Abstract:
Background. Traumatic arteriovenous (AV) fistula is considered to be a pathologic communication between the arterial and venous systems following injury caused mostly by firearms, sharp objects or blasting agents. Almost 50% of all traumatic AV fistulas are localized in the extremities. In making diagnosis, besides injury anamnesis data, clinical image is dominated by palpable thrill and auscultator continual sounds at the site of fistula, extremities edemas, ischemia distally of fistula, pronounced varicose syndrome, and any signs of the right heart load in high-flow fistulas. Case report. We presented a male 32-year-old patient self-injured the region of the right lower and upper leg by shotgun during hunting in 2005. The same day the patient was operated on in a tertiary traumatology health care institution under the diagnosis of vulnus sclopetarium femoris et cruris dex; AV fistula reg popliteae dex; fractura cruris dex. The performed surgery was ligatura AV fistulae; reconstructio a. popliteae cum T-T anastomosis; fasciotomia cruris dex. Postoperatively, in the patient developed a multiple AV fistula of the femoral and popliteal artery and neighboring veins. The patient was two more times operated on for closing the fistula but with no success. Three years later the patient was referred to the Clinic for Vascular Surgery, Military Medical Academy, Belgrade, Serbia. A physical examination on admission showed the right upper leg edema, pronounced varicosities and high thrill, signs of the skin induration and initial ischemia with ulceration in the right lower leg, as well as numerous scars in the inner side of the leg from the previously performed operations. Due to the right heart load there were also present easy getting tired, tachypnoea and tachycardia. CT and contrast angiography verified the presence of multiple traumatic AV fistulas in the surface femoral and popliteal artery and neighboring veins of the highest diameter being 1 cm. Also, numerous metallic balls - grains of shotgun were present. After the preoperative preparation under local infiltrative anesthesia, transfemoral endovascular reconstruction was done of the surface femoral and popliteal artery by the use of stent grafts Viabahn 6 ? 50 mm and excluder PXL 161 007. Within the immediate postoperative course a significant reduction of the leg edema and disappearance of thrill occurred, and, latter, healing of ulceration, and disappearance of signs of the foot ischemia. Also, patient's both cardiac and breathing functions became normal. Conclusion. In patients with chronic traumatic AV fistulas in the femoropopliteal region, especially with multiple fistulas, the gold standard is their endovascular reconstruction which, although being minimally traumatic and invasive, offers a complete reconstruction besides keeping integrity of both distal and proximal circulation in the leg.

OPERATIONAL PERFORMANCE OF AN ENTIRE TRACTOR-SEEDER CONTINUOUS FLOW G I N FUNCTION OF TIRE PRESSURE OF INFLATION AND DISPLACEMENT SPEED ABSTRACT: Sowing process is a considerable steps that need attention during the crop planting. Factors as tire pressure or sower displacement speed, can directly interfere in energetic and operational performance of the entire tractor-seeder , initial development and crop implementation . The aim o of this study was to evaluate the energetic and operational performance of the entire tractor-seeder in function of tire inflation pressure and tractor displacement speed. The study was conducted at Federal University of Ceará in Fortaleza (CE), in the area of the Accident Investigation Laboratory in Agricultural Machinery (LIMA). The experiment was design in randomized block in a factorial 2 x 3, with four replications and two tire inflation pressures of sower wheelsets (138 kPa and 165 kPa) and three tractor displacement speed (3.9 km h-1, 4, 9 km h-1, 7.0 Km h-1). The inflation pressure of 138 kPa had lower fuel consumption and reduced wheel slip. The theoretical speed of 7.0 km h-1 allowed greater displacement speed and higher to effective and operational field capacity. Keywords: adaptation, energy efficiency, sowing.

Abstract. Size-segregated (9 stages) n-alkanes, polycyclic aromatic hydrocarbons (PAHs) and hopanes in the urban (Baoji city in inland China), mountain (Mt. Tai in east coastal China) and marine (Okinawa Island, Japan) atmospheres over East Asia were studied using a GC/MS technique. Ambient concentrations of n-alkanes (1698±568 ng m−3 in winter and 487±145 ng m−3 in spring), PAHs (536±80 and 161±39 ng m−3), and hopanes (65±24 and 20±2.4 ng m−3) in the urban air are 1–2 orders of magnitude higher than those in the mountain aerosols and 2–3 orders of magnitude higher than those in the marine samples. Mass ratios of n-alkanes, PAHs and hopanes clearly demonstrate coal-burning emissions as their major source. Size distributions of fossil fuel derived n-alkane, PAHs and hopanes were found to be unimodal in most cases, peaking at 0.7–1.1 μm size. In contrast, plant wax derived n-alkanes presented a bimodal distribution with two peaks at the sizes of 0.7–1.1 μm and >4.7 μm in the summer mountain and spring marine samples. Among the three types of samples, geometric mean diameter (GMD) of the organics in fine mode (<2.1 μm) was found to be smallest (av. 0.63 μm in spring) for the urban samples and largest (1.01 μm) for the marine samples, whereas the GMD in coarse mode (≥2.1 μm) was found to be smallest (3.48 μm) for the marine aerosols and largest (4.04 μm) for the urban aerosols. The fine mode GMDs of the urban and mountain samples were larger in winter than in spring and summer. Moreover, GMDs of 3- and 4-ring PAHs were larger than those of 5- and 6-ring PAHs in the three types of atmospheres. Such differences in GMDs can be interpreted by the repartitioning of organic compounds and the coagulation and hygroscopic growth of particles during a long-range transport from the inland continent to the marine area, as well as the difference in their sources among the three regions.

Abstract:
Abstract Objectives Standard chemical methods that determine oxidized lipids in foods are laborious and require toxic chemicals. Our goal was to develop a mathematical lipid oxidation index (LOI) to predict oxidized lipid content of oven fried (OF) and deep fried (DF) potatoes and compare it to standard chemical assays and redox lipidomic analyses. Methods We calculated the LOI value of each recipe using a mathematical formula which consisted of the parameters of 16 nutrients, time and temperature. Next, we OF and DF potatoes in soy, olive, and walnut oils and then extracted oil from the cooked potatoes using the modified Hara and Radin method. We assayed samples of each oil to quantify the conjugated diene (CD), conjugated triene (CT), peroxide (PV), acid (AV), and p-Anisidine (p-Ad) values. In addition, aliquots of each oil sample were sent to collaborators to determine the relative value of oxidized lipids by mass spectrometry and lipidomic informatics. Results Overall, the chemical assays demonstrated that oven frying promoted significantly more oxidation than deep frying. Potatoes OF in walnut oil had greater mean CD (24.1 ± 0.44 vs 19.45 ± 0.06), CT (16.43 ± 0.25 vs 15.67 ± 0.12), AV (0.29 ± 0.01 vs 0.14 ± 0.003), PV (361 ± 7.6 vs 200 ± 6.06), and p-Ad (0.50 ± 0.03 vs 0.14 ± 0.01) when compared to DF potatoes (P ≤ 0.05). This increase in lipid oxidation was not consistent for potatoes prepared with soy and olive oils. LOI predicted a two-fold increase in lipid oxidation in OF potatoes as compared to DF potatoes no matter the type of oil (soy: 308 vs 150; olive: 319 vs; walnut: 330 vs 161). Finally, lipidomic analysis revealed a similar trend in the relative amounts of total oxidized lipids in OF potatoes (soy oil: 24% vs 12%; olive oil: 22% vs 7%; walnut oil 31% vs 17%) and was used to tentatively annotate over 3000 unique oxidized lipids. Conclusions This preliminary study demonstrates that two of the three methods, LOI and lipidomic analysis, are in good agreement in quantifying lipid oxidation in OF and DF potatoes. However, the suboptimal agreement of the chemical assays suggests that the parameters of LOI require further examination. Abstract:
This paper presents a brief review on the ternary phase equilibria in the ternary AV–BVI–I systems (AV = Sb, Bi; BVI = S, Se, Te). These systems includes the series of ternary compounds those are very attractive source materials for photo-, thermos- and ferroelectric energy transformation along the recently discovered semiconductors that exhibit Rashba-type spin splitting in their surface states. In the Rashba semiconductors, a unique toroidal 3D Fermi surface appears on the crystal surface, which leads to unusual properties that make it possible to realize unique electronic devices based on these compounds. The thorough knowledge on the ternary phase diagram of these systems shed light on the chemical and structural design of new multifunctional materials with tunable properties. This knowledge is very important whenfocusing on the chemistry of such multifunctional materials based on complex element systems. REFERENCES Audzijonis A., Sereika R., Ћaltauskas R. Abstract:
The effect of amiodarone (30 mg/kg p.o. each day for 3 weeks) on noradrenaline (NA) overflow into coronary sinus (CS) blood during left stellate stimulation (15 V, 2-ms square waves, 30 s duration at 1, 2, 4, and 8 Hz in random order) was investigated in an open-chest dog preparation. CS blood samples were taken before and during the stimulation period for plasma NA and hematocrit determinations. CS blood flow was monitored (extracorporal circulation with an electromagnetic flow meter) and used for NA output computation. The right atrium was paced throughout the experimental period. However, because AV block occurred at a high pacing rate in some amiodarone-treated dogs, pacing rate was lower in that group than in control dogs (132 ± 13 vs. 161 ± 10 min−1, ns). Mean arterial pressure was also lower in the treated group (95 ± 9 vs. 110 ± 13 mmHg, ns) but increased in every dog upon stimulation (p < 0.05). Basal left ventricular dP/dtmax was comparable in the two groups of dogs and increased in a similar fashion upon stimulation (p < 0.05). The increase in plasma NA concentration upon stimulation was comparable between the control and the amiodarone-treated group (0.38 ± 0.08 vs. 0.40 ± 0.12 ng/mL at 1 Hz and 12.7 ± 3.1 vs. 11.3 ± 2.3 ng/mL at 8 Hz, ns). The increase in NA output was also comparable (7.0 ± 1.6 vs. 10.7 ± 5.4 ng/min at 1 Hz and 356 ± 124 vs. 334 ± 102 ng/min at 8 Hz, ns). Amiodarone did not alter the myocardial NA content. We conclude that amiodarone, administered orally for 3 weeks, does not interfere with neural NA release, or with the positive inotropic response, following sympathetic nerve stimulation in dogs.Key words: amiodarone, noradrenaline release, antiadrenergic effect, sympathetic nerve stimulation.

Abstract:
Background: ST-segment depression in ECG is a common finding during paroxysmal supraventricular tachycardia. The exact mechanism and etiology of this ST-segment depression is not always evident. In this study we have tried to evaluate the significance of ST-segment depression during supraventricular tachycardia. Methods: Hospitalized patients for elective electrophysiological study with previous (EPS) history of supraventricular tachycardia with or without ST-segment depression were evaluated clinically, by coronary angiogram and EPS. Data were analyzed by appropriate statistical methods and comparison made between groups with ST-segment depression (Group A) and without ST-segment depression (Group B). Results: Total number of patients was 66. Equal number of patients (33) was in each group. The mean age of patients was 43.8 years. There was female predominance (M:F ratio 2:3). The mean heart rate during supraventricular tachycardia was 161 beats/min. Age, sex, coronary artery disease risk factors and heart rate during an episode of supraventricular tachycardia did not have any significant influence on ST-segment depression. Significant coronary artery disease was found in two patients in Group A and one patient in Group B. Electrophysiological study revealed that Atrio ventricular reentry tachycardia (AVRT) and AV nodal reentry tachycardia (AVNRT) were present in 28 (42.4%) and 38 (57.6%) cases respectively. Patients of group A exhibit AVRT significantly more than patients of group B.The sensitivity of ST-segment depression in correctly diagnosing coronary artery disease (CAD) was 66.7% while the specificity was 50.8%. The sensitivity of ST-segment depression in correctly differentiating AVRT was 83.3% while the specificity was 66.7%. The positive predictive value (PPV) and Negative predictive value (NPV) of the test were 75% and 76.9% respectively. Conclusion: ST-segment depression during episode of supraventricular tachycardia is a poor indicator of coronary artery disease. Presence of ST-segment depression can differentiate AVRT from AVNRT. However, >2 mm ST-segment depression was proved to be an excellent predictor of AVRT. DOI: http://dx.doi.org/10.3329/cardio.v7i2.22249 Cardiovasc. j. 2015; 7(2): 93-97

Abstract:
AbstractBackground and aimsNeuropathic pain is a significant medical problem needing more effective treatments with fewer side effects. Overactive glutamatergic transmission viaN-methyl-D-aspartate receptors (NMDARs) are known to play a role in central sensitization and neuropathic pain. Although ketamine, a NMDAR channel-blocking antagonist, is often used for neuropathic pain, its side-effect profile and abusive potential has prompted the search for a safer effective oral analgesic. A novel oral prodrug, AV-101 (L-4 chlorokynurenine), which, in the brain, is converted into one of the most potent and selective GlyB site antagonists of the NMDAR, has been demonstrated to be active in animal models of neuropathic pain. The two Phase 1 studies reported herein were designed to assess the safety and pharmacokinetics of AV-101, over a wide dose range, after daily dosing for 14-days. As secondary endpoints, AV-101 was evaluated in the capsaicin-induced pain model.MethodsThe Phase 1A study was a single-site, randomized, double-blind, placebo-controlled, single oral ascending dose (30–1800 mg) study involving 36 normal healthy volunteers. The Phase 1B study was a single-site randomized, double-blind, placebo-controlled, study of multiple ascending doses (360, 1080, and 1440mg/day) of AV-101 involving 50 normal healthy volunteers, to whom AV-101 or placebo were administered orally daily for 14 consecutive days. Subjects underwent PK blood analyses, laboratory assessments, physical examination, 12-lead ECG, ophthalmological examination, and various neurocognitive assessments. The effect of AV-101 was evaluated in the intradermally capsaicin-induced pain model (ClinicalTrials.gov Identifier: NCT01483846).ResultsTwo Phase 1, with an aggregate of 86 subjects, demonstrated that up to 14 days of oral AV-101, up to 1440mg per day, was safe and very well tolerated with AEs quantitively and qualitatively like those observed with placebo. Mean half-life values of AV-101 were consistent across doses, ranging with an average of 1.73 h, with the highestCmax(64.4 μg/mL) and AUC0-t(196 μgh/mL) values for AV-101 occurring in the 1440-mg dose group. In the capsaicin induce-pain model, there was no significant change in the area under the pain time curve (AUPC) for the spontaneous pain assessment between the treatment and the placebo groups on Day 1 or 14 (the primary endpoint). In contrast, there were consistent reductions at 60–180 min on Day 1 after dosing for allodynia, mechanical hyperalgesia, heat hyperalgesia, and spontaneous pain, and on Day 14 after dosing for heat hyperalgesia.ConclusionsAlthough, AV-101 did not reach statistical significance in reducing pain, there were consistent reductions, for allodynia pain and mechanical and heat hyperalgesia. Given the excellent safety profile and PK characteristics demonstrated by this study, future clinical trials of AV-101 in neuropathic pain are justified.ImplicationsThis article presents the safety and PK of AV-101, a novel oral prodrug producing a potent and selective GlyB site antagonist of the NMDA receptor. These data indicate that AV-101 has excellent safety and PK characteristics providing support for advancing AV-101 into Phase 2 studies in neuropathic pain, and even provides data suggesting that AV-101 may have a role in treating depression.

Abstract:
Right ventricular refractory periods obtained using atrial and ventricular pacing with an atrioventricular (AV) interval of 160 ms were compared with those obtained with an AV interval of 0 ms in a group of 53 patients. The mean right ventricular effective refractory periods were longer at an AV interval of 160 ms than at an AV interval of 0 ms at pacing cycle lengths of 600 (mean 2.5 +/- 1.8 ms difference) and 400 ms (mean 3.8 +/- 2.4 ms difference; P less than 0.01). In a subset of 10 patients, left ventricular volumes measured by two-dimensional echocardiography demonstrated that pacing with an AV interval of 160 ms was associated with a higher end-diastolic volume and stroke volume than pacing with an AV interval of 0 ms (P less than 0.001). Autonomic blockade did not alter the relationship between AV interval and ventricular refractoriness. We conclude that 1) right ventricular refractory periods are shorter at an AV interval of 0 ms than at an AV interval of 160 ms; 2) these differences are not caused by changes in autonomic tone but are associated with differences in ventricular volumes; and 3) if precise determinations of ventricular refractoriness are desired, then the timing of atrial systole should be controlled by atrial pacing.

Esimerkiksi Yle on tehnyt Kirjojen Suomi -nimisen ohjelmakokonaisuuden, jossa on ilmestynyt muun muassa radio-ohjelmasarja 101 kirjaa. Se esittelee yhden suomalaisen kirjan jokaiselta itsenäisyyden vuodelta.

Abstract:
Background. Aerobic vaginitis (AV) is an aberration within the balanced vaginal microbiota. Only few reports have documented the adverse pregnancy outcomes related to AV. Nonetheless, the exact role of AV in pregnancy and the potential benefit of its screening need further study. Our goal was to evaluate the association between aerobic vaginitis (AV) in late pregnancy and maternal and neonatal outcomes. Methods. In this prospective observational study, a total of 600 singleton pregnant women with intact fetal membranes at a gestational age of 34-36 weeks were recruited (one hundred women with AV and 500 pregnant women without AV). The study protocol excluded patients with other forms of vaginal infection. Pregnancy outcomes were traced and documented. The primary outcome was the association between AV and preterm labor. The current study compared the maternal and neonatal outcomes among pregnant women with and without AV in unadjusted and adjusted analyses with the odds ratio (OR) and 95% confidence interval (CI) reported. Results. There was an association between AV and with preterm birth (adjusted OR 3.06, 95% CI 1.58-5.95) and prelabor rupture of membranes (adjusted OR 6.17, 95% CI 3.24-11.7). For neonatal outcomes, AV was associated with a higher incidence of neonatal ICU admission (adjusted OR 2.19, 95% CI 1.1-4.34). Severe forms of AV significantly increased the incidence of PTB (p=0.0014) and PROM (p=0.0094) when compared to less severe forms of AV. Conclusion. AV is common in late pregnancy and is linked to a diversity of adversative pregnancy outcomes including preterm birth, PROM, and neonatal ICU admission. Moreover, the incidence of PTB and PROM might further increase with the severity of AV. Clinicians should pay more consideration to vaginal microbiota assessment during pregnancy.

Abstract:
In the isolated rabbit heart the ventricular response during irregular atrial pacing and atrial fibrillation was studied. Irregular pacing was performed by an algorithm that generated a population of random stimuli within a chosen range of intervals. During incremental regular pacing atrioventricular (AV) Wenckebach conduction occurred at an atrial cycle length of 164 +/- 24 ms (n = 11). During irregular atrial pacing with a variation of 20, 60, and 100 ms second-degree AV block developed at average cycle lengths of 171, 182, and 198 ms, respectively. In contrast, the pacing interval resulting in stable 2:1 AV block was shortened from 131 ms to 120 and 112 ms during irregular pacing with 20- and 60-ms variation. During pacing with 100-ms variation stable 2:1 AV block no longer occurred. Addition of acetylcholine (0.25-0.75 X 10(-6) M) increased the maximal degree of stable AV block during regular pacing. During irregular pacing, however, stable AV block disappeared. Instead, during second-degree AV block a monophasic inverse relationship (r = -1.5 +/- 1.7) between atrial and ventricular rates appeared as a result of an increased occurrence of concealed conduction in the AV junction. Also during electrically induced atrial fibrillation an inverse relationship was found between the atrial and ventricular rate. We conclude that the ventricular rate during atrial fibrillation is not only determined by the properties of the AV node but also by the rate and irregularity of the fibrillatory process.

Abstract:
Introduction: Autovalidation (AV) is an algorithm based on predefined rules designed, among others, to automate and standardize the postanalytical phase of laboratory work. The aim of this study was to examine the overall opinion of Croatian medical biochemistry laboratories regarding various aspects of AV. Material and methods: This retrospective study is an analysis of the responses of a survey about AV comprised of 18 questions, as part of Module 10 (“Postanalytical phase of laboratory testing”) of national External Quality Assessment program, administered by the Croatian Centre for Quality Assessment in Laboratory Medicine. Results were reported as percentages of total number of participants in survey or as proportions of observed data if the overall number of data was &lt;100. Results: 121 laboratories responded to the survey, of which 76% do not use AV, while 11% of laboratories use AV in routine laboratory work. 16/29 laboratories implemented semi-automated AV for general biochemistry (7/29), haematology (5/29), and coagulation (4/29) tests. Analytical measurement ranges, critical values, flags from analysers, interference indices and delta check were the most commonly used rules in the algorithm. 12/29 laboratories performed validation of AV with less than 500 samples (8/29). 7/13 laboratories report the percentage of AV being 20-50%, while 10/13 answered that introduction of AV significantly reduced turnaround time (TAT) (for 20 - 25%), especially for biochemistry tests. Conclusions: Despite of its numerous benefits (i.e. shorter TAT, less manual validation, standardization of the postanalytical phase), only a small number of Croatian laboratories use AV.

Abstract:
ObjectiveThe outcome of female patients after adult cardiac surgery has been reported to be less favourable compared with the outcome of male patients. This study compares men with women with respect to patient and procedural characteristics and early mortality in a contemporary national cohort of patients who underwent aortic valve (AV) and combined aortic valve/coronary (CABG/AV) surgery.MethodsAll patients who underwent AV (n=8717, 56% male) or a combined CABG/AV surgery (n=5867, 67% male) in the Netherlands between January 2007 and December 2011 were included.ResultsIn both groups, women were generally older than men (p<0.001) and presented with higher logistic EuroSCORES. In isolated AV surgery, men and women had comparable in-hospital mortality (OR 1.20, 95% CI 0.90 to 1.61; p=0.220). In concomitant CABG/AV surgery, in-hospital mortality was higher in women compared with men (OR 2.00, 95% CI 1.44 to 2.79; p<0.001). The area under the curve for logistic EuroSCORE 1 was systematically higher for men versus women in isolated AV surgery 0.82 (95% CI 0.78 to 0.86) vs 0.75 (95% CI 0.69 to 0.80) and in concomitant CABG/AV surgery 0.78 (95% CI 0.73 to 0.82) vs 0.69 (95% CI 0.63 to 0.74). Finally, (the weight of) risk factors associated with in-hospital mortality differed between men and women.ConclusionsThere are substantial male-female differences in patient presentation and procedural aspects in isolated AV and concomitant CABG/AV surgery in the Netherlands. Further studies are necessary to explore the mechanisms underlying the observed differences. In addition, the observation that standard risk scores perform worse in women warrants exploration of male-female specific risk models for patients undergoing cardiac surgery.Brief title:

Abstract:

Abstract:

Life cycle stages, including daughter sporocysts, cercariae, and metacercariae, of Parvatrema duboisi (Dollfus, 1923) Bartoli, 1974 (Digenea: Gymnophallidae) have been found in the Manila clam Ruditapes philippinarum from Aphaedo (Island), Shinan-gun, Jeollanam-do, Korea. The daughter sporocysts were elongated sac-like and 307-570 (av. 395) μm long and 101-213 (av. 157) μm wide. Most of the daughter sporocysts contained 15-20 furcocercous cercariae each. The cercariae measured 112-146 (av. 134) μm in total length and 35-46 (av. 40) μm in width, with 69-92 (av. 85) μm long body and 39-54 (av. 49) μm long tail. The metacercariae were 210-250 (av. 231) μm in length and 170-195 (av. 185) μm in width, and characterized by having a large oral sucker, genital pore some distance anterior to the ventral sucker, no ventral pit, and 1 compact or slightly lobed vitellarium, strongly suggesting P. duboisi. The metacercariae were experimentally infected to ICR mice, and adults were recovered at day 7 post-infection. The adult flukes were morphologically similar to the metacercariae except in the presence of up to 20 eggs in the uterus. The daughter sporocysts and metacercariae were molecularly (ITS1-5.8S rDNA-ITS2) analyzed to confirm the species, and the results showed 99.8-99.9% identity with P. duboisi reported from Kyushu, Japan and Gochang, Korea. These results confirmed the presence of various life cycle stages of P. duboisi in the Manila clam, R. philippinarum, playing the role of the first as well as the second intermediate host, on Aphae-do (Island), Shinan-gun, Korea.

Abstract:
The mechanism behind the female preponderance for atrio-ventricular node reentrant tachycardia (AVNRT) is not clear.<br />We compared baseline electrophysiological measurements and clinical data in 141 consecutive patients (96 women) who<br />underwent successful AVNRT ablation at their fi rst therapeutic procedure. Women had on average 9% higher resting<br />heart rate than men (p&lt;0.05), but were similar in all measures of AV node function. Isoproterenol infusion was required<br />for AVNRT induction in 69 cases (49%), and the need for isoproterenol was associated with lower resting heart rate and<br />longer anterograde and retrograde AV node refractory periods (p&lt;0.05 for comparisons), but not with sex. We conclude<br />that the spectrum of baseline AV node physiology in AVNRT patients is wide, and is similar in men and women. The female<br />preponderance for AVNRT cannot be explained from comparisons of baseline AV node electrophysiological properties.

Abstract:
Although Aloe vera contains numerous bioactive components, the activity principles of widely used A. vera extracts are uncertain. Therefore, we analyzed the effects of genuine A. vera aqueous extract (AV) on human cells with respect to the effects of hydrogen peroxide (H2O2) and 4-hydroxynonenal (HNE). Fully developed A. vera leaves were harvested and analyzed for vitamin C, carotenoids, total soluble phenolic content, and antioxidant capacity. Furthermore, human cervical cancer (HeLa), human microvascular endothelial cells (HMEC), human keratinocytes (HaCat), and human osteosarcoma (HOS) cell cultures were treated with AV extract for one hour after treatment with H2O2 or HNE. The cell number and viability were determined using Trypan Blue, and endogenous reactive oxygen species (ROS) production was determined by fluorescence, while intracellular HNE–protein adducts were measured for the first time ever by genuine cell-based HNE–His ELISA. The AV extract expressed strong antioxidant capacities (1.1 mmol of Trolox eq/g fresh weight) and cell-type-specific influence on the cytotoxicity of H2O2, as well as on endogenous production of ROS and HNE–protein adducts induced by HNE treatment, while AV itself did not induce production of ROS or HNE–protein adducts at all. This study, for the first time, revealed the importance of HNE for the activity principles of AV. Since HMEC cells were the most sensitive to AV, the effects of AV on microvascular endothelia could be of particular importance for the activity principles of Aloe vera extracts.

Abstract:

Abstract:
OBJECTIVEThe aim of this study was to determine if the apical vertebra (AV) in patients with adolescent idiopathic scoliosis (AIS) is the most rotated vertebra in the scoliotic segment.METHODSA total of 158 patients with AIS (Cobb angle range 20°–101°) underwent biplanar radiography with 3D reconstructions of the spine and calculation of vertebral axial rotations. The type of major curvature was recorded (thoracic, thoracolumbar, or lumbar), and both major and minor curvatures were included. The difference of levels (DL) between the level of maximal vertebral rotation (LMVR) and the AV was calculated as follows: DL = 0 if LMVR and AV were the same, DL = 1 if LMVR was directly above or below the AV, and DL = 2 if LMVR was separated by 1 vertebra or more from the AV. To investigate which factors explained the divergence of the LMVR from the AV, multinomial models were computed.RESULTSThe distribution of the DL was as follows: for major curvatures, 143 were DL = 0, 11 were DL = 1, and 4 were DL = 2; and for minor curvatures, 53 were DL = 0, 9 were DL = 1, and 31 were DL = 2. The determinants of a DL = 2 (compared with DL = 0) were lumbar curvature (compared with thoracic; adjusted OR 0.094, p = 0.001), major curvature (compared with minor; adjusted OR 0.116, p = 0.001), and curvatures with increasing apical vertebral rotation (adjusted OR 0.788, p < 0.001).CONCLUSIONSThis study showed that the AV is the most rotated vertebra in the majority of major curvatures, while in minor curvatures, the most rotated vertebra appears to be the junctional vertebra between major and minor curvatures in a significant proportion of cases.

Abstract:
Mechanisms of cardiac pacemaking and conduction system (CPCS) development are difficult to study, in part because of the absence of models that are physiologically similar to humans in which we can label the entire CPCS. Investigations of the adult rabbit heart have provided insight into normal and abnormal cardiac conduction. The adult and the embryonic rabbit have an endogenous marker of the entire cardiac conduction system, neurofilament 160 (NF-160). Previous work suggested that ventricular septation correlates with critical phases in avian CPCS development, in contrast to the mouse CPCS. Combining high-resolution optical mapping with immunohistochemical analysis of the embryonic rabbit heart, we investigated the significance of ventricular septation in patterning the rabbit embryonic conduction system. We hypothesized that 1) completion of ventricular septation does not correlate with changes in the ventricular activation sequence in rabbit embryos and 2) CPCS anatomy determines the activation sequence of the embryonic heart. We found that preseptated ( days 11–13, n = 13) and postseptated (day 15, n = 5) hearts had similar “apex-to-base” ventricular excitation. PR intervals were not significantly different in either group. CPCS anatomy revealed continuity of the NF-160-positive tract connecting the presumptive sinoatrial node, atrioventricular (AV) junction, and ventricular conduction system. The presence of collagen in the AV junction coincided with the appearance of an AV interval. We conclude that the apex-to-base ventricular activation sequence in the rabbit embryo is present before completion of ventricular septation. CPCS anatomy reflects global cardiac activation as demonstrated by high-resolution optical mapping.

Abstract:
Background: The goal of this study was to compare the early and late outcomes of different techniques of proximal root reconstruction during the repair of acute Type A aortic dissection, including aortic valve (AV) resuspension, aortic valve replacement (AVR), and a root replacement procedure. Methods: All patients who underwent acute Type A aortic dissection repair between January 2000 and October 2010 at four academic institutions were compiled from each institution’s Society of Thoracic Surgeons Database. This included 189 patients who underwent a concomitant aortic valve (AV) procedure; 111, 21, and 57 patients underwent AV resuspension, AVR, and the Bentall procedure, respectively. The median age of patients undergoing a root replacement procedure was significantly younger than the other two groups. Early clinical outcomes and 10-year actuarial survival rates were compared. Trends in outcomes and surgical techniques throughout the duration of the study were also analyzed. Results: The operative mortality rates were 17%, 29%, and 18%, for AV resuspension, AVR, and root replacement, respectively. Operative mortality (p = 0.459) was comparable between groups. Hemorrhage related re-exploration did not differ significantly between groups (p = 0.182); however, root replacement procedures tended to have decreased rates of bleeding when compared to AVR (p = 0.067). The 10-year actuarial survival rates for the AV resuspension, Bentall, and AVR groups were 72%, 56%, and 36%, respectively (log-rank p = 0.035). Conclusions: The 10-year actuarial survival was significantly lower in those receiving AVR compared to those receiving root replacement procedures or AV resuspension. Operative mortality was comparable between the three groups.

Abstract:
Background and aims: Frequent failure of adrenal vein (AV) cannulation is a major obstacle to the universal use of adrenal vein sampling (AVS) for subtyping primary aldosteronism (PA). This study aimed to confirm and modify the value of a previously reported AVS parameter for PA subtyping in the case of cannulation failure on one side. Methods: Successfully catheterized AVS studies in 157 patients (121 patients as a derivation cohort and 36 patients as a validation cohort) from two tertiary hospitals were retrospectively reviewed. The AV/inferior vena cava (IVC) index was defined by dividing the aldosterone/cortisol ratio (ACR) of AV by the ACR of the IVC. Cutoff values for lateralized PA were obtained from two methods: scatterplots and the values corresponding to Youden’s index in receiver operating characteristic (ROC) curves, on the assumption of catheterization failure on one side. Results: Due to multiple samplings in a single AVS procedure, 252 left AV/IVC ratios (LIRs) and 272 right AV/IVC ratios (RIRs) were calculated. Scatterplot cutoffs of LIR >5.4 or <0.5 predicted unilateral PA with a sensitivity of 42.1% and a specificity of 98.6%. Scatterplot cutoffs of RIR <0.5 or >7.0 showed a sensitivity of 55.1% and a specificity of 98.6%. ROC curve cutoffs of LIR ⩽0.8 or >3.1 predicted unilateral PA with a sensitivity of 82.5% and a specificity of 69.6%. ROC curve cutoffs of RIR ⩽0.8 or >3.9 resulted in 87.4% sensitivity and 80.7% specificity. Conclusion: In the case of unilateral AVS failure, the AV/IVC index may help in diagnosing PA subtype.

Abstract:
Radiofrequency catheter ablation is a newly introduced technique that does not require open heart surgery and is designed for curing patients with arrhythmia. We present our experience with this technique in 223 patients, with recurrent supraventricular tachycardia due to accessory pathways associated with the Wolff-Parkinson-White syndrome or AV nodal reentrant tachycardia. Of the patients, 119 underwent radiofrequency ablation of accessory pathways, while 101 underwent AV nodal modification. Two patients underwent both AV nodal modification and accessory pathway ablation during the same session. One patient had AV nodal ablation. Mean age was 39.4 ± 14.1 years (13–73 years). There were 108 males and 115 females. Except for 1 patient, all had significant symptoms. Radiofrequency ablation performed during the first session was successful in 215 patients (96.4%). With repeat ablation, 218 (97.8%) of the patients were successfully ablated. When compared with surgery, the efficacy is similar; however, radiofrequency ablation is less costly and results in less morbidity. Radiofrequency catheter ablation is highly efficacious and is the treatment of choice in patients who are at risk for sudden death or have failed drug therapy. It should also be offered as an alternative to lifelong drug therapy.

Abstract:
Background:Anti-neutrophil cytoplasmic antibody (ANCA) associated vasculitides (AV) can increase risk of atherosclerosis. 1 Patients with autoimmune disease have increased rates of coronary events that are not fully explained by classic risk factors. 2 A prior study has shown that autoimmune vasculitides –including polyarteritis nodosa, hypersensitivity granulomatosis with polyangiitis, giant cell arteritis, thrombotic angiitis, Takayasu arteritis, Goodpasture syndrome, and Kawasaki disease –are associated with increased cost of care and increased 30-day readmission after percutaneous coronary intervention (PCI). 3 It is important to distinguish the cardiovascular outcomes of small–vessel ANCA-associated vasculitides from the other vasculitides included in this prior study because coronary arteritis is typically a complication of medium to large vessel vasculitis. Objectives:To investigate the impact of comorbid ANCA-associated small vessel vasculitis (AV) on outcomes of PCI in a nationally representative cohort. Methods:Patients undergoing PCI in the 2016-2018 National Inpatient Sample were selected and those with ANCA-associated vasculitis were identified using the ICD-10 classification system. The NIS is an all-payer, survey-weighted, inpatient database estimating 37 million annual U.S. hospitalizations managed by the Healthcare Cost and Utilization Project (HCUP). Primary study outcomes were prevalence of comorbid vasculitis, mortality, complications, and cost of care. Results:Of 1,351,670 patients undergoing PCI were identified, 410 (0.03%) had diagnosis of ANCA-vasculitis. AV patients were similar to non-AV patients in distribution of gender, race, and median household income. AV patients were older (70.9 vs 65.2 years, P<0.001) and more likely to utilize Medicare (78% vs 53%, P<0.005). AV patients were more likely to present with non-ST segment myocardial infarction (NSTEMI) (57.3% vs 41.5%, P<0.005) and less likely to present with ST-elevation myocardial infarction (NSTEMI) (18.3% vs 31.3%, P<0.05). AV patients had similar adjusted mortality (6.1% vs 2.6%, aOR: 2.1, P=0.8), cost of care ($32,985 vs $25,767, β = 0.02, P=0.7), and length of hospitalization (6.0 vs 3.9, IRR: 1.2, P=0.13).Conclusion:The presence of ANCA-associated vasculitis did not appear to negatively impact inpatient clinical outcomes or utilization of care among patients undergoing PCI in the United States. Cost of care was not increased, in contrast to an increase cost of care when small, medium, and large vasculitides are grouped together. The greater proportion of NSTEMI patients among those with AAV corresponds with a growing body of literature implicating inflammatory processes in the pathogenesis of certain forms of myocardial infarction meriting further investigation.References:[1]Misra DP, Shenoy SN. Abstract:
Despite numerous studies on general human–robot interactions, in the context of transportation, automated vehicle (AV)–human driver interaction is not a well-studied subject. These vehicles have fundamentally different decision-making logic compared with human drivers and the driving interactions between AVs and humans can potentially change traffic flow dynamics. Accordingly, through an experimental study, this paper investigates whether there is a difference between human–human and human–AV interactions on the road. This study focuses on car-following behavior and conducted several car-following experiments utilizing Texas A&M University’s automated Chevy Bolt. Utilizing NGSIM US-101 dataset, two scenarios for a platoon of three vehicles were considered. For both scenarios, the leader of the platoon follows a series of speed profiles extracted from the NGSIM dataset. The second vehicle in the platoon can be either another human-driven vehicle (scenario A) or an AV (scenario B). Data is collected from the third vehicle in the platoon to characterize the changes in driving behavior when following an AV. A data-driven and a model-based approach were used to identify possible changes in driving behavior from scenario A to scenario B. The findings suggested there is a statistically significant difference between human drivers’ behavior in these two scenarios and human drivers felt more comfortable following the AV. Simulation results also revealed the importance of capturing these changes in human behavior in microscopic simulation models of mixed driving environments.

Abstract:
An autologous arteriovenous (AV) fistula is the preferred form of angioaccess for chronic hemodialysis. A prospective study was carried out to evaluate the potential of regional anesthesia to improve AV fistula prevalence. One hundred ninety-three patients underwent preoperative duplex ultrasound evaluation over a 14-month period. The qualification of each patient to receive either an autologous AV fistula or a prosthetic graft was based on specific sonographic criteria. Patients scheduled for placement of a graft received an ultrasound-directed supraclavicular brachial plexus block, which produces dense sympathetic blockade. After the regional block, those patients who met criteria for primary fistula construction on repeat ultrasound received a fistula instead of a graft. Of 62 patients scheduled to receive an AV graft, 23 or 37 per cent were recruited to receive a fistula instead. The outcome of the recruited fistulas was compared with the 121 planned fistulas. There was no statistically significant difference in primary failure rate (4.3 vs 5.8%). The recruited fistulas had a shorter average maturation time, 83 ± 48 versus 132 ± 82 days ( P = 0.023). Within the study population, functioning fistula prevalence was increased from 61.7 to 79.8 per cent. Regional anesthesia and immediate preoperative ultrasound is a useful strategy for increasing fistula prevalence.

Abstract:

Abstract:
The direct effects of alinidine (N-allyl-clonidine) on the sinus node and atrioventricular (AV) junction were studied in 18 anesthetized dogs. Stimulus frequency-response curves to right stellate ganglion and right cervical vagus stimulations as well as responses to norepinephrine or acetylcholine were determined before and after selective perfusion of alinidine into the sinus node artery. Alinidine (1 microgram/ml) had no effect on spontaneous sinus rate [148 +/- 5 (SE) beats/min]. However, alinidine concentrations of 5, 10, and 25 micrograms/ml produced significant (P less than 0.05) sinus slowing to 138, 127, and 121 beats/min, respectively. Recovery to control rate was dose dependent and took from 4 to 33 min. Sinus rate increases with right stellate stimulations were not affected by alinidine. However, sinus rate decreases with right vagal stimulations were significantly (P less than 0.01) attenuated by alinidine. The negative chronotropic effects of acetylcholine were not influenced by alinidine. Alinidine (1-100 micrograms/ml into AV node artery) had no effect on the A-H interval of the His bundle electrogram. However, alinidine (10 and 25 micrograms/ml) diminished the AV block produced by stimulation of the left vagus in electrically paced hearts but not the negative dromotropic actions of directly administered acetylcholine. Thus alinidine has direct negative chronotropic effects, no effect on sinus node responses to sympathetic stimulation, ability to diminish sinus node and AV junctional responses to vagal stimulations without interference at the cholinergic muscarinic receptor, and 4) no effect on AV nodal conduction.

Sorbtion of dipeptides leucylisoleucine, threonylthreonine and their monomeric amino acids leucine and threonine by anionite AV-17 and cationite KU-2-8 in a wide range of equilibrium concentrations has been studied. It was shown that the presence of hydrophilic OH-groups in the threonine molecule promotes superequivalent sorbtion of threonine on the cation exchanger. The presence of an OH-groups in the side chein of the dipeptide practically does not affect the sorbtion an KU-2-8. Sorbtion of dipeptides on AV-17-8 is higher in comparison with their monomeric amino acids.

Abstract:
During the early stages of heart development, there are two main foci of cell death: outflow tract (OT) and atrioventricular (AV) endocardial cushions. These tissues contribute to the septa and valves of the mature heart and receive cell populations from neural crest (NC) cell migration and epicardial cell invasion. We examined embryonic chick hearts for expression, in the cushions, of bcl-2 family members, caspase-9, and the caspase substrate poly(ADP-ribose) polymerase. Antiapoptotic bcl-2 is expressed heavily in the OT and AV regions throughout embryonic days (ED) 4–7, with a decrease in levels at ED 4 and 5 in OT and AV cushions, respectively. Proapoptotic bax predominantly associated with the prongs of the NC-derived aorticopulmonary (AP) septum but was expressed throughout the AV cushions. Proapoptotic bak also associated with the prongs of the AP septum in the OT, while protein levels were upregulated at ED 4–5 and 4–6 in OT and AV cushions, respectively. Bid expression showed a similar time course. We found the 10-kDa cleavage fragment of active caspase-9 at ED 4–8 and 5–8 in OT and AV cushions, respectively, and the 24-kDa cleavage fragment of poly(ADP-ribose) polymerase throughout ED 3–8 and 7–8 in OT and AV cushions, respectively. Caspase-3 cleavage occurred throughout the time period examined. Using cushion cell cultures, we found that inhibitors of caspases-3 and -9 and a universal caspase inhibitor significantly reduced apoptosis, as did retroviral overexpression of bcl-2 using an RCAS expression vector. Premigratory NC cells were fluorescently labeled in vivo with 1,1-didodecyl-3,3,3′,3′-tetramethylindocarbocyanine. Subsequent nuclear staining of cushion cells with 4,6-diamidino-2-phenylindole revealed the presence of apoptotic nuclei in the NC cells in the OT cushions and in the prongs of the AP septum. These results demonstrate a developmentally regulated role for the bcl-2 and the caspase families of molecules in the endocardial cushions of the developing heart and lend support to the possibility that some of the dying cells in the cushions are derived from the NC.
