Abstract:
La infección del tracto urinario de origen bacteriano es una patología frecuente en la mujer embarazada. Constituye un importante factor de riesgo asociado con el desarrollo de sepsis neonatal. La sepsis neonatal es una infección invasiva generalmente acompañada de bacteriemia que se presenta en el neonato en el primer mes de vida. Es una de las causas principales de morbilidad y mortalidad neonatal. El objetivo de esta investigación fue mostrar la relación entre la infección en el tracto urinario en la madre embrazada y la morbilidad y mortalidad del neonato. Se realizó un estudio de casos y controles que incluyó una muestra intencional y no probabilística de 224 pacientes, dividida en dos grupos de interés: 70 casos de neonatos nacidos de mujeres diagnosticadas conla infección y 154 controles de neonatos nacidos de mujeres sanas. Se realizó un análisis bivariado aplicando la prueba Chi cuadrado y estimando el Odds Ratio con apoyo del software OpenEpi, v3. El estudio mostró que los neonatos nacidos de madres con infección del tracto urinario tienen mayor riesgo de desarrollar sepsis neonatal, y sugiere que esta patología, una vez diagnosticada, puede ser tratada con eficacia evitando consecuencias graves para la salud del recién nacido. Abstract:
A new blue electroluminescent conjugated polymer, poly(BP-alt-BCV), was prepared by Hornor–Emmons polycondensation of [5-carbazol-9-yl-3-(diethoxy-phosphorylmethyl)-2-(2-ethyl-hexyloxy)-benzyl]-phosphonic acid diethyl ester (PHBP) and 4,4′-biphenyldicarboxaldehyde (BPCA) in the presence of tert-BuOK in THF. UV-visible absorption maximum (λmax,UV, π-π* transition) of poly(BP-alt-BCV) was measured at 340 nm. PL maximum (λmax,PL) values of poly(BP-alt-BCV) were obtained at 406 and 430 nm, when excited at its own λmax,UV. It was found that double layer [ITO/poly(BP-alt-BCV)/Alq3 (50 Å)/Al] PLED exhibited maximum EL peak at 440 nm and blue emission (x = 0.1794, y = 0.1878), similar to the NTSC standard blue (x = 0.14, y = 0.08).

В производственных условиях исследовали возможность использования озимого и ярового ячменя, выращенных в Кабардино-Балкарской республике для пивоваренной промышленности. В качестве объектов исследований были выбраны сорта озимого ячменя — Мастер, Михайло, Козырь, Добрыня 3 и ярового — Приазовский 9, Гетьман, Виконт, Мамлюк, допущенные к использованию в Северокавказском регионе. Наибольшей зимостойкостью отличился сорт Мастер. Близким к нему показателем характеризовался сорт Михайло. Сорта Козырь и Добрыня 3 показали пониженную, на 12–14% по сравнению с 1-м сортом, зимостойкость. Наибольшие показатели по ним отмечены у сорта Приазовский 9–45,2 г при 21 зерне в колосе. Мамлюк отличается от перечисленных сортов меньшей озерненностью колоса (19 шт.) и минимальной среди них абсолютной массой зерна (41,6 г). Высокий коэффициент продуктивной кустистости отмечен у сорта Мастер, благодаря чему на 1 га насчитывалось 3,42 млн колосьев с наибольшим числом зерен в колосе — 32 шт. и их абсолютной массой 42,9 г. Остальные сорта уступили ему по числу растений на 1 м2 на 3,4–13,3%, количеству продуктивных стеблей на 4,4–17,8%, числу зерен в колосе на 3,3–31,2%, абсолютной массе зерна на 1,2–4,0%. Яровые сорта Виконт и Мамлюк обладали средней крупностью зерна, а у сортов Приазовский 9 и Гетьман она выше на 3–4%. Из предложенных озимых сортов наибольшей крупностью отличился сорт Мастер. Он же имел максимальные показатели по натуре и массе 1000 зерен, процентному содержанию крахмала и экстрактивности. Михайло и Козырь имеют близкие значения по всем перечисленным показателям и уступают Мастеру по крупности зерна на 13,0–17,2%, массе 1000 зерен на 0,9–1,9%, натурной массе — на 2,5–3,4%, содержанию крахмала — на 9,1–10,0%, экстрактивности — на 3,2–1,6%. The possibility of using winter and spring barley grown in the Kabardino-Balkarian Republic for the brewing industry was investigated under production conditions. The research objects were winter barley varieties — Master, Mikhailo, Kozyr, Dobrynya 3 and spring barley — Priazovsky 9, Getman, Viscount, Mamluk, approved for use in the North Caucasus region. The Master variety has the greatest winter hardiness. Mikhailo showed a figure close to his. Kozyr and Dobrynya 3 were characterized by reduced winter hardiness, by 12–14% compared to the first variety. The highest indicators for them were noted in the Priazovsky 9 variety — 45.2 g with 21 grains per year. Mamluk differs from the listed varieties in the smaller grain size of the ear — 19 pieces. and the minimum among them with an absolute grain weight of 41.6 g. A high coefficient of productive tillering was noted in the Master variety, due to which there were 3,42 million ears per 1 hectare with the largest number of grains per ear — 32 pieces. — and their absolute weight — 42.9 g. The remaining varieties were inferior to it in the number of plants per 1 m2 by 3.4–13.3%, the number of productive stems by 4.4–17.8%, the number of grains per year by 3.3–31.2%, absolute grain weight by 1.2–4.0%. The spring varieties Viscount and Mamluk had an average grain size, while in the Priazovsky 9 and Getman varieties it was 3–4% higher. Of the proposed winter varieties, the Master variety distinguished itself by the largest size. It also had the maximum indicators in terms of nature and weight of 1000 grains, percentage of starch and extract content. Mikhailo and Kozyr have similar values for all of the above indicators and are inferior to the Master in grain size by 13.0–17.2%, weight of 1000 grains by 0.9–1.9%, natural weight — by 2.5–3.4%, starch content — by 9.1–10.0%, extract content — by 3.2–1.6%.

440 Background: Approximately 12–23% G/GEJC patients have human epidermal growth factor receptor 2 (HER2)-positive disease. Despite survival benefit from combination therapy with trastuzumab and chemotherapy, the prognosis remains unsatisfactory; more effective treatments are needed. This phase 2 study is evaluating the combination of HLX22 (an anti-HER2 monoclonal antibody targeting a different epitope than trastuzumab), trastuzumab, and XELOX chemotherapy as first-line treatment for patients with advanced/metastatic G/GEJC. Following the most recent report of 17 to 18 patients in each group at ASCO 2024 Annual Meeting, herein we present the updated efficacy and safety results with 31 patients in each group. Methods: Patients with locally advanced or metastatic HER2-positive G/GEJC and no prior systemic antitumor therapy were enrolled. Herein reported are results from Stage 2 of the study. Eligible patients were randomized to receive either HLX22 + trastuzumab + XELOX or placebo + trastuzumab + XELOX in 3-week cycles. Primary endpoints were independent radiology review committee (IRRC)-assessed progression-free survival (PFS) and objective response rate (ORR) per Response Evaluation Criteria in Solid Tumors (RECIST) v1.1. Secondary endpoints included other efficacy and safety endpoints. Results: As of June 30, 2024, 62 patients were randomized to the respective groups (31 vs 31), of whom 51 (82.3%) patients were male. Median follow-up duration was 20.3 and 24.0 months for the respective groups. The efficacy results are shown in the table. Treatment-emergent adverse events (TEAEs) were reported in 30 (96.8%) and 31 (100%) patients, and HLX22- or placebo-related TEAEs of grade 3 or higher were reported in 9 (29.0%) and 6 (19.4%) patients in the respective groups. One patient (3.2%) in the placebo + trastuzumab + XELOX group had a grade 5 HLX22-/placebo-related TEAE. Conclusions: With a manageable safety profile, the addition of HLX22 to first-line treatment with trastuzumab plus XELOX conferred survival benefit for HER2-positive G/GEJC patients. Abstract:
Nitriding is as an effective technique applied for many years to improve the surface hardness and wear resistance of low carbon and tool steels [1]. In the case of stainless steels, increase of surface hardness and wear resistance accompany by a drop in corrosion resistance due to the precipitation of CrN. In this respect, many attempts have been made to modify the surfaces of austenitic stainless steels to increase their surface hardness and wear resistance without scarifying the corrosion resistance [2-6]. It is finally concluded that, nitriding at temperatures lower than conventional nitriding process (which is generally about 550°C) has potentiality to produce a nitrogen expanded austenite (also known as S-phase), on the surface without formation of CrN. Due to the superb properties of the S-phase, the low temperature nitrided austenitic stainless steels exhibit very high surface hardness, a good wear resistance, and more importantly, an excellent corrosion resistance. Recently some attempts have been made to apply low temperature nitriding to martensitic stainless steels, which are widely used in the industries of medicine, food, mold and other civil areas [7-9]. In these works, where nitriding has been conducted by plasma processes, superior surface hardness, along with excellent wear and corrosion resistances have been reported for AISI 410 and AISI 420 grade martensitic stainless steels. This work focuses on low temperature gas nitriding of AISI 420 grade martensitic stainless steel in a fluidized bed reactor. In this respect the microstructures, phase compositions, hardness, wear and corrosion behaviours of the original and nitrided martensitic stainless steels have been compared.

Abstract:
In this study, 13% of fresh lettuce (Lactuca sativa) samples collected from markets and supermarkets in two cities of Mexico were contaminated with Salmonella spp. From those samples, amplicons of ~300 base pairs (bp) were amplified, corresponding to the expected size of the invasion (invA) and internal transcribed spacer regions of the 16S and 23S rRNA genes of Salmonella spp. Additionally, Salmonella strains were isolated and harbored plasmids ranging from ~9 to 16 kbp. From these strains, 91% were resistant to ampicillin and nitrofurantoin, whereas 55% were resistant to cephalothin and chloramphenicol. No resistance was detected to amikacin, carbenicillin, cefotaxime, gentamicin, netilmicin, norfloxacin, and sulfamethoxazole-trimethoprim. When Salmonella isolates were tested against novel bacteriocins (morricin 269, kurstacin 287, kenyacin 404, entomocin 420, and tolworthcin 524) produced by five Mexican strains of Bacillus thuringiensis, 50% were susceptible to these antimicrobial peptides. This is the first report showing that Salmonella strains isolated from lettuce are susceptible to bacteriocins produced by the most important bioinsecticide worldwide, suggesting the potential use of these antibacterial peptides as therapeutic agents or food preservatives to reduce or destroy populations of Salmonella spp.

Purpose: Prader–Willi syndrome (PWS) is a genetic neurobehavioral disorder presenting hypothalamic dysfunction and adiposity. At rest, PWS exhibits hypoventilation with hypercapnia. We characterized ventilatory responses in children with PWS during exercise. Methods: Participants were children aged 7–12 years with PWS (n = 8) and without PWS with normal weight (NW; n = 9, body mass index ≤ 85th percentile) or obesity (n = 9, body mass index ≥ 95th percentile). Participants completed three 5-minute ambulatory bouts at 3.2, 4.0, and 4.8 km/h. Oxygen uptake, carbon dioxide output, ventilation, breathing frequency, and tidal volume were recorded. Results: PWS had slightly higher oxygen uptake (L/min) at 3.2 km/h [0.65 (0.46–1.01) vs 0.49 (0.34–0.83)] and at 4.8 km/h [0.89 (0.62–1.20) vs 0.63 (0.45–0.97)] than NW. PWS had higher ventilation (L/min) at 3.2 km/h [16.2 (13.0–26.5) vs 11.5 (8.4–17.5)], at 4.0 km/h [16.4 (13.9–27.9) vs 12.7 (10.3–19.5)], and at 4.8 km/h [19.7 (17.4–31.8) vs 15.2 (9.5–21.6)] than NW. PWS had greater breathing frequency (breaths/min) at 3.2 km/h [38 (29–53) vs 29 (22–35)], at 4.0 km/h [39 (29–58) vs 29 (23–39)], and at 4.8 km/h [39 (33–58) vs 32 (23–42)], but similar tidal volume and ventilation/carbon dioxide output to NW. Conclusion: PWS did not show impaired ventilatory responses to exercise. Hyperventilation in PWS may relate to excessive neural stimulation and metabolic cost.

Abstract:
Purity, morphology, and structural characterization of synthesized deep eutectic solvent (DES)-lignins (D6h, D9h, D12h, D18h, D24h) extracted from willow (Salix matsudana cv. Zhuliu) after treatment with a 1:10 molar ratio of choline chloride and lactic acid at 120 °C for 6, 9, 12, 18, and 24 h were carried out. The purity of DES-lignin was ~95.4%. The proportion of hydrogen (H) in DES-lignin samples increased from 4.22% to 6.90% with lignin extraction time. The DES-lignin samples had low number/weight average molecular weights (1348.1/1806.7 to 920.2/1042.5 g/mol, from D6h to D24h) and low particle sizes (702–400 nm). Atomic force microscopy (AFM) analysis demonstrated that DES-lignin nanoparticles had smooth surfaces and diameters of 200–420 nm. Syringyl (S) units were dominant, and total phenolic hydroxyl content and total hydroxyl content reached their highest values of 2.05 and 3.42 mmol·g−1 in D12h and D6h, respectively. β-Aryl ether (β-O-4) linkages were eliminated during DES treatment.

This abstract was retracted on June 29, 2021. e12552 Background: Breast cancer is the commonest cancer among women in Kenya. We wanted to examine breast cancer seen at the Kenyatta National Hospital in relation to local factors that influence prognosis and quality of life, local variations in treatment and outcomes; describe the clinical care patterns, monitor the safety of the therapies provided to patients in a routine clinic setting. Methods: A prospective study of patients with breast cancer between 11.08.2011 and 11.09.2014 inclusive. Data included demographic details, diagnostic and staging procedures, stage, treatment and outcome. Estimates of relative survival used period approach. hi-square tests and analysis of variance (ANOVA) were utilised to make comparisons. Cross sectional data are presented in proportions, means and medians. Results: Four hundred patients were included, age range 20 to 83, median 49 years. Out 312, 65 (20.8%) were obese. Eight of 397 (2%) were smokers and 22(5.5%) took alcohol. Early disease was diagnosed in 269 out of 354 (76%) and metastatic disease in 85(24%). Breast lump presented in 388 out of 400 (97%), breast pain in 104 out of 388 (26.8%). Fifteen of 394 (3.8%) had second breast cancer, 4 (1%) had had ovarian cancer and 9 (2.3%) had had had other malignancy. History of breast cancer in first and second-degree relative was elicited in 41 out of 394 (10.4%). Ductal carcinoma NOS was commonest in 343 (88.2%), lobular carcinoma in 9(2.5%). Cases by T stage were T1 - 25(7.2%), T2 -130(37.4%), T3 - 96(27.6%), T4 - 87(25%). Of 322 cases, 187(58.1%) were ER positive and 175 (54.4%) PR positive. Her2 positive cases were 78 out of 322 (24.2%). Neo adjuvant and adjuvant chemotherapy mainly consisted of combinations of cyclophosphamide and doxorubicin [AC] +/- a taxane[AC→T] ( mainly by medical oncologists) or AC+ 5-FU [CAF] (mainly by clinical oncologists). Of 305 cases 272 (89.2%) completed adjuvant therapy, 8(2.6%) died during treatment. Median overall survival was 57.1 months (95% CI; 55.6 to 59.5 months). For metastatic disease, median PFS was worse for patients < 40 years. Conclusions: Pathology and biology mirrored global situation, over 75% of patients had non metastatic disease. A significant proportion of early disease patients did not complete treatment.

AbstractThe purpose of the study was to examine changes in performance and match-induced fatigue over a 27-week training period. Eight national-level water-polo players performed a 5 x 200 m swimming test to calculate velocities corresponding to blood lactate concentration of 4.0, 5.0 and 10.0 mmol.l-1 at three testing periods: i) baseline, ii) end of the pre-season (8 weeks of 4 x 4 min swimming bouts), iii) end of the in-season (8 weeks of 8 x 20 m swimming sprints). During each testing period, four competitive matches were played and repeated sprints (8 x 20 m), 400 m swimming, and shooting accuracy were evaluated at the pre- and post-match. Repeated sprint tests were also conducted at mid-game. Analysis of variance for repeated measures was used to detect changes among training periods and within games. Swimming velocities corresponding to 4.0, 5.0 and 10.0 mmol.l-1 were increased after the pre-season by 9%, 7.7%, and 6.7% (p < 0.01) and decreased following the in-season compared to the pre-season by 8.9%, 7.0% and 3.3% (p < 0.01), respectively. Pre-match repeated sprints and 400 m performance were improved after the pre-season by 4.3% and 3.8% (p < 0.01) and decreased by ~3% after the in-season compared to the pre-season (p < 0.01). Mid- and post-match repeated sprint performance was improved after the pre-season by 4.8 ± 1.4% and 4.4 ± 1.1% and remained unchanged after the in-season compared to the pre-season. Post-match 400 m speed was improved by 3.2% after the pre-season (p < 0.01) and decreased by 2.8% after the in-season (p = 0.04).Pre-season training improved players’ aerobic endurance and performance. Intensified in-season training decreased aerobic power, endurance, and pre-match performance while maintaining match repeated sprint performance.

The results of the improvement of the selection and genetic parameters of the goose population obtained by purebred linear breeding and geese with blood transfusion of the large gray breed (IV generation) of Obroshyn breeding are given. The research was conducted on geese of the Obroshyn gray breed group with the aim of increasing the selection and breeding nucleus of geese with high-performance indicators. As a result of the selection and breeding work carried out in 2022, the effectiveness of the introductory crossbreeding of Obroshyn gray geese with great gray geese was proven. Improvement of productive qualities of Obroshyn gray geese was achieved with the infusion of large gray breed blood (IV generation) and preservation of the main qualities of this breed group: good adaptability to local conditions of feeding and maintenance, exterior, quality of meat and feather and down raw materials. Productivity indicators of Obroshyn gray geese (I group) were: laying capacity – 38,4 pcs.; egg weight – 164,0 g; fertilization of eggs – 86,3 %; hatching of goslings – 76,4 %; live weight of geese at the age of 9 weeks - males 4310 g, females - 3800 g; preservation – 95,0 %. Obroshyn gray geese of the 4th generation obtained by transfusion of large gray breed blood (group II) were characterized by correspondingly higher indicators: in terms of laying by 3,1 %, however, fertilization was lower by 5,8 % and hatching was lower by 4.4%. The preservation of geese in this group was higher by 3,2 %, and the live weight at the age of 9 weeks was 4.4% higher in males and 3.2% higher in females. The geese of the II experimental group were superior to their peers from the I group in terms of meat quality, in terms of the weight of the uncut and cut carcass, and the yield of edible parts. Geese of experimental group II were also characterized by better meat productivity at the age of 9 weeks, in which the mass of pectoral muscles was 7.0% higher in males and 1,4 % higher in females, and the mass of thigh muscles was higher in males and females - by 4,0 and 4,8 %, respectively, than in geese of the I group. Geese of experimental group II were also characterized by better meat productivity at the age of 9 weeks, in which the mass of pectoral muscles was 7.0% higher in males and 1.4% higher in females, and the mass of thigh muscles was higher in males and females - by 4.0 and 4.8%, respectively, than in geese of the I group.

Abstract Introduction Febrile neutropenia (FN) is a serious adverse effect of myelosuppressive chemotherapy, which often results in hospitalization and chemotherapy dose modification. FN risk depends on patient characteristics and chemotherapy regimen risk. Understanding the FN risk associated with individual chemotherapy regimens can help guide the use of prophylactic granulocyte colony-stimulating factor (G-CSF) and patient monitoring. To this end, the NCCN has classified regimens into high (≥20%), intermediate (10%-20%), or low (<10%) FN risk based primarily on clinical trial data. However, even for the same regimen, the FN risk is often higher in clinical practice than in clinical trials. In this study, we assessed the FN risk associated with several regimens for which FN risk has not been determined or has shown substantial variability outside of a clinical trial setting, using data from Kaiser Permanente Southern California (KPSC), a large, community-based practice. Methods Included were patients diagnosed with incident non-Hodgkin's lymphoma (NHL), breast cancer (BC), or multiple myeloma (MM) between 2008 and 2013 at KPSC who initiated the following chemotherapy regimens: bendamustine ± rituximab for NHL; docetaxel, carboplatin, and trastuzumab (TCH) or docetaxel and cyclophosphamide (TC) for BC; or Q4W lenalidomide 25 mg/dexamethasone for MM. Bendamustine ± rituximab, TCH, and lenalidomide are not classified by NCCN; TC is classified as intermediate FN risk but has shown considerable variability of FN incidence when used in clinical practice. Data on cancer diagnosis, chemotherapy use, G-CSF use, neutrophil count, and infections were obtained from KPSC's electronic medical records to estimate the incidence proportions of FN and grade 3 and 4 neutropenia. FN was defined as (1) hospitalization with absolute neutrophil count (ANC) <1000/µL or (2) hospitalization with primary or secondary diagnosis codes of neutropenia (ICD-9 288.0x) and fever (ICD-9 780.6), diagnosis code for bacterial/fungal infection, or antibiotic use. Grade 3 neutropenia was defined as ANC ≥500/µL to <1000/µL; grade 4 neutropenia as ANC <500/µL. Patients who received prophylactic G-CSF within 5 days of chemotherapy initiation were excluded from analysis. Results Overall, 40 (12%) NHL patients; 149 (24%) and 340 (28%) BC patients who received TCH and TC, respectively; and 0 (0%) MM patients were excluded due to prophylactic G-CSF. Over the first 6 cycles of bendamustine (median 338.4 mg/m2) ± rituximab for NHL patients (n = 307), 7.2% experienced FN, 4.2% grade 3 neutropenia, and 17.6% grade 4 neutropenia. Over the first 6 cycles of TCH for BC patients (n = 462), 24.2% experienced FN, 10.6% grade 3 neutropenia, and 44.6% grade 4 neutropenia. Over the first 6 cycles of TC for BC patients (n = 859), 20.5% experienced FN, 9.5% grade 3 neutropenia, and 37.5% grade 4 neutropenia. Over the first 4 cycles of lenalidomide/dexamethasone for MM patients (n = 186), 3.8% experienced FN, 5.9% grade 3 neutropenia, and 18.3% grade 4 neutropenia (Table 1). Conclusions Using NCCN criteria, bendamustine ± rituximab for NHL and lenalidomide/dexamethasone for MM would be classified as low-FN-risk regimens (<10%). By contrast, BC regimens TCH and TC would be classified as high-FN-risk regimens (>20%) based on our data. These results could help inform prophylactic G-CSF use for the selected regimens in clinical practice. Table 1. Tektonika dan anatomi arsitektur yang ada di Candi Batu Dieng ini dapat memberikan bukti kemajuan dan teknologi konstruksi Nusantara pada abad tersebut, Candi Batu Dieng juga menyimpan cerita mengenai relasi antara Arsitektur Candi Nusantara dengan Arsitektur Candi di tempat lain seperti di India Utara dan Selatan. Penelitian ini menemukan bahwa terjadi berbagai macam transformasi yang dikaji melalui teori transformasi arsitektur yang ditulis oleh Mustafa (2010) seperti transformasi reka cipta, peminjaman budaya, dan varian. Selain itu penelitian ini juga memberikan gambaran mengenai candi-candi di Dieng yang sudah ditemukan runtuh seperti bagian kepala pada Candi Srikandi, Gatotkaca, dan Dwarawati. 440 Background: Early detection of treatment response of neoadjuvant chemotherapy (CTx) for locally advanced rectal cancer may spare patients the toxicity of ineffective CTx. We evaluated prospectively tumor response with 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) in the early course of preoperative CTx. Methods: A total of 15 patients with locally advanced rectal cancer (T3 or T4 and N0-N2) received neoadjuvant CTx (XELOX or XELOX+bevacizumab). Patients underwent 18F-FDG PET before CTx, at the end of first cycle of CTx (12-38 days: median 15days), and before surgical resection. Scans were interpreted using SUVmax parameter. Magnetic resonance imaging (MRI) was undergone before CTx, at the end of 2 cycles of CTx (25-54 days: median 38 days), and before resection. The tumor longest diameter was measured by MRI T2-weighted images. After resection, SUVmax and diameter were compared with the Tumor Regression Grade (TRG). Results: Of 15 patients, TRG1 was in 1 patient, TRG2 was in 5 patients, and TRG3 was in 9 patients. We divided into 2 groups, non-responder (NR) group was TRG1 and TRG2, and responder (R) group was TRG3. There was no significant difference as for clinicopathological parameters before CTx including tumor diameter and SUVmax. The tumor size before surgery was significantly smaller in R group than NR group (22.2 mm vs. 35.0 mm; p = 0.017). SUVmax in R group was significantly lower at the end of first cycle of CTx (4.9 vs. 9.3; p = 0.023), and before surgical resection (3.2 vs. 10.3; p = 0.007) than that in NR group. Conclusions: In this study, 18F-FDG PET at the end of first neoadjuvant CTx successfully predict pathological response of locally advanced rectal cancer.

AbstractHerein we present the synthesis, crystal structure, and the infrared and Raman spectra of K3Na4[SiF6]3[BF4]. The compound also occurs in nature as the mineral Knasibfite. We obtained it from the reaction of stoichiometric amounts of SiO2, Na[BF4], K2CO3, and Na2CO3 in hydrofluoric acid at room temperature. Hydrofluorothermal synthesis at 200 °C lead to a product of higher purity. Knasibfite is colorless and crystallizes in space group Im 2m with a = 5.546(2), b = 9.261(2), c = 17.184(4) Å, V = 882.5(4) Å3, Z = 2, at T = 400(2) K. At 342 K a phase transition to a monoclinic structure was observed (I 2, a = 5.5003(6), b = 9.0890(9), c = 17.0048(2) Å, β = 90.041(9)°, V = 852.2(2) Å3, Z = 2, T = 100(2) K). By replacing SiO2 with GeO2 as a starting material K3Na4[GeF6]3[BF4] was obtained. This compound also crystallizes in form of two polymorphs, both of which are isotypic to the respective ones of K3Na4[SiF6]3[BF4].

Background:The Janus kinase inhibitors (JAKi) have been increasingly used for the treatment of rheumatoid arthritis (RA) in Sweden, with baricitinib representing ~80% of prescriptions. Evidence regarding the comparative effectiveness of JAKis versus biologics (bDMARDs), and in particular non- tumour-necrosis-factor inhibitor (TNFi) bDMARDs, in real-life is limited.Objectives:To compare RA patients treated with bDMARDs and JAKi in Sweden, in terms of: (1) patient characteristics at treatment start; (2) proportions of patients remaining on therapy, and response rates, at 12 months.Methods:RA patients starting treatment in 2017 and 2018 with either a TNFi, rituximab, abatacept, interleukin 6 inhibitors (IL6i) or a JAKi as different lines of treatment were identified in the Swedish Rheumatology Quality Register. One patient could contribute with more than one treatment episode.Treatment response at 12 months was measured as EULAR good response, HAQ improvement >0.2 units, DAS28 and CDAI remission, and as 0 tender and swollen joint counts (28JC). Patients were classified as non-responders if they stopped treatment before evaluation due to safety or inefficacy. Responses for patients who stopped treatment due to pregnancy or death and patients on treatment but with missing response were imputed using multiple imputation.Proportions of responders and differences in proportions between treatment groups, adjusted using inverse probability of treatment weighting, were estimated using linear regression with robust standard errors.Results:JAKi were often used after bDMARDs, and less frequently prescribed in combination with methotrexate. IntroductionData from two, identically designed, placebo-controlled, randomised, double-blind clinical trials (D1444C00132+D1444C00133) for once-daily extended-release quetiapine fumarate (QTP-XR) were pooled and analysed.ObjectiveEvaluate dose response, efficacy and safety for QTP-XR in schizophrenia.MethodsPost-hoc analysis of data from patients receiving QTP-XR 400, 600, 800 mg/day or placebo. Endpoints: least squares means (LSM) change from baseline to Day 42 in PANSS total and positive and negative subscale scores. No corrections for multiplicity were performed. Adverse events (AEs) were recorded.Results914 patients were included; PANSS scores were assessed in the MITT population (n = 889). LSM change from baseline in PANSS total score diverged significantly from placebo at: Day 14 for QTP-XR 800 mg/day (-15.3 vs -12.1 for placebo; p = 0.018); Day 21 for 600 mg/day (-17.3 vs -14.2; p = 0.039); Day 42 for 400 mg/day (-19.2 vs -15.4; p = 0.033).Jonckheere-Terpstra analysis of change in PANSS total score at Day 42 showed a significant QTP-XR dose response (p = 0.0196; p < 0.001 with placebo). PANSS positive scores diverged by Day 21 for both QTP-XR 800 (-5.7 vs -4.8; p = 0.049) and 600 mg/day (-5.8 vs -4.8; p = 0.046). PANSS negative scores diverged by Day 21 (-4.0 vs -3.2; p = 0.040) and 42 (-4.8 vs -3.6; p = 0.009) for QTP-XR 800 and 600 mg/day, respectively. AEs occurred in 59.4%, 66.5%, 62.1% and 56.2% of patients in the QTP-XR 800, 600, 400 mg/day and placebo groups, respectively. Most common AEs were somnolence, dry mouth, sedation, insomnia, dizziness, headache, constipation and nausea.ConclusionsQTP-XR was generally well tolerated and demonstrated efficacy that increased with dose in schizophrenia.Financial support: AstraZeneca.

Abstract. The Langley plot method applied on the Brewer Ozone measurements can provide accurate Aerosol Optical Depth (AOD) in the UV-B. We present seven intercomparisons between AOD retrieved from Brewer Ozone measurements at 320 nm and AOD measured by CIMEL sunphotometer at 340 nm or 440 nm (shifted to 320 nm in using the Angström's law), which are stored in the international AERONET database. Only the intercomparisons between co-located instruments can be used to validate the Langley Plot Method applied to the Brewer measurements: in this case, all the correlation coefficients are above 0.82. If the instruments are not at the same site, the correlation between the AOD retrieved by both instruments is much lower. In applying the Angström's law the intercomparison is improved compared to previous study.

In this work, 9H-carbazole acted as precursor to synthesize a new organic dye, which involved the alkylation of carbazole with butyl bromide in the presence of phase transfer catalyst tetrabutylammonium bromide (TBAB) to give BC (9-butyl-9H-carbazole) followed by the formylation with hexamethylenetetramine (HMTA) in the presence of trifluoroacetic acid (TFA) and tetrahydrofuran (THF). Formyl groups at 3- and 3,6-positions afforded CC (9-butyl-9H-carbazole-3-carbaldehyde) and CD (9-butyl-9H-carbazole-3,6-dicarbaldehye), respectively. The formylation reaction with HMTA suffered with low yield, therefore another methodology was adopted for the formylation of BC with POCl3 in the presence of dry DMF to afford CC and CD. Then condensation reaction was carried out CC and CD with cyanoacetic acid in the presence of ammonium acetate as catalyst. This reaction resulted in formation of CA (3-(9-butyl-9H-carbazol-3-yl)-2-cyanoacrylic acid) and DA (3,6-(9-butyl-9H-carbazol-3-yl)-2,2'-cyanoacrylic acid. All the synthesized compounds were characterized by FTIR, GC-MS, MS and UV-visible methods. The UV-Visible absorption spectra of CA was observed at 239 nm while the absorption region is from 190 nm to 340 nm and DA shows the absorption peak at 320 nm while the absorption region is from 290 to 440 nm. The energy band gaps for CA and DA were found to be 5.2 eV and 3.6 eV, respectively.

ABSTRACTThree randomized-block experiments involving 87 British Friesian, 18 Simmental × Friesian and 18 Hereford × Friesian, artificially reared calves (mean initial live weight 47 kg and age 9 days), were carried out to examine the effects of the quantity of milk substitute consumed on the lifetime performance of beef cattle. The three treatments were (1) 400 g milk powder per head daily, weaned at 42 days of age (2) ad libitum intake of milk powder, weaned at 42 days and (3) ad libitum intake of milk powder, weaned at 60 days. All calves were individually penned and fed until after weaning; those on treatment 1 were bucket fed twice daily, while those on treatments 2 and 3 sucked the milk from 25·1 containers through artificial teats. They were offered a pelleted, barley/soya-bean meal concentrate (197 g crude protein per kg dry matter (DM)) ad libitum from purchase until intake reached a maximum of 2·7 kg per head daily, and well preserved grass silage ad libitum from 6 weeks of age. Three commercial, skimmed milk-based, acidified milk substitutes (242 and 187 g crude protein and oil per kg respectively) were used. Intakes of milk substitute, concentrate DM and silage DM per calf to 12 weeks of age for treatments 1 to 3 respectively were as follows: 13·0, 39·7 and 54·8 (s.e. 1·67) kg; 88, 73 and 58 (s.e. 1·5) kg and 9·4, 10·6 and 11·2 (s.e. 0·23) kg. Live-weight gains to 12 weeks of age and carcass weights adjusted to 19 months of age were 0·71, 0·82 and 0·84 (s.e. 0·026) kg/day and 335, 340 and 341 (s.e. 3·8) kg. It is concluded that increasing the input of milk substitute above 400 g per head daily until 6 weeks of age produced only a small and uneconomic increase in the lifetime performance of beef cattle.

Objective. To evaluate the clinical and demographic characteristics of a group of patients under 35 years old admitted with acute coronary syndrome, as well as the features of coronary bed damage and endovascular treatment.Material and methods. A retrospective analysis of the group of patients aged 30 to 35, admitted to the Regional Vascular Center with a diagnosis of acute coronary syndrome in the period from 2019 to June 2021, was carried out. The study included 72 patients with ACS, regardless of the ST segment changes on the electrocardiogram, were admitted to the hospital by the emergency medical service referral. Positive troponins were detected in all the patients by qualitative analysis. Upon admission, they were sent to a catheterization laboratory. An examination was carried out according to the recommendations of medical care, as well as selective coronary angiography and, if indicated, stenting of the coronary arteries was performed.Results. When analyzing the clinical and demographic characteristics of the group, attention has been drawn to the absolute predominance of males — 71 (98.7%), urban residents — 64 (88.9%), low frequency of bad habits: tobacco smoking in 13 (18.1%) and alcohol abuse in 2 (2.8%) patients, the absence of concomitant pathology, a signifi cant time from the onset of symptoms to calling an ambulance (165 [90; 263]). According to the results of angiography, it should be noted the possibility of acute coronary syndrome with intact coronary arteries is 9 (27.3%) among all ACS cases with ST segment elevation and 29 (74.3%) with ACS without ST segment elevation, while in one third of the above episodes (13 (34.2%)) ACS was caused by the presence of a muscle “bridge” in the basin of the anterior descending artery, a concomitant phenomenon of slowed blood fl ow. The attention has been also drawn to the almost equal proportions of acute thrombotic occlusion (19 (55.9%)) and haemodynamically signifi cant hemadostenosis (14 (41.2%)) as the cause of ACS.Conclusion. In patients under 35 years old with a typical clinical. picture, positive troponins, with a qualitative analysis, there is a high probability of pathological changes in the coronary bed, regardless of the ST segment changes, which requires X-ray endovascular methods of diagnosis and treatment. Angiographic features of the coronary artery lesion in young people are the discreteness of the lesion in the proximal or middle sections of the arteries, the high frequency of thrombotic occlusions, the predominant lesion of the anterior descending or right coronary arteries, the low frequency of calcifi cation of the coronary arteries.

ABSTRACT Second instar Helicoverpa armigera larvae were treated with B.t. strains isolated from soil samples and recorded mortality in the range of 16.67% to 94.44%. Maximum mortality was observed in reference strain HD1 which was on par with the B.t. strain 281 isolated from North Telangana Zone. There was no mortality of larvae in the untreated control. B.t. isolates 122 and 22 recorded 83.33% mortality. Lowest mortality was recorded in B.t. strains 252 and 261 isolated from North Coastal Zone. 50% of B.t. strains recorded more than 50% mortality. Among six B.t. strains isolated from bacteria infected silkworms except B.t. strain 432, remaining other B.t. strains showed more than 50% mortality. Third instar larvae of H. armigera were treated with native B.t. strains isolated from soil samples collected in different zones of A.P. and mortality was recorded in the range of 5.56% (B.t. strain 261) to 83.33% (HD1). Highest mortality was recorded in B.t. strain HD1 which was on par with the B.t. strains 22 and B.t strains 44, 87, 122. No larval mortality was observed in the control. 35% of the B.t. strains showed more than 50% mortality. Among six native B.t. strains isolated from bacteria infected silkworms, 50% mortality was recorded in B.t. strains 431, 434, 440 and 447. Forty B.t. strains (12, 22, 25, 44, 53, 58, 65, 71, 83, 87, 91, 95, 106, 122, 139, 153, 169, 175, 182, 193, 206, 224, 229, 242, 281, 291, 299, 307, 317, 341, 351, 372, 376, 408, 422, 424, 431, 434, 440 and 447) recorded more than 50% mortality against second and third instar H. armigera.

The results of the analysis show that the application of green manufacturing is able to reduce waste and increase added value by 50%. So that the increase in added value has the potential to improve the welfare of employees. Within a mixed deciduous forest in central Virginia, daily patterns of light interception and stomatal conductance were related to crown architecture for the deciduous Asimina triloba and the evergreen Ilex opaca. For A. triloba, branches were up to 54% longer, the bifurcation ratio was 15% lower, branch internode lengths were 15% greater, and leaf angles were closer to the horizontal plane (9 vs. 22°) than for I. opaca, possibly contributing to a greater percentage of sunlit leaves for A. triloba at 1100 and 1400 (22.4 vs. 10.0 and 30.0 vs. 10.1%, respectively). Maximum stomatal conductances to water vapor diffusion were 4.0 and 4.6 mm∙s−1 for sunlit leaves of A. triloba and I. opaca, respectively. Sunlit and shaded leaf stomatal conductances were similar in A. triloba throughout the day, but in I. opaca, values for sunlit leaves were up to 1.3 mm∙s−1 greater than values measured in shaded leaves. In addition, stomatal conductances for shaded leaves of A. triloba remained approximately 1.0 mm∙s−1 greater than for I. opaca throughout most of the afternoon. Therefore, within a forest understory environment, sympatric deciduous and evergreen species may differ in crown architecture, light interception, and stomatal conductance patterns.

Quantitative and qualitative composition of alloys and powders of electrode materials was determined by scanning electron microscopy and energy-dispersive X-ray spectroscopy. The Tb/R/Ni and Tb/Ni/Mg ratio in the samples was confirmed also by X-ray fluorescence spectroscopy. The cell parameters of RxTb2-xNi17 (x = 0.5) ternary phases are: a = 8.2987(9) Å, c = 8.0206(8) Å, V = 478.37(9) Å3 for R = Zr, a = 8.3161(6) Å, c = 8.0482(8) Å, V = 482.03(6) Å3 for R = Y and a = 8.3690(6) Å, c = 8.0560(7) Å, V = 488.66(6) Å3 for R = La. Tb atoms were partially substituted by Y, Zr and La atoms because of closeness of atomic radii size. Under experimental condition capacity parameters were 1.81 H/f.u. for the Zr-containing electrode, 2.29 H/f.u. for the Y-containing electrode and 2.31 H/f.u. for the La-containing electrode. In the case of Li,Mg co-doped electrodes we observed more than 2.5 H/f.u. Cell parameters of the Zr- and La-containing phases after hydrogenation increased isotropically. Synthesized hydrides can be interpreted as superstructures with the Tb2Mn17C2.5-type (filled-up of Th2Ni17). The Y0.5Tb1.5Ni17-based electrode demonstrates the potential corrosion at -0.540 V, electrodes with the compositions Zr0.5Tb1.5Ni17 and La0.5Tb1.5Ni17 show -0.413 V and -0.405 V, respectively. Li and Mg-codoped electrodes shoved the corrosion potential -0.410 V (Tb2Ni16.4Li0.2Mg0.4) and -0.550 V for Tb2Ni15.6Li0.6Mg0.8, respectively.

Gilts and sows without MMA after farrowing had a lower frequency of animals without estrus after weaning of the piglets (.2 % vs 1.1 %), a lower rate of late onset of estrus (3.4 % vs 6.2 %), a lower frequency of non-pregnant sows (16.0 % vs 22.2 %), less sows with abortion (.9 % vs 2.2 %) and a lower rate of mortality (1.1 % vs 4.0 %) compared with gilts and sows with MMA. The same effects have been demonstrated both in gilts and sows.

Chiral phenylindanediamnine (+)-2 was synthesized starting from chiral phenyl-indanedicarboxylic acid by a Schmidt rearrangement. Ordered amine-acid AB-type monomers, 8 and 9, were also prepared by a controlled reaction of (+)-2 with diacid chlorides. Disordered and ordered polyamides were synthesized by the reaction of (+)-2 with dicarboxylic acids, and the self-condensation of 8 and 9, respectively. Both the ordered and disordered polyamides were soluble in various solvents. The polyamides composed of the same dicarboxylic acid prepared by the different routes showed the same circular dichroism spectra. X-ray diffraction of the polyamide films indicated that all of the polyamides were amorphous. The glass transition and decomposition temperatures ranged from 170 to 340°C and from 320 to 400 °C, respectively. Additionally, polyamides containing the same diacids displayed nearly equivalent physical properties.

Meniscus is the most commonly injured structure in the knee joint. Resection of the meniscus as well as the torn menisci is known to induce the degeneration of the articular cartilage. Replacement of the resected meniscus by allograft is limited by its availability and potential disease transmission. Artificial prostheses are being tried in an attempt to regenerate the meniscal tissue and we developed the biodegradable porous polycaprolactone(PCL) scaffold, which acts as a temporary scaffold to enable the regeneration of a new tissue in time. We report the results of rabbit implantation model. Biodegradable PCL scaffold coated with type I collagen with pores sized 100~150 +m and with compression modulus 400 kpa were fabricated by melt-molding particulate- leaching method. The molds were made using the native meniscus of the rabbit. Medial meniscus of right knee was partially removed through arthrotomy, leaving anterior 1/5 of the meniscus, after sectioning medial collateral ligament. The implant was attached to the peripheral capsule and remnant anterior meniscus with sutures. The medial meniscus of the left knee was removed and served as a control without replacement. The regenerated meniscus was harvested at 4 & 12wks after implantation. In addition to the routine histology of the tissue regenerated and remnant scaffold, junction between the normal meniscus and the regenerated tissue, and cartilage surface degeneration was observed. After 4 and 12 weeks the scaffolds, although considerable amount of the materials remained, were largely filled and covered with fibrous tissue which was assumed to be derived from synovial tissue of peripheral capsule. The tissue grossly resembling the native meniscus was maintained and spindle shaped cells with extracellular matrices were observed histologically. Neither cells with chondrocytic phenotype nor distinct cartilage matrices were observed until 12 weeks. The bonding between the regenerated tissue and the peripheral synovial capsule was firm and solid in all cases. The tissue bridges between the native meniscus and the regenerated tissue were found in 9/10 operated knees. Articular surface degeneration was not different between experimental and control groups except one case. More or less, the extrusion of the meniscus was found in almost all knees. This study revealed that meniscal replacement with PCL polymer prosthesis was feasible and led to adequate tissue formation. Long term studies on adaptive remodeling will be required.

Soft red winter wheat (27.27 kg replicates) treated with combinations of deltamethrin + chlorpyrifos-methyl, with and without piperonyl butoxide, and cyfluthrin, applied alone or with either chlorpyrifos-methyl or piperonyl butoxide, was stored from 15 May 1992 to 9 February 1993 (6 chemical treatments and an untreated control). Each replicate was artificially infested with 50 adult lesser grain borers, Rhyzopertha dominica (Fab.), and 50 adult rice weevils, Sitophilus oryzae (L.), upon initial storage, sampled with probe traps at monthly intervals, and subsequently reinfested. Trap catch of lesser grain borer was always 0, except for two traps at month 4 that contained one adult in each trap. Maximum population levels of rice weevil and a natural infestation of the red flour beetle, Tribolium castaneum (Herbst), occurred at month 5. Trap catch of these two species in the chemical treatments averaged 2.2 to 34.0 and 0.0 to 19.5 live adults per trap, respectively. From months 4 to 9, the percentage of insect-damaged kernels averaged 4.0 to 5.5% in wheat treated with 0.25 ppm deltamethrin + 6.0 ppm chlorpyrifos-methyl + 4.0 ppm piperonyl butoxide. Maximum kernel damage in the other chemical treatments was 3.2%.

Abstract Matched unrelated donor hematopoietic stem cell transplantation (MUD HSCT) is a potentially curative treatment for patients with acute myeloid leukemia (AML). The graft-versus-leukemia (GVL) effect may be potent enough to overcome the otherwise poor prognosis associated with AML though its efficacy for high risk cytogenetic subgroups is uncertain. To test this hypothesis, we analyzed outcomes by cytogenetic risk group in 324 patients in first complete remission (CR1), and 440 in CR2 undergoing NMDP-facilitated MUD HSCT between 1988–2002. Using the SWOG/ECOG classification of cytogenetic risk groups (Slovak et al. Blood, 2000) cytogenetics were classified as favorable in 14% of patients, intermediate in 71% and unfavorable in 16%. 56% of the patients were male and 42% were > 35 years at HSCT. 76% of patients and donors were matched at HLA-A, -B and -DRB1, 17% were mismatched at one or more loci and 7% were potentially matched (serologically matched at HLA-A and -B and potentially allele matched at -DR). Untuk mengetahui karakteristik ibu dengan persalinan sectio caesarea di Rumah Sakit Dr. R. Ismoyo Kota Kendari tahun 2019. maka di gunakan metode penelitian Jenis penelitian deskriptif dengan populasi adalah semua ibu bersalin dengan tindakan sectio caesarea yang berjumlah 128 orang pada bulan Januari-Juni tahun 2019. Sampel sebanyak 57 orang dengan teknik pengambilan sampel menggunakan simple random sampling. Kategori umur terdapat 9 orang (15,8%) yang risiko tinggi dan 48 orang (84,2%) umur risiko rendah. Kategori paritas terdapat 31 orang (54,4%) yang risiko tinggi dan 26 orang (45,6%) umur risiko rendah. Kategori KPD terdapat 34 orang (59,6%) yang KPD dan 23 orang (40,4%) yang tidak KPD. Kategori makrosomia terdapat 30 orang (52,6%) yang makrosomia dan 27 orang (47,4%) yang tidak makrosomia.

Light weight materials which possess high strength and durability are being utilized by the automotive industry to increase fuel economy. Rapidly solidified (RS) Al alloys are currently being extensively studied for this purpose. In this investigation the microstructure of an extruded Al-8Fe-2Mo alloy, produced by Pratt & Whitney Aircraft, Goverment Products Div. was examined in a JE0L 2000FX AEM. Both electropolished thin sections, and extraction replicas were examined to characterize this material. The consolidation procedure for producing this material included a 9:1 extrusion at 340°C followed by a 16:1 extrusion at 400°C, utilizing RS powders which have also been characterized utilizing electron microscopy.

346 Background: Prolaris combines RNA expression analysis of cell cycle progression genes with clinicopathologic information to create a combined clinical cell-cycle risk score (CCR). We evaluated the ability of CCR to predict metastasis (mets) in men for whom guidelines indicate that multimodality therapy (MTx) should be considered. Methods: A commercial cohort (N=15669) of National Comprehensive Cancer Network unfavorable intermediate-risk (UFI) and high-risk (HR) men revealed a distribution of 70.5% and 29.5% respectively. A CCR threshold of 2.112 was selected so that 29.5% of these men were above the threshold. MTx was defined as combined use of androgen deprivation therapy with radiation (RT) or surgery, or with adjuvant RT. Associations were evaluated in a 718-person retrospective, multi-institutional database of Prolaris-tested UFI and HR men. Kaplan-Meier (KM) analyses and Cox regressions were used to estimate the effects of prognostic covariates. Results: Median follow-up was 5.13 years. CCR predicted mets in the full cohort (HR =3.8 [2.7,5.2], p<10−15) and after accounting for CAPRA (HR=4.3 [2.7,7.0], p< 10−7). CCR also was a significant predictor of mets in patients who received STx, as a continuous predictor (HR=4.0 [2.6,6.1], p<10−9) and when dichotomized at the threshold (HR=15.9 [5.4,46.5], p< 10−9). The KM probability of mets by 10 years for those below and above the threshold was 4.3% and 20.4% respectively. MTx reduced patients’ risk of mets (HR=0.46 [0.22,0.97], p=0.04), and treatment benefit can be evaluated as a function of CCR score (Table). Conclusions: The CCR score prognosticates a clinically meaningful different risk of metastasis for those receiving MTx versus STx. Approximately 27% and 73% of people with HR or UFI risk cancer have CCR scores below the risk threshold and may consider STx after considering the difference in risk of mets.[Table: see text]

Introducción. La diabetes mellitus es una de las enfermedades crónicas con mayor prevalencia en la población pediátrica y juvenil, con efectos en la calidad de vida de los pacientes.Objetivo. Evaluar la calidad de vida de una población pediátrica menor de 18 años con diagnóstico de diabetes de tipo 1, de dos instituciones pediátricas de la ciudad de Bogotá.Materiales y métodos. Se recolectaron los datos sociodemográficos, y se emplearon la versión validada en español del cuestionario PedsQL 4.0™ y el módulo 3.2 sobre diabetes. Los datos se procesaron en el software estadístico STATA 17™.Resultados. Con el puntaje global del módulo 3.2 sobre diabetes, de la versión validada del PedsQL™, se evaluó la correlación entre los valores de la hemoglobina A1c (HbA1c) y los del cuestionario. Los pacientes con valores por debajo del 9 % de HbA1c presentaron una mejor calidad de vida relacionada con la salud, mientras que, en el grupo con HbA1c mayor de 9 %, se observó una baja percepción de calidad de vida (p=0,025). En cuanto el tipo de terapia y la relación con los dominios del PedsQL™ 3.2, versión diabetes, los pacientes que utilizaban la bomba de insulina o microinfusor presentaban mejor puntaje en los dominios barreras, cumplimiento, preocupación y comunicación, y en el puntaje global, respecto a quienes usaban múltiples inyecciones de insulina como tratamiento (p=0,0363).Conclusiones. En nuestros pacientes, un mejor control metabólico (medido por el valor de HbA1c) y el uso de microinfusora contribuyen a una percepción de mejor calidad de vida.

322 Background: After first-line treatment with gemcitabine-based regimen, there is not standard second-line chemotherapy for patients with metastatic gallbladder cancer (GB) and cholangiocarcinoma (CC). The aim of this study was to evaluate the activity, progression free survival (PFS), and overall survival (OS) of the FOLFOX4 regimen as second-line chemotherapy. Methods: We performed a retrospective analysis of all metastatic GB and CC that were treated with gemcitabine (Gem) based first-line chemotherapy, and second-line FOLFOX4 chemotherapy (oxaliplatin 85 mg/m2, day 1; leucovorin 200 mg/m intravenously, days 1 and 2; 5-fluorouracil 400 mg/m intravenous bolus, days 1 and 2; and 600 mg/m in 22 h intravenously, continuous infusion days 1 and 2; every 14 days) in our Institution since 2003. The response rate was evaluated every 2 to 4 months according to the RECIST criteria, and PFS/OS were calculated according to the Kaplan-Meier method. Results: Eighty patients (n=40 GB; n=40 CC) received Gem based first-line treatment (42 Gem alone; 38 Gem + platinum). There was no survival difference between Gem or Gem+platinum treatment (161 versus 254 days; p=0.09). Response rate (complete and partial response) was 16% and stable disease was 31%. Ca19-9 at presentation was predictive for OS: <100U/ml =403 days versus >100 U/ml =223 days (p=0.025). Eighteen patients (22%; n=10 GB and n=8 CC) were treated with second-line FOLFOX4. In the second-line group, stable disease was achieved in 4 patients (22%), and 11 patients progressed (66%). The median DFS and OS were 96 and 138 days respectively. The median OS for patients that received one versus both lines of chemotherapy were 161 versus 400 days (p=0.009). Conclusions: FOLFOX4 has activity in second line chemotherapy for patients with metastatic GB and CC. A prospective study is warranted to confirm this observation.
