Abstract:
Abstract Background Studies of hospital discharge data and large observational cohorts show that the incidence of venous thromboembolism (VTE) varies by race. We sought to determine the incidence of VTE in each of the following ethnic groups Caucasians, Africans, Asians, and Hispanics. Methods A systematic literature search strategy was used to identify potential studies in MEDLINE, EMBASE, and CENTRAL using an OVID interface. The methodological quality of eligible studies was assessed according to Newcastle-Ottawa Quality Assessment Scale. The primary outcome measure was to identify the incidence/prevalence of VTE of each ethnic group available for the study in the context of a population based study. Results Out of 3418 potential studies, 19 met our inclusion criteria (Table). Of these, 9 studies were population based studies: 5 reported VTE incidence per 100,000 person-years (PY), 2 measured the standardized incidence ratio, and 2 European studies assessed VTE rate according to country of origin but not ethnicity (data not shown). In addition, 7 studies measured the hospital incidence rate of VTE, and 3 assessed VTE prevalence. Data could not be pooled due to marked heterogeneity including varied periods of study. VTE incidence per 100,000 PY was found to be between 162 and 439 in Caucasians, 143 and 746 in Africans, 3.2 and 16.6 in Asians. Hospital incidence of VTE per 100,000 PY was found to be between 21 and 131 in Caucasians, 22 and 155 in Africans, 2 and 26 in Asians, 33.1 and 71 in American Indians/Alaskan Indians, and 9 in Hispanics. Conclusions Our findings suggest a wide variation in reported incidence rates for VTE among different ethnic groups, even within the same group. In general, studies in Asian populations suggest a lower incidence of VTE. A marked heterogeneity of study designs, population settings prevent drawing firm conclusions. A significant risk of bias cannot be excluded for several studies. Further studies assessing concurrently the risk of VTE among different ethnicities in the same geographical area are needed. Abstract:
Background. Acute pancreatitis (AP) is a common surgical acute abdomen. Different kinds of pancreatitis may have different pathophysiological characteristics each other. The objective of this research was to investigate the early clinical features and complications of different types of acute pancreatitis. Methods. 787 AP patients admitted in the Huadu District People’s Hospital of Guangzhou during January 2009 and December 2019 were analyzed retrospectively. Among 787 AP patients, 520 (66.1%) were biliary AP (group I), 69 (8.7%) were alcoholic AP (group II), and 198 (25.2%) were hypertriglyceridemic AP (group III). According to the local and systemic complications and mortality in the early stage, we compared and analyzed the clinical characteristics and prognosis of different types of pancreatitis. Results. Mild acute pancreatitis accounted for the highest proportion (79.4%) in group I, while moderately severe acute pancreatitis in group II (36.2%) and severe acute pancreatitis in group III (62.6%). In terms of severity score of the pancreatitis, the average scores of BISAP, Ranson, APACHE-II, and MCTSI of the patients in group III were the highest ( p < 0.01 ). The incidence of acute peripancreatic fluid collection and infectious pancreatic necrosis was the highest in group III. The incidences of acute necrotic collection, pancreatic pseudocyst, and walled-off necrosis in group III were significantly higher than those in the other two groups ( p < 0.01 ). The incidences of systemic inflammatory response syndrome, sepsis, multiple organ failure, intra-abdominal hypertension, and mortality were highest in group III. Conclusions. There is an upward trend of the incidence rate of hypertriglyceridemic AP in recent years; it has been gradually developed into the second type of acute pancreatitis which is second only to the acute biliary pancreatitis. It is worthy to pay more and more attentions to it due to the feature of its younger onset, high incidence of complications, and high mortality.

RESUMENDe entre todos los objetos, tiempos, espacios, palabras y seres que componen el mundo físico, algunos están investidos de un valor especial por cuanto se les atribuye la virtud de visibilizar las instancias invisibles de las que dependemos los mortales. Es lo sagrado. A lo sagrado se le depara un trato singular hecho de respeto, veneración o miedo, pero en ocasiones también de rencor y de odio por lo que encarna o representa. Es adorado, pero también, y acaso por las mismas razones, puede ser insultado, destruido, objeto de burla y, si tiene forma humana, martirizado o asesinado. La violencia contra lo sagrado puede caber en sistemas religiosos que le otorgan a la agresión un papel central en su universo mítico o ritual. También se ofende u agrede lo santo para grupos o pueblos a someter, puesto que en ello está resumido su orden del mundo. Desde esta perspectiva, el agravio, la irreverencia y el daño pasan a reclamar un lugar protagonista en los estudios sobre la institución religiosa de la cultura bajo las figuras del sacrilegio y la profanación.PALABRAS CLAVE: sagrado, profanación, sacrilegio, violencia religiosa, iconoclastia.ABSTRACTOf all the objects, times, spaces, words and beings that make up the physical world, some are invested with a special value because they are attributed the virtue of making visible the invisible instances on which we mortals depend. Violence against the sacred can fit into religious systems that give aggression a central role in their mythical or ritual universe. Also offended or attacked is what is sacred for groups or peoples to be subdued, since in it an embodiment of their world order. RESUMEN:El objetivo del artículo es aproximarnos a la controvertida biografía del comisario Manuel Brabo Portillo. El trabajo está basado en fuentes primarias y secundarias. El método utilizado es empírico. En el imaginario del mundo sindicalista revolucionario, Brabo Portillo era el policía más odiado, la reencarnación de la cara más turbia del Estado. Fue, así mismo, un espía alemán relacionado con el hundimiento de barcos españoles, el asesinato del empresario e ingeniero Barret y el primer jefe de los terroristas vinculados a la patronal barcelonesa. La conflictividad que afectó a España en el período de la Primera Guerra Mundial es fundamental para entender los orígenes del terrorismo vinculado al pistolerismo, que marcó la historia político social española del primer tercio del siglo XX.PALABRAS CLAVE: Brabo Portillo, pistolerismo, espionaje, sindicalismo, Primera Guerra Mundial.ABSTRACT:The objective of the article is an approach to the controversial biography of Police Chief Manuel Brabo Portillo. LBA1 Background: MDV3100, a novel androgen receptor signaling inhibitor (ARSI), competitively inhibits binding of androgens to the androgen receptor (AR), inhibits AR nuclear translocation, and inhibits association of the AR with DNA (Tran et al, Science. 2009;324:787). MDV3100 was selected for development based on activity in prostate cancer model systems with overexpressed AR, and was active in a phase I-II trial enrolling pre- and post-chemotherapy treated patients with progressive castration resistant disease (CRPC) (Scher et al, Lancet. 2010;375:1437). The AFFIRM trial evaluated whether MDV3100 could prolong overall survival (OS) in men with CRPC who progressed following docetaxel-based chemotherapy. Methods: In this randomized, double-blind, placebo-controlled, multinational phase III study ( NCT00974311 ), patients who had received ≤ 2 regimens of docetaxel-based chemotherapy were randomized 2:1 to MDV3100 160 mg/day or placebo. Treatment with corticosteroids was allowed but not required. Patients were stratified by baseline ECOG performance status and mean brief pain inventory score. The primary endpoint was OS. Other efficacy endpoints included radiographic progression-free survival (PFS), time to first skeletal-related event, time to prostate-specific antigen (PSA) progression, and circulating tumor cell count conversion rate. Results: 1,199 patients were randomized between Sep 2009 and Nov 2010. Based on a planned interim analysis at 520 death events, the Independent Data Monitoring Committee (IDMC) recommended the study be unblinded and placebo patients offered MDV3100 due to a significant OS benefit (p<0.0001; hazard ratio 0.631). The estimated median OS was 18.4 months for MDV3100 treated compared to 13.6 months for placebo treated men, a median OS difference of 4.8 months. Data to be available include PFS, time to PSA progression, and safety. Conclusions: MDV3100, a novel ARSI, significantly improves OS in men with postdocetaxel-treated CRPC reducing the risk of death by 37% relative to placebo. The IDMC determined the risk:benefit of MDV3100 was favorable and recommended the phase III AFFIRM trial be unblinded.

Aims and methodTo assess trends of the American Board of Psychiatry and Neurology examination pass rates before and after the 2003 duty hours regulations (DHR). We obtained the pass rates for part I and II for years 2000–2010. Data were divided pre-DHR (2000–2003) and post-DHR (2007–2010).ResultsDuring the pre-DHR period, first- and multiple-attempt group pass rates were 80.7% and 39.0% which changed in the post-DHR period to 89.7% and 39.1% respectively. Similarly for the part II exam, the pre-DHR first- and multiple-attempt group pass rates were 60.2% and 43.5% respectively, which increased to 78.7% and 53.8%, among the post-DHR group. Overall, there was a significant increase in the first-attempt candidates pass rates for parts I and II, whereas multiple-attempt candidates did not benefit as strongly.Clinical implicationsThe results suggest that the 2003 DHR may have had a positive impact on examination-based medical knowledge in psychiatry.

Buscou-se trazer elementos contidos na legislação e considerações de diversos autores, por meio de pesquisa bibliográfica, assim como foi aplicado um formulário com 19 questões, sobre o comportamento de 20 produtores rurais frente às informações relativas à devolução das embalagens vazias de agrotóxicos. As discussões revelam que cerca de 60% dos respondentes são indiferentes, conhecem muito pouco ou desconhecem sua responsabilidade como primeiro elo da cadeia da logística reversa das embalagens vazias de agrotóxicos, talvez em virtude de falhas no processo de comunicação e na fiscalização. Sugere-se que deva haver maior empenho do Poder Público com vistas a intensificar a fiscalização e tornar mais compreensível as responsabilidades desses agricultores.PALAVRAS-CHAVE: Logística reversa, comunicação e agronegócios, embalagens vazias de agrotóxicos, legislação ambiental. Abstract Abstract 1761 Poster Board I-787 Introduction Myelodysplastic syndromes (MDS) are clonal hematopoietic stem cell disorders characterized by ineffective hematopoiesis culminating in peripheral blood cytopenias and a propensity to Acute Myelogenous Leukemia (AML). The gold standard diagnostic test is the bone marrow aspirate and biopsy but clinicians are often hesitant to refer patients with unexplained cytopenias or macrocytosis for this test. Consequently undiagnosed patients are deprived access to effective treatments. Previously (Buckstein et al., Leukemia Research 33 (2009) 1313–1318) we identified factors which were independently predictive of diagnosing MDS at the time of bone marrow, including; age, mean corpuscular volume (MCV), red cell distribution (RDW), and lactate dehydrogenase (LDH). In this current study, we set out to validate our findings and determine the sensitivity and specificity of our scoring system for routine clinical practise. Methods We reviewed all bone marrow reports conducted at a tertiary care center for the years (January through December) 2006–2008 inclusive. Our inclusion criteria included bone marrows done for unexplained cytopenias and/or macrocytosis. We excluded all outside consultative referrals and bone marrows done for staging or remission assessment in patients with pre-existing or strongly suspected hemato-lymphoid diagnoses. In cases where the patient had more than one bone marrow, only the most recent marrow was used. The bone marrows were reviewed by two experienced hematopathologists, and the diagnosis of MDS was made using the WHO and/or FAB classification systems. Marrows were classified as ‘confirmed MDS’, ‘suspected MDS’ or ‘Not MDS’. Electronic patient charts were reviewed to determine the age, MCV, LDH and RDW at the time of diagnostic bone marrow. A factor was considered positive if the age was > 65 or if its value exceeded the upper range of normal for our laboratory standards (MCV > 96 fl, LDH > 250 IU/L, RDW >14.5%). All patients for whom we had 4 recorded factors were then assigned a score that ranged from 0 to 4. We determined the distribution of scores within this population, and the sensitivity and specificity of this scoring method in predicting a histopathological diagnosis of MDS. Results Three-hundred and forty bone marrows met our inclusion criteria, and 289 (85%) had all four factors recorded at the time of diagnosis. The predictive ability of the MDS score is summarized in Table 1. The probability of diagnosing MDS increased from 8% with a score of 0 to 46% with a score of 3 or 4. A similar trend was seen when all 344 marrows were analysed. Our scoring system had high sensitivity (> 96%), when only one predictive factor was present and high specificity (96%) when 4 predictive factors were present at the time of diagnostic marrow. (Table 2) Conclusions In patients with unexplained cytopenias, or macrocytosis undergoing a diagnostic BM test, the pre-test probability of a MDS diagnosis increases as the number of predictive factors increases. The specificity of our MDS score also follows a similar trend. The sensitivity is high with only one positive factor indicating a lower score can be used as a screening tool. We have effectively validated a scoring system that may help clinicians decide on the utility of arranging diagnostic bone marrow examinations for patients with undiagnosed cytopenias and macrocytosis. A tactic that has been used to increase EET levels has been inhibition of the soluble epoxide hydrolase enzyme. Epoxide hydrolase inhibitors have been demonstrated to be antihypertensive and renal protective. Thus the renal and cardiovascular protective actions of increasing epoxygenase levels could be translated to therapies for preventing end-organ damage.

To take more account of the terroir effect in varietal choice and address the concerns of cotton farmers who disapprove the agricultural policy of single variety, research on varietal regionalization was initiated during four campaigns, by the Agricultural Research Center for Cotton Fibers. This idea is reinforced by the diversity of agro-ecological zones of the country. The nonrenewal of the variety in extension since the 2002-2003 campaign is also an element of justification concerns of industry players. This study aims to exploit the terroir effect to identify the most suitable varieties for each agroecological zone and meet the needs of industry players including producers and ginners. The experiment was conducted in 2011-2012 on Angaradébou sites, Okpara, Savalou and Ketou respectively representing the zones 1 (Alibori / Atakora); 2 (Borgou / Donga); 3 (Hills) and 4 (Zou / Ouemé Plateau / Mono-Couffo). The adopted experimental device is a Fisher block with four replicate and eight treatments or varieties H 279-1 in extension witness. Other varieties include E 944-2; E 956-2; H 769-5; H 782-3; I 875-3; K 768-3 and K 787-2.The measured parameters are the earliness of production, seed cotton yield in the field, the weight of 100 seeds and the fiber ginning outturn. Data analysis revealed significant differences between and inside sites for most measured parameters. The results showed that varieties E 956-2 (1983.85 kg / ha), H 769-5 (1820.49 kg / ha), K 768-3 (1735.42 kg / ha) and H 782-3 (1533.59 kg / ha) in terms of seed cotton yield in the field and compared their performance to shattering were the best respectively in zones 1, 2, 3 and 4.

Abstract Background: Over 10,000 new cases of myelodysplastic syndrome (MDS) are diagnosed in the US annually (MA X, Cancer 2007). Hypomethylating agents (azacitidine and decitabine), immunomodulating drugs (lenalidomide) and erythropoietin stimulating agents (epoetin α and darbepoetin) have been approved for more than a decade for the treatment of MDS. However, changes in overall outcome after the approval of these treatments are unknown. We used the National Cancer Data Base (NCDB), a nationwide oncology registry covering 70% of the US cancer population to answer these questions. Methods: We identified patients with MDS diagnosed from 2004-2013 using ICD-O-3 codes 9980-9983 and 9985-99912. Overall survival (OS) was estimated using the Kaplan-Meier method. We compared OS using the log-rank test according to demographic subgroups (sex, age [<40, 41-65, and ≥65 years], race [White, Black, Native American, Asian/Pacific Islander, and other], and ethnicity) across years of diagnosis (Period 1: 2004-2006, Period 2: 2007-2009, and Period 3: 2010-2013). Results: i) Characteristics: There were 54,953 MDS patients in our study. The median age at diagnosis was 76 years (78.7% older than 65) and 44% were females. White race (88.9%) was the most common, followed by black (7%). Four percent were Hispanics. A third (30.8%) of all patients were treated in academic centers. Three quarters (75.8%) had either Medicaid or Medicare insurance. ii) MDS Subtypes: 5q Syndrome was seen in 2008 cases (3.7%) while the most common subtype was MDS-Unclassifiable (U) (55.6%). Therapy related MDS (t-MDS) was seen in 1125 cases (2%), however coding for t-MDS was discontinued in 2009. Refractory anemia (RA), refractory anemia with ring sideroblasts (RARS), refractory cytopenia with multilineage dysplasia (RCMD), and refractory anemia with excess blasts were seen in 9%, 7.6%, 6%, and15.6%, respectively. iii) OS: Compared to their counterparts, patients who were male, older, had a diagnosis of RAEB, covered by Medicare insurance, and treated at non-academic centers had the worse OS (all P <0.001). Across Periods 1-3, we did not see improvement in OS among demographic subgroups except for those who were <40 years old (P=0.03). Conclusions: In the past decade, the overall outcome of MDS did not improve despite the advent of new therapies. More than half of the cases were classified as MDS-U reflecting the difficulty of subtyping MDS based on World Health Organization classification. We found disparity in outcomes by demographics and type of treatment facility. This is the largest study of MDS outcome to-date and should help in understanding the gaps and areas that need future research. Figure Figure. Abstract Abstract 4974 Background The bone marrow trephine biopsy (BMB) has a crucial role for the diagnosis of essential thrombocythemia (ET), both according to the PVSG and the WHO criteria. The WHO 2001 criteria enhanced the role of BMB also by distinguishing the true-ET (ET) from the prefibrotic and the early fibrotic chronic idiopathic myelofibrosis. The WHO 2008 criteria, in the JAK2 era, confirmed the diagnostic and prognostic relevance of the histopathological features in ET as well as in the other Ph-neg myeloproliferative neoplasms (MPN). Otherwise, only few validated data are presently available, and the reproducibility in the evaluation of some morphological details is still controversy. Objective To validate ET diagnosis in a large registry-based series of patients by revising the BMB specimens according to the WHO criteria and to evaluate the potential relationship between the histopathological and the clinical parameters at presentation. Methods The hematological centers of the Registro Italiano Trombocitemia (RIT) were invited to participate to this study by sending the BMB specimens (hematoxylin-eosin, Giemsa and silver stains) obtained at diagnosis. The clinic-pathological panel of the RIT (with three hematopathologists as permanent members coordinated by a chairman) performed a centralized revision of the BMB specimens, concomitantly (multiheaded microscopy) and blindly (only patient sex and age were known). The panel described for each case the morphological features and gave a diagnostic conclusion according to WHO criteria as follows: true-ET (ET); or initial primary myelofibrosis (i-PMF) distinguished in prefibrotic PMF (pf-PMF/MF-0) and early fibrotic PMF (ef-PMF/MF-1); or advanced PMF (MF-2 and MF-3); or early polycythemia vera (e-PV); or MPN unclassifiable (MPN-U); or diagnosis other than MPN (No MPN). Results Thirteen centers sent the specimen of BMB at diagnosis of 272 patients registered into the RIT and diagnosed in the years 1986-2002 (group A, cases 66, 24.3%), 2003-2005 (group B, cases 95, 34.9%) and 2006-2008 (group C, cases 111, 40.8%). The patients, 104 (38%) males and 168 (62%) females, had at diagnosis: age over 50 in 64% of cases (median 58); PLT count (109/L) >1000 in16% and <= 600 in 24% of cases (median 789); Hgb level (g/dL) >17 in 1% and <=9 in 5% of cases (median 14.3); WBC count (109/L) >9 in 44% of cases (median 8.6); splenomegaly in 18% of cases. Disease symptoms, thrombotic and hemorrhagic events were reported in 40%, 21%, and 7% of cases, respectively. During the follow-up (median 2.7 years) 65% of patients received cytoreductive drugs (Hydroxyurea 59%, Anagrelide 20%, Interferon 14%, Pipobroman 6.5%, Busulfan 0.5%). The revision of the 272 BMB specimens allowed to the following diagnosis: ET 142 cases, 52.2%; i-PMF 72 cases, 26.5% (distinguished in pf-PMF 19 cases, 7% and ef-PMF 53 cases, 19.5%); e-PV 13 cases, 4.8%; MPN-U 16 cases, 14.3%; No MPN 8 cases, 2.9%. In the patients of groups A, B, and C the rate of ET was increasing (from 44%, to 50% and to 59%, respectively) while the rate of i-PMF was decreasing (from 34%, to 31% and to 18%, respectively); moreover, the rate of e-PV was 0%, 6%, and 6%, respectively. The ET patients compared with the i-PMF patients showed at diagnosis a lower rate of splenomegaly (16/142, 11.3% vs 24/72, 33.3%, p<0.0001), of age >50 years ( 84/142, 59.2% vs 53/72, 73.6%, p<0.03), and a higher rate of PLT count (109/L) <=600 (39/142, 27.5% vs 11/72, 15,3%, p<0.04 ); no significant differences were found between the two groups for sex, Hgb level, WBC count, symptoms, thrombotic and hemorrhagic events. The treatment rate was lower in ET than in i-PMF (ratio 0.7). Conclusion This preliminary report on the revision of the BMB at diagnosis in 272 patients of the Registro Italiano Trombocitemia (RIT) shows that: the rate of true ET is continously increasing (from 44% before 2003 to 59% in the period 2006-2008), with i-PMF rate concomitantly decreasing (from 34% to 18%); the ET patients, compared with the i-PMF ones, were younger, with lower PLT count and with lower rate of splenomegaly, and received a less intensive cytoreductive treatment. In this paper, we review on diurnal variations of the foF2 ionospheric parameter predicted by the IRI-2012 model, and data from Ouagadougou ionosonde station located in the crest of the Equatorial Anomaly (Lat: 12.5°N; Long: 358.5°E, dip: 1.43°) during fluctuating geomagnetic activity conditions for the solar cycles 21 and 22. Our investigations are focused on the electrodynamic aspects, the influence of the ionospheric electric currents as well as the variations of the hourly values given by model and experimental measurements. A comparative study pointed out that the IRI-2012 model, through its URSI and CCIR subroutines, gives a good prediction of the critical frequency of the F2 layer between 0700 TL and 0000 TL. In addition, IRI -2012 tries to reproduce, as best as possible, the vertical drift E × B during minimum, decreasing phase, winter, and autumn. However, there is no effect of drift during the other seasons and solar cycle phases. The dynamical mechanisms underlying the redistribution of mean momentum and vorticity are explored for transitional two-dimensional boundary-layer flow at nominally zero pressure gradient. The analyses primarily employ the direct numerical simulation database of Wu & Moin (J. Fluid Mech., vol. 630, 2009, p. 5), but are supplemented with verifications utilizing subsequent similar simulations. The transitional regime is taken to include both an instability stage, which effectively generates a finite Reynolds stress profile, −ρuv(y), and a nonlinear development stage, which progresses until the terms in the mean momentum equation attain the magnitude ordering of the four-layer structure revealed by Wei et al. (J. Fluid Mech., vol. 522, 2005, p. 303). Self-consistently applied criteria reveal that the third layer of this structure forms first, followed by layers IV and then II and I. For the present flows, the four-layer structure is estimated to be first realized at a momentum thickness Reynolds number Rθ = U∞ θ/ν ≃ 780. The first-principles-based theory of Fife et al. (J. Disc. Cont. Dyn. Syst. A, vol. 24, 2009, p. 781) is used to describe the mean dynamics in the laminar, transitional and four-layer regimes. As in channel flow, the transitional regime is marked by a non-negligible influence of all three terms in the mean momentum equation at essentially all positions in the boundary layer. During the transitional regime, the action of the Reynolds stress gradient rearranges the mean viscous force and mean advection profiles. This culminates with the segregation of forces characteristic of the four-layer regime. Empirical and theoretical evidence suggests that the formation of the four-layer structure also underlies the emergence of the mean dynamical properties characteristic of the high-Reynolds-number flow. These pertain to why and where the mean velocity profile increasingly exhibits logarithmic behaviour, and how and why the Reynolds stress distribution develops such that the inner normalized position of its peak value, ym+, exhibits a Reynolds number dependence according to $y_m^+ {\,\simeq\,} 1.9 \sqrt{\delta^+}$.

INTRODUCTION: The aim of this study was to assess the epidemiological and operational characteristics of the Leprosy Program before and after its integration into the Primary healthcare Services of the municipality of Aracaju-Sergipe, Brazil. METHODS: Data were drawn from the national database. The study periods were divided into preintegration (1996-2000) and postintegration (2001-2007). Annual rates of epidemiological detection were calculated. Frequency data on clinico-epidemiological variables of cases detected and treated for the two periods were compared using the Chi-squared (χ2) test adopting a 5% level of significance. RESULTS: Rates of detection overall, and in subjects younger than 15 years, were greater for the postintegration period and were higher than rates recorded for Brazil as a whole during the same periods. A total of 780 and 1,469 cases were registered during the preintegration and postintegration periods, respectively. Observations for the postintegration period were as follows: I) a higher proportion of cases with disability grade assessed at diagnosis, with increase of 60.9% to 78.8% (p < 0.001), and at end of treatment, from 41.4% to 44.4% (p < 0.023); II) an increase in proportion of cases detected by contact examination, from 2.1% to 4.1% (p < 0.001); and III) a lower level of treatment default with a decrease from 5.64 to 3.35 (p < 0.008). Only 34% of cases registered from 2001 to 2007 were examined. CONCLUSIONS: The shift observed in rates of detection overall, and in subjects younger than 15 years, during the postintegration period indicate an increased level of health care access. The fall in number of patients abandoning treatment indicates greater adherence to treatment. However, previous shortcomings in key actions, pivotal to attaining the outcomes and impact envisaged for the program, persisted in the postintegration period.

Prokofiev (2015a) described two new genera and species of Amblyopinae and reported new amblyopine records from waters off Indonesia, the Philippines, and Vietnam. In reviewing Prokofiev’s figures and reading the descriptions of his new genera, Biendongella Prokofiev, 2015 and Sovvityazius Prokofiev, 2015, it appeared likely that these new taxa were based on species also under examination by Koichi Shibukawa and me. However, some differences existed between our specimens and those described by Prokofiev in the pterygiophore formula (DF), the presence/absence of hypural 5, and the number of epurals. The data and information on these characters were not abundantly clear from the text and clarification was needed. For example, on page 786 of Prokofiev (2015a) the following diagnostic feature of Biendongella was provided, “DF 3 1221, vacant interneural gap between spined and soft parts of D present; two first pterygiophores of soft part of D each located in its interneural gap…”. Similar wording exists for Sovvityazius on page 789, “DF 3 1221, vacant gap between spined and soft parts of D present; two first pterygiophores of soft part of D each in its interneural space…”. As for hypural 5 and the number of epurals, the same statement is given for both Biendongella and Sovvityazius on pages 786 and 789, respectively, “Hypural 5 and epural 2 absent.” These character states struck me as remarkable as they are atypical for amblyopines. With respect to the DF character above, amblyopine gobies with a pterygiophore formula of 3-1221 have no interneural gap and the interneural space immediately posterior to the pterygiophore of the ultimate spinous ray contains two pterygiophores (Murdy 2011). As for the latter characters, hypural 5 and two epurals are present in amblyopine gobies (Shibukawa and Murdy 2012). In a detailed email to Prokofiev, I asked for clarification of these discrepancies and whether Biendongella and Sovvityazius have the amblyopine synapomorphy of the ultimate pterygiophore supporting a single ray (Murdy & Shibukawa 2001), which was not mentioned in the description of either genus.

Abstract Abstract 3481 Poster Board III-418 Hereditary FVII deficiency (FVIID) is a rare congenital bleeding disorder with an estimated prevalence of symptomatic individuals of 1:500,000. In the “Greifswald Registry FVII Deficiency” molecular defects of more than 1000 FVII deficient patients were described. By direct sequencing of the F7 genes in congenital FVIID revealed 146 different F7 gene mutations including 25 different mutations (18% of all) in the naturally-occurring acceptor or donor splice sites (Tab.1) were identified. In seven FVIID patients from Sweden and Germany the novel lesion g.IVS5+78G>A - downstream of the naturally-occurring donor splice sites of exon 5 - was identified. This variation was detected heterozygous in FVIID patients with FVII: C levels of 15%, 27%, 31%, 40% and 65%, and FVII: Ag levels between 25-50%. In two compound heterozygous patients with FVII: C levels of 1% und FVII: Ag levels of 2% and 3% respectively, one well-known causative FVII mutation is combined with the novel lesion g.IVS5+78G>A. The influence of this novel F7 gene variation on splicing was investigated by RT-PCR analysis and in vitro expression studies using exon-trap vector constructs. The total RNA was isolated from peripheral leucocytes and analyzed by one step RT-PCR and sequencing. Fragments of exon 5 and a part of the flanking intron 5 region (g.7679 –g.8073) were amplified of patients' DNA and cloned into the exon trap-vector pET01. Different vector constructs containing minigenes of the wild type (g.IVS5+78G) or mutant form (g.IVS5+78A) and the corresponding minigenes with an “optimized” naturally-occurring donor splice site in position +5 respectively were transfected into HEK293 cells. The expressed RNA was isolated and characterized. Consensus Values (CV) for all donor splice sites were calculated using a splice site detection tool according Shapiro and Senapathy (1987). The RT-PCR analysis in patients indicate that the novel variation g.IVS5+78G>A in intron 5 created an aberrant splice site in position 79bp downstream of exon 5 even though the naturally-occurring donor-splice-site of exon 5 is not abolished. An insertion of 79bp of intron 5 into the mRNA leads to a frame shift and predicts a premature termination 11 codons past the last unaltered codon. Minigenes include the naturally-occurring splice site and the variation g.IVS5+78A used exclusively the aberrant splice position 79bp downstream of exon 5 whereas wild type minigenes with the naturally-occurring splice site and the wild type form g.IVS5+78G produced normally spliced mRNA. In a following experiment the “naturally-occurring splice site” of exon 5 was optimized by the additional substitution g.IVS5+5C>G which increased their CV from 76.6 to 90.9 compared to the CV of the novel mutant g.IVS5+78A of 80.3. In presence of both mutations (g.IVS5+5G and g.IVS5+78A) only normal spliced mRNA was expressed of this minigene. In this construct the mutation g.IVS5+78G>A was without importance for the mRNA splicing. The results of the in vitro experiments demonstrated, that the Consensus Values (CV) seems to be an important factor for the selection of donor splice sites in the F7 gene. In the “Greifswald Registry FVII Deficiency” 26 different splice site variations in F7 gene were identified (Tab. 1). The atypical splice site variation g.IVS5+78G>A, +78bp downstream of exon 5 was present in 7 FVIID patients from Sweden and Germany in different genotypes. This novel F7 gene mutation g.IVS5+78G>A creates an aberrant splice site in position +79 of intron 5 and predicts premature termination. RNA analysis and expression studies demonstrated, that this novel F7 gene lesion is a type I mutation with low FVII:C and FVII: Ag levels and is the basis defect in 7 FVIID patients of the “Greifswald Registry FVII Deficiency”. miRNA are known as short RNA molecules that usually consist of 18 – 25 nucleotides and are considered as posttranscriptional regulators of gene expression occurring in plants and animals [1]. Over 60% of all genes which encode proteins are controlled by miRNA [2]. miRNA is engaged in various processes including differentiation, proliferation and apoptosis as well as inflammation. Hence, they might be considered as potential biomarkers of diseases such as cancer, neurodegenerative and cardiovascular [3]. A huge challenge is the actual detection of miRNA because of their short length, low concentration and matrix interferences. The miRNA analysis can be performed with the following techniques such as real-time PCR, northern blotting and microarray [4-6]. However, the above mentioned techniques are rather sophisticated and are costly. An alternative could be the application of electrochemical techniques which are characterized with low detection limits, possibility of miniaturization and high sensitivity. In such a case miRNA could be detected using biosensors containing receptor layers formed of single-stranded DNA or PNA. Herein, we present the studies on the development of ssDNA/PNA – based receptor layers for the electrochemical analysis of miRNA 141, which is a biomarker of pancreatic cancer. Various receptor layers were tested including unlabelled ssDNA, methylene blue-labelled DNA as well as PNA-based. The formation of ssDNA/PNA receptor layer along with its binding to miRNA was confirmed using QCM and SPR techniques. The electrochemical studies revealed that it was not possible to obtain low detection limits for miRNA when MB-labelled ssDNA or PNA molecules were applied. On the contrary, detection limit of 10-4 nM was reached with the application of unlabelled ssDNA molecules. The further analysis showed the possibility of detection of point mutation in ssDNA or miRNA sequence as well as miRNA detection in spiked serum sample. Bibliography: [1] D. P. Abstract:
Методом импедансной спектроскопии исследованы протонпроводящие свойства полисурьмяной кислоты (ПСК), допированной ионами ванадия. Для твердых растворов состава H2Sb2–xVxO6·nH2O, кристаллизующихся в структурном типе пирохлора (пр. гр. симм. Fd3m), показано, что увеличение количества ванадия в образце приводит к росту удельной протонной проводимости, которая для крайнего твердого раствора замещения H2Sb1.52V0.48O6·nH2O составляет 66 мСм/м. Из анализа данных диэлектрической спектроскопии при температурах 218–298 К определена энергия активации проводимости, которая составила 30±2 КДж/моль. Предложен механизм протонного транспорта, согласно которому в допированных ионами ванадия ПСК проводимость осуществляется посистеме водородных связей, образованных молекулами воды, расположенными в гексагональных каналах структуры типа пирохлора, и анионами кислорода октаэдра, формирующего каркас структуры REFERENCES Stenina I. Z dniem 7 czerwca 2023 r. wejdą w życie przepisy przekształconej wersji rozporządzenia 1371/2007 dotyczącego praw i obowiązków pasażerów w ruchu kolejowym. Pierwsza nowelizacja tego podstawowego aktu prawnego w codziennej praktyce pasażerskich przewoźników kolejowych, zarządców infrastruktury, sprzedawców biletów oraz Prezesa UTK wprowadza istotne zmiany w wielu obszarach. Modyfikacji poddane zostały zasady odpowiedzialności przedsiębiorstw kolejowych w przypadku opóźnienia, odwołania lub utraty połączenia. Wprowadzono nowe zasady sprzedaży i odpowiedzialności przedsiębiorstw kolejowych podczas podróży objętych wspólnymi biletami. Wzmocniono uprawnienia osób z niepełnosprawnością oraz o ograniczonej możliwości poruszania się. W istotny sposób zmodyfikowano także zasady dotyczące możliwości przyznawania przez państwa członkowskie terminowych i bezterminowych zwolnień ze stosowania wybranych przepisów rozporządzenia 2021/782. W artykule omówiono najważniejsze zmiany we wskazanych wyżej obszarach.

G-protein-coupled receptors (GPCRs) are important therapeutic targets for many areas of drug research and development. Although chimeric Gα16 proteins are valuable tools for detecting the activation of Gαi/o-coupled receptors, the details of the activation process remain unclear. The authors introduce a series of chimeras that combine both Gα16 and Gα i/o (Gα16/o, Gα16/i2, and Gα 16/i3) into a well-established transient expression system to examine the ability of these chimeras to interact with D2 long-form (D 2L) dopamine and 5-HT1A serotonin receptors. The pEC 50 data obtained for known agonists were similar to results from previous studies that used other cell-based assays, thus indicating sufficient sensitivity for the assay. Moreover, quinpirole exhibited similar intrinsic activity to dopamine at the D2L receptor, whereas S-(—)-3-PPP displayed partial activity of dopamine and quinpirole in the presence of the Gα16/o chimera. The potency of dopamine for D2L receptors was similar among Gα16/o, Gα16/i2, and Gα 16/i3. In contrast, the 5-HT1A receptor exhibited a significantly preferential coupling for Gα16/i3 compared with Gα 16/i2 when serotonin was used as a ligand. This finding was in close agreement with the results of previous reports. The present system could therefore be used as a rapid functional assay for high-throughput screening and deorphanization. В селекции крупного рогатого скота (КРС) большое значение имеет изучение концентрации аллелей групп крови и динамики частоты встречаемости для выявления изменений в генетической структуре, происходящих в процессе селекции, как породы в целом, так и отдельного стада. Установлены изменения генетической ситуации в стаде КРС голштинской породы в условиях Сахалинской области. Генетическую ситуацию контролировали с помощью EAB-локуса групп крови у 782 животных, рожденных в 1999 – 2018 гг. Анализ динамики аллелофонда во времени показал сокращение общего числа аллелей, участвующих в формировании генотипов EAB-локуса, с 42 (потомки 1999 – 2000 гг. рождения) до 21 – 23 (потомки 2015 – 2018 гг. рождения). Произошло изменение уровня гомозиготности в сторону его увеличения с 8,4 (потомки 1999 – 2000 гг. рождения) до 18,2 (потомки 2017 – 2018 гг. рождения), и, как следствие этого, наблюдалось уменьшение числа эффективных аллелей. Использование большого числа быков, носителей аллели G2Y2E2’Q’, привело в хозяйстве к увеличению рождения потомков (2017 – 2018 гг.), носителей данной аллели, в 1,7 раза. У потомков, рожденных с 2003 по 2010 г., элиминировали аллели Q, B2B’’, B2G2KI1O1D’O’Q’, O1Y2, I’, I2I’, O1Y2D’G’Q’, G3O1T1A2’E3’K’, I1O1Y2, QQ’, Y2Q’ и др. За весь период исследования генофонд восполнился только 25 аллелями, что привело к снижению генетического разнообразия в стаде.

In 1993, a Haemophilus influenzae serotype b (Hib) conjugate vaccine was introduced in the Dutch national immunization program, resulting in a sharp decrease in invasive Hib disease. We used a population-based set of serum samples collected in The Netherlands in 2006–2007 (Pienter-II, 5696 sera) to assess the concentration of antibodies to the capsular polysaccharide of Hib, and compared the results with those obtained from a similar set collected in 1995–1996 (Pienter-I, 7837 sera). Post-primary vaccination serum samples from children aged 6–11 months from the Pienter-II study contained approximately 4-fold lower anti-Hib antibody concentrations than samples from children from the Pienter-I study. No such difference was found in post-booster samples from children older than 11 months of age. In Pienter-II, the proportion of children aged 6–11 months with anti-Hib antibody concentrations below the putative protective concentration of 0.15 µg/mL was 30%, which is significantly higher than in the Pienter-I study (12%). Fewer children in the Pienter-II group developed antibodies able to kill Hib in a serum bactericidal assay compared to the Pienter-I children. The cause of the lagged response in Pienter-II children remain uncertain, but lack of natural boosting, interference by the acellular pertussis vaccine, combining vaccines and acceleration of the schedule may have contributed.

Abstract Abstract 1920 Poster Board I-943 Introduction: The prevalence of HTLV-1 is mostly evaluated by the age-specific seroprevalence in blood donors, and the results have been conventionally used to estimate the age-specific incidence of ATL in Japan. However, the results may be underestimated due to an age limit (16-69 yr) for donation, a healthy donor effect, and a birth cohort effect. Data concerning the birth-year specific incidence of ATL among HTLV-1 carriers other than blood donors are scarce. Methods: The study evaluated data of the anti-HTLV-1 antibody testing of 10,261 patients (males: 5,523, females: 4,737) who visited the Nagasaki University Hospital during 2000-2007 and data of 360 ATL cases (males: 188, females: 172) who were diagnosed in Nagasaki City (an endemic area in Japan) in a population-based Nagasaki Prefectural Cancer Registry (NPCR). To estimate birth-year specific incidence rates of ATL in population-based HTLV-1 carriers, we used the 2006 census population for Nagasaki City by applying the hospital-based seroprevalence data. Results: Of 10,261 patients, 1,392 (males: 653, females: 739) were HTLV-1 antibody positive. The overall HTLV-1 seroprevalence was 13.57% (95%CI: 12.90-14.23%). The seroprevalence was significantly higher in females than in males (15.60% vs. 11.82%, P<0.0001). The birth-year specific seroprevalence was 18.69% (before 1926), 17.83% (1927-1936), 15.91% (1937-1946), 13.80% (1947-1956), 9.19% (1957-1966), 4.07% (1967-1976), 2.07% (1977-1986), and 0% (after 1987) (a significantly declining trend: P <0.0001). The estimated annual number of HTLV-1 carriers by birth-year in Nagasaki city was 5257, 8093, 8151, 8083, 4434, 2180, 785, and 0, respectively. Finally, we estimated the annual incidence rate of ATLL per 100,000 HTLV-1 carriers by birth-year; 171 (before 1926), 86 (1927-1936), 41 (1937-1946), 32 (1947-1956), 11 (1957-1966), and 0 (after 1967). The crude lifetime risk of developing ATLL in HTLV-1 carriers was estimated to be 7.29% for males and 3.78% for females. Conclusions: The birth-year specific HTLV-1 seroprevalnces in the present study were approximately 50% higher than those previously reported in blood donors1 (for example: 6.22% in those born before 1950). Although it is possible that our results are over-estimated2, the present study suggests that there is still a large pool of elderly HTLV-1 carriers in this endemic area. Can Gio mangrove forest (CGM) is located downstream of Ho Chi Minh City (HCMC), situated between an estuarine system of Dong Nai - Sai Gon river and a part of Vam Co river. The CGM is the largest restored mangrove forest in Vietnam and the UNESCO’s Mangrove Biosphere Reserve. The CGM has been gradually facing to numeric challenges of global climate change, environmental degradation and socio-economic development for the last decades. To evaluate sediment quality in the CGM, we collected 13 cores to analyze for sediment grain size, organic matter content, and trace element concentration of Cd, Cr, Cu, Ni, Pb, Zn. Results showed that trace element concentrations ranged from uncontaminated (Cd, Cu, and Zn) to very minor contaminated (Cr, Ni, and Pb). Abstract:
A number of researchers have performed studies on the application of chemical reaction visualization by means of computational chemistry techniques for the purpose of teaching and learning chemistry at secondary and higher education levels. However, on practical grounds, the presented approach thus far was generally unattainable for teachers due to the time-consuming process of finding the transition states. This research offers a less time-consuming computational chemistry technique for producing organic reaction-modelling videos upon which the students may sufficiently build their understanding of fundamental concepts of chemical reactions. The technique being proposed here consists of (1) intermediate structure modelling, (2) reactant-wise and product-wise optimizations of the intermediate structures without the search for transition states, and (3) visualizations of the optimized results. The application of this technique has managed to produce a video demonstration of an organic reaction of benzene derivatives that may serve as modelling videos in chemistry lessons at secondary education or undergraduate degree levels.REFERENCESBode, B. e16013 Background: Squamous cell carcinomas of upper aerodigestive tract and endemic undifferenciated nasopharyngeal carcinomas are the most frequent head and neck (H&N) cancers. Apart from these two entities, malignant H&N tumors are uncommon due to their location and/or their histological type. They account for 10% of H&N cancers (around 2,000 new cases/year in France). The rarity of these cancers limits the progress in all aspects. Treatments are based on low level scientific proofs. In most histological types, chemotherapy is uneffective. Prognosis is usually threatening. It was necessary to federate medical centers at a national level to enhance epidemiological knowledge, quality of patient management and to promote research projects. Methods: Structuration of the network included: 2008-2009: 4 working groups to edit recommandations of good practice in diagnosis and treatment of sinus cancers, salivary glands cancers, ear cancers, upper aerodigestive tract non squamous cell carcinoma cancers; 2009: A national database including sociodemographical, clinical, epidemiological, histological and follow-up data; 2009: Organization of biorepositories; 2010: A national group of expert pathologists for diagnosis advice (telepathology); 2011: Specialized multidisciplinary clinics at regional and national levels. Results: To date, 783 patients were included in the database by 29 centers. Tumor sites were sinus/nasal fossa (53%), Salivary glands (28%), Upper aerodigestive tract (18%), ear (1%). Stage at diagnosis repartition was 15% stage I, 24% stage II, 17% stage III, 44% stage IV. Main histological types are adenocarcinoma (17%), Adenoid Cystic carcinoma (9%), mucoepidermoid carcinoma (5%), muquous melanoma (5%). 337 biospecimens are referenced. Conclusions: The majority of french centers involved in H&N cancers treatment contribute to this project. This network is a unique tool for improving the knowledge of rare H&N tumors. Ongoing epidemiological studies will shortly be published. Translational studies based on biospecimens will follow. Collaboration with other networks such as the Salivary Gland Tumors Biorepository could be useful in the next future. Funding source: French National Institute of Cancer (INCa).

There is something splendid about the word ageful – a word which I had not encountered before reading Bytheway (2000). Ironically, Bytheway does not share my affection for his invention; rather, for him it seems to have negative connotations, reflected in his comment: ‘as gerontologists we are vulnerable to seeing ourselves and those we study entirely in terms of age: as ageing rather than living individuals: ageful rather than ageless’ (2000: 785–6). But why this juxtaposition between ageing and living? Surely there is an interconnectedness between the two. We are ageing from the moment we are born; the longer we live, the more full of age we become.

e4910061The object of this study is to identify the contents about Food and Nutrition Education (FNE) in textbooks of sixth to ninth grades of Brazilian elementary school and to comprehend the teaching conceptions related to possible links between those concepts and the FNE. The National Program of School Feeding - NPSF guides the FNE actions towards different pieces of knowledge considering culture, history, geography (BRASIL, 2020). FNE can be a pedagogical resource of contextualization to many knowledge areas. It is a qualitative research, which results were obtained through discursive textual analysis of twenty didactic books, semi-structured interviews, and discussions between the nutritionist, the teachers and the students that took part in the study. Analysis showed that didactic books present few contents of FNE, generally referring to biomedical aspects. Teachers recognized the importance of FNE as a constitutive element of the curriculum, but they found it difficult to link such a theme with disciplinary concepts. The study reveals the importance of collective work of both health and education professionals since school age is propitious to create healthy habits and lifestyles.Resumo O objetivo deste estudo foi identificar os conteúdos sobre Educação Alimentar e Nutricional (EAN) presentes em livros didáticos do 6º ao 9º anos do Ensino Fundamental brasileiro e compreender as concepções docentes relacionadas às possíveis articulações entre os conceitos específicos disciplinares e EAN. O Programa Nacional de Alimentação Escolar (PNAE) orienta que as ações de EAN se articulem com os diferentes saberes que consideram a cultura, a história e a geografia (BRASIL, 2020). A EAN pode ser um recurso pedagógico de contextualização das diferentes áreas do conhecimento. Trata-se de uma pesquisa qualitativa, cujos resultados foram obtidos por meio da análise textual discursiva de 20 livros didáticos, de entrevistas semiestruturadas e discussões entre nutricionista, professores e alunos participantes do estudo. A análise mostra que os livros didáticos apresentam poucos conteúdos de EAN, normalmente centrados em aspectos biomédicos. As professoras reconhecem a importância da EAN como constitutiva do currículo escolar, mas têm dificuldades em articular a temática com os conceitos disciplinares. Abstract Background Enterobacter cloacae is a Gram-negative rod with chromosomally-induced Amp-C β-lactamase with multidrug-resistant potential. Joint Trauma System guidelines for treating combat wounds include prophylaxis with cefazolin and ertapenem, potent inducers of Amp-C. We evaluated clinical characteristics, antibiotic utilization, and outcomes associated with battlefield-related E. cloacae infections. Methods All initial solitary (those with single isolates) and serial E. cloacae isolates (≥24 hours from initial isolate from any site) were collected from the Trauma Infectious Disease Outcomes Study (6/2009-12/2014). Inclusion required E. cloacae isolation from a clinical infection. Amp-C-inducing β-lactams were classified based on induction potential and lability to the Amp-C β-lactamase as Amp-C induction levels. Results Of 653 E. cloacae isolates, 253 met inclusion criteria – 64 patients had only initial isolates, 54 patients had serial isolates. Patients were largely male (99%), median age 23 years (IQR 21-27), with injury severity score of 34 (IQR 24-45). Initial isolates were wound (70%), respiratory (22%), blood (7%), urine (1%), and other (1%). Patients commonly had blast injuries (89%), required ICU admission (95%), and had a median hospital stay of 57 days (IQR 39-82). Patients with serial isolates showed a trend towards earlier clinical infection (5 vs 8 days, P = 0.07). They were also less likely to receive carbapenems prior to E. cloacae isolation compared to those with only initial isolates (4% vs 38%) and more likely to receive 1st generation cephalosporins (79% vs 58%, P = 0.01). The serial isolate group received more days of 1st generation cephalosporins (median 6 days vs 2.5 days, P = 0.01). Cumulative antimicrobial therapy trended towards significance and was greater with the serial isolates (median 100 days vs 74 days, P = 0.08). There was no difference in number of surgical interventions between those with and without serial isolates (P = 0.54). Overall, 6 patients died. Conclusion E. cloacae infections after battlefield trauma were frequently encountered and associated with exposure to 1st generation cephalosporins. Serial infections did not correlate to worse patient outcomes but displayed a trend towards an overall greater duration of antibiotic use. Disclosures William N. 784 Background: Current clinical trial data are largely driven by younger participants, yet half the population diagnosed with CRC are 70+. Upon progression, many CRC patients (pts) with refractory cancer are referred to phase 1 clinical trials; however, tolerability and clinical benefit is unclear in the elderly. Given our robust phase 1 program, we assessed the efficacy and safety of early therapeutics in the elderly CRC pts. Methods: A historical cohort analysis included mCRC pts enrolled amongst 44 phase I trials from 3/2004 - 9/2012. Median Progression free survival (mPFS) and overall survival (mOS) were estimated from Kaplan-Meier curves and groups were statistically compared with the log rank test. The magnitude of association between dichotomous factors and survival was estimated with the hazard ratio (HR). Results: 139 pts, median age 59 (33-81), 70+ 16.5% (n = 23); KRAS mut 39.4 %. ≥3 prior lines of therapy 73.9 %; Prior EGFR-I 75.5% (100% of KRAS wt). Location: Right 20.9 %, Left 61.9 %, Transverse 4.3 %, Unknown 12.9 %. Phase I agents: VEGF I 19.4%, EGFR/Growth Factor I 9.3%. mOS for < 70 (n = 108) vs 70+ (n = 22): 6.5 vs 4.2 months (HR 1.88, p 0.007). mPFS for < 70 (n = 116) vs 70+ (n = 23): 2.0 months vs x 1.5 months (HR 1.73, P 0.03). There was not a statistical difference between < 70 versus 70+ in regards to dose cohort, number of cycles completed, or adverse events. Survival analysis in terms of risk factors in pts 70+ are in the Table below. Conclusions: Elderly CRC pts are underrepresented (16.5%) in phase 1 clinical trials and have worse survivals than non-elderly pts. Elderly CRC pts with liver and hematologic toxicities had a trend toward worse PFS but no OS. Phase I studies should incorporate prospective assessments to identify risk factors of morbidity and mortality in elderly CRC pts. [Table: see text]

Abstract:
El aprendizaje autorregulado, los estilos de aprendizaje y la motivación para el trabajo intelectual tienen gran importancia en la actualidad por su influencia sobre los procesos de aprendizaje. Este estudio, descriptivo y correlacional, procura determinar el grado de relación que existe entre estas variables y su nivel de significación, pues esta información podría ser de gran ayuda en el momento de planear las estrategias de aprendizaje dentro de las aulas. Se administró el Cuestionario Honey-Alonso de Estilos de Aprendizaje (CHAEA) y el Cuestionario de Estrategias de Aprendizaje y Motivación (CEAM) para el trabajo intelectual a una muestra de 110 alumnos de una escuela preparatoria de Mérida, Yucatán, México. Se encontró que la supervisión y la autoevaluación, como estrategias metacognitivas, fueron las mejores valoradas. Por otro lado, la organización de la información resaltando las ideas principales obtuvo una valoración muy pobre. El modelo planteado para analizar la relación entre las estrategias metacognitivas, la motivación y los estilos de aprendizaje alcanzó niveles de ajuste satisfactorios. Se observa una aparente ausencia de relación entre la motivación y los estilos de aprendizaje en coincidencia con resultados de otros estudios consultados. Por otro lado, en la literatura se ha encontrado que estudiantes con ciertos estilos de aprendizaje tienden a tener una preferencia por determinadas estrategias metacognitivas. No obstante, en la presente investigación no se observaron tales correlaciones, lo cual puede atribuirse a que en esta investigación el desarrollo de los diferentes estilos de aprendizaje es muy equilibrado, además de que únicamente la mitad de los encuestados tiende a tener un estilo preferente. Abstract:
In early pregnancy, the concentrations of IGFs increase in maternal blood. Treatment of pregnant guinea pigs with IGFs in early to midpregnancy enhances placental glucose transport and fetal growth and viability near term. In the current study, we determined whether exogenous IGFs altered placental gene expression, transport, and nutrient partitioning during treatment, which may then persist. Guinea pigs were infused with IGF-I, IGF-II (both 1 mg/kg·d) or vehicle sc from d 20–35 of pregnancy and killed on d 35 (term is 70 d) after administration of [3H]methyl-d-glucose (MG) and [14C]amino-isobutyric acid (AIB). IGF-I increased placental and fetal weights (+15 and +17%, respectively) and MG and AIB uptake by the placenta (+42 and +68%, respectively) and fetus (+59 and +90%, respectively). IGF-I increased placental mRNA expression of the amino acid transporter gene Slc38a2 (+780%) and reduced that of Igf2 (−51%), without altering the glucose transporter Slc2a1 or Vegf and Igf1 genes. There were modest effects of IGF-I treatment on MG and AIB uptake by individual maternal tissues and no effect on plasma glucose, total amino acids, free fatty acids, triglycerides, and cholesterol concentrations. IGF-II treatment of the mother did not alter any maternal, fetal or placental parameter. In conclusion, exogenous IGF-I, but not IGF-II, in early pregnancy increases placental transport of MG and AIB, enhancing midgestational fetal nutrient uptake and growth. This suggests that early pregnancy rises in maternal circulating IGF-I play a major role in regulating placental growth and functional development and thus fetal growth throughout gestation.

AbstractMagnetic clouds are thought to be large flux ropes propagating through the heliosphere. Their twisted magnetic fields are mostly modeled by a constant-alpha force-free field in a circular cylindrical flux rope (the Lundquist solution). However, the interplanetary flux ropes are three dimensional objects. In reality they possibly have a curved shape and an oblate cross section. Recently we have found two force-free models of flux ropes which takes into account the mentioned features. These are (i) a constant-alpha force-free configuration in an elliptic flux rope (Vandas & Romashets 2003, A&A, 398, 801), and (ii) a non-constant-alpha force-free field in a toroid with arbitrary aspect ratio (Romashets & Vandas 2003, AIP Conf Ser. 679, 180). Two magnetic cloud observations were analyzed. The magnetic cloud of October 18-19, 1995 has been fitted by Lepping et al. (1997, JGR, 102, 14049) with use of the Lundquist solution. The cloud has a very flat magnetic field magnitude profile. We fitted it by the elliptic solution (i). The magnetic cloud of November 17-18, 1975 has been fitted by Marubashi (1997) with use of a toroidally adjusted Lundquist solution. The cloud has a large magnetic field vector rotation and a large magnetic field magnitude increase over the background level. We fitted it by the toroidal solution (ii). The both fits match the rotation of the magnetic field vector in a comparable quality to the former fits, but the description of the magnetic field magnitude profiles is remarkable better. It is possible to incorporate temporal effects (expansion) of magnetic clouds into the new solutions through a time-dependent alpha parameter as in Shimazu & Vandas (2002, EP&S, 54, 783).

O carcinoma de células escamosas (CEC) acomete principalmente homens com mais de 50 anos, tabagistas e etilistas. Muitos pacientes com CEC em mucosa bucal já apresentam lesões avançadas, reduzindo seu prognóstico. O objetivo deste estudo foi analisar as características sociais dos pacientes com CEC bucal, correlacionando-as com o tamanho do tumor. Foi realizado levantamento dos casos de CEC bucal, a partir das informações cadastradas de 2000 a 2003, no Hospital do Câncer I (HC I) do Instituto Nacional de Câncer (INCA). Foram coletados dados referentes a sexo, idade, grau de escolaridade, tabagismo, etilismo, distância entre o bairro de residência e o local de assistência médica e local anatômico e tamanho do tumor no momento da admissão do paciente no INCA. Foram analisados 1.308 prontuários, sendo 1.029 homens (78,7%) e 279 (21,3%) mulheres. Tumores de tamanhos maiores foram relacionados a um menor grau de escolaridade (p=0,011). Por outro lado, a distância entre o bairro de residência e o local de assistência não se mostrou estatisticamente significante em relação ao tamanho do tumor (p=0,134). O baixo grau de escolaridade conferiu-se como risco para o tamanho do tumor, mas tal relação não foi observada em relação à distância entre o bairro de residência e o local de assistência. Novos estudos são necessários para minimizar as falhas referentes às variáveis sociais selecionadas neste estudo de modo a permitir uma avaliação mais precisa em relação à associação avaliada.

<b><i>Introduction:</i></b> Subcutaneous allergen-specific immunotherapy (SCIT) is one of the main cornerstones in the treatment of allergic rhinitis in pediatric patients. It has demonstrated symptoms and quality of life improvement, but it is not exempt from adverse reactions (ADVrs). Nevertheless, there are a few reports that have evaluated their safety. Our objective was to evaluate the ADVr to SCIT in pediatric patients. <b><i>Methods:</i></b> We reviewed 786 clinical records with SCIT from 2005 to 2018, comparing the clinical characteristics of patients with ADVrs with SCIT versus a group of a similar number of patients who completed SCIT (control group, CG). The analysis of ADVrs was according to the World Allergy Organization (WAO) 2010 grading system by frequency analysis, survival curve, and log rank. <b><i>Results:</i></b> Of 786 patients, 106 (13.4%) presented ADVrs, and the patients with ADVr had sensitivity and immunotherapy with at least 2 allergens versus CG <i>p</i> &#x3c; 0.001, containing a combination of standardized and nonstandardized allergens (<i>p</i> = 0.003). The ADVrs were in the buildup phase (<i>p</i> &#x3c; 0.001). The survival curve showed that 50% had some reaction at 12 weeks of SCIT. The most frequent ADVr was grade 1 in 73/106 patients (68.8%) and grade 2 in 33/106 (31.1%). The log-rank analysis between the grades of the WAO grading system showed a statistically significant difference (<i>p</i> = 0.02). <b><i>Conclusions:</i></b> The SCIT is safe in pediatric patients. The ADVrs are infrequent, grade 1 being the most reported; however, at &#x3e;12 weeks, the risk of ADVrs that involve 2 organs systems increases.

Estratégias metacognitivas podem ser ferramentas muito úteis, se aplicadas no processo de estruturação e execução do estágio supervisionado, especialmente nos cursos de licenciatura. Por metacognição, compreende-se um tipo elaborado de gerenciamento sobre o próprio pensamento, uma atitude autorregulatória cujo traço distintivo consiste em promover a busca constante do “aprender a aprender”. O andaimento, a documentação e a organização de registros por meio de dossiês ou portfólios, os contratos de aprendizagem firmados entre estudantes e professores, a autoavaliação e o planejamento coletivo são algumas estratégias autorregulatórias que visam instrumentalizar o estudante em contexto de estágio, para que possa atuar nele de forma autônoma e participativa. Para tanto, o contexto que envolve todas as etapas do estágio, desde a sua concepção, passando pela implementação e avaliação, deve proporcionar uma atmosfera que instigue o aluno a conduzir as suas ações e decisões com segurança e assertividade. Consequentemente, o planejamento precisa ser entendido como “coletivo”, contemplando os principais envolvidos no processo, ou seja, os professores orientador e supervisor bem como o próprio acadêmico. Para chegar a esse ponto de maneira satisfatória, a configuração final do estágio não deve ser “engessada”, mas organizada, permitindo antecipar ao mesmo tempo problemas e alternativas para resolvê-los. Tal movimento não é viável se não houver articulação entre universidade e escola visando a um constante aprimoramento dos métodos e técnicas empregados no processo. Nessa perspectiva, além de cumprirem seu papel previsto em lei de associarem de forma eficiente os conhecimentos teóricos e o fazer docente, as práticas de estágio podem se constituir em situações propícias a reflexões, iniciativas, projetos e propostas de intervenção capazes de beneficiar os atores e cenários envolvidos nessa ação.Referências:ALCÂNTARA, Marcelo Silveira de.
