Abstract:
В роботі був проведений моніторинг захворюваності собак на дерматити поліетіологічного походження. Дерматит – загальне визначення для будь-якого типу запалення шкіри. Цей термін, зазвичай, використовують для опису стану шкіри до досягнення конкретного діагнозу. Існує безліч причин запалення шкіри, включаючи зовнішні подразники, опіки, алергени, травми та інфекції (бактеріальні, вірусні, паразитарні або грибкові). Дерматит також може бути пов’язаний із захворюваннями внутрішніх органів Дослідження проводилися на базі клініки ветеринарної медицини «Альфа-вет» (м. Конотоп, вул. Красногірська 4а) в період 2020-2021 рр. Пацієнтами для проведення досліджень були хворі на дерматити собаки порід мопс, ягд-тер’єр, лабрадор, стафордширський тер’єр, німецька вівчарка, шарпей, такса, французький бульдог, різних вікових груп та статі. Всього клінічно за цей період було обстежено 730 собак. При постановки діагнозу враховували дані анамнезу, клінічні ознаки, результати мікроскопічних досліджень, клінічні та біохімічні показники крові, цитологічні дослідження. Саркоптичний дерматит був зареєстрований у восьми випадках, в яких, крім саркоптичних кліщів, також були помічені нейтрофіли, макрофаги та плазматичні клітини та ороговілі епітеліальні клітини. Гематологічними дослідженнями встановлена відносну нейтрофілія та легка еозинофілію. Важкого та генералізованого демодекозу, ускладненого бактеріями та Malassezia sp. також було зафіксовано у 28 випадках зараження. Гістопатологічно численні Demodex sp. було виявлено кліщів у різній стадії дозрівання, які пошкоджують волосяні фолікули разом із супутніми патологічними змінами та гранулемами стороннього тіла. Крім того, алергічний дерматит від бліх також спостерігався у 89 собак. Цитологія виявилася однозначно ефективною при діагностиці паразитарного дерматиту. Собак з атопічним дерматитом було зафіксовано 28 випадків. Однак виникнення нових випадків постійно зростає. Патогенетичні механізми цього захворювання до кінця не вивчені, проте доведено участь аномальних генів та змінених імунологічних процесів. У собак діагноз атопічного дерматиту ґрунтується на зборі анамнезу, клінічному обстеженні та диференційній діагностиці. Основну частину захворювань займають алергічні хвороби шкіри та бактеріальні захворювання шкіри собак. При проведенні моніторингу дерматитів у собак було встановлено, що алергічні дерматити складали 63,01 % випадків; паразитарні – 24,66 %, грибкові – 9,32 % та бактеріальні – 3,01% Найбільш складні у діагностуванні та лікуванні атопічний, контактний та алергічний дерматити.

Abstract:
Метою роботи було вивчення діагностичного значення загальноклінічних та параклінічних показників у дітей із проявами дисбактеріозу кишечника на тлі атопічного дерматиту для підвищення ефективності лікування даного захворювання. Проведене комплексне спостереження за 26 дітьми першого року життя, які мали клінічні прояви атопічного дерматиту. За даними бактеріологічного посіву випорожнень дітей розподіляли на дві клінічні групи порівняння. Першу групу становили 19 дітей зі значними порушеннями складу мікрофлори кишечника, що проявлялося дисбактеріозом ІІ–ІІІ ступеня тяжкості. Решта пацієнтів із відсутніми ознаками дисбактеріозу кишечника (за даними бактеріологічного дослідження випорожнень) увійшла до складу другої клінічної групи. Комплексне обстеження передбачало клінічну оцінку змін на шкірі за шкалою Eczema Area and Severity Index, відповідно до якої враховують площу ураження шкіри та інтенсивність морфологічних елементів висипання, а також загальний аналіз крові та визначення рівня загального імуноглобуліну E. У результаті проведених досліджень встановлено, що патологічне порушення мікробіоценозу кишечника у дітей, хворих на атопічний дерматит, підвищує ризик тяжкого перебігу цього захворювання у 2,8 раза, а ризик виявлення високої концентрації загального імуноглобуліну E у сироватці крові > 130,0 МО/мл — практично у 5 разів (відношення шансів — 4,8 (95% довірчий інтервал — 2,4–9,5)).

The article is devoted to the topical problem of clinical allergology — differential diagnosis of atopic dermatitis (AD). The clinical manifestations of AD are quite similar to the extremely large number of different diseases and pathological conditions in children, which often leads to misdiagnosis. The complexity of the differential diagnosis of AD in children with a detailed description of different pediatrics diseases: from dermatological diseases (seborrheic dermatitis, eczema, shingles, scabies, ichthyosis, psoriasis, bullous epidermolysis, allergic dermatitis, cutaneous mastocytosis) to oncological (T-cell skin lymphoma), orphan metabolic diseases (hereditary disorders of tryptophan metabolism, phenylketonuria, Nezerton’s syndrome, etc.) and primary immunodeficiencies (Bruton’s disease, Job’s syndrome, Wiskott-Aldrich syndrome, selective IgA immunodeficiency etc.) is described. Сommon features of above mentioned diseases are listed in the article, also, special attention is paid to variety of the pathognomonic symptoms. Abstract:
The objective: the study of genetic affinity of S. aureus strains isolated from patients with AD Materials and methods. Genetic variability of the 79 S. aureus isolates was evaluated according to the results of the RAPD-PCR study. Results. It was revealed a generally high genetic polymorphism of S. aureus. It was established that with an increase in the severity of АD, the monotypism of pathogen isolates grows (p ≤ 0,05) from three different genetic clusters in patients with AD with a mild disease course to only one cluster with a severe disease course. A high level of genetic affinity was established between S. aureus strains isolated from different biotopes from the same patients, which for strains isolated from the nasal mucosa and from locus morbi reached (79,5 ± 1,6)%, and for strains isolated from intact and affected skin are as increased from (75,1 ± 4,4)% to (98,8 ± 0,8)% in parallel with the increase in the severity of the disease course in patients with AD from mild to severe forms, respectively (p ≤ 0,05). Conclusions. Both the generally high genetic polymorphism of S. aureus strains isolated from allergodermatosеs patients and the high affinity of isolates from different biotopes of the same patient, justify the auto-origin of the strains of staphylococci colonizing locus morbi.
