Body fluids and tissues for sampling include hair, blood, saliva, sweat, and urine; all yield information regarding drug use within unique limitations and at different levels of convenience. The test methodology imposes a need to know respective sensitivities and specificities, particularly in a forensic setting. Breath testing (alcohol) is not within the scope of this chapter. Recommended frequencies of urine testing are addressed. The utility of other forms of monitoring (self-report, collateral sources, PDMP) is assessed in the final section. Two tables are provided, (1) urine drug detection times, and (2) sources of false positive tests in urine immunoassay studies. Supplemental information sources are cited in a text box.

Outcome measures may be combined in a composite outcome. The value of urinalysis for diagnosis and management of renal and genitourinary disease is well accepted. Urinalysis aids in the diagnosis of renal disease especially in cases when a renal biopsy is not immediately available or is contraindicated. It is most informative when done by the treating physician with knowledge of the clinical context. Inspection is done by eye. Routine chemical analysis is done by dipstick but urine microscopy is essential for it may reveal abnormalities even when chemical evaluation is normal. Dysmorphic red cells, red cell casts, white blood cells, renal cells, and specific crystals may be diagnostically important. Urinalysis and microscopy can narrow the differential diagnosis faster than many more complex tests are able to.

The first section—Team Formation—provides detailed information on issues to consider in establishing the team in a manner that will be most beneficial to the conduct of qualitative research. The second section—Research Design and Project Organization—discusses early writing tasks, establishing a project management system, and the importance of linking all of this to a data archiving plan. Digital tools are discussed in some depth. The third section—Caring: Internalized and Externalized—suggests a novel approach to the issue of ethics and team management.

Ascertaining an individual’s history of alcohol consumption is an important component in the proper treatment of accidental trauma or acute or chronic illness, as well as for matters of public health and safety, legal issues, insurance coverage, and the management of and recovery from hazardous/harmful levels of alcohol consumption. Although self-report of alcohol consumption in both research and clinical settings represents the most common mode of assessment, there is long-standing interest in developing objective measures of alcohol consumption that do not rely on the ability or willingness of a person to truthfully report consumption. Biologic diagnostic tests or biomarkers can provide information on current and past quantity and frequency of alcohol consumption. This chapter discusses and evaluates many of the biomarker candidates that have been investigated and provides insights into future searches for optimal diagnostic tools to provide biologic evidence of duration, quantity, and frequency of individual alcohol consumption. We have included a limited discussion of biomarkers for assessing cannabis use since cannabis and alcohol use many times are a concomitant feature of intoxication.

When to convert to pharmacologic stress or add ancillary imaging, including myocardial perfusion imaging, echocardiography, coronary calcium scoring, and magnetic reference imaging are reviewed. The use of stress testing in the management of obstructive and non-obstructive coronary artery disease (CAD), heart failure, cardiac rehabilitation, peripheral vascular disease, congenital heart and other cardiovascular diseases (CVD) is examined. Options to optimize the diagnostic capabilities of exercise and other diagnostic testing for women are highlighted. Strategic use of exercise testing in the face of a decreasing burden of CAD in the developed world, as well as the opportunity to rely on exercise testing as the first test to evaluate CVD in the developing world, are reviewed.
