Summary Palmaris brevis spasm syndrome (PBSS) appears to reflect benign muscle hyperactivity, with unprovoked recurrent spasm of the muscle and consequent dimpling of the hypothenar eminence. Two cases are reported, being the first known report of the condition among manual workers or those using vibratory tools. Each was diagnosed during routine health surveillance for hand–arm vibration syndrome. With hand cramps being a frequent report among vibration-exposed workers, this condition should be considered in such cases, although the lack of reports means that the frequency of the condition is unknown. One of the employees described more frequent episodes of spasm during periods of increased manual work. Diagnosis is possible by witnessing the characteristic spasm, or by viewing photographs of it. Functional impairment is not usual, except for the brief duration of an attack. There is no consensus regarding appropriate treatment.

Abstract This paper explains the components of an integrated approach to oak wilt control which has strong potential for adaptation to red oak forests throughout the Lake States. Oak wilt epicenter containment involves (1) separating grafted root systems with a vibratory plow line or Vapam barrier, followed by (2) removal of all living red oaks inside the barrier, and sanitary treatment of wood capable of producing fungal mats. Use of a reference table developed for proper barrier location is described. Epicenters are established by insect vectors, often as a consequence of human activity. Establishment of epicenters can be prevented by: (1) sanitary treatment of diseased trees which may yet produce mats, (2) discouraging unsupervised movement of diseased wood, (3) minimizing the wounding of oaks during the spring and early summer, and (4) immediately painting all spring and summer wounds on oak trees with a tree wound dressing. Detection of new epicenters is important. Treatment of small epicenters minimizes site disturbance and the number of fungal mats produced, as well as the number of trees lost. Systematic aerial surveillance is useful for effective detection in large acre-ages of oak type. Prevention is the best control measure. Unfortunately, public relations are often inadequate. North. <p><strong>Abstract.</strong> This paper discusses the <i>Points Further North project</i>, a VR documentary that was undertaken with a view to foregrounding how sound can be deployed as the primary mechanism for laying out the complex, often subjugated relationships manifested between physical spaces and those who inhabit them. Specifically, It examines how ambisonic and haptic audio’s profoundly affective emotional, tactile and topologically enveloping capacities can be articulated within an acoustemological framework (acoustemology is best defined by ethnographer Steven Feld as “sonic ways of being in and knowing the world”) in order to evoke a heightened sense of awareness, perhaps even an agency, with respect to the largely abstracted ramifications arising from the consumerist lifestyles that are endemic to the developed world. The project exploits the possibilities inherent in the amplification of the vibratory and electromagnetic spectra that permeate our urban environments: infrasonic/tactile elements are disseminated via the Subpac wearable haptic interface in order to constitute a corporeal and emotional presence, and the radiant (yet invisible) transmissions of our information, economic and surveillance networks are captured and sonified via the via use of electromagnetic transducers. Both sonically and thematically, <i>Points Further North</i> seeks to uncover that which sound studies scholar Salomé Voegelin, terms “our locality on the invisible index of sound”, capitalizing upon sound’s capacity to delineate the ethereal topographies engendered via the vast, sublime – yet sublimated – infrastructures that we find ourselves immersed within.</p>

This paper addresses the use of several signal processing tools for monitoring and diagnosis of assembly faults in diesel engines through the cold test technology. One specific fault is considered here as an example: connecting rod with incorrectly tightened screws. First, the experimental apparatus concerning the vibration tests is introduced. Subsequently, the dynamic analysis of the engine has been carried out in order to calculate the connecting rod forces against the crankpin for predicting the position where mechanical impacts are expected. Then, a vibration signal model for this type of faults is introduced. It deals with the cyclostationary model in which the signal is subdivided into two main parts: deterministic and nondeterministic. Finally, the acceleration signals acquired from the engine block during a cold test cycle at the end of the assembly line are analyzed. For quality control purposes in order to obtain reliable thresholds for the pass/fail decision, a method based on the image correlation of symmetrized dot patterns is proposed. This method visualizes vibration signals in a diagrammatic representation in order to quickly detect the faulty engines in cold tests. Moreover, the fault identification is discussed on the basis of the cyclostationary model of the signals. The first-order cyclostationarity is exploited by the analysis of the time synchronous average (TSA). In addition, the residual signal is evaluated by subtracting the TSA from the raw synchronized signal, and thus, the second-order cyclostationarity analysis is developed by means of the Wigner–Ville distribution (WVD), Wigner–Ville spectrum (WVS), and mean instantaneous power. Moreover, continuous wavelet transform is presented and compared with the WVD and WVS.

BACKGROUND: Constant use of hand-held vibratory tools may cause health issues, including hand dysfunction, among farmers. Thus, exposure to these vibratory instruments may cause hand-arm vibration syndrome (HAVS). OBJECTIVE: The study aimed to measure the prevalence of HAVS and evaluate neurological and vascular symptoms among farmers. METHODS: This cross-sectional study included 20 villages in Pakistan and used a three-part questionnaire to collect data from farmers exposed to vibration while farming. The study included demographics, the Health Surveillance Questionnaire, and the Stockholm Workshop Classification Scale. SPSS version 22 was used for the data entry and statistical analysis. RESULTS: 433 (43%) participants had musculoskeletal discomfort and 568 (57%) had hand vibration. Raynaud phenomenon was evident in 430 (43%) participants, whereas carpal tunnel syndrome was found in 218 (22%). Tingling was reported by 255 participants (26%), and numbness was reported by 543 (54%). According to the grading of neurological symptoms, 461 (46%) participants were exposed to vibration but did not experience any symptoms, placing them in stage 0. 185 (18.5%) participants with HAVS with infrequent episodes affecting just the tips of one or more fingers were categorized in stage 01 based on grading of vascular symptoms. CONCLUSION: HAVS are common among harvesting farmers, with the most noticeable vibrations occurring in the shoulder region. Similarly, most farmers had no vascular symptoms, and most were exposed to vibration but had no neurological symptoms.

Objective This state-of-the-art article reviews the epidemiology, diagnosis, and management of vocal fold leukoplakia, with focus on recent advances. It focuses on the clinical challenges that otolaryngologists face balancing both oncological efficacy and functional outcomes in leukoplakia and presents the current philosophies and techniques to consider when managing such patients. Data Sources PubMed/MEDLINE. Review Methods We conducted a detailed review of publications related to vocal cord and laryngeal leukoplakia, dysplasia, hyperkeratosis, leukoplakia endoscopy, and leukoplakia management focusing specifically on oncologic outcomes, voice preservation, current and emerging diagnosis, and management techniques. Conclusions There has been a paradigm shift away from performing “vocal cord stripping” procedures that can cause irreversible hoarseness toward voice preservation surgery while achieving comparable oncologic control. Surgical technical and instrumental developments have been designed to maximally treat superficial disease while preserving underling vibratory mucosa. Recent improvements in histopathological grading systems and advances in biomarker classification may allow for improved oncologic risk stratification. Furthermore, improvements in endoscopic imaging capabilities and contact endoscopy are currently being studied for their potential diagnostic significance. Implications for Practice To optimally manage vocal fold leukoplakia, the otolaryngologist should become familiar with the oncologic implications of the disease and the importance of obtaining pathologic diagnosis to rule out malignancy. In addition, the surgeon should maintain surgical techniques and knowledge of available instruments and lasers that can assist in surgical management while prioritizing the preservation of vibratory tissue and voice quality. Finally, the surgeon and the patient should understand the clinical importance of routine endoscopic surveillance.
