Abstract:
Abstract Background Studies of hospital discharge data and large observational cohorts show that the incidence of venous thromboembolism (VTE) varies by race. We sought to determine the incidence of VTE in each of the following ethnic groups Caucasians, Africans, Asians, and Hispanics. Methods A systematic literature search strategy was used to identify potential studies in MEDLINE, EMBASE, and CENTRAL using an OVID interface. The methodological quality of eligible studies was assessed according to Newcastle-Ottawa Quality Assessment Scale. The primary outcome measure was to identify the incidence/prevalence of VTE of each ethnic group available for the study in the context of a population based study. Results Out of 3418 potential studies, 19 met our inclusion criteria (Table). Of these, 9 studies were population based studies: 5 reported VTE incidence per 100,000 person-years (PY), 2 measured the standardized incidence ratio, and 2 European studies assessed VTE rate according to country of origin but not ethnicity (data not shown). In addition, 7 studies measured the hospital incidence rate of VTE, and 3 assessed VTE prevalence. Data could not be pooled due to marked heterogeneity including varied periods of study. VTE incidence per 100,000 PY was found to be between 162 and 439 in Caucasians, 143 and 746 in Africans, 3.2 and 16.6 in Asians. Hospital incidence of VTE per 100,000 PY was found to be between 21 and 131 in Caucasians, 22 and 155 in Africans, 2 and 26 in Asians, 33.1 and 71 in American Indians/Alaskan Indians, and 9 in Hispanics. Conclusions Our findings suggest a wide variation in reported incidence rates for VTE among different ethnic groups, even within the same group. In general, studies in Asian populations suggest a lower incidence of VTE. A marked heterogeneity of study designs, population settings prevent drawing firm conclusions. A significant risk of bias cannot be excluded for several studies. Further studies assessing concurrently the risk of VTE among different ethnicities in the same geographical area are needed. Abstract:
Background. Acute pancreatitis (AP) is a common surgical acute abdomen. Different kinds of pancreatitis may have different pathophysiological characteristics each other. The objective of this research was to investigate the early clinical features and complications of different types of acute pancreatitis. Methods. 787 AP patients admitted in the Huadu District People’s Hospital of Guangzhou during January 2009 and December 2019 were analyzed retrospectively. Among 787 AP patients, 520 (66.1%) were biliary AP (group I), 69 (8.7%) were alcoholic AP (group II), and 198 (25.2%) were hypertriglyceridemic AP (group III). According to the local and systemic complications and mortality in the early stage, we compared and analyzed the clinical characteristics and prognosis of different types of pancreatitis. Results. Mild acute pancreatitis accounted for the highest proportion (79.4%) in group I, while moderately severe acute pancreatitis in group II (36.2%) and severe acute pancreatitis in group III (62.6%). In terms of severity score of the pancreatitis, the average scores of BISAP, Ranson, APACHE-II, and MCTSI of the patients in group III were the highest ( p < 0.01 ). The incidence of acute peripancreatic fluid collection and infectious pancreatic necrosis was the highest in group III. The incidences of acute necrotic collection, pancreatic pseudocyst, and walled-off necrosis in group III were significantly higher than those in the other two groups ( p < 0.01 ). The incidences of systemic inflammatory response syndrome, sepsis, multiple organ failure, intra-abdominal hypertension, and mortality were highest in group III. Conclusions. There is an upward trend of the incidence rate of hypertriglyceridemic AP in recent years; it has been gradually developed into the second type of acute pancreatitis which is second only to the acute biliary pancreatitis. It is worthy to pay more and more attentions to it due to the feature of its younger onset, high incidence of complications, and high mortality.

RESUMENDe entre todos los objetos, tiempos, espacios, palabras y seres que componen el mundo físico, algunos están investidos de un valor especial por cuanto se les atribuye la virtud de visibilizar las instancias invisibles de las que dependemos los mortales. Es lo sagrado. A lo sagrado se le depara un trato singular hecho de respeto, veneración o miedo, pero en ocasiones también de rencor y de odio por lo que encarna o representa. Es adorado, pero también, y acaso por las mismas razones, puede ser insultado, destruido, objeto de burla y, si tiene forma humana, martirizado o asesinado. La violencia contra lo sagrado puede caber en sistemas religiosos que le otorgan a la agresión un papel central en su universo mítico o ritual. También se ofende u agrede lo santo para grupos o pueblos a someter, puesto que en ello está resumido su orden del mundo. Desde esta perspectiva, el agravio, la irreverencia y el daño pasan a reclamar un lugar protagonista en los estudios sobre la institución religiosa de la cultura bajo las figuras del sacrilegio y la profanación.PALABRAS CLAVE: sagrado, profanación, sacrilegio, violencia religiosa, iconoclastia.ABSTRACTOf all the objects, times, spaces, words and beings that make up the physical world, some are invested with a special value because they are attributed the virtue of making visible the invisible instances on which we mortals depend. It is adored, but also, and perhaps for the same reasons, it can be insulted,destroyed, mocked and, if it has a human form, martyred or killed. Violence against the sacred can fit into religious systems that give aggression a central role in their mythical or ritual universe. Also offended or attacked is what is sacred for groups or peoples to be subdued, since in it an embodiment of their world order. RESUMEN:El objetivo del artículo es aproximarnos a la controvertida biografía del comisario Manuel Brabo Portillo. El trabajo está basado en fuentes primarias y secundarias. El método utilizado es empírico. En el imaginario del mundo sindicalista revolucionario, Brabo Portillo era el policía más odiado, la reencarnación de la cara más turbia del Estado. Fue, así mismo, un espía alemán relacionado con el hundimiento de barcos españoles, el asesinato del empresario e ingeniero Barret y el primer jefe de los terroristas vinculados a la patronal barcelonesa. La conflictividad que afectó a España en el período de la Primera Guerra Mundial es fundamental para entender los orígenes del terrorismo vinculado al pistolerismo, que marcó la historia político social española del primer tercio del siglo XX.PALABRAS CLAVE: Brabo Portillo, pistolerismo, espionaje, sindicalismo, Primera Guerra Mundial.ABSTRACT:The objective of the article is an approach to the controversial biography of Police Chief Manuel Brabo Portillo. He was also a German spy connected with the sinking of Spanish ships, the murder of businessman and engineer Josep Barret and the first head of the terrorists linked to Barcelona employers. LBA1 Background: MDV3100, a novel androgen receptor signaling inhibitor (ARSI), competitively inhibits binding of androgens to the androgen receptor (AR), inhibits AR nuclear translocation, and inhibits association of the AR with DNA (Tran et al, Science. 2009;324:787). MDV3100 was selected for development based on activity in prostate cancer model systems with overexpressed AR, and was active in a phase I-II trial enrolling pre- and post-chemotherapy treated patients with progressive castration resistant disease (CRPC) (Scher et al, Lancet. 2010;375:1437). The AFFIRM trial evaluated whether MDV3100 could prolong overall survival (OS) in men with CRPC who progressed following docetaxel-based chemotherapy. Methods: In this randomized, double-blind, placebo-controlled, multinational phase III study ( NCT00974311 ), patients who had received ≤ 2 regimens of docetaxel-based chemotherapy were randomized 2:1 to MDV3100 160 mg/day or placebo. Treatment with corticosteroids was allowed but not required. Patients were stratified by baseline ECOG performance status and mean brief pain inventory score. The primary endpoint was OS. Other efficacy endpoints included radiographic progression-free survival (PFS), time to first skeletal-related event, time to prostate-specific antigen (PSA) progression, and circulating tumor cell count conversion rate. Results: 1,199 patients were randomized between Sep 2009 and Nov 2010. Based on a planned interim analysis at 520 death events, the Independent Data Monitoring Committee (IDMC) recommended the study be unblinded and placebo patients offered MDV3100 due to a significant OS benefit (p<0.0001; hazard ratio 0.631). The estimated median OS was 18.4 months for MDV3100 treated compared to 13.6 months for placebo treated men, a median OS difference of 4.8 months. Data to be available include PFS, time to PSA progression, and safety. Conclusions: MDV3100, a novel ARSI, significantly improves OS in men with postdocetaxel-treated CRPC reducing the risk of death by 37% relative to placebo. The IDMC determined the risk:benefit of MDV3100 was favorable and recommended the phase III AFFIRM trial be unblinded.

Aims and methodTo assess trends of the American Board of Psychiatry and Neurology examination pass rates before and after the 2003 duty hours regulations (DHR). We obtained the pass rates for part I and II for years 2000–2010. Data were divided pre-DHR (2000–2003) and post-DHR (2007–2010).ResultsDuring the pre-DHR period, first- and multiple-attempt group pass rates were 80.7% and 39.0% which changed in the post-DHR period to 89.7% and 39.1% respectively. Similarly for the part II exam, the pre-DHR first- and multiple-attempt group pass rates were 60.2% and 43.5% respectively, which increased to 78.7% and 53.8%, among the post-DHR group. Overall, there was a significant increase in the first-attempt candidates pass rates for parts I and II, whereas multiple-attempt candidates did not benefit as strongly.Clinical implicationsThe results suggest that the 2003 DHR may have had a positive impact on examination-based medical knowledge in psychiatry.

Buscou-se trazer elementos contidos na legislação e considerações de diversos autores, por meio de pesquisa bibliográfica, assim como foi aplicado um formulário com 19 questões, sobre o comportamento de 20 produtores rurais frente às informações relativas à devolução das embalagens vazias de agrotóxicos. As discussões revelam que cerca de 60% dos respondentes são indiferentes, conhecem muito pouco ou desconhecem sua responsabilidade como primeiro elo da cadeia da logística reversa das embalagens vazias de agrotóxicos, talvez em virtude de falhas no processo de comunicação e na fiscalização. Sugere-se que deva haver maior empenho do Poder Público com vistas a intensificar a fiscalização e tornar mais compreensível as responsabilidades desses agricultores.PALAVRAS-CHAVE: Logística reversa, comunicação e agronegócios, embalagens vazias de agrotóxicos, legislação ambiental. Abstract Abstract 1761 Poster Board I-787 Introduction Myelodysplastic syndromes (MDS) are clonal hematopoietic stem cell disorders characterized by ineffective hematopoiesis culminating in peripheral blood cytopenias and a propensity to Acute Myelogenous Leukemia (AML). The gold standard diagnostic test is the bone marrow aspirate and biopsy but clinicians are often hesitant to refer patients with unexplained cytopenias or macrocytosis for this test. Consequently undiagnosed patients are deprived access to effective treatments. Previously (Buckstein et al., Leukemia Research 33 (2009) 1313–1318) we identified factors which were independently predictive of diagnosing MDS at the time of bone marrow, including; age, mean corpuscular volume (MCV), red cell distribution (RDW), and lactate dehydrogenase (LDH). In this current study, we set out to validate our findings and determine the sensitivity and specificity of our scoring system for routine clinical practise. Methods We reviewed all bone marrow reports conducted at a tertiary care center for the years (January through December) 2006–2008 inclusive. Our inclusion criteria included bone marrows done for unexplained cytopenias and/or macrocytosis. We excluded all outside consultative referrals and bone marrows done for staging or remission assessment in patients with pre-existing or strongly suspected hemato-lymphoid diagnoses. In cases where the patient had more than one bone marrow, only the most recent marrow was used. The bone marrows were reviewed by two experienced hematopathologists, and the diagnosis of MDS was made using the WHO and/or FAB classification systems. Marrows were classified as ‘confirmed MDS’, ‘suspected MDS’ or ‘Not MDS’. Electronic patient charts were reviewed to determine the age, MCV, LDH and RDW at the time of diagnostic bone marrow. A factor was considered positive if the age was > 65 or if its value exceeded the upper range of normal for our laboratory standards (MCV > 96 fl, LDH > 250 IU/L, RDW >14.5%). All patients for whom we had 4 recorded factors were then assigned a score that ranged from 0 to 4. We determined the distribution of scores within this population, and the sensitivity and specificity of this scoring method in predicting a histopathological diagnosis of MDS. Results Three-hundred and forty bone marrows met our inclusion criteria, and 289 (85%) had all four factors recorded at the time of diagnosis. The predictive ability of the MDS score is summarized in Table 1. The probability of diagnosing MDS increased from 8% with a score of 0 to 46% with a score of 3 or 4. A similar trend was seen when all 344 marrows were analysed. Our scoring system had high sensitivity (> 96%), when only one predictive factor was present and high specificity (96%) when 4 predictive factors were present at the time of diagnostic marrow. (Table 2) Conclusions In patients with unexplained cytopenias, or macrocytosis undergoing a diagnostic BM test, the pre-test probability of a MDS diagnosis increases as the number of predictive factors increases. The specificity of our MDS score also follows a similar trend. The sensitivity is high with only one positive factor indicating a lower score can be used as a screening tool. We have effectively validated a scoring system that may help clinicians decide on the utility of arranging diagnostic bone marrow examinations for patients with undiagnosed cytopenias and macrocytosis. EETs also possess anti-inflammatory actions that could protect the kidney vasculature from injury during renal and cardiovascular diseases. A tactic that has been used to increase EET levels has been inhibition of the soluble epoxide hydrolase enzyme. Epoxide hydrolase inhibitors have been demonstrated to be antihypertensive and renal protective. Thus the renal and cardiovascular protective actions of increasing epoxygenase levels could be translated to therapies for preventing end-organ damage.

Tales prácticas, que denominaremos genéricamente web.art 2.0, vinieron a determinar una nueva relación con la Red basada en que los usuarios pasaron a operar como pro-consumidores de información en términos de explotación de lo interactivo, lo multimedial y lo hipertextual, y en base a la potenciación de la creación de contenidos, la negativa de una organización vertical de la Red y una visión global de la realidad local
his thesis examines certain types of artistic production developed over a specific time frame, from 2003 with the evolution of the web to version 2.0 to 2008 as the starting date of the current financial and economic crisis. We will value the particularities of artistic practices developed on the Internet since the end of net.art period, which preceded artistic strategies that have shaped the 2.0 paradigm focused on the management of social networks. El terme de world music, en moltes ocasions, està associat amb la comercialització de la diferència ja que ofereix la possibilitat de promocionar, principalment, a Europa i als E.E.U.U., músiques i musics procedents de països no occidentals. Músiques que molt sovint són adaptades als gustos occidentals, un procés que les descontextualitza i que afavoreix la seva manipulació. A més a més, aquesta etiqueta acostuma a fixar músiques i pràctiques socio-musicals, negant d’aquesta manera la seva capacitat d’evolució i transformació. El context contemporani caracteritzat per la globalització i la intensitat de la circulació dels fluxos culturals, és l’escenari ideal per la world music. Les qüestions relacionades amb el nexe global-local, amb la modernitat, amb la hibridació, amb la interculturalitat, etc., també afecten a la world music ja que la música inclou al mateix temps pràctiques musicals i pràctiques extra-musicals. La capacitat que tenen els fenòmens musicals per construir i expressar imaginaris i narratives, els converteixen en una excel•lent eina de comunicació i evocació. La música entesa en aquest sentit, posa en contacte diferents actors (músics, públic, productors musicals, etc.), que articulen una escena musical. En aquest cas concret, la world music ha posat en contacte músics i musiques de tota la Mediterrània, marcada històricament per encontres i desencontres constants entre diferents cultures i pobles, així com a molt més agents. Tots ells han articulat una escena musical on es reflecteixen relacions desiguals de poder i que és, al mateix temps, testimoni directe de diferents narratives sobre la identitat, la interculturalitat i la hibridació.
El término de world music, en muchas ocasiones, está asociado con la comercialización de la diferencia ya que ofrece la posibilidad de promocionar, principalmente, en Europa y en los E.E.U.U., músicas y músicos procedentes de países no occidentales. Músicas que en muchas ocasiones son adaptadas a los gustos occidentales, un proceso que las descontextualiza y favorece su manipulación. Además, esta etiqueta acostumbra a fijar músicas y prácticas socio-musicales, negando de esta manera su capacidad de evolución y transformación. El contexto contemporáneo caracterizado por la globalización y la intensidad de la circulación de los flujos culturales, es el escenario ideal para la world music. Las cuestiones relacionadas con el nexo global-local, con la modernidad, con la hibridación con la interculturalidad, etc., también afectan a la world music ya que la música incluye al mismo tiempo prácticas musicales y prácticas extra-musicales. La capacidad que tienen los fenómenos musicales para construir y expresar imaginarios y narrativos, les convierten en una excelente herramienta de comunicación y evocación. La música entendida en este sentido, pone en contacto diferentes actores (músicos, público, productores musicales, etc.), que articulan una escena musical. En este caso concreto, la world music ha puesto en contacto músicos y músicas de todo el Mediterráneo, marcado históricamente por encuentros y desencuentros constantes entre diferentes culturas y pueblos, así como a muchos más agentes. Todos ellos han articulado una escena musical donde se reflejan relaciones desiguales de poder y que es, al mismo tiempo, testimonio directo de diferentes narrativas sobre la identidad, la interculturalidad y la hibridación.
Frequently, the term world music is associated with the marketing of the difference because it offers the possibility of promoting, mainly in Europe and the USA, music and musicians from non-Western countries. Usually, in world music’s market, the non-western music adopts Western commercial music patterns. This process promotes the fixation of musical genres and socio-musical practices and, at the same time, denies the experimentation and innovation. The contemporary context characterized by globalization and the intensity of the circulation of cultural flows, is perfect for the world music. Issues relating to the questions about global-local, modernity, hybridization and multiculturalism, etc., also they affect the world music because music includes musical and extra-musical practices. The music has the ability to build and to express, at the same time, different imaginaries and narratives. Music is an excellent tool of communication. Music understood in this sense, contacted different actors (musicians, audience, music producers, etc.) which articulate a musical scene. In this case, the world music has connected musicians and music from the Mediterranean area, historically characterized by encounters of different intensity between different cultures and peoples, as well as many more agents. All they have articulated a musical scene where unequal power relations are reflected and, at the same time, this musical scene is a direct testimony of different narratives about identity, intercultural processes and hybridization.

Abstract:
A fototerapia com lasers em baixa intensidade de lesões de herpes simples tem sido demonstrada clinicamente ora prevenindo a formação de vesículas, ora cicatrizando rapidamente as lesões e até aumentando o espaço de tempo entre o aparecimento dessas manifestações recorrentes. No entanto, os mecanismos básicos de ação dos lasers nessas situações são desconhecidos. Dessa forma, o objetivo do trabalho foi realizar ensaios in vitro utilizando células epiteliais em cultivo e culturas do vírus HSV-1 para estudar a interferência do laser em baixa intensidade na infecção do HSV-1. Material e Métodos: Culturas de vírus HSV-1 e de células epiteliais de macaco (linhagem Vero) infectadas ou não infectadas, crescidas em déficit nutricional (2 % de soro fetal bovino - sfb) foram utilizadas. As irradiações foram realizadas com um laser de GaAlAs (660 e 780 nm, área focal de 3,6 mm2). Uma, duas e três irradiações com intervalos de 6 h foram realizadas. Os grupos experimentais foram: Controle: não-irradiadas; 660 nm/ 3 J/cm2 (28 s); 660 nm/ 5 J/cm2 (38 s); 780 nm/ 3 J/cm2 (19 s) e, 780 nm/ 5 J/cm2 (25 s). Os efeitos citopáticos do HSV-1 e a viabilidade celular de culturas irradiadas e controles foram analisadas em 4 condições: 1) irradiação das células epiteliais não infectadas; 2) células epiteliais irradiadas antes da infecção; 3) irradiação dos vírus antes da infecção; 4) irradiação das células previamente infectadas pelo HSV-1. A viabilidade celular foi obtida pelo teste da redução do MTT e os efeitos citopáticos por observação em microscopia de luz. Resultados: A viabilidade celular de culturas irradiadas crescidas em déficit nutricional, independentemente do número de irradiações, foi sempre significantemente menor que aquela de culturas não-irradiadas e crescidas nas condições ideais de concentração de sfb (10 %). A viabilidade celular de culturas não infectadas foi similar em todos os grupos. O número de irradiações influenciou o crescimento celular positiva e proporcionalmente ao número de irradiações, exceto para o grupo 660 nm/ 3 J/cm2. Nenhuma diferença nos efeitos citopáticos foi observada entre os grupos, independentemente do número de irradiações nas 3 condições do estudo. A viabilidade celular de todos os grupos não mudou nem pela irradiação das células nem do vírus antes da inoculação nas células. A viabilidade de células infectadas antes da irradiação foi significantemente maior que o controle quando 2 irradiações foram realizadas. Conclusão: Nas condições deste estudo a radiação laser em baixa intensidade é capaz de aumentar o crescimento de células Vero crescidas em déficit, no entanto, não o suficiente para atingir o crescimento característico dessas células crescidas nas suas condições ideais. O número de irradiações influencia o crescimento das células de forma positiva e proporcional ao número de irradiações, exceto para o parâmetro 660 nm/ 3 J/cm2. A radiação laser não altera nem a susceptibilidade das células à infecção, nem a virulência do HSV-1. No entanto, ela prolonga a viabilidade das células infectadas pelo HSV-1. Efeitos positivos da fototerapia que tem sido relatados clinicamente parecem ser devido a efeitos no hospedeiro não relacionados com a replicação viral nas células infectadas.
Purpose: The clinical effects attributed to phototherapy relative to Herpes simplex lesions have included prevention of lesion formation, speeding the healing of lesions, and decreasing the frequency of recurrent lesions. The mechanisms underlying these findings have not been established yet. The aim of this in vitro study was to analyze the effect of phototherapy on epithelial cells, on HSV-1, and on infected epithelial cells in culture. Material and Methods: Cultures of HSV-1 and infected or non-infected monkey epithelial cells (Vero cell line) grown in deficient media (2 % fetal bovine serum-fbs) were used. The laser irradiation was delivered using a GaAlAs laser (660 and 780 nm, focal spot of 3.6 mm2). One, two and three irradiations with 6 hourintervals were done. The experimental groups were: Control: non-irradiated; 660 nm/3 J/cm2 (28 sec); 660 nm/5 J/cm2 (38 sec); 780 nm/3 J/cm2 (19 sec), and 780 nm/5 J/cm2 (25 sec). The HSV-1 cytopatic effects and the cell viability of irradiated cultures and controls were analyzed in four different conditions: 1) irradiation of noninfected epithelial cells; 2) epithelial cells irradiated prior infection; 3) virus irradiated prior infection; and 4) irradiation of HSV-1 infected cells. The cell viability was assessed by the reduction of the MTT test and the cytopatic effects by the light microscopy observation. Results: The cell viability of irradiated cultures grown in nutritional deficit, independently of the irradiation numbers, was always significantly smaller than that of non-irradiated cultures grown at the ideal serum concentration condition (10 %). The cell viability of non-infected cells was similar amongst the groups. The number of irradiations influenced the cell growth positively and proportionally to the number of irradiations, except for the 660 nm/3J/cm2 group. Any variation in cytopatic effects was observed amongst the experimental groups, independently of the irradiation numbers at the 3 conditions analyzed. The cell viability of all experimental groups were not altered either by irradiation of the cells or of the virus prior infection. The viability of infected cells prior irradiation was significantly higher than that of non-irradiated cultures when 2 irradiations were done. Conclusion: The experimental conditions for this study demonstrate that the phototherapy is capable of enhancing the growth of Vero cells grown under nutritional deficit conditions, however, not enough to reach the characteristic cell growth of cells grown at the ideal serum concentration condition. The number of irradiations influences the cell growth positive and proportionally, except when the parameter 660 nm and 3 J/cm2 was used. The laser radiation does not change either the susceptibility of the Vero cell to the HSV-1 infection or the HSV-1 virulence; however, prolongs the cell viability of HSV-1 infected cells. Positive benefits of phototherapy that have been reported clinically would appear to be due to host effects unrelated to viral replication in infected cells.

Aquest corpus s’investigarà a partir de l’anàlisi de quatre casos que ocupen diferents espais en la cadena de valor industrial de la música: la banda Txarango, les Cases de la Música, el Grup Enderrock i l’Associació de Sales de Concerts de Catalunya. La metodologia combina tècniques qualitatives i quantitatives, i aposta per un procés argumentatiu comprensiu, altament dialògic i interdisciplinar. Comunicació, Ciències Polítiques i Estudis culturals i Etnomusicologia són els tres vèrtexs temàtics de la recerca. Les aportacions de la tesi són fonamentalment tres. This thesis studies how culture organizes itself to counter the threat of industrial, commercial, global and totalist hegemony with a cultural and communicative counterhegemonic media resistance. The object of the study is Catalan popular music over the 2003-2018 period in a postmodern time frame of political crisis, digital culture, postindustrial communication and progressive independence from institutions. This corpus was researched through the analysis of four instances occupying different spaces in music's industrial value chain: the band Txarango, the dynamization projects of les Cases de la Música, the Enderrock team and the Catalan Concert Venues Association. The chosen methodology combines qualitative and quantitative techniques and relies on a comprehensive highly dialogical and interdisciplinary process setting different theories and perspectives in opposition. Communication, political sciences and cultural studies and etnomusicology are the three thematic focuses of research. The thesis's foremost contributions are three. Ingredients are provided to define a plural third cultural sector, characterized by its micro size, legal and organizational diversity, civil initiative, a vocation for solidarity and community, non-profit nature, mixed funding and pluralistic economic practices. One would locate this third cultual space in a hybrid cultural paradigm found between state and market. The existence of a non-commercial culture is likewise asserted. It emanates from a civil perspective with a vocation for activism and social change. Its organizational model is largely horizontal and highly networked, as local and community ecosystems are its frames of reference for action. Such a phenomenon may foster regional specialization, combining cultural production and a specific symbolic territorial framework, facilitating the flow of symbolic counterhegemonic contents in a musical ecosystem merging coaches, authors, producers, distributors, organizers and consumers. Lastly the thesis proves that digital technology allows for a more interdisciplinary and media-based culture making media resistance possible. Coupled with progressive values and a commitment to social change, a renew cultural resistance emerges that is media-based, independent of mediators and intermediaries, rooted in the DIY tradition, crossmedia, normalizing multidisciplinary and all-encompassing work (360 action), and based on a narrative justifying the very existence of each project socially and politically.
Cette thèse a pour objet d’étude la façon dont la culture, face à la ménace de l’hégémonie industrielle, commerciale, globale et totaliste s’organise à fin d’y répondre avec une résistance médiatique contrahégémonique culturelle et communicative. L’objet d’étude est la musique populaire catalane de la période 2003-2018 dans un cadre postmoderne de crise politique, désinstitutionalisation, culture numérique et communication postindustrielle. Ce corpus sera investigué en partant de l’analyse de quatre cas occupant des espaces différents dans la chaîne de valeur industrielle de la musique: le groupe Txarango, les Maisons de la Musique, le Groupe Enderrock et l’Association de Sales de Concerts de la Catalogne. Quant à la méthodologie, on combine des techniques qualitatives et quantitatives, et on mise sur le processus d’argumentation compréhensif, hautement dialogique et interdisciplinaire. Communication, Sciences Politiques et Études culturels et Ethnomusicologie sont les trois vertex thématiques de la recherche. Les contributions de la thèse sont principalement trois. On apporte des éléments pour définir un tiers secteur pluriel de la culture, caractérisé par sa taille micro, sa diversité organisationnelle et juridique, son initiative civile, sa vocation solidaire et communautaire, sa nature non lucrative, son financement mixte et ses pratiques économiques plurielles. Ce troisième espace de la culture se situerait dans un paradigme hybride de la culture, entre l’État et le marché. On affirme aussi l’existence d’une culture non commerciale, qui se constitue une perspective civile avec une vocation de transformation sociale et d’activisme. Son modèle organisationnel est à tendance plutôt horizontale, avec une puissante structure en réseau et un degré élevé d’intercoopération. Cette culture s’appuie sur l’écosystème local et la communauté pour agir; ce phénomène est capable de promouvoir une spécialisation régionale, qui marie la production culturelle avec un cadre territorial symbolique concret, facilitant la circulation de contenus symboliques contrahégémoniques dans un écosystème sonore qui agglutine les formateurs, les créateurs, les producteurs, les distributeurs, les programmateurs et les consommateurs. Finalement, on démontre que la technologie numérique permet une culture plus médiatique et interdisciplinaire qui rend possible la résistance médiatique, et qui combinée aux valeurs transformatrices fait possible une résistance culturelle renouvelée sous la forme de résistance médiatique, non-intermédiée, avec des traits de tradition DIY, naturalisant l’action 360, narrative et cross-média.

AbstractThis chapter focuses on the carbon footprint of travelling to academic conferences. The cases I present are the last seven General Conferences of the European Consortium for Political Research (ECPR), which are the biggest European conferences in political science, with up to 2000 participants. My estimations show that the travel-induced carbon footprint of a single conference can amount to more than 2000 tons of greenhouse gases—as much as approximately 270 UK citizens emit in a whole year. The average participant produces between 500 and 1500 kg of CO2-eq per conference round-trip. However, by applying three measures (more centrally located conference venues, the promotion of more land-bound travel and the introduction of online participation for attendees from distant locations), the carbon footprint could be reduced by 78–97 per cent. In 2020, the COVID-19 pandemic caused a general shift towards online conferences—the ECPR switched to a virtual event as well. Estimating the carbon footprint of this online-only conference in a more detailed manner shows that the travel-induced carbon emissions—if the event had taken place in physical attendance as originally intended—would have been between 250 and 530 times higher than those from the online conference.

Abstract The other day, tunneling through boxes of books stored for lack of space, I came across a 1945 anthology, The Bedside Tales, subtitled “A Gay Collection with an Introduction by Peter Arno.” The unidentified editor merged golden oldies by Hemingway, Faulkner, and Perelman; forgotten efforts by Hammett, Cain, and Joseph Mitchell; once famous tales now known for the movies they inspired (“The Most Dangerous Game,” “The Sobbin Women”—Seven Brides for Seven Brothers); neglected writers like Bemelmans, Hecht, and Asbury; humor and journalism—a large compilation of found treasure. So it never occurred to me to question the compiler or complain about the inclusion of so-and-so at the expense of what’s-her-name. The delight of anthologies is that of giving oneself up to someone else’s taste. It was precisely that delight that chaperoned many of us through historic jazz during the LP era. If you bought the Columbia Golden Era Series or RCA-Vintage compilations or Decca Heritage collections, you got a short-order survey of flash points, one masterpiece after another. In the 78 era, reissues meant pricey, bulky photo-albums, rarely with more than six or eight sides. The LP greased the process of salvaging history. A jazz expert like George Avakian, Mike Lipskin, or Stanley Dance would select 12 or 14 or 16 tracks that best represented a given artist, and a generation was weaned on the old even as it pursued the new. Then a strange thing happened. After you had committed to memory every measure of the 12 classics on Lady Day, you wanted more. So the label released a boxed set (48 tracks), which did well enough to justify a second box. By now you had heard a helluva lot of Billie Holiday and you wanted to get beyond the anthologist’s subjectivity: You wanted everything. You bought imports and bootlegs, traded tapes, focused on chronology and the sessions themselves, and railed against the capitalist pigs who were keeping all this stuff in their corporate vaults.

Background: The COVID-19 pandemic had an impact on the dynamics of other existing diseases in Brazil, such as Leprosy. Objectives: To analyze the impact of the COVID-19 pandemic on leprosy indicators in the general population and in children under 15 in the state of Sergipe, in 2020. Methods: An ecological time series study was carried out including all new cases of leprosy diagnosed and reported in the state of Sergipe between 2017 and 2020. The variables were analyzed: i. Number of new cases, monthly average and detection coefficient in the general population and in children under 15 years old/ 00 thousand inhabitants, ii. Proportion of ignored / blank / unclassified in the epidemiological variables, iii. Proportion of contacts of new leprosy cases that were examined during the year. A descriptive analysis was conducted. Results: In 2020, there was a 22.2% reduction in the number of new leprosy cases in the general population (325 in 2019; 253 in 2020) and a 44.4% reduction in the general detection coefficient (14.1/100 thousand in 2019; 7.9 cases/100 thousand in 2020). In children under 15 years old, there was a 50.0% decrease in records (22 cases in 2019; 11 cases in 2020) and 49.8% in the detection coefficient (4.2/100 thousand in 2019; 2.1/100 thousand in 2020). The number of municipalities without a record of the disease increased, both in the general population, which went from 25 (33.3%) municipalities in 2019 to 29 (38.6%) in 2020, and in those under 15 years of age, from 63 (84.0%) to 68 (90.7%). There was an increase in the proportion of ignored/ blank/ unclassified fields in the variables race/color, education, clinical form and assessment of physical disability in the diagnosis. The contact examination decreased from 78.7% to 68.9%. Conclusions: The COVID-19 pandemic resulted in a negative impact in the fight against leprosy in Sergipe. The challenges posed by the pandemic must urgently be considered in the development of mitigation plans and or strategies.

Due to its renewable nature, emission advantage and easy adaptation, biodiesel is emerging as an alternative to fossil diesel. There are however concerns on biodiesel storage stability aspect due to the presence of unsaturated content in its composition. This paper discusses on studying the effect of long term storage stability of Karanja derived biodiesel (KME) on the performance, combustion and emission characteristics of a turbo-charged, multicylinder, direct injection diesel engine. For aged karanja derived biodiesel (A-KME) stored in a mild steel container for 500 days, both the fuel properties and the engine performance are found to change. It is observed that the aged fuel as compared to the fresh karanja derived biodiesel (F-KME) showed i) an increase in the acid value and kinematic viscosity from 0.374 mg KOH/g to 0.89 mg KOH/g and 5.6 to 5.7 cSt respectively, ii) a slight decrease in ignition delay and maximum rate of pressure rise, iii) a maximum increase in peak cylinder pressure of about 10% and duration of combustion of 8 deg. CA, iv) a decrease in brake thermal efficiency to an extent of over 4%, v) an increase in un burnt hydrocarbon emissions particularly at low loads, and, vi) a significant increase in exhaust nitric oxide (∼30%) and the smoke emissions (∼78%) at higher loads.

Abstract The purpose of this paper is to demonstrate the results achieved in the first three years of FPSO projects execution after a Program called PROFORT (New FPSOs Program) in Company, which combines relevant contract, engineering and market pillars to achieve Company goals on New FPSOs Program. This paper describes Company surface installation department main initiatives in the PROFORT program. Those initiatives were first identified in 2019 as main pillars of Company Strategic Plan to build FPSOs. The first part of the Program was concluded with the successful signature of the contracts for the P-78 and P-79 FPSOs construction. Some of the initiatives of PROFORT program are related to: (i) contract terms and exhibits; (ii) basic engineering; (iii) bidding process; (iv) basic engineering endorsement and (v) contractual tools. Company has been collecting lessons learned from projects executed over the last 10 years. Based on a prioritized list of lessons learned, PROFORT program defined its action plan, with following key points: 1. The contract was updated to have a single main contractor with a construction (and design) track record consistent with the upcoming complexity of new FPSOs generation (Oil processing capacity: 180 kbpd; Liquid processing: 200 kpd; Water injection: 240 kbpd @220 bar; Gas Compression - 7,2 SMMm³/d; Export – 3 SMMm³/d; Water Depth: ~2.000 m; Spread Mooring; high CO2; contaminants etc.). 2. The FPSO basic engineering evolved to integrate the contract and together with an initial period of validation by contractor (through a basic engineering endorsement). The intention is to bring to the initial phase of the contract the major discussions and therefore to minimize scope changes and consequent impacts on late execution phase. 3. The contractual tools, as a pre-defined small amount of unit prices, were included so that the management of the contract can solve minor scope changes promptly and stop a "snowball" effect. The combined initiatives are the PROFORT program. This program fosters stronger integration and coordination in Petrobras surface installation project teams by sharing lessons learned and existing knowledge. From an external perspective, this way of working has allowed a better alignment of interests between Company and its suppliers with focus on Strategic Plan target of delivering the new FPSOs.

Introduction: Sexually transmitted diseases (STDs) are more common in young people. There are few studies on STDs in the older population, particularly women. Objective: The aim of this study was to evaluate and characterize, with epidemiological variables, the prevalence of STDs in the female population over 50 years old, in a public reference service in Niterói city, Rio de Janeiro State, Brazil. Methods: The study was carried out at the STD Sector of Universidade Federal Fluminense. It was a descriptive retrospective study of quantitative character, carried out with women aged 50 years or older, attended at the aforementioned teaching, research, and extension unit, from 2000 to 2017. Data collection was performed with documentary research from the records of Sexually Transmitted Diseases Sector of Universidade Federal Fluminense. A total of 6,822 records were analyzed, of which 2,363 were of women. Of these, 50 were medical records of women over 50 years old. The variables used were age, education, marital status, use of condom, diagnosis, sexual and behavior characteristics (extramarital relationships and history of homosexuality), skin color, history of STDs, sex education, the number of sexual partners, and family income. Results: There was a higher prevalence of human papillomavirus (HPV) infection in the form of condyloma acuminata in 48% of cases and cervical intraepithelial neoplasia (CIN) I, II, or III in 20%. Syphilis occurred in 14%, genital herpes and trichomoniasis in 6% each, HIV in 4%, and gonorrhea in 2% of cases. Notably, 64% of women had no pathological history of STDs, 6% had a previous diagnosis of syphilis, and 6%, of HPV. The predominant age group was 50–59 (78%), with a higher prevalence in white women (54%). Most patients (66%) reported having one fixed partner, were married (54%), and had no history of extramarital relationships (64%). In addition, 64% of patients had no degree of sex education and 56% lived on less than two minimum wages. Most patients (78%) reported not using condoms. In 50% of cases, the level of education was incomplete primary education and only 8% had concluded higher education. Conclusion: STDs were more frequent in white women who did not use condoms. The most prevalent STD was HPV infection, as condyloma acuminata in pardo women. HPV infection as a cervical intraepithelial neoplasia (CIN) was the second most common STD in white and pardo women.

We report for the first time a stable and simple method for intracavity frequency doubling of a solid-state laser passively modelocked by using a low-finesse antiresonant Fabry-Perot saturable absorber (A-FPSA [1]). With a Neodymium-Lanthanum-Scandium-Borate laser (Nd:LSB) as gain material and a beta-Barium-Borate (β-BBO) nonlinear crystal with type I phase matching, we achieved a conversion efficiency of 48% resulting in 190 mW output power of TEM00 green light at modest pump powers of 1.2 W. Intracavity frequency doubling is favourable for the low to intermediate power regime, where the typical peak intensities of the output beam are not sufficient to obtain high single pass nonlinear frequency conversion efficiencies. But random noise fluctuations in cw intracavity requency doubled lasers, caused by nonlinear coupling between the laser modes and referred to as the green problem [2], made these lasers inappropriate for many applications. This problem can be overcome with a ring laser approach, where 780 mW of green light at a pump power of 4 W could be shown [3], by the use of additional intracavity elements, which make the laser rather complex and critical to operate, or with the use of an external resonator, where up to 200 mW of green light were produced at a diode pump power of 1 W, but which requires electronic frequency locking elements [4], Another possibility to obtain stable output are modelocking techniques, which lock all longitudinal modes together in phase. We describe an all solid-state, completely passive approach for intracavity frequency doubling of a solid-state laser, which is stable, non critical to align, and does not require any electronic control elements.

Objective: To study the risk factors, management protocols and outcome of vulvar cancer cases over a period of 10 years in a tertiary care hospital. Methods: It is a retrospective cohort study of vulvar cancer from January 2004 to January 2014 at King George Medical University, Lucknow. Hospital records of 41 patients with histologically proven diagnosis of vulvar cancer were studied from Department of Obstetrics and Gynecology and Department of Radiotherapy. The presence of risk factors, stage of disease, treatment modalities used and disease outcome in terms survival were studied. The data collected was analyzed and compared with the published literature. Results: The mean age for diagnosis of vulvar cancer was 52 years and peak incidence was seen in age group of 50-70 years. Incidence was significantly more in multiparous (p = 0.001) and postmenopausal women (p = 0.007). An average of 4.1 cases were seen per year. 97.56% cases were squamous cell carcinomas including one case of verrucous carcinoma. Only one non-squamous case of Bowen’s disease was seen. 20 cases belonged to early stage (1 and 2) while 21cases had advanced disease (3 and 4). 48.78% cases were primarily treated with surgery, 26.83% with radiotherapy, 7.3% with chemotherapy and 17.07% with combined chemoradiation. 78% of surgically treated cases had mean survival of 5 years. Mean survival of 1 year was recorded in advanced disease cases. Limitation of the study was poor follow up after treatment. Conclusion: Incidence of vulvar cancer is significantly high in multiparous and postmenopausal women. Surgical treatment is the best option in early stage of disease (stage I and II) and gives high survival rates while advanced disease treated with chemoradiation has poor survival.

Objective: Breast cancer (BC) in young women with positive hormone receptors (HR+) has a poor prognosis. There are possible clinical and biological explanations for these findings, being partially attributed to suboptimal adherence to endocrine therapy (ET). Fertility is one of the key factors for treatment discontinuation. This study aims to estimate the adherence of ET and the overall survival (OS) rate of BC/HR+ in young patients. Methods: We identified women from the public health system, diagnosed with stage I–III BC presenting at one single BC Center, between January 2006 and December 2015. Using the medical records of the hospital database, we constituted a cohort of 74 women aged ≤40 years. The discontinuation rate with associated factors and OS were summarized as percentages. Results: A total of 51 women were BC/HR+. The mean age at diagnosis was 35 years. The median follow-up was 89 months. Among them, 45% had BC recurrence (local and/or distance), and 21% died. A total of 15% of patients interrupted the ET. The reasons for interruption were pregnancy (three patients), menstrual disorders (two patients), and irregular adherence (three patients). Tamoxifen (TMX) was prescribed in 74% of cases. About 19% switched their treatment to aromatase inhibitors. The genetic risk assessment was recommended to 58% of patients, 13% performed genetic tests, and 2% of patients carried out the pathogenic mutation in the high-risk BC genes. Conclusion: This cohort showed 84% of ET intake should be improved. Pregnancy issues and irregular adherence were the main reasons for discontinuation. Considerations of 78% OS in 7 years are that TMX was the only ET for most of the cohort, and it is considered undertreatment according to the current recommendations, and the low rate of genetic tests performed leads unrecognized high risk of potential recurrence in women with hereditary BC. Research in medical records should be addressed as a limitation.

This paper is aiming to investigate the impact of perceived autonomy and perceived risk on attitudes and opinion about two assistive robots (Paro© and Asimo©), as factors explaining the probability to become “friend” with a robot. The worldwide population of elderly people is growing rapidly and in the coming decades the proportion of older people in the developed countries will change significantly. This demographic shift will create a huge increase in demand for domestic and health-care robotics systems. But the spread of robots in everyday life particularly for purposes of healthcare already gives rise to questions about acceptability, moral and legal responsibility. A robotics system can be powerful and useful, there is not a reason why this system is usable and/or desirable and in fine, accepted. It is still unclear how well these new “faux-people” will be accepted by society, for they raise fundamental questions about what it means to be human, especially at home or in nursing house.METHOD. In a large online survey conducted in France, 2 783 participants (936 adolescents with a mean-age of 12.2 years; 1077 adults with a mean-age of 33.4 years; and 770 seniors with a mean-age of 71.3 years) were asked to complete three questionnaires: (1) The DOSPERT scale (for Domain-Specific Risk-Taking; Blais & Weber, 2006) to assess risk attitude and perception of risks for our participants; (2) The revised version of the FQUA-R scale (for Friendship Quality- Revised; Thien, Razak & Jamil, 2012) to assess close relationships and potential friendship with a robot; (3) The PAS (for Perception of Autonomy Scale; Lombard & Dinet, 2015) to assess positive and negative attitudes towards autonomy of robots. Each participant was asked to complete the three questionnaires twice: before and after viewing two videos showing two assistive robots (Paro© and Asimo©) interacting with human people: In one of the videos, a young woman interacts with the robotic baby-seal Paro©, and gives many explanations about the interests for elderly people (“Paro© gives kindness”, “its allows to create an attachment”). Moreover, we can see an elderly woman who caresses Paro©. In the other video, several physical characteristics of Asimo© are presented (size, weight) and the robot performs several tasks by interacting with a young woman (Asimo© walks, runs, plays football, opens a bottle, serves a glass, etc.).RESULTS AND DISCUSSION. For the two robots, structural equation modelling was used to determine the relationships between all the variables. Results have mainly showed that (i) Perceived risk is mainly and significantly explained by attitudes about risks in health and social domains whatever the gender and the age, and (ii) Perceived autonomy has a direct and positive effect on Friendship quality. In other words, our results tend to confirm that our three factors (perceived risk, perceived autonomy and friendship) are strongly interrelated and should be integrated in studies investigating the acceptability of assistive robots, and confirm that these three factors have different impact according to the physical appearance of the robot (human-like for Asimo© or animal-like shape for Paro©). Industrial and theoretical perspectives are discussed.

The increased automation of the car manufacturing process, in which much of the assembly has been delegated from man to machine, has done much to relieve workers of the burden of heavy lifting. However, despite ergonomic improvements in the workplace, many jobs still require workers to perform repetitive tasks [1]. In the automotive industry, Work-related Musculoskeletal Disorders (WMSD) are one of the most common occupational problems due to repetitive working tasks. Workers that perform manual work are often prone to awkward postures, repetitive movements, forceful exertions, and overextensions, which are some of the main factors for the arising of WMSD [2]. Besides, these work-related factors, also the personal factors contribute to the occurrence of this kind of injury, making WMSD a complex condition that involves contributions from many factors [3,4]. The current study aims to compare different observational methods commonly used to assess the WMSD risk in repetitive tasks. To accomplish this goal a case of study in assembly workstation of an automotive company in Portugal was applied. It was selected methods that are widely used by ergonomists and are validated for implementation in the industry [5,6]. Therefore, the following methods were applied to an assembly workstation: (i) Rapid Upper-Limb Assessment (RULA), (ii) Occupational Repetitive Actions (OCRA), (ii) Key Indicator Method – Manual Handling Operations (KIM-MHO), and (iv) Revised Strain Index (RSI) This multi-method approach was very important, as it allowed for a more comprehensive assessment, which will support the proposals for improvement The results show that workstation present a considerable WMSD risk in 3 of 4 methods applied. These results suggest that a change to the workstation is necessary. A possible solution would be to implement a Human-robot collaboration solution, in order to reduce the physical demands associated with repetitive movements [7] to which workers are subjected. Abstract:
This report (MIF/GN-78-9) comprises the evaluation of MIF's projects supporting private participation in infrastructure performed by the Office of Evaluation and Oversight. The evaluation was initiated in 2002, covering four thematic groups of projects: Alternative Dispute Resolution (GN-78-2), Microfinance (GN-78-3), and Capital Markets & Financial Reform (GN-78-4). During 2003, as established in the program approved by the Donors Committee (GN-78-1), the project groups to be evaluated include the rest of the MIF thematic areas of intervention: (i) Private Provision of Infrastructure Services; (ii) Human Resources Development (including skills standards and labor market reforms); (iii) Business Development Services (including quality standards and promotion of trade and investment); (iv) Venture Capital Development; (v) Environment and Eco-Efficiency; and (vi) Promotion of Competition and Consumer Protection.

Abstract:
The objective of this report (MIF/GN-78-11) is to present the results of the evaluation of the initiative taken by the MIF to support Venture Capital Development in the region. The point of departure for this evaluation was to look at the international experience in SME/Venture Capital development and the benefits that this dynamic segment can bring to the economy as a whole. order to proceed with the evaluation, a special methodological framework was developed by OVE to capture the specific characteristics of MIF interventions. Also an initial estimate of its entire project portfolio was done, identifying the main thematic project groups for which common reference points could be established and meaningful lessons could be drawn. The evaluation was initiated in 2002, covering four groups of projects: (i) Financial Reform, (ii) Capital Markets; (iii) Microfinance; and (iv) Alternative Dispute Resolution. These four groups represented 134 projects from 16 countries with a total approved value US$159.75 millions of MIF resources.
