Abstract:
<p>In September 2011, the National Collegiate Athletic Association (NCAA) announced the <em>Policy on Transgender Inclusion</em>. It provides guidelines for transgender student athletes to participate in sex-separated athletic teams according to their gender identity. The <em>2012 LGBTQ National College Athlete Report</em>, the first of its kind, provided information to help serve gay, lesbian, bisexual, transgender, and queer (GLBTQ) students. Although the <em>Policy on Transgender Inclusion</em> has been around since 2011 and the <em>2012 Report</em> provided insights, the extent by which best practices have been implemented has not been adequately explored. This study examined the relationship of athletic directors’ leadership frames to transgender inclusion policies at institutions with NCAA athletics. Athletic directors from active member NCAA schools were contacted: 340 in Division I; 290 in Division II; and 436 in Division III. Leadership was examined according to the Multi-frame Model for Organizations in addition to Intersectionality Theory. The human resource frame was the most common and the political frame was the least. There were no statistically significant differences among NCAA Divisions or between private and public institutions. Although athletic directors acknowledged transgender policies and were aware of the legal parameters, lack of policy presence was prevalent on campuses.</p>

Abstract:
Background This study assessed the feasibility and acceptability of pharmacy and home-based sexually transmissible infection (STI) screening as alternate testing venues among emergency contraception (EC) users. Methods: The study included two phases in February 2011–July 2012. In Phase I, customers purchasing EC from eight pharmacies in Manhattan received vouchers for free STI testing at onsite medical clinics. In Phase II, three Facebook ads targeted EC users to connect them with free home-based STI test kits ordered online. Participants completed a self-administered survey. Results: Only 38 participants enrolled in Phase I: 90% female, ≤29 years (74%), 45% White non-Hispanic and 75% college graduates; 71% were not tested for STIs in the past year and 68% reported a new partner in the past 3 months. None tested positive for STIs. In Phase II, ads led to >45000 click-throughs, 382 completed the survey and 290 requested kits; 28% were returned. Phase II participants were younger and less educated than Phase I participants; six tested positive for STIs. Challenges included recruitment, pharmacy staff participation, advertising with discretion and cost. Conclusions: This study found low uptake of pharmacy and home-based testing among EC users; however, STI testing in these settings is feasible and the acceptability findings indicate an appeal among younger women for testing in non-traditional settings. Collaborating with and training pharmacy and medical staff are key elements of service provision. Future research should explore how different permutations of expanding screening in non-traditional settings could improve testing uptake and detect additional STI cases.

"The Effect of Trabecular Aspiration on Intraocular Pressure, Medication and the Need for Further Glaucoma Surgery in Eyes with Pseudoexfoliation Glaucoma." Abstract:
Purpose: To investigate whether trabecular aspiration (TA) has an effective medium-term intraocular pressure (IOP)-lowering and medication-saving effect in patients with pseudoexfoliation glaucoma (PEG). In addition, a subgroup analysis of patients with or without a previous trabeculectomy was performed. Methods: Records of 290 consecutive eyes with PEG that underwent TA between 2006 and 2012 at the Department of Ophthalmology, Mainz, Germany, were retrospectively analyzed with a follow-up period of 3 years. The main outcomes were IOP and the need for further medical treatment. Results: Of the 290 eyes with PEG that received TA, 167 eyes from 127 patients met the inclusion criteria. Among these eyes, 128 received TA and cataract surgery (Phaco-TA) without having had a trabeculectomy (group I) before, 29 had Phaco-TA after a previous trabeculectomy (group II) and 10 underwent stand-alone TA after a previous trabeculectomy (group III). In the whole cohort, the median IOP decreased immediately after TA and remained significantly lower compared to the baseline throughout the period of 36 months. Likewise, the median number of antiglaucoma drugs was reduced over the whole period. At the same time, in group I, the median IOP and the number of antiglaucoma drugs were reduced over 36 months. In contrast, in the post-trabeculectomy groups (group II and III), the median IOP and the number of antiglaucoma drugs could not be reduced. While most of the patients that received Phaco-TA with or without a previous trabeculectomy (group I and II) did not require further surgical intervention during the follow-up period, almost all patients receiving stand-alone TA after a previous trabeculectomy (group III) needed surgical therapy, most of them between the second and the third year following TA. Conclusions: Phaco-TA has an effective medium-term pressure-lowering and medication-saving effect, especially in patients without a previous trabeculectomy. In trabeculectomized eyes, the effect of TA is limited but still large enough to delay more invasive surgical interventions in some patients.

Abstract:
Therese Martin, ed., Reassassing the Roles of Women as ‘Makers’ of Medieval Art and Architecture, 2012). Abstract:
290 Background: In patients with gastric cancer (GC), the most common double cancer is colorectal cancer (CRC). However, the meaning of screening colonoscopy has not been established. The aim of this retrospective study was to evaluate the useful of screening colonoscopy in preoperative patients with GC. Methods: This study included 689 patients who received screening colonoscopy before gastric surgery between 2012 and 2016. Multivariate analysis using logistic regression model was conducted to elucidate independent risk factors of CRC. Then, we investigated the clinicopathological factors for CRC. Results: Colorectal adenomas and CRC were observed in 315 patients (46%) and 37 patients (5.4%), respectively. The clinical T classification of the CRC were as follows; Tis: 24 patients (65%), T1: 8 patients (21%), T2: 2 patients (6%), and T3: 3 patients (8%). In multivariate analysis, male (OR 5.04, 95% C.I. 1.29-19.6, p = 0.020) was revealed as risk factor for affecting CRC. The treatments for CRC were as follows; EMR was performed in 27 patients, simultaneous resection with GC was performed in 9 patients, resection after gastrectomy was performed in 1 patient, respectively. Pathological stage of CRC was as follows; Stage 0: 24 patients, Stage I: 10 patients, and Stage IIA: 3 patients, respectively. As for the patients who underwent surgery for CRC, all of them received radical colectomy. No patient died for CRC who received colonoscopy before gastric surgery. Conclusions: Screening colonoscopy is useful for GC patients. Because most of the synchronous CRC were found early stage and curatively treated.

Abstract:
The purpose of this thesis is to investigate the new real estate brokerage Act and to see whether the main intent, to strengthen the consumer protection, has been fulfilled through the implementation of the 2011 Act. The real estate agent legislation is made to protect both the buyer and seller throughout the property transaction. Although the intent is to protect the consumer, it is not unusual that disputes arises. To avoid disputes concerning the property transaction, the 2011 legislation opens up for greater transparency in the mediation process. The protection for the consumer has been considerably stronger since the 2011 legislation was implemented. The author is of the opinion that some minor modifications would further enhance consumer protection

Abstract:

Abstract:
AbstractEmissions trading schemes (ETS) have become popular as a policy instrument to tackle climate change. This chapter analyses the decision to deploy carbon markets and their interaction with other instruments in Mexico’s climate policy. Instrument selection has been thoroughly explored in the regulation and public policy literature (Kern et al. in Res Policy 48, 2019; Capano and Lippi in Policy Sci 50(2):269–293, 2016; Wurzel et al. in German Policy Studies 9:21–48, 2013; Harker et al. in Climate Policy 17(4):485–500, 2017; Baldwin et al. in Understanding regulation, Oxford University Press, 2012; Jordan et al. in Policy instruments in practice. Oxford handbooks online 536–549, 2011), but its application to carbon markets is mainly focused on environments such as Europe, the US and, more recently, China. The decision to adopt an ETS relies not only on specific characteristics of each instrument but also on institutional constraints and messy political considerations. A combination of preferences and institutional factors affect the choice of instruments, and the ultimate decision must be legitimate and instrumental for each context. I analyse the considerations involved in the deployment of the ETS pilot project, looking at its distinctive characteristics and those it shares with other available instruments, as well as the requirements for its implementation.

AbstractWe work in the ruins of a world that has produced those ruins (Sauvé, 2017; Tsing in The mushroom at the end of the world: On the possibility of life in capitalist ruins. Princeton University Press, 2015), this time often referred to as the Anthropocene, science educators and researchers have been called to break with post-positivism, dualisms, and reductionism to settle on new onto-epistemological grounds (Bazzul and Kayumova,.Educational Philosophy and Theory 48:284–299, 2016; Haraway, D. (2016). Staying with the trouble: Making kin in the Chthulucene. Duke University Press.; Lather & St. Pierre in International Journal of Qualitative Studies in Education 26:629–633, 2013). One promising proposition lies in ontologies of process and epistemologies that expand to encompass affect with new combinations of knowing/experiencing/researching that honor the more-than-human world we need to navigate (Manning, E. (2013). Always more than one: Individuation’s dance. Duke University Press.; Muraca,.Environmental Values 20:375–396, 2011). Abstract:
AbstractCritical or extreme atmospheric conditions which could result in flood disasters are important output for numerical weather forecast. This research applied thermodynamic variables to investigate the environment of two flood scenarios in West Africa as captured by the Tropical Rainfall Measurement Mission (TRMM) satellite. Results from the two case studies of flood events, in (i) Burkina Faso and (ii) Nigeria savannah, investigated in this research work, indicated that the September 1st 2009 flood, which was as a result of a single volumetric rainfall event of 408,070.60 ((mm/h)*km2) with 65% convective region in Burkina Faso, was initiated by interactions between extremely large lower tropospheric wind shear and cold pool dynamics. The case of the Nigeria savannah floods between July and September, 2012, was triggered by both continuous rainfall and release of water from the lagdo dam in Cameroon, which affected most of the communities in the river Benue axis. The continuous rainfalls were found to be as a result of extremely high convergence of moisture in the river Benue axis at different locations and periods. One of such rainfall events, as captured by TRMM satellite during September 29, 2012 in the Nigeria rainforest zone, indicated that the volumetric rainfall is 351,310.9 ((mm/h)*km2) with only 34% convective portion. From these results, it can be deduced that a combination of thermodynamic environmental variables, volume rainfall, and other satellite-derived convective parameters could provide important information for flood forecasting.

Abstract:
W opracowaniu podjęto próbę dokonania przeglądu historycznych, współczesnych oraz identyfikacji przewidywanych kierunków rozwoju przestrzennego Łodzi postrzeganych z perspektywy strategicznego podejścia do planowania i zarządzania miastem.

Abstract:
Abstract Human’s motion and its mechanisms had become interesting in the last years, where the medecine’s field search for rehabilitation methods for handicapped persons. Other fields, like sport sciences, professional or military world, search to distinguish profiles and ways to train them with specific purposes. Besides, recent findings in neuroscience try to describe these mechanisms from an organic point of view. Until now, different researchs had given a model about control motor that describes how the union between the senses’s information allows adaptable movements. One of this sense is the proprioception, the sense which has a quite big factor in the orientation and position of the body, its members and joints. For this reason, research for new strategies to explore proprioception and improve the theories of human motion could be done by three different vias. At first, the sense is analysed in a case-study where three groups of persons are compared in a controlled enviroment with three experimental tasks. The subjects belong to each group by the kind of sport they do: sedentary, normal sportsmen (e.g. athletics, swimming) and martial sportmen (e.g. karate, judo). They are compared thinking about the following hypothesis: “Martial Sportmen have a better proprioception than of the other groups’s subjects: It could be due to the type of exercises they do in their sports as empirically, a contact sportsman shows significantly superior motor skills to the members of the other two groups. The second via are records from encephalogram (EEG) while the experimental tasks are doing. These records are analised a posteriori with a set of processing algorithms to extract characteristics about brain’s activity of the proprioception and motion control. Finally , the study tries to integrate graphic tools to make easy to understand final scientific results which allow us to explore the brain activity of the subjects through easy interfaces (e.g. space-time events, activity intensity, connectivity, specific neural netwoks or anormal activity). In the future, this application could be a complement to assist doctors, researchers, sports center specialists and anyone who must improve the health and movements of handicapped persons. Abstract:
The reduction of CO2 concentration in our atmosphere consists in a big challenge for researchers, who are trying to explore novel technologies in order to transform CO2 into high added-value products. CO2 conversion into methane using microwave plasma (MWP) manifests as a very promising solution due to the ease of transport of methane and its storage. Microwave plasma represents a source of high-energy electrons, active ions and radicals that enhance or enable chemical reaction. It can be supplied by electricity generated from renewable resources. Then, MWP does not require any electrode to be generated and thus, the cost of those electrodes and of maintenance is reduced compared to glow discharge or DBD plasmas. MWP also can be generated over wide range of pressure (between 10 mbar-1bar). In addition, in the case of MWP, more electrons and active species are produced in comparison with other type of plasma[1–4]. MWP is a very suitable medium for this chemical reaction and leads to an efficient dissociation of CO2. The catalytic reduction of CO2 with H2 using MWP has been investigated in this work and the synergetic effects between the plasma and several catalysts were studied. First, the reaction was carried out without any catalysts and the effect of CO2/H2 ratio, total flow rate and input energy were evaluated. Then, a microwave generated plasma process was coupled with several Nickel catalysts that we prepared and characterized [5] in order to lead the reaction into methane formation. Multiple configurations were studied by changing the position of the catalyst bed. Obtained results were compared with conventional catalytic tests made with the same catalysts. It was found that the conversion of CO2 and energy efficiency increased using plasma assisted catalytic methanation of CO2 in comparison with conventional process. Operating conditions were studied in order to optimize methane production and energy efficiency of Plasma-catalytic process. References Qin, Y., G. Niu, X. Wang, D. Luo, Y. Duan, J. Abstract:
As promising material for gas storage applications, MIL-101(Cr) can further be modified by doping with alkali metal (Li+, Na+, K+) ions. However, the doping concentration should be optimized below 10% to improve the methane adsorption. This article presents (i) the synthesis of MIL-101 (Cr) Metal Organic Frameworks, (ii) the characterization of the proposed doped adsorbent materials by X-ray Diffraction, Scanning Electron Microscopy, N2 Adsorption, Thermo-Gravimetric Analyzer, and (iii) the measurements of methane uptakes for the temperatures ranging from 125 K to 303 K and pressures up to 10 bar. It is found that the Na+ doped MIL-101(Cr) exhibits CH4 uptake capacity of (i) 295 cm3/cm3 at 10 bar and 160 K and (ii) 95 cm3/cm3 at 10 bar at 298 K. This information is important to design adsorbed natural gas (ANG) storage tank under ANG-LNG (liquefied natural gas) coupling conditions.

Abstract:
BackgroundCare recipients in care and welfare are increasingly presenting themselves with complex needs (Huber et al., 2016). An answer to this is the integrated organization of care and welfare in a way that personalized care is the measure (Topol, 2016). The reality, however, is that care and welfare are still mainly offered in a standardized, specialized and fragmented way. This imbalance between the need for care and the supply of care not only leads to under-treatment and over-treatment and thus to less (experienced) quality, but also entails the risk of mis-treatment, which means that patient safety is at stake (Berwick, 2005). It also leads to a reduction in the functioning of citizens and unnecessary healthcare cost (Olsson et al, 2009).Integrated CareIntegrated care is the by fellow human beings experienced smooth process of effective help, care and service provided by various disciplines in the zero line, the first line, the second line and the third line in healthcare and welfare, as close as possible (Ettema et al, 2018; Goodwin et al, 2015). Integrated care starts with an extensive assessment with the care recipient. Then the required care and services in the zero line, the first line, the second line and / or the third line are coordinated between different care providers. The care is then delivered to the person (fellow human) at home or as close as possible (Bruce and Parry, 2015; Evers and Paulus, 2015; Lewis, 2015; Spicer, 2015; Cringles, 2002).AimSupport societal participation, quality of live and reduce care demand and costs in people with complex care demands, through integration of healthcare and welfare servicesMethods (overview)1. Abstract:
The number of areas of society that are being digitised continues to increase and so Internet offers are becoming increasingly important. This development also affects social work, whose specialists meanwhile see a trend to expand the internet-related offerings. This has been further strengthened in germany by the lockdowns since March 2020. This poses a particular challenge for youth workers, as their addressees, the so-called digital natives, did not experience the time before digitization and use the Internet as a matter of course to cultivate existing friendships and meet new people. Today’s young people no longer distinguish between offline and online, they are onlife. However, even in this age group there is every level of the digital divide, because not all have the same equipment, fast internet access or the necessary application skills.Up to now, they have only had limited success in switching to hybrid offers. In addition to the requirements of the recipients, this is partly due to the technical scepticism of the social work-ers, the relatively one-sided orientation of further training and the lack of equipment (Klein-schmidt/Scheibe). All this is still incomplete and does not apply equally to all social workers. This unfinished list shows that this phenomenon is interrelated and cannot be described and explained solely by describing the skills of the social workers. At this point, the actor-network theory (ANT) could be a useful theoretical perspective, because it allows the entanglement and reciprocal influences of human and non-human components in a network to be visible and then analysed. I take up the previous considerations on the use of the ANT in contexts of internet-related social work and think about them along the question of how a case study could be structured in which the individual situation of youth workers in relation to the respective conditions of success of their internet-related activities can be explored and subsequently analysed. To answer the question, I first outline the basic elements of the ANT, and then explore the potential of a thematic examination of specialists in case studies on object-related theory buil-ding. Based on this, I present a possible research design. Finally, I summarize the relevant findings and discuss ideas for further development. Abstract:
The influence of Raskila bioorganic fertilisers on the productivity of conventional winter wheat ‘Olivin’, was investigated in order to stimulate and save synthetic herbicide Arrat and fungicide Tango Super for spring spraying. Scheme of treatment: 1. Control; 2. Winter wheat sprayed (BBCH 20-29) with fertilizer Raskila 3 l ha-1; 3. Winter wheat sprayed (BBCH 20-29) with Arrat 0.2 kg ha-1+Tango super 1.5 l ha-1; 4. Winter wheat sprayed (BBCH 20-29) with Arrat 0.2 kg ha-1+Tango super 1.5 l ha-1+Raskila 3.0 l ha-1; 5. Winter wheat sprayed (BBCH 20-29) with Arrat 0.1 kg ha-1+Tango super 0.75 l ha-1+Raskila 3.0 l ha-1. The best result in the period of 2014-2016 was received after application of the following combination in spring: Arrat + Tango super + Raskila. This combination allows to reduce the rate of pesticides in half (50%), the differences compared to control are significant, statistically reliable. A statistically significant increase in the following winter wheat ‘Olivin’ quality parameters was found: protein 13.1-14.8%, gluten 24.3-29.7%, number of falls 228-292 s, starch 65.7-70.0%. Application of Raskila fertilizers has raised the grain quality class. The best results were in variants 3 and 4, where the I class of grain quality was achieved. Combination of Raskila fertilisers and pesticides: herbicide Arrat and fungicide Tango super statistically significantly increased the following winter wheat ‘Olivin’ characteristics - plant height 101.2-104.2 cm, ear length 6.9-7.1 cm, grain number per ear 28,96- 30.02, grain yield 6.71-7.03 t ha-1. Application of Raskila fertilizer 1.0 l ha-1 and herbicide Arrat 0.1 kg ha-1 decreased the number of weeds from 62.5 to 57.6 units per m2 and the weed weight decreased from 41.30 to 33.70 g m2. Stronger wheat crop overshadowed weeds better. Combination of Raskila and Tango super reduced the prevalence and severity of diseases in winter wheat such as Septoria spp., Pyrenophora tritici-repentis, Erysiphe graminis, powdery rust (Puccinia recondita).

Abstract:

Abstract:
Ekologisk status beskrivs enligt bedömningsgrunden VIX (Vattendragsindex). Sammanlagt elfiskas 102 provplatser i 29 vattendrag inom programmet.

Näslund, Joacim, Björn Ardestam, Malin Hällbom, Ola Renman, and Thomas Staveley. Båtelfiske i lugnflytande åar 2021 – metod, resultat och erfarenheter. Department of Aquatic Resources, Swedish University of Agricultural Sciences, 2023. http://dx.doi.org/10.54612/a.76acmsrd73.

Full text
Abstract:

Full text
Abstract:

Abstract:
As a student undertaking a Longitudinal Integrated Clerkship (LIC)1 based in a GP practice in a rural community in the North of Scotland, I have been lucky to be given responsibility and my own clinic lists. Every day I conduct consultations that change my practice: the challenge of clinically applying the theory I have studied, controlling a consultation and efficiently exploring a patient's problems, empathising with and empowering them to play a part in their own care2 – and most difficult I feel – dealing with the vast amount of uncertainty that medicine, and particularly primary care, presents to both clinician and patient. I initially consulted with a lady in her 60s who attended with her husband, complaining of severe lower back pain who was very difficult to assess due to her pain level. Her husband was understandably concerned about the degree of pain she was in. After assessment and discussion with one of the GPs, we agreed some pain relief and a physio assessment in the next few days would be a practical plan. The patient had one red flag, some leg weakness and numbness, which was her ‘normal’ on account of her multiple sclerosis. At the physio assessment a few days later, the physio felt things were worse and some urgent bloods were ordered, unfortunately finding raised cancer and inflammatory markers. A CT scan of the lung found widespread cancer, a later CT of the head after some developing some acute confusion found brain metastases, and a week and a half after presenting to me, the patient sadly died in hospital. While that was all impactful enough on me, it was the follow-up appointment with the husband who attended on the last triage slot of the evening two weeks later that I found completely altered my understanding of grief and the mourning of a loved one. The husband had asked to speak to a Andrew MacFarlane Year 3 ScotGEM Medical Student 2 doctor just to talk about what had happened to his wife. The GP decided that it would be better if he came into the practice - strictly he probably should have been consulted with over the phone due to coronavirus restrictions - but he was asked what he would prefer and he opted to come in. I sat in on the consultation, I had been helping with any examinations the triage doctor needed and I recognised that this was the husband of the lady I had seen a few weeks earlier. He came in and sat down, head lowered, hands fiddling with the zip on his jacket, trying to find what to say. The GP sat, turned so that they were opposite each other with no desk between them - I was seated off to the side, an onlooker, but acknowledged by the patient with a kind nod when he entered the room. The GP asked gently, “How are you doing?” and roughly 30 seconds passed (a long time in a conversation) before the patient spoke. “I just really miss her…” he whispered with great effort, “I don’t understand how this all happened.” Over the next 45 minutes, he spoke about his wife, how much pain she had been in, the rapid deterioration he witnessed, the cancer being found, and cruelly how she had passed away after he had gone home to get some rest after being by her bedside all day in the hospital. He talked about how they had met, how much he missed her, how empty the house felt without her, and asking himself and us how he was meant to move forward with his life. He had a lot of questions for us, and for himself. Had we missed anything – had he missed anything? The GP really just listened for almost the whole consultation, speaking to him gently, reassuring him that this wasn’t his or anyone’s fault. She stated that this was an awful time for him and that what he was feeling was entirely normal and something we will all universally go through. She emphasised that while it wasn’t helpful at the moment, that things would get better over time.3 He was really glad I was there – having shared a consultation with his wife and I – he thanked me emphatically even though I felt like I hadn’t really helped at all. After some tears, frequent moments of silence and a lot of questions, he left having gotten a lot off his chest. “You just have to listen to people, be there for them as they go through things, and answer their questions as best you can” urged my GP as we discussed the case when the patient left. Almost all family caregivers contact their GP with regards to grief and this consultation really made me realise how important an aspect of my practice it will be in the future.4 It has also made me reflect on the emphasis on undergraduate teaching around ‘breaking bad news’ to patients, but nothing taught about when patients are in the process of grieving further down the line.5 The skill Andrew MacFarlane Year 3 ScotGEM Medical Student 3 required to manage a grieving patient is not one limited to general practice. Patients may grieve the loss of function from acute trauma through to chronic illness in all specialties of medicine - in addition to ‘traditional’ grief from loss of family or friends.6 There wasn’t anything ‘medical’ in the consultation, but I came away from it with a real sense of purpose as to why this career is such a privilege. We look after patients so they can spend as much quality time as they are given with their loved ones, and their loved ones are the ones we care for after they are gone. We as doctors are the constant, and we have to meet patients with compassion at their most difficult times – because it is as much a part of the job as the knowledge and the science – and it is the part of us that patients will remember long after they leave our clinic room. Abstract:
Informantene ble rekruttert via to ulike interesseorganisasjoner.

Seiler, Andreas, Aina Winsvold, Mattias Olsson, Svein Morten Eilertsen, and Carolin Berndt. Swedish University of Agricultural Sciences, 2022. http://dx.doi.org/10.54612/a.7pbjcv25mc.

Full text
Abstract:
I juni ble DASS-enheter tatt i bruk langs Gjøvikbanen.

Esteve, Albert, Andrés Castro, and Federica Becca. Family Change in Latin America: Schooling and Labor Market Implications for Children and Women. Inter-American Development Bank, September 2023. http://dx.doi.org/10.18235/0005145.

Full text
Abstract:
This chapter provides an account of the major family transformations that occurred in recent decades across Latin American and Caribbean countries and examines the implications of such transformations for childrens school attendance and progress and womens labor force participation. Latin American and Caribbean families and households have undergone substantial changes in recent years while keeping some of their distinctive features unchanged (Esteve et al., 2022; Esteve & Florez-Paredes, 2018a; Juárez & Gayet, 2014). This combination of stability and change has had profound transformations in the family status in which women raise their children and the family context in which children are raised. We refer to family context as the combination of womens marital status and the type of households in which children reside. We combine references to the literature and own calculations based on Latin American and Caribbean population census samples, available at the Integrated Public-use Microdata Series International (IPUMS) (Minnesota Population Center, 2020). We use data from 25 countries based on the most recent census microdata and, in some instances, historical samples starting in the late 1950s (see Appendix 1). The chapter is organized as follows. First, we document trends in family change and childrens status. To illustrate family change empirically, we focus on women aged 25 to 29 and children aged 7 to 16. For reasons that will be displayed during the paper, these groups offer a reliable overview of major transformations with the advantage of avoiding overlapping cohorts when data are analyzed over time. Variations by educational attainment are also examined to illustrate the role of inequality of opportunities in family change. Second, we focus on the implications of family forms on children's school attendance and progress and women's participation in the labor market. In the absence of tailored indicators about progress in cognitive and non-cognitive skills, school attendance and progress are standard indicators of early human capital accumulation (UNESCO, 2022). We examine these two outcomes among more than 15 million children included in the IPUMS-I census samples. For women, we examine the degree of participation in the labor market (n 16 million).

Abstract:
Background An estimated 292 million people are living with chronic hepatitis B virus (HBV) infection globally, including 223,000 people in Australia. HBV diagnosis and linkage of people living with HBV to clinical care is suboptimal in Australia, with 27% of people living with HBV undiagnosed and 77% not receiving regular HBV clinical care. This systematic review aimed to characterize population-level interventions implemented to enhance all components of HBV care cascade and analyse the effectiveness of interventions. Review questions Question 1: What population-level interventions, programs or policy approaches have been shown to be effective in reducing the incidence of hepatitis B; and that may not yet be fully rolled out or evaluated in Australia demonstrate early effectiveness, or promise, in reducing the incidence of hepatitis B? Question 2: What population-level interventions and/or programs are effective at reducing disease burden for people in the community with hepatitis B? Methods Four bibliographic databases and 21 grey literature sources were searched. Studies were eligible for inclusion if the study population included people with or at risk of chronic HBV, and the study conducted a population-level interventions to decrease HBV incidence or disease burden or to enhance any components of HBV care cascade (i.e., diagnosis, linkage to care, treatment initiation, adherence to clinical care), or HBV vaccination coverage. Studies published in the past 10 years (since January 2012), with or without comparison groups were eligible for inclusion. Studies conducting an HBV screening intervention were eligible if they reported proportion of people participating in screening, proportion of newly diagnosed HBV (participant was unaware of their HBV status), proportion of people received HBV vaccination following screening, or proportion of participants diagnosed with chronic HBV infection who were linked to HBV clinical care. Studies were excluded if study population was less than 20 participants, intervention included a pharmaceutical intervention or a hospital-based intervention, or study was implemented in limited clinical services. The records were initially screened by title and abstract. The full texts of potentially eligible records were reviewed, and eligible studies were selected for inclusion. For each study included in analysis, the study outcome and corresponding 95% confidence intervals (95%CIs) were calculated. For studies including a comparison group, odds ratio (OR) and corresponding 95%CIs were calculated. Random effect meta-analysis models were used to calculate the pooled study outcome estimates. Stratified analyses were conducted by study setting, study population, and intervention-specific characteristics. Key findings A total of 61 studies were included in the analysis. A large majority of studies (study n=48, 79%) included single-arm studies with no concurrent control, with seven (12%) randomised controlled trials, and six (10%) non-randomised controlled studies. A total of 109 interventions were evaluated in 61 included studies. On-site or outreach HBV screening and linkage to HBV clinical care coordination were the most frequent interventions, conducted in 27 and 26 studies, respectively. Question 1 We found no studies reporting HBV incidence as the study outcome. One study conducted in remote area demonstrated that an intervention including education of pregnant women and training village health volunteers enhanced coverage of HBV birth dose vaccination (93% post-intervention, vs. 81% pre-intervention), but no data of HBV incidence among infants were reported. Question 2 Study outcomes most relevant to the HBV burden for people in the community with HBV included, HBV diagnosis, linkage to HBV care, and HBV vaccination coverage. Among randomised controlled trials aimed at enhancing HBV screening, a meta-analysis was conducted including three studies which implemented an intervention including community face-to-face education focused on HBV and/or liver cancer among migrants from high HBV prevalence areas. This analysis demonstrated a significantly higher HBV testing uptake in intervention groups with the likelihood of HBV testing 3.6 times higher among those participating in education programs compared to the control groups (OR: 3.62, 95% CI 2.72, 4.88). In another analysis, including 25 studies evaluating an intervention to enhance HBV screening, a pooled estimate of 66% of participants received HBV testing following the study intervention (95%CI: 58-75%), with high heterogeneity across studies (range: 17-98%; I-square: 99.9%). A stratified analysis by HBV screening strategy demonstrated that in the studies providing participants with on-site HBV testing, the proportion receiving HBV testing (80%, 95%CI: 72-87%) was significantly higher compared to the studies referring participants to an external site for HBV testing (54%, 95%CI: 37-71%). In the studies implementing an intervention to enhance linkage of people diagnosed with HBV infection to clinical care, the interventions included different components and varied across studies. The most common component was post-test counselling followed by assistance with scheduling clinical appointments, conducted in 52% and 38% of the studies, respectively. In meta-analysis, a pooled estimate of 73% of people with HBV infection were linked to HBV clinical care (95%CI: 64-81%), with high heterogeneity across studies (range: 28-100%; I-square: 99.2%). A stratified analysis by study population demonstrated that in the studies among general population in high prevalence countries, 94% of people (95%CI: 88-100%) who received the study intervention were linked to care, significantly higher than 72% (95%CI: 61-83%) in studies among migrants from high prevalence area living in a country with low prevalence. In 19 studies, HBV vaccination uptake was assessed after an intervention, among which one study assessed birth dose vaccination among infants, one study assessed vaccination in elementary school children and 17 studies assessed vaccination in adults. Among studies assessing adult vaccination, a pooled estimate of 38% (95%CI: 21-56%) of people initiated vaccination, with high heterogeneity across studies (range: 0.5-93%; I square: 99.9%). A stratified analysis by HBV vaccination strategy demonstrated that in the studies providing on-site vaccination, the uptake was 78% (95%CI: 62-94%), significantly higher compared to 27% (95%CI: 13-42%) in studies referring participants to an external site for vaccination. Conclusion This systematic review identified a wide variety of interventions, mostly multi-component interventions, to enhance HBV screening, linkage to HBV clinical care, and HBV vaccination coverage. High heterogeneity was observed in effectiveness of interventions in all three domains of screening, linkage to care, and vaccination. Strategies identified to boost the effectiveness of interventions included providing on-site HBV testing and vaccination (versus referral for testing and vaccination) and including community education focussed on HBV or liver cancer in an HBV screening program. Further studies are needed to evaluate the effectiveness of more novel interventions (e.g., point of care testing) and interventions specifically including Indigenous populations, people who inject drugs, men who have sex with men, and people incarcerated.
