In the title of a 1903 American Journal of Sociology essay, Ernest W. Clement announces a new phenomenon: “The New Woman in Japan.” By this title, he quickly explains, he does not mean to satirically compare this Japanese sociological development to the American “parody of man” usually associated with the phrase, because “such a creature as that called the ‘new woman’ in the Occident has not yet appeared to any great extent among the Japanese.” Although sometimes in Japan “the process of the new woman's evolution may be disfigured by some accident” producing “a sickening sort of person,” Clement's interest is not in particular aberrations, but rather in “the abstract, legal new woman” created by recent changes in Japan's civil code. In this abstraction Clement sees improvement on previous Japanese laws that “relegat[ed] woman to an abnormally inferior position.” Clement thus assures readers that, although Japan's modernization hinges upon its women's legal and cultural status, female advancement in Japan will not approach the “abnormal” excesses of the United States. Quoting Alice Mabel Bacon's influential book Japanese Girls and Women to stress this point, Clement explains that Japanese men are adopting many Western habits and opinions, but they still “shrink aghast, in many cases, at the thought that their women may ever become the forward, self-assertive, half-masculine women of the West.” Yet still, many of these Japanese men express “a growing dissatisfaction with the smallness and narrowness of the lives of their wives and daughters — a growing belief that better educated women make better homes.”

AbstractContrary to conventional wisdom, some of the most contentious disputes over international labor standards and worker rights occurred not between Western nations and the “rest” but within single nations. To explore the deep fissures in Japanese society over the rights of women and workers, I offer the first scholarly account of Japan's only woman representative to the ILO's inaugural 1919 Washington conference, elite social feminist Tanaka Taka, grandniece of renowned Japanese capitalist Shibusawa Eiichi. I recount her efforts in Japan and in Washington to secure free speech and economic rights for Japan's workers, men and women, and detail the hostilities she encountered from employers and organized labor. In addition, I reconstruct the parallel tale of factory supervisor Masumoto Uhei whose appointment as Japan's labor delegate led to widespread labor protests and a power struggle between trade unions and the state in Japan. The debate over who would speak for Japan's workers at the ILO and whether Japan would accept the labor standards being proposed by Western nations captured worldwide attention. It changed ideas in the East and the West about what Japan's workers deserved and desired and had lasting consequences for global politics and social policy.

Abstract This article situates Angela Carter’s Japanese writings in the context of multiple interconnected discourses in the 1960s and 1970s: on the one hand, anti-imperialism, postcolonialism, and feminism among European intellectuals and, on the other hand, “self-orientalism,” anti-Western nationalism, and global capitalism in postwar Japan. As a white woman in Japan, Carter was not only, as previously argued by scholars, doubly objectified as woman and foreigner, but “subjectified” in a patriarchal/colonialist discourse and made aware of her complicity in objectifying, emasculating, and exploiting the native. Carter’s Japanese and post-Japan writings reflect a newly acquired awareness of gender in a colonial context, demonstrated by her conscious use of male pronouns and gender transitions, effectively problematizing the othering tendency of European orientalists and the Japanese self-orientalist exploitation of it.

This article offers the reader methodological insights emerging from a hermeneutic phenomenological study that examined the meaning of the woman–midwife relationship in Japan. The methodology of hermeneutic phenomenology was chosen because it is well suited to reveal women’s and midwives’ lived experience that is often taken for granted in day-to-day maternity care settings. However, implementing the methodology was not without its challenges. These challenges included whether hermeneutic phenomenology, based on Western philosophy, could be appropriate for conducting a study involving a researcher and participants who identify as Japanese. Further, while the study required final write up in English, the interviews were conducted in Japanese. Utilizing hermeneutic phenomenology relies on language as the tool for accessing the phenomenon of enquiry. However, Japanese culture is less expressive and, relative to Western cultures, values non-verbal communication. Beyond verbal expression, language also conveys unique influences of each culture. Although it may be challenging to conduct research between different cultures, and their unique ways of thinking and languages, it is not an impossible situation and can be rewarding. The value of using hermeneutic phenomenology for a Japanese centered study helped to convey the meaning of the woman–midwife relationship in Japan. This article details the unique process of the study, in terms of the philosophical foundation and languages, to provide methodological insights and advances for future cross-cultural qualitative research.

Double-suicide cases have long been common in Japan, particularly among lovers. Classical studies conducted in the 1950s are well known. According to a report by Ohara, the double-suicide rate in Japan was recorded as 3.1% in 1954. Nevertheless, recent tendencies have not been reported. To assess the latest trends of double suicide, extensive studies were conducted in a populous area of Kanagawa, Japan, during 1999–2011. Suicides during the period in the domestic area claimed 23,195 victims. In all, 82 cases of double suicide were extracted, with 170 victims, meaning that double suicides occurred with incidence of 0.73% among all suicides. The mean age and standard deviation were 51.6±16.1 years, with 83 men and 87 women. The suicide-partner relationships included 40 cases (48.8%) of married couples, 13 (15.9%) of unmarried lovers, and 10 (12.2%) of elderly woman and her son/daughter dyads. No significant difference was found from Western countries in the incidence of double-suicide cases in Japan, except for a higher incidence of suicide involving elderly people and their sons/daughters. The traditional style of double suicide by lovers has become less frequent in modern Japanese society, presumably because of changing marriage styles and values.

Insulin autoimmune syndrome (IAS) is a rare syndrome characterized by fasting or postprandial hypoglycemia, high levels of anti-insulin antibodies and high concentration of total serum immunoreactive insulin. It is relatively known in Japan, rare in remaining Asia and it is extremely uncommon in Western countries, being characterized by a different race-related incidence and associated with HLADR4 alleles. Usually IAS is related to particular drugs, or to autoimmune, rheumatologic or hematological diseases, while it is very rare as a primary form. Here we described a case of an Italian woman affected by a primary form of Hirata syndrome.

Severe fever with thrombocytopenia syndrome (SFTS) is an emerging infectious and hemorrhagic disease recently described in China and western Japan. A 71-year-old healthy Japanese woman noticed a tick biting her after harvesting in an orchard and removed it herself. She developed diarrhea, anorexia, and chills eight days later. Because these symptoms continued, she visited a primary care physician 6 days after the onset. Laboratory data revealed thrombocytopenia, leukocytopenia, and elevated liver enzymes. She was then referred to our hospital. Although not completely fulfilling the diagnostic criteria used in a retrospective study in Japan, SFTS was suspected, and we detected SFTS virus in the patient’s blood using RT-PCR. However, she recovered without intensive treatment and severe complications 13 days after the onset. In this report, we present a mild clinical course of SFTS virus infection in Japan in detail.

This study aimed to examine area differences in the prevalence of and factors associated with frailty. This cross-sectional study included metropolitan (eastern and western areas), suburban (districts A and B), and rural areas of Japan (n = 9182, woman 50.9%). Frailty was defined by using a standardized questionnaire comprising three subcategories (fall, nutritional status, and social activities). The prevalence of frailty in the five areas was 14.2% to 30.6% for men and 11.5% to 21.4% for women. The areas with a high frailty prevalence had a significantly lower nutritional status or social activity, or both. Compared to the western metropolitan area, among men, the multivariable-adjusted prevalence ratio (APR) of frailty was significantly higher in the eastern metropolitan area and lower in suburban district A, and among women, the eastern metropolitan and rural areas had significantly higher APRs. Area-stratified multiple Poisson regression analysis showed that age, bone and joint disease, and a subjective economic status were associated with frailty in most areas and that some factors were area-specific, i.e., living alone (for men living in metropolitan areas) and underweight (for women living in suburban areas). The frailty prevalence differed by area, even after multivariable adjustment. Area-specific characteristics and factors associated with frailty may result in area differences.

Alma Karlin (1889–1950), a round-the-world traveller, intellectual, and writer from Celje, Slovenia, arrived in Japan and lived in Tokyo in the early 1920s, an era which historians consider to be an interim period between the initial expansion of the Japanese Empire to mainland Asia and its end in 1945. The writer’s fascination with the land can be inferred, among other things, from a 35-page description of Japan and the Japanese in her most famous book, Einsame Weltreise. Die Tragödie einer Frau (The Odyssey of a Lonely Woman), and passages in Reiseskizzen (Travel Sketches), an earlier work. The article aims to place these travel accounts in the historical and ideological contexts of their time while highlighting some similarities and differences between the representations of the land and its people by Karlin and those by Isabella Bird (1831–1904). Although Karlin makes no explicit reference to the famous British traveller in her writing on Japan, the article demonstrates that she must have known about Bird’s book Unbeaten Tracks in Japan. It is, above all, her decision to introduce her (German) readers to topoi that were typical of Victorian women’s travel writing which suggests that Karlin partly based her image of Japan, if not even the itinerary of her journey there, on Bird’s bestselling work. Nevertheless, Karlin does not seem to have conformed to the then dominant orientalist discourses on Japan, her representations generally showing none of the Western arrogance that was so typical of her fellow travellers of both sexes.

La présente étude se veut une analyse du voyage féminin français au Japon après la fin du Sakoku et jusqu’au lendemain de la Seconde Guerre mondiale. La présence des femmes françaises dans l’histoire du voyage et les écrits de voyage restent très peu analysés et tendent à être éclipsés par des pratiques de voyage et d’écriture viatique qui se déclinent principalement au masculin et qui dominent l’historiographie française. Les Françaises bien qu’identifiées pour certaines dans les études anglophones et japonaises sur le voyage occidental féminin au Japon, restent assez peu étudiées, faute de données suffisantes. Qui étaient les femmes françaises qui ont voyagé et vécu au Japon ? Pourquoi et comment ont-elles entrepris ce voyage ? Quelles traces écrites de leur voyage et de leur présence ont-elles laissé ? Mobilisant des investigations sur la présence féminine française au Japon à la fois dans les archives françaises et japonaises mais aussi une recherche des documents produits par les voyageuses identifiées dans différents fonds documentaires, il a été possible de dresser une typologie des Françaises au Japon mais aussi de leurs écrits (publiés et manuscrits) produits entre 1859 et 1949 présentés dans un catalogue raisonné. L’élaboration de ce panorama inédit a été complété par des entretiens avec des descendants et a aussi mobilisé de très nombreuses sources en main privée afin de comprendre les fonctionnements et les mécanismes de l’exotisme et l’imaginaire japonais en français produits par ces voyageuses. Cinq sources remarquables ont été traitées dans la seconde partie de l’étude, prolongeant les analyses des topoi de l’imaginaire japonais et questionnant à la fois l’expérience du voyage et les modalités d’écriture de celui-ci pour différentes périodes. Puisque la pratique du voyage au féminin implique des particularités, les discours des voyageuses en ont été impacté. Ces déclinaisons et variations d’images mentales associées au Japon, soulignées à la fois par le catalogue raisonné et l’étude d’un corpus spécifique, ont mis en exergue une écriture qui ne venaient pas s’opposer à un discours dominant et masculin, mais au contraire enrichir l’exotisme japonais en français
This study proposes an analysis of French women’s travels to Japan from the end of the Sakoku to the period just after the WWII. French women’s presence in History of travel and travel writing has been quite undervalued. Those subjects tend to be silenced in French historiography by the fact that main resources are dominated by male travelers. Even English-language and Japanese studies about Western Women’s travels in Japan, may have somehow muted them. Despite being identified for some, they aren’t studied, mostly because an apparent lack of resources. Who were those French women travelling to Japan and for some even settling there? Why and how did they travel? Did they leave their mark by writing about their experience or their settlement?By bringing together investigations in French and Japanese archives about the travelers and their possible writings (published, unpublished and personal handwritten papers) but also interviews with women travelers’ descendants it was possible to elaborate an overview of French women travelling situation in Japan (19th and 20th century) and build a resources database for their travel writings between 1859 and 1949. Because travelling as a women had specificities, how women travelers did write about their experiences has been impacted. Results of crossing the resources database and a corpus of 5 documents showed how women’s travel writings were not opposing to males ones but completing each other by bringing different representations of Japanese exoticism and alterity

Western art songs written on Japanese poems, Tanka, appeared in the early twentieth century as a late manifestation of Japonisme, the Japanese influence on Western art and music. The songs discussed in this dissertation include Japanisches Regenlied (1909) by Joseph Marx, Three Japanese Lyrics (1912-13) by Igor Stravinsky, Petits Poi!mes Japonais (1919) by Francesco Santoliquido, and Romances on Texts by Japanese Poets (1928-32) by Dmitri Shostakovich. Japonisme emerged as a significant movement in late-nineteenth-century Western art when Japanese artworks were first exported to Europe. Under the influence of these works, Western painters soon adopted Japanese techniques especially from traditional wood-block prints (Ukiyo-e). The appreciation of Japanese art and culture eventually emerged in Western music as a part of Orientalism and exoticism, first in opera, then in Debussy's music, and lastly in art songs. The Japanese poems used in Western art songs examined here are most commonly referred to as Tanka (a short poem), a genre that flourished between the third and tenth centuries. Because of the unique characteristics of the Japanese language, translating Japanese poems into European languages requires a certain imagination. The purpose of this dissertation is to explore the relationship between the original Japanese poems and their translations into European languages, and to discuss their transformation. The introduction provides a brief overview of Japonisme in Western art in the late nineteenth century. Chapter One focuses on the basic elements of Japanese poetry in order to outline the characteristics unique to the Japanese language. Considering Japanese influence within the category of "Orientalism" and "Exoticism" in music, Chapter Two explores the evidence for Oriental and exotic influences on Western music. Chapter Three focuses more specifically on Japanese influences in Western music. A detailed study of poems and translations, and their relationship to music is the core focus of Chapter Four. Chapter Five concludes that Tanka vanished from Western art songs soon after the songs under consideration were composed.

The objectives of this study were (1) to examine the nature and extent of cultural diversity in Japan; (2) to ascertain the meaning of multicultural education in both the North American and Japanese contexts; and (3) to make judgements concerning the relevance of multicultural education to Japanese education.
It was determined that Japanese society is indeed a culturally diverse one, that the cultural minorities are relatively small in numbers and that the Japanese government has traditionally followed a policy of the cultural assimilation of minorities.
Using conceptual analysis to investigate the meanings of multicultural education, the study found that the concept as developed in North America includes such elements as intercultural education, multiethnic education, minority education, human rights education, anti-racist education, democratic education, political education, education for social justice and peace education. These supporting meanings were found to have both distinctiveness yet also overlapping value associations.
The study reached the conclusion that a qualified concept of multicultural education has relevance to Japanese society, but that the degree of relevance depends upon the extent to which the government follows policies that strengthen or moderate traditional cultural values, recognizes and supports the development of minority cultural communities and encourages openness in its immigration and refugee policies.

This study explores the acculturation experiences of long-term Western residents in Japan using a broad intercultural studies approach. First, the historical context of Westerners coming to Japan is discussed. Next, literature from the field of intercultural studies is considered. This literature is used to provide a framework for analyzing Western sojourners’ acculturation experiences in Japan, as well as for choosing the research methods for conducting a qualitative analysis. The research involved interviewing 12 expatriates from English-speaking countries who have been in Japan for at least 10 years. Their acculturation experiences were analyzed, with a particular focus on finding hidden barriers to acculturation. The study concludes that numerous barriers to acculturation do exist. Commonly observable barriers included lack of employment security for some sojourners, and a poor fit between an individual’s personality and the host culture. These issues are applicable to anyone adapting to life in a different culture. However, this research also exposed a number of hidden barriers arguably unique to the Western sojourner’s acculturation experience in Japan. These barriers include the challenge of developing satisfying relationships with Japanese, due primarily to different expectations for psychological closeness and self-disclosure in Japan and the West. Another is the social effect of Othering, the centuries-old process where Japanese society divides things into those which are Japanese and those which are not, and the consequent empathy deficit that Japanese experience toward outsiders. The thesis concludes by offering recommendations for how long-term Western residents in Japan can improve their adaptation and acculturation experience. These include the obvious advantages of learning the Japanese language, and the less obvious benefits of learning and practicing mindful intercultural communication.

As a long-term resident of Japan with production credits in Japanese cinema, I have for many years been interested in how to write stories set in Japan that will appeal to domestic and international audiences. The study examines the challenges I face as a Western screenwriter writing a screenplay for Japanese cinema, and how those challenges inform my creative practice, bringing into being a screenplay that is intended to enhance screenwriting craft in mainstream Japanese cinema and provide new knowledge to transnational cinema and screenwriting research. The critical commentary that accompanies the screenplay takes a dialogic approach in practice-led research to explore how various issues emerge for the Western screenwriter in Japanese cinema. These problems are examined with regard to relevant theory, and contextualised in considerations of various films in Japanese-language cinema written by non-Japanese screenwriters. One salient issue is the application of the Hollywood ‘universal’ model of screenwriting to stories about Japan. I also explore the role of agency in screenplay authorship, in particular with regard to notions of ‘Japaneseness.’ I suggest notions of ‘Japaneseness’ are a particular challenge for my creative practice, and examine them in the context of national-transnational tensions in cinema. I draw on theories of transnational cinema to argue that the screenplay written for this PhD, Welcome to Prime-time, is an ‘accented Japanese screenplay.’ I go on to outline how accented Japanese screenplays might be positioned in relation to Japanese national cinema and transnational cinema discourses. I then discuss ‘Japaneseness’ in terms of a related issue: Orientalism. I show how Orientalism remains a trenchant concern for non-Japanese screenwriters representing Japan. This leads to a discussion of how a process of reflective authenticity might equip such screenwriters to depict ‘the Other’ in ways that circumvent Orientalist tropes in order to synthesise both local and global concerns. The process of critical reflection is threaded throughout the PhD, and concludes with a consideration of the notion of ‘becoming Japanese’ as it is depicted in my screenplay, and in my own journey within practice-led research. I posit that this PhD adds to our understanding of transnational screenplays and the contexts transnational screenwriters work within. Furthermore, I suggest the screenplay exhibits a new approach to achieving an ‘authentic’ representation of Japan and the Japanese by Western screenwriters. Note: It is recommended that the reader start with Chapters 1-4 of the critical exegesis. Ideally, the screenplay should be read after Chapter 4 and before Chapter 5. This is indicated in the text.

AbstractEsophageal cancer is the sixth most frequent cause of cancer death worldwide and affects more than 450,000 people all over the world [1]. Most patients with esophageal cancer in Asian countries such as Japan and China have squamous cell carcinoma (SCC), while most of those in Western countries have adenocarcinoma [2, 3]. In particular, the incidence of esophageal adenocarcinoma in the USA and the UK is rapidly increasing, in which the age-adjusted incidence has risen by 39·6% for men and 37·5% for women every 5 years in the UK [3]. Despite improvements in surgical techniques and perioperative management [4, 5] and surgery combined with chemotherapy and/or radiotherapy [6, 7], the prognosis of esophageal cancer at advanced stage remains poor with 30–40% in a 5-year survival globally [8] and the 5-year survival rate for the patients receiving esophagectomy in Japan was 55.5% [9].

This paper is about the Tanizaki Jun’ichirō’s (1886–1965) experience of creating a 1920–1950s Japanese woman image inspired by the urban environment of Yokohama — the city as a mirror of the Western culture in Japan. The writer who had combined the images of European and Oriental women in order to obtain the architype of new Japanese woman in his early works no longer wanted to do this. He began to portray a new-age woman — his new ideal, which writer observed in Hollywood movies. Tanizaki Jun’ichirō anticipated the appearance of Modan gāru on the Japanese stage, women who became objects of public attention because they followed Western fashion trends and lifestyle. Precisely this kind of woman occupied Tanizaki’s mind for the first half of the 1920s. The novel Blue Flower (Aoi hana, 青い花, 1921) was the first Tanizaki Jun’ichirō’s work, in which we meet the modan garu, a new type of Japanese woman of the first half of the XX c. In this novel, he presents the West as a source of femininity, and western attributes now as being able to change both the exterior and interior of a human. Blue Flower is the result of Tanizaki Jun’ichirō’s research of these new women in his quest for their physical perfection.

The article analyses the concepts of a woman external beauty in the scientific literature. The research shows differences in the stereotypes of the external beauty of women who lived in rural and urban Lithuania in the beginning of the 20th century in Lithuanian culture: the ethnic stereotype of the external beauty of rural women was strongly influenced in cities by western fashion trends. Empirical research carried out of analysis of the women beauty standards, based on the 2000-2020 publications of the women journal "Moteris". In these publications, new standards of beauty were formed over the course of two decades: women are encouraged to create their own beauty, using various tools of the beauty industry, but at the same time, women are encouraged to create inner values, human relationships and career achievements. This paper is an essay in comparative educational history and its possible relevance to educational development today. It addresses the question of whether Japan and Korea’s history in using educational development to further national development can be useful as (partial) models for dealing with the educational challenges of today’s lower- and lower-middle income countries. The hypothesis of the paper is that there is much to learn from these countries, but that the lessons one could learn are not at all obvious or superficial, and are only partially about what was done (specific education policies) and are more importantly about how it was done (the high purpose and thoroughness of policy engagement). The paper first characterizes educational development, especially in terms of the intense emphasis on equality of high achievement in Korea and Japan, in quantitative terms, to demonstrate that these countries possess certain admirable characteristics. Caveats regarding learner stress and rote learning are dealt with by looking at the relevant statistics. A framework for assessing the quality of policy borrowing processes is built, based on the literature on this subject. The paper then analyzes the historical development of education as a means of resisting Western colonialist probes into Japan and Korea (end of the 19th C), but also Japan itself into Korea (first half of 20th C). How both countries borrowed from the West, but in a contested and very deep manner, and as part of a resistance to being colonized, is documented. The paper also shows that part of the healthy, contested borrowing was the involvement of teacher groups and civil society. The paper concludes by taking into consideration the fraught issue that potentiating the role of education in national development could be seen as tantamount to using education for nationalism. The paper links to the possibility that there may be a more inclusive and rights-oriented use of the concept of the nation to foster human well-being, and that education could play a role in such processes. Some practical suggestions for taking these ideas forward, or at least exploring them in more depth, are made at the very end.

While debates over China’s Belt and Road Initiative (BRI) dominate the discourse over global infrastructure development, countries sceptical of the purpose or potential of the BRI have launched multiple alternative initiatives. This analysis compares two case studies: the Asia-Africa Growth Corridor (AAGC) launched by Japan and India in 2017, in part building on Japan’s Quality Infrastructure concept, and the G7’s 2021 Build Back Better World (B3W) plan, which is effectively a follow-up on the Blue Dot Network announced by the United States, Japan, and Australia in 2019. The paper concludes that the set of high financial and project quality standards of these initiatives may lead to better overall return but also prohibitive initial costs, while admirable goals like gender equity or digitised governance may not always respond adequately to the infrastructure priorities of developing countries. Furthermore, while these initiatives rely heavily on mobilising private capital, the literature clearly shows that infrastructure projects, especially in developing regions, are typically rather unattractive for private investors. Nevertheless, with a staggering USD 15 trillion gap in projected needs and actual spending on global infrastructure by 2040, there is no reason for a zero-sum competition between Chinese and Western connectivity programmes. Thus, Hungary should remain open to all and not commit exclusively to any of these initiatives.

Situated at one of the far-end of the world, as it may be viewed from the Triad countries (Northern America, Western Europe and Japan) Chile performs really well in the world of globalization in sharp contrast with all its neighbors. At first glance this success story may appear as the first Chilean paradox: the highly centralized political and administrative system does not seem to have hampered the economic development although it is generally considered that decentralization is an advisable way to enhance economic performance. In that context one could expect Chilean cities to be success stories as well, which is not the case at least for the second largest city: Valparaiso. That is the second Chilean paradox. This paper tries to consider these two entwined paradoxes and to suggest that a better and more decentralized framework could contribute to produce stronger municipalities and stronger cities without hampering the Chilean macroeconomic success story.

The decade of the 1990s has witnessed a wave of regional integration initiatives in Latin America: more than 14 agreements -free trade areas or customs unions- since 1990 with a handful more in varying degrees of negotiation (see Table 1). However, this was not just a Latin American phenomenon, as regionalism has more than ever become a global trend (Mistry [1996]). Indeed, now Japan, South Korea and Hong Kong are the only World Trade Organization (WTO) members which are not signatories to at least one preferential trade agreement (WTO [1995]). Regional integration is not new to Latin America. Economic integration played an important role in the region¿s early Post-War economic history. The 1960s and 1970s saw a number of very ambitious initiatives inspired by the successful Western European experience (Ffrench-Davis, Muñoz and Palma [1994]). Indeed, at its peak in the late 1960s and early 1970s, the topic of integration was hard to avoid in the discussion of Latin American development. However, disillusionment with integration processes had clearly set in by the late 1970s and the discussion of regional integration was all but silenced by the external crisis of the early 1980s.

Despite the tremendous resources invested in training Traditional Birth Attendants (TBAs) over the past two decades, scientific evidence from around the world has shown that training TBAs has not reduced maternal mortality. Any improvement observed when TBA training programs have been introduced was because of the associated supervision and referral systems, and the quality of essential obstetric services available at first referral level. Conversely, evidence has shown reduced maternal and perinatal morbidity and mortality when women have a “Skilled Attendant” (a qualified health care provider who has midwifery or obstetric skills) present at every birth. Thus, national safe motherhood programs, including in Kenya, are now focusing on increasing the number of Skilled Attendants, whether a woman delivers in a facility or at home. Since TBAs are highly regarded by their communities, it is critical that they still be enabled to play a role in improving maternal health. As noted in this brief, the continued preference for TBAs in Western Province can be attributed to their proximity to the woman’s home, respectful attitude toward women, and flexible modes of payment. Problems can arise, however, when TBAs delay seeking skilled care for women in difficult labor.
