) remains controversial. To assess LV inotropic state during PEEP using a load-independent index, we generated end-systolic pressure-volume relationships (ESPVRs) in eight closed-chest, chronically instrumented, anesthetized dogs undergoing 0 [zero end-expiratory pressure for the 1st time (ZEEP1)], 5 (PEEP-5), 10 (PEEP-10), and again 0 (ZEEP2) cmH2O PEEP. LV volume was calculated from three orthogonal internal diameters (sonomicrometry), and LV pressure was measured using an implanted transducer. ESPVRs at each level of PEEP were generated by transient inflation of a vena caval occluder. Despite significant decreases in cardiac output with PEEP-5 (1.81 +/- 0.38 l/min, means +/- SE; P less than 0.05) and PEEP-10 (1.70 +/- 0.46; P less than 0.01) with respect to ZEEP1 (2.12 +/- 0.41), no change was found in the slope (ZEEP1: 6.99 +/- 1.03 mmHg/ml; PEEP-5: 7.48 +/- 1.20; PEEP-10: 7.17 +/- 1.02; ZEEP2: 7.38 +/- 1.02), the volume intercept (ZEEP1: 7.4 +/- 3.4 ml; PEEP-5: 6.6 +/- 3.0; PEEP-10: 7.2 +/- 4.0; ZEEP2: 6.6 +/- 3.6), or the new index area beneath the ESPVR (ZEEP1: 304 +/- 98; PEEP-5: 329 +/- 104; PEEP-10: 310 +/- 98; ZEEP2: 343 +/- 114). We conclude that these levels of PEEP do not affect LV contractility as assessed by the ESPVR.

In this complex, Sb-O-Sb is quasi-linear, 173·1(3)°, in contrast to the previously recorded benzene solvate, in which it is 139·0(3)°; in the nitrate, [(O2NO)Ph3SbOSbPh3(ONO2)], triclinic, P-1, Z = 2 f.u., a 15·609(5), b 13·238(4), c 10·140(2) Å, α 87·11(2), β 88·46(7), γ 72·93(2)°, R 0·036 for No 5275, it is also bent (137·0(2)°). Abstract:
Background:Exposure to severe or chronic life stressors may alter immune function and high levels of subsequent distress have been implicated in autoimmune disease pathogenesis. Post-traumatic stress disorder (PTSD) is a debilitating psychiatric condition affecting 1-12% of the general population1, occurring in response to traumatic events. Growing evidence supports an association between trauma exposure and PTSD with systemic lupus erythematosus (SLE) onset2.Objectives:To cross-sectionally assess PTSD prevalence in a cohort of patients with SLE and to examine its correlation with quality of life.Methods:A 189-item anonymous questionnaire including demographics, disease features, the 9-domain Trauma and Loss Spectrum – Self Report (TALS-SR) and the 8-domain Lupus Quality of Life (Lupus QoL) was administered via web to a cohort of patients with SLE. Patients were classified as PTSD cases based on TALS-SR items corresponding to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria for PTSD.Results:Ninety-nine (95% female and 5% male) patients with a median follow-up of 16.5 years completed the questionnaire. Self-reported fatigue prevalence was 75%. Fifteen patients (15%) reportedly were on psychological and/or psychiatric support. Thirty-one patients (31%) met the DSM-5 criteria for PTSD. The average LupusQoL interdomain score was 80/100. PTSD cases reported significantly lower scores compared to non-cases in three LupusQoL domains: planning (83 vs. 100, p=0.035), body image (85 vs. 95, p=0.031), and fatigue (67 vs. 92, p=0.001). An inverse correlation between TALS-SR scores and Lupus QoL subscales was found (Table1). In particular, the degree of stress secondary to losses or upsetting events was strongly correlated to fatigue intensity (rho= -0.458, p<0.001).Conclusion:PTSD prevalence might be higher in SLE than in the general population and have a detrimental influence on quality of life. Fatigue perception might be more significantly affected by PTSD. Abstract:
Background and Purpose: A variety of definitions for minor stroke have been proposed. We aimed to compare the clinical characteristics and outcomes of minor stroke defined as the National Institutes of Health Stroke Scale (NIHSS) score ≤5 versus ≤3. Methods: We retrieved acute ischemic stroke patients with NIHSS score ≤5 in the CSCA study (China Stroke Center Alliance) between August 2015 and 2019. In-hospital clinical outcomes including all-cause mortality, stroke, and myocardial infarction were compared between the NIHSS score ≤5 and NIHSS score ≤3 groups using absolute standardized differences (ASD). Results: A total of 1 006 798 patients were registered in the CSCA program from 1476 hospitals, 472 352 patients had NIHSS score ≤5, of whom 356 314 patients had NIHSS score ≤3. The in-hospital composite events of death, myocardial infarction, or recurrent stroke were not significantly different between the NIHSS score ≤5 and NIHSS score ≤3 groups (5.6% [26 346/472 352] versus 5.2% [18 682/356 314]; ASD, 1.8). The in-hospital all-cause mortality (0.1% [443/472 352] versus 0.1% [255/356 314]; ASD, <0.01), recurrent ischemic stroke (5.3% [25 026/472 352] versus 5.0% [17 777/356 314]; ASD, 1.4), and hemorrhagic stroke (0.5% [2151/472 352] versus 0.4% [1475/356 314]; ASD, 1.5) were not significantly different between both the NIHSS score ≤5 and NIHSS score ≤3 groups. Conclusions: Our large-scale study identified that minor stroke using NIHSS scores ≤5 and ≤3 as the definition was comparable with each other regarding in-hospital all-cause mortality, recurrent stroke, and hemorrhagic stroke. This observation may be useful for future comparison studies and clinical trial design.

Abstract:
ABSTRACT Background: Cardiac troponin elevation portends a worse prognosis in diverse patient populations. The significance of troponin elevation in patients discharged from emergency departments (EDs)without inpatient admission is notwell known. Methods: Patients without a diagnosis of acute coronary syndrome discharged fromtwo EDs between April 1, 2006, and December 31, 2007, with an abnormal cardiac troponin (troponin positive [TP]) were compared to a troponin-negative (TN) cohort matched for age, sex, and primary discharge diagnosis. Outcomes were obtained by linking with a regional ED and a provincial vital statistics database and adjusted for the following: estimated glomerular filtration rate, do-not-resuscitate status, history of coronary artery disease, Canadian Triage and Acuity Scale, and left ventricular hypertrophy on electrocardiography. The primary outcome was a composite of death or admission to hospital within 1 year. Results: Our total cohort (n 5 344) consisted of 172 TP and 172 TN patients. In the univariate analysis, TP patients had a higher rate of the primary outcome (OR 3.2, 95% CI 2.1–5.0, p &lt; 0.001) and both of its components (p &lt; 0.001). After adjusting for covariates, positive troponin remained an independent predictor of the primary outcome (OR 2.1, 95% CI 1.3–3.4, p 5 0.005) and inpatient admission (OR 2.0, 95% CI 1.2–3.4, p 5 0.006). There was no significant difference in death (OR 1.3, 95% CI 0.6–2.9, p 5 0.5) after adjustment. Conclusions: A positive troponin assay during ED stay in discharged patients is an independent marker for risk of subsequent admission. Our findings suggest that the prognostic power of an abnormal troponin extends to patients discharged from the ED.

Abstract:
Explosive-type strength training may alter kinetics and neuromuscular activity during stair ascent in elderly women. This may improve functional ability. Nineteen women (69.7 ± 3.4 yr) were randomly allocated to strength training (TG; twice per wk, 12 wk) or a control group (CG). Stair ascent was assessed at self-chosen (AFV), standardized (ASV), and maximal velocity (AMV) pre- and posttraining. Ground-reaction force (GRF) and EMG quantified kinetics and neuromuscular activity. After training, TG increased AMV and AFV velocity by 8% (p= .02) and 17% (p= .007), respectively (TG vs.CG; p< .05). This was accompanied by elevated rectus femoris EMG (from 21% to 48%,p< .047). At AFV, TG increased GRF first peak force 4% (p= .047), and CG increased second peak force 5% (p= .036). Muscle coactivation remained unaltered in both groups. Explosive-type strength training led to enhanced stair-climbing performance at maximal and self-chosen speed, reflecting an improved functional ability.

Moreover, some theoretical prerequisites and technological processes have been developed, the necessary technological equipment has been designed and made to provide the efficiency increase and the required quality of the surfaces under treatment.

Neste sentido, o presente trabalho de investigação pretende fazer uma reflexão em que medida as atividades de animação sociocultural contribuem para que o idoso se sinta mais motivado e feliz. Foram utilizados instrumentos de recolha de dados aos utentes que se encontravam numa das três respostas sociais qua a Santa Casa da Misericórdia de Valpaços disponibiliza nomeadamente apoio domiciliário, centro de dia e institucionalização do idoso. No presente estudo fizemos uma revisão no qual se fez o enquadramento teórico com uma breve referência á problemática do envelhecimento, ao papel que representa a animação sociocultural na terceira idade e uma descrição do contexto espacial da pesquisa. Seguidamente procedemos ao levantamento da problemática e á definição das finalidades do estudo. Esta investigação permitiu constatar que a existência de um programa de animação sociocultural rigorosamente estruturado e com possibilidade de atividades frequentes e adaptadas aos diversos contextos-institucional ou domiciliário-permite obter resultados positivos junto da população idosa.
This master’s thesis, is developed in the 2nd cycle of studies, leading to the Degree of Master of Education Science- specialization in Socio-cultural, held at the University of Trás-os-Montes, school of Humanities and Social Sciences, represents a study about the role of animation in the third age in improving the psychological indexes of well-being, motivation and happiness of the users of the study. In this way the presente investigation pretends to do a reflexion in the sociocultural activities contribues to the motivation and felicity of the eldery people. In this sense, this research aims to reflect on the extent to which socio-cultural activities contribute to the elderly feeling more motivated and happy. Data collection instruments were used for the users who were in one of the three social responses that the Santa Casa da Misericórdia de Valpaços provides, namely, home support, day care center and institutionalization of the elderly poplulation. In the present study we did a review in which the theoretical framework was made with a brief reference to the problematic of aging, the role that sociocultural animation represents in the third age and a description of the spatial context of the research. Then we proceed to the survey of the problem and the definition of the purposes of the study. This research showed that the existence of a program of socio-cultural animation rigorously structured and with the possibility of frequent activities and adapted to the different contexts-institutional or domiciliary-allows to obtain positive results among the elderly population.

No decorrer do Mestrado em Educação Pré-Escolar e Ensino do 1º Ciclo do Ensino Básico (1º CEB), foram realizados dois estágios: o estágio I em Educação Pré-Escolar e o estágio II no 1º CEB ambos numa Instituição Pública. Primeiramente, na parte I do relatório, caraterizamos o contexto, tanto do meio envolvente como de ambas as instituições onde ocorreram os estágios. A parte II aborda o tema por mim escolhido “Os contos de fadas: um contributo para o desenvolvimento da criança”. Optei pela escolha deste tema com base nas atividades que desenvolvi ao longo dos dois estágios, principalmente ao longo do estágio I no Pré-Escolar, pois verifiquei que o grupo prendia bastante a sua atenção às histórias que lhes contava e daí a minha curiosidade em tentar perceber melhor o papel dessas histórias no desenvolvimento das mesmas. Por último a parte III destina-se à atividade educativa, onde faço referência à gestão do tempo, à planificação e à avaliação, articulando sempre com o trabalho desenvolvido ao longo dos estágios.
During the masters of pre-primary and primary education, I did two internships: the first was in pre-primary education and the second was in primary education. Both were developed at public institutions. In the first part of the report I described the context, both background and the institutions, where the internships took place. The second part was about the subject that I chose for myself “ Fairy tales: an advantage for child development”. I chose this subject, mainly, because of the activities developed during the internships. Especially, when I noticed kids attention towards stories, that I told them. That was the moment that I knew, I wanted to do a research about the influence of those stories and tales in child development. For last, I wrote about educational activities. I refered about time management, planning and evaluation and always minding the work I had done in the internships.

Esta Dissertação tem como foco principal entender o grau de participação das crianças e jovens que se encontram acolhidas em Lar de Infância e Juventude, no que diz respeito às decisões sobre a sua própria vida. Estas crianças/jovens, tal como qualquer outra, são influenciadas pelas vicissitudes inerentes à Infância contemporânea, e provêm de famílias também elas inseridas na designada ‘sociedade de risco’, estando imersas nas ambiguidades de uma sociedade globalizada e caraterizada pelo risco e pela incerteza. Além disso, tais crianças/jovens foram retiradas das suas famílias de origem e passaram a viver num ambiente institucional estranho. É perante este contexto certamente traumático que os técnicos sociais da Instituição têm que delinear o seu ‘Projeto de Vida’ e o respetivo ‘Plano Socioeducativo Individual’. Assim, pretendemos clarificar se elas são realmente escutadas, ou seja, se o ‘Eu’ é colocado no centro do processo. Sabemos que é plausível serem ouvidas mas não escutadas, o que nos remete para a necessidade da participação deste público naquilo que é a sua própria vida. Este é um tema que, embora também seja assunto de interesse para outras áreas de estudo, no caso do Serviço Social a pertinência coloca-se na possibilidade de melhorar os processos de intervenção junto destas crianças/jovens que se encontram em Instituição. Para atingir tal objetivo, foi utilizado o Método Biográfico nesta pesquisa, por entendermos ser aquele que melhor pode captar as subjetividades, as singularidades e as construções de sentido de cada indivíduo. Com este propósito, foram utilizadas Entrevistas semidirectas, ‘Roteiros de Vida’ e a análise documental. Para interpretar os resultados desta investigação, foi tido em conta o Modelo Ecossistémico e a Teoria da Vinculação, por entendermos que apresentam fatores explicativos determinantes. Também procurámos compreender esses resultados com o que designámos de ‘Tese da Identificação e Desidentificação’, numa tentativa de dar voz às implicadas. Atendendo aos resultados e ao respetivo quadro explicativo, apresentámos alguns contributos para a intervenção junto das crianças e jovens que se encontram em contexto institucional, com o objetivo de colocar o ‘Eu’ no centro do processo e de melhorar a escuta ativa, trazendo também para o centro da intervenção o acompanhamento às famílias de origem.
This dissertation has as its main focus to understand the degree of participation of children and young people who are living in Home to Children and Youth, with regard to decisions about her own life. These children/youth, like any other, are influenced by the vicissitudes inherent in the contemporary Childhood and come from families too they inserted in designated ‘risk society’, being immersed in the ambiguities of a globalized society and characterized by risk and by uncertainty. In addition, such children/young people were removed from their families of origin and lived in a strange institutional environment. It is in this context certainly traumatic that the social technicians of the institution have to delineate his 'Life Project' and the respective ‘Individual Social and Educational Plan'. So, we want to clarify if they are really heard, i.e., if the 'I' is placed in the center of the process. We know that it is plausible to be heard but not listened to, which brings us to the need of participation of this public in what is his own life. This is a subject which, although it is also subject of interest to other fields of study, in the case of the Social Work the relevance arises in the possibility of improving the intervention processes along these children/young people who find themselves in an Institution. To achieve this goal, we used the Biographical Method in this research, by understanding being the one who can best capture the subjectivities, the singularities and the construction of meaning of each individual. With this purpose, indirect Interviews were used, ‘Life Scripts’ and documentary analysis. To interpret the results of this investigation, has been taken into account the Ecosystem Model and Binding Theory, by understanding that present explanatory factors determinants. We also understand these results with what we term ‘Thesis of Identifying and Misidentifying’, in an attempt to give voice to those involved. In the light of the results and the respective explanatory framework, we have made some contributions to the intervention with children and young people who are in institutional context, with the aim of putting the ‘I’ in the centre of the process and to improve active listening, bringing also to the centre of the intervention the follow-up for the origin families.

Ambicionou-se estudar os procedimentos e as técnicas que o Animador Sociocultural e o Gerontólogo podem e devem adotar num Centro de Dia. Para tal, através da aplicação de inquéritos por entrevista aos Utentes do contexto indicado, e a Profissionais da área, pretendemos identificar que tipo de atividades se podem desenvolver, para além de conhecermos as Histórias de Vida do público-alvo, de forma a realizamos um Projeto de Intervenção, que vá de encontro às necessidades e gostos dos Utentes. Verificou-se que cada público-alvo deve ser entendido de uma determinada forma, não procurando implementar as mesmas atividades em todas as pessoas do contexto, uma vez que possuem gostos e necessidades diferentes, devendo ter em conta todas as vivências e desejos que cada uma delas possui. É do conhecimento geral que o envelhecimento da população é um fenómeno global. A faixa etária das pessoas com mais de 65 anos é a que tem atualmente o maior crescimento. Apresentam-se vários desafios que são consequência desse crescimento, incluindo assim os Centros de Dia, que levam a que muitas pessoas recorram a eles. Os Centros de Dia assumem variadas funções para com o seu público-alvo, incluindo a Animação Sociocultural, que os ajuda a adaptarem-se, procurando estabelecer as suas prioridades e proporcionar-lhes atividades que visem corresponder aos seus interesses, promovendo-lhes um envelhecimento com qualidade. Neste trabalho, a afetividade e o sentimento são constantes, uma vez que já existia uma ligação e confiança com os Utentes do contexto. Realizamos o Enquadramento Teórico, apresentamos a Metodologia, o Trabalho Empírico, e propomos um Projeto de Intervenção em Animação Sociocultural com a finalidade de ser aplicado no Centro de Dia da Casa do Povo de Fontelas. Posteriormente, remetemos a conclusão, a bibliografia e webgrafia, os anexos e apêndices que compõem este trabalho de investigação.
This research work has as its theme “The Sociocultural Animation in the Third Age – Intervention Project in the Day Centre of the People's House in Fontelas”. It was developed in the 2nd Cycle of Studies in Science Education – Specialization in Sociocultural Animation at the University of Trás-os-Montes e Alto Douro. We pretend to study the procedures and techniques that Sociocultural Animator and Gerontologist can and should adopt at a Day Centre. For this, by applying interview surveys to Users of the indicated context, and Professionals of the area, we intend to identify what kind of activities can be developed, in addition to knowing the Life Stories of the target audience, in order to carry out an Intervention Project, which meets the needs and tastes of the Users. It was found that each target audience should be understood in a certain way, not seeking to implement the same activities in all people of context, since they have different tastes and needs, taking into account all the experiences and desires that each them has. It is well known that the ageing of population is a global phenomenon. The age group of people over 65 is the one that currently has the highest growth. So, there are a number of challenges that are a result of this growth, including Day Centers, which lead a lot of people to use them. The Day Centers assume various functions to his target audience, including the Sociocultural Animation, which helps them to adapt, seeking to establish their priorities and provide them with activities that aim to match their interests by promoting them Ageing with quality. In this work, affection and feelings are constant, since there was already a connection and trust with the Users of the context. We carry out the Theoretical Framework, we present the Methodology, the Empirical Study, and we propose a research Project in Sociocultural Animation that can be applied in the Day Center of people’s house in Fontelas. Later, we refer to conclusion, bibliography and webgrafia, attachments and appendices that compose this research work.

Este trabalho de investigação, subordinado ao tema “Animação Sociocultural e Práticas Intergeracionais- Estudo de caso da Santa Casa da Misericórdia de Vila Flor: Lar Nossa Senhora da Lapa e Jardim de Infância Flor de Liz, pretende saber qual o contributo do animador e da animação sociocultural relativamente às práticas intergeracionais desenvolvidas nesta instituição situada em meio rural. Neste contexto, procedeu-se ao Enquadramento Teórico, onde foi revista a literatura referente ao tema em estudo. Efetuou-se também uma descrição sobre o contexto em que se desenvolveu a investigação- meio rural- recorrendo à metodologia qualitativa, através de entrevistas semiestruturadas a vários técnicos e de recolha das histórias de vida dos idosos, bem como a observação participante e a análise documental. São apresentados e discutidos os resultados, tendo em conta o cruzamento das diferentes vozes intervenientes no processo. Por fim, apontamos algumas reflexões e um plano de atividades que pode melhorar as relações intergeracionais e a promoção da educação intergeracional, pois de acordo com os dados disponíveis são raras as vezes que a instituição promove este género de práticas, mesmo possuindo técnicos especializados na área da Animação Sociocultural embora manifestem ter vontade de o fazer, no entanto, sentem necessidade de uma nova cultura interdisciplinar e interprofissional.
This investigation project, about the subject of “Sociocultural Animation and Intergenerational Practices” – a case study by the Santa Casa da Misericórdia de Vila Flor: Lar Nossa Senhora da Lapa e Jardim de Infância Flor de Liz, intends to find out what is the contribution of the animator and the sociocultural animation to the intergenerational practices developed in this institution, located in the countryside. In this context, one proceeded to the theoretical framework, reviewing the existing literature on the study subject. One has also elaborated a description about the context where this investigation was developed – the countryside – using the qualitative methodology, through semistructured interviews to several technicians and the collection of life stories of the elderly, as well as the participating observation and the documental analysis. Results are presented and discussed, considering the crossing of the different voices of those who participated in the process. Finally, we point to some reflexions and an activity plan that can improve intergenerational relations and the promotion of intergenerational education since, according to available data, seldom does the institution promote this kind of practices, although they have the specialized technicians in the area of Sociocultural Animation. They show the will to, however, they feel the need of a new interdisciplinary and interprofessional culture.

A institucionalização de crianças e jovens é uma realidade, e o meio envolvente destes menores coloca-os em situações de exclusão social, sobretudo ao nível educacional, já que grande parte da infância e juventude é passada nas instituições escolares. Esta investigação foi desenvolvida na Escola de Artes e Ofícios – Prof. Nuno Rodrigues e nas escolas pertencentes ao Agrupamento de Escolas Dr. Júlio Martins, frequentadas pelos menores institucionalizados, ambas na cidade de Chaves. A metodologia utilizada foi o estudo de caso, uma vez que é um processo eficaz na aproximação do estudo à realidade, permitindo identificá-la, conhecê-la e compreendê-la, num curto espaço de tempo e com escassez de recursos. Tendo em conta a situação de exclusão social e os relacionamentos interpessoais das crianças e jovens, procurou-se perceber de que forma é que a animação sociocultural pode contribuir para minimizar a exposição negativa destes jovens, colaborando para a sua formação pessoal, educacional e cognitiva. Este estudo foi realizado tendo em conta as necessidades e expetativas das crianças e jovens, aos quais se recorreu para manifestarem a sua opinião, permitindo uma maior fidedignidade do estudo apresentado. Os dados mostram que a exclusão acontece em algumas situações - tanto parte do exterior como de posturas adotadas pelos crianças e jovens, que acabam por conduzir a este fenómeno. Apurou-se que a ASC pode desenvolver um trabalho importante junto desta comunidade, contribuindo para a sua integração.
The institutionalization of children and teenagers is a constant reality, and their environment can put them in situations of social exclusion, particularly in terms of education, since much of childhood and youth is spent in schools. This research was developed in the School of Arts and Crafts - Prof. Nuno Rodrigues and schools belonging to the Group of Schools Dr. Júlio Martins, frequented by minors institutionalized both in Chaves. The methodology used was the case study, since it is an effective approach in the study of reality, allowing to identify it, recognize it and understand it in a short time with limited resources. Based to the situation of social exclusion and interpersonal relationships of children and young people sought to understand how it is that the sociocultural can help to minimize the negative exposure of these young people, contributing to their personal, educational and cognitive. This study was always performed taking into account the requirements and needs of children and young people, to whom it appealed to express your opinion and allow, therefore, for greater reliability of the study presented. The results show that the exclusion is expressed in some situations, and that both as part of the exterior due to postures adopted by minors, which ultimately lead to this phenomenon. It was found that the ASC can develop important work with this community, contributing to their integration.

Nas últimas décadas, o aumento da longevidade ficou a dever-se essencialmente aos progressos das ciências da saúde e à melhoria das condições sociais, deixando prever um cenário demográfico que requer respostas integradas capazes de incorporar as especificidades do envelhecimento e de garantir a sua expressão mais sublime – envelhecer com qualidade e dignidade. Apesar da forte matriz individual que caracteriza o envelhecimento, torna-se premente conhecer a realidade do problema no que concerne à qualidade de vida percebida pelo idoso. No contexto deste estudo, a compreensão do conceito de qualidade de vida do idoso institucionalizado configura-se como uma experiência fenomenológica importante para uma participação mais pró-ativa na prestação de cuidados de enfermagem a este grupo populacional. O presente estudo, de carácter empírico não experimental, inscrito num paradigma quantitativo, pretende conhecer a perceção sobre a qualidade de vida dos idosos institucionalizados no concelho de Vila Real. Fazem parte da amostra 126 idosos, com uma média de idades de 81 anos, sendo a maioria (63,5%) viúva. Relativamente à escolaridade dos idosos, 47,5% não frequentou o ensino formal ou não completou o 1º ciclo do ensino básico. No que concerne à institucionalização, 58,7% mencionou que foi por iniciativa própria, enquanto 32,5% referiu terem sido os filhos a indicar ser mais conveniente a sua ida para um lar. Referiram como motivos mais preponderantes para a institucionalização o facto de se sentirem sós (45,2%), seguido da pouca disponibilidade de familiares para atender as suas necessidades (30,2%). O tempo de permanência na instituição é em média de 4,8 anos. A participação em atividades de lazer organizadas pelo lar, foi referida “às vezes” por 54,8% dos inquiridos. Relativamente ao nível de dependência para a realização de Atividades Básicas de Vida Diária, 29,4% dos idosos são completamente independentes em todas as atividades, e 54% apresenta uma “dependência leve”. No que diz respeito à perceção da Qualidade de Vida, os dados permitem constatar que a Qualidade de Vida Global e o domínio psicológico apresentam uma média mais elevada (60,0) e o domínio ambiental apresenta a média mais baixa (55,8). Os resultados deste estudo permitem dizer que os idosos deveriam ser motivados para a procura de interesses próprios e promotores do seu envolvimento pessoal e, por outro lado, as instituições deveriam fomentar a adesão dos idosos em atividades diferentes das habituais.
In the last decades, the increase of longevity has been due, essentially, to the progress in medicine and to the improvement of the social conditions, thus anticipating a demographic scenario that requires integrated answers capable of incorporating the specificities of ageing and assuring its most sublime expression – grow old with quality and dignity. Although ageing is strongly individualistic, it is necessary to know the reality of the problem regarding the quality of life perceived by the elder. In the context of this study, understanding the elder´s concept of quality of life is seen as an important phenomenological experience in a more proactive participation in providing nursing care to this group. It is intended with this study, empirical non experimental, to have the perception of life quality of the institutionalized elder in the municipality of Vila Real. 126 elder compose the sample, with an average of 81 years old, being the majority (63.5%) widowed. As for schooling, 47.5% of the elder did not attend or complete elementary school. Regarding institutionalization, 58.7% of the elder mentioned it was his/her own initiative, while 32.5% referred it was his/her children to indicate the convenience of an old people’s home. 45.2% of the elder referred as the most decisive reasons for institutionalization the fact of feeling lonely, followed by 30,2% who referred little availability of relatives to suit their needs. The time spent in the institution is in average 4.8 years. The participation in leisure activities organized by the old people’s home was referred “sometimes” by 54.8% of the interviewed. As for the level of dependence for doing Everyday Basic Activities, 29.4% of the elders are totally independent in all activities and 54% present a “slight dependence”. Concerning the perception of Life Quality, the data allow verifying that the Global Life Quality and the psychological field present a higher average, both with 60.0 and the environmental field presents the lowest average (55.8). The results of this study allow us to conclude that the elder should be motivated to search for their own interests and promote their own personal involvement and, on the other hand, the institutions should foment the attendance of the elders in activities different from the usual.

В работе представлен сравнительный анализ данных «Российского мониторинга экономического положения и здоровья населения НИУ ВШЭ» (РМЭЗ НИУ ВШЭ) за 1994–2021 гг. Данные РМЭЗ НИУ ВШЭ свидетельствуют о том, что в 2021 г. наблюдался рост реальных доходов населения. После сокращения почти на треть в 2020 г. средние реальные доходы домохозяйств выросли в 2021 г. на 7,7%. При этом рост доходов от государственных трансфертных платежей составил 14%, в то время как доходы домохозяйств от заработной платы увеличились лишь на 3,9%. Снизилось неравенство домохозяйств по доходам. Если в 2019 и 2020 гг. средний ежемесячный доход наиболее состоятельных домохозяйств превышал доход наименее состоятельных в 2,6 раза, то в 2021 г. доход наиболее состоятельных был выше в 2,3 раза. После сокращения в 2020 г. на 30% ежемесячные расходы домохозяйств на продукты питания и непродовольственные товары выросли в 2021 г. на 3,9%. Разрыв по уровню расходов между состоятельными и малообеспеченными домохозяйствами сократился с 5,1 до 5 раз. Уровень безработицы среди экономически активного населения снизился с 3,4% в 2020 г. до 3,1% в 2021 г. Коэффициент участия в рабочей силе, напротив, вырос с 80,9 до 81,5%, вернувшись к значениям 2019 г. Доля работников, столкнувшихся с задолженностью по заработной плате, сократилась и достигла минимума за все годы наблюдений (0,6%). Продолжила расти доля пенсионеров, столкнувшихся с задолженностью по пенсионным выплатам. Доля таких пенсионеров в 2021 г. составила 8,2% среди мужчин и 6,7% среди женщин. В 2021 г. россияне были в целом удовлетворены жизнью (50,6% удовлетворенных), но не своим материальным положением (55,2% неудовлетворенных). Продолжала ухудшаться субъективная оценка материальных и финансовых перспектив домохозяйств. Напротив, немного улучшилась оценка россиянами своего положения на рынке труда (стабильности текущего трудоустройства и перспектив найти новую работу). По сравнению с 2020 г. снизилась доля увольнений по причине закрытия предприятий и выхода на пенсию, чаще причинами увольнения стали ухудшение здоровья и продолжение образования.

Objetivo: Averiguar a relevância da gestão da sala de aula, nos cursos de formação continuada, na visão dos professores regentes da sala da Rede Municipal de Educação de Jacareí – SP. Métodos: Foi um estudo de caso com abordagem mista, de natureza aplicada realizado de maneira exploratória e descritiva que aplicou questionários com perguntas abertas e fechadas, a 96 docentes de Pedagogia (85 experientes e 11 profissionais inexperientes) atuantes no Ensino Fundamental I, nas 29 escolas da Secretaria Municipal de Educação de Jacareí – SP; aprovado pela Comissão Nacional de Ética em Pesquisa (CONEP) com o Parecer N° 5.634.016. Resultados: Questionados sobre suas participações anuais em Palestras, Oficinas, Seminários, Congressos tendo a gestão da sala de aula como tema, os resultados mostraram que dos 11 inexperientes, 01 (9%) respondeu nenhuma, 05 (46%) disseram ter feito aperfeiçoamento, 02 (18%) participaram de oficinas, 03 (27%) palestras. Dos 85 experientes, 04 (5%) responderam não ter participado de nenhuma, 21 (25%) fizeram aperfeiçoamento, 24 (28%) participaram de oficinas, 29 (34%) de palestras, 05 (6%) de seminários e 02 (2%) congressos. Conclusão: O conhecimento específico e prático de gestão da sala de aula na formação continuada de professores das escolas municipais de Jacareí-SP revelou-se insuficientes para gerir uma sala de aula.

Abstract:
The Philippine Department of Environment and Natural Resources (DENR) has issued Department Order 2015-04 (DAO 2015-04) or the “The Implementation of Vehicle Emission Limits for EURO 4/IV, and In-Use Vehicle Emission Standards”. This policy, coupled with the Philippine Biofuels Act of 2016 (RA 9367) should greatly reduce the environmental impact of local automobile emissions. Commercial fuel is a mix of local coconut methyl ester (CME) and neat diesel blend. However, commercial diesel fuel is still at 2% v/v as of 2016 in contrast to the 5% v/v set by the policy, part due to the fact that only few local studies are done to show the effects of higher percentage of CME, with most recent studies showing results against increased CME usage. The study shows the effects of the usage of higher CME percentage in accordance to the set schedule of RA 9367. Five fuel blends with varying CME percentages v/v (2%, 5%, 10%, 15%, and 20%) are used in a heavy duty turbocharged common rail direct injection (CRDI) engine. The engine is run on an eddy current dynamometer with steady state measurements at 50 to 500Nm with 50Nm intervals. Each fuel is tested at three pedal positions, alpha, (25%, 50%, and 60%) controlled directly from the engine control unit (ECU). Results show that higher CME usage does not result in reduction of power and achieved torque. There is, however, a significant increase in brake specific fuel consumption at higher percentages of CME. No significant change in carbon monoxide (CO) and unburned hydrocarbons of diesel (HCD) is observed but there is a significant increase in Nitrous Oxides (NOx) concentration as CME percentage is increased. However, brake specific CO and HCD were found to be out of standard for near no load and near full load operations. A similar trend is observed for brake specific. Brake specific NOx is particularly more problematic since all measurements were observed to be out of standard with maximum values of 1350% of the set EURO 4/IV standard. However, it is also seen that the performance of each blend does not significantly differ from each other. Nevertheless, there is still some potential in the usage of CME due to the fact that power and torque requirements are still met at the expense of higher fuel consumption; but with the opportunity of being self-sufficient as coconut-producing country.

Abstract:
As wavelength decreases and numerical aperture (N.A.) increases, the bandwidth of available excimer lasers can most easily be matched using lenses that are catadioptric. Beam splitters allow such lenses to maintain high N.A. without obstructing the pupil or compromising the field size. We present a compact 0.6-N.A. 248-nm design with 30-mm field diameter. The most challenging element in this system is the 170-mm SiO2 cube beamsplitter. The dichroic beam splitter coating supports alignment at 364 nm, and operates in converging light over a 36° range without introducing aberration, apodization or image displacement. The cube is a good thermal insulator, and any index gradients arising from residual absorption in the prisms or coating will be traversed in a nonrotationally symmetric way, due to the folded path. Thermal modeling and gradient-index ray-tracing suggest an absorption tolerance of 1 % for the coating and 10% m-1 for the SiO2. We have obtained material meeting these tolerances. The beam splitter prisms can be strategically cut to minimize aberrations arising from residual SiO2 index inhomogeneities, which amount to 2 ppm in the best material we have obtained. An initial study suggests that optimal positioning of the cut surfaces can meet λ/5 wavefront and 20-nm distortion tolerances in a half-scale cube. Further correction can be obtained with external compensation. Other beam splitter-related issues are vulnerability to stray light in the presence of reflective surfaces at mask, wafer, beam splitter and mirror, and the effect of polarizing beam splitter coatings of lithographic imaging.

Abstract:
Wildland firefighters (WFF) face a set of specific work-related factors that directly affect their physical and cognitive abilities and compromise their health and safety. The working conditions include hard physical work and environmental conditions that combine high temperatures and high radiant heat. Such environments make using personal protective equipment (PPE) mandatory to protect them from risks. This fact restricts heat removal and adds extra weight, increasing thermal strain and the risk of heat-related illnesses on WFF. Since the number of females WFF has increased, it is necessary to study the repercussions of heat stress on this group. To date, it is not yet well-known whether sex-related differences in thermoregulation will be relevant when the individuals are wearing PPE and performing high physical effort in a hot environment. Therefore, we aimed to investigate the physiological response when performing moderate to high-intensity effort in a hot-dry environment while wearing PPE according to sex. Twenty WFF 10 females [23.9 ± 3.2 yr, 163.8 ± 3.4 cm and 62.7 ± 9.1 kg] and 10 males [31.9 ± 6.6 yr, 178.8 ± 5.8 cm and 73.9 ± 7.7 kg]) performed a 125 min treadmill test in a controlled ambient (30 ºC and 30% relative humidity). The protocol consisted of two exercise stages where WFF performed different continuous and variable exercise bouts in order to mimic the effort performed during real deployments. Participants wore the full standard PPE during the test. Oxygen uptake (VO2), heart rate (HR), core temperature (CT) and chest temperature (SkT) were monitored throughout the test. HR and CT were used to calculate the physiological strain index (PSI). Differences in body mass pre-post trials corrected for fluid intake were used to calculate sweat production (SwP), sweating rate (SwR), and evaporative efficiency (EE). Differences (p < 0.05) between females and males were found in %VO2max (62.5 ± 7.4 vs 55.3 ± 5.), HR (155 ± 10 vs 134 ± 14 beats·min–1), % of maximal HR (81.3 ± 3.5 vs 42.3 ± 6.5), CT (38.0 ± 10 vs 37.7 ± 0.33 ºC), SkT (36.0 ± 0.6 vs 35.3 ± 0.6 ºC) and PSI (4.1 ± 0.5 vs 3.5 ± 0.6). Even though SwR was higher (p < 0.05) for male participants (1001.5 ± 268.3 ml) compared to females (647.5 ± 145.9 ml), females had higher EE (32.9 ± 4.6 vs 16.7 ± 6.2 %). In conclusion, performing high-intensity exercise in hot-dry conditions while wearing PPE leads to a higher thermal and cardiovascular load for female WFF, making them more susceptible to heat illness. These results could be linked to lower aerobic fitness, sweating rate, and hormonal aspects that increased the thermal burden.

Abstract:
Introdução: a doença renal crônica (DRC) consiste na perda progressiva e irreversível da função dos rins e é um problema de saúde pública em todo o mundo. Apesar de silenciosa, a DRC é uma doença que pode ser rastreada e com isso tratada precocemente. A Sociedade Brasileira de Nefrologia (SBN) organiza o Dia Mundial do Rim (DMR) no Brasil, na segunda quinta-feira de março todos os anos. Em 2020 o tema escolhido foi ‘’Saúde dos Rins Para Todos’’. Nesta campanha, foram realizadas atividades de triagem dos pacientes com fatores de risco para DRC e orientações para um estilo de vida mais saudável. Objetivos: descrever a presença de fatores de risco para DRC em voluntários participantes da campanha realizada no DMR, e relacionar esses fatores de risco com a presença de possíveis doenças que predispõem a DRC. Materiais e Métodos: foi realizado um mutirão pela Liga de Nefrologia da UNINOVE no ambulatório da Vergueiro no DMR de 2020, antes das medidas de quarentena da pandemia de COVID-19, com a participação de alunos dos cursos de medicina e enfermagem. Os atendimentos ocorreram durante dois turnos de uma hora e meia. Foram realizadas medidas antropométricas, aferição de PA, glicemia capilar, análise da urina e avaliação da acuidade visual. Foram preenchidos formulários sobre hábitos de vida, antecedentes pessoais e familiares. Em seguida foi feita a avaliação dos resultados e orientação em relação aos fatores de risco identificados. As alterações foram detectadas e encaminhadas para o ambulatório da Uninove. Os resultados foram descritos em média e porcentagem. Resultados: foram atendidos 60 voluntários, 51,6% eram brancos; 9,7% pretos; 37,1% pardos e 1,6% eram amarelos. 61,7% do sexo feminino e 38,3% masculino. Tinham em média: IMC de 26,7±5,8, circunferência abdominal 88,9±16 cm; PAS 116±12,7 mmHg; PAD 78,9±12,2 mmHg; FC 70±9,1 bpm e glicemia capilar 96,1±34,4mg/dl. Entre os fatores de risco: 13,6% eram hipertensos (HA), 10% diabéticos (DM), 6,8% doença cardiovascular (DCV), 5% usavam AINES. Tinham história prévia de litíase renal: 21,7%; infecção urinária: 21,7%; e nenhum paciente referiu ter doença renal. Hábitos de vida: 11,7% tabagistas, 15,3% etilistas e 60% sedentários. Antecedentes familiares: 65% referiram HA; 51,7% DCV; 57,6% DM; 15,3% doença renal e 22,4% litíase renal. Avaliação oftalmológica: 25,4% tiveram redução da acuidade visual. Na aferição de PA: 13,6% estavam hipertensos. Na avaliação da Glicemia Capilar: 34,4% tiveram valores maiores que 140 mg/l Análise da urina: 16,1% hematúria, 3,6% proteinúria e 0,6% glicosúria. Conclusão: nessa campanha, foram identificados fatores de risco para a DRC como HA e DM. A partir dos atendimentos, foi realizado o encaminhamento e o pedido de exames como ureia e creatinina para os pacientes que demonstraram necessidade, a fim de proporcionar o rastreio da doença renal no ambulatório e evitar complicações futuras. Abstract:
Metaplastic breast carcinoma (MpBC) is a rare and morphologically diverse group of tumors in which a variable proportion or the entire tumor is composed of nonglandular epithelium or mesenchymal cells. It is defined by the histological presence of at least two cellular types, typically epithelial and mesenchymal components. It is composed of ductal, squamous, and/or chondroid, and spindle elements, with squamous cell carcinoma being the most frequent histological subtype. MpBC represents 0.2%–5% of all breast cancers and it is very aggressive. This type of breast cancer is typically triplenegative and is therefore not targetable with hormone therapy or anti-HER2 therapies, leaving only chemotherapeutics for management. MpBCs are known for their aggressive course and poor response to chemotherapy. PDL1/PD1 expression is a predictor of the effectiveness of immune checkpoint therapy in breast cancer. Finally, there are currently no standardized treatment guidelines specifically for MpBC2. A 42-year-old female patient, lactating, who had her only pregnancy at age 40, visited a Mastology Clinic on July 16, 2019, complaining of huge left breast pain. She did not know about her family background, as she was adopted. On physical examination, she had lactating breasts and two palpable lumps of hard consistency, contiguous, and mobile in the upper outer quadrant of the left breast, measuring 3 and 2.5 cm. Mammography described dense breasts, with no other changes and breast ultrasound revealed two solid nodules, measuring 2.7 and 0.6 cm, and a simple cyst measuring 3.4 cm, all of which were contiguous in the upper outer quadrant of the left breast — BIRADS 4. A fine-needle aspiration puncture was performed in the simple cyst, with a histopathological result of poorly differentiated malignant neoplasm with pleomorphic focus, and a core-needle biopsy, with histopathological result of breast tissue infiltrated by pleomorphic malignant neoplasm. The immunohistochemical analysis showed positive for pan cytokeratin AE1/AE3 and negative for CD45, S100, myogenin, and myodio; bringing the conclusion of poorly differentiated carcinoma, suggestive of MpBC. She received neoadjuvant chemotherapy, with doxorubicin + cyclophosphamide, but had rapid local tumor progression. A new ultrasound revealed a heterogeneous and partially delimited mass, measuring 8.8×6.1 cm — BIRADS 6. The patient underwent a left total mastectomy and axillary lymph node dissection on September 23, 2019 — without breast reconstruction, and confirmed invasive metaplastic carcinoma with chondroid-type mesenchymal differentiation, measuring 7 cm, histological grade III, nuclear grade III, associated with solid and cribriform ductal carcinoma in situ, with comedonecrosis — grade III; free surgical margins, but with axillary lymph node metastasis (8/20). The immunohistochemical analysis of the surgical specimen revealed a triple-negative carcinoma: estrogen and progesterone receptors negative, and HER2 negative. The patient had a good postoperative recovery and received radiotherapy (50 Gy). Thereafter, she received adjuvant chemotherapy with capecitabine, within which she evolved with axillary, supraclavicular, and pulmonary lymph node metastasis. The PDL1 marker showed a negative result; therefore, palliative paclitaxel and bevacizumab were prescribed. The patient rapidly evolved with worsening of the lung lesions and was hospitalized on March 9, 2020, with serious dyspnea, progressing to death on March 19, 2020.

Abstract:
Background: COVID-19 had a huge impact on the wellbeing of healthcare workers (HCWs). This is well documented during the first peak of the pandemic. With cases in the UK rising for a third peak, hospitalisations and deaths surpassing the first, there is very little known about the mental health of HCWs during this time. Methods: Using a questionnaire, data was collected from patient-facing staff at Barking, Havering, and Redbridge University Trust to quantify and compare the period prevalence of symptoms of depression, anxiety, and PTSD during the first peak (P1: March-May 2020) and third peak (P3: December 2020-Feburary 2021) of the COVID-19 pandemic as well as wellbeing service use, demographics of responders and what they found most difficult during the peaks. Results: Of 158 responders, only 22·4% felt they had enough access to wellbeing services during P1 and 21·5% in P3. Of those who used wellbeing services 34·4% found them useful in P1 and 34·6% in P3. 70·3% of responders felt that not enough was done for staff wellbeing. The median anxiety score decreased from P1 (10(range 5-17)) to P3 (8(range 4-16)) p=0·031. Under 30-year-olds’ depression and PTSD scores increased from P1 to P3 (depression: P1 7(1-11), P3 8(3-14), p=0·048, PTSD: P1 4(0-7) peak 3 5(2-9), p=0·037). Several groups showed a decrease in anxiety scores from P1 to P3 including; over 30-year-olds (P1 10(5-17), P3 7(3-15) p=0·002), BME responders (P1 8(3·75-15) P3 6·5(1-12) p=0·006), AHP (P1 14(7-19), P3 11(5-19) p=0·005), ITU workers (P1 15(8-18·25) P3 12(5·75-18·25) p=0·004), and those who were redeployed (P1 8(5-18·25), P3 5(2-14·75), p=0·032). Conclusion: We have observed changes in mental health symptoms within the study population as the peaks of the pandemic continue. With the majority of responders reporting they felt not enough had been done for their wellbeing support - and of those who used the wellbeing services only around 1/3 felt they were useful - we hope that this paper can help inform wellbeing provision and identify groups at higher risk of developing mental health symptoms.

Materiales y métodos: estudio descriptivo retrospectivo. Se analizaron los indicadores de transmisibilidad, gravedad e impacto de las temporadas de circulación de VSR en Colombia durante 2013 a 2018. Se estableció la estacionalidad se calcularon umbrales de intensidad mediante el método de medias epidémicas móviles (MEM) y el método de líneas basales de la OMS. Resultados: el comportamiento del VSR en Colombia presenta dos curvas epidémicas sobrepasando el umbral estacional desde la semana epidemiológica 06 hasta la 30; la mayor intensidad se concentra entre marzo a junio. El 2015 fue el año con la mayor circulación con 2 374 casos positivos. la mayor transmisibilidad se reportó en 2015 y 2016 con 19 915 consultas en menores de 5 años; la gravedad fue baja en tres de los siete años estudiados. En cinco de los siete años el impacto se ubicó en categoría baja, con un promedio anual de 537 defunciones en menores de 5 años. Conclusiones: la estacionalidad del VSR coincide con el primer periodo de lluvia del país entre abril y junio, con un segundo pico entre octubre y noviembre; los indicadores de transmisibilidad, gravedad e impacto fueron bajos.

Abstract:
This document is a collection of available data on solid waste management in Latin America and the Caribbean (LAC). The following bullet points contain a brief description of the most relevant data: The regional average per capita generation of Domestic Solid Waste (DSW) and Municipal Solid Waste (MSW) is 0.6 kg/cap/day and 0.9 kg/cap/day, respectively. DSW represents, on average, 67% of the MSW generated in the region. The MSW regional average collection rate is 89.9% (as a percentage of the population). Compared with a worldwide average of 73.6%, LAC has a high level of coverage, reflecting the priority the region gives to this service. LAC's coverage is higher than that of Africa (46%), South Asia (65%), and the Middle East and Northern Africa (approximately 85%). Argentina, Chile, Colombia, Dominican Republic, Trinidad & Tobago, Uruguay and Venezuela have MSW collection rates close to 100% (universal coverage). About 53% of LAC's population receives 2to-5-days a week waste collection service, 45% daily service and 2% once-a-week service. Separate MSW collection, commonly known as selective collection is still uncommon in the region. However, there are cases like that of Brazil, where 62% of municipalities implement selective MSW collection programs. Average unitary collection costs have been estimated at US$34 per collected ton, with high cost variance between countries. For example, the cost in Argentina is US$54, while in Paraguay is only US$6.6 (a US$47 difference). These variations typically reflect differences associated with the (higher-lower) quality of service. Adequate MSW final disposal coverage (i.e., in landfills) is approximately 55% (as a percentage of the population), which means that there is still a large amount of waste that is not disposed and/or treated adequately (45%). The average final disposal cost is US$20.4 per ton disposed. There is a first group of countries where disposal costs are around US$10 (US$5.6 in Ecuador, US$11.4 in Chile), a second group where it costs around US$20 (US$18.8 in Costa Rica, US$23.3 in Colombia), and then there¿s Brazil, where costs are, on average, considerably higher (US$31.5). In several LAC countries MSW management costs are directly afforded by the municipality. The average cost recovery is 51.6%xii. Municipalities typically use property taxes as the main revenue collection system. This collection mechanism represents 52.1% of the total, followed by direct billing to users, with 20.2%; the electricity bill (15.3%); and the water & sanitation bill (12.4%).
