A series of mononuclear boron halides of the type LBX2 (LH = N-phenyl-3,5-di-tbutylsalicylaldimine,X = Cl (2), Br (3)) and LBX ( LH2 = N-(2-hydroxyphenyl)-3,5-di-tbutylsalicylaldimine,X = Cl (7), Br (8); LH2 = N-(2-hydroxyethyl)-3,5-di-tbutylsalicylaldimine,X = Cl (9), Br (10); LH2= N-(3-hydroxypropyl)-3,5-di-tbutylsalicylaldimine,X = Cl (11), Br (12)) were synthesized from their borate precursorsLB(OMe)2 (1) (LH = N-phenyl-3,5-di-t-butylsalicylaldimine ) and LB(OMe) (LH2 = N-(2-hydroxyphenyl)-3,5-di-t-butylsalicylaldimine (4), N-(2-hydroxyethyl)-3,5-di-tbutylsalicylaldimine(5), N-(3-hydroxypropyl)-3,5-di-t-butylsalicylaldimine (6)). Theborate precursors, 1, 4 - 6, in turn, were prepared by refluxing the corresponding ligandsLH or LH2 with excess B(OMe)3. The boron halide compounds were air- and moisturesensitiveand compound 7 on hydrolysis gave the oxo-bridged compound 13 thatcontained two seven-membered boron heterocycles. The boron halide compoundsdealkylated trimethyl phosphate in stoichiometric reactions to produce methyl halide andunidentified phosphate materials. Compounds 8 and 12 were found to be the mosteffective dealkylating agents. Compound 8 on reaction with t-butyl diphenyl phosphinateproduced a unique boron phosphinate compound LB(O)OPPh2 (14) containing a terminalphosphinate group. Compounds 1-14 were characterized by 1H, 13C, 11B, 31P NMR, IR,MS, EA and MP. Compounds 5, 6, 11, 12 and 13 were also characterized by singlecrystalX-ray diffraction.The alkane elimination reaction between Salen(tBu)H2 ligands and diethylaluminumbromide was used to prepare the four Salen aluminum bromide compounds,salen(tBu)AlBr (15) (salen = N,N'-ethylenebis(3,5-di-tert-butylsalicylideneimine)),salpen(tBu)AlBr (16) (salpen = N,N'-propylenebis(3,5-di-tert-butylsalicylideneimine)),salben(tBu)AlBr (17) (salben = N,N'-butylenebis(3,5-di-tert-butylsalicylideneimine)) andsalophen(tBu)AlBr (18) (salophen = N,N'-o-phenylenenebis(3,5-di-tertbutylsalicylideneimine)).The compounds contained five-coordinate aluminum either in adistorted square planar or a trigonal bipyramidal environment. The bromide group inthese compounds could be displaced by triphenylphosphine oxide or triphenyl phosphateto produce the six-coordinate cationic aluminum compounds [salen(tBu)Al(Ph3PO)2]Br(19), [salpen(tBu)Al(Ph3PO)2]Br (20), [salophen(tBu)Al(Ph3PO)2]Br (21) and[salophen(tBu)Al{(PhO)3PO}2]Br (22). All the compounds were characterized by 1H,13C, 27Al and 31P NMR, IR, mass spectrometry and melting point. Furthermore,compounds 15, 16, 17, 18, 20, 21 and 22 were structurally characterized by single-crystalX-ray diffraction. Compounds 15, 17 and 18 dealkylated a series of organophosphates instoichiometric reaction by breaking the ester C–O bond. Also, they promoted thedealkylation reaction between trimethyl phosphate and added boron tribromide.Stoichiometric reaction of compound 15 with trimethyl phosphate produced thealuminophosphinate compound salen(tBu)AlOP(O)Ph2 (23). Compound 16 on reactionwith tributyl phosphate produced the aluminophosphate compound[salpen(tBu)AlO]2[(BuO)2PO]2 (24). Compounds 23 and 24 were characterized by singlecrystalX-ray diffraction and spectroscopically.

Abstract:
ЦЕЛЬ: оценить динамику показателей углеводного обмена, липидного спектра крови, показателей артериального давления, массы тела, частоты гипогликемий, навыков по технике инъекций инсулина, поведения у больных сахарным диабетом 1 типа (СД1) и 2 типа на инсулине (СД2), прошедших групповое обучение по структурированной программе в 2021 и 2022 гг. МАТЕРИАЛЫ И МЕТОДЫ: в исследование было включено 293 пациента, из них с СД1 161 чел. в возрас- те 44,4±5.7 лет и с СД2 на инсулине 132 чел. в возрасте 59,4±8.7 лет. Все пациенты получали терапию инсу- лином более 1 года. Средняя длительность инсулинотерапии у пациентов с СД1 10.8±2,1 лет, у пациентов с СД2 на инсулине - 9.9±1,7 лет. Большинство пациентов с СД2 получали только инсулин (72%), остальные (28%) - пероральные сахароснижающие препараты и инсулин. У 47% пациентов с СД1 и 75% с СД2 на инсулине регистрировалась гиперлипидемия. Средний уровень ЛПНП составил 3.2±1.2 ммоль/л у пациентов СД 1 и 3.8±1.4 ммоль/л у пациентов с СД2 на инсулине. 16% па- циентов с СД1 и 24% больных с СД2 на инсулине получали гиполипидемическую терапию. У 32% пациентов с СД1 и 78% с СД2 на инсулине была артериальная гипертензия. Регулярный прием гипотензивных препаратов осуществляли 50% больных с СД1 и 40% пациентов с СД2 на инсулине. Оценка параметров проводилась исходно и через 6 месяцев после окончания обучения. РЕЗУЛЬТАТЫ: в процессе наблюдения отмечено достоверное снижение HbAc у пациентов с СД 1 типа с 8.9±0.82% до 7.6±0.8% (p<0.05), у пациентов с СД2 на инсулине с 9.4±0.9% до 8.1±1.1% (p<0.05). Исходно 22% больных СД1 и 10% больных СД2 на инсулине имели уровень HbAc <7%. Через 6 месяцев после обучения 56% пациентов с СД1 и 39% больных СД2 на инсулине имели HbAc <7%. У пациентов с СД1 не было отмечено изменение ИМТ. У пациентов с СД2 на инсулине наблюдалось снижение ИМТ с 36.60±1.9 до 33.80±2.0 (p<0.05). Через 6 месяцев средний уровень ЛПНП составил 1.8±0.6 ммоль/л у пациентов СД1 и 2.0±0.8 ммоль/л у пациентов с СД2 на инсулине. 76% пациентов с СД1 и 80% больных с СД2 на инсулине получали гипо- липидемическую терапию. Целевые показатели АД были достигнуты у 90% пациентов с СД1 и 86% боль- ных с СД2 на инсулине. Регулярный прием гипотензивных препаратов осуществляли 90% больных с СД1 и 85% - с СД2 на инсулине. Число пациентов, осуществляющих самоконтроль гликемии с частотой 4 раза в день увеличился с 113 (70%) до 150 чел. (93%) среди пациентов с СД1 и с 13 (10%) до 90 чел. (68%) в группе пациентов с СД2 на инсулине. Увеличилось число пациентов, самостоятельно корректирующих дозы инсулина и правиль- но купирующих гипогликемии. Самостоятельно мониторировали АД 90% пациентов с СД1 и 86% – с СД2 на инсулине. ВЫВОДЫ: структурированное обучение позволяет ликвидировать пробелы в знаниях и навыках боль- ного, способствует увеличению доли пациентов, достигших целей лечения, позволяет повысить привер- женность к выполнению лечебных действий, связанных с диабетом, и сопутствующими заболеваниями. Abstract:
ЦЕЛЬ: оценить структурно-функциональное состояние щитовидной железы (ЩЖ) в период госпитализации с COVID-19, а также изучить особенности тиреоидных заболеваний, впервые выявленных в течение года после перенесенной коронавирусной инфекции. Материалы и методы: под наблюдением находились 2 выборки пациентов. В 1-ю вошло 40 госпитализированных больных COVID-19; в стационаре и в динамике через 1-3 месяца у них был исследован тиреоидный статус. Во вторую выборку вошло 250 лиц с впервые выявленным заболеванием ЩЖ, в том числе 73 без (контроль) и 177 с COVID-19 в анамнезе (основная группа, включая 92 пациента с давностью COVID-19 не более и 85 - более 6 месяцев). РЕЗУЛЬТАТЫ: в 1-й выборке в ходе госпитализации по поводу COVID-19 13 из 40 (32,5%) пациентов имели признаки синдрома эутиреоидной патологии; через 1-3 месяца у 11 тиреоидный статус нормализовался, у 2-х (5,0%) выявлялся тиреотоксикоз и было диагностировано аутоиммунное заболевание ЩЖ. Во 2-й выборке основная группа не отличалась от контрольной по частоте аутоиммунного тиреоидита, болезни Грейвса, узлового зоба (р>0,05 по всем показателям), но в ней чаще выявлялись тиреоидные дисфункции (р=0,037). В контроле и в подгруппах больных с давностью COVID-19 не более и более полугода доля лиц с тиреотоксикозом составила соответственно 7 (9,6%), 22 (23,9%) и 8 (9,4%), р=0,009; с диффузной неоднородностью ЩЖ по УЗИ - 33 (45,2%), 53 (57,6%) и 31(36,5%), р=0,019; с наличием гипоэхогенных зон по УЗИ - 26 (35,6%), 44 (47,8%), 25(29,4%), р=0,037. Давность COVID-19 обратно коррелировала с наличием тиреоидной дисфункции (R=-0,21, р=0,008). В когорте перенесших COVID-19 отдельно сопоставили подгруппы лиц с вакцинацией и без нее. Пациенты, которые не были привиты и переболели COVID-19, чаще страдали подострым тиреоидитом, имели тиреодные дисфункции и ультразвуковые признаки структурных нарушений ЩЖ (р<0,05 по всем показателям). Выводы: около трети нуждающихся в госпитализации больных со среднетяжелым и тяжелым COVID-19 имеют признаки синдрома эутиреоидной патологии; у 5% выявляется стойкий тиреотоксикоз в постгоспитальном периоде. Заболевания ЩЖ, впервые выявленные в постковидном периоде, чаще ассоциируются с нарушениями функции ЩЖ (прежде всего, с тиреотоксикозом). Влияние COVID-19 на течение тиреоидной патологии очевидно в первые 6 месяцев после перенесенной инфекции. Предшествующая вакцинация минимизирует риск развития дисфункции ЩЖ в постковидном периоде.
