Abstract:
This chapter considers some determinants that lie completely outside of people’s own control. For such exogenous causes of ill health, the unlucky ones cannot be held responsible for their misfortune. Still, some of these causes are avoidable, in the sense that effective policy interventions exist. Biological variations are in general unavoidable. The chapter investigates two types of determinants associated with early life circumstances. The most systematic health difference that an individual is affected by is whether born a boy or girl: women live 5–6% longer than men. Childhood differences in health follow a strong social gradient, and some figures are included to prove this sad fact. One additional exogenous determinant is the physical environment that affects people’s health. A simple model is presented to show how unhealthy externalities can be reduced by imposing pollution taxes: the polluter pay principle.

Abstract:
In the last twenty to thirty years, research on affective determinants of health behavior has proliferated. For the first time, this burgeoning area of research is reviewed and discussed in a single volume, with chapters from leading experts in their respective areas. Authors focus on a range of affective concepts, including, but not limited to, hedonic response, incidental affect, perceived satisfaction, anticipated affect, affective attitudes, and affective associations. In the first part of the book (chapters 2–10), the role of affective concepts is highlighted and expanded in multiple theories of health behavior, including theories of action control, self-regulation, self-determination, dual-processing, affective associations, and hedonic motivation. The second part of the book (chapters 11–20) focuses on the role of affective concepts in specific health behavior domains, including physical activity, eating, smoking, substance use, sex, tanning, blood donation, performance of health professionals, cancer screening, and cancer control. Each chapter offers a summary of existing research, as well as the authors’ insights on possible new directions and implications for intervention.

Abstract:
This chapter summarizes common themes and some ongoing controversies addressed in this volume. Common themes include the growing agreement regarding distinctions between affect proper and cognition about affect, between incidental and integral affect, between implicit and explicit affective attitudes, and between instrumental and affective outcome expectancies/attitudes. The need for a common taxonomy of affect constructs is clear, and this chapter provides preliminary distinctions as a starting point for further refinement. It overviews aspects that require more in-depth theorizing such as behavior-specificity in affect–behavior relationships, and the relationship among affect, social processes, and behavior. Finally, it highlights how the impact of affective determinants on health behavior may be intervened on via three separate routes (independent, direct, moderated), using examples across various chapters within the volume.

Abstract:
Abstract Until recently, the commercial determinants have remained largely absent from conceptual frameworks of the social determinants of health, despite their clear importance to health and well-being. This is especially challenging because even a single large industry sector can have a profound, intersectional impact on sociocultural and physical environments. Such a sector can affect everything from the consumption patterns of a particular product and the health and social problems caused by those patterns of consumption to the social norms surrounding when and how much of it people use, the tax and regulatory frameworks surrounding it, the science regarding its harms and benefits, how policymakers view the problem and its causes, and the framing of possible solutions. The commercial determinants of health are emerging as a field of study in their own right; however, currently, there is no book that seeks to synthesize current definitions, frameworks, and empirical research into a coherent research and translational entity and to help the field make the leap to its next level. This book aims to fill that gap by making the case for why commercial determinants of health matter and discussing what is known about them, examples of the ways in which they affect population health, and ways to research and influence them as the field continues to grow and evolve.
