Abstract:
The presence of PCBs in water bodies is an emerging issue for water quality. This is because, once PCBs reach a lake or river, they concentrate in the sediments by attaching to the organic matter. Therefore, the objective of this paper is to evaluate the level of polychlorinated biphenyls (PCBs) in sediment from surface waters of Igodan, Okunmo, Lebi, Idepe and Oaustech in Okitipupa, Ondo State Nigeria using appropriate standard methods. Data obtained reveals that in Igodan the concentration of PCBs in sediment w.as PCB 44 (5.42±0.00), PCB 18 (3.04±0.00), PCB 28 (3.27±0.07) and PCB 170 (6.60±0.00). Okunmo PCBs concentration in sediment was PCB 44 (0.67±0.00), PCB 18 (0.03±0.00), PCB 28 (0.32±0.00) and PCB 170 (0.18±0.00). Lebi PCBs concentration in Sediment was PCB 44 (0.82±0.00), PCB 18 (3.96±0.00), PCB 28 (1.12±0.00) and PCB 170 (1.08±0.00). Except for PCB 170 (0.33±0.02), the PCB concentration in sediment from the Idepe and the OAUSTECH were both below the detection limit (BDL). Correlation between Physicochemical Parameters and PCBs homologs shows a negative correlation for the relationship of EC (Electrical Conductivity), TOC (Total Organic Carbon) and pH at ( -0.283), (-0.262) and ( -0.246) respectively. According to multivariate investigation, releases from nearby factories, deterioration of electrical cables and transformers, waste from university laboratories and facilities, as well as diesel residues from lister engines could be associated with the elevated levels of PCBs in sediments collected from the five sampled sites in Ondo State. TriPCBs > tetraPCBs were in this order in which the median concentrations of PCB homologues were found in sediments from the surface water.

Abstract:
Alterations in glycoproteins, important cell surface constituents, have long been associated with various malignancies. The present investigation therefore explored the clinical significance of a glycoproteomics approach in patients with oral precancerous conditions (OPC) and patients with oral cancer. The study included 80 oral cancer patients, 50 patients with OPC, and 84 controls. Native polyacrylamide gel electrophoresis followed by Schiff's staining was carried out to study the alterations in glycoproteins. The results showed significant elevation (p<0.0001) of 192 kDa, 170 kDa, 116 kDa and 44 kDa glycoproteins in oral cancer patients and patients with OPC compared with controls. The odds ratio indicated a significantly higher risk for oral cancer among users and especially chewers of tobacco. The levels of all the glycoprotein bands (192 kDa, 170 kDa, 116 kDa and 44 kDa) were higher in patients with a habit of tobacco use (WHT) than in patients with no habit of tobacco (NHT) and were also higher in WHT controls than in NHT controls. Moreover, a 230 kDa glycoprotein consistently appeared only in individuals with tobacco habits and an increasing trend was observed from WHT controls to patients with OPC to WHT oral cancer patients. In conclusion, the results indicated the potential utility of glycoprotein alterations in monitoring sequential changes occurring due to tobacco consumption during neoplastic transformation.

Abstract:
AbstractMaternal elaborative reminiscing supports preschool children's autobiographical memory, self-concept, and emotion understanding. What are the factors contributing to mothers' elaborative style of reminiscing? In a longitudinal community sample (n = 170 at the final data point), this study explored the role of maternal depression (8–44 months), maternal sensitivity and maternal mind-mindedness (8 months), as well as child factors of joint attention (15 months), attachment security (15 months), and language (26 months) for mother–child reminiscing about a positive (happy) and a negative (scared) event at 44 months. Mothers could be classed into two groups of low versus increasing depression from 8 to 44 months, yet maternal depression did not uniquely predict mother–child reminiscing after accounting for maternal sensitivity and other factors. Instead, maternal sensitivity, children's joint attention, and language uniquely predicted children's elaborations about the scared event at 44 months, and maternal sensitivity uniquely predicted mothers' elaborations about the scared event at 44 months. Mothers who are more sensitive in early interactions may later be better at engaging their children when reminiscing about negative emotions. These findings have implications for the design of interventions targeted at supporting mothers to engage in elaborative reminiscing with their preschool children.

Abstract:
Sesame (Sesamum indicum L.) is a short duration, low input and highly drought tolerant conventional oilseed crop with high edible seed oil contents. This study was aimed to evaluate the response of four sesame genotypes to silicon (Si) application under a semi-arid climate. For this study, we hypothesized that Si application may improve seed yield, oil contents and fatty acid composition in sesame. The experimental design was a two factor-factorial randomized complete block, replicated four times, and the treatments were four sesame genotypes (TS-3, SG-120, SG-169 and SG-170) and three Si levels (0, 22 and 44 kg ha-1). The data was analyzed statistically with the ‘Statistics 8.1’ software. The genotype TS-3 had the highest (p£0.05) seed yield (479.1 kg ha-1), oil content (40.2%), oleic acid (41.8%), and the lowest content of palmitic acid (6.37%) and linoleic acid (38.5%). Silicon application significantly enhanced the seed yield, oil and unsaturated fatty acids (oleic and linoleic acid) contents and reduced the saturated fatty acid (palmitic and stearic acid) over control. The highest seed yield (487.8 kg ha-1), oil contents (38%), oleic acid (40.9%), linoleic acid (41.7%), as well as the lowest palmitic acid (6.49%) and stearic acid (3.66%) were recorded with the application of 44 kg Si ha-1. The seed yield of sesame genotypes followed the order TS-3>SG-120>SG-169>SG-170.

Abstract:
CYP450AAM [arachidonic acid metabolites of the CYP450 (cytochrome P450) enzyme system] have a range of biological functions. CYP450AAM are involved in the pathogenesis of hypertension, renal function and vascular function, yet their role in stroke has not been clarified. We aimed at determining the levels of circulating CYP450 metabolites in patients with acute ischaemic stroke (<96 h) compared with healthy age- and gender-matched controls. This was a retrospective case-controlled study of 44 acute ischaemic stroke patients and 44 matched controls. A subset of acute ischaemic stroke patients was available for follow-up. Acute ischaemic stroke patients had elevated plasma CYP450AAM, including 20-HETE (20-hydroxyeicosatetraenoic acid) (1921±170 compared with 1108±170 pmol/l, P<0.001), EETs (epoxyeicosatrienoic acids) (77.88±3.34 compared with 35.35±3.34 nmol/l, P<0.0001) and DiHETEs (dihydroxyeicosatetraenoic acids) (92.87±4.61 compared with 68.17±4.61 nmol/l, P<0.0001), as well as increased plasma F2-isoprostane levels (3754±538 compared with 1947±538 pmol/l, P<0.02), the latter a marker of oxidative stress, compared with controls. In a subset analysis of the stroke patients, plasma 20-HETE, EETs and F2-isoprostanes were attenuated 30 days after the stroke. Baseline 20-HETE levels were also associated with lesion size and functional indices within the stroke patients. The present study highlights the elevation in CYP450AAM and oxidative stress in acute ischaemic stroke patients. Further investigation of the effect this has on long-term clinical outcome or whether this can be modified by treatment is warranted.

Objective: To describe the epidemiological characteristics of human rabies at IDH, Dhaka in 2006. Methods: Analysis of the records of rabies cases admitted into the Infectious Disease Hospital, Dhaka in 2006 (Jan-Dec). Results: Of total 170 rabies cases, 116(68.2%) cases were admitted to IDH from Dhaka division and rest 44(31.8%) cases came from other areas of Bangladesh. Majority (59.5%) of the victims were within 15 years of age. Male (67.1%) gender & rural people (89.4%) constituted the main bulk of the cases. 86.5% cases had WHO category III exposure. Majority cases had incubation period between 16-90 days (82.9%). Hydrophobia which is the main pathognomonic feature of human rabies, was present in cent percent cases. Principal biting animals was dog (92.94%). Others animals were cat(5.3%),jackal (0.6%), mongoose (1.2%). Most cases (143/170=84.1%) were not vaccinated. 99% cases did not take wound care. Among 27 cases who received vaccines, only 6 cases (22.2%) received TCV inadequately and remaining 21(77.8%) received NTV. None of 170 cases were treated with RIG. Conclusion: Rabies is a major health problem in Bangladesh. The data indicates that the epidemiological characteristics of the disease are not much different from other studies and have not changed much over the decades. Hence, there are need to educate the community and health workers about the importance of immediate and adequate post-exposure treatment, to start effective control programme for dog and to make availability of TCV as well as RIG. Patients were followed for a minimum of 36 months, with the primary outcome being time to disease relapse.Results:190 patients were enrolled and 170 randomized at 4 months (85 to RTX; 85 to AZA). The data are complete on all patients up to at least month 24. Median age was 59 years (range 19-89), with a prior disease duration of 5.3 years (0.4-38.5). 123/170 (72%) patients had a history of testing positive for anti-proteinase 3 ANCA; 47/170 (28%) for myeloperoxidase ANCA; 104/170 (61%) were enrolled having suffered a major relapse, and 48/170 (28%) received a pre-specified higher dose glucocorticoid induction regimen (Table 1).Table 1.Baseline characteristics of patients enrolled in RITAZAREM trialRituximab (N=85)Azathioprine (N=85)Total (N=170)Age, years: median (range)57 (18-89)61 (27-83)59 (18-89)Female, number (%)42 (49.4%)44 (51.8%)86 (50.6%)Disease duration, years: median (range)5.8 (0.4-38.5)4.9 (0.4-25.8)5.3 (0.4-38.5)Prior cyclophosphamide therapyNumber of patients (%)67/85 (78.8%)66/85 (77.6%)133/170 (78.2%)Cumulative dose, grams (g): median (range)7.1 g (0.2-301)12 g (1.0-146)10 g (0.2-301)Prior rituximab therapyNumber (%) patient33/85 (38.8%)27/85 (31.8%)60/170 (35.3%)Cumulative dose, grams (g): median (range)3.2 g (2.0-16.0)5.4 g (1.5-14.0)3.9 g (1.5-16.0)Glucocorticoid induction regimen1mg/kg/day starting dose24/85 (28.2%)24/85 (28.2%)48/170 (28.2%)0.5mg/kg/day starting dose61/85 (71.8%)61/85 (71.8%)122/170 (71.8%)ANCA typeAnti-proteinase 361/85 (71.8%)62/85 (72.9%)123/170 (72.4%)Anti-myeloperoxidase24/85 (28.2%)23/85 (27.1%)47/170 (27.6%)Relapse type upon entry into trialSevere52/85 (61.2%)52/85 (61.2%)104/170 (61.2%)Non-severe33/85 (38.8%)33/85 (38.8%)66/170 (38.8%)RTX was superior to AZA in preventing disease relapse with a preliminary overall hazard ratio (HR) estimate of 0.36 (95% CI 0.23-0.57, p <0.001) and a during-treatment HR estimate of 0.30 (95% CI 0.15-0.60, p<0.001) (Figure 1). Abstract:
Asphyxia is a condition in which a newborn cannot breathe spontaneously and regularly which is characterized by hypoxemia, hypercarbia and acidosis. The infant mortality rate in Klaten Regency in 2018 was 10.77 / 1000 live births. The absolute number of infant deaths is 170 out of 15,786 live births. Of the 170 infant deaths, 68 infant deaths were caused by Low Birth Weight (LBW), 15 deaths caused by asphyxia, 46 deaths caused by congenital disorders, 5 deaths caused by sepsis, 1 death caused by pneumonia, 3 deaths caused by diarrhea, and 32 death from other causes. The research objective was to determine the relationship between premature rupture of membranes and the incidence of newborn asphyxia. This type of research is an analytic survey with a cross time approach. The population of this study were 44 people with premature rupture of membranes and asphyxia of newborns in RSUI PKU Muhammadiyah Delanggu in 2019. The sampling technique used was non-probability as many as 44 people. The data collection tool used documentation sheets. Data analysis was performed using Chi-square. The results showed that 37 infants (54%) mild asphyxia, 6 infants (14%) moderate asphyxia, 1 infant (2%) severe asphyxia and 36 mothers (82%) had no to 8 mothers (18)%. The conclusion in this study is that there is no relationship between premature rupture of membranes and the incidence of newborn asphyxia at RSUI PKU Muhammadiyah Delanggu with a value of p = 0.397 (p <0.05). Health workers and the government should work together in order to improve the quality of health services to prevent premature rupture of membranes and asphyxia of newborns and for further research it is hoped that research can be carried out with primary data.

Abstract:
Inhaled Corticosteroids (ICS) are commonly prescribed to patients with severe COPD and recurrent exacerbations. It is not known what impact ICS cause in terms of COVID-19 positivity or disease severity in COPD. This study examined 27,810 patients with COPD from the Cleveland Clinic COVID-19 registry between March 8th and September 16th, 2020. Electronic health records were used to determine diagnosis of COPD, ICS use, and clinical outcomes. Multivariate logistic regression was used to adjust for demographics, month of COVID-19 testing, and comorbidities known to be associated with increased risk for severe COVID-19 disease. Amongst the COPD patients who were tested for COVID-19, 44.1% of those taking an ICS-containing inhaler tested positive for COVID-19 versus 47.2% who tested negative for COVID-19 (p = 0.033). Of those who tested positive for COVID-19 (n = 1288), 371 (28.8%) required hospitalization. In-hospital outcomes were not significantly different when comparing ICS versus no ICS in terms of ICU admission (36.8% [74/201] vs 31.2% [53/170], p = 0.30), endotracheal intubation (21.9% [44/201] vs 16.5% [28/170], p = 0.24), or mortality (18.4% [37/201] vs 20.0% [34/170], p = 0.80). Multivariate logistic regression demonstrated no significant differences in hospitalization (adj OR 1.12, CI: 0.90–1.38), ICU admission (adj OR: 1.31, CI: 0.82–2.10), need for mechanical ventilation (adj OR 1.65, CI: 0.69–4.02), or mortality (OR: 0.80, CI: 0.43–1.49). In conclusion, ICS therapy did not increase COVID-19 related healthcare utilization or mortality outcome in patients with COPD followed at the Cleveland Clinic health system. These findings should encourage clinicians to continue ICS therapy for COPD patients during the COVID-19 pandemic.

Abstract:
Production of the honeycomb or thin walled structures by the selective laser melting of powder is of a great scientific and practical interest. It’s because that such approach allows producing structures practically of any configuration and thickness, unobtainable or very hard to produce by traditional methods. Using the additive technology, the principal difference from the traditional is that the honeycomb structures can be produced practically of any shape, and the method itself is 100% waste-free, because there is no need in supporting structures. The results of the structure investigation of the honeycomb structures with the wall thickness of 80-170 μm and 44 mm height are presented along with the correlation dependencies between samples mass, wall thickness, activation and compression loads.

Abstract:
Abstract Evaluation of recurrence in post-treatment glioma is challenging because contrast-enhancing (CE) lesions are a mixture of tumor and treatment effect. This study characterizes intratumoral heterogeneity using quantitative digital pathology to correlate intraoperative MRI-localized biopsies with histopathology in the post-treatment setting. Findings will inform multiparametric radiographic models of intratumoral heterogeneity. A retrospective review was performed on adult patients with MRI-localized biopsies obtained during resection for post-treatment recurrent high-grade glioma. 68 patients and 170 MRI-localized samples were analyzed (median 2 samples/patient). Immunohistochemistry (IHC) for markers of glioma cells (SOX2), macrophages (CD68), and proliferating cells (KI67) was used to characterize biopsies. Slides were digitized and quantified using an automated cell-counting algorithm. Histopathological criteria based on IHC data was developed to classify biopsies. IHC quantification was compared across histological groups using ANOVA and paired t-tests. Most patients (52/68) yielded multiple biopsies. 75% (39/52) demonstrated heterogeneity in histological classification of all specimens obtained from their lesion. 47/170 (28%) biopsies were predominantly treatment effect, and most were CE (31/47 or 66%). Only 75/170 (44%) biopsies contained recurrent glioma, and 21/75 (28%) were NE. SOX2 labeling index was higher in biopsies containing recurrent tumor (p=5.13E-25). CD68 labeling index was higher in biopsies with predominant treatment effect (p=1.35E-12). IHC data from MRI-localized biopsies informed a multiple linear regression model which demonstrated significant predictive value for determining the distribution of recurrent tumor in the post-treatment setting. Contrast enhancement is not a reliable predictor of tumor in recurrent high-grade glioma. Most patients demonstrated marked intratumoral heterogeneity, highlighting the difficulty of accurate tumor sampling post-treatment glioma. Our histopathological classification significantly distinguished recurrent tumor from treatment effect and informed a multiparametric radiomic model which can guide surgical sampling and assess response to therapy.

Abstract:
Here we describe the livestock performance and baseline productivity over a two-year period, following the establishment of the infrastructure on the North Wyke Farm Platform across its three farmlets (small farms). Lowland permanent pastures were continuously stocked with yearling beef cattle and ewes and their twin lambs for two years in three farmlets. The cattle came into the farmlets as suckler-reared weaned calves at 195 ± 32.6 days old weighing 309 ± 45.0 kg, were housed indoors for 170 days then turned out to graze weighing 391 ± 54.2 kg for 177 days. Therefore, it is suggested for predominantly grass-based systems with minimal supplementary feeding that target live weight gains should be 0.5 kg/day in the first winter, 0.9 kg/day for summer grazing and 0.8 kg/day for cattle housed and finished on silage in a second winter. The sheep performance suggested that lambs weaned at 100 days and weighing 35 kg should finish at 200 days weighing 44 to 45 kg live weight with a killing out percentage of 44%. Good levels of livestock production are possible with grass and forage-based systems using little or no additional supplementary concentrate feeds.

Abstract:
As the production, distribution and consumption of iodized salt has increased in recent years, this study was carried out to assess iodine status in Tehran in 1996. 1146 families comprising 5140 subjects in the twenty districts of Tehran city from all age groups were randomly selected. Thyroid size was examined by palpation and graded according to the WHO classification. In 163 families selected randomly, thyroid size was determined by ultrasonography and urinary iodine was measured by digestion method. Serum T4, T3 and TSH (IRMA) concentrations were also assayed by kits. Percentage of grades 1 & 2 goiter were 44 & 44% in females and 49 & 33% in males respectively. Median urinary iodine was 17.5 mug/dl. Mean serum T4, T3 and TSH were 8.41 ± 1.4 mug/dl, 170 ± 37 ng/dl and 1.4 ± 0.8 mu/ml, respectively. In 118 children aged 6–10 years median urinary iodine was 17.5 mug/dl. Thyroid volume in children was 4.3 ± 1.9 ml. No correlation was established between the thyroid volume and goiter grade. This study points to the adequacy of iodine intake in the majority of families residing in Tehran.

Abstract:
e18060 Background: Efficacy of nasopharyngeal carcinoma (NPC) with T4 or N3 remains unsatisfactory due to high-risk of distant metastasis or recurrence, indicating the need of more comprehensive treatment. We aim to analysis the efficacy and safety of tislelizumab and GP regimen in combination with neoadjuvant therapy and concurrent chemoradiotherapy (CCRT) plus nimotuzumab in patients with high-risk NPC. Methods: Our team conducted a retrospective analysis of 44 adult patients with high-risk locally advanced (T4 or N3; M0) nasopharyngeal carcinoma between March 2021 and January 2022, the median follow-up time was 599 days. The patients had received neoadjuvant chemotherapy (gemcitabine,1000 mg/m2, days 1 and 8, cisplatin,80 mg/m2, day 1), and tislelizumab (200mg) on day 1 every 3 weeks for 4 cycles followed by chemoradiotherapy including intensity modulated radiotherapy (IMRT), lobaplatin and nimotuzumab. Lobaplatin was intravenously administered at a dose of 30 mg/m2 on days 1 and 22. Nimotuzumab was intravenously administered at a dose of 200 mg on day 1 every week for 6 weeks. All patients underwent response assessments during neoadjuvant therapy, chemoradiotherapy and follow-up at regular intervals. Results: 44 patients were identified with the median age being 50y(24y-70y). At neoadjuvant treatment period, 44 patients completed 170 cycles of the combination of tislelizumab and GP regimen. During concurrent chemo-radiotherapy period, 42 (95.45%) of 44 patients completed 6 weeks of nimotuzumab treatment and 41 (93.18%) of 44 patients completed 2 cycles of lobaplatin treatment. Overall, 20 patients (45.5%) had a complete response, and 24 (54.5%) had a partial response after neoadjuvant therapy before the start of chemoradiotherapy. At 3 months and 6 months after radiotherapy, 90.9% (40 of 44) and 97.7% (43 of 44) of the patients had a complete response. 42 of the 44 patients were alive (95.5%). 3 events of recurrence(1) or death(2) were recorded. 19 (43.2%) of 44 patients had grade 3-4 treatment-related adverse events, and grade 3-4 immune-related adverse events(irAE) was recorded in 3 patients (6.8%) in the entire treatment. Grade 3-4 irAEs were rash(2) and hypothyroidism(1). Conclusions: Neoadjuvant therapy with tislelizumab and GP regimen followed by concurrent chemoradiotherapy plus nimotuzumab seems to have promising efficacy, lower toxic effects and better treatment compliance, which is a new treatment strategy for high-risk NPC.

Abstract:
Screening and brief intervention (SBI) decreases alcohol use and related consequences among trauma patients. Although SBI is required in Level I and II trauma centers, implementation often is difficult. This study used the Plan-Do-Study-Act approach to identify and implement measures to increase the number of patients receiving SBI at a Level I trauma center. A multidisciplinary Quality Improvement Committee with representation from the Trauma Service and SBI Team met monthly during 2011. Stepwise interventions included identifying a resident “champion” responsible for screening, brief intervention, and referral to treatment, including an SBI report at monthly trauma conferences, and incorporating SBI into the trauma order set. Outcomes measures were number of patients screened, patients screening positive, and the number of patients receiving SBI. At baseline, 170 of 362 patients (47%) were screened, 68/170 (40%) had positive screens, and 30/68 (44% of those with positive screens) received SBI services. Quarter 2 saw increases in patients screened—275/437 (63%), patients screening positive (106/275; 39%) and those receiving SBI (60/106; 57%). Increases culminated in Quarter 4 with screening 401/466 (86%; P < 0.001) patients, 208/401 (52%; P < 0.001) patients screening positive, and 114 patients (55%; P = 0.296) receiving services. Use of similar quality improvement measures nationwide could improve rates of provision of this important service.

Development goals, priorities and strategies need to be based on available (local) resources, which are identified as either comparative advantages or uniform development goals at certain planning level. Core development policies of the region take centrum-periphery theory as the primary approach in planning allocating development funds. In this paper authors developed four settlement clusters based on their local resources (HCSO, AC, T-STAR database, 44 variables, cross section 2018). Few Strong and capable cities (n=31) are accompanied by mainly Agricultural villages (n=155) and there are settlements (n=170) with Concentrated capacities. But majority of the region’s area (n=299) has Limited resources. The classification of the settlements can be used in gravity models that test the pull effect of central cities on the complex development of the surrounding settlements.

Abstract:
Abstract The Entamoeba histolytica galactose-binding lectin is a surface glycoprotein composed of 170- and 35-kDa subunits. Inhibition of this lectin with galactose or anti-170 kDa subunit polyclonal antibody blocks amebic adherence to target cells and colonic mucin glycoproteins. We describe the properties of 10 mAb with specificity for the 170-kDa subunit. Based on competitive binding studies, six nonoverlapping antigenic determinants on the lectin were identified. The effect of the mAb on adherence of amebic trophozoites to both Chinese hamster ovary (CHO) cells and human colonic mucins was measured. Antilectin antibodies directed against epitopes 1 and 2 enhanced adherence, with the number of amebae having at least three adherent CHO cells increasing with the addition of epitope 1 mAb from 26 +/- 9 to 88 +/- 2% and the binding of colonic mucins increasing from 34 +/- 1 to 164 +/- 3 pg/10(5) amebae. Antibody-enhanced adherence remained 90 to 100% galactose inhibitable, occurred at 4 degrees C and was not Fc mediated. Univalent Fab fragments of epitope 1 mAb augmented mucin binding by 238% and CHO cell adherence by 338%. The binding of purified lectin to CHO cells was increased from 1.1 +/- 0.1 to 2.4 +/- 0.3 ng/10(3) CHO cells by mAb directed to epitope 1, demonstrating that enhanced adherence was due to direct activation of the lectin. mAb to epitope 3 bound to the lectin only upon its solubilization from the membrane and had no effect on adherence. Adherence to CHO cells and mucins was inhibited from 50 to 75% by mAb to epitopes 4 and 5; epitope 6 mAb inhibited amebic adherence to CHO cells but not mucins. The pooled sera from 10 patients with amebic liver abscess blocked the binding to the 170-kDa subunit of mAb directed to all six epitopes. Striking individual variations in the effects of immune sera on adherence were observed. Although the sera of all 44 South African patients with amebic liver abscess had high titer anti-lectin antibodies, 16 patients' sera significantly (more than 3 SEM) enhanced adherence whereas 25 patients' sera significantly inhibited adherence. Antilectin antibodies exert profound functional effects on the interaction of E. histolytica with target cells and human colonic mucins. Exploration of the clinical consequences of adherence-enhancing and inhibitory antibody responses may give insight into the role of antilectin antibodies in immunity to invasive amebiasis.

Abstract:
Abstract Abstract 1903 Poster Board I-926 Introduction: The discovery of the JAK2 V617F gene mutation has significantly altered the clinical diagnostic approach to the myeloproliferative neoplasms, reflected in the revised 2008 WHO diagnostic criteria. Both the 2001 and 2008 diagnostic criteria for essential thrombocythemia (ET) require a bone marrow biopsy showing megakaryocytic proliferation to make a diagnosis of ET. Published expert opinion based on clinical studies suggests that ET patients > 60 years old or with history of thrombosis should be characterized as high risk and treated with hydroxyurea. It is unclear whether physicians in clinical practice utilize the WHO diagnostic criteria or follow expert treatment recommendations for ET. Methods: We conducted a retrospective chart review of all patients with a clinical diagnosis of ET made by a hematologist who were seen in clinic from 2006 to 2008 at two university teaching hospitals in Vancouver, Canada. Data collected included demographic information, thrombosis history, diagnostic tests performed and treatment administered. Testing for JAK2 V617F became locally available in 2006, so for assessment of diagnostic tests performed, patients were divided into cohorts of diagnosis pre-2006 and 2006–2008. Patients were characterized as high risk if > 60 y or history of thrombosis at the time of diagnosis. All other patients were considered low risk. Results: Diagnostic information was available for 116 patients diagnosed prior to the availability of testing for JAK2 V617F. 65% (75/116) of patients in this cohort had a bone marrow biopsy performed (table 1). 44 patients received a new diagnosis of ET from 2006–2008. Only 48% (21/44) patients in this cohort had a bone marrow biopsy performed, significantly less than in the historical cohort (p = 0.019). 41/44 had JAK2 V617F testing performed: 41% (17/41) were JAK2 V617F negative, 56% (23/41) positive and 1 equivocal. Bone marrow biopsy was performed in 59% (10/17) of JAK2 V617F negative patients and 39% (9/23) of JAK2 V617F positive patients (p = 0.055) (table 1). Bone marrow biopsy was also performed in 1 patient with equivocal JAK2 V617F testing and 1 patient not tested for JAK2 V617F. 170 patients diagnosed with ET were seen in follow up 2006–2008. 64% (109/170) were high risk due to age > 60 y or history of thrombosis. The remaining 36% (61/170) were considered low risk. Hydroxyurea was used preferentially over anagrelide for treatment of ET (table 2). Only 76% of high risk patients were receiving cytoreductive treatment. 23% of low risk patients received cytoreductive treatment. ASA was prescribed to 89% of high risk and 79% of low risk patients. Conclusion: Despite the requirement for a bone marrow biopsy to meet the WHO criteria for ET, hematologists performed a bone marrow biopsy in less than half of patients they diagnosed with ET since 2006. Hematologists performed bone marrow biopsy less frequently after JAK2 V617F testing became available, particularly in JAK2 V617F positive patients. A substantial portion of high risk patients (24%) were receiving no cytoreductive therapy, contrary to expert recommendation. Further study is required to understand the barriers to implementing treatment recommendations in clinical practice. This study highlights the challenges in translating published diagnostic criteria and treatment guidelines into changes in patient care. Disclosures: No relevant conflicts of interest to declare.

Abstract:
Aim: This study was conducted to determine the biological effects of acute poisoning, the nature of agents involved and the pattern of poisoning in Diyarbakir City, in the Southeast Anatolian region of Turkey, during 2000. Method: Hospital records of all admissions to the Emergency Department (ED) of Dicle University Hospital following acute poisoning were revised and all data from January to December 2000 were analysed. The present study included 44 (25.9%) male (M) and 126 (74.1%) female (F), a total of 170 patients. The M/F ratio was 1.0/3.5. Results: The mean age of patients was 23.39±6.3 years; 63 (37.1%) of them were under 20 years of age and 147 (86.5%) were under 30 years of age. Most intoxication cases occurred during the summer season (93 of 170 patients). On a monthly basis, admissions during April, May and July were most common (24, 26 and 30 patients, respectively). Sixty-two (36.5%) cases involved accidental poisoning while 108 (63.5%) involved deliberate poisoning. In suicide attempts, intoxications were more common in females (77 cases, 71.3%, P < 0.05), and in unmarried persons (74 cases, 68.5%, P < 0.05). There were only two deaths (1.2%) among the 170 admissions of acute poisonings. One of the deaths was due to pesticide poisoning and the other was due to medical drug abuse. Tachycardia (59, 34.7%), vomiting (55, 32.4%) and loss of consciousness (42, 24.7%) were frequently observed, whereas hypersecretion (15, 8.8%), bradycardia (5, 2.9%), convulsion (8, 4.7%) and hypertension (2, 1.2%) were less frequent. Among pesticide poisoning cases the incidence of convulsion (6, 10.2%), miosis (6, 10.2%), and hypersecretion (12, 20.3%) were significantly higher when compared to other cases (P–0.018, P B < 0.0001 and P B < 0.0001, respectively). Conclusion: In the Southeast Anatolian region of Turkey, pesticide intoxication is common especially among young, unmarried females and most of these intoxications are intentional self-poisonings. The annual rate of poisoning-related ED visits and mortality were found to be within expected ranges; psychoactive agents being the most common cause.

e13500 Background: COVID-19 pandemic has affected healthcare delivery, particularly in the hard-hit areas. During the peak of COVID-19 pandemic crisis in the New York city, our outpatient oncology infusion center, located within a public teaching hospital in the South Bronx remained active. We implemented twice daily team huddle, staff and patient education, and infection screening tools and modified treatment plans based on social, personal and disease related factors. We evaluate the effectiveness of the above strategies in timely delivery of critical oncological care. Methods: Patients treated from the March 1, 2020 to the May 8, 2020 were included. De-identified data from medical charts were analyzed using IBM SPSS Version 27.0. Bivariate logistic regression analysis was applied to identify factors associated with COVID-19. Results: In total, 170 patients were treated in 576 visits. Median age was 60.7 years, 44% Hispanic and 41% Black, median Charlson Comorbidity index (CCI) was 6.6. Fifty percent received cytotoxic chemotherapy, 44% targeted therapies and the remaining received immune-checkpoint inhibitors. Of the 170 patients, six developed severe COVID-19 requiring hospitalization. Their median age was 63 years with average of 10.5 days from infusion center visit to COVID-19 and median CCI score was 9, higher than the rest of the cohort. Two patients died, 3 made complete recovery, 1 enrolled in hospice. Two patients contracted mild COVID-19 managed in the outpatient setting. Diabetes mellitus was associated with severe COVID-19 [OR: 25.9 (95%CI: 1.3-519, p=0.03)]. Age, gender, type of cancer and oncological treatment, smoking, CCI, growth factor support, nursing home residence, statin use were not associated with risk of developing severe COVID-19 Conclusions: Cancer treatment in the outpatient setting using an approach focused on careful patient selection, infection prevention strategies and strong team communication is feasible and allows for continuity of critical oncological care. Receipt of cancer directed therapy was not associated with higher risk for infection compared to risks associated with community-based transmission.. In communities with high community-based transmission, careful selection of patients for oncological based treatment is paramount.

Abstract:
Coronavirus disease (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), emerged in China in December 2019 and now has led to a global pandemic. The incidence is lower in neonates. A five days old newborn infant got admitted with history of reluctance to feeding and less movement. On admission, the newborn infant was found to be lethargic, icteric, tachypnoeic, afebrile with SPO2 of 90% throughout his hospital stay. Chest x-ray was normal but RT-PCR for COVID-19 was positive. His mother had symptoms of COVID-19 with positive RTPCR. The neonate was isolated and was treated with antibiotics along with supportive care, then improved gradually and was discharged on day 10 of his age. Suspicion about COVID-19 in newborn infants with prompt identification is essential to limit its transmission in the community. Bangladesh J Child Health 2020; VOL 44 (3) :170-173

Frog virus 3 (FV3), a member of the genus Ranavirus (family Iridoviridae), was isolated from the leopard frog Rana pipiens. Members of this genus are recognized pathogens of amphibians, reptiles and fish and have been associated with declines of amphibians in the United Kingdom and the USA. Neutron-scattering studies showed that FV3 virions are composed of four concentric layers: an outer lipid envelope containing protein VP58; an icosahedral capsid shell with major capsid protein VP48; an inner lipid membrane with VP63 and 44; and a central core of dsDNA (∼170 kbps) and associated proteins. The FV3 genome has a terminally-redundant and circularly-permuted sequence. Naked virions (i.e. without the envelope lipid) contain more than 20 different proteins and about 9% lipid, which is required for infectivity. to date, structural information about FV3 has primarily been obtained in electron microscopy studies by means of thin-sectioning and negative staining techniques.

AbstractThis paper presents a simple framework that allows us to survey and relate some different strands of the game theory literature. We describe a “canonical” way of adding incomplete information to a complete information game. This framework allows us to give a simple “complete theory” interpretation (Kreps in Game theory and economic modelling. Abstract:
Antifouling compounds are widely used in paints applied on ship hulls to prevent attachment of fouling organisms. However, a certain amount of these chemicals could leach from the painted surface, enter seawater, and pose deleterious effects on various marine biotas. The present study aimed to determine the concentration of organotin (OT) compounds and booster biocides in sediments collected from the seagrass area of Sungai Pulai estuary, Malaysia. The sediment samples were collected from three points on the seagrass bed, brought back to the laboratory, extracted using standard extraction procedure, and the analytes were analysed using gas chromatography-mass spectrometry (GC-MS) method. The results showed that tributyltin (TBT) concentrations in sediments were within the range of 8.1 ± 0.4 to 10.6 ± 0.5 µg/kg, whereas the values of triphenyltin (TPT) were between 17.1 ± 0.9 and 19.4 ± 1.0 µg/kg. The range of concentration of booster biocides, namely diuron, dichlofluanid chlorothalonil, Irgarol 1051, M1, and Sea-Nine 211, were from <0.1 to 22.9 ± 1.1, 48.7 ± 2.4 to 800 ± 40, <0.1 to 6.2 ± 0.3, <0.1 to 1.4 ± 0.1, 44 ± 2.2 to 877 ± 44, and 9.1 ± 0.5 to 170 ± 8.5 µg/kg, respectively. The concentration of organotin was much lower than the previous study conducted in southern Johor. Meanwhile, the increased concentration of booster biocides proves the use of these compounds as antifouling paints in shipping systems nowadays.

Abstract:
Paroxysmal supraventricular tachycardia is accompanied by hypotension, which can affect vascular hemodynamics. Here, we hypothesized that a fall in blood flow as a result of hypotension has a larger effect on hemodynamics in medium-sized peripheral arteries compared with increased pulsatility in rapid pacing. To test this hypothesis, we experimentally and theoretically investigated hemodynamic changes in femoral, carotid, and subclavian arteries at heart rates of 95–170 beats/min after acute pacing. The arterial pressure, blood flow, and other hemodynamic parameters remained statistically unchanged for heart rates ≤135 beats/min. Systemic pressure and flow velocities, however, showed an abrupt decrease, resulting in larger alteration of hemodynamic parameters for heart rates ≥155 beats/min after pacing (initial period) and then recovered close to baseline after several minutes of pacing (recovery period). During the initial period, the pressure dropped from 88 mmHg (baseline) to 44 mmHg, and the flow velocity decreased to about one-third of baseline at heart rate of 170 beats/min. A hemodynamic analysis showed a velocity profile with a near-wall retrograde flow or a fully reversed flow during the initial period, which vanished at the recovery period. It was concluded that the initial fall of blood flow due to pressure drop led to transient flow reversal and negative wall shear stress because this phenomena was not observed at the recovery period. This study underscores the significant effects of hypotension on vascular hemodynamics, which may have relevance to physiology and chronic pathophysiology in paroxysmal supraventricular tachycardia.
